Abstract:Diffusional kurtosis imaging (DKI) is a methodology for measuring the extent of non-Gaussian diffusion in biological tissue, which has shown great promise in clinical diagnosis, treatment planning, and monitoring of many neurological diseases and disorders. However, robust, fast, and accurate estimation of kurtosis from clinically feasible data acquisitions remains a challenge. In this study, we first outline a new accurate approach of estimating mean kurtosis via the sub-diffusion mathematical framework. Crucially, this extension of the conventional DKI overcomes the limitation on the maximum b -value of the latter. Kurtosis and diffusivity can now be simply computed as functions of the sub-diffusion model parameters. Second, we propose a new fast and robust fitting procedure to estimate the sub-diffusion model parameters using two diffusion times without increasing acquisition time as for the conventional DKI. Third, our sub-diffusion-based kurtosis mapping method is evaluated using both simulations and the Connectome 1.0 human brain data. Exquisite tissue contrast is achieved even when the diffusion encoded data is collected in only minutes. In summary, our findings suggest robust, fast, and accurate estimation of mean kurtosis can be realised within a clinically feasible diffusion-weighted magnetic resonance imaging data acquisition time.

Abstract:Purpose Diffusional kurtosis imaging (DKI) measures the deviation of the displacement probability from a normal distribution, complementing the data commonly acquired by diffusion MRI. It is important to elucidate the sources of kurtosis contrast, particularly in biological tissues where microscopic kurtosis (intrinsic kurtosis) and diffusional heterogeneity may co‐exist. Methods We have developed a technique for microscopic kurtosis MRI, dubbed microscopic diffusional kurtosis imaging (µDKI), using a symmetrized double diffusion encoding (s‐DDE) EPI sequence. We compared this newly developed µDKI to conventional DKI methods in both a triple compartment phantom and in vivo. Results Our results showed that whereas conventional DKI and µDKI provided similar measurements in a compartment of monosphere beads, kurtosis measured by µDKI was significantly less than that measured by conventional DKI in a compartment of mixed Gaussian pools. For in vivo brain imaging, µDKI showed small yet significantly lower kurtosis measurement in regions of the cortex, CSF, and internal capsule compared to the conventional DKI approach. Conclusions Our study showed that µDKI is less susceptible than conventional DKI to sub‐voxel diffusional heterogeneity. Our study also provided important preliminary demonstration of our technique in vivo, warranting future studies to investigate its diagnostic use in examining neurological disorders.

Abstract:Diffusion Kurtosis Imaging (DKI) is more sensitive to microstructural differences and can be related to more specific micro-scale metrics (e.g., intra-axonal volume fraction) than diffusion tensor imaging (DTI), offering exceptional potential for clinical diagnosis and research into the white and gray matter. Currently DKI is acquired only at low spatial resolution (2-3 mm isotropic), because of the lower signal-to-noise ratio (SNR) and higher artifact level associated with the technically more demanding DKI. Higher spatial resolution of about 1 mm is required for the characterization of fine white matter pathways or cortical microstructure. We used restricted-field-of-view (rFoV) imaging in combination with advanced post-processing methods to enable unprecedented high-quality, high-resolution DKI (1.2 mm isotropic) on a clinical 3T scanner. Post-processing was advanced by developing a novel method for Retrospective Eddy current and Motion ArtifacT Correction in High-resolution, multi-shell diffusion data (REMATCH). Furthermore, we applied a powerful edge preserving denoising method, denoted as multi-shell orientation-position-adaptive smoothing (msPOAS). We demonstrated the feasibility of high-quality, high-resolution DKI and its potential for delineating highly myelinated fiber pathways in the motor cortex. REMATCH performs robustly even at the low SNR level of high-resolution DKI, where standard EC and motion correction failed (i.e., produced incorrectly aligned images) and thus biased the diffusion model fit. We showed that the combination of REMATCH and msPOAS increased the contrast between gray and white matter in mean kurtosis (MK) maps by about 35% and at the same time preserves the original distribution of MK values, whereas standard Gaussian smoothing strongly biases the distribution.

