Abstract:Background and Aims: Assessment is an educational process that measures student ability to accomplish specific objectives, helping to bridge the gap between teaching and learning. This study evaluated the quality of multiple-choice questions (MCQs) used to assess undergraduate medical students’ knowledge in the Nutrition & Metabolism course at Taibah University. Methods: The study used item analysis to evaluate the MCQs, including item difficulty, item discrimination, reliability, standard error of measurement, and distractor analysis. The study enrolled 171 undergraduate medical students who took a 30-item MCQ exam. Results: The results showed that the exam was reliable and consistent in measuring students’ knowledge and understanding, but some items had issues that needed revision or deletion from the question bank. The report provides useful information for improving the exam’s quality and highlights the importance of ongoing assessment and revision of exam items. The findings suggest that ongoing assessment and revision of exam items are crucial to improving the quality of assessments and creating more effective measures of student learning. Conclusions: By leveraging these results, educators can identify areas where students may need further support and improve the design of their assessments.
The effect of amino acids on the metabolic fate of 15NH4Cl in isolated sheep hepatocytes.

Abstract:Ruminants characteristically absorb a large proportion of dietary nitrogen across the portal-drained viscera as ammonia nitrogen which is detoxified by conversion to urea in the liver. In theory, ammonia can supply both nitrogen atoms of the urea molecule via mitochondrial (carbamoyl phosphate) and cytoplasmic (aspartate) precursor pathways of the ornithine cycle but the effect of amino acids on the flux of nitrogen from ammonia to each of the two urea nitrogen atoms has not been determined. We report a study designed to determine the distribution of [15N] ammonia between [15N1]urea and [15N2]urea in sheep hepatocytes in response to ammonia concentrations (0.33, 0.67 and 1.00 mM) in the presence or absence of amino acids. In the absence of amino acids, the enrichment of [15N2]urea rose more rapidly during incubations than [15N1]urea and attained enrichments of 66-88% within 5 min of incubation. At the end of 2.5 h of incubation, [15N2]urea represented 60% and 90% of the total urea molecules at low and high ammonia concentrations, respectively. The enrichments of glutamate and aspartate were similar to [15N1]urea in the cells at the end of the incubations, even in the presence of unlabelled amino acids, supporting the concept of mitochondrial ammonia being in equilibrium with cytosolic aspartate formation. In the presence of amino acids basal urea synthesis increased but ammonia uptake and 15NH4Cl conversion to urea was less than in the absence of amino acids. The rate of formation of [15N1]urea was greater in incubations containing amino acids but when ammonia concentration in the media was raised only [15N2]urea flux increased with no change in either [15N1]urea or the unlabelled species. Measurement of media amino acid concentrations after 2.5 h of incubation in the presence of amino acids revealed that arginine, glutamine, glycine and alanine were removed while there was net formation of aspartate, threonine, serine, glutamate, and the branched chain amino acids. However, less than 12% of the 15N transfer appeared in free amino acids. The increases in basal and unlabelled urea synthesis in the presence of amino acids could be numerically accounted as the sum of arginine and glutamine removal from incubations. It is concluded that in sheep hepatocytes 15NH4Cl removal leads to quantitative formation of [15N2]urea, even in the presence of a physiological mixture of amino acids. The increase in the formation of the [15N1]urea in the presence of amino acids can be explained by the preferential utilisation of the amide nitrogen of glutamine for urea synthesis.

