Abstract:Epilepsy is one of the most common neurological disorders and is characterized by recurrent seizures. We use theoretical neuroscience tools to study brain dynamics during seizures. We derive and simulate a computational model of a network of hippocampal neuronal populations. Each population within the network is based on a model that has been shown to replicate the electrophysiological dynamics observed during seizures. The results provide insights into possible mechanisms for seizure spread. We observe that epileptiform activity remains localized to a pathological region when a global connectivity parameter is less than a critical value. After establishing the critical value for seizure spread, we explored how to correct the effect by altering particular synaptic gains. The spreading of seizures is quantified using numerical methods for seizure detection. The results from this study provide a new avenue of exploration for seizure control.

Individual structural connectivity defines propagation networks in partial epilepsy

Abstract:Neural network oscillations are a fundamental mechanism for cognition, perception and consciousness. Consequently, perturbations of network activity play an important role in the pathophysiology of brain disorders. When structural information from non-invasive brain imaging is merged with mathematical modeling, then generative brain network models constitute in-silico platforms for the exploration of causal mechanisms of brain function and clinical hypothesis testing. We here demonstrate along the example of drug-resistant epilepsy that patient-specific virtual brain models derived from diffusion MRI have sufficient predictive power to improve diagnosis and surgery outcome. In partial epilepsy, seizures originate in a local network, the so-called epileptogenic zone, before recruiting other close or distant brain regions. We create personalized large-scale brain networks for 15 patients and simulate the individual seizure propagation patterns. Model validation is performed against the presurgical stereotactic electroencephalography (SEEG) data and the standard-of-care clinical evaluation. We demonstrate that the individual brain models account for the temporal variability in patient seizure propagation patterns and explain the variability in postsurgical success. Our results show that individual variations in structural connectivity, when linked with mathematical dynamic models, have the capacity to explain changes in spatiotemporal organization of brain dynamics as observed in network-based brain disorders, thus opening up avenues towards discovery of novel clinical interventions.
Controlling seizure propagation in large-scale brain networks

Abstract:Information transmission in the human brain is a fundamentally dynamic network process. In partial epilepsy, this process is perturbed and highly synchronous seizures originate in a local network, the so-called epileptogenic zone (EZ), before recruiting other close or distant brain regions. We studied patient-specific brain network models of 15 drug-resistant epilepsy patients with implanted stereotactic electroencephalography (SEEG) electrodes. Each personalized brain model was derived from structural data of magnetic resonance imaging (MRI) and diffusion tensor weighted imaging (DTI), comprising 88 nodes equipped with region specific neural mass models capable of demonstrating a range of epileptiform discharges. Each patients virtual brain was further personalized through the integration of the clinically hypothesized EZ. Subsequent simulations and connectivity modulations were performed and uncovered a finite repertoire of seizure propagation patterns. Across patients, we found that (i) patient-specific network connectivity is predictive for the subsequent seizure propagation pattern; (ii)seizure propagation is characterized by a systematic sequence of brain states; (iii) propagation can be controlled by an optimal intervention on the connectivity matrix; (iv) the degree of invasiveness can be significantly reduced via the here proposed seizure control as compared to traditional resective surgery. To stop seizures, neurosurgeons typically resect the EZ completely. We showed that stability analysis of the network dynamics using graph theoretical metrics estimates reliably the spatiotemporal properties of seizure propagation. This suggests novel less invasive paradigms of surgical interventions to treat and manage partial epilepsy.

Neural mass modeling of slow-fast dynamics of seizure initiation and abortion

Abstract:Epilepsy is a dynamic and complex neurological disease affecting about 1% of the worldwide population, among which 30% of the patients are drug-resistant. Epilepsy is characterized by recurrent episodes of paroxysmal neural discharges (the so-called seizures), which manifest themselves through a large-amplitude rhythmic activity observed in depth-EEG recordings, in particular in local field potentials (LFPs). The signature characterizing the transition to seizures involves complex oscillatory patterns, which could serve as a marker to prevent seizure initiation by triggering appropriate therapeutic neurostimulation methods. To investigate such protocols, neurophysiological lumped-parameter models at the mesoscopic scale, namely neural mass models, are powerful tools that not only mimic the LFP signals but also give insights on the neural mechanisms related to different stages of seizures. Here, we analyze the multiple time-scale dynamics of a neural mass model and explain the underlying structure of the complex oscillations observed before seizure initiation. We investigate population-specific effects of the stimulation and the dependence of stimulation parameters on synaptic timescales. In particular, we show that intermediate stimulation frequencies (>20 Hz) can abort seizures if the timescale difference is pronounced. Those results have the potential in the design of therapeutic brain stimulation protocols based on the neurophysiological properties of tissue.

