Abstract:The objective of this study was to characterize the spatiotemporal dynamics and climatic and social-ecological risk factors associated with the largest dengue epidemic to date in Machala, Ecuador, to inform the development of a dengue EWS. The following data were included in analyses: neighborhood-level georeferenced dengue cases, national census data, and entomological surveillance data from 2010; time series of weekly dengue cases (aggregated to the city-level) and meteorological data from 2003 to 2012. We applied LISA and Morans I to analyze the spatial distribution of the 2010 dengue cases, and developed multivariate logistic regression models through a multi-model selection process to identify census variables and entomological covariates associated with the presence of dengue at the neighborhood level. Using data aggregated at the city-level, we conducted a time-series (wavelet) analysis of weekly climate and dengue incidence (2003-2012) to identify significant time periods (e.g., annual, biannual) when climate co-varied with dengue, and to describe the climate conditions associated with the 2010 outbreak. We found significant hotspots of dengue transmission near the center of Machala. The best-fit model to predict the presence of dengue included older age and female gender of the head of the household, greater access to piped water in the home, poor housing condition, and less distance to the central hospital. Wavelet analyses revealed that dengue transmission co-varied with rainfall and minimum temperature at annual and biannual cycles, and we found that anomalously high rainfall and temperatures were associated with the 2010 outbreak. Our findings highlight the importance of geospatial information in dengue surveillance and the potential to develop a climate-driven spatiotemporal prediction models to inform disease prevention and control interventions.

Abstract:The distribution and intensity of viral diseases transmitted by Aedes aegypti mosquitoes, including dengue, have rapidly increased over the last century. Here, we study dengue virus (DENV) transmission across the ecologically and demographically distinct regions or Ecuador. We analyzed province-level age-stratified dengue incidence data from 2000–2019 using catalytic models to estimate the force of infection of DENV over eight decades. We found that provinces established endemic DENV transmission at different time periods. Coastal provinces with the largest and most connected cities had the earliest and highest increase in DENV transmission, starting around 1980 and continuing to the present. In contrast, remote and rural areas with reduced access, like the northern coast and the Amazon regions, experienced a rise in DENV transmission and endemicity only in the last 10 to 20 years. The newly introduced chikungunya and Zika viruses have age-specific distributions of hospital-seeking cases consistent with recent emergence across all provinces. To evaluate factors associated with geographic differences in DENV transmission potential, we modeled the DENV vector risk using 11,693 Aedes aegypti presence points to the resolution of 1 hectare. In total, 56% of the population of Ecuador, including in provinces identified as having increasing DENV transmission in our models, live in areas with high risk of Aedes aegypti , with population size, trash collection, elevation, and access to water as important determinants. Our investigation serves as a case study of the changes driving the expansion of DENV and other arboviruses globally and suggest that control efforts should be expanded to semi-urban and rural areas and to historically isolated regions to counteract increasing dengue outbreaks. The factors driving the increasing burden of arboviruses like dengue virus are not fully understood. In this study, we measured changes in dengue virus transmission intensity and arbovirus disease risk across Ecuador, an ecologically and demographically diverse South American country. We found that differences in the distribution of dengue cases could be explained by changes in transmission of dengue virus over time: transmission was limited to coastal provinces with large cities between 1980–2000, expanding thereafter to higher elevation areas and ecologically suitable but previously geographically and socially isolated provinces. We also used species distribution mapping to show that both urban and rural areas in Ecuador are at medium to high risk for Aedes aegypti presence, with population size, trash removal, elevation, and access to water as strong predictors. Our investigation reveals changes driving the expansion of dengue virus and other arboviruses globally and provides an approach for identifying areas at early stages of establishing endemic transmission that should be targeted for intense preventative efforts to avert future epidemics.

