Abstract:Background: Changing societal views and the increasing prevalence of online education has created an environment ideal for the evolution and change of the medical curriculum. One area in need of improvement is LGBTQ+ health care teaching. Current literature demonstrates that LGBTQ+ individuals have higher levels of poor mental and physical health than heterosexual, cis-gender individuals (6). Therefore, it is key that the medical curriculum is made more inclusive of the LGBTQ+ population to ensure future doctors can provide inclusive care. This review aimed to examine the current literature on LGBTQ+ health care teaching in UK medical curricula, identify potential barriers to change, and explore efforts to improve the LGBTQ+ health care teaching in the UK. Methods: Literature searches were carried out using the PRISMA framework. The databases used were PubMed, Ovid, Embase, AMED, Global Health and Scopus. The searches were carried out in July 2021. Results: 15 relevant papers were reviewed. Three main themes were identified, 1) medical students (or medical schools) believed current education on LGBTQ+ health care was insufficient, 2) students reported wanting more in-depth practical education on LGBTQ+ health, 3) the potential impact of a lack of LGBTQ+ health care education on clinicians’ confidence and ability to treat LGBTQ+ patients. Discussion: LGBTQ+ health care is lacking in the UK medical curriculum which has the potential to negatively impact patients. The teaching that is present is firstly inconsistent which leads to disparities and unreliability for LGBTQ+ patients. It is also heavily focused on sexual health which can lead to damaging stereotypes. Despite there being barriers to improving LGBTQ+ health care education, we present practical suggestions to overcome these. Further research should explore in greater depth the level of knowledge of medical students on LGBTQ+ health care, and their perceptions of curriculum improvements, in order to establish a basis for future curriculum change.
Online sexual, reproductive, and transgender healthcare for LGBTQI+ youth: A scoping review

Abstract:Background. Lesbian, gay, bisexual, trans, queer, questioning, intersex, and other sexual and gender minority (LGBTQI+) youth have poor sexual and reproductive health outcomes and low uptake of sexual and reproductive healthcare (SRHC). Online SRHC and transgender healthcare could overcome known barriers to in-person SRHC, such as confidentiality concerns. Therefore, we aimed to describe existing literature on online SRHC and transgender healthcare for LGBTQI+ youth, synthesise study findings, and make recommendations for future research. Methods. We conducted a scoping review following the Joanna-Briggs Institute methodology. Eligibility were online SRHC and transgender healthcare for LGBTQI+ youth (aged 10-35 years) in high-income countries. Search strings were framed around the eligibility criteria and 265 search terms were selected to identify published literature from nine databases. Searches were exported to Rayyan and studies screened by two reviewers. Data from included studies were extracted to Excel and analyzed descriptively. Results: Of 91 included papers, 41 were quantitative, 26 were qualitative, and 24 were mixed methods. Seventy-one papers focused on sexual health (HIV/sexually transmitted infection (STI) prevention 52/71; HIV management 9/71; sexual health per se 9/71; and HIV stigma reduction 2/71); 3/91 on reproductive health (pregnancy prevention 2/3); 2/91 sexual and reproductive health; and 16/91 on transgender health (gender identity/transition per se 8/16; gender affirming care 8/16). Papers explored the provision of or engagement with education/information (72/91); non-clinical support (56/91, e.g., reminders for HIV/STI testing); and clinical care (18/91) for sexual health (10/18, e.g., home HIV/STI self-sampling kits 6/10) or transgender health (8/18, i.e., eConsultation with a healthcare provider 8/8). Studies targeted young men who have sex with men (62/91) for sexual health; trans and gender diverse youth (26/91) for transgender healthcare (16/26) and sexual health (14/26); LGBTQI+ youth (6/91); and young sexual minority women (4/91) for reproductive health (3/4) and sexual and reproductive health (1/4). Conclusions: There is a large and varied literature base for online SRHC and transgender healthcare for LGBTQI+ youth. However, most research focused on sexual healthcare, particularly HIV/STI prevention, for men who have sex with men. Very little explored reproduction or sexual health other than HIV prevention. Young sexual minority women and trans and gender diverse youth are notably under-researched for online SRHC. Research is needed to understand how to enhance the potential of online healthcare for LGBTQI+ youth.

