Abstract:Introduction: Metabolic abnormalities and hepatic steatosis are increasingly significant risk factors for liver cancer in Asia and worldwide, as the incidence of obesity and Type 2 diabetes rise. Previous data from Asian countries indicate varying prevalence rates of epidemiology of metabolic syndrome (MetS) and metabolic dysfunction-associated steatotic liver disease (MASLD) in CHB patients. Methods: This study aims to investigate the epidemiology of MetS and MASLD and their association with liver histology and hepatitis B virus (HBV) biomarkers in Asian Americans (AAs) with CHB. We used the AoU Research Program which contains 14,162 AAs in the Controlled Tier Dataset v7 and we created a cohort containing 274 (1.93%) AAs with CHB. Results: Of the 274 AAs, 48 (17.52%) have type-2 diabetes, 41 (14.96%) have obesity, 75 (27.37%) have high cholesterol, 97 (35.40%) have hypertension, and 49 (17.88%) have liver steatosis. MASLD is characterized by having liver steatosis along with at least one component of the MetS. Based on the definition, there are 46 individuals classified as having MASLD, which was 16.79% of all AAs with CHB. The high prevalence indicates that liver steatosis may be a significant risk factor for the development of MASLD in this population. Currently, we are still analyzing the data to examine the relationship and impact of MetS, MASLD, and other relevant factors. In this presentation, we will share our experience in utilizing the AoU datasets and discuss our next steps and timeline. In this presentation, we will also share our experience navigating the Researcher Workbench for data cleaning and analysis for our current NCI/NIH-funded projects that focuses on examining the metabolic and steatotic profile and risk factors among Asian Americans living with chronic hepatitis B. Conclusion: While the AoU Research Program offers unprecedented access to electronic health records, the dataset has proven challenging to work with. Addressing the remaining challenges entails several key aspects. Firstly, it involves the meticulous task of accounting for single-concept variables sourced from diverse origins. Secondly, there's the crucial step of synthesizing variables that essentially gauge identical concepts. Additionally, it requires a deep comprehension of the data structure and hierarchy, particularly when coding for disease presence. Equally vital is ensuring data accuracy and reliability throughout the process. Finally, we'll delve into previous setbacks, elucidating the solutions we've since devised. Through this presentation, we hope to facilitate communication and build connection with researchers in cancer health disparities field. Abstract:Purpose The Lanxi Cohort was established to systematically investigate the aetiology and interplay of body fat distribution and multiple factors with obesity and obesity-related non-communicable diseases in China. Participants The baseline investigation of the Lanxi Cohort study took place between June 2015 and August 2017 in Lanxi, Zhejiang Province, China. Permanent residents from one urban community and four rural villages were involved in this study. The baseline investigation included questionnaire survey, physical examination, dual-energy X-ray absorptiometry (DXA) scan, blood samples collection and traditional Chinese medicine (TCM) inquiry. Findings to data A total of 5132 participants, aged 18 to 80 years, were recruited at baseline; among them, 38.7% were men and 64.8% were from the urban area. The mean age was 53.04 +/- 12.77 years. The completion rates of physical examination, DXA scan, blood collection and TCM inquiry were 99.9%, 98.5%, 99.9% and 96.5%, respectively. The mean body mass index (BMI) was 23.42 +/- 3.20 kg/m(2) with 8.1% of the study population being obese (BMI >= 28 kg/m(2)). The crude prevalence of hypertension, diabetes and metabolic syndrome were 34.9%, 10.0% and 30.4%, respectively. Future plans All participants will be monitored annually for cause-specific mortality and morbidity and hospital admission and will be followed up by in-person survey every 4 years. The baseline population is considered to expand in the future depending on the availability of funding support. Abstract:Background: The large and diverse All of Us Research Program offers tremendous opportunities for health research. However, results may not be generalizable to the US population due to the program's targeted recruitment efforts. Methods: We compared All of Us participants to those from the nationally representative 1999-2018 National Health and Nutrition Examination Survey (NHANES) with respect to: overall mortality rates; the distribution of sociodemographic, health-related, and clinical characteristics; the association of each characteristic with mortality estimated using Cox proportional-hazards models; and population attributable fractions (PAFs) for each characteristic and mortality. Results: All of Us participants were older, less likely to be Non-Hispanic White, had more years of education, and had a higher prevalence of major chronic conditions than NHANES. Mortality rates were generally lower for All of Us participants, especially at older ages. The direction of associations in All of Us and NHANES matched for almost all comparisons, but differed in magnitude for some conditions, primarily clinical diagnoses. For example, in All of Us, mortality among participants with a prevalent cancer diagnosis was 2.79 (95% CI: 2.59 to 3.01) times higher than among participants without cancer; in NHANES the hazard ratio was only 1.24 (95% CI: 1.16 to 1.33). PAFs were generally higher in All of Us. Conclusions: Predictors of mortality in All of Us do not consistently generalize to the US population. Analytical approaches are needed to address non-representativeness and mitigate potential biases associated with the selection into the All of Us cohort.