Neural Dynamics and Seizure Correlations: Insights from Neural Mass Models in a Tetanus Toxin Rat Model of Epilepsy

Abstract:This study focuses on the use of a neural mass model to investigate potential relationships between functional connectivity and seizure frequency in epilepsy. We fitted a three- layer neural mass model of a cortical column to intracranial EEG (iEEG) data from a Tetanus Toxin rat model of epilepsy, which also included responses to periodic electrical stimulation. Our results show that some of the connectivity weights between different neural populations correlate significantly with the number of seizures each day, offering valuable insights into the dynamics of neural circuits during epileptogenesis. We also simulated single-pulse electrical stimulation of the neuronal populations to observe their responses after the connectivity weights were optimized to fit background (non-seizure) EEG data. The recovery time, defined as the time from stimulation until the membrane potential returns to baseline, was measured as a representation of the critical slowing down phenomenon observed in nonlinear systems operating near a bifurcation boundary. The results revealed that recovery times in the responses of the computational model fitted to the EEG data were longer during 5 min periods preceding seizures compared to 1 hr before seizures in four out of six rats. Analysis of the iEEG recorded in response to electrical stimulation revealed results similar to the computational model in four out of six rats. This study supports the potential use of this computational model as a model-based biomarker for seizure prediction when direct electrical stimulation to the brain is not feasible.

Abstract:The aim of this article is to infer the connectivity structures of brain regions before and during epileptic seizure. Our contributions are fourfold. First, we propose a 6N-dimensional stochastic differential equation for modelling the activity of N coupled populations of neurons in the brain. This model further develops the (single population) stochastic Jansen and Rit neural mass model, which describes human electroencephalography (EEG) rhythms, in particular signals with epileptic activity. Second, we construct a reliable and efficient numerical scheme for the model simulation, extending a splitting procedure proposed for one neural population. Third, we propose an adapted Sequential Monte Carlo Approximate Bayesian Computation algorithm for simulation-based inference of both the relevant real-valued model parameters as well as the {0,1}-valued network parameters, the latter describing the coupling directions among the N modelled neural populations. Fourth, after illustrating and validating the proposed statistical approach on different types of simulated data, we apply it to a set of multi-channel EEG data recorded before and during an epileptic seizure. The real data experiments suggest, for example, a larger activation in each neural population and a stronger connectivity on the left brain hemisphere during seizure.

Abstract:Epilepsy is a disorder characterised by spontaneous, recurrent seizures. Both local and network abnormalities have been associated with epilepsy, and the exact processes generating seizures are thought to be heterogeneous and patient-specific. Due to the heterogeneity, treatments such as surgery and medication are not always effective in achieving full seizure control and choosing the best treatment for the individual patient can be challenging. Predictive models constrained by the patient's own data therefore offer the potential to assist in clinical decision making. In this chapter, we describe how personalised patient-derived networks from structural or functional connectivity can be incorporated into predictive models. We focus specifically on dynamical systems models which are composed of differential equations capable of simulating brain activity over time. Here we review recent studies which have used these models, constrained by patient data, to make personalised patient-specific predictions about seizure features (such as propagation patterns) or treatment outcomes (such as the success of surgical resection). Finally, we suggest future research directions for patient-specific network models in epilepsy, including their application to integrate information from multiple modalities, to predict long-term disease evolution, and to account for within-subject variability for treatment.