Abstract:Diffusion Imaging in Python (Dipy) is a free and open source software project for the analysis of data from diffusion magnetic resonance imaging (dMRI) experiments. dMRI is an application of MRI that can be used to measure structural features of brain white matter. Many methods have been developed to use dMRI data to model the local configuration of white matter nerve fiber bundles and infer the trajectory of bundles connecting different parts of the brain. Dipy gathers implementations of many different methods in dMRI, including: diffusion signal pre-processing; reconstruction of diffusion distributions in individual voxels; fiber tractography and fiber track post-processing, analysis and visualization. Dipy aims to provide transparent implementations for all the different steps of dMRI analysis with a uniform programming interface. We have implemented classical signal reconstruction techniques, such as the diffusion tensor model and deterministic fiber tractography. In addition, cutting edge novel reconstruction techniques are implemented, such as constrained spherical deconvolution and diffusion spectrum imaging (DSI) with deconvolution, as well as methods for probabilistic tracking and original methods for tractography clustering. Many additional utility functions are provided to calculate various statistics, informative visualizations, as well as file-handling routines to assist in the development and use of novel techniques. In contrast to many other scientific software projects, Dipy is not being developed by a single research group. Rather, it is an open project that encourages contributions from any scientist/developer through GitHub and open discussions on the project mailing list. Consequently, Dipy today has an international team of contributors, spanning seven different academic institutions in five countries and three continents, which is still growing.

Abstract:Diffusional Kurtosis Imaging (DKI) is a sensitive biomarker for microstructure in health and disease. However, DKI is not specific to any microstructural property since it may emerge from several different sources. Q-space trajectory encoding has been proposed for decoupling isotropic from anisotropic kurtosis. Still, this method assume that the system is comprised of multiple Gaussian diffusion components. Here, we develop a more general framework for resolving the underlying kurtosis sources. We introduce Correlation Tensor MRI (CTI) - an approach harnessing the versatility of double diffusion encoding (DDE) and capable of explicitly decoupling isotropic and anisotropic kurtosis components from intra-compartmental kurtosis effects arising from restricted diffusion. Additionally, CTI provides an index that is potentially sensitive to intra-compartmental kurtosis. The theoretical foundations of CTI, as well as the first proof-of-concept CTI ex vivo experiments in mouse brains at a field of 16.4T, are presented. We find that anisotropic and isotropic kurtosis can decouple microscopic anisotropy from substantial partial volume effects between tissue and free water. Our intra-compartmental kurtosis index exhibited positive values in both white and grey matter tissues. Simulations in different microenvironments show, however, that our current CTI protocol for estimating intra-compartmental kurtosis is limited by higher-order terms that were not considered in this study. CTI measurements were then extended to in vivo settings and used to map healthy rat brains at 9.4T. These in vivo CTI results were consistent with our ex vivo findings. Although future studies are still required to mitigate the higher-order effects on the intra-compartmental kurtosis index, results show that CTI's more general estimates of anisotropic and isotropic kurtosis contributions are already ripe for future studies.

Body diffusion kurtosis imaging: Basic principles, applications, and considerations for clinical practice

Abstract:Technologic advances enable performance of diffusion-weighted imaging (DWI) at ultrahigh b-values, where standard monoexponential model analysis may not apply. Rather, non-Gaussian water diffusion properties emerge, which in cellular tissues are, in part, influenced by the intracellular environment that is not well evaluated by conventional DWI. The novel technique, diffusion kurtosis imaging (DKI), enables characterization of non-Gaussian water diffusion behavior. More advanced mathematical curve fitting of the signal intensity decay curve using the DKI model provides an additional parameter Kapp that presumably reflects heterogeneity and irregularity of cellular microstructure, as well as the amount of interfaces within cellular tissues. Although largely applied for neural applications over the past decade, a small number of studies have recently explored DKI outside the brain. The most investigated organ is the prostate, with preliminary studies suggesting improved tumor detection and grading using DKI. Although still largely in the research phase, DKI is being explored in wider clinical settings. When assessing extracranial applications of DKI, careful attention to details with which body radiologists may currently be unfamiliar is important to ensure reliable results. Accordingly, a robust understanding of DKI is necessary for radiologists to better understand the meaning of DKI-derived metrics in the context of different tumors and how these metrics vary between tumor types and in response to treatment. In this review, we outline DKI principles, propose biostructural basis for observations, provide a comparison with standard monoexponential fitting and the apparent diffusion coefficient, report on extracranial clinical investigations to date, and recommend technical considerations for implementation in body imaging.
Robust frequency-dependent diffusion kurtosis computation using an efficient direction scheme, axisymmetric modelling, and spatial regularization