Abstract:Asian Americans (AAs) are the only racial/ethnic group in the United States for whom cancer is the leading cause of death. In AA women, the rate of breast cancer (BC) has significantly increased in the past two decades. However, the reasons behind the observed increase in risk are not well understood. The All of Us (AoU) Research Program aims to enroll one million participants nationwide, focusing on including individuals from diverse backgrounds, to advance precision health research. The goal of this pilot study was to evaluate the feasibility of using AoU data to analyze factors potentially associated with breast cancer risk in AAs and to develop enhanced breast cancer prediction models. There were 3,433 self-reported AA women with available genomic data. In addition to the genomic data, data was available from surveys, participants' electronic health records (EHRs), and physical measurements taken at the time of their study visit. BC cases were identified by self-report on surveys (n=38), EHR diagnosis data (n=104), or both survey and EHR (n=59), totaling 201 cases identified. Of these, only 17 were incident cases. Of the 163 cases detected in the EHR, 96.3% were invasive BCs, and the median age at diagnosis was 54.2 (±12.6) years. Among the 38 survey-only-identified cases, age at diagnosis was unclear due to the large age ranges (e.g.18-64 years) in the categories presented in the survey. For purposes of the case-control analysis, five AA women with no BC diagnosis were matched to each BC case by age group and geographic region. Body mass index, annual household income, marital status, history of alcohol consumption, and history of cigarette smoking and vaping were not statistically significantly different between cases and controls (a=0.05). One third (34.3%) of cases were born in the U.S. and 65.2% were born in another country vs. 25.0% and 74.0% of controls, respectively (p= 0.01). Employment status was also different, with 50.8% of cases being unemployed vs. 42.0% of controls (p=0.02). There appeared to be a difference in first-degree family history of BC (reported in 8% of cases vs. 5% of controls) although the results were not statistically significant (p=0.08). However, family history of BC data was not available for 43.3% and 51.0% of the cases and controls, respectively. Gynecologic factors, such as ages at menarche and menopause, and hormone use, were not asked in the surveys and were not widely captured in the EHR data. In our analysis of nine BC- associated genes (BRCA1, BRCA2, ATM, TP53, PTEN, CHEK2, CDH1, PALB2, STK11), pathogenic variants were identified in 1.0% of cases and 0.7% of controls. In summary, the AoU data set enables the study of a diverse national cohort with rich genomic data. However, the low representation of AAs, small numbers of incident cases, limited data on personal gynecologic history, cancer family history and social determinants of health, and broad categorization of age at diagnosis present challenges to using this data to build accurate and effective risk prediction models for BC risk in AAs. Abstract:OBJECTIVE:To investigate the health-related quality of life (HR-QOL) in African-American (AA) and white (W) obese women. RESEARCH METHODS AND PROCEDURES:Participants were 145 obese women (80 AA and 65 W; 87 premenopausal and 58 postmenopausal) who completed the Medical Outcomes Study short form, the Brief Symptom Inventory, the Life Distress Inventory, the Satisfaction With Life Scale, and the Rosenberg Self-Esteem Scale before entering a weight-loss study. The mean age of the subjects was 46.3 +/- 11.1 years and the mean body mass index was 35.2 +/- 4.2 kg/m(2). RESULTS:Although AA women were slightly heavier (95.3 +/- 10.3 kg vs. 91.5 +/- 11.6 kg, p < 0.05) and less educated (14.2 +/- 3.7 years vs. 15.7 +/- 3.7 years, p < 0.05) than the W women in the sample, there was no difference between the two ethnic groups in any of the reported HR-QOL variables. Menopausal status had a significant effect on HR-QOL, with premenopausal women being more distressed (p = 0.002), having more limitations in social activity (p = 0.007), and having less vitality (p < 0.001) than the postmenopausal women. This was especially true in the AA women. DISCUSSION:These data show no difference in HR-QOL between AA and W obese women and suggest that menopausal status may have an impact on HR-QOL, especially in AA women.

