Abstract:OBJECTIVES/GOALS: Our goal is to explore the extent to which organoids can serve as models for the protective mechanisms of the stomach–the mucus barrier and the pH gradient across it. We aim to first optimize and validate an organoid-based model of the gastric mucus layer, and then define the cellular mechanisms by which the gastric pH gradient is maintained across it. METHODS/STUDY POPULATION: We have developed a method for the in vitro engineering of gastric mucus by growing epithelial cells at the air-liquid interface (ALI). We use microrheology with fluorescent microspheres to define and compare the biophysical and viscoelastic properties of our lab-grown mucus to those of native mucus. We will perform CryoFE-SEM to compare the internal heterogeneity of our lab-grown mucus to fresh mucus obtained from patient tissue. For our mechanistic studies, we will use a pH-sensitive dye (methyl red) to assess the ability of our lab-grown mucus to maintain an artificial pH gradient in a microfluidic device. Next, we will use a pH microelectrode to measure proton flux through our mucus in vitro, investigating the potential for a physiological gradient in both 2D and 3D organoid models. RESULTS/ANTICIPATED RESULTS: Here we show that gastric organoids and their corresponding epithelial monolayers produce a mucus gel that does indeed mimic in vivo functions. Immunohistochemical staining, electron microscopy, microrheology, and particle tracking analyses revealed that our gastric organoid mucus is viscoelastic and structurally heterogeneous–both properties that are crucial to the stomach's mucosal first line of defense. Mechanically similar mucus was also engineered using two-dimensional air-liquid interface cultures of the same epithelia. Lastly, live confocal imaging revealed that H. pylori motility–an important virulence factor–was drastically hindered by our lab- grown mucus. DISCUSSION/SIGNIFICANCE: We describe a novel method for the in vitro engineering of gastric mucus and highlight biophysical properties that contribute to our stomach's defense against pathogens. This work will lead to an improved understanding of gastric physiology and may contribute to the development of novel drug delivery systems to tackle diseases of the gastric mucosa.

ENHANCED ELECTRODIFFUSIVE TRANSPORT ACROSS A MUCUS LAYER

Abstract:Diffusive transport of small ionic species through mucus layers is a ubiquitous phenomenon in physiology. However, some debate remains regarding how the various characteristics of mucus (charge of the polymers themselves, binding affinity of ions with mucus) impact the rate at which small ions may diffuse through a hydrated mucus gel. Indeed it is not even clear if small ionic species diffuse through mucus gel at an appreciably different rate than they do in aqueous solution. Here, we present a mathematical description of the transport of two ionic species (hydrogen and chloride) through a mucus layer based on the Nernst-Planck equations of electrodiffusion. The model explicitly accounts for the binding affinity of hydrogen to the mucus material, as well as the Donnan potential that occurs at the interface between regions with and without mucus. Steady state fluxes of ionic species are quantified, as are their dependencies on the chemical properties of the mucus gel and the composition of the bath solution. We outline a mechanism for generating enhanced diffusive flux of hydrogen across the gel region, and hypothesize how this mechanism may be relevant to the apparently contradictory experimental data in the literature.

Abstract:In a healthy colon, the stratified mucus layer serves as a crucial innate immune barrier to protect the epithelium from microbes. Mucins are complex glycoproteins that serve as a nutrient source for resident microflora and can be exploited by pathogens. We aimed to understand how the intestinal pathogen, , independently uses or manipulates mucus to its benefit, without contributions from members of the microbiota. Using a 2-D primary human intestinal epithelial cell model to generate physiologic mucus, we assessed mucus interactions through growth assays, RNA-Seq, biophysical characterization of mucus, and contextualized metabolic modeling. We found that host-derived mucus promotes growth both and in an infection model. RNA-Seq revealed significant upregulation of genes related to central metabolism in response to mucus, including genes involved in sugar uptake, the Wood-Ljungdahl pathway, and the glycine cleavage system. In addition, we identified differential expression of genes related to sensing and transcriptional control. Analysis of mutants with deletions in highly upregulated genes reflected the complexity of -mucus interactions, with potential interplay between sensing and growth. Mucus also stimulated biofilm formation , which may in turn alter viscoelastic properties of mucus. Context-specific metabolic modeling confirmed differential metabolism and predicted importance of enzymes related to serine and glycine catabolism with mucus. Subsequent growth experiments supported these findings, indicating mucus is an important source of serine. Our results better define responses of to human gastrointestinal mucus and highlight a flexibility in metabolism that may influence pathogenesis.