Abstract:Frequency-dependent diffusion MRI (dMRI) using oscillating gradient encoding and diffusion kurtosis imaging (DKI) techniques have been shown to provide additional insight into tissue microstructure compared to conventional dMRI. However, a technical challenge when combining these techniques is that the generation of the large b-values required for DKI is difficult when using oscillating gradient diffusion encoding. While efficient encoding schemes can enable larger b-values by maximizing multiple gradient channels simultaneously, they do not have sufficient directions to enable fitting of the full kurtosis tensor. Accordingly, we investigate a DKI fitting algorithm that combines axisymmetric DKI fitting, a prior that enforces the same axis of symmetry for all oscillating gradient frequencies, and spatial regularization, which together enable robust DKI fitting for a 10-direction scheme that offers double the b-value compared to traditional direction schemes. Using data from mice (oscillating frequencies of 0, 60, and 120 Hz) and humans (0 Hz only), we first show that axisymmetric modelling is advantageous over full kurtosis tensor fitting in terms of preserving contrast and reducing noise in DKI maps, and improved DKI map quality when using an efficient encoding scheme with averaging as compared to a traditional scheme with more encoding directions. We also demonstrate how spatial regularization during fitting preserves spatial features better than using Gaussian filtering prior to fitting, which is an oft-reported preprocessing step for DKI, and that enforcing consistent axes of symmetries across frequencies improves fitting quality. Thus, the use of an efficient 10-direction scheme combined with the proposed DKI fitting algorithm provides robust maps of frequency-dependent directional kurtosis parameters that can be used to explore novel biomarkers for various pathologies.

Abstract:Purpose Our study aimed to develop accelerated microscopic diffusional kurtosis imaging (μDKI) and preliminarily evaluated it in a rodent model of chronic epilepsy. Methods We investigated two μDKI acceleration schemes of reduced sampling density and angular range in a phantom and wild-type rats, and further tested μDKI method in pilocarpine-induced epilepsy rats using a 4.7 Tesla MRI. Single slice average μDapp and μKapp maps were derived, and Nissl staining was obtained. Results The kurtosis maps from two accelerated μDKI sampling schemes (sampling density and range) are very similar to that using fully sampled data (SSIM > 0.95). For the epileptic models, μDKI showed noticeably different contrast from those obtained with conventional DKI. Specifically, the average μKapp was significantly less than that of the average of Kapp (0.15 ± 0.01 vs. 0.47 ± 0.02) in the ventricle. Conclusions Our study demonstrated the feasibility of accelerated in vivo μDKI. Our work revealed that μDKI provides complementary information to conventional DKI method, suggesting that advanced DKI sequences are promising to elucidate tissue microstructure in neurological diseases.

Abstract:Diffusion kurtosis imaging (DKI) is a diffusion-weighted imaging method that describes non-Gaussian signal behavior using a relatively simple mathematical model. A parameter, kurtosis K, describes the deviation of the diffusion signal decay from a gaussian pattern. The deviation reflects the complexity of the tissue microstructure affecting water diffusion. Several studies have investigated the diagnostic performance of DKI in distinguishing malignant from benign breast lesions. DKI has been reported to correlate with subtypes and with several molecular and other factors related to the treatment and prognosis of breast cancer. Some technical considerations remain to be resolved for the clinical application of DKI in the breast. DKI, which increases the sensitivity to complex tissue microstructure compared to standard DWI, has been applied in the breast, allowing to increase clinical performance in distinguishing malignant from benign lesions and in predicting prognosis or treatment response in breast cancer.