Abstract:Obesity is a major public health crisis associated with high mortality rates. Previous genome-wide association studies (GWAS) investigating body mass index (BMI) have largely relied on imputed data from European individuals. This study leveraged whole-genome sequencing (WGS) data from 88,873 participants from the Trans-Omics for Precision Medicine (TOPMed) Program, of which 51% were of non-European population groups. We discovered 18 BMI-associated signals (P < 5 × 10-9). Notably, we identified and replicated a novel low frequency single nucleotide polymorphism (SNP) in MTMR3 that was common in individuals of African descent. Using a diverse study population, we further identified two novel secondary signals in known BMI loci and pinpointed two likely causal variants in the POC5 and DMD loci. Our work demonstrates the benefits of combining WGS and diverse cohorts in expanding current catalog of variants and genes confer risk for obesity, bringing us one step closer to personalized medicine.

Data purporting to support a substituted enzyme mechanism have been analysed and shown to be ambiguous and equally consistent with a sequential mechanism. Suitable rate constants for a sequential mechanism involving the transient formation of the quaternary complex eIF-2.eIF-2B.GDP.GTP are suggested.

Abstract:Background: Although largewaist-height ratio(WHtR) is associated with increased cardiometabolic risks, its ethnic difference in adolescents is unclear. This study aimed to investigate the ethnic differences in cardiometabolic risk across the WHtR spectrum among adolescents.Methods: Data for this cross-sectional study were obtained from National Health and Nutrition Examination Surveys (2001-2014), and a total of 7385 participants aged 12-17 years were involved. The associations between WHtR and cardiometabolic outcomes measures were assessed with fractional polynomial regression and logistic regression models.Results: The risk of all cardiometabolic risk factors increased with increasing WHtR. However, significant ethnic differences in risk of adverse cardiometabolic outcomes were observed across the WHtR spectrum. At a WHtR of 0.5, compared with non-Hispanic (NH) whites, NH blacks revealed higher risks of elevated HbA1c, insulin resistance and hypertension, and lower risks of abnormal lipid profile, high glucose, elevated uric acid, and metabolic syndrome, with odds ratios (ORs) ranging from 1.36 (95% confidence interval: 1.17, 1.55) to 4.81 (2.94, 6.67) and 0.33 (0.23, 0.43) to 0.72 (0.53, 0.91), respectively. Additionally, significant differences in risks of abnormal lipid profile, elevated uric acid, abnormal glucose and insulin resistance were observed between Mexican Americans and NH whites as well.Conclusions: Significant ethnic disparities in cardiometabolic risk were observed in adolescents across the WHtR spectrum. Our findings pointed to opportunities to target particular cardiometabolic risk factors in specific ethnicities, and suggested the development of ethnicity-specific WHtR thresholds. Racial Differences and Temporal Obesity Trends in Heart Failure with Preserved Ejection Fraction

Abstract:Background/objectives: Obesity increases with age, is disproportionately prevalent in black populations, and is associated with heart failure with preserved ejection fraction (HFpEF). An "obesity paradox," or improved survival with obesity, has been reported in patients with HFpEF. The aim of this study was to examine whether racial differences exist in the temporal trends and outcomes associated with obesity among older patients with HFpEF. Design: Community surveillance of acute decompensated heart failure (ADHF) hospitalizations, sampled by stratified design from 2005 to 2014. Setting: Atherosclerosis Risk in Communities Study (NC, MS, MD, MN). Participants: A total of 10,147 weighted hospitalizations for ADHF (64% female, 74% white, mean age 77 years), with ejection fraction ≥50%. Measurements: ADHF classified by physician review, HFpEF defined by ejection fraction ≥50%. Body mass index (BMI) calculated from weight at hospital discharge. Obesity defined by BMI ≥30 kg/m2 , class III obesity by BMI ≥40 kg/m2 . Results: When aggregated across 2005-2014, the mean BMI was higher for black compared to white patients (34 vs 30 kg/m2 ; P < .0001), as was prevalence of obesity (56% vs 43%; P < .0001) and class III obesity (24% vs 13%; P < .0001). Over time, the annual mean BMI and prevalence of class III obesity remained stable for black patients, but steadily increased for white patients, with annual rates statistically differing by race (P-interaction = .04 and P = .03, respectively). For both races, a U-shaped adjusted mortality risk was observed across BMI categories, with the highest risk among patients with a BMI ≥40 kg/m2 . Conclusion: Black patients were disproportionately burdened by obesity in this decade-long community surveillance of older hospitalized patients with HFpEF. However, temporal increases in mean BMI and class III obesity prevalence among white patients narrowed the racial difference in recent years. For both races, the worst survival was observed with class III obesity. Effective strategies are needed to manage obesity in patients with HFpEF.

Abstract:Background: Obesity is more prevalent in minoritized groups, including Hispanics, American Indian (AI), and non-Hispanic Black (NHB), and these groups have higher renal cell carcinoma (RCC) incidence and mortality rates than non-Hispanic White (NHW). However, the role of abdominal adiposity in RCC disparities are still unknown. To address this gap, we investigated body composition variation and associations with RCC pathological characteristics accounting for neighborhood characteristics. Methods: Social vulnerability index (SVI) scores were linked to the zip code of patient's residence. Preoperative CT and MRI scans were obtained. SliceOmatic software was used to measure intermuscular, visceral, and subcutaneous adipose tissue and skeletal muscle areas and were normalized for height to obtain intermuscular, visceral, and subcutaneous adipose tissue index (IMATI, VATI, and SATI) and skeletal muscle index (SMI). Non-parametric tests were used to examine correlations, and logistic regression analyses were performed to obtain odds ratio (OR) and confidence interval (CI). Results: A total of 235 patients, including 70 (29.8%) Hispanic, 17 (7.2%) AI, and 20 (8.5%) NHB, were included. Obesity was more common in Hispanic (58.6%), AI (70.6%), and NHB (60.0%) than NHW (47.9%) patients. Compared to NHW patients, SATI was higher in Hispanic (P<0.001) and AI (P=0.002), and VATI was higher in Hispanic patients (P=0.04). Patients from minoritized racial and ethnic groups were more likely to come from neighborhoods with high SVI. SVI was significantly positively correlated with BMI (P=0.02) and SATI (P<0.001). Among SVI themes, socioeconomic, housing and transportation, and minority and language characteristics were significantly positively correlated with SATI (P<0.01). All body composition measurements were significantly positively correlate with BMI (P<0.001). Patients from minoritized racial and ethnic groups were also more likely to have hypertension and diabetes, and patients with both conditions had higher BMI (P=0.002), IMATI (P<0.001), VATI (P<0.001), and SATI (P=0.01) than patients without either condition. Obesity was associated with reduced odds of high grade RCC (OR 0.53, 95% CI 0.29-0.93). After including comorbidities in the model, association was no longer significant. In the same model, AI patients showed a trend for increased odds of high grade (OR 3.28, 95% CI 0.85-12.62, P=0.08) compared to NHW patients. AI patients also had significantly increased odds of having advanced stage RCC (OR 3.72, 95% CI 1.14-12.19). The highest quartile of IMATI significantly increased odds of advanced stage (OR 4.53, 95% CI 1.43-14.36) and clear cell subtype (OR 5.87, 95% CI 1.17-29.55) compared to the lowest quartile with significant P for trend (0.009 and 0.03 respectively). Abstract:OBJECTIVE: To analyze health care treatment experiences among a diverse cohort of reproductive-aged women from the All of Us study, focusing on variations due to body mass index (BMI). METHODS: We conducted a cross-sectional study that used data from the All of Us Research Program. We analyzed weight bias among reproductive-aged women aged 18–44 years. Weight bias was assessed using a proxy 7-item survey that evaluated discrimination in health care experiences, including courtesy, respect, service quality, being treated as