Abstract:To the Editor: We have read with interest the recent article published by Circulation Research entitled “Heteromultimeric Kv1 Channels Contribute to Myogenic Control of Arterial Diameter.”1 The study adds to an expanding body of evidence suggesting pivotal roles of the voltage-gated potassium channels (KV channels) in vascular smooth muscle cells as regulators of arterial tone. Nevertheless, we feel it is necessary to point out some issues relating to representation of the literature by Plane et al. Abstract:We present a Rolling-Circle-Enhance-Enzyme-Activity-Detection (REEAD) system with potential use for future point-of-care diagnosis of malaria. In the developed setup, specific detection of malaria parasites in crude blood samples is facilitated by the conversion of single Plasmodium falciparum topoisomerase I (pfTopI) mediated cleavage-ligation events, happening within nanometer dimensions, to micrometer-sized products readily detectable at the single molecule level in a fluorescence microscope. In principle, REEAD requires no special equipment and the readout is adaptable to simple colorimetric detection systems. Moreover, with regard to detection limit the presented setup is likely to outcompete standard gold immuno-based diagnostics. Hence, we believe the presented assay forms the basis for a new generation of easy-to-use diagnostic tools suitable for the malaria epidemic areas in developing countries.

Abstract:The human gut microbiota relies on complex carbohydrates (glycans) for energy and growth, primarily dietary fiber and host-derived mucins. We introduce a mathematical model of a glycan generalist and a mucin specialist in a two-compartment chemostat model of the human colon. Our objective is to characterize the influence of dietary fiber and mucin supply on the abundance of mucin-degrading species within the gut ecosystem. Current mathematical gut reactor models that include the enzymatic degradation of glycans do not differentiate between glycan types and their degraders. The model we present distinguishes between a generalist that can degrade both dietary fiber and mucin, and a specialist species that can only degrade mucin. The integrity of the colonic mucus barrier is essential for overall human health and well-being, with the mucin specialist Akkermanisa muciniphila being associated with a healthy mucus layer. Competition, particularly between the specialist and generalists like Bacteroides thetaiotaomicron, may lead to mucus layer erosion, especially during periods of dietary fiber deprivation. Our model treats the colon as a gut reactor system, dividing it into two compartments that represent the lumen and the mucus of the gut, resulting in a complex system of ordinary differential equations with a large and uncertain parameter space. To understand the influence of model parameters on long-term behavior, we employ a random forest classifier, a supervised machine learning method. Additionally, a variance-based sensitivity analysis is utilized to determine the sensitivity of steady-state values to changes in model parameter inputs. By constructing this model, we can investigate the underlying mechanisms that control gut microbiota composition and function, free from confounding factors.
Physiological Shear Stress Enhances Differentiation, Mucus-Formation and Structural 3D Organization of Intestinal Epithelial Cells In Vitro

Abstract:Gastrointestinal (GI) mucus plays a pivotal role in the tissue homoeostasis and functionality of the gut. However, due to the shortage of affordable, realistic in vitro GI models with a physiologically relevant mucus layer, studies with deeper insights into structural and compositional changes upon chemical or physical manipulation of the system are rare. To obtain an improved mucus-containing cell model, we developed easy-to-use, reusable culture chambers that facilitated the application of GI shear stresses (0.002-0.08 dyn∙cm-2) to cells on solid surfaces or membranes of cell culture inserts in bioreactor systems, thus making them readily accessible for subsequent analyses, e.g., by confocal microscopy or transepithelial electrical resistance (TEER) measurement. The human mucus-producing epithelial HT29-MTX cell-line exhibited superior reorganization into 3-dimensional villi-like structures with highly proliferative tips under dynamic culture conditions when compared to static culture (up to 180 vs. 80 µm in height). Additionally, the median mucus layer thickness was significantly increased under flow (50 ± 24 vs. 29 ± 14 µm (static)), with a simultaneous accelerated maturation of the cells into a goblet-like phenotype. We demonstrated the strong impact of culture conditions on the differentiation and reorganization of HT29-MTX cells. The results comprise valuable advances towards the improvement of existing GI and mucus models or the development of novel systems using our newly designed culture chambers.
Optimal antimicrobial response to a changing microbial background at a mucus interface

Abstract:Complex lifeforms host microbiota, microbes that live synergistically with their host. Accordingly, hosts have mechanisms to defend against and tolerate the microbiota. The intestinal mucus, where these systems collide, plays a pivotal role in managing this relationship, yet lacks an integrative theoretical framework. We propose a minimal model to elucidate dynamics at this interface, focusing on the ileum’s mucus defense. The model considers the effect of delay in host antimicrobial peptide secretion and how the host can use two different signals, from the bulk microbiota and from segmented filamentous bacteria (SFB), assuming that the SFB anticipate the bulk microbiota. We propose a theory whereby the host can optimize defense by minimizing antimicrobial peptide production and controlling bacterial exposure. Integrating two recent experiments, we show host dynamics are consistent with sensing both bulk and SFB, supporting our ‘optimal defense’ hypothesis. Therefore, we propose that similar mechanisms could prove advantageous to other species and applicable beyond the ileum’s mucus barrier.

