Abstract:Background: The transition from training to qualified practice is inherently challenging. Structured support following training was once available, but this no longer exists and the current climate of primary care places increasing demands on GPs. Professional learning, and the transition to independent practice, may thus be problematic. Aim: To explore newly qualified GPs' experiences of professional learning following transition into qualified practice. Design and setting: Qualitative study using semi-structured interviews and a focus group, conducted between December 2010 and April 2011. Participants were GPs within five years of qualification, working in the East of England. Method: Interviews were conducted with nine participants. These were followed by a focus group with four additional participants. Data collection, transcription and analysis were simultaneous, allowing iterative evolution of the topic guide to test emerging themes. Results: New GPs wished to relinquish 'dependency' and become self-directed learners, yet still wanted guidance when needed. They described a situated experience of learning, in which inclusion in a practice community afforded greater learning opportunities. Social interactions also affected the issue of constructing an independent professional identity; a transitional process through which new GPs could become self-aware 'experts' with confidence in self-directed learning. Conclusion: Potential harms of the transitional period may be mitigated by support, which needs to emphasise inclusion, validation, affirmation and the provision of feedback. Preferences for adult learning should be encouraged, but guidance needs to be available when requested. Recent proposals for enhanced GP training introduce an opportunity for a focus on independent practice during training.
Professionalism dilemmas, moral distress and the healthcare student: insights from two online UK-wide questionnaire studies

Abstract:Objective: To understand the prevalence of healthcare students' witnessing or participating in something that they think unethical (professionalism dilemmas) during workplace learning and examine whether differences exist in moral distress intensity resulting from these experiences according to gender and the frequency of occurrence. Design: Two cross-sectional online questionnaires of UK medical (study 1) and nursing, dentistry, physiotherapy and pharmacy students (study 2) concerning professionalism dilemmas and subsequent distress for (1) Patient dignity and safety breaches; (2) Valid consent for students' learning on patients; and (3) Negative workplace behaviours (eg, student abuse). Participants and setting: 2397 medical (67.4% female) and 1399 other healthcare students (81.1% female) responded. Main results: The most commonly encountered professionalism dilemmas were: student abuse and patient dignity and safety dilemmas. Multinomial and logistic regression identified significant effects for gender and frequency of occurrence. In both studies, men were more likely to classify themselves as experiencing no distress; women were more likely to classify themselves as distressed. Two distinct patterns concerning frequency were apparent: (1) Habituation (study 1): less distress with increased exposure to dilemmas 'justified' for learning; (2) Disturbance (studies 1 and 2): more distress with increased exposure to dilemmas that could not be justified. Conclusions: Tomorrow's healthcare practitioners learn within a workplace in which they frequently encounter dilemmas resulting in distress. Gender differences could be respondents acting according to gendered expectations (eg, males downplaying distress because they are expected to appear tough). Habituation to dilemmas suggests students might balance patient autonomy and right to dignity with their own needs to learn for future patient benefit. Disturbance contests the 'accepted' notion that students become less empathic over time. Future research might examine the strategies that students use to manage their distress, to understand how this impacts of issues such as burnout and/or leaving the profession.
The effects of implant strategy on finished body weight of beef cattle.

Abstract:We summarized experimental data to quantify the change in final BW due to a particular implant strategy when cattle are adjusted to the same final body composition. The database developed for this study included 13 implant trials involving a total of 13,640 animals (9,052 steers and 4,588 heifers). Fifteen different implant strategies were used among these trials, including no implant (control), single implants, and combinations of implants. Individual carcass data collected at slaughter were used to calculate the adjusted final shrunk BW at 28% empty body fat (AFBW) for each treatment group within a trial, then the implant treatments were grouped into categories according to their effect on weight at 28% empty body fat (four groups for steers and two groups for heifers). All differences in AFBW between categories were significant (P < 0.01), indicating an incremental anabolic implant dose response in AFBW over unimplanted animals. Values for AFBW ranged from 520 kg in unimplanted steers to 564 kg in steers implanted and reimplanted with Revalor-S. For heifers, AFBW ranged from 493 kg in unimplanted heifers to 535 kg in heifers implanted and reimplanted with Revalor-H. After accounting for differences in mean BW and composition of gain, implanted steers and heifers had 4.2 and 3.1% higher apparent diet ME values, respectively. Increasing the anabolic implant dose increases the weight at which animals reach a common body composition. This study indicates that anabolic implant response is due to a combination of a reduced proportion of the DMI required for maintenance, reduced energy content of gain, and efficiency of use of absorbed energy.
Change on the horizon: issues and concerns of neophyte advanced healthcare practitioners