Abstract:Neuroplasticity refers to functional and structural changes in brain regions in response to healthy and pathological activity. Activity dependent plasticity induced by epileptic activity can involve healthy brain regions into the epileptogenic network by perturbing their excitation/inhibition balance. In this article, we present a new neural mass model, which accounts for neuroplasticity, for investigating the possible mechanisms underlying the epileptogenic network expansion. Our multiple-timescale model is inspired by physiological calcium-mediated synaptic plasticity and pathological extrasynaptic N-methyl-D-aspartate (NMDA) dependent plasticity dynamics. The model highlights that synaptic plasticity at excitatory connections and structural changes in the inhibitory system can transform a healthy region into a secondary epileptic focus under recurrent seizures and interictal activity occurring in the primary focus. Our results suggest that the latent period of this transformation can provide a window of opportunity to prevent the expansion of epileptogenic networks, formation of an epileptic focus, or other comorbidities associated with epileptic activity. Repetitive epileptic activity leads to a large variety of plastic changes in brain. Understanding the mechanism and its time course can indicate windows of opportunity for preventing the devastating impact of focal epileptic activity on brain networks. Animal models of focal epilepsy have underlined the role of NMDA receptors and deregulations in the GABAergic system in the expansion of epileptic activity. Here we develop a plastic neural mass model integrating physiological and pathologic plasticity. Our results show that neuroplasticity mediated by epileptic activity increases synaptic efficacy between epileptic and healthy regions, and causes irreversible changes in the GABAergic system in the healthy region. The model gives mechanistic insights into the physio-pathological impact of focal epileptic activity on large-scale networks and suggests that seizures may beget seizures.

Abstract:Background: Epilepsy was defined as an abnormal brain network model disease in the latest definition. From a microscopic perspective, it is also particularly important to observe the Mutual Information (MI) of the whole brain network based on different lead positions. Methods: In this study, we selected EEG data from representative temporal lobe and frontal lobe epilepsy patients. Based on Phase Space Reconstruction and the calculation of MI indicator, we used Complex Network technology to construct a dynamic brain network function model of epilepsy seizure. At the same time, about the analysis of our network, we described the index changes and propagation paths of epilepsy discharge in different periods, and spatially monitors the seizure change process based on the analysis of the parameter characteristics of the complex network. Results: Our model portrayed the functional synergy between the various regions of the brain and the state transition during the seizure process. We also characterized the EEG synchronous propagation path and core nodes during seizures. The results shown the full node change path and the distribution of important indicators during the seizure process, which makes the state change of the seizure process more clearly. Conclusion: In this study, we have demonstrated that synchronization-based brain networks change with time and space. The EEG synchronous propagation path and core nodes during epileptic seizures can provide a reference for finding the focus area.

Abstract:Epilepsy is a common neurological disorder, affecting over 65 million people worldwide. Unfortunately, despite resective surgery, over 30 % of patients with drug-resistant epilepsy continue to experience seizures. Retrospective studies considering connectivity using intracranial electrocorticography (ECoG) obtained during neuromonitoring have shown that treatment failure is likely driven by failure to consider critical components of the seizure network, an idea first formally introduced in 2002. However, current studies only capture snapshots in time, precluding the ability to consider seizure network development. Over the past few years, multiwell microelectrode arrays have been increasingly used to study neuronal networks in vitro. As such, we sought to develop a novel in vitro MEA seizure model to allow for study of seizure networks. Specifically, we used 4-aminopyridine (4-AP) to capture hyperexcitable activity, and then show increased network changes after 2 days of chronic treatment. We characterize network changes using functional connectivity measures and a novel technique using dimensionality reduction. We find that 4-AP successfully captures persistently elevated mean firing rate and significant changes in underlying connectivity patterns. We believe this affords a robust in vitro seizure model from which longitudinal network changes can be studied, laying groundwork for future studies exploring seizure network development.