competent, displaying fear, being treated as inferior, and being listened to during health care visits. RESULTS: Overall, 16,791 reproductive-aged women (mean [SD] age, 35.8 [6.3]) were included in the study. Of 11,592 patients with data available on BMI, 582 were Asian (5.0%), 1,984 (17.1%) were Hispanic or Latinx, 1,007 (8.7%) were non-Hispanic Black, and 7,370 (63.6%) were non-Hispanic White. Approximately 24.2% of participants (n=2,878) had obesity, and 11.1% of participants (n=1,292) had severe obesity. In general, participants in higher BMI categories had negative experiences in the health care setting. Women with severe obesity were 1.5 to 2 times more likely to report poor experiences with their health care clinicians compared with women with healthy weight. CONCLUSION: Results show significant disparities in the health care experiences among reproductive-aged women. As participants' BMI category increased, so did their negative experiences in the health care setting. These findings underscore the urgent need for targeted interventions to address these inequities. Health care systems must prioritize strategies to ensure that all individuals, regardless of weight or BMI, receive equitable and respectful care.

Abstract:Background: At least six communities with unusually good health and longevity have been identified, but their lifestyles aren't adopted widely. Informal evidence suggests that women associated with Universal Medicine (UM), a complementary medicine health care organization in Eastern Australia and the United Kingdom with normal lifestyles, also have several unusual health indicators. Objective: Our objective was to determine how UM participants compared with women in the Australian population at large on a variety of health indicators. Methods: In an Internet survey conducted July to September 2015, a total of 449 female UM participants from 15 countries responded to 43 health indicator questions taken from the Australian Longitudinal Study on Women's Health (ALSWH). Results: Survey responses revealed large positive differences in mental and physical health when compared with the ALSWH respondents, except for abnormal Pap test and low iron history. Differences and corresponding effect size estimates (Cohen d; ≥0.8 is a high difference, ≥0.5 a medium and ≥0.2 a small one with P<.001 except where indicated) included body mass index (BMI; 1.11), stress level (0.20, P=.006), depression (0.44), summary physical (0.31) and mental health (0.37), general mental health (0.39), emotional (0.15, P=.009) and social functioning (0.22), vitality (0.58), and general health (0.49), as well as lower incidences of diabetes, hypertension, and thrombosis (P<.001 each). Neither education levels nor country of residence had predictive value. Neither education levels nor country of residence had predictive value. Age did not predict BMI. Conclusions: The women's responses notably claim substantially lower levels of illness and disease than in the general Australian population. Abstract:OBJECTIVE: Some obese individuals appear to be protected from developing type 2 diabetes mellitus and cardiovascular disease (CVD). This has led to characterizing body size phenotypes based on cardiometabolic risk factors specifically as obese or overweight, and as metabolically healthy (MH) or metabolically abnormal (MA) based upon blood pressure, lipids, glucose homeostasis, and inflammatory parameters. The aim of this study was to measure the prevalence of and describe fat distribution across these phenotypes in a minority population.DESIGN AND METHODS: Hispanic participants (N = 1054) in the IRAS Family Study were categorized into different body size phenotypes. Computed tomography (CT) abdominal scans were evaluated for measures of nonalcoholic fatty liver disease (NAFLD) and abdominal fat distribution. Statistical models adjusting for familial relationships were estimated.RESULTS: Seventy percent (70%) of the Hispanic cohort was overweight (32%) or obese (38%). Forty-one percent (n = 138) of overweight participants and 19% (n = 74) of obese participants met criteria for MH. Adjusted analyses showed the MH phenotype was associated with lower visceral adipose tissue (VAT) and higher liver density (indicating lower fat content) in obese participants (p = 0.0005 and p = 0.0002, respectively), and lower VAT but not liver density in overweight participants (p = 0.008 and p = 0.162, respectively) compared to their MA counterparts. Odds of NAFLD were reduced in MH obese (OR = 0.34, p = 0.0007) compared to MA obese. VAT did not differ between MH obese or overweight and normal weight groups.CONCLUSIONS: These findings suggest that lower levels of visceral and liver fat, despite overall increased total body fat, may be a defining feature of MH obesity in Hispanic Americans.