Abstract:Dengue (DENV) transmission is endemic throughout coastal Ecuador, showing heterogeneous incidence patterns in association with fine-scale variation in Aedes aegypti vector populations and other factors. Here, we investigated the impact of micro-climate and neighbourhood-level variation in urbanization on Aedes abundance, resting behaviour and associations with dengue incidence in two endemic areas. Aedes aegypti were collected in Quinindé and Portoviejo, two urban cantons with hyperendemic dengue transmission in coastal Ecuador. Aedes vectors were sampled in and around houses within urban and peri-urban neighbourhoods at four time periods. We tested for variation in vector abundance and resting behaviour in relation to neighbourhood urbanization level and microclimatic factors. Aedes abundance increased towards the end of the rainy season, was significantly higher in Portoviejo than in Quinindé, and in urban than in peri-urban neighbourhoods. Aedes vectors were more likely to rest inside houses in Portoviejo but had similar abundance in indoor and outdoor resting collections in Quinindé. Over the study period, DENV incidence was lower in Quinindé than in Portoviejo. Relationships between weekly Ae . aegypti abundance and DENV incidence were highly variable between trapping methods; with positive associations being detected only between BG-sentinel and outdoor Prokopack collections. Aedes aegypti abundance was significantly higher in urban than peri-urban neighbourhoods, and their resting behaviour varied between study sites. This fine-scale spatial heterogeneity in Ae . aegypti abundance and behaviour could generate site-specific variation in human exposure and the effectiveness of indoor-based interventions. The trap-dependent nature of associations between Aedes abundance and local DENV incidence indicates further work is needed to identify robust entomological indicators of infection risk. Dengue transmission is hyperendemic in coastal Ecuador. However, there is limited understanding of how exposure to Aedes aegypti vectors varies at the neighbourhood-level and in response to local micro-climatic conditions. Here, we investigated how the abundance and resting behaviour (use of indoor versus peri-domestic sites) of Ae . aegypti varied within and between 2 coastal urban settings, Portoviejo and Quinindé cantons. Additionally we tested for associations between Aedes abundance and dengue case incidence. We found that the abundance of Ae . aegypti and their preference for indoor resting sites varied between the two study sites. Vectors were more abundant in urban than in peri-urban neighbourhoods at both sites. Associations between Ae . aegypti abundance and "city-wide" dengue incidence were highly variable depending on the trapping method used; with collections made in peri-domestic settings (BG sentinel traps and Prokopack aspiration) showing a positive association with incidence in contrast to indoor Prokopack aspirations. These results emphasize the need for fine-scale vector surveillance at the neighbourhood-level to identify hotspots of Aedes abundance and optimize vector control strategies.

A dengue outbreak in a rural community in Northern Coastal Ecuador: An analysis using unmanned aerial vehicle mapping

Abstract:Dengue is recognized as a major health issue in large urban tropical cities but is also observed in rural areas. In these environments, physical characteristics of the landscape and sociodemographic factors may influence vector populations at small geographic scales, while prior immunity to the four dengue virus serotypes affects incidence. In 2019, a rural northwestern Ecuadorian community, only accessible by river, experienced a dengue outbreak. The village is 2-3 hours by boat away from the nearest population center and comprises both Afro-Ecuadorian and Indigenous Chachi households. We used multiple data streams to examine spatial risk factors associated with this outbreak, combining maps collected with an unmanned aerial vehicle (UAV), an entomological survey, a community census, and active surveillance of febrile cases. We mapped visible water containers seen in UAV images and calculated both the green-red vegetation index (GRVI) and household proximity to public spaces like schools and meeting areas. To identify risk factors for symptomatic dengue infection, we used mixed-effect logistic regression models to account for the clustering of symptomatic cases within households. We identified 55 dengue cases (9.5% of the population) from 37 households. Cases peaked in June and continued through October. Rural spatial organization helped to explain disease risk. Afro-Ecuadorian (versus Indigenous) households experience more symptomatic dengue (OR = 3.0, 95%CI: 1.3, 6.9). This association was explained by differences in vegetation (measured by GRVI) near the household (OR: 11.3 95% 0.38, 38.0) and proximity to the football field (OR: 13.9, 95% 4.0, 48.4). The integration of UAV mapping with other data streams adds to our understanding of these dynamics.