Abstract:Aims and objectives: The aim was to interview new entrants to a Master's level programme of educational preparation for advanced healthcare practice roles. The objectives were to gain an understanding of their issues and concerns as they looked ahead to their future development and to illuminate issues concerning their experiences as students and as employees. Background: As advanced practice roles become more widespread in the United Kingdom and abroad, nurses and other healthcare professionals are becoming more involved in innovation and service development and experiencing greater autonomy in many areas, particularly in regard to clinical examination and diagnosis. Design: A qualitative design was employed to illuminate issues and concerns of new students on a Master's degree programme of educational preparation for advanced healthcare practitioners. Methods: Two focus group interviews were conducted in 2004. Sixteen students from two programme cohorts participated in the two groups. Findings: Two main themes were outlined. These were: 'opportunities for development' and 'time pressures'. Subcategories within these two themes are also discussed. Conclusions: Students undertaking Master's level preparation for advanced healthcare practitioner roles perceive personal and professional benefits and opportunities to develop innovative services to improve patient care as accruing from the programme. These opportunities and benefits were related to the United Kingdom national health policy agenda. Some role difficulties were experienced. The additional burdens that the programme of study provided were an issue for some, but these were perceived as a temporary problem, worth undergoing for the likely long-term benefits. Relevance to clinical practice: Students undertaking educational preparation for advanced practice roles see themselves as innovators, specifically aiming to develop and improve aspects of service provision. Education for advanced practice roles should not be undertaken lightly, but can equip students with various skills, particularly regarding clinical assessment, and these skills should underpin practitioners' autonomy.
How prepared are newly qualified allied health professionals for practice in the UK? A systematic review

Abstract:Objectives It is important that allied health professionals (AHPs) are prepared for clinical practice from the very start of their working lives to provide quality care for patients, for their personal well-being and for retention of the workforce. The aim of this study was to understand how well newly qualified AHPs were prepared for practice in the UK. Design Systematic review. Data sources Embase, MEDLINE, CINAHL, ERIC and BEI were searched from 2012 to 2024. Grey literature searching and citation chasing were also conducted. Eligibility criteria We included primary studies reporting the preparedness for practice of UK graduates across 15 professions; all study types; participants included graduates who were up to 2 years postgraduation, their supervisors, trainers, practice educators and employers; and all outcome measures. Data extraction and synthesis A standardised data extraction form was used. Studies were quality assessed using the Quality Appraisal for Diverse Studies tool. 10% of articles were independently double-screened, extracted and quality assessed; 90% was completed by one researcher. Results 14 reports were included (9 qualitative, 3 mixed-method and 2 quantitative). Six papers focused on radiographers, three on a mixture of professions, two on paramedics, and one each on physiotherapists, clinical psychologists and orthotists. An important finding of the review is the paucity and low-medium quality of research on the topic. The narrative synthesis tentatively suggests that graduates are adequately prepared for practice with different professions having different strengths and weaknesses. Common areas of underpreparedness across the professions were responsibility and decision-making, leadership and research. Graduates were generally well prepared in terms of their knowledge base. Conclusion High-quality in-depth research is urgently needed across AHPs to elucidate the specific roles, their nuances and the areas of underpreparedness. Further work is also needed to understand the transition into early clinical practice, ongoing learning opportunities through work, and the supervision and support structures in place. Abstract:THE outstanding feature of cartilage in the evolution of the vertebrates. is the constancy of its morphological characters. Of all the tissues of the human body none display greater persistence of histological entity. Cartilage can be traced through all the modifications of form in the evolution of the skeleton from the dogfish to Man; in the evolution of joints from union by fibrocartilage in fish to diarthrodial joints of a high degree of specialization in the mammal. The cartilaginous skull of the human embryo persists in part as the septum and cartilages of the nose in the adult; the hyaline cartilage of the embryo continues to proliferate in proximity to the diarthrodial joints of Man into extreme old age. Since repeated attempts at the analysis of the protean manifestations of disorders in bone have failed, and since the classification of disorders of bone-growth have become increasingly complex, an attempt has been made to analyse bone-growth in terms of the three fundamental processes of proliferation in cartilage, calcification in cartilage, and actual bone formation. Inasmuch as the study of osteogenesis has proved to be relativelv unfertile in interpreting disorders of growth, a return has been made to the study of the morphological characters of cartilage. Bichat described very clearly the evolution from the slimy (mesenchymal) by way of the gristly (cartilaginous) to the bony skeleton. Accordingly the mode of growth of cartilage in the mammalian embryo was examined anew in order to ascertain what light, if any, could be thrown upon disorders of skeletal growth.