Abstract:Background: Most research on obesity trends and projections has focused on changes across calendar years. However, as the risk of disease increases with cumulative exposure to obesity, it is crucial to characterize the obesity landscape through a life-course perspective and across birth cohorts. Objective: To enhance the accuracy of obesity epidemic projections throughout life course and across birth cohorts in the US. Design: Cross-sectional and cohort study. Setting: United States Participants: Individuals participated in three National Health Examination Surveys (NHES) from 1959 to 1970 and 18 National Health and Nutrition Examination Surveys (NHANES) from 1971 to 2020. Measurements: Body mass index (BMI) distributions by sex, race, and birth cohort. Results: By leveraging over 40 years of cross-sectional and longitudinal data from nationally representative surveys, we developed models to estimate historical and future BMI distributions in the US for both children and adults throughout their life course. We also calculated life-years of exposure to overweight and obesity, according to sex, race, and birth cohort. Our findings reveal significant increases in these metrics among birth cohorts since 1965 and highlight differential trends by sex and race for the 1965, 1985, and 2005 cohorts. Limitations: Assumption that model parameters will hold in the future. Conclusion: Our approach significantly expands upon previous models by projecting life course with continuous BMI distributions informed by longitudinal trajectories, explicitly accounting for variations in birth cohorts. Abstract:Background: Non-Hispanic Black (NHB) women experience disparities in cancer outcomes and mortality compared with non-Hispanic White (NHW) women. While social determinants of health contribute to these differences, investigating potential biological factors behind the observed racial differences is essential for developing effective health interventions. Therefore, we performed a comprehensive analysis comparing the metabolomic profiles of NHB and NHW women to identify whether differences exist in metabolites. Methods: We performed metabolomic profiling on 674 women recruited at Washington University School of Medicine, St. Louis, MO. A total of 1074 metabolites were profiled at Metabolon. We excluded 246 metabolites with missing values in ≥300 women from the analyses. We used the 10-nearest neighbor method for imputing missing data. We investigated the associations between metabolites and race using linear regression models, adjusting for age, body mass index, alcohol consumption, and education level. Metabolites were log-transformed before regression analyses, and linear coefficients were back-transformed to the original scale. A Bonferroni-adjusted p-value<10-5 was considered statistically significant. Pathway enrichment analyses were conducted for differential metabolites within each super-pathway. Results: Twenty-five (3.0%) of the 828 metabolites differed significantly between NHW and NHB women at the Bonferroni-adjusted p- value<10-5 level, with 19 metabolites higher in NHW women and 6 metabolites higher in NHB women. These metabolites belong to amino acid (n=12), xenobiotics (n=9), lipid (n=2), cofactors and vitamins (n=1), and partially characterized molecules (n=1) super-pathways. In NHB women, the top 3 metabolites with the highest percentage differences compared with NHW women were 2-naphthol sulfate (89.7%, p=3.6×10-6)[chemical], 2-hydroxyfluorene sulfate (71.6%, p=5.9×10-8)[tobacco metabolite], and carotene diol (3) (35.6%, p=1.1×10-6)[vitamin A metabolism]. Conversely, the top 3 metabolites with the highest percentage differences in NHW women compared with NHB women were N6-methyllysine (59.0%, p=1.2×10-26)[lysine metabolism], tryptophan