Abstract:Background: Gastric cancer (GC) is one of the most common cancers in Iran. Knowledge of the epidemiology of the disease is essential in planning for prevention. So this study aimed to investigate the epidemiological aspects of gastric cancer including prevalence, incidence, mortality, and risk factors of Iran. Methods: This systematic review study was based on articles published in both English and Persian languages during the years of 1970-2020 in international databases (PubMed, Web of Science, Scopus) and national databases (including SID, Magiran, and IranDoc). Papers related to epidemiological aspects of the disease including mortality, prevalence, incidence, and risk entered the final review. Results: According to the studies, the minimum and maximum prevalence of gastric cancer in northwestern Iran (Ardabil) is between 0.2 and 100 per 100,000. Also, the death rate per 100,000 people ranged from 10.6 to 15.72 and the ASMR ranged from 4.2 to 32.2%. On the other hand, the incidence of GC was higher in men than in women (74.9 vs 4.6%). The GC risk ratio was 8-times higher in the elderly than in the other age groups (HR=8.0, 2.7-23.5). The incidence of gastric cancer in patients with H. pylori infection was 18-times and that of smokers 2-times higher than other populations. Low level of economic situation and food insecurity increased the odds of GC by 2.42- and 2.57-times, respectively. It should be noted that there was a direct relationship between consumption of processed red meat, dairy products, fruit juice, smoked and salty fish and legumes, strong and hot tea, and consumption of salt and gastric cancer incidence. There was also an inverse relationship between citrus consumption, fresh fruit, garlic, and gastric cancer. In addition, the mRNA genes are the most GC-related genes. Conclusion: Given the high incidence of GC in Iran, changing lifestyle and decreasing consumption of preservatives in food, increasing consumption of fruits and vegetables, and improving the lifestyle can be effective in reducing the incidence of this disease.

Abstract:Gastric cancer (GC) is the fifth most prevalent cancer and the fourth in terms of cancer-related death worldwide. The GC is the most commonly diagnosed and the most important cause of cancer-related death in Iranians. Due to the significant burden of stomach cancer in Iran, we aimed to review all articles that have been conducted on the Incidence, Prognostic Factors, and survival of Gastric Cancer in Iran and compare them with the global situation. We surveyed Google Scholar, Pub Med, Scopus, Web of Science (ISI), and Science Direct databases using the keywords of Gastric Cancer, Stomach Cancer, Gastric Neoplasm, Stomach Neoplasm, and Iran and obtained English language articles. After a two-step screening of articles, 51 out of 202 remained eligible for our study (related to Incidence, Prognostic Factors, and survival of stomach Cancer in Iran). We found that although Iran is one of the high-burden countries for GC, the incidence rate and mortality rate are very different in different provinces (up to a six-fold and thirty-fold difference in mortality rate and incidence rate have been reported respectively in reviewed articles between Northern and Southern Iran). Considering that the initial stage is one of the most important and probably the most important prognostic factors, through upper endoscopy screening, the disease can be diagnosed in earlier stages and the survival of patients can be improved. We recommend the Ministry of Health identify high-risk people according to their combination of risk factors (especially geographical region) and make a national cost-effective screening algorithm for Iran.

Abstract:To determine the role of the epidermal growth factor (EGF) in the regenerative stimulation of intestinal epithelium and hepatocyte proliferation after partial resection of these organs the labeling index of the intestine and liver in sialadenectomized rats was studied. EGF concentration in the saliva and serum was determined using radioimmunoassay. The decrease in EGF concentration after the removal of submandibular salivary glands was shown to slower hepatocytes entering the mitotic cycle and to inhibit the activity of enterocyte proliferation in the small intestine. The data show pronounced in vivo mitogenic effect of EGF on the liver and intestinal epithelium.
Dietary habits and gastric cancer risk in north-west Iran