Abstract:Aedes aegypti presence, human-vector contact rates, and Aedes -borne virus transmission are highly variable through time and space. The Lower Rio Grande Valley (LRGV), Texas, is one of the few regions in the U.S. where local transmission of Aedes -borne viruses occurs, presenting an opportunity to evaluate social, urbanistic, entomological, and mobility-based factors that modulate human exposure to Ae . aegypti . Mosquitoes were collected using BG-Sentinel 2 traps during November 2021 as part of an intervention trial, with knowledge, attitudes, and practices (KAP) and housing quality surveys to gather environmental and demographic data. Human blood samples were taken from individuals and a Bitemark Assay (ELISA) was conducted to quantify human antibodies to the Ae . aegypti Nterm-34kDa salivary peptide as a measure of human exposure to bites. In total, 64 houses were surveyed with 142 blood samples collected. More than 80% of participants had knowledge of mosquito-borne diseases and believed mosquitoes to be a health risk in their community. Our best fit generalized linear mixed effects model found four fixed effects contributed significantly to explaining the variation in exposure to Ae . aegypti bites: higher annual household income, younger age, larger lot area, and higher female Ae . aegypti abundance per trap night averaged over 5 weeks prior to human blood sampling. Most surveyed residents recognized mosquitoes and the threat they pose to individual and public health. Urbanistic (i.e., lot size), social (i.e., income within a low-income community and age), and entomological (i.e., adult female Ae . aegypti abundance) factors modulate the risk of human exposure to Ae . aegypti bites. The use of serological biomarker assays, such as the Bitemark Assay, are valuable tools for surveillance and risk assessment of mosquito-borne disease, especially in areas like the LRGV where the transmission of target pathogens is low or intermittent. Aedes aegypti is a mosquito vector with public health importance on the global scale as it transmits viruses such as dengue, chikungunya, and Zika. Although transmission rates of dengue and Zika are low in the U.S., there are a few regions, including south Texas, where local transmission has occurred. Our study aimed to evaluate the factors associated with risk of exposure to these viruses using a serological bioassay that measured antibody response to an Ae . aegypti salivary protein to assess human-vector contact. We collected mosquitoes, took human-blood samples, and conducted urbanistic and demographic surveys in November 2021 in eight communities in the Lower Rio Grande Valley, Texas. Our knowledge, attitude, and practices survey found that most residents recognized adult mosquitoes, though few individuals knew someone personally who been sick with a mosquito-borne disease. Outdoor adult female Ae . aegypti abundance was positively associated with exposure to mosquito bites. Household income, individual age, and lot area also significantly affected exposure levels. The Bitemark Assay we used in this study can be utilized as a tool for entomological risk assessment and could be used as an alternative to infection exposure in areas where mosquito-borne disease levels are low.

High prevalence of Zika virus infection in populations of Aedes aegypti from South-western Ecuador

Abstract:We performed an arboviral survey in mosquitoes from four endemic Ecuadorian cities (Huaquillas, Machala, Portovelo and Zaruma) during the epidemic period 2016–2018. Collections were performed during the pre-rainy season (2016), peak transmission season (2017) and post-rainy season (2018). Ae . aegypti mosquitoes were pooled by date, location and sex. Pools were screened by RT-PCR for the presence of ZIKV RNA, and infection rates (IRs) per 1,000 specimens were calculated. A total of 2,592 pools (comprising 6,197 mosquitoes) were screened. Our results reveal high IRs in all cities and periods sampled. Overall IRs among female mosquitoes were highest in Machala (89.2), followed by Portovelo (66.4), Zaruma (47.4) and Huaquillas (41.9). Among male mosquitoes, overall IRs were highest in Machala (35.6), followed by Portovelo (33.1), Huaquillas (31.9) and Zaruma (27.9), suggesting that alternative transmission routes (vertical/venereal) can play important roles for ZIKV maintenance in the vector population of these areas. Additionally, we propose that the stabilization of ZIKV vertical transmission in the vector population could help explain the presence of high IRs in field-caught mosquitoes during inter-epidemic periods. Zika virus (ZIKV) is a mosquito-borne virus native to Africa that can cause a wide range of symptoms in humans, including neurological complications and fetal abnormalities. ZIKV arrived to South America in 2015, and quickly spread through the continent. In Ecuador, this virus caused several thousand infections during 2016 and 2017, but its incidence dropped suddenly thereafter, with just 11 cases reported between 2018 and 2022. The reasons behind this abrupt decrease are not well understood. In this work, we performed an arboviral survey in mosquitoes from four endemic Ecuadorian cities during the period 2016–2018. We screened 2,592 pools (comprising 6,197 specimens) for the presence of ZIKV, and estimated infection rates in female and male mosquitoes. We found high infection rates in all cities and periods sampled. Considering the low incidence of human cases during 2018, our results suggest that in our study areas the virus could be maintained by mechanisms that either do not depend on human infection, or cause low rates of symptomatic infections. High rates of ZIKV infection in male mosquitoes found during our study suggest that vertical and/or venereal transmission routes could play important roles in the inter-epidemic maintenance of this pathogen in our study area.