Abstract:ObjectiveProfessionalism in radiography is a complex, multidimensional concept seldom investigated. During clinical placements, students may observe or be involved in unprofessional practice/professionalism lapses which result in professionalism dilemmas. Establishing what constitutes a professionalism dilemma and what action to take may be challenging for students and also practitioners. This is due to unclear reporting pathways and fear of retribution, both deterrents to raising concerns. The aim of this integrative literature review was to investigate how and why professionalism dilemmas occur. In addition, to explore the types of dilemma students experience during clinical placement and to contextualise and reflect on these findings within radiography. Ovid MEDLINE, PubMed, Google Scholar and the grey literature were reviewed, analysed and themed.Key findingsTwenty-eight papers (published between 2004 and 2020) were analysed and summarised. Four themes emerged, and were discussed within the radiography context; the nature of professionalism lapses (mistreatment, verbal abuse, exclusion and intimidation), reasons for professionalism lapses (burnout and poor role modelling), student response at the time of the professionalism dilemma (accept, resist or report) and long-term impacts on students (moral/emotional distress, professional development and choice of career).ConclusionProfessionalism dilemmas are not reported in the radiography literature but are cited in radiology and other health professions studies. Student experiences of such dilemmas can be profound and long lasting. Thus, research is required to investigate specifically the professional dilemmas experienced by radiography students in the unique environment in which they develop their clinical skills.Implications for practiceThe literature review findings can be used to support the development of strategies to enhance future teaching and modelling of professionalism and develop related research in radiography.
How does role transition affect the experience of trainee Advanced Clinical Practitioners: Qualitative evidence synthesis

Abstract:Background: Advanced Clinical Practitioners have been developed to address current and future gaps in the medical workforce. Insight into problems associated with Advanced Clinical Practitioner transition may help present and future trainees adapt to their changing and demanding health environment. Aims: To identify potential problems experienced by trainee Advanced Clinical Practitioners during transition and what the implications might be for workforce planning. Design: A qualitative evidence synthesis to examine the issue of role transition for Advanced Clinical Practitioners. Data sources: The electronic databases accessed (1997-2016) were MEDLINE, EMBASE, CINAHL, BNI, AMED and PubMed and also included Researchgate, thesis publications, hand searching and NHS staffing reports. Review methods: Eleven studies were identified between 1997 - 2016. Thematic synthesis was undertaken, creating codes, descriptive and analytical themes. Quality appraisal of individual studies was conducted using the tool of Walsh and Downe. Findings: Six analytical themes were identified that addressed the key issues of transition discussed in the 11 articles and which were directly related to the proposed research project: experience of change, orientation to role, mentorship, clinical skills, clinical supervision and Masters' level education. Conclusions: Findings from all 11 articles were similar. Where these six themes were ignored, there was often either a failure to reach expected goals or resignation from the role. Future employers must ensure that they establish a comprehensive orientation and education programme to be certain that qualified Advanced Clinical Practitioners are suitably prepared for their role in health care.

