Abstract:Tissue adhesives are a versatile and valuable alternative for wound closure. With fast application, the prevention of body fluid leakage and additional trauma to the wound, adhesives are desirable, especially in friable tissues. Although several options are already in the market, these present some drawbacks, namely poor adhesion in wet substrates and toxicity. Here, the main adhesives both synthetic and biomimetic, commercially available and those still in research, are analyzed with a focus on their adhesion mechanisms. At present, the strongest adhesive able to replace sutures in skin wounds is cyanoacrylate based. However it is toxic and therefore cannot be used internally. More recently alternatives have emerged, with PEG-based adhesives being biocompatible but mainly used as tissue sealants due to their low strength. The most recent approaches under development try to mimic the adhesion strategies of several organisms, such as the extensively studied blue mussels, sandcastle worms, barnacles and geckos. Although no ideal results have been achieved so far, large improvements have been accomplished in the last decade and research is reaching to a new level where the results are starting to meet the needs for medical applications.

Abstract:Recent advancements in naturally derived bioadhesives have transformed their application across diverse medical fields, including tissue engineering, wound management, and surgery. This review focuses on the innovative development and multifunctional nature of these bioadhesives, particularly emphasizing their role in enhancing adhesion performance in wet environments and optimizing mechanical properties for use in dynamic tissues. Key areas covered include the chemical and physical mechanisms of adhesion, the incorporation of multi‐adhesion strategies that combine covalent and non‐covalent bonding, and bioinspired designs mimicking natural adhesives such as those of barnacles and mussels. Additionally, the review discusses emerging applications of bioadhesives in the regeneration of musculoskeletal, cardiac, neural, and ocular tissues, highlighting the potential for bioadhesive‐based therapies in complex biological settings. Despite substantial progress, challenges such as scaling lab‐based innovations for clinical use and overcoming environmental and mechanical constraints remain critical. Ongoing research in bioadhesive technologies aims to bridge these gaps, promising significant improvements in medical adhesives tailored for diverse therapeutic needs.

Abstract:In the last decades, adhesives derived from natural resources (i.e., bioadhesives) have emerged as promising alternative to the standard wound closure devices, including sutures, clips, and strips, owing to relatively easy and rapid application, minimal tissue damage, fast hemostasis, and ability to decrease the risk of infection. Various synthetic and natural materials have been utilized as bioadhesives. These materials find extensive application in various biomedical fields, ranging from simple wound sealing to controlled drug delivery, tissue regeneration, and noninvasive therapy. Considering the weak underwater adhesion, degradability, and biological performances of synthetic adhesives, naturally derived-based adhesives are more attractive. The first generation of these bioadhesives provided primarily only one function. Moreover, they had issues including long curing time, slow adhesion, high degradation rate, low mechanical properties, and the risk of transferring contamination to the wound. Various chemically and genetically engineered strategies have been applied to advance their multifunctionality. The synergy of bonding chemistry, topography, and mechanics of dissipation in their structure supports the improved adhesion and controlled degradation rate. Various naturally derived bioadhesives are developed that cover subjects from innovative biomaterial synthesis or functionalization and cutting-edge manufacturing processes. However, to fulfill all the criteria of an ideal bioadhesive for clinical applications, more efforts should be devoted to investigating the surface characteristics of target tissues and the long-term relationship between the physiochemical properties of natural polymers and cohesion and adhesion mechanisms, as well as adhesive functionality. This review outlines the recent progress on naturally-derived bioadhesives, including proteins and polysaccharides, focusing on designing approaches based on chemically and genetically engineering strategies, development, and applications. Furthermore, the challenges of current studies are summarized to show future perspectives for developing bioengineered and high-performance naturally-derived bioadhesives for clinical use.

Abstract:AbstractProtein‐based adhesives with their robust adhesion performance and excellent biocompatibility have been extensively explored over years. In particular, the unique adhesion behaviours of mussel and sandcastle worm inspired the development of synthetic adhesives. However, the chemical synthesized adhesives often demonstrate weak underwater adhesion performance and poor biocompatibility/biodegradability, limiting their further biomedical applications. In sharp contrast, genetically engineering endows the protein‐based adhesives the ability to maintain underwater adhesion property as well as biocompatibility/biodegradability. Herein, we outline recent advances in the design and development of protein‐based adhesives by genetic engineering. We summarize the fabrication and adhesion performance of elastin‐like polypeptide‐based adhesives, followed by mussel foot protein (mfp) based adhesives and other sources protein‐based adhesives, such as, spider silk spidroin and suckerin. In addition, the biomedical applications of these bioengineered protein‐based adhesives are presented. Finally, we give a brief summary and perspective on the future development of bioengineered protein‐based adhesives.

Abstract:Bone fracture healing involves complex physiological processes that require biological events that are well coordinated. In recent decades, the process of fracture healing has been upheld through various treatments, including bone implants and bio-adhesive utilization. Bio-adhesion can be interpreted as the process in which synthetic or natural materials adhere to body surfaces. Bio-based adhesives have superiority in many value-added applications because of their biocompatibility, biodegradability, and large molecular weight. The increased variety and utilization of bio-based materials with strong adhesion characteristics provide new possibilities in the field of orthopedics in terms of using bio-based adhesives with excellent resorbability, biocompatibility, ease of use, and low immunoreactivity. The aim of this review is to provide comprehensive information and evaluation of the various types of bio-based adhesives used clinically with a specific focus on their application in orthopedics. The main properties of bio-based adhesives, their benefits, and challenges compared with the traditional bio-based materials in orthopedics, as well as the future perspectives in the field, have also been outlined and discussed.
Strong and bioactive bioinspired biomaterials, next generation of bone adhesives

Abstract:The bone adhesive is a clinical requirement for complicated bone fractures always articulated by surgeons. Applying glue is a quick and easy way to fix broken bones. Adhesives, unlike conventional fixation methods such as wires and sutures, improve healing conditions and reduce postoperative pain by creating a complete connection at the fractured joint. Despite many efforts in the field of bone adhesives, the creation of a successful adhesive with robust adhesion and appropriate bioactivity for the treatment of bone fractures is still in its infancy. Because of the resemblance of the body's humid environment to the underwater environment, in the latest decades, researchers have pursued inspiration from nature to develop strong bioactive adhesives for bone tissue. The aim of this review article is to discuss the recent state of the art in bone adhesives with a specific focus on biomimetic adhesives, their action mechanisms, and upcoming perspective. Firstly, the adhesive biomaterials with specific affinity to bone tissue are introduced and their rational design is studied. Consequently, various types of synthetic and natural bioadhesives for bone tissue are comprehensively overviewed. Then, bioinspired-adhesives are described, highlighting relevant structures and examples of biomimetic adhesives mainly made of DOPA and the complex coacervates inspired by proteins secreted in mussel and sandcastle worms, respectively. Finally, this article overviews the challenges of the current bioadhesives and the future research for the improvement of the properties of biomimetic adhesives for use as bone adhesives.

Abstract:In clinical practice, tissue adhesives have emerged as an alternative tool for wound treatments due to their advantages in ease of use, rapid application, less pain, and minimal tissue damage. Since most tissue adhesives are designed for internal use or wound treatments, the biodegradation of adhesives is important. To endow tissue adhesives with biodegradability, in the past few decades, various biodegradable polymers, either natural polymers (such as chitosan, hyaluronic acid, gelatin, chondroitin sulfate, starch, sodium alginate, glucans, pectin, functional proteins, and peptides) or synthetic polymers (such as poly(lactic acid), polyurethanes, polycaprolactone, and poly(lactic-co-glycolic acid)), have been utilized to develop novel biodegradable tissue adhesives. Incorporated biodegradable polymers are degraded in vivo with time under specific conditions, leading to the destruction of the structure and the further degradation of tissue adhesives. In this review, we first summarize the strategies of utilizing biodegradable polymers to develop tissue adhesives. Furthermore, we provide a symmetric overview of the biodegradable polymers used for tissue adhesives, with a specific focus on the degradability and applications of these tissue adhesives. Additionally, the challenges and perspectives of biodegradable polymer-based tissue adhesives are discussed. We expect that this review can provide new inspirations for the design of novel biodegradable tissue adhesives for biomedical applications.

Abstract:Over millions of years, nature has created countless unique features frequently used in the manufacture of adhesive formulations for wound healing. The bioinspired adhesives could accommodate dynamic tissue movement with excellent biocompatibility to facilitate external and internal wound healing. In this review, we summarise two types of bioinspired adhesives used for wound healing, including chemical-rationale bioinspired adhesives, such as mussel-inspired injectable hydrogel and physical-rationale bioinspired adhesives, such as octopus-inspired adhesive patches. Herein, we mainly focus on fundamental design, multi-faceted bionic strategies and potential clinical utility in surgical wound closure, haemostasis and tissue regeneration. We discuss the feasibility of incorporating nature-inspired properties into formulation design to offer a new reference for the application of bioinspired wound dressings.

Abstract:Abstract Tissue loss through injury, surgery, and disease motivates the development of new biomaterials to enable tissue repair and regeneration. Emerging at the interface between bioadhesives and regenerative medicine, a new generation of regenerative bioadhesives is created to possess dual functions of seamless tissue adhesion and effective tissue repair. This bioadhesive innovation has wide clinical applications, ranging from wound management to the regeneration of musculoskeletal tissues such as tendons and intervertebral discs. This perspective covers the design principles of regenerative bioadhesives in considering both mechanical and biological elements. Case studies of regenerative bioadhesives for load‐bearing organs such as skin, tendon, and intervertebral discs are presented here. Finally, immediate opportunities and future perspectives are outlined to further advance the field of regenerative bioadhesives.
Bio-inspired adhesive hydrogel for biomedicine-principles and design strategies

Abstract:The adhesiveness of hydrogels is urgently required in various biomedical applications such as medical patches, tissue sealants, and flexible electronic devices. However, biological tissues are often wet, soft, movable, and easily damaged. These features pose difficulties for the construction of adhesive hydrogels for medical use. In nature, organisms adhere to unique strategies, such as reversible sucker adhesion in octopuses and nontoxic and firm catechol chemistry in mussels, which provide many inspirations for medical hydrogels to overcome the above challenges. In this review, we systematically classify bioadhesion strategies into structure-related and molecular-related ones, which cover almost all known bioadhesion paradigms. We outline the principles of these strategies and summarize the corresponding designs of medical adhesive hydrogels inspired by them. Finally, conclusions and perspectives concerning the development of this field are provided. For the booming bio-inspired adhesive hydrogels, this review aims to summarize and analyze the various existing theories and provide systematic guidance for future research from an innovative perspective.

Abstract:Injectable bioadhesives offer several advantages over conventional staples and sutures in surgery to seal and close incisions or wounds. Despite the growing research in recent years few injectable bioadhesives are available for clinical use. This review summarizes the key chemical features that enable the development and improvements in the use of polymeric injectable hydrogels as bioadhesives or sealants, their design requirements, the gelation mechanism, synthesis routes, and the role of adhesion mechanisms and strategies in different biomedical applications. It is envisaged that developing a deep understanding of the underlying materials chemistry principles will enable researchers to effectively translate bioadhesive technologies into clinically-relevant products.
Targeted radiotherapy of osteosarcoma using 153 Sm-EDTMP. A new promising approach.

Abstract:We report a case where targeted radionuclide therapy using 153Sm-EDTMP gave substantial palliative effect. A 35-year-old male with a primary osteosarcoma located in the first lumbar vertebra relapsed with progressive back pain after conventional treatment modalities had failed. He became bedridden, and developed paraparesis and impaired bladder function. On a diagnostic bone-scan intense radioactivity was localized in the tumor. He therefore was given 153Sm-EDTMP treatment twice, 8 weeks apart, 35 and 32 MBq/kg body weight respectively. After a few days the pain was significantly relieved and by the second radionuclide treatment the pareses subsided. For six months he was able to be up and about without any neurological signs or detectable metastases. Eventually, however, he experienced increasing local pain, developed paraparesis, was re-operated but died 4 months later. The dramatic transient improvement observed in this case warrants further exploration using 153Sm-EDTMP as a boost technique, supplementary to conventional external radiotherapy.

Abstract:Chitosan bio-adhesives bond strongly with various biological tissues, such as skin, mucosa, and internal organs. Their adhesive ability arises from amino acid and hydroxyl groups in chitosan, facilitating interactions with tissue surfaces through chemical (ionic, covalent, and hydrogen) and physical (chain entanglement) bonding. As non-toxic, biodegradable, and biocompatible materials, chitosan bio-adhesives are a safe option for medical therapies. They are particularly suitable for drug delivery, wound healing, and tissue regeneration. In this review, we address chitosan-based bio-adhesives and the mechanisms associated with them. We also discuss different chitosan composite-based bio-adhesives and their biomedical applications in wound healing, drug delivery, hemostasis, and tissue regeneration. Finally, challenges and future perspectives for the clinical use of chitosan-based bio-adhesives are discussed.

Engineering Bio-Adhesives Based on Protein–Polysaccharide Phase Separation

Abstract:Glue-type bio-adhesives are in high demand for many applications, including hemostasis, wound closure, and integration of bioelectronic devices, due to their injectable ability and in situ adhesion. However, most glue-type bio-adhesives cannot be used for short-term tissue adhesion due to their weak instant cohesion. Here, we show a novel glue-type bio-adhesive based on the phase separation of proteins and polysaccharides by functionalizing polysaccharides with dopa. The bio-adhesive exhibits increased adhesion performance and enhanced phase separation behaviors. Because of the cohesion from phase separation and adhesion from dopa, the bio-adhesive shows excellent instant and long-term adhesion performance for both organic and inorganic substrates. The long-term adhesion strength of the bio-glue on wet tissues reached 1.48 MPa (shear strength), while the interfacial toughness reached ~880 J m−2. Due to the unique phase separation behaviors, the bio-glue can even work normally in aqueous environments. At last, the feasibility of this glue-type bio-adhesive in the adhesion of various visceral tissues in vitro was demonstrated to have excellent biocompatibility. Given the convenience of application, biocompatibility, and robust bio-adhesion, we anticipate the bio-glue may find broad biomedical and clinical applications.

Abstract:Engineered protein bioadhesives with instantaneous and strong adhesion performances are developed for clinical dynamic wound repair and noncompressible hemorrhage. They can be massively produced and showed outstanding preponderances in terms of adhesion performance, production cost, curing rate, and flexibility, overwhelming existing clinical bioglues. This work offers promising alternatives to current commercial bioadhesives for clinical surgery. Surgical adhesives are playing an important role in wound repair and emergency hemostasis in clinical treatment. However, the development of strong bioglue with rapid in situ adhesion, durable adhesiveness, and flexibility in dynamic and moist physiological environments is still challenging. Herein, a new type of biosynthetic protein bioadhesives with superior adhesion performance is reported by developing a protein aldimine condensation strategy. Lysine‐rich recombinant proteins are designed and massively biosynthesized to instantaneously react with aldehyde cross‐linkers to realize in situ strong adhesion. The obtained bioadhesives show an ultra‐high adhesion strength of ≈101.6 kPa on porcine skin, outperforming extant clinical bioglues. In addition, they possess super biocompatibility, flexibility, biodegradability, and compliance with the tissues. Owing to the strong and instantaneous adhesion properties, the bioadhesives are qualified for dynamic wound closure, facilitating wound repair, and noncompressible hemorrhage. Importantly, they can be industrially encapsulated into custom‐made cartridge delivery tubes at low cost for clinical use. Therefore, biosynthetic bioadhesives have great potential for biological applications and are capable of scaling up to the industrial level for clinical transformation, which will be a successful paradigm for reforming existing clinical products.

Abstract:The need for wound closure or surgical procedures has been commonly met by application of sutures. Unfortunately, these are often invasive or subject to contamination (and consequent infection and negative impact on patients). Alternative solutions are offered by surgical adhesives that can be applied and set without major disruption; a new class of supramolecular‐based adhesives provide potential solutions to some of these challenges. In this study a series of polymers utilising dopamine as a self‐assembling unit were synthesized. It was found that these motifs act as extremely effective adhesives, with control over the mechanical strength of the adhesion and materials' tensile properties enabled by changing monomer feed ratios and levels of crosslinking. These materials significantly outperform commercially available bio‐adhesives, showing yield strengths after adhesion at least 2 times higher than that of BioGlue and Tisseel, as well as the ability to re‐adhere with significant recovery of adhesion strength. Promisingly, the materials were shown to be non‐cytotoxic, with cell viability > 90%, and able to perform in aqueous environments without significant loss in strength. Finally, the removal of the materials, after use as an adhesive, was possible using benign organic solvents such as ethanol, ensuring recyclability. These properties all demonstrate the effectiveness of the materials as potential bio‐adhesives, with potential advantages for use in surgery. This article is protected by copyright. Abstract:Traditional adhesives or glues such as cyanoacrylates, fibrin glue, polyethylene glycol, and their derivatives have been widely used in biomedical fields. However, they still suffer from numerous limitations, including the mechanical mismatch with biological tissues, weak adhesion on wet surfaces, biological incompatibility, and incapability of integrating desired multifunction. In addition to adaptive mechanical and adhesion properties, adhesive biomaterials should be able to integrate multiple functions such as stimuli-responsiveness, control-releasing of small or macromolecular therapeutic molecules, hosting of various cells, and programmable degradation to fulfill the requirements in the specific biological systems. Therefore, rational molecular engineering and structural designs are required to facilitate the development of functional adhesive materials. This review summarizes and analyzes the current supramolecular design strategies of representative adhesive materials, serving as a general guide for researchers seeking to develop novel adhesive materials for biomedical applications.

Abstract:Although tissue adhesives have potential advantages over traditional sutures, existing ones suffer from several limitations: slow adhesion kinetic, low mechanical strength, and poor interfacial bonding with wet biological tissues. Herein, a cooperative mussel/slug double‐bioinspired hydrogel adhesive (DBHA) composed of a robust adhesive interface and a stretchable dissipative matrix is developed. The DBHA is formed by a cationic polysaccharide (chitosan), an anionic polysaccharide (carboxymethyl cellulose), and a barbell‐like dendritic lysine grafted with catechol groups. Compared to various commercial bio‐glues and traditional adhesives, the DBHA has significantly stronger tissue adhesion and enhanced toughness both ex vivo and in vivo. Meanwhile, the DBHA exhibits fast, strong, tough and durable adhesion to diverse ex vivo tissue surfaces with blood. The adhesion energy between the adhesive and porcine skin can reach 200–900 J m−2. Additionally, in vivo studies prove that DBHA has good hemostasis of rabbit artery trauma and achieves better wound healing of tissue incision than commercial bio‐glues. This study provides a novel strategy for fabricating fast and strong wet adhesives, which can be used in many applications, such as soft robots, tissue adhesives, and hemostats. Abstract:Wound closure and healing have been a problem that humans have faced since the ancient eras. An appropriate tissue connector must hold the edges of injured tissues close together to support healing and also prevent the leakage of biological fluids when resisting against the tensile forces. Even though clinical usage of mechanical methods is convenient for wound closure, their application has some limitations and drawbacks such as being painful for patients and hard to apply for surgeons when the injured site is not in touch. Furthermore, they do not have desirable cosmetic results. To solve these problems, closing the wounds with sticky materials has been introduced to prevent bleeding and induce the wound healing process. To this regard, many types of surgical adhesives including tissue adhesives have been developed to be a suitable alternative for sutures and staples. There is also a new approach which aims at producing bioadhesives by mimicking the nature along with applying nanotechnology methods. Today, many studies have been done to develop new adhesives inspired by nature. We have attempted to introduce the fundamentals of wound healing along with the different types of bioadhesives, their properties, and clinical applications in a simple and illustrated comprehensive way.

Abstract:Bioadhesive films are considered a novel drug delivery system thatprovides improved therapy and better patient compliance than conventional dosage forms. The type and concentration of polymer or the blend of polymers used are important factors that can influence the properties of bioadhesive films, the product outcome and the performance. But the modification of available polymers to synthesize polymers with improved properties is the current area of research. Furthermore, addressing the challenges in formulating bioadhesive films for transdermal delivery of a variety of different classes of drug molecules is the frontier for future research. With the advancement of synthetic chemistry and the development of newer technology like 3D printing, more studies are being conducted on bioadhesive films that have opened up many opportunities in this field of research. This article reviews the current advancements in developing bio-adhesive films and provides basic principles that will be helpful to overcome the difficulties that are associated with the design of formulation.
