Abstract:Acute compartment syndrome of the lower limb usually presents with a preceding history of limb trauma. 1 Spontaneous compartment syndrome of the lower limb is unusual. We present a case of compartment syndrome of the lower limb presenting as the initial sign of fulminant Streptococcal sepsis. A 55 year old man presents to our Emergency Department with a 1 day history of increasing left leg pain. There was no preceding history of trauma or factors predisposing to venous thrombosis. He had no other significant medical history. On examination, the patient was afebrile, normotensive but tachycardic (110 b.p.m). The anterior compartment of his left leg appeared tense compared to the posterior compartment and the compartments of the right leg. He had pain on passive and active dorsiflexion of his left ankle. There were no sensory or motor deficits and distal pedal pulses were present. Leg X-ray was normal. An ultrasound of the leg excluded venous thrombosis and septic collections. Using electrical transducers, the anterior and lateral compartment pressures of the left leg were measured as 76 and 75 mmHg, respectively. We decided to proceed with a fasciotomy of the anterior and lateral compartments of the left leg under general anaesthetic. On induction, the patient became profoundly hypotensive and required inotropic support. He was febrile (39.1 ° C) and developed truncal erythema. The anterior and lateral compartments were released through a single antero-lateral fasciotomy. The underlying muscles were dusky but viable. As the patient required significant inotropic support, he was left intubated and transferred to the intensive care unit. Broad spectrum antibiotics were commenced as treatment for septic shock of unknown cause. The following day, the patient developed multiorgan failure and disseminated intravascular coagulopathy. Blood cultures taken on admission and wound swabs taken during fasciotomy grew group A β -haemolytic Streptococcus (GABS). The patient’s wife recalled that the patient had a sore throat 5 days previously, which had resolved. In the intensive care unit, the fasciotomy was extended and the leg compartments irrigated. A further fasciotomy was performed on his lateral thigh to exclude necrotizing fasciitis. Arthroscopic washouts of both knees were performed and aspirates from the knees also grew GABS. Over the next week, the patient’s condition improved whereby he was discharged to the ward on day 10. Primary closure of the thigh wound was performed and a vacuum dressing was applied to the leg wound. He was discharged home on day 14 with community nursing follow up. Acute compartment syndrome of the lower limb refers to an increase in tissue pressure beyond vascular perfusion pressure within a closed fascial space (compartment) resulting in muscle ischaemia, necrosis, rhabdomyolysis and renal failure. 1–3 Common causes for acute compartment syndrome of the lower limb are trauma, reperfusion after vascular obstruction and following surgery in the lithotomy position. 1 Spontaneous lower limb compartment syndrome as the initial presenting sign of systemic sepsis is unusual, although it is well established that sepsis can cause increased tissue pressures due to release of endotoxins and inflammatory mediators. 2–4 Streptococcal infections commonly cause pharyngitis and cellulitis. 2,4 GABS has been reported to rarely (three cases in the literature since 1970) cause necrotizing limb gangrene, fasciitis, myositis and limb compartment syndrome associated with a toxic shock syndrome, involving refractory hypotension, multiorgan failure and coagulopathy. 2–4 Early diagnostic clues may include a preceding history of upper respiratory symptoms, skin cellulitis and purpura with pain out of proportion to inflammatory signs and indicators of systemic toxicity (fever, tachycardia, tachypnoea). 2,4 Often the disease progresses within hours from seemingly trivial limb symptoms in fit and young persons, often misdiagnosed as thrombophlebitis, cellulitis, myalgia, minor soft tissue injury to limb and life threatening conditions. 2–4 The presented case highlights that limb compartment syndrome presenting without any preceding history of trauma should prompt consideration of sepsis and early institution of a septic screen and appropriate broad spectrum antimicrobial therapy. In retrospect, it is likely that our patient’s untreated pharyngitis led to subsequent systemic streptococcal sepsis. However, broad-spectrum therapy is initially recommended as gram negative organisms have been reported to cause limb compartment syndromes in the early stages of systemic sepsis. 4 Furthermore, our case suggests that in the unconscious patient with septic shock, continued monitoring of the limbs are required such that early diagnosis and management of subsequent compartment syndromes in other limbs and/or compartments may be facilitated. The diagnosis of acute compartment syndrome is essentially a clinical diagnosis, incorporating a high index of suspicion and clinical findings of tense tissues and limb pain. 1,2 Motor, sensory and vascular deficits are late signs. 1 Measurement of compartment pressures is useful in equivocal cases but is complementary to clinical suspicion and should not delay definitive treatment. The present case highlights that compartment syndrome presenting without an obvious cause often leads to unnecessary investigations and delayed decompression. Treatment of acute compartment syndrome consists of urgent fasciotomy to reduce compartmental pressure. 1–4 Mortality and morbidity is directly related to time to fasciotomy. 1–4 Our case emphasizes that when performing fasciotomy for compartment syndrome without a preceding history of trauma, swabs and fluid should be sent for microscopy, culture and sensitivities in order to guide specific antimicrobial therapy. Acute limb compartment syndrome secondary to sepsis inevitably requires treatment in an intensive care unit as all reported cases in the literature developed severe systemic toxicity. 2–4 In our case, a high suspicion for compartment syndrome and supportive treatment in an intensive care unit resulted in a favourable outcome.