Abstract:BackgroundPharmacy education and pharmacy practice are facing remarkable changes following new scientific discoveries, evolving patient needs and the requirements of advanced pharmacy competency for practices. Many countries are introducing or undertaking major transformations in pharmacy education. The Thai pharmacy curriculum has been changed from a 5-year BPharm and a 6-year PharmD to only a 6-year PharmD programme. Curriculum change processes usually involve stakeholders, including both internal and external educational institutions, at all levels. This study aims to understand the experiences and perceptions of stakeholders regarding the transition to an all-PharmD programme in Thailand.MethodsSemi-structured interviews were conducted in Thailand with 130 stakeholders (e.g., policy makers, pharmacy experts, educators, health care providers, patients, students and parents) from August-October 2013. The interviews were audio recorded, transcribed verbatim and analysed using an inductive thematic analysis.ResultsThree main themes were derived from the findings: 1. influences on curriculum change (e.g., the needs of pharmacists to provide better patient care, the US-Thai consortium for the development of pharmacy education); 2. perceived benefits (e.g., improve pharmacy competencies from generalists to specialists, ready to work after graduation, providing a high quality of patient care); and 3. concerns (e.g., the higher costs of study for a longer period of time, the mismatch between the pharmacy graduates’ competency and the job market’s needs, insufficient preceptors and training sites, lack of practical experience of the faculty members and issues related to the separate licenses that are necessary due to the difference in the graduates’ specialties).ConclusionsThis is the first study to highlight the issues surrounding the transition to the 6-year PharmD programme in Thailand, which was initiated due to the need for higher levels of competency among the nation’s pharmacists. The transition was influenced by many factors. Many participants perceived benefits from the new pharmacy curriculum. However, some participants were concerned about this transition. Although most of the respondents accepted the need to go forward to the 6-year PharmD programme, designing an effective curriculum, providing a sufficient number of qualified PharmD preceptors, determining certain competencies of pharmacists in different practices and monitoring the quality of pharmacy education still need to be addressed during this transitional stage of pharmacy education in Thailand.

Abstract:PURPOSE OF THE RESEARCH: This paper reports on the evaluation of the educational benefits to third-year MPharm students attached to four Nottingham inner-city training practices as part of the first cohort of learners to come to a multiprofessional learning organisation (MPLO). The aim was to identify areas of benefit above and beyond those available at their core course in order to identify areas for further study.METHODOLOGY: This study used a mixture of both quantitative and qualitative methods to generate ideas through grounded theory. It used a hermeneutic approach to define multiprofessional learning and professional roles.METHODS: A questionnaire was developed and all students were invited to complete it at the end of their attachments. It was analysed using an SPSS computer package.FINDINGS: The main benefits perceived by pharmacy students were concerning development of professional role, behaviour in the field (legitimate peripheral participation) and emergence of professional identity. From the findings the authors have described an MPLO as 'an educational enterprise between different professions which has an ethos of mutual co-operation and understanding of each other's roles, emphasises communication and idea sharing and exists within a environment of 'smooth systems functioning". • An MPLO can be defined as: 'an educational enterprise between different professions which has an ethos of mutual co-operation and understanding of each other's roles, emphasises communication and idea sharing, and exists within a environment of 'smooth systems functioning'. • Neophyte professionals want to practise interprofessional behaviour. • Professional identity is enhanced when group differences are emphasised and the move to homogenise professional groups through interprofessional working may not be successful. • Educators must help learners challenge professional stereotypes and enable them to develop flexibility to meet future challenges.