Abstract:Thrombopoietin (TPO) is a hemopoietic cytokine that specifically stimulates the growth and development of megakaryocytes. In addition to full-length TPO (corresponding to TPO-1), cDNAs of three truncated variants (TPO-2 to 4) have been isolated. However, whether or not these variants have biological activities has not been clarified presumably because of poor secretion into extracellular milieu. To examine the biological significance of the shortest variants, TPO-4, we prepared two fusion cDNAs of TPO-1 and TPO-4, each of which was combined with the finger-growth factor-kringle 1 (FGK1) domains of tissue plasminogen activator (tPA) as a tag and transfected them into baby hamster kidney (BHK) cells. An immunodetection assay for FGK1 antigen revealed that the amount of secreted TPO-4-FGK1 fusion protein was about 2% of that of TPO-1-FGK1. These fusion proteins after affinity purification had comparable activities on a molar basis in both megakaryocyte colony stimulating activity towards mouse bone marrow cells and growth promotion activity towards the TPO-responsive cell line UT-7/GM. These results indicate that the TPO-4 is biologically active in vitro, although its primary structure of the C-terminal half after the 111th amino acid residues is completely different from authentic TPO-1 due to frame shift.

Abstract:Multiple-choice questions (MCQs) are the most popular type of items used in knowledge-based assessments in undergraduate and postgraduate healthcare education. MCQs allow assessment of candidates' knowledge on a broad range of knowledge-based learning outcomes in a single assessment. Single-best-answer (SBA) MCQs are the most versatile and commonly used format. Although writing MCQs may seem straight-forward, producing decent-quality MCQs is challenging and warrants a range of quality checks before an item is deemed suitable for inclusion in an assessment. Like all assessments, MCQ-based examinations must be aligned with the learning outcomes and learning opportunities provided to the students. This paper provides evidence-based guidance on the effective use of MCQs in student assessments, not only to make decisions regarding student progression but also to build an academic environment that promotes assessment as a driver for learning. Practical tips are provided to the readers to produce authentic MCQ items, along with appropriate pre- and post-assessment reviews, the use of standard setting and psychometric evaluation of assessments based on MCQs. Institutions need to develop an academic culture that fosters transparency, openness, equality and inclusivity. In line with contemporary educational principles, teamwork amongst teaching faculty, administrators and students is essential to establish effective learning and assessment practices.

Abstract:Construct: Authors examined the use of multiple choice question tests in assessment of undergraduate students, and made a docimological evaluation of the quality of the tests. Background: Moroccan universities are gradually integrating multiple-choice questions as a method of evaluation. Abstract:INTRODUCTION: Despite the ubiquity of single-best answer multiple-choice questions (MCQ) in assessments throughout medical education, question writers often receive little to no formal training, potentially decreasing the validity of assessments. While lengthy training opportunities in item writing exist, the availability of brief interventions is limited.METHODS: We developed and performed an initial validation of an item-quality assessment tool and measured the impact of a brief educational intervention on the quality of single-best answer MCQs.RESULTS: The item-quality assessment tool demonstrated moderate internal structure evidence when applied to the 20 practice questions (κ=.671, p<.001) and excellent internal structure when applied to the true dataset (κ=0.904, p<.001). Quality scale scores for pre-intervention questions ranged from 2-6 with a mean ± standard deviation (SD) of 3.79 ± 1.23, while post-intervention scores ranged from 4-6 with a mean ± SD of 5.42 ± 0.69. The post-intervention scores were significantly higher than the pre-intervention scores, <i>x</i> <sup>2</sup>(1) =38, p <0.001.CONCLUSION: Our study demonstrated short-term improvement in single-best answer MCQ writing quality after a brief, open-access lecture, as measured by a simple, novel, grading rubric with reasonable validity evidence.

[Retinal blood flow changes in eyes with rhegmatogenous retinal detachment and scleral buckling procedures].