Abstract:ObjectivesNorth-west Iran is a high-risk area for gastric cancer (GC). Dietary practices may increase risk of GC. For the first time, the diet–GC association in this area was assessed using a validated food frequency questionnaire.MethodsCases and controls were recruited in a population-based study. In addition to collecting dietary data using a food frequency questionnaire, Helicobacter pylori antibody level was measured. Multiple logistic regression models were used to estimate odds ratios for associations between dietary factors and GC among 286 cases and 304 controls.ResultsA positive association was estimated for total fat intake (OR = 1.33/20 g, 95% CI: 1.12–1.57) and risk of GC. Inverse associations were observed for vitamin C, iron, and zinc intake and risk of GC and its subgroups (cardia, non-cardia). Fruits and vegetables consumption and refrigerator use showed inverse associations (OR = 0.72/100 g, 95% CI: 0.65–0.80 and OR = 0.75/10 years, 95% CI: 0.60–0.95, respectively). Positive association was observed among those who preferred fried food (OR = 2.21, 95% CI: 1.45–3.37) or consumed highly salted/roasted seeds (OR = 1.97, 95% CI: 1.13–3.43).ConclusionGC in north-west Iran is associated with dietary practices: foods, nutrients and food preparation habits.

Now, GC therapy is multidisciplinary, and multiple option strategies are well-known; therefore, the present study reviewed the new insight into chemotherapy agents and various alternative and complementary strategies such as neoadjuvant &Adjuvant therapy, nanotherapy, natural medicines, etc. which are suggested for GC treatment. Moreover, we reviewed the current surgery techniques such as endoscopic resection, laparoscopic resection. We also summarized new insights into pathophysiology, epidemiology, risk factors, diagnosis, prevention and screening approaches in the GC.

Abstract:Gastric cancer (GC) is a global health problem, with more than 1 million people newly diagnosed with GC worldwide each year. GC is more prevalent in less developed countries than in more developed countries. About half of all GC cases worldwide occur in East Asia, notably China. Globally, overall incidence rates of GC are declining, which is potentially attributed to a decrease in Helicobacter pylori (H. pylori) infection and the use of refrigeration to preserve foods rather than salt. GC is a multifactorial disease, and its occurrence and development were impacted by environmental and genetic factors. H. pylori infection is the primary risk factor for GC, especially for non-cardia. The prognosis of GC is poor due to stages at the first diagnosis. The 5-year survival rate is less than 10% when patients are diagnosed at an advanced stage, but the rate is as high as 85% if patients are detected at an earlier stage. Endoscopic screening can potentially prevent GC by early diagnosis and early treatment and has been widely adopted in screening programs in East Asian countries, such as Japan and Korea. This review summarizes updated epidemiological aspects, risk factors, and prevention strategies of GC in recent years to help researchers determine the most effective intervention strategies for reducing risk of GC.

Abstract:Th cells within the granulomas of murine schistosomiasis mansoni produce IL-5, which is essential for granuloma eosinophil growth and development. The mechanisms regulating granuloma IL-5 production are unknown. The granulomas also make IL-2 in small quantities. rIL-2 therapy stimulates eosinophilia and IL-5 synthesis. Therefore, we studied the effect of IL-2 on IL-5 production within the liver granulomas of murine Schistosoma mansoni. Dispersed granuloma cells and intact granulomas cultured in vitro released IL-5. Adding anti-IL-2 or anti-IL-2R to the cultures, to block IL-2 activity, significantly inhibited IL-5 production. However, supplementing the cultures with small quantities of rIL-2 markedly stimulated IL-5 release in a dose-dependent fashion. Blocking anti-IL-4 mAb had no effect. Also, granuloma T cells were isolated by FACS. These highly purified T cells produced IL-5 both in the presence and absence of plate-bound anti-CD3. Once again, the IL-5 production was dependent on IL-2. The requirement of IL-2 for normal IL-5 production was not dependent on an IL-2-induced expansion of the IL-5-producing, T lymphocyte population. Thus, IL-2 mediates T cell interactions within the granuloma that regulate granuloma IL-5 secretion.
Trends in dental caries in 12- and 13-year-old schoolchildren from Florianópolis between 1971 and 2009