Abstract:The displacement distribution of a water molecular is characterized mathematically as Gaussianity without considering potential diffusion barriers and compartments. However, this is not true in real scenario: most biological tissues are comprised of cell membranes, various intracellular and extracellular spaces, and of other compartments, where the water diffusion is referred to have a non-Gaussian distribution. Diffusion kurtosis imaging (DKI), recently considered to be one sensitive biomarker, is an extension of diffusion tensor imaging, which quantifies the degree of non-Gaussianity of the diffusion. This work proposes an efficient scheme of maximum likelihood estimation (MLE) in DKI: we start from the Rician noise model of the signal intensities. By augmenting a Von-Mises distributed latent phase variable, the Rician likelihood is transformed to a tractable joint density without loss of generality. A fast computational method, an expectation-maximization (EM) algorithm for MLE is proposed in DKI. To guarantee the physical relevance of the diffusion kurtosis we apply the ternary quartic (TQ) parametrization to utilize its positivity, which imposes the upper bound to the kurtosis. A Fisher-scoring method is used for achieving fast convergence of the individual diffusion compartments. In addition, we use the barrier method to constrain the lower bound to the kurtosis. The proposed estimation scheme is conducted on both synthetic and real data with an objective of healthy human brain. We compared the method with the other popular ones with promising performance shown in the results.

Abstract:<p>Multicenter magnetic resonance imaging is gaining more popularity in large-sample projects. Since both varying hardware and software across different centers cause unavoidable data heterogeneity across centers, its impact on reliability in study outcomes has also drawn much attention recently. One fundamental issue arises in how to derive model parameters reliably from image data of varying quality. This issue is even more challenging for advanced diffusion methods such as diffusion kurtosis imaging (DKI). Recently, deep learning–based methods have been demonstrated with their potential for robust and efficient computation of diffusion-derived measures. Inspired by these approaches, the current study specifically designed a framework based on a three-dimensional hierarchical convolutional neural network, to jointly reconstruct and harmonize DKI measures from multicenter acquisition to reformulate these to a state-of-the-art hardware using data from traveling subjects. The results from the harmonized data acquired with different protocols show that: 1) the inter-scanner variation of DKI measures within white matter was reduced by 51.5% in mean kurtosis, 65.9% in axial kurtosis, 53.7% in radial kurtosis, and 61.5% in kurtosis fractional anisotropy, respectively; 2) data reliability of each single scanner was enhanced and brought to the level of the reference scanner; and 3) the harmonization network was able to reconstruct reliable DKI values from high data variability. Overall the results demonstrate the feasibility of the proposed deep learning–based method for DKI harmonization and help to simplify the protocol setup procedure for multicenter scanners with different hardware and software configurations.</p>

Abstract:Magnetic resonance diffusion kurtosis imaging (DKI) is an emerging magnetic resonance imaging (MRI) technique that can reflect microstructural changes in tissue through non‐Gaussian diffusion of water molecules. Compared to traditional diffusion weighted imaging (DWI), the DKI model has shown greater sensitivity for diagnosis of musculoskeletal diseases and can help formulate more reasonable treatment plans. Moreover, DKI is an important auxiliary examination for evaluation of the motor function of the musculoskeletal system. This article briefly introduces the basic principles of DKI and reviews the application and research of DKI in the evaluation of disorders of the musculoskeletal system (including bone tumors, soft tissue tumors, spinal lesions, chronic musculoskeletal diseases, musculoskeletal trauma, and developmental disorders) as well as the normal musculoskeletal tissues. Abstract:Magnetic resonance imaging technique known as DWI (diffusion-weighted imaging) allows measurement of water diffusivity on a pixel basis for evaluating pathology throughout the body and is now routinely incorporated into many body MRI protocols, mainly in oncology. Indeed water molecules motion reflects the interactions with other molecules, membranes, cells, and in general the interactions with the environment. Microstructural changes as e.g. cellular organization and/or integrity then affect the motion of water molecules, and consequently alter the water diffusion properties measured by DWI. Then DWI technique can be used to extract information about tissue organization at the cellular level indirectly from water motion. In general the signal intensity in DWI can be quantified by using a parameter known as ADC (Apparent Diffusion Coefficient) emphasizing that it is not the real diffusion coefficient, which is a measure of the average water molecular motion. In the simplest models, the distribu- tion of a water molecule diffusing in a certain period of time is considered to have a Gaussian form with its width proportional to the ADC. However, water in biological structures often displays non-Gaussian diffusion behavior, consequently the DWI signal shows a more complex behavior that need to be modeled following different approaches. In this work we explore the possibility to quantify the degree to which water diffusion in biologic tissues is non-Gaussian introducing the AKC parameter (Apparent Kurtosis Coefficient). In this work we have realized DWI non-Gaussian diffusion maps to be used in the clinical routine along with standard ADC maps, giving to the radiologist another tool to explore how much structure inside a voxel is organized. In particular in this work some prostate DWI examples have been analyzed and will be shown.