Abstract:Bidirectional laser Doppler technique and monochromatic photography was used to measure the absolute blood flow rate (F) in the major superior or inferior temporal retinal arteries in 3 patients with rhegmatogenous retinal detachment, in 4 patients following unilateral scleral buckling procedures, and in one patient before and after removal of scleral buckling elements. In 2 of the eyes with retinal detachment, F in arteries supplying the detached portion of the retina was approximately 30% lower than in arteries supplying the attached portion. On average, F measured in the arteries supplying the detached portion of the retina in 2 patients was 8.9 +/- 1.3 microliters/min, 67% lower than normal controls. F measured in the arteries supplying the attached portion of the retina in 3 patients was 15.2 +/- 3.9 microliters/min, on average, 43% lower than in normal controls. In 4 patients who had undergone uncomplicated, unilateral scleral buckling procedures, F in an artery of the affected eye was compared with F in the corresponding artery of the fellow eye. F in the artery of the affected eyes was 8.7 +/- 3.2 microliters/min, on average, 59% lower than that of the fellow eyes (p less than 0.05), and 68% lower than in normal controls (p less than 0.0001). Removal of scleral buckling elements in one patient produced a 79% increase in F in the measured artery. Then, F returned to normal levels after removal of scleral buckling elements.

Abstract:An outbreak of enzootic intranasal tumour (EIT) in a goat herd in Central Italy is described. From October 1990 to September 1992 41 tumors were diagnosed in goats ranging in age from 6 months to 6 years and of both the Alpine and Saanen breeds. Clinically the affected goats showed nasal discharge, facial swelling, sneezing and snoring respiration and weight loss. Post-mortem examinations revealed uni- or bilateral tumours of the olfactory mucous membrane. The neoplasms were classified as low grade adenocarcinomas. Ultrastructurally, retrovirus-like particles were observed in 10/10 tumours examined. Epidemiological and ultrastructural findings strongly suggest a viral aetiology for intranasal enzootic adenocarcinoma in the goats examined.

Abstract:Introduction: In the field of medical education, multiple-choice questions (MCQs) represent the most commonly utilized method of assessment. It is necessary to analyze the assessment results through item analysis to ensure the quality is appropriate. This study evaluated the quality of the MCQs utilized for summative evaluation of the students in the General Surgery Course conducted in the year 2023-24, at the College of Medicine (Unaizah), Qassim University, Saudi Arabia. Methods: Using a number of established parameters for item analysis, the study evaluated the multiplechoice questions for difficulty, discrimination power, and quality of distractors. Results: The quality of the questions varied. The means of the facility index, discrimination index, discriminative efficiency, and distractor efficiency were, in order, 76.31%, 0.28, -0.7743, and 32%. Conclusion: Item analysis is a crucial technique for evaluating the quality of MCQs. There were multiple defects in the MCQs used in summative assessments, revealing the scope for further improvement in future courses. It is important to plan faculty development events often to impart knowledge and skills related to creating MCQs that are valid, reliable, and of high quality.

Abstract:BACKGROUND: Multiple choice questions (MCQs) are frequently used to assess students in different educational streams for their objectivity and wide reach of coverage in less time. However, the MCQs to be used must be of quality which depends upon its difficulty index (DIF I), discrimination index (DI) and distracter efficiency (DE).OBJECTIVE: To evaluate MCQs or items and develop a pool of valid items by assessing with DIF I, DI and DE and also to revise/ store or discard items based on obtained results.SETTINGS: Study was conducted in a medical school of Ahmedabad.MATERIALS AND METHODS: An internal examination in Community Medicine was conducted after 40 hours teaching during 1(st) MBBS which was attended by 148 out of 150 students. Total 50 MCQs or items and 150 distractors were analyzed.STATISTICAL ANALYSIS: Data was entered and analyzed in MS Excel 2007 and simple proportions, mean, standard deviations, coefficient of variation were calculated and unpaired t test was applied.RESULTS: Out of 50 items, 24 had "good to excellent" DIF I (31 - 60%) and 15 had "good to excellent" DI (> 0.25). Mean DE was 88.6% considered as ideal/ acceptable and non functional distractors (NFD) were only 11.4%. Mean DI was 0.14. Poor DI (< 0.15) with negative DI in 10 items indicates poor preparedness of students and some issues with framing of at least some of the MCQs. Increased proportion of NFDs (incorrect alternatives selected by < 5% students) in an item decrease DE and makes it easier. There were 15 items with 17 NFDs, while rest items did not have any NFD with mean DE of 100%.CONCLUSION: Study emphasizes the selection of quality MCQs which truly assess the knowledge and are able to differentiate the students of different abilities in correct manner.