Abstract:Background: Dentistry in the UK has a number of new graduate-entry programmes. The aim of the study was to explore the motivation, career expectations and experiences of final year students who chose to pursue a dental career through the graduate entry programme route in one institution; and to explore if, and how, their intended career expectations and aspirations were informed by this choice. Method: In-depth interviews of 14 graduate entry students in their final year of study. Data were transcribed verbatim and analysed using framework analysis. Results: There were three categories of factors influencing students' choice to study dentistry through graduate entry: 'push', 'pull' and 'mediating'. Mediating factors related to students' personal concerns and circumstances, whereas push and pull factors related to features of their previous and future careers and wider social factors. Routes to Graduate Entry study comprised: 'early career changers', 'established career changers' and those pursuing 'routes to specialisation'. These routes also influenced the students' practice of dentistry, as students integrated skills in their dental studies, and encountered new challenges.Factors which students believed would influence their future careers included: vocational training; opportunities for specialisation or developing special interests and policy-related issues, together with wider professional and social concerns.The graduate entry programme was considered 'hard work' but a quick route to a professional career which had much to offer. Students' felt more could have been made of their pre-dental studies and/or experience during the programme. Factors perceived as influencing students' future contribution to dentistry included personal and social influences. Overall there was strong support for the values of the NHS and 'giving back' to the system in their future career. Conclusion: Graduate entry students appear to be motivated to enter dentistry by a range of factors which suit their preferences and circumstances. They generally embrace the programme enthusiastically and seek to serve within healthcare, largely in the public sector. These students, who carry wider responsibilities, bring knowledge, skills and experience to dentistry which could be harnessed further during the programme. The findings suggest that graduate entry students, facilitated by varied career options, will contribute to an engaged workforce.

Abstract:Abstract Background General Practice training in Australia is delivered through the apprenticeship model. General Practice supervisors support trainees transitioning from hospital-based work towards competent independent community-based practice. The timing and manner in which support should be provided is still not well understood. This study aimed to establish the variation in clinical and educational experiences and behaviours, and location, of general practice trainees’ consultations by stage of their vocational training. It was hypothesised that change is greater in earlier stages of training. Methods A cross-sectional analysis of data (2010–2018) from the Registrar Clinical Encounters in Training (ReCEnT) study, an ongoing cohort study of Australian GP registrars’ in-consultation clinical and educational experience and behaviours. Multinomial logistic regression assessed the association of demographic, educational, and clinical factors in different stages of training. The outcome factor was the training term. Results Two thousand four hundred sixteen registrars contributed data for 321,414 patient consultations. For several important variables (seeing patients with chronic disease; new patients; seeking in-consultation information or assistance; ordering pathology and imaging; and working in a small or regional practice), odds ratios were considerably greater for comparisons of Term 1 and 3, relative to comparisons of Term 2 and 3. Conclusion Differences experienced in demographic, clinical and educational factors are significantly more pronounced earlier in registrars’ training. This finding has educational and training implications with respect to resource allocation, trainee supervision and curriculum design. Sociocultural learning theory enables an understanding of the impact of transitions on, and how to support, general practice trainees and supervisors.

A qualitative study exploring experiences of racism, inclusivity and diversity on career progression in pharmacy practice and education