Abstract:The variability in the propagation pathway in epilepsy is a main factor contributing to surgical treatment failure. Ways to accurately capture the brain propagation network and quantitatively assess its evolution remain poorly described. This work aims to develop a dynamic step effective network (dSTE) to obtain the propagation path network of multiple seizures in the same patient and explore the degree of dissimilarity. Multichannel stereo-electroencephalography (sEEG) signals were acquired with ictal processes involving continuous changes in information propagation. We utilized high-order dynamic brain networks to obtain propagation networks through different levels of linking steps. We proposed a dissimilarity index based on singular value decomposition to quantitatively compare seizure pathways. Simulated data were generated through The Virtual Brain, and the reliability of this method was verified through ablation experiments. By applying the proposed method to two datasets consisting of 29 patients total, the evolution processes of each patient's seizure networks was obtained, and the within-patient dissimilarities were quantitatively compared. Finally, three types of brain network connectivity patterns were found. Type I patients have a good prognosis, while type III patients are prone to postoperative recurrence. This method captures the evolution of seizure propagation networks and assesses their dissimilarity more reliably than existing methods, demonstrating good robustness for studying the propagation path differences for multiple seizures in epilepsy patients. The three different patterns will be important considerations when planning epilepsy surgery under sEEG guidance.

Abstract:INTRODUCTION: Epilepsy is increasingly conceptualized as a network disorder, and advancing methods for its diagnosis and treatment requires characterizing both the epileptic generator and related networks. We combined multimodal magnetic resonance imaging (MRI) and high-density electroencephalography (HD-EEG) to interrogate alterations in cortical microstructure, morphology, and local function within and beyond spiking tissue in focal epilepsy. METHODS: We studied 25 patients with focal epilepsy (12F, mean ± SD age = 31.28 ± 9.30 years) and 55 age- and sex-matched healthy controls, subdivided into a group of 30 for imaging feature normalization (15F, 31.40 ± 8.74 years) and a group of 25 for replication (12F, 31.04 ± 5.65 years). The 3T MRI acquisition included T1-weighted, diffusion, quantitative T1 relaxometry, and resting-state functional imaging. Open-access MRI processing tools derived cortex-wide maps of morphology and microstructure (cortical thickness, mean diffusivity, and quantitative T1 relaxometry) and intrinsic local function and connectivity (timescales, connectivity distance, and node strength) for all participants. Multivariate approaches generated structural and functional alteration scores for each cortical location. Using HD-EEG, the predominant spike type was localized and we quantified MRI alterations within spike sources, as well as in proximal and connected networks. RESULTS: Regions harboring spike sources showed increased structural MRI alterations (mean: 27.98%) compared to the rest of the brain (mean: 17.67%) in patients. Structural compromise extended to all regions with close functional coupling to spike sources (paired t-tests; FDR-corrected p < 0.05), but not to anatomical neighbors of spike sources. This finding was replicated using average control functional, structural, and anatomical matrices instead of patient-specific matrices. CONCLUSION. Spiking regions contain more marked alterations in microstructure and morphology than the remaining cortex, which may help localize the epileptogenic zone non-invasively. There are nevertheless broader networks effects, which may relate to a cascading of structural changes to functionally connected cortices. These results underscore the utility of combining high-definition MRI and EEG approaches for characterizing epileptogenic tissue and assessing distributed network effects.

Abstract:We have investigated the theoretical constraints of the interactions between coupled cortical columns. Each cortical column consists of a set of neural populations where each population is modelled as a neural mass. The existence of semi-stable states within a cortical column is dependent on the type of interaction between the neuronal populations, i.e., the form of the synaptic kernels. Current-to-current coupling has been shown, in contrast to potential-to-current coupling, to create semi-stable states within a cortical column. The interaction between semi-stable states of the cortical columns is studied where we derive the dynamics for the collected activity. For small excitations the dynamics follow the Kuramoto model; however, in contrast to previous work we derive coupled equations between phase and amplitude dynamics with the possibility of defining connectivity as a stationary and dynamic variable. The turbulent flow of phase dynamics which occurs in networks of Kuramoto oscillators would indicate turbulent changes in dynamic connectivity for coupled cortical columns which is something that has been recorded in epileptic seizures. We used the results we derived to estimate a seizure propagation model which allowed for inversions using the Laplace assumption (Dynamic Causal Modelling). The seizure propagation model was trialed on simulated data, and future work will investigate the estimation of the connectivity matrix from empirical data. This model can be used to predict changes in seizure evolution after virtual changes in the connectivity network, something that could be of clinical use when applied to epilepsy surgical cases.
Multiscale neuro-inspired models for interpretation of EEG signals in patients with epilepsy