Abstract:Aim: To estimate the prevalence and severity of dental caries in schoolchildren from the city of Florianopolis, SC, Brazil, in 2009 and to compare these results with data from previous studies carried out in the same institution since 1971. Methods: All 145 12- and 13-year-old schoolchildren enrolled in a public school were eligible for the study. Dental caries was assessed with the DMFS/ DMFT indexes, the significant caries index (SiC) and their 95% confidence intervals (CI 95%). Results: The response rate was 68.2%. The DMFT ranged from 9.2 in 1971 to 0.78 in 2009, although different diagnostic criteria were adopted to estimate these counts. The prevalence of dental caries declined from 98.0% (CI 95% 96.0-100.0) in 1971 to 43.5% (CI 95% 33.7-53.4) in 2009. There was a reduction of 47% in the mean SiC index between 2002 and 2009, i.e. from 3.4 (CI 95% 3.0-3.8) to 1.8 (CI 95% 1.5-2.1). The mean DMFS index was 2.0, and it was composed of an average of 1.8 and 0.2 decayed and filled surfaces, respectively. Conclusions: Even though different diagnostic criteria were adopted during the study period, an effective decline in the prevalence and severity of dental caries in adolescents was observed throughout 38 years of monitoring.
Rectal cancer survival and prognostic factors in Iranian population: A retrospective cohort study

Abstract:Background: Rectal cancer (RC) poses a significant global health challenge, causing substantial morbidity and mortality. This study aims to investigate the survival rates of RC patients and identify the factors that influence their survival. The study considers demographic characteristics, tumor features, and treatment received as the factors under consideration. Methods: A retrospective analysis was conducted on the medical records of 593 RC patients. Data were collected through a comprehensive review of medical records and conducting telephone interviews. Survival rates were estimated using the life table method, and subgroup comparisons were performed using the log-rank test. Cox regression analysis was utilized to assess the independent associations between RC survival time and various covariates. Results: The study cohort comprised 593 RC patients, with a predominantly male representation. The mean age at diagnosis was 58.18 years, and the majority of patients (78.6 %) underwent surgical interventions. The median age at symptom onset and diagnosis were 58 and 59 years, respectively. Survival rates at 1st, 3rd, 5th, and 10th years were estimated to be 85 %, 59 %, 47 %, and 36 %, respectively. Statistical analysis revealed several significant prognostic factors, including age, education, symptoms, and cancer stage. In the multivariate Cox proportional-hazards analysis, advanced regional stage (HR = 1.54, 95 % CI, 1.13-2.08), presence of metastasis (HR = 3.73, 95 % CI, 2.49-5.58), and age over 70 (HR = 1.65) were associated with a higher risk of mortality. Conclusion: Given the alarming prognosis of RC observed in the study area and the significant delay between symptom onset and diagnosis, it is crucial to address this issue and potentially improve the survival rates of RC patients.

Abstract:Background: Gynecologic cancers (GCs) are among the leading causes of morbidity and mortality in females worldwide. Estimating the cancer burden is invaluable to set up priorities for research funding allocations, cancer control policies, and prevention strategies. The International Agency for Research on Cancer (IARC) has recently released the latest estimates on the prevalence, incidence, and mortality for 36 types of cancer and all cancer sites combined in 185 countries in 2020. We aimed to report GCs estimates in the I.R. Iran from 2012 to 2040 based on the GLOBOCAN 2020. Methods: We obtained data on the incidence, mortality, and prevalence of GCs in the Iranian female population from the GLOBOCAN 2020 database presented by the IARC, compared the burden with the previous reports presented in 2012 and 2018, and provided the estimates for 2040. In addition, we compared the burden to that of the WHO Eastern Mediterranean Region (EMRO) and the world. Results: The top 3 incident GCs were ovarian, uterine, and cervical cancer, respectively. In 2020, approximately 2,000 new cases of ovarian cancer (ASR: 4.4), 1,535 new patients with uterine cancer (ASR: 3.5), and 1,056 incident cervical cancer cases (ASR: 2.3) were diagnosed in the I.R. Iran. With 1,269 cancer deaths and an age-standardized mortality rate (ASMR) of 3.0, ovarian cancer is considered to be the leading cause of death from GCs, followed by cervical cancer (644 deaths; ASMR: 1.5) and uterine cancer (537 deaths; ASMR: 1.3). Among 157,930 prevalent female cases reported in the GLOBOCAN 2020 database, 13,663 cases were categorized as GC. With a 5-year prevalence of 5,539, ovarian cancer was the most prevalent type, while uterine cancer (with 4,904 prevalent cases) and cervical cancer (with 2,948 prevalent cases) were less prevalent types. Conclusion: There has been a slight increase in the incidence of GCs in recent years after stable rates for a couple of decades. Therefore, primary and secondary prevention measures such as lifestyle modifications and screening programs must be prioritized.
Burden of upper gastrointestinal cancers in the east of Golestan province (Golestan cohort study)

