Abstract:OBJECTIVE To characterize amplitude-integrated electroencephalo graphic (aEEG) traces in neonates with acute bilirubin encephalopathy (ABE), explore the value of aEEG in early diagnosis and prediction of neurological outcome of ABE. METHOD aEEG records of 10 cases with ABE (Oct 2009-Nov 2011) were reviewed to identify neonates with a diagnosis of ABE. Clinical data were collected. The aEEG traces were classified according to background activity (normal, moderate, or severely abnormal), presence of seizures and sleep-wake cycling (SWC). Brainstem auditory evoked potential (BAEP) and magnetic resonance imaging (MRI) were studied. The neuromotor development of survivors with ABE was assessed by using the Infant Neurological International Battery (INFANIB). RESULT The characteristics of aEEG tracings in these infants with ABE were shown continuous normal voltage (CNV, n = 5), discontinuous voltage (DNV, n = 4), discontinuous voltage with burst-suppression (BS)BS+ (n = 1); mature SWC (n = 2), immature SWC (n = 5), no SWC (n = 3); 8 infants (80%) had electrical seizures: single seizure (n = 2); repetitive seizures (n = 2), and status epilepticus (SE) (n = 4). Among the 10 infants with ABE, no infants had normal aEEG, 3 had mildly abnormal aEEG, and 7 had severely abnormal aEEG. Eight infants accepted BAEP test, 2 were mildly abnormal and 6 were severely abnormal. Six infants accepted MRI, 1 was normal and 5 were abnormal. By chi-square analysis and Spearman rank correlation analysis, the results of aEEG classification were correlated with the phase of ABE and the severity of BAEP. These infants were followed up for more than 6 months (range 6 months to 1 year). In 3 infants with mildly abnormal aEEG, 2 were normal and 1 was transit in infanib score at 6 months of age. Of 7 infants with severely abnormal aEEG, 1 died, 3 were abnormal (2 Spastic dyskinesia and 1 hypotonia), 2 were transit in infanib score at 6 months old. 1 lost to follow-up. CONCLUSION Amplitude-integrated electroencephalography can provide important information of the status of cerebral function in neonates with ABE and help to predict its neurological outcome.

Abstract:Background: Deviations from the regular pattern of growth and development could lead to early childhood diseases, suggesting the importance of evaluating early brain development. Through this study, we aimed to explore the changing patterns of white matter and gray matter during neonatal brain development using diffusion kurtosis imaging (DKI). Materials and methods: In total, 42 full-term neonates (within 28 days of birth) underwent conventional brain magnetic resonance imaging (MRI) and DKI. The DKI metrics (including kurtosis parameters and diffusion parameters) of white matter and deep gray matter were measured. DKI metrics from the different regions of interest (ROIs) were evaluated using the Kruskal–Wallis test and Bonferroni method. Spearman rank correlation analysis of the DKI metrics was conducted, and the age at the time of brain MRI acquisition was calculated. The subjects were divided into three groups according to their age at the time of brain MRI acquisition: the first group, neonates aged ≤7 days; the second group, neonates aged 8–14 days; and the third group, neonates aged 15–28 days. The rate of change in DKI metrics relative to the first group was computed. Results: The mean kurtosis (MK), axial kurtosis (Ka), radial kurtosis (Kr), and fractional anisotropy (FA) values showed positive correlations, whereas mean diffusion (MD), axial diffusion (Da), and radial diffusion (Dr) values showed negative correlations with the age at the time of brain MRI acquisition. The absolute correlation coefficients between MK values of almost all ROIs (except genu of the corpus callosum and frontal white matter) and the age at the time of brain MRI acquisition were greater than other metrics. The kurtosis parameters and FA values of central white matter were significantly higher than that of peripheral white matter, whereas the MD and Dr values were significantly lower than that of peripheral white matter. The MK value of the posterior limb of the internal capsule was the highest among the white matter areas. The FA value of the splenium of the corpus callosum was significantly higher than that of the other white matter areas. The kurtosis parameters and FA values of globus pallidus and thalamus were significantly higher than those of the caudate nucleus and putamen, whereas the Da and Dr values of globus pallidus and thalamus were significantly lower than those of the caudate nucleus and putamen. The relative change rates of kurtosis parameters and FA values of all ROIs were greater than those of MD, Da, and Dr values. The amplitude of MK values of almost all ROIs (except for the genu of the corpus callosum and central white matter of the centrum semiovale level) was greater than that of other metrics. The relative change rates of the Kr values of most ROIs were greater than those of the Ka value, and the relative change rates of the Dr values of most ROIs were greater than those of the Da value. Conclusion: DKI parameters showed potential advantages in detecting the changes in brain microstructure during neonatal brain development.