Abstract:Objective The aim is to gain insight into the pathophysiological mechanisms underlying interictal epileptiform discharges observed in electroencephalographic (EEG) and stereo-EEG (SEEG, depth electrodes) recordings performed during pre-surgical evaluation of patients with drug-resistant epilepsy. Methods We developed novel neuro-inspired computational models of the human cerebral cortex at three different levels of description: i) microscale (detailed neuron models), ii) mesoscale (neuronal mass models) and iii) macroscale (whole brain models). Although conceptually different, micro- and mesoscale models share some similar features, such as the typology of neurons (pyramidal cells and three types of interneurons), their spatial arrangement in cortical layers, and their synaptic connectivity (excitatory and inhibitory). The whole brain model consists of a large-scale network of interconnected neuronal masses, with connectivity based on the human connectome. Results For these three levels of description, the fine-tuning of free parameters and the quantitative comparison with real data allowed us to reproduce interictal epileptiform discharges with a high degree of fidelity and to formulate hypotheses about the cell- and network-related mechanisms underlying the generation of fast ripples and SEEG-recorded epileptic spikes and spike-waves. Conclusions The proposed models provide valuable insights into the pathophysiological mechanisms underlying the generation of epileptic events. The knowledge gained from these models effectively complements the clinical analysis of SEEG data collected during the evaluation of patients with epilepsy. Significance These models are likely to play a key role in the mechanistic interpretation of epileptiform activity.

Simulation-Based Inference for Whole-Brain Network Modeling of Epilepsy using Deep Neural Density Estimators

Abstract:Whole-brain network modeling of epilepsy is a data-driven approach that combines personalized anatomical information with dynamical models of abnormal brain activity to generate spatio-temporal seizure patterns as observed in brain imaging signals. Such a parametric simulator is equipped with a stochastic generative process, which itself provides the basis for inference and prediction of the local and global brain dynamics affected by disorders. However, the calculation of likelihood function at whole-brain scale is often intractable. Thus, likelihood-free inference algorithms are required to efficiently estimate the parameters pertaining to the hypothetical areas in the brain, ideally including the uncertainty. In this detailed study, we present simulation-based inference for the virtual epileptic patient (SBI-VEP) model, which only requires forward simulations, enabling us to amortize posterior inference on parameters from low-dimensional data features representing whole-brain epileptic patterns. We use state-of-the-art deep learning algorithms for conditional density estimation to retrieve the statistical relationships between parameters and observations through a sequence of invertible transformations. This approach enables us to readily predict seizure dynamics from new input data. We show that the SBI-VEP is able to accurately estimate the posterior distribution of parameters linked to the extent of the epileptogenic and propagation zones in the brain from the sparse observations of intracranial EEG signals. The presented Bayesian methodology can deal with non-linear latent dynamics and parameter degeneracy, paving the way for reliable prediction of neurological disorders from neuroimaging modalities, which can be crucial for planning intervention strategies.