Abstract:Background: Cancers, especially Upper Gastrointestinal Cancers (UGCs), pose a substantial burden on society, particularly in developing nations. Golestan province, Iran, is known for its high UGC rates globally. Aims: This study delves into the disease burden of UGCs in the eastern part of Golestan province. Methods and results: This study was conducted using the results of the Golestan cohort study. 2711 patients participating in this cohort, who visited Atrak Clinic during 2001-2020, participated in this study. After excluding patients with incomplete records, 2481 patients were included in the study. To compute the metrics of years of life lost (YLL), years of life lived with disability (YLD), and disability-adjusted life years (DALY), we utilized the World Health Organization's standard life table, stratified by age and gender. The majority of UGC patients in our study were married (81.8%), had limited formal education (82.6%), and were predominantly male (61.1%). A substantial proportion resided in suburban areas (85.8%), and over half of the patients (52%) reported a history of drug addiction. The mean age at diagnosis for men was 65.76 years with a standard deviation of 11.34, while for women, it was 64.38 years with a standard deviation of 11.66. Regarding disease impact, YLL, YLD, and DALY for men were 21 240, 1956, and 23 196 (307.8 per 100 000), respectively. For women, these figures were 15 609 for YLL, 1367 for YLD, and 16 976 (223.1 per 100 000) for DALY. Conclusion: After the increasing trend of the burden of UGCs in Golestan province in the early years of the study, this rate has been decreasing in recent years. Effective strategies necessitate collaborative efforts across various sectors to alleviate this burden, focusing on preventive measures, timely diagnosis, and well-coordinated therapeutic interventions.

Abstract:Gastric cancer continues to be an important healthcare problem from a global perspective. Most of the cases in the Western world are diagnosed at late stages when the treatment is largely ineffective. Helicobacter pylori (H. pylori) infection is a well-established carcinogen for gastric cancer. While lifestyle factors are important, the efficacy of interventions in their modification, as in the use of antioxidant supplements, is unconvincing. No organized screening programs can be found outside Asia (Japan and South Korea). Although several screening approaches have been proposed, including indirect atrophy detection by measuring pepsinogen in the circulation, none of them have so far been implemented, and more study data is required to justify any implementation. Mass eradication of H. pylori in high-risk areas tends to be cost-effective, but its adverse effects and resistance remain a concern. Searches for new screening biomarkers, including microRNA and cancer-autoantibody panels, as well as detection of volatile organic compounds in the breath, are in progress. Endoscopy with a proper biopsy follow-up remains the standard for early detection of cancer and related premalignant lesions. At the same time, new advanced high-resolution endoscopic technologies are showing promising results with respect to diagnosing mucosal lesions visually and targeting each biopsy. New histological risk stratifications (classifications), including OLGA and OLGIM, have recently been developed. This review addresses the current means for gastric cancer primary and secondary prevention, the available and emerging methods for screening, and new developments in endoscopic detection of early lesions of the stomach.

A 12-year trend analysis of the incidence of gastrointestinal cancers in East Azerbaijan: last updated results of an ongoing population-based cancer registry

Abstract:Background: The most recent results of Global Cancer Statistics indicated that gastrointestinal cancers, including gastric, colorectal, esophageal, and liver cancers, are among the most commonly diagnosed cancers worldwide. Previous reports from cancer registries in East Azerbaijan have shown that there is a high incidence of gastrointestinal cancer in this region, so we performed a trend analysis to determine the pattern of change over the last decade. Methods: In total, 12 years of cancer registry data were collected from different sources in East Azerbaijan, and a data quality check was performed to ensure clean data. Using the 2000 World Health Organization standard population, we then generated age-standardized incidence rates (ASRs) for different cancers, and for each year from 1383 to 1394 of the Persian calendar (i.e., 19 March 2004 to 20 March 2015). Annual percent changes (APCs) and Average annual percent changes (AAPCs) in the ASRs for esophageal, gastric, small intestine, colorectal, anal, liver, gallbladder, and pancreatic cancers were calculated using Joinpoint Software (Version 4.5.0.1, June 2017). Results: An increase in most types of cancer was observed during the study period. The ASR for colorectal cancer increased from 2.9 to 13.6 per 100,000 women (APC, 9.7%) and from 2.2 to 17.8 per 100,000 men (APC, 10.2%). The ASR for gastric cancer showed a slight increasing trend from 10.5 to 13.5 per 100,000 women (APC, 1.3%) and from 3.1 to 29.9 per 100,000 men (APC, 3.2%). However, trend analysis showed a decreasing pattern for the ASR of esophageal cancer in both genders (APC,- 3%), with APCs of - 1.1% in females and - 0.4% in males. Conclusions: The latest results of the East Azerbaijan Population-Based Cancer Registry indicate that gastrointestinal cancers remain common, with significant increasing trends in their ASRs. Improved screening and early detection are needed in this region.