Abstract:Background Diffusion kurtosis imaging (DKI) has the potential to provide microstructural insights into myelin and axonal pathology with additional kurtosis parameters. To our knowledge, few studies are available in the current literature using DKI by tract-based spatial statistics (TBSS) analysis in patients with multiple sclerosis (MS). The aim of this study is to assess the performance of commonly used parameters derived from DKI and diffusion tensor imaging (DTI) in detecting microstructural changes and associated pathology in relapsing remitting MS (RRMS). Methods Thirty-six patients with RRMS and 49 age and sex matched healthy controls underwent DKI. The brain tissue integrity was assessed by fractional anisotropy (FA), mean diffusivity (MD), axial diffusivity (Da), radial diffusivity (Dr), mean kurtosis (MK), axial kurtosis (Ka) and radial kurtosis (Kr) of DKI and FA, MD, Da and Dr of DTI. Group differences in these parameters were compared using TBSS ( P < 0.01, corrected). To compare the sensitivity of these parameters in detecting white matter (WM) damage, the percentage of the abnormal voxels based on TBSS analysis, relative to the whole skeleton voxels for each parameter was calculated. Results The sensitivities in detecting WM abnormality in RRMS were MK (78.2%) > Kr (76.7%) > Ka (53.5%) and Dr (78.8%) > MD (76.7%) > FA (74.1%) > Da (28.3%) for DKI, and Dr (79.8%) > MD (79.5%) > FA (68.6%) > Da (40.1%) for DTI. DKI-derived diffusion parameters (FA, MD, and Dr) were sensitive for detecting abnormality in WM regions with coherent fiber arrangement; however, the kurtosis parameters (MK and Kr) were sensitive to discern abnormalities in WM regions with complex fiber arrangement. Conclusions The diffusion and kurtosis parameters could provide complementary information for revealing brain microstructural damage in RRMS. Dr and DKI_Kr may be regarded as useful surrogate markers for reflecting pathological changes in RRMS.