Abstract:Background: There are different types of exam formats for educational assessment. Multiple choice questions (MCQs) are frequently utilized assessment tools in health education. Considering the reliability and validity in developing MCQ items is vital. Educators often face the difficulty of developing credible distractors in MCQ items. Poorly constructed MCQ items make an exam easier or too difficult to be answered correctly by students as intended learning objectives. Checking the quality of MCQ items is overlooked and too little is known about it. Therefore, this study aimed to assess the quality of MCQ items using the item response theory model. Methods: A descriptive cross-sectional study was conducted among MCQ items of public health courses administered to 2nd year nursing students at Injibara university. A total of 50 MCQ items and 200 alternatives were evaluated for statistical item analysis. The quality of MCQ items was assessed by difficulty index (DIF), discrimination index (DI), and distractor efficiency (DE) using students’ exam responses. Microsoft excel sheet and SPSS version 26 were used for data management and analysis. Results: Post-exam item analysis showed that 11 (22%) and 22 (44%) MCQs had too difficult and poor ranges for difficulty and discriminating powers respectively. The overall DE was 71.3%. About forty (20%) distractors were non-functional. Only 8 (16%) MCQs fulfilled the recommended criteria for all-DIF, DI, and DE parameters. Conclusions: The desirable criteria for quality parameters of MCQ items were satisfied only in a few items. The result implies the need for quality improvement. Continuous trainings are required to improve the instructors’ skills to construct quality educational assessment tools.

Abstract:Background: Integration of assessment with education is vital and ought to be performed regularly to enhance learning. There are many assessment methods like Multiple-choice Questions, Objective Structured Clinical Examination, Objective Structured Practical Examination, etc. The selection of the appropriate method is based on the curricula blueprint and the target competencies. Although MCQs has the capacity to test students’ higher cognition, critical appraising, problem-solving, data interpretation, and testing curricular contents in a short time, there are constraints in its analysis. The authors aim to accentuate some consequential points about psychometric analysis displaying its roles, assessing its validity and reliability in discriminating the examinee’s performance, and impart some guide to the faculty members when constructing their exam questions bank. Methods: Databases such as Google Scholar and PubMed were searched for freely accessible English articles published since 2010. Synonyms and keywords were used in the search. First, the abstracts of the articles were viewed and read to select suitable match, then full articles were perused and summarized. Finally, recapitulation of the relevant data was done to the best of the authors’ knowledge. Results: The searched articles showed the capacity of MCQs item analysis in assessing questions’ validity, reliability, its capacity in discriminating against the examinee’s performance and correct technical flaws for question bank construction. Conclusion: Item analysis is a statistical tool used to assess students’ performance on a test, identify underperformed items, and determine the root causes of this underperformance for improvement to ensure effective and accurate students’ competency judgment. Abstract:Background: Assessment plays an essential role in the evaluation of learning, and multiple-choice questions (MCQs) are one of the components of examinations. Item analysis enables identifying good MCQs based on difficulty index (DIF I), discrimination index (DI), and distractor effectiveness (DE). The assessment of learning will be more meaningful with routine item analysis. Aims and Objectives: The item analysis helps us to assess the validity and reliability of MCQs and to generate a valid MCQ question bank for future use. Materials and Methods: A set of 30 single best response type MCQs or items was used for assessment in 155 phase II MBBS students at a medical college in Mangalore. Each item was pre-validated before and later analyzed for its DIF I, DI, and DE. Results: In the study, the means of DIF I were 55.91 ± 18.48%, DI, was 0.35 ± 0.16%, and distractor efficiency was 72.19 ± 29.15%. Out of 30 items, 22 had a “good to acceptable” level of DIF I, and 18 items had “good to excellent” discrimination power. 81% were functional distractors among 120 distractors. A correlation r = 0.1968 indicates a positive correlation between DIF I and DI. Conclusion: The majority of the MCQs in this study fell within the acceptable range when measured against the three factors analyzed. This suggests that the key to developing valid and reliable MCQs is possible only when regular item analysis is done for all MCQ-based tests.