Abstract:Diffusion MRI (dMRI) is known to be sensitive to hypoxic-ischemic encephalopathy (HIE) , however, the existing dMRI studies only used diffusion tensor metrics in a few selected brain regions and primarily focus on severe-to-moderate HIE. Here we investigated microstructural alterations using multi-shell dMRI across the whole-brain in severe, moderate, and mild HIE babies (n=5/13/13) in comparison with control neonates (n=11) with region-of-interest-based, tract-based, and fixel-based analysis. We found microstructural alternations in projection and association fibers, especially the inferior fronto-occipital fasciculus and inferior longitudinal fasciculus that are important for visual functions.
Clinical Features of 116 Near Term and Term Infants with Acute Bilirubin Encephalopathy in Eastern China

Abstract:Purpose: This study delineates the clinical features of near term and term infants with acute bilirubin encephalopathy (ABE) from east China and shares our experience in managing these patients. Methods: This is a retrospective study conducted in a children's hospital in east China. Clinical charts of infants with ABE were reviewed. The data were collected by a detailed questionnaire and analysed. Results: From August 2004 to July 2011, 116 infants assessed to have developed ABE were recorded in this hospital. The infants had a mean birth weight of 3176±453 grams with gestational age of 38.1±1.6 weeks. Seventy infants (60.3%) were males. After admission, the highest bilirubin level was (486.0±169.4) μmol/L. The most common cause of ABE was ABO incompatibility (38, 32.8%), followed by sepsis and infection (14, 12.1%). Phototherapy was performed in 84.5% infants, and exchange transfusion was done in 45 infants (38.8%). Based on clinical bilirubin-induced neurologic dysfunction (BIND) scoring, 14 (12.1%), 83 (71.6%) and 19 (16.4%) infants were classified as subtle, moderate ABE and severe ABE respectively on admission. The severe ABE group had worse short-time outcomes than the subtle and the moderate ABE groups. Conclusions: Neonatal acute bilirubin encephalopathy in term and near-term infants is not a benign entity. It carries fairly high death rate and often leads to significantly poor short-term outcomes. BIND score should be evaluated after admission to evaluate short-time outcomes. Recognition of the whole clinical spectrum and progress of ABE is of paramount importance in the prevention and management of ABE.

Abstract:OBJECTIVE:To study brainstem auditory evoked potential (BAEP) in neonates with hyperbilirubinemia using short auditory stimuli (60 dBnHL), and to investigate the differences in the inter-aural latency difference (ILD) of wave V between neonates with different total serum bilirubin (TSB) levels. METHODS:A prospective study was conducted in neonates with hyperbilirubinemia who were admitted to the Department of Neonatology, Yuhuan People's Hospital of Zhejiang Province, from May 2019 to October 2020. The neonates were divided into a severe group (n=50) and a mild group (n=50) according to their TSB levels. The mild group was divided into two subgroups: 7-10 days (n=20) and 11-14 days (n=20) according to their age. ILD was compared between the neonates with different TSB levels, and its diagnostic value was analyzed. RESULTS:Compared with the mild group, the severe group had significantly higher proportions of neonates with abnormal hearing threshold and abnormal ILD (P < 0.05) and a significantly larger ILD of wave V (P < 0.05). The latency of wave V (left ear) in the 7-10 days subgroup was significantly longer than that in the 11-14 days subgroup (P < 0.05), but there was no significant difference in the ILD of wave V between the two groups (P > 0.05). The receiver operating characteristic (ROC) analysis showed that ILD had predictive value for hearing impairment caused by neonatal hyperbilirubinemia (P < 0.05), with an area under the ROC curve of 0.727 as well as a sensitivity of 52.4% and a specificity of 90.9% at the optimal cut-off value of 0.365 ms. CONCLUSIONS:Serum bilirubin in neonates affects the ILD of BAEP wave V, especially in those with severe hyperbilirubinemia. ILD at the optimal cut-off value of ≥0.4 ms shows potential value in the diagnosis of hearing impairment caused by neonatal hyperbilirubinemia.