Majority of patients refer for the treatment in advanced stages of tumor progression.</p><h3 class="Heading">Main body</h3><p class="Para">It is required to define an efficient local panel of diagnostic and prognostic markers for the Iranians. Indeed such efficient specific panel of markers will pave the way to decrease the mortality rate and increase the 5 years survival among the Iranian patients via the early diagnosis and targeted therapy.</p><h3 class="Heading">Conclusion</h3><p class="Para">in present review we have reported all of the molecular markers in different signaling pathways and cellular processes which have been assessed among the Iranian esophageal cancer patients until now.</p>

Abstract:Background: Recent evidence suggests overall diet quality, as assessed by dietary scores, may play a role in the development of upper gastrointestinal (UGI) cancers. However, the existing dietary scores are derived from high-income countries with different dietary habits than regions with the highest burden of UGI cancers, where limited data is available. This study aimed to investigate the association between overall diet quality and risk of esophageal and stomach cancers in a high-risk region for UGI cancers. Methods: We recruited 50045 individuals aged 40-75 between 2004-2008 from northeastern Iran and followed them annually through July 2020. Data on demographics, diet, and various exposures were collected using validated questionnaires. Diet quality was assessed by calculating the Healthy Eating Index (HEI), Alternative Healthy Eating Index (AHEI), Alternative Mediterranean Diet (AMED), Dietary Approaches to Stop Hypertension (DASH), and World Cancer Research Fund-American Institute for Cancer Research (WCRF-AICR) scores. Results: During an average 12 years of follow-up, 359 participants developed esophageal cancer and 358 developed stomach cancer. After adjustments, each standard deviation increase in baseline dietary scores was associated with up to 12% reduction in esophageal cancer risk and up to 17% reduction in stomach cancer risk. Esophageal cancer showed stronger inverse associations with adherence to AMED (HRQ4-vs-Q1=0.69 (0.49-0.98), P-trend=0.038). Stomach cancer showed stronger inverse correlation with WCRF-AICR (HRQ4-vs-Q1=0.58 (0.41-0.83), P-trend=0.004), and DASH (HRC4-vs-C1=0.72 (0.54-0.96), P-trend=0.041). These associations were comparable across different population subgroups. We did not observe significant associations between HEI and AHEI scores and UGI cancers in this population. Conclusion: Despite the differences in consuming individual food groups, adherence to the available dietary recommendations (derived from high-income countries) was associated with lower risk for subsequent esophageal and gastric cancers in this high-risk population. Educating the public to have a healthy eating pattern might be an effective strategy towards prevention of UGI cancers in high-risk regions.

Abstract:Gastric cancer is a major cause of cancer-related death worldwide, despite the reduction in its incidence. The disease is still burdened with a poor prognosis, particularly in Western countries. The main risk factor is the infection by Helicobacter pylori, classified as a class I carcinogen by the IARC, and It is well-known that primary prevention of gastric cancer can be achieved with the eradication of the infection. Moreover, non-invasive measurement of pepsinogens (PGI and PGI/PGII ratio) allows the identification of patients that should undergo upper gastrointestinal (GI) endoscopy. Gastric non-cardia adenocarcinoma is indeed preceded by a well-defined precancerous process that involves consecutive stages, described for the first time by Correa et al. more than 40 years ago, and patients with advance stages of gastric atrophy/intestinal metaplasia and with dysplastic changes should be followed-up periodically with upper GI endoscopies. Despite these effective screening and surveillance methods, national-level screening campaigns have been adopted only in few countries in eastern Asia (Japan and South Korea). In this review, we describe primary and secondary preventive measures for gastric cancer, discussing the need to introduce screening also in Western countries. Moreover, we propose a simple algorithm for screening that could be easily applied in clinical practice.