Abstract:Background: Deviations from the regular pattern of growth and development could lead to early childhood diseases, suggesting the importance of evaluating early brain development. Through this study, we aimed to explore the changing patterns of white matter and gray matter during neonatal brain development using diffusion kurtosis imaging (DKI). Materials and methods: In total, 42 full-term neonates (within 28 days of birth) underwent conventional brain magnetic resonance imaging (MRI) and DKI. The DKI metrics (including kurtosis parameters and diffusion parameters) of white matter and deep gray matter were measured. DKI metrics from the different regions of interest (ROIs) were evaluated using the Kruskal–Wallis test and Bonferroni method. Spearman rank correlation analysis of the DKI metrics was conducted, and the age at the time of brain MRI acquisition was calculated. The subjects were divided into three groups according to their age at the time of brain MRI acquisition: the first group, neonates aged ≤7 days; the second group, neonates aged 8–14 days; and the third group, neonates aged 15–28 days. The rate of change in DKI metrics relative to the first group was computed. Results: The mean kurtosis (MK), axial kurtosis (Ka), radial kurtosis (Kr), and fractional anisotropy (FA) values showed positive correlations, whereas mean diffusion (MD), axial diffusion (Da), and radial diffusion (Dr) values showed negative correlations with the age at the time of brain MRI acquisition. The absolute correlation coefficients between MK values of almost all ROIs (except genu of the corpus callosum and frontal white matter) and the age at the time of brain MRI acquisition were greater than other metrics. The kurtosis parameters and FA values of central white matter were significantly higher than that of peripheral white matter, whereas the MD and Dr values were significantly lower than that of peripheral white matter. The MK value of the posterior limb of the internal capsule was the highest among the white matter areas. The FA value of the splenium of the corpus callosum was significantly higher than that of the other white matter areas. The kurtosis parameters and FA values of globus pallidus and thalamus were significantly higher than those of the caudate nucleus and putamen, whereas the Da and Dr values of globus pallidus and thalamus were significantly lower than those of the caudate nucleus and putamen. The relative change rates of kurtosis parameters and FA values of all ROIs were greater than those of MD, Da, and Dr values. The amplitude of MK values of almost all ROIs (except for the genu of the corpus callosum and central white matter of the centrum semiovale level) was greater than that of other metrics. The relative change rates of the Kr values of most ROIs were greater than those of the Ka value, and the relative change rates of the Dr values of most ROIs were greater than those of the Da value. Conclusion: DKI parameters showed potential advantages in detecting the changes in brain microstructure during neonatal brain development.

Abstract:PyDesigner is a Python-based software package based on the original Diffusion parameter EStImation with Gibbs and NoisE Removal (DESIGNER) pipeline (Dv1) for dMRI preprocessing and tensor estimation. This software is openly provided for non-commercial research and may not be used for clinical care. PyDesigner combines tools from FSL and MRtrix3 to perform denoising, Gibbs ringing correction, eddy current motion correction, brain masking, image smoothing, and Rician bias correction to optimize the estimation of multiple diffusion measures. It can be used across platforms on Windows, Mac, and Linux to accurately derive commonly used metrics from DKI, DTI, WMTI, FBI, and FBWM datasets as well as tractography ODFs and .fib files. It is also file-format agnostic, accepting inputs in the form of .nii, .nii.gz, .mif, and dicom format. User-friendly and easy to install, this software also outputs quality control metrics illustrating signal-to-noise ratio graphs, outlier voxels, and head motion to evaluate data integrity. Additionally, this dMRI processing pipeline supports multiple echo-time dataset processing and features pipeline customization, allowing the user to specify which processes are employed and which outputs are produced to meet a variety of user needs.

Abstract:Purpose The purpose of the study is to identify differences between axisymmetric diffusion kurtosis imaging (DKI) and standard DKI, their consequences for biophysical parameter estimates, and the protocol choice influence on parameter estimation. Methods Noise‐free and noisy, synthetic diffusion MRI human brain data is simulated using standard DKI for a standard and the fast "199" acquisition protocol. First the noise‐free "baseline" difference between both DKI models is estimated and the influence of fiber complexity is investigated. Noisy data is used to establish the signal‐to‐noise ratio at which the baseline difference exceeds noise variability. The influence of protocol choices and denoising is investigated. The five axisymmetric DKI tensor metrics (AxTM), the parallel and perpendicular diffusivity and kurtosis and mean of the kurtosis tensor are used to compare both DKI models. Additionally, the baseline difference is also estimated for the five parameters of the WMTI‐Watson model. Results The parallel and perpendicular kurtosis and all of the WMTI–Watson parameters had large baseline differences. Using a Westin or FA mask reduced the number of voxels with large baseline difference, that is, by selecting voxels with less complex fibers. For the noisy data, precision was worsened by the fast "199" protocol but adaptive denoising can help counteract these effects. Conclusion For the diffusivities and mean of the kurtosis tensor, axisymmetric DKI with a standard protocol delivers similar results as standard DKI. Fiber complexity is one main driver of the baseline differences. Using the "199" protocol worsens precision in noisy data but adaptive denoising mitigates these effects.