Abstract:Purpose: This study explores the current knowledge and overall awareness of the faculty at an Apex institute about the use and difficulties of scenario-based multiple-choice questions (SB-MCQs), short-answer questions (SB-SAQs), and modified essay questions (SB-MEQs) in the assessment of the undergraduate and postgraduate students. Objectives: To assess faculty perception of awareness and use of SB-MCQs, SB-SAQs, and SB-MEQs and to understand the challenges encountered while designing scenario-based questions (SBQs) and the ways to overcome them. Study procedure: The tool used for data collection was a Google form questionnaire designed with a total of 16 questions-12 Likert-scale format items and four open-ended questions. The quantitative data collected as a response to close-ended questions was analyzed by descriptive statistics and percentage values. For qualitative data, thematic analysis was done for open-ended questions. Conclusion: The study showed that the faculty has the motivation and agreeability to switch over from traditional questions to scenario-based questions but constant training in the form of regular faculty development programs and workshops is required for its effective implementation. On the administrative level, challenges like lack of sufficient faculty and proper inter-departmental integration for designing scenarios must be addressed. Abstract:PROBLEM: Multiple-choice questions (MCQs) are the main method of assessing medical student knowledge. As a result there is a high demand from medical students for formative MCQs. However, teaching staff rarely have the time or incentive to develop high-quality formative questions, focusing instead on material for high-stakes assessments.INTERVENTION: We have developed a novel student-led approach involving an interactive online question database, created by medical students for medical students. We adopted a staged approach to create an online bank of formative MCQ questions. First, students write MCQs following a standardized format. Questions are then peer-reviewed by other students, discussing relevant clinical topics, guidelines, and journals to improve question quality. The questions are then scrutinized by specialist doctors and academics. Next, questions are piloted online. Finally, question performance is evaluated statistically. This 5-stage student-led process produced a bank of more than 200 MCQs in three months.CONTEXT: This intervention was carried out by two final-year medical student leads at Cardiff University School of Medicine, UK. Final-year students were recruited to write and peer-review questions, and senior content specialists were recruited from the department. After piloting and evaluation of the questions, the question bank was made available as a learning resource to all medical students at Cardiff University.OUTCOME: Objective analysis of the created MCQs (discrimination indices and distractor analysis) indicated that the random sample of questions piloted were of high quality. When the questions were made available as online tests to approximately 600 students, usage data revealed that 2,800 tests were taken over a 3-month period, indicating that the resource was popular. In addition, subjective feedback from students question writers/reviewers was gathered via free text feedback forms and was invariably positive. We plan to continue the question generation process in Cardiff and would encourage other medical schools to adopt this approach.LESSONS LEARNED: Our 5-stage approach can generate a large volume of high-quality MCQs, addressing the demand from students for formative MCQ questions, with minimal teaching staff input. Abstract:Facioscapulohumeral dystrophy (FSHD) is a rare genetic disease that has been described more than a hundred years ago. The Miogen Lab has been able to collect a large amount of data on patients affected by FSHD and their relatives over the years, also extending the research to their ancestors. Collected data include molecular analysis, clinical information on health status, family pedigree and geographic origin. The challenge of FSHD Registry is to investigate these large amount of information, discover additional elements related to disease onset and better understand the clinical progression and genetic inheritance of the disease, exploiting data integration capabilities and Big Data techniques. In this paper we describe the tools we used to collect, integrate and display these data in a framework that allows users to search among clinical records to elaborate brief reports and discover new relations on collected data. The solution provides charts, maps and search tools customized on the specific needs that came to light during the collaboration between DataRiver and Miogen Lab, joining the clinical knowledge of the latter with the information technology expertise of the former. The framework offers a single entry point for all genomic and therapeutic studies.