Abstract:Mean-field models have been developed to replicate key features of epileptic seizure dynamics. However, the precise mechanisms and the role of the brain area responsible for seizure onset and propagation remain incompletely understood. In this study, we employ computational methods within The Virtual Brain framework and the Epileptor model to explore how the location and connectivity of an Epileptogenic Zone (EZ) in a mouse brain are related to focal seizures (seizures that start in one brain area and may or may not remain localized), with a specific focus on the hippocampal region known for its association with epileptic seizures. We then devise computational strategies to confine seizures (prevent widespread propagation), simulating medical-like treatments such as tissue resection and the application of an anti-seizure drugs or neurostimulation to suppress hyperexcitability. Through selectively removing (blocking) specific connections informed by the structural connectome and graph network measurements or by locally reducing outgoing connection weights of EZ areas, we demonstrate that seizures can be kept constrained around the EZ region. Abstract:Epilepsy is a complex disease spanning across multiple scales, from ion channels in neurons to neuronal circuits across the entire brain. Over the past decades, computational models have been used to describe the pathophysiological activity of the epileptic brain from different aspects. Traditionally, each computational model can aid in optimizing therapeutic interventions, therefore, providing a particular view to design strategies for treating epilepsy. As a result, most studies are concerned with generating specific models of the epileptic brain that can help us understand the certain machinery of the pathological state. Those specific models vary in complexity and biological accuracy, with system-level models often lacking biological details. Here, we review various types of computational models of epilepsy and discuss their potential for different therapeutic approaches and scenarios, including drug discovery, surgical strategies, brain stimulation, and seizure prediction. We propose that we need to consider an integrated approach with a unified modelling framework across multiple scales to understand the epileptic brain. Our proposal is based on the recent increase in computational power, which has opened up the possibility of unifying those specific epileptic models into simulations with an unprecedented level of detail. A multi-scale epilepsy model can bridge the gap between biologically detailed models, used to address molecular and cellular questions, and brain-wide models based on abstract models which can account for complex neurological and behavioral observations. With these efforts, we move toward the next generation of epileptic brain models capable of connecting cellular features, such as ion channel properties, with standard clinical measures such as seizure severity.

Abstract:Epilepsy affects millions globally, with a significant subset of patients suffering from drug-resistant focal seizures. Understanding the underlying neurodynamics of seizure initiation and propagation is crucial for advancing treatment and diagnostics. In this study, we present a novel, inference-based approach for analyzing the temporal evolution of cortical stability and chaos during focal epileptic seizures. Utilizing a multi-region neural mass model, we estimate time-varying synaptic connectivity from intracranial electroencephalography (iEEG) data collected from individuals with drug-resistant focal epilepsy. Our analysis reveals distinct preictal and ictal phases characterized by shifts in cortical stability, heightened chaos in the ictal phase, and highlight the critical role of inter-regional communication in driving chaotic cortical behaviour. We demonstrate that cortical dynamics are consistently destabilized prior to seizure onset, with a transient reduction in instability at seizure onset, followed by a significant increase throughout the seizure. This work provides new insights into the mechanisms of seizure generation and offers potential biomarkers for predicting seizure events. Our findings pave the way for innovative therapeutic strategies targeting cortical stability and chaos to manage epilepsy.

Abstract:Understanding the pathophysiological dynamics which underline interictal epileptiform events (IEEs) such as epileptic spikes, spike-and-waves or High-frequency oscillations (HFOs) is of major importance in the context of neocortical refractory epilepsy, as it paves the way for the development of novel therapies. Typically, these events are detected in local field potential (LFP) recordings obtained through depth electrodes during pre-surgical investigations. Although essential, the underlying pathophysiological mechanisms for the generation of these epileptic neuromarkers remain unclear. The aim of this paper is to propose a novel neurophysiologically relevant reconstruction of the neocortical microcircuitry in the context of epilepsy. This reconstruction intends to facilitate the analysis of a comprehensive set of parameters encompassing physiological, morphological, and biophysical aspects that directly impact the generation and recording of different IEEs. Accordingly, a novel microscale computational model of an epileptic neocortical column was introduced. This model incorporates the intricate multilayered structure of the cortex and allows for the simulation of realistic interictal epileptic signals. The proposed model was validated through comparisons with real IEEs recorded using intracranial stereo-electroencephalography (SEEG) signals from both humans and animals. Using the model, the user can recreate epileptiform patterns observed in different species (human, rodent, and mouse) and study the intracellular activity associated with these patterns. Our model allowed us to unravel the relationship between glutamatergic and GABAergic synaptic transmission of the epileptic neural network and the type of generated IEE. Moreover, sensitivity analyses allowed for the exploration of the pathophysiological parameters responsible for the transitions between these events. Finally, the presented modeling framework also provides an Electrode Tissue Model (ETI) that adds realism to the simulated signals and offers the possibility of studying their sensitivity to the electrode characteristics. The model (NeoCoMM) presented in this work can be of great use in different applications since it offers an framework for sensitivity analysis and hypothesis testing. It can also be used as a starting point for more complex studies.