Abstract:Background Viruses such as dengue virus (DENV), Zika virus (ZIKV), and chikungunya virus (CHIKV) pose major threats to human health, causing endemic, emerging, and reemerging diseases. These arboviruses have complex life cycles involving Aedes mosquitoes, driven by environmental, ecological, socioeconomic, and cultural factors. In Colombia, Aedes aegypti is the primary vector, but Aedes albopictus is expanding across the country. Understanding the unique characteristics of each species is crucial for managing arbovirus spread, particularly in areas where they coexist. Methodology/Principal Findings We conducted an entomological survey of A. aegypti and A. albopictus (larvae, pupae, and adults) in urban and rural areas of four municipalities across different elevations (200-2200 meters above sea level) in Colombia. Household conditions and knowledge of DENV were assessed through interviews. Female A. albopictus were tested individually for arbovirus RNA, while A. aegypti were tested in pools. Both species were found up to 2100 masl. A. aegypti comprised 78% of the immature forms collected, while A. albopictus made up 22%. Larvae from both species coexisted in common artificial breeding sites in urban and rural areas, with no evidence of competition. A. albopictus preferred rural areas, lower elevations (<1500 masl), high precipitation (>270 mm), and poor household conditions, while A. aegypti was more abundant in urban areas, intradomicile environments, and areas with moderate precipitation (100-400 mm). Biting risk was higher for A. aegypti (0.02-0.22 females per person), particularly in urban areas, while A. albopictus exhibited lower biting risk (0.001-0.08), with the highest values in rural Patia. Natural infections of DENV (12.4%) and CHIKV (12.4%) were found in A. aegypti, while A. albopictus showed CHIKV (41.2%) and DENV (23%) infections, with virus dissemination to the legs and salivary glands. Conclusions/Significance Integrating household conditions and community knowledge with environmental data can enhance predictive models for vector presence and guide surveillance and educational strategies. Our findings highlight the need to consider A. albopictus as a potentially significant arbovirus vector in Colombia, especially given the presence of arboviruses in its salivary glands, its use of artificial breeding sites, its biting risk inside homes, and its differing ecological preferences and seasonal associations compared to A. aegypti.

Abstract:Introduction: Aedes aegypti is the vector of several arboviruses such as dengue, Zika, and chikungunya. In 2015-16, Zika virus (ZIKV) had an outbreak in South America associated with prenatal microcephaly and Guillain-Barré syndrome. This mosquito's viral transmission is influenced by microbiota abundance and diversity and its interactions with the vector. The conditions of cocirculation of these three arboviruses, failure in vector control due to insecticide resistance, limitations in dengue management during the COVID-19 pandemic, and lack of effective treatment or vaccines make it necessary to identify changes in mosquito midgut bacterial composition and predict its functions through the infection. Its study is fundamental because it generates knowledge for surveillance of transmission and the risk of outbreaks of these diseases at the local level. Methods: Midgut bacterial compositions of females of Colombian Ae. aegypti populations were analyzed using DADA2 Pipeline, and their functions were predicted with PICRUSt2 analysis. These analyses were done under the condition of natural ZIKV infection and resistance to lambda-cyhalothrin, alone and in combination. One-step RT-PCR determined the percentage of ZIKV-infected females