Abstract:Objective: Preterm infants are at high risk of the adverse neurodevelopmental outcomes. Our aim is to explore the value of diffusion kurtosis imaging (DKI) in diagnosing brain developmental disorders in premature infants. Materials and Methods: A total of 52 subjects were included in this study, including 26 premature infants as the preterm group, and 26 full-term infants as the control group. Routine MRI and DKI examinations were performed. Mean kurtosis (MK), radial kurtosis (RK), fractional anisotropy (FA), and mean diffusivity (MD) values were measured in the brain regions including posterior limbs of the internal capsule (PLIC), anterior limb of internal capsule (ALIC), parietal white matter (PWM), frontal white matter (FWM), thalamus (TH), caudate nucleus (CN), and genu of the corpus callosum (GCC). The chi-squared test, t -test, Spearman's correlation analysis, and receiver operating characteristic curve were used for data analyses. Results: In the premature infant group, the MK and RK values of PLIA, ALIC, and PWM were lower than those in the control group ( p < 0.05). The FA values of PWM, FWM, and TH were also lower than those of the control group ( p < 0.05). The area under curves of MK in PLIC and ALIC, MD in PWM, and FA in FWM were 0.813, 0.802, 0.842, and 0.867 ( p < 0.05). In the thalamus and CN, the correlations between MK, RK values, and postmenstrual age (PMA) were higher than those between FA, MD values, and PMA. Conclusion: Diffusion kurtosis imaging can be used as an effective tool in detecting brain developmental disorders in premature infants.

Amplitude of Low-Frequency Fluctuation May Be an Early Predictor of Delayed Motor Development Due to Neonatal Hyperbilirubinemia: a Fmri Study

Abstract:Background: Acute bilirubin encephalopathy or kernicterus is the worst consequence of brain damage caused by the elevation of total unbound serum bilirubin (TSB) in neonates. The present study aimed to visualize the characteristic brain regions of neonates with hyperbilirubinemia (HB) using functional magnetic resonance imaging (fMRI) and to measure the amplitude of low-frequency fluctuation (ALFF) values. Methods: This was a prospective cohort study, which included newborns with HB who were hospitalized at the Children's Hospital of Fudan University. The control group included neonates admitted with neonatal simple wet lung or pneumonia without neurological disease or brain injury. Newborns were divided into a severe hyperbilirubinemia group (SHB), moderate HB group, and control group based on TSB levels. The newborns completed routine MRI combined with fMRI scans and brainstem auditory evoked potentials (BAEPs) during their hospitalization. Results: A total of 251 newborns were included in this study. There were 45 patients in the SHB group, 65 in the HB group, and 141 in the control group. The average ALFF value in the basal ganglia region in the SHB group was the highest, which was greater than that in the HB and control groups (P<0.001). The ALFF increased with an increase in TSB concentration. Based on the results of the Bayley Scales of infant development assessment, we further found that the most significant difference in ALFF remained in the basal ganglia region between newborns with motor development scores above 70 (including 70) and below 70. Correlation analysis revealed a strong negative correlation between motor development scores and ALFF (r=-0.691, P<0.001). When ALFF alone was used to predict motor development, the sensitivity was 89%. When ALFF was combined with TSB and BEAP results, the area under the ROC curve was the largest (AUC =0.85). The sensitivity and specificity of the model were 67.86% and 90.77%, respectively. Conclusions: The ALFF value may be able to serve as an early imaging biomarker and has a greater sensitivity than TSB or BAEP results in predicting long-term motor development (18 m) in HB.

Abstract:Objective: Hypoxic-ischemic encephalopathy can lead to lifelong morbidity and premature death in full-term newborns. Here, we aimed to determine the efficacy of diffusion kurtosis (DK) [mean kurtosis (MK)] and diffusion tensor (DT) [fractional anisotropy (FA), mean diffusion (MD), axial diffusion (AD), and radial diffusion (RD)] parameters for the early diagnosis of early brain histopathological changes and the prediction of neurodegenerative events in a full-term neonatal hypoxic-ischemic brain injury (HIBD) rat model. Methods: The HIBD model was generated in postnatal day 7 Sprague-Dawley rats to assess the changes in DK and DT parameters in 10 specific brain structural regions involving the gray matter, white matter, and limbic system during acute (12 h) and subacute (3 d and 5 d) phases after hypoxic ischemia (HI), which were validated against histology. Sensory and cognitive parameters were assessed by the open field, novel object recognition, elevated plus maze, and CatWalk tests. Results: Repeated-measures ANOVA revealed that specific brain structures showed similar trends to the lesion, and the temporal pattern of MK was substantially more varied than DT parameters, particularly in the deep gray matter. The change rate of MK in the acute phase (12 h) was significantly higher than that of DT parameters. We noted a delayed pseudo-normalization for MK. Additionally, MD, AD, and RD showed more pronounced differences between males and females after HI compared to MK, which was confirmed in behavioral tests. HI females exhibited anxiolytic hyperactivity-like baseline behavior, while the memory ability of HI males was affected in the novel object recognition test. CatWalk assessments revealed chronic deficits in limb gait parameters, particularly the left front paw and right hind paw, as well as poorer performance in HI males than HI females. Conclusions: Our results suggested that DK and DT parameters were complementary in the immature brain and provided great value in assessing early tissue microstructural changes and predicting long-term neurobehavioral deficits, highlighting their ability to detect both acute and long-term changes. Thus, the various diffusion coefficient parameters estimated by the DKI model are powerful tools for early HIBD diagnosis and prognosis assessment, thus providing an experimental and theoretical basis for clinical treatment.

Abstract:Purpose To identify brain regions affected by Hypoxic-Ischemic Encephalopathy (HIE) in neonates using Amide Proton Transfer (APT) imaging and Apparent Diffusion Coefficient (ADC). Materials and Methods Twenty neonates were divided into HIE and control groups. all neonates were undergoing MRI, including APT and DWI. Imaging analysis was performed using SPM12. The independent-samples t-test was used to analyze the difference in APTw values and ADC values between the mild HIE neonates and the control group. The receiver operating characteristic (ROC) curves were established to assess the diagnostic values of APTw and ADC values in different brain regions for HIE. Pearson's correlation analysis was used to analyze the correlation between APTw values and ADC values for each region. Results APTw values were significantly higher in 26 regions of the HIE group. ADC values were lower in the right anterior temporal lobe and higher in bilateral Subthalamic nucleus in HIE. The APTw values of 22 regions showed very high area under the curve (AUC), whereas the AUC of ADC values in right anterior temporal lobe and right subthalamic nucleus were both 0.802. Notably, the right anterior temporal lobe exhibited significant differences in both APTw and ADC values between the HIE and control groups, additionally, APTw value was significant positive correlated with ADC values in right anterior temporal lobe. Conclusion APTw and ADC are effective in detecting HIE, with APTw being more sensitive. The right anterior temporal lobe is particularly affected by HIE, with significant changes in APTw and ADC values and a positive correlation between them. This suggests that temporal lobe damage may be critical in the long-term neurological consequences of HIE.
Detecting neonatal acute bilirubin encephalopathy based on T1-weighted MRI images and learning-based approaches

Abstract:Background Neonatal hyperbilirubinemia is a common clinical condition that requires medical attention in newborns, which may develop into acute bilirubin encephalopathy with a significant risk of long-term neurological deficits. The current clinical challenge lies in the separation of acute bilirubin encephalopathy and non-acute bilirubin encephalopathy neonates both with hyperbilirubinemia condition since both of them demonstrated similar T1 hyperintensity and lead to difficulties in clinical diagnosis based on the conventional radiological reading. This study aims to investigate the utility of T1-weighted MRI images for differentiating acute bilirubin encephalopathy and non-acute bilirubin encephalopathy neonates with hyperbilirubinemia. Methods 3 diagnostic approaches, including a visual inspection, a semi-quantitative method based on normalized the T1-weighted intensities of the globus pallidus and subthalamic nuclei, and a deep learning method with ResNet18 framework were applied to classify 47 acute bilirubin encephalopathy neonates and 32 non-acute bilirubin encephalopathy neonates with hyperbilirubinemia based on T1-weighted images. Chi-squared test and t-test were used to test the significant difference of clinical features between the 2 groups. Results The visual inspection got a poor diagnostic accuracy of 53.58 ± 5.71% indicating the difficulty of the challenge in real clinical practice. However, the semi-quantitative approach and ResNet18 achieved a classification accuracy of 62.11 ± 8.03% and 72.15%, respectively, which outperformed visual inspection significantly. Conclusion Our study indicates that it is not sufficient to only use T1-weighted MRI images to detect neonates with acute bilirubin encephalopathy. Other more MRI multimodal images combined with T1-weighted MRI images are expected to use to improve the accuracy in future work. However, this study demonstrates that the semi-quantitative measurement based on T1-weighted MRI images is a simple and compromised way to discriminate acute bilirubin encephalopathy and non-acute bilirubin encephalopathy neonates with hyperbilirubinemia, which may be helpful in improving the current manual diagnosis.
Progressive Microstructural Alterations in Subcortical Nuclei in Parkinson's Disease: A Diffusion Magnetic Resonance Imaging Study.

Abstract:Objectives: To explore the microstructural alterations in subcortical nuclei in Parkinson's disease (PD) at different stages with diffusion kurtosis imaging (DKI) and tensor imaging and to test the performance of diffusion metrics in identifying PD. Methods: 108 PD patients (64 patients in early-stage PD group (EPD) and 44 patients in moderate-late-stage PD group (MLPD)) and 64 healthy controls (HC) were included. Tensor and kurtosis metrics in the subcortical nuclei were compared. Partial correlation was used to correlate the diffusion metrics and Unified Parkinson's Disease Rating Scale part-III (UPDRS-III) score. Logistic regression and receiver operating characteristic analysis were applied to test the diagnostic performance of the diffusion metrics. Results: Compared with HC, both EPD and MLPD patients showed higher fractional anisotropy and axial diffusivity, lower mean kurtosis (MK) and axial kurtosis in substantia nigra, lower MK and radial kurtosis (RK) in globus pallidus (GP) and thalamus (all p < 0.05). Compared with EPD, MLPD patients showed lower MK and RK in GP and thalamus (all p < 0.05). MK and RK in GP and thalamus were negatively correlated with UPDRS-III score (all p < 0.01). The logistic regression model combining kurtosis and tensor metrics showed the best performance in diagnosing PD, EPD, and MLPD (areas under curve were 0.817, 0.769, and 0.914, respectively). Conclusions: PD has progressive microstructural alterations in the subcortical nuclei. DKI is sensitive to detect microstructural alterations in GP and thalamus during PD progression. Combining kurtosis and tensor metrics can achieve a good performance in diagnosing PD.

Abstract:DKI showed that children with ADHD had abnormal gray matter and white matter development; DKI parameters could be one of the brain area markers in children with ADHD. In this study, we explored the application of diffusion kurtosis imaging (DKI) technology in the brains of children with attention‐deficit/hyperactivity disorder (ADHD). Seventy‐two children with ADHD and 79 age‐ and sex‐matched healthy controls were included in the study. All children were examined by means of 3D T1‐weighted image, DKI, and conventional sequence scanning. Abstract:Background Differentiating acute bilirubin encephalopathy (ABE) from non-ABE in neonates with hyperbilirubinemia (HB) from routine magnetic resonance imaging (MRI) is extremely challenging since both conditions demonstrate similar T1 hyperintensities. To this end, we investigated whether the integration of multimodal MRI from routine clinical scans with deep-learning approaches could improve diagnostic performance. Methods A total of 75 neonates with ABE and 75 neonates with HB (non-ABE) were included in the study. Each patient had three types of multimodal images taken, i.e., a T1-weighted image (T1WI), a T2-weighted image (T2WI), and an apparent diffusion coefficient (ADC) map. The three types of MRI contrasts and their combination were fed into two deep convolutional neural networks (CNNs), i.e., ResNet18 and DenseNet201. The performance of CNNs was compared with a traditional statistical method named logistic regression. Results We demonstrated that diagnostic methods with the multimodal data were better than any of the single-modal data. Both CNN models outperformed the logistic regression method. The best performance was achieved by DenseNet201 with the combination of three modalities of T1WI, T2WI, and ADC, with an accuracy of 0.929 ± 0.042 and an area under the curve (AUC) of 0.991 ± 0.007. Conclusions Our study demonstrated that CNN models with multimodal MRI significantly improve the accuracy of diagnosing ABE. Impact We proposed an efficient strategy of detecting ABE in neonates based on multimodal MRI with deep learning, which achieved an accuracy of 0.929 ± 0.042 and an AUC of 0.991 ± 0.007. We demonstrated the advantage of integrating multimodal MRI in detecting ABE in neonates with HB, using deep-learning models. Our strategy of diagnosing ABE using deep-learning techniques with multimodal MRI from routine clinical scans is potentially applicable to clinical practice.
Diffusion Kurtosis Imaging of Microstructural Changes in Gray Matter Nucleus in Parkinson Disease

Abstract:Objective: To explore the microstructural damage of extrapyramidal system gray matter nuclei in Parkinson disease (PD) using diffusion kurtosis imaging (DKI). Materials and Methods: We enrolled 35 clinically confirmed PD patients and 23 healthy volunteers. All patients underwent MR examination with conventional MRI scan sequences and an additional DKI sequence. We subsequently reconstructed the DKI raw images and analyzed the data. A radiologist in our hospital collected the Mini-Mental State Examination (MMSE) score of all subjects. Results: In the PD group, the mean kurtosis and axial kurtosis level decreased in the red nucleus (RN) and thalamus; the radial kurtosis increased in the substantia nigra (SN) and globus pallidus (GP). Fractional anisotropy decreased in the putamen. The largest area under the ROC curve of mean diffusion in GP was 0.811. Most kurtosis parameters demonstrated a positive correlation with the MMSE score, while several diffusion parameters showed a negative correlation with the same. Conclusion: DKI can qualitatively distinguish PD from healthy controls; furthermore, DKI-derived parameters can quantitatively evaluate the modifications of microstructures in extrapyramidal system gray matter nucleus in PD.
In Vivo Microscopic Diffusional Kurtosis Imaging with Symmetrized Double Diffusion Encoding EPI.

Abstract:Purpose Diffusional kurtosis imaging (DKI) measures the deviation of the displacement probability from a normal distribution, complementing the data commonly acquired by diffusion MRI. It is important to elucidate the sources of kurtosis contrast, particularly in biological tissues where microscopic kurtosis (intrinsic kurtosis) and diffusional heterogeneity may co‐exist. Methods We have developed a technique for microscopic kurtosis MRI, dubbed microscopic diffusional kurtosis imaging (µDKI), using a symmetrized double diffusion encoding (s‐DDE) EPI sequence. We compared this newly developed µDKI to conventional DKI methods in both a triple compartment phantom and in vivo. Results Our results showed that whereas conventional DKI and µDKI provided similar measurements in a compartment of monosphere beads, kurtosis measured by µDKI was significantly less than that measured by conventional DKI in a compartment of mixed Gaussian pools. For in vivo brain imaging, µDKI showed small yet significantly lower kurtosis measurement in regions of the cortex, CSF, and internal capsule compared to the conventional DKI approach. Conclusions Our study showed that µDKI is less susceptible than conventional DKI to sub‐voxel diffusional heterogeneity. Our study also provided important preliminary demonstration of our technique in vivo, warranting future studies to investigate its diagnostic use in examining neurological disorders.
Noninvasive assessment of clinical and pathological characteristics of patients with IgA nephropathy by diffusion kurtosis imaging

Abstract:Abstract Objectives To explore the diagnostic performance of diffusion kurtosis imaging (DKI) in evaluating the clinical and pathological characteristics of patients with immunoglobulin A nephropathy (IgAN) compared with conventional DWI. Materials and methods A total of 28 IgAN patients and 14 healthy volunteers prospectively underwent MRI examinations including coronal T2WI, axial T1WI, T2WI, and DWI sequences from September 2020 to August 2021. We measured mean kurtosis (MK), mean diffusivity (MD), and apparent diffusion coefficient (ADC) by using MR Body Diffusion Toolbox v1.4.0 (Siemens Healthcare). Patients were divided into three groups according to their estimated glomerular filtration rate (eGFR) (Group1, healthy volunteers without kidney disease or other diseases that affect renal function; Group2, IgAN patients with eGFR > 60 mL/min/1.73 m 2 ; Group3, IgAN patients with eGFR < 60 mL/min/1.73 m 2 ). One-way analysis of variance, Pearson or Spearman correlation, and receiver operating characteristic curves were applied in our statistical analysis. Results MK Cortex and ADC Cortex showed significant differences between the Group1 and Group2. MK Cortex , MD Cortex , ADC Cortex , MK Medulla , and ADC Medulla showed significant differences between Group2 and Group3. MK Cortex had the highest correlation with CKD stages ( r = 0.749, p < 0.001), and tubulointerstitial lesion score ( r = 0.656, p < 0.001). MD Cortex had the highest correlation with glomerular lesion score ( r = − 0.475, p = 0.011). MK Cortex had the highest AUC (AUC = 0.923) for differentiating Group1 from Group2, and MD Cortex had the highest AUC (AUC = 0.924) for differentiating Group2 from Group3, followed by MK Medulla (AUC = 0.923). Conclusions DKI is a feasible and reliable technique that can assess the clinical and pathological characteristics of IgAN patients and can provide more valuable information than conventional DWI, especially MK Cortex .

Abstract:The Neuroradiology Journal, Ahead of Print. Rationale and ObjectivesMore than half of the bipolar depression (BD) subjects are misdiagnosed as unipolar depression (UD) due to lack of objective diagnostic criteria. We aimed to identify microstructural neuronal changes differentiating BD from UD groups using diffusion kurtosis imaging (DKI). The objective of the study is to identify an objective neuro-imaging marker to differentiate BD from UD.Materials and MethodsA prospective, cross-sectional study included total of 62 subjects with diagnosis of bipolar depression (n = 21), unipolar depression (n = 21), and healthy controls (n = 20). All subjects underwent diffusion-weighted imaging (b = 0,1000,2000) of the whole brain on 3-Tesla MR scanner. DKI data was analyzed using 189 region whole-brain atlas. Eight diffusion and kurtosis metrics including mean diffusivity (MD), axial diffusivity (AD), radial diffusivity (RD), fractional anisotropy (FA), mean kurtosis (MK), axial kurtosis (AK), radial kurtosis (RK), and kurtosis fractional anisotropy (FKA) were measured against these 189 regions. Principle component analysis (PCA) was utilized to identify the most significant regions of the brain. ANOVA with post hoc tests was used for analyzing these regions.ResultsBD group showed increased MD, RD, decreased AK at the left amygdala and decreased MK and RK at the right hemi-cerebellum. UD group showed increased MK and RK at the right external capsule; and increased AK, MK, and RK at the right amygdala.ConclusionThe right and left amygdala, right external capsule, and right hemi-cerebellum showed microstructural abnormalities capable of differentiating BD and UD groups. Diffusion imaging especially DKI can aid in management of depression patients.
Dynamic magnetic resonance imaging findings in the early stages of neonatal hypoglycemic brain injury

Abstract:Hypoglycemia is common among neonates and can cause neurological dysfunction. This study aimed to identify the specific time window to perform diffusion-weighted imaging (DWI) for detecting early brain injury in neonatal hypoglycemia and assess the clinical characteristics and neurological outcomes of different patterns of brain injury in neonatal hypoglycemia. We conducted a retrospective analysis of the changes in conventional magnetic resonance imaging (MRI) and DWI at different time points and clinical characteristics in 86 neonates with hypoglycemic brain injury. Among 139 DWI scans, 84 showed injury site hyperintensities within 7 days after hypoglycemia. Thirty-nine scans revealed low DWI and abnormal T1-/T2-weighted imaging signals within 11-23 days after onset, of which 21 showed DWI hyperintensities in the corpus callosum. Abnormal signals were undetected in the second scans of 13 infants within 9-20 days after onset. Vulnerable sites comprised the occipital lobe (98%), splenium of the corpus callosum (60%), and parietal lobe (30%). Compared with focal injury (n = 66), extensive injury (n = 20) had higher involvement of the internal capsule, basal ganglia, and thalamus (P < 0.05); higher incidence of recurrent convulsions, respiratory failure, and coma (P < 0.01); and a higher proportion of death and severe neurodevelopmental impairment (P < 0.05). Conclusions: Neonatal hypoglycemic brain injury was most common in the occipital lobes. Severe clinical symptoms were significantly associated with extensive brain injury involving the internal capsule, basal ganglia, and thalamus, which could be attributed to hypoxia-ischemia and lead to a poor prognosis. What is known: • There is no clear neurological safe threshold for hypoglycemia in neonates. Studies suggest that symptomatic hypoglycemia can be related to brain injury. What is new: • Severe clinical symptoms are significantly associated with extensive brain injury on MRI scans. Extensive brain injuries involving the internal capsule, basal ganglia, and thalamus, which could be due to associated hypoxia-ischemia and lead to a poor prognosis.
Pathological assessment of chronic kidney disease with DWI: Is there an added value for diffusion kurtosis imaging?

Abstract:Background Chronic kidney disease (CKD) is a worldwide health problem, precise functional and pathological assessment is beneficial to better treatment. Diffusion kurtosis imaging (DKI) can evaluate non-Gaussian diffusion and may help to assess renal pathology and function. Purpose To assess pathological and functional alterations in CKD using DKI compared with diffusion-weighted imaging (DWI). Study Type Prospective study. Population 70 CKD patients and 20 healthy volunteers. Field Strength 1.5 T. Assessment All participants underwent DKI, and apparent diffusion coefficient (ADC), mean diffusivity (MD), and mean kurtosis (MK) of renal parenchyma were acquired. Correlation between renal parenchymal ADC, MD, MK, and estimated glomerular filtration rate (eGFR), pathological scores were assessed. The diagnostic efficacy of ADC, MD, and MK for assessing the degree of renal pathological injury were compared. Statistical Tests ANOVA, Spearman correlation analysis, and ROC curve analysis. Results The cortical ADC, MD were significantly higher than medulla for all participants, whereas medullary MK was significantly higher than cortex (P < 0.01). Whether eGFR reduced or not, renal parenchymal MK were significantly higher in patients than controls (P < 0.05). Positive correlation was found between eGFR and ADC (cortex, r = 0.562; medulla, r = 0.527), and negative correlation between eGFR and MK (cortex, r = -0.786; medulla, r = -0.709) (all P < 0.001). There was positive correlation between MK and glomerular injury (cortex, r = 0.681; medulla, r = 0.652), tubulointerstitial lesion (cortex, r = 0.650; medulla, r = 0.599) (all P < 0.001). For discrimination between mild and m-s renal injury group, the AUC values of ADC, MD, MK were cortex: 0.723, 0.655, 0.864 and medulla: 0.718, 0.581, 0.829. The AUC values of ADC, MD, MK were cortex: 0.708, 0.679, 0.770 and medulla: 0.713, 0.830, 0.780 for differentiating control group from mild renal injury group. Data Conclusion DKI is practicable for noninvasive assessment of renal pathology and function of CKD, DKI offer better diagnostic performance than DWI. 1 2 Evidence Level Technical Efficacy

Abstract:Objective To detect the microstructural changes in patients with cognitive impairment after acute cerebral infarction using diffusion kurtosis imaging (DKI). Materials and Methods A total of 70 patients with acute cerebral infarction were divided into two groups: 35 patients with cognitive impairment (VCI group), and 35 patients without cognitive impairment (N-VCI group), according to mini-mental state examination (MMSE) score. Healthy individuals ( n = 36) were selected as the normal control (NORM) group. DKI parameters from 28 different brain regions of interest (ROIs) were selected, measured, and compared. Results VCI group patients had significantly higher mean diffusion (MD) and significantly lower mean kurtosis (MK) values in most ROIs than those in the N-VCI and NORM groups. DKI parameters in some ROIs correlated significantly with MMSE score. The splenium of corpus callosum MD was most correlated with MMSE score, the correlation coefficient was −0.652, and this parameter had good ability to distinguish patients with VCI from healthy controls; at the optimal cut-off MD value (0.9915), sensitivity was 91.4%, specificity 100%, and the area under the curve value 0.964. Conclusions Pathological changes in some brain regions may underlie cognitive impairment after acute cerebral infarction, especially the splenium of corpus callosum. These preliminary results suggest that, in patients with VCI, DKI may be useful for assessing microstructural tissue damage.