betaine (57.7%, p=2.1×10-11)[tryptophan metabolism], and theophylline (52.8%, p=2.2×10-7)[xanthine metabolism]. Conclusions: We identified racial differences in several functional metabolites among women that may contribute to the observed cancer disparities and have implications for cancer prevention. Abstract:Background: The All of Us Research Program enrolls diverse US participants which provide a unique opportunity to better understand the problem of opioid use. This study aims to estimate the prevalence of opioid use and its association with sociodemographic characteristics from survey data and electronic health record (EHR). Methods: A total of 214,206 participants were included in this study who competed survey modules and shared EHR data. Adjusted logistic regressions were used to explore the associations between sociodemographic characteristics and opioid use. Results: The lifetime prevalence of street opioids was 4%, and the nonmedical use of prescription opioids was 9%. Men had higher odds of lifetime opioid use (aOR: 1.4 to 3.1) but reduced odds of current nonmedical use of prescription opioids (aOR: 0.6). Participants from other racial and ethnic groups were at reduced odds of lifetime use (aOR: 0.2 to 0.9) but increased odds of current use (aOR: 1.9 to 9.9) compared with non-Hispanic White participants. Foreign-born participants were at reduced risks of opioid use and diagnosed with opioid use disorders (OUD) compared with US-born participants (aOR: 0.36 to 0.67). Men, Younger, White, and US-born participants are more likely to have OUD. Conclusions: All of Us research data can be used as an indicator of national trends for monitoring the prevalence of receiving prescription opioids, diagnosis of OUD, and non-medical use of opioids in the US. The program employs a longitudinal design for routinely collecting health-related data including EHR data, that will contribute to the literature by providing important clinical information related to opioids over time. Additionally, this data will enhance the estimates of the prevalence of OUD among diverse populations, including groups that are underrepresented in the national survey data.
Differences in metabolomic profiles between Black and White women in the U.S.: Analyses from two prospective cohorts

Abstract:There is growing interest in incorporating metabolomics into public health practice. However, Black women are under-represented in many metabolomics studies. If metabolomic profiles differ between Black and White women, this under-representation may exacerbate existing Black-White health disparities. We therefore aimed to estimate metabolomic differences between Black and White women in the U.S. We leveraged data from two prospective cohorts: the Nurses' Health Study (NHS; n = 2077) and Women's Health Initiative (WHI; n = 2128). The WHI served as the replication cohort. Plasma metabolites (n = 334) were measured via liquid chromatography-tandem mass spectrometry. Observed metabolomic differences were estimated using linear regression and metabolite set enrichment analyses. Residual metabolomic differences in a hypothetical population in which the distributions of 14 risk factors were equalized across racial groups were estimated using inverse odds ratio weighting. In the NHS, Black-White differences were observed for most metabolites (75 metabolites with observed differences |0.50| standard deviations). Black women had lower average levels than White women for most metabolites (e.g., for N6, N6-dimethlylysine, mean Black-White difference = − 0.98 standard deviations; 95% CI: − 1.11, − 0.84). In metabolite set enrichment analyses, Black women had lower levels of triglycerides, phosphatidylcholines, lysophosphatidylethanolamines, phosphatidylethanolamines, and organoheterocyclic compounds, but higher levels of phosphatidylethanolamine plasmalogens, phosphatidylcholine plasmalogens, cholesteryl esters, and carnitines. In a hypothetical population in which distributions of 14 risk factors were equalized, Black-White metabolomic differences persisted. Most results replicated in the WHI (88% of 272 metabolites available for replication). Substantial differences in metabolomic profiles exist between Black and White women. Future studies should prioritize racial representation.