Abstract:Abstract Introduction Approximately 40% of pharmacists in Great Britain identify as part of minority ethnic group, however research has identified that these pharmacists are under-represented in senior management positions.1 Additionally, there is a well documented attainment differential in pharmacy education, suggesting disparity that extends beyond practice, to initial training and education.2 Further work is needed to explore career progression, racism, inclusivity, and diversity in pharmacy practice and education. Aim To explore experiences of racism, inclusivity, and diversity on career progression in pharmacy practice and education. Methods Six online focus groups lasting one hour were competed on Microsoft Teams with a convenience sample of 29 pharmacists, trainees and students recruited via email and social media. Automatic verbatim transcripts were quality checked then thematically analysed using NVivo Version 1.2 and a step-wise approach of familiarization, coding, and clustering to identify key themes that described participants’ experiences. Ethical approval was obtained by an institutional ethics committee. Results The main themes were Defining racism; Experiencing racism; Race talk; and Career progression. Participants defined racism as a combination of ignorance and malice, manifested in derogatory language, physical violence and judgement based on skin colour, accent, hair type, and clothing. Racism was experienced both overtly and subtly, by senior colleagues, contemporaries, patients, and staff members from other disciplines. Two white participants reported never experiencing racism. Engagement with race-related training or practice issues, such as addressing racist encounters with colleagues or patients, was limited by senior managers, who were perceived to prioritise economic interests (not losing customers) and protecting organisational branding. Talking about race (Race talk) was mostly reported as uncomfortable with managers from white backgrounds, as this led to misunderstanding and confusion, misrepresentation as “playing the race card” and minimisation. Equally, white participants reported feeling very uncomfortable and being afraid to talk about race. This meant challenging racist behaviours, attitudes or systems was difficult for all participants and had to pass a threshold to be ‘worth’ bringing up. This threshold was lower if racism was observed directed at somebody else, rather than at participants personally. Career progression was reported as limited due to racial biases by recruiting managers; with participants reporting “there is a certain point where pharmacists of colour can get to”. White participants reported no perceivable impact of their race on career progression. Discussion/Conclusion The aim of the study was to explore racism, inclusivity, and diversity on career progression in pharmacy practice and education. The results demonstrate racism in pharmacy practice and education which cuts across all levels of workforce education, training, and development, creating a perceived ceiling effect. A strength of the study is that participants were recruited from across social and ethnic groups in Great Britain. Although qualitative data are not generalisable, they may be transferable to other settings. Quantitative work is needed to assess the scale of these experiences across the profession. References 1. Abstract:Healthcare graduates are often characterised as ill-prepared for workplace entry. Historically, research on health professional's work preparedness has focused on the quality of graduates' clinical knowledge, skills and problem-solving. This ignores the role of professional identity formation in determining readiness for clinical practice. Yet, professional identity defines graduate self-perception, how others perceive them and informs clinical behaviour. The scholarship of identity formation at the transition from undergraduate to graduate is characterised by individual (cognitive) rather than relational (sociocultural) perspectives. Yet there is growing recognition that identity formation is not just individually mediated, but is also constructed between individuals and social context. The aim of this study was to explore professional identity formation among undergraduates and graduates from one healthcare profession (speech and language therapy-SLT) using a sociocultural theoretical standpoint. A qualitative descriptive methodology was used. Final (4th) year SLT undergraduate students and graduate SLTs with less than 2 years' clinical experience participated in individual semi-structured interviews. Thematic analysis was used to describe patterns in the data, which were subsequently subjected to interpretation informed by the constructs of Figured Worlds. Data analysis revealed that undergraduate professional identity was characterised by dependency, self-centredness (as opposed to patient-centredness), and a naïve role concept. Graduate identity on the other hand included expectations of self-sufficiency, patient-centredness and a more nuanced perception of the professional role. Undergraduates have naïve, prototypical understandings of what it is to be a graduate practitioner. The nature of undergraduate clinical placement hinders meaningful identity development. This suggests that curriculums should facilitate undergraduates to act with meaningful autonomy and to be positioned in more patient-centred roles, e.g. involvement in the decision-making process for patients. Graduates may then feel more authentic as autonomous professionals in their early graduate posts. This leads to better graduate, patient and service outcomes.

Abstract:Context: The challenging nature of the transition from medical student to doctor is highlighted by the associated negative consequences to new doctors' mental health and wellbeing. Enhanced understanding of the lived experience of recent medical graduates as they move through the stages of transition over the first year of practice can inform interventions to ease the difficulties encountered. Methods: Using interpretative phenomenological analysis (IPA), a novel approach to this topic, we explored the lived experience of transition from student to doctor over the first year of practice after graduation. Twelve new graduates were purposively recruited. We conducted semi-structured interviews at the end of their first year of practice with respect to their experience over the first year. Results: The experience of transition was characterised by overlapping temporal stages. Participants' initial adjustment period was characterised by shock, coping and stabilisation. A phase of development followed, with growth in confidence and a focus on self-care. Adversity was experienced in the form of interprofessional tensions, overwork, isolation and mistreatment. Finally, a period of reflection and rationalisation marked the end of the first year. Discussion: Following initial anxiety regarding competence and performance, participants' experience of transition was predominantly influenced by cultural, relational and contextual aspects of clinical practice. Solutions to ease this challenging time include stage-specific transitional interventions, curricular change at both undergraduate and postgraduate levels and a re-evaluation of the clinical learning environment to mitigate the difficulties endured.