LGBTQ+ health education interventions for nursing students: A systematic review

Abstract:Objectives: Nurses are the largest segment of the American healthcare workforce, yet little is known about how American nursing students are educated about lesbian, gay, bisexual, transgender, queer, and other individuals' (LGBTQ+) health and the efficacy of these trainings. This systematic review assessed articles in English that quantitatively evaluated the effects of curricula and training for nursing students regarding LGBTQ+ health and examined their effects on students' outcomes. Design: Systematic review. Data sources: Six electronic databases (PubMed, CINAHL, British Nursing Index, Embase, Scopus and PsycINFO). Review methods: Controlled vocabulary and/or keywords were used for each database. Of the initial 1766 citations, 26 articles met inclusion criteria. Data extracted included study design, sample characteristic/size, training topic, modality, duration, trainer, measurement tool, training target, effect size, and key finding. Results: Nearly 80 % of the reviewed studies were published between 2018 and 2021. All studies included were quasi-experimental: pretest-posttest with control (n = 3), without control (n = 20), and posttest only without control (n = 3). Training modalities were categorized into four groups: multimodal pedagogy with (n = 7) and without simulation (n = 6), single modality with simulation (n = 6) and with didactic lectures (n = 7). Eight studies employed LGBTQ+ people as trainers. Eight studies included interprofessional students; 18 studies engaged only nursing students. Interventions improved students' knowledge about LGBTQ+ health (n = 8), attitudes toward LGBTQ+ communities (n = 8), comfort level regarding interacting with LGBTQ+ individuals (n = 7), confidence in providing LGBTQ+ inclusive care in practice (n = 13) and LGBTQ+ cultural competence in nursing care (n = 2). Conclusions: Educating and qualifying nursing students about LGBTQ+ health may improve their knowledge, attitudes, and skills when interacting with LGBTQ+ patients and may reduce health disparities for LGBTQ+ clients. Broad integration of content and competencies in LGBTQ+ health and well-being in nursing curricula should be prioritized. Future research should include rigorous, longitudinally designed evaluation of training curricula focusing on students' actual behavior changes. Evaluation should include comparison groups and validated measurement instruments about LGBTQ+ health education constructs specifically for nursing students.

Abstract:Introduction Young people (16–24-year-olds) experience the highest diagnosis rates of STIs. Lack of engagement, public awareness, lack of training, and long-standing, widespread stigma around STIs remain barriers to greater and more effective use of interventions such as sexual health education and other services. Whilst relationship and sexual health education (RSE) has been part of many school curriculums for generations, young people's experiences of RSE can vary widely. Objective This research aims to provide insight into the experiences of sexual health education, sexual health behaviours and recommendations to improve engagement and sexual health outcomes amongst adolescents. Methods An online quantitative survey was conducted between June and July 2021 (n=5,457). 855 respondents were recruited from a market research panel (answering a 20-minute survey) and a further 4,602 respondents were recruited via public links (answering a cut-down survey). All respondents were living in the UK and aged 18+. Data were analysed using descriptive statistics and adjusted to reflect the sample size of LGBTQ+ and heterosexual respondents. Results 90% of respondents received formal sex education, however, only 12% agreed sex education had a positive long-lasting effect on their sexual life. Reproductive biology, pregnancy, safe sex, and avoiding STIs were the core pillars of sex education, whilst different sexualities, pleasurable intimate relations, gender identity and trans-specific topics were covered less than 13% of the time. Although 90% of respondents declared they have a good understanding of sexual health risks, sexual health outcomes in this group were sub-optimal; safe sex was only consistently practiced by 43% of respondents and regular testing only carried out by 25% of respondents. Conclusions Whilst most participants reported receiving sexual health education, their experiences varied. Despite a high self-reported understanding of sexual health risks, sexual health outcomes were sub-optimal. This research highlights the potential beneficial effects of an exhaustive sexual health education on attitude, knowledge, and behaviours among adolescents to improve the long-term sexual health outcomes. Recommendations include designing programmes based on young people's experiences of sexual education, reviewing current guidance and practices adding on key topics outside the main pillars of sexual education, increasing guidance and training for educational institutions and supporting RSE provisions in alliance with external educators and services. Perspectives on Sexual and Reproductive Health

Abstract:Background Transgender and gender diverse youth experience multiple disproportionate adverse sexual health outcomes. Sexual health education teaches knowledge, attitudes, and skills for promoting sexual health, including reducing risk for sexually transmitted infection, HIV acquisition, and unintended pregnancy. Provision of sexual health education may be protective, but research remains scarce. Methods We conducted a multi‐stage thematic analysis of 33 in‐depth interviews among transgender and gender diverse youth (ages 15–24) living in the southeastern United States on their sexual health education experiences. Results Our study participants described school‐based sexual health education as unhelpful due to a lack of relevant information, inadequately prepared teachers, and a perceived negative tone toward sexuality. They reported relying on online sources of sexual health information, finding relevant content and community despite some limitations. Participants desired content and pedagogy that expands beyond binary and white‐centric presentations of sexuality and gender and sought resources that provide relevant, accurate, and judgment‐free information while holding positive framing around sexuality and gender. Conclusion There is much work needed to improve the breadth, quality, and relevance of school‐based sexual health education. Sexual health education can improve by strengthening critical media literacy skills of youth; raising staff cultural competency on gender, race, and sexual identity through training and supports; using culturally relevant and inclusive curricula; and partnering with community‐based organizations. Transgender and gender diverse youth would benefit from sexual health education from multiple sources which is queer‐friendly, affirms their existence, and provides information on gender, race, and sexuality in positive and expansive ways.

Factors influencing the integration of comprehensive sexuality education into educational systems in low- and middle-income countries: a systematic review

Abstract:Background: Comprehensive sexuality education (CSE) plays a critical role in promoting youth and adolescent's sexual and reproductive health and wellbeing. However, little is known about the enablers and barriers affecting the integration of CSE into educational programmes. The aim of this review is to explore positive and negative factors influencing the integration of CSE into national curricula and educational systems in low- and middle-income countries. Methods: We conducted a systematic literature review (January 2010 to August 2022). The results accord with the Preferred Reporting Items for Systematic Reviews and Meta-analysis standards for systematic reviews. Data were retrieved from the PubMed, Cochrane, Google Scholar, and Web of Hinari databases. The search yielded 442 publications, of which 34 met the inclusion criteria for full-text screening. The review is guided by an established conceptual framework that incorporates the integration of health innovations into health systems. Data were analysed using a thematic synthesis approach. Results: The magnitude of the problem is evidenced by sexual and reproductive health challenges such as high teenage pregnancies, early marriages, and sexually transmitted infections. Awareness of these challenges can facilitate the development of interventions and the implementation and integration of CSE. Reported aspects of the interventions include core CSE content, delivery methods, training materials and resources, and various teacher-training factors. Reasons for adoption include perceived benefits of CSE, experiences and characteristics of both teachers and learners, and religious, social and cultural factors. Broad system characteristics include strengthening links between schools and health facilities, school and community-based collaboration, coordination of CSE implementation, and the monitoring and evaluation of CSE. Ultimately, the availability of resources, national policies and laws, international agendas, and political commitment will impact upon the extent and level of integration. Conclusion: Social, economic, cultural, political, legal, and financial contextual factors influence the implementation and integration of CSE into national curricula and educational systems. Stakeholder collaboration and involvement in the design and appropriateness of interventions is critical.

Abstract:According to the World Health Organization and the World Association for Sexual Health (WAS) Declaration of Sexual Rights, sex education aims to provide children and adolescents with comprehensive knowledge, skills, attitudes, and values, promoting equality and nondiscrimination while upholding freedom of thought and expression. However, current school curricula often focus on dichotomous sexual identity and hetero-cis-normative prevention strategies, neglecting the needs of gender minority (GM) and sexual minority (SM) youths. In family settings, discussions typically revolve around sexual risk reduction and basic contraception, omitting relational aspects and components of sexual identity such as orientations and gender expressions. This discrepancy highlights a gap between the official goals of sex education and its practical implementation, reflecting a cultural deficit in familial sex education. This study reviews the scientific literature on sexual health promotion interventions from 2015 to 2024 to identify inclusive approaches that enhance the participation of all youths, not just hetero-cis-normative individuals. The hypothesis is that most interventions primarily serve heterosexual and cisgender youths, indicating a need for more inclusive strategies to achieve better sexual health and educational outcomes. The study also suggests expanding curricula to align with recognized guidelines and the diverse needs of youth.

What do young people think about their school-based sex and relationship education? A qualitative synthesis of young people's views and experiences

Abstract:OBJECTIVES: Although sex and relationship education (SRE) represents a key strand in policies to safeguard young people and improve their sexual health, it currently lacks statutory status, government guidance is outdated and a third of UK schools has poor-quality SRE. We aimed to investigate whether current provision meets young people's needs.DESIGN: Synthesis of qualitative studies of young people's views of their school-based SRE.SETTING: Eligible studies originated from the UK, Ireland, the USA, Australia, New Zealand, Canada, Japan, Iran, Brazil and Sweden.PARTICIPANTS: Studies of students aged 4-19 in full-time education, young adults ≤19 (not necessarily in full-time education) or adults ≤25 if recalling their experiences of school-based SRE.RESULTS: -69 publications were identified, with 55 remaining after quality appraisal (representing 48 studies). The synthesis found that although sex is a potent and potentially embarrassing topic, schools appear reluctant to acknowledge this and attempt to teach SRE in the same way as other subjects. Young people report feeling vulnerable in SRE, with young men anxious to conceal sexual ignorance and young women risking sexual harassment if they participate. Schools appear to have difficulty accepting that some young people are sexually active, leading to SRE that is out of touch with many young people's lives. Young people report that SRE can be negative, gendered and heterosexist. They expressed dislike of their own teachers delivering SRE due to blurred boundaries, lack of anonymity, embarrassment and poor training.CONCLUSIONS: SRE should be 'sex-positive' and delivered by experts who maintain clear boundaries with students. Schools should acknowledge that sex is a special subject with unique challenges, as well as the fact and range of young people's sexual activity, otherwise young people will continue to disengage from SRE and opportunities for safeguarding and improving their sexual health will be reduced.

Abstract:Objective: Given the vulnerability of and the importance of caring for the specific health care needs of the growing lesbian, gay, bisexual, transgender, and queer (LGBTQ) population, the authors attempted to identify all educational interventions in psychiatric settings with quantitative outcomes targeting medical students, residents, and physicians in postgraduate settings. To gain insight from other disciplines that have published research in this area, a second objective was to review studies in teaching in those other disciplines. The authors sought to describe the methods of selected studies. Methods: The authors searched the published English-language literature indexed in PubMed, EMBASE, and PsycINFO using key terms for health care education concerning LGBTQ populations. The authors described and critically appraised studies with quantitative outcomes designed to enhance knowledge, skills, and attitudes in treating the LGBTQ community. Results: Of the 15 trials identified, 10 included medical students, 4 included internal medicine residents or medical school faculty, and 1 included oncologists. We did not find any randomized controlled trials or controlled nonrandomized trials of curricula dedicated to teaching learners in psychiatry. All of the studies included a presurvey, followed by an educational intervention and then a postsurvey assessment. The educational interventions, outcome measures, and quality of studies varied widely. Four studies enrolled self-identified members of the LGBTQ community as trainers and facilitators of the educational interventions. Conclusions: The lack of high-quality controlled studies indicates the need to develop evidence-based curricula to support the education of the psychiatric workforce to provide for the special needs of LGBTQ persons.

Abstract:This study explored the extent to which organisations responsible for initial teacher training, prepare pre-service teachers to deliver comprehensive sexuality education in schools. Five electronic databases were searched to identify publications that reported the provision of CSE training for pre-service teachers. Methodological quality was appraised using The Joanna Briggs Institute Critical Appraisal Checklist for Systematic Reviews and Research Syntheses. Descriptive analyses were conducted to summarise the scope of pre-service teacher preparation. Twenty-two documents, primarily from the UK, Australia and the USA were identified for review. Overall, information regarding training in comprehensive sexuality education for pre-service teachers is limited. The review identified wide variability in training content and positioning in primary and/or secondary programmes as either core or elective requirements. Current provision does not appear to align with international and best-practice guidelines in most instances. Further research and greater transparency regarding the preparation of pre-service teachers to deliver comprehensive sexuality education is recommended to identify and prioritise best-practice holistic teaching concepts to improve teacher and student outcomes.

Abstract:In the assessment of chronic hepatitis, the liver biopsy serves the following purposes: 1) to confirm the fact that it is chronic. 2) to evaluate its degree of activity. 3) to check if there is cirrhosis or not. 4) to seek morphological proofs for the presence of viral, drug-induced or other aetiological agents. 5) to follow the evolution (in the natural course or during treatment) of the disease by comparing repeated liver biopsies. The initial classification of chronic hepatitis--proposed in 1968--has been reviewed by numerous authors; at the moment, it seems necessary, in order to correlate clinical and histological results better, to emphasize two important facts: 1) the spreading of liver necrosis if there is chronic active hepatitis. 2) the presence--or not--of cirrhosis. Some morphological arguments could be useful in order to diagnose the aetiology of chronic hepatitis. Empirical and specific immunological methods have become available for demonstrating HBsAg. If a chronic active hepatitis with multisystem involvement and/or abnormal serum antibodies is diagnosed, corticosteroid therapy is indicated. In others cases, especially those with virus B disease, the treatment differs according to medical teams.

Abstract:Abstract Background Delivered globally to promote adolescents’ sexual and reproductive health, comprehensive sex education (CSE) is rights-based, holistic, and seeks to enhance young people’s skills to foster respectful and healthy relationships. Previous research has demonstrated that CSE programmes that incorporate critical content on gender and power in relationships are more effective in achieving positive sexual and reproductive health outcomes than programmes without this content. However, it is not well understood how these programmes ultimately affect behavioural and biological outcomes. We therefore sought to investigate underlying mechanisms of impact and factors affecting implementation and undertook a systematic review of process evaluation studies reporting on school-based sex education programmes with a gender and power component. Methods We searched six scientific databases in June 2019 and screened 9375 titles and abstracts and 261 full-text articles. Two distinct analyses and syntheses were conducted: a narrative review of implementation studies and a thematic synthesis of qualitative studies that examined programme characteristics and mechanisms of impact. Results Nineteen articles met the inclusion criteria of which eleven were implementation studies. These studies highlighted the critical role of the skill and training of the facilitator, flexibility to adapt programmes to students’ needs, and a supportive school/community environment in which to deliver CSE to aid successful implementation. In the second set of studies ( n = 8), student participation, student-facilitator relationship-building, and open discussions integrating student reflection and experience-sharing with critical content on gender and power were identified as important programme characteristics. These were linked to empowerment, transformation of gender norms, and meaningful contextualisation of students’ experiences as underlying mechanisms of impact. Conclusion and policy implications Our findings emphasise the need for CSE programming addressing gender and power that engages students in a meaningful, relatable manner. Our findings can inform theories of change and intervention development for such programmes.

Abstract:Comprehensive sex education from schools and parents can reduce adolescent sexual risk. Little is known about the associations between adolescent sex education and sexual satisfaction among young adults—particularly lesbian, gay, bisexual, and other sexual minority (LGB+) young adults. We examined how adolescent sex education (comprehensive versus abstinence-only) was associated with young adult sexual satisfaction (sexual contentment and sexual communication satisfaction). More comprehensive school-based sex education had a stronger association with sexual contentment and communication satisfaction in young adulthood for heterosexual participants than LGB+ participants. More comprehensive family-based sex education was associated with higher communication satisfaction for all participants.

Abstract:Forty patients aged 65-80 with minor-medium signs of brain aging were treated for 6 months with daily 90 mg doses of nimodipine. Treatment was well tolerated in that the minor side effects reported by a few of the patients never required withdrawal of the drug. Treatment proved useful, as shown by a variety of mental tests, in 69.5% of cases. In 20% conditions remained unchanged and in 9.5% further deterioration was observed. The authors conclude that nimodipine may be considered a useful therapeutic resource for phenomena of brain aging.

Abstract:<span>While sexuality education can support children and young people with disabilities in their sexual development and contribute to their wellbeing, challenges to its provision exist. This study identifies barriers to sexuality education for children and young people with disabilities in the WHO European Region via a scoping review of research published since 2006. Using the PRISMA-ScR Guidelines and predefined selection criteria, 14 studies were selected for inclusion. Together, these studies identified seven barriers to sexuality education for children and young people with disabilities, including the social misperception of people with disabilities as asexual and in need of protection which, combined with limited support for educators, resulted in noncomprehensive and normative sexuality education. Educators seemed inclined to redirect responsibility for sexuality education to others, and diversity among children and young people with disabilities, as well as cultural and religious diversity, makes it difficult to define a general approach. Finally, competing priorities related to the general health and wellbeing of children and young people with disabilities may appear to render sexuality education less important. We identify gaps in the research and highlight implications for the reduction of the barriers to sexuality education for children and young people with disabilities within the WHO European Region.</span>

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Introduction</h3><p>Adolescents are key stakeholders in sexual health education, yet they are rarely consulted when developing sexual health programs. Their voices are integral to improving the delivery of relevant and appropriate school-based sexual health education to promote safer adolescent sexual behaviors.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>An integrative review was conducted utilizing three databases: Cumulative Index of Nursing and Allied Health Literature (CINAHL) Complete, PubMed, and Education Resources Information Center (ERIC). The PRISMA and matrix method were used to search the literature and synthesize the findings from 16 articles regarding adolescent perceptions of school-based sexual health education.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The main themes that emerged from this review included: (a) factors influencing adolescent perceptions of sexual health education programs, (b) characteristics of good sexual health education programs, and (c) areas of improvement in sexual health education programs.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Adolescents overwhelmingly requested honest, comprehensive content delivered by nonjudgmental, well-educated health professionals in a comfortable environment.</p>

Abstract:Introduction: Sexual ill-health is an urgent public health issue with subsequent social and economic costs. There is, therefore, a need for more effective sexual health promotion interventions in an early stage of life. Previous research has focused on preventive sexual health interventions applying a risk perspective, and the limited and scattered evidence concerning school-based sexual health promotion interventions employing a health-resource perspective has not been compiled and synthesized. Hence, this study aimed to systematically review the current evidence on the effects of sexual health promotion interventions conducted in schools in Europe. Method: A systematic review based on the JBI and PRISMA standards was performed, encompassing searches in seven databases to identify sexual health promotion interventions conducted in European schools between 2012 and 2022. Data coding was performed according to a predetermined protocol and included information on study characteristics, intervention content, methods, and outcomes relevant to the current review. A narrative synthesis of the included studies was performed, highlighting the collective results. Result: Seventeen records were included in the review, reporting on 16 individual studies conducted in 7 European countries. Of the 16 included studies, 13 had a quantitative research design, and three had a qualitative design. All three studies with a qualitative research design described positive effects experienced by the participants. Six of thirteen quantitative studies showed statistically significant positive effects on at least one of the outcomes of interest. The outcomes of interest were grouped into five areas, and most studies focused on the area of attitudes toward sexual health. Conclusion: The findings indicate promising evidence of effect for interventions with a health promotion approach, highlighting the importance of strengthening sexual health resources related to respect, communication skills, attitudes, and other positive psycho-social aspects of sexual health. Most sexual health promotion intervention studies have focused on sexual health resource outcomes connected to attitudes and skills, whereas a comprehensive focus on the multi-dimensional sexual health literacy concept is less common and can be recommended to be included in future intervention research.

Abstract:A review on the reported uses of chemicals derived from botanical sources is presented, along with the part of the plant used for extraction, the mosquito species studied and the bioactivity observed for 344 plant species. Examples of phytochemicals evaluated against mosquitoes as general toxicants, growth and reproduction inhibitors, repellents and ovipositional deterrents are given. The effects of mosquito species and life stage specificity, solvents used for extraction, phototoxic activity and the geographical source from where the plant compounds are derived are discussed.