Abstract: The ability of adherent, macrophage-like spleen cells from BCG-infected C57BL/6 mice to suppress primary cytotoxic responses to alloantigen in vitro has been studied. Suppression was dependent on a viable suppressor cell, for both heating (56°C 30 min) and glutaraldehyde treatment (0.1%, 2 min) ablated the suppressive action. Membrane preparations were not suppressive. Suppression was reversible and independent of the suppressor cell's Fc receptor sites. Thus, suppressor cell populations pretreated with heat-aggregated IgG or bound to immobilized immune complexes were still capable of suppressing the cytotoxic response. We could find no evidence that the suppressor cells function through the production of a soluble mediator. The principal mode of action of the suppressor cell seemed to reflect a capacity to modify the production and action of a mediator whose production was dependent upon helper (Lyt 1+) T cells. This alloantigen-induced mediator, which augments the proliferation and/or differentiation of cytotoxic T cells, was not produced in mixed lymphocyte cultures containing spleen cells of BCG-infected animals nor by normal spleen cells in the presence of the BCG-induced suppressor cells. Under appropriate conditions, mediator produced from mixed lymphocyte cultures was able to reverse the suppression of primary cytotoxic responses by BCG-induced suppressor cells.

Abstract:The human gut microbiota rely on complex carbohydrates for energy and growth, particularly dietary fiber and host-produced mucins. These complex carbohydrates must first be hydrolysed by certain microbial groups to enable cross-feeding by the gut microbial community. We consider a mathematical model of the enzymatic hydrolysis of complex carbohydrates into monomers by a microbial species. The resulting monomers are subsequently digested by the microbial species for growth. We first consider the microbial species in a single compartment continuous stirred-tank reactor where dietary fiber is the only available substrate. A two compartment configuration in which a side compartment connected by diffusion is also studied. The side compartment is taken to be the mucus layer of the human colon, providing refuge from washout and an additional source of complex carbohydrate in the form of mucins. The two models are studied using stability analysis, numerical exploration, and sensitivity analysis. The delay in substrate availability due to hydrolysis results in bistability and the unconditional asymptotic stability of the trivial equilibrium. The addition of the mucus compartment allows the microbial species to survive under conditions that would otherwise result in washout in a comparable single compartment reactor. This would suggest that depending on the features of the gut microbiota being studied, extracellular hydrolysis and a representation of the mucus layer should be included in mathematical and lab reactor models of the human gut microbiota.

Enhanced microscopic dynamics in mucus gels under a mechanical load in the linear viscoelastic regime

Abstract:Mucus is a biological gel covering the surface of several tissues and insuring key biological functions, including as a protective barrier against dehydration, pathogens penetration, or gastric acids. Mucus biological functioning requires a finely tuned balance between solid-like and fluid-like mechanical response, insured by reversible bonds between mucins, the glycoproteins that form the gel. In living organisms, mucus is subject to various kinds of mechanical stresses, e.g. due to osmosis, bacterial penetration, coughing and gastric peristalsis. However, our knowledge of the effects of stress on mucus is still rudimentary and mostly limited to macroscopic rheological measurements, with no insight into the relevant microscopic mechanisms. Here, we run mechanical tests simultaneously to measurements of the microscopic dynamics of pig gastric mucus. Strikingly, we find that a modest shear stress, within the macroscopic rheological linear regime, dramatically enhances mucus reorganization at the microscopic level, as signalled by a transient acceleration of the microscopic dynamics, by up to two orders of magnitude. We rationalize these findings by proposing a simple yet general model for the dynamics of physical gels under strain and validate its assumptions through numerical simulations of spring networks. These results shed new light on the rearrangement dynamics of mucus at the microscopic scale, with potential implications in phenomena ranging from mucus clearance to bacterial and drug penetration.