Abstract:Abstract This session will provide an overview of the All of Us Research Program (All of Us) dataset, discuss a mechanism to build the capacity of research institutions using the All of Us dataset, and outline the career benefits researchers experienced from using the dataset. All of Us is building a national health dataset to accelerate medical breakthroughs. It is used by cancer researchers to advance health research and career development. This dataset is large and diverse, both in the data types available as well as the demographics of participants: as of April 2023, health data from 413,450+ participants is available to researchers, from a variety of distinct data types, including survey responses, physical measurements, electronic health records, whole genome sequencing, wearable data, and genotyping arrays, with 75% of participants self-identifying as underrepresented in biomedical research and about 45% of participants self-identifying as racial and ethnic minorities. Researchers already leverage this dataset, with All of Us data used in more than 210 publications across peer-reviewed journals. In addition, this dataset is useful for cancer researchers - All of Us EHR data indicates over 60,700 participants are diagnosed with at least one type of cancer, and more than 10% of research projects underway (725/6,945 as of September 2023) using the All of Us dataset are cancer-related, making it the top condition studied by researchers. To help build capacity at research institutions, All of Us collaborated with the NCI Center to Reduce Cancer Health Disparities (CRCHD) Partnerships to Advance Cancer Health Equity (PACHE) to develop a Notice of Special Interest (NOSI) titled Administrative Supplements to Support "All of Us" and Health Disparities-Related Pilot Research Projects at NCI CRCHD-Funded PACHE Partnerships. The focus of this NOSI was on P20 and U54 awards associated with research projects led by PACHE researchers interested in exploring the connections between social determinants of health outcomes and populations experiencing cancer health disparities. NIH awarded investigators at the following institutions: City College of New York, University of California, Irvine, and Temple University. Abstract:Background Research examining the role of obesity in cardiovascular disease (CVD) often fails to adequately consider heterogeneity in obesity severity, distribution, and duration. Methods and Results We here use multivariate latent class mixed models in the biracial Atherosclerosis Risk in Communities study (N=14 514; mean age=54 years; 55% female) to associate obesity subclasses (derived from body mass index, waist circumference, self‐reported weight at age 25, tricep skinfold, and calf circumference across up to four triennial visits) with total mortality, incident CVD, and CVD risk factors. We identified four obesity subclasses, summarized by their body mass index and waist circumference slope as decline (4.1%), stable/slow decline (67.8%), moderate increase (24.6%), and rapid increase (3.6%) subclasses. Compared with participants in the stable/slow decline subclass, the decline subclass was associated with elevated mortality (hazard ratio [HR] 1.45, 95% CI 1.31, 1.60, P <0.0001) and with heart failure (HR 1.41, 95% CI 1.22, 1.63, P <0.0001), stroke (HR 1.53, 95% CI 1.22, 1.92, P =0.0002), and coronary heart disease (HR 1.36, 95% CI 1.14, 1.63, P =0.0008), adjusting for baseline body mass index and CVD risk factor profile. The moderate increase latent class was not associated with any significant differences in CVD risk as compared to the stable/slow decline latent class and was associated with a lower overall risk of mortality (HR 0.85, 95% CI 0.80, 0.90, P <0.0001), despite higher body mass index at baseline. The rapid increase latent class was associated with a higher risk of heart failure versus the stable/slow decline latent class (HR 1.34, 95% CI 1.10, 1.62, P =0.004). Conclusions Consideration of heterogeneity and longitudinal changes in obesity measures is needed in clinical care for a more precision‐oriented view of CVD risk.