Abstract:BACKGROUND Pertussis in infants is often severe, resulting in complications and prolonged hospitalization. Treatment is limited to supportive care. Antibiotics do not significantly alter the course of the disease. Therapies directed at pertussis toxin, a major virulence factor of Bordetella pertussis, might be beneficial. This study examines the safety and pharmacology of intravenous pertussis immunoglobulin (P-IGIV), which has high levels of pertussis toxin antibodies. METHODS P-IGIV was prepared as a 4% IgG solution from the pooled plasma from donors immunized with inactivated pertussis toxoid. The IgG pertussis toxin antibody concentration of 733 microg/ml is >7-fold higher than contained in conventional intravenous immunoglobulin products. Children with presumptive pertussis were allocated to one of three treatment doses of P-IGIV. RESULTS Twenty-six of 30 enrolled children had confirmed pertussis. There were no adverse events associated with P-IGIV except one patient who had transient hypotension that responded to an infusion rate decrease. P-IGIV doses of 1500, 750 and 250 mg/kg achieved > or =4-fold, 3-fold and >2-fold rises in peak geometric mean titers of pertussis toxin IgG antibodies, respectively. P-IGIV exhibited a half-life of 38.4 days and a volume of distribution of 87.8 ml/kg. All three treatment groups showed declines in lymphocytosis (P < 0.05) and paroxysmal coughing by the third day after P-IGIV infusion compared with preinfusion values. CONCLUSION P-IGIV is safe and achieves high pertussis toxin antibody titers in infants. This study provides data for a prospective, controlled trial of P-IGIV.

Abstract:Previous studies have demonstrated a stimulatory effect of interleukin-1 beta (IL-1 beta) on insulin and glucagon release from the perfused rat pancreas, accompanied by selective lysis of 20% of beta-cells as assessed by electronmicroscopy. However, we have not observed an inhibitory action of IL-1 beta on insulin release from the perfused pancreas as shown for isolated islets. To test whether periodical exposure of the endocrine pancreas to circulating IL-1 beta in vivo affects insulin release from the intact perfused pancreas, rats were treated with daily intraperitoneal injections of 4 micrograms IL-1 beta/kg or saline for 5 days. On day 5 the pancreata were isolated 2 h after the last injection and perfused from 0 to 72 min with 11 mmol/l D-glucose and from 72 to 84 min with 20 mmol/l D-glucose. Saline or IL-1 beta was added from 12 to 72 min. In pancreata from animals pre-treated with IL-1 beta glucose-stimulated as well as IL-1 beta potentiated glucose-stimulated insulin release was almost completely abolished. Furthermore, a decline in insulin release was observed at 11 mmol/l D-glucose, in contrast to an increase in insulin release in controls. The total extractable insulin content in pancreata from IL-1 beta pre-treated rats was higher than in pancreata from saline-treated controls. In contrast to the inhibitory effect of in vivo administration of IL-1 beta on beta-cell function glucagon secretion was stimulated. These observations suggest that circulating IL-1 beta is an important modulator of alpha- and beta-cell secretory function in vivo and that IL-1 beta should be considered a contributory pathogenetic factor in the development of insulin-dependent (type 1) diabetes mellitus.

Abstract:We present an experimentally guided, bi-phasic, multi-species ionic gel model to compare and make quantitative predictions on the viscoelastic properties of healthy mucus versus Cystic-Fibrosis infected mucus. The mixture theory consists of the mucus (polymer phase) and water (solvent phase) as well as several different ions: H$^+$, Na$^+$, Cl$^-$ and Ca$^{2+}$. The model is calibrated with the experimental data of mucus diffusivities. Numerical studies reveal that the Donnan forces are the dominating mechanism driving the mucus swelling/deswelling transition. However, these transitions could be continuous or discontinuous, depending on the complex interplay between ionic bath concentrations, pH, polymer mass in the solvent and the average charge per monomer.

Modelling the dynamics of mammalian gut homeostasis

Abstract:Homeostatic balance in the intestinal epithelium relies on a fast cellular turnover, which is coordinated by an intricate interplay between biochemical signalling, mechanical forces and organ geometry. We review recent modelling approaches that have been developed to understand different facets of this remarkable homeostatic equilibrium. Existing models offer different, albeit complementary, perspectives on the problem. First, biomechanical models aim to explain the local and global mechanical stresses driving cell renewal as well as tissue shape maintenance. Second, compartmental models provide insights into the conditions necessary to keep a constant flow of cells with well-defined ratios of cell types, and how perturbations can lead to an unbalance of relative compartment sizes. A third family of models address, at the cellular level, the nature and regulation of stem fate choices that are necessary to fuel cellular turnover. We also review how these different approaches are starting to be integrated together across scales, to provide quantitative predictions and new conceptual frameworks to think about the dynamics of cell renewal in complex tissues.

Abstract:Mucus is a viscoelastic hydrogel that lines and protects the epithelial surfaces of the body that houses commensal microbiota and functions in host defense against pathogen invasion. As a first‐line physical and biochemical barrier, intestinal mucus is involved in immune surveillance and spatial organization of the microbiome, while dysfunction of the gut mucus barrier is implicated in several diseases. Mucus can be collected from a variety of mammalian sources for study, however, established methods are challenging in terms of scale and efficiency, as well as with regard to rheological similarity to native human mucus. Therefore, there is a need for mucus‐mimetic hydrogels that more accurately reflect the physical and chemical profile of the in vivo human epithelial environment to enable the investigation of the role of mucus in human disease and interactions with the intestinal microbiome. This review will evaluate the material properties of synthetic mucus mimics to date designed to address the above need, with a focus towards an improved understanding of the biochemical and immunological functions of these biopolymers related to utility for research and therapeutic applications. Abstract:The gastric mucosa maintains structural integrity and function despite continuous exposure to noxious factors, including 0.1 mol/L HCl and pepsin, that are capable of digesting tissue. Under normal conditions, mucosal integrity is maintained by defense mechanisms, which include preepithelial factors (mucus-bicarbonate-phospholipid "barrier"), an epithelial "barrier" (surface epithelial cells connected by tight junctions and generating bicarbonate, mucus, phospholipids, trefoil peptides, prostaglandins (PGs), and heat shock proteins), continuous cell renewal accomplished by proliferation of progenitor cells (regulated by growth factors, PGE(2) and survivin), continuous blood flow through mucosal microvessels, an endothelial "barrier," sensory innervation, and generation of PGs and nitric oxide. Mucosal injury may occur when noxious factors "overwhelm" an intact mucosal defense or when the mucosal defense is impaired. We review basic components of gastric mucosal defense and discuss conditions in which mucosal injury is directly related to impairment in mucosal defense, focusing on disorders with important clinical sequelae: nonsteroidal anti-inflammatory drug (NSAID)-associated injury, which is primarily related to inhibition of cyclooxygenase (COX)-mediated PG synthesis, and stress-related mucosal disease (SRMD), which occurs with local ischemia. The annual incidence of NSAID-associated upper gastrointestinal (GI) complications such as bleeding is approximately 1%-1.5%; and reductions in these complications have been demonstrated with misoprostol, proton pump inhibitors (PPIs) (only documented in high-risk patients), and COX-2 selective inhibitors. Clinically significant bleeding from SRMD is relatively uncommon with modern intensive care. Pharmacologic therapy with antisecretory drugs may be used in high-risk patients (eg, mechanical ventilation >or=48 hours), although the absolute risk reduction is small, and a decrease in mortality is not documented.
Anion exchange regulates intestinal mucus expansion while Cftr regulates mucus secretion

Abstract:The intestinal epithelium is covered by mucus that protects the tissue from the luminal content. Studies have shown that anion secretion via the cystic fibrosis conductance regulator (Cftr) regulates mucus formation in the small intestine. However, mechanisms regulating mucus formation in the colon are less understood. The aim of this study was to explore the role of anion transport in regulation of mucus formation during steady state, and in response to carbamylcholine (CCh) and prostaglandin E (PGE ). CftrΔF508 (CF) mice were used to assess the role of Cftr, and 4,4′-diisothiocyanatostilbene-2,2′-disulfonate (DIDS) was used to inhibit anion exchange. In the distal colon, steady state mucus expansion was reduced by apical DIDS, and normal in CF mice. PGE stimulated mucus expansion without mucus secretion in wild type (WT) and CF distal colon via DIDS sensitive mechanisms, while CCh induced mucus secretion in WT but not in CF colon. However, when added simultaneously, CCh and PGE , stimulated mucus secretion in CF colon via DIDS sensitive pathways. A similar response was observed in CF ileum that responded to CCh and PGE with DIDS sensitive mucus secretion. In conclusion, this study suggests that apical anion exchange regulates intestinal mucus expansion, while Cftr regulates mucus secretion from ileal and distal colon crypts. Furthermore, these findings demonstrate that in the absence of a functional Cftr channel, activation of anion exchange can help release mucus from intestinal goblet cells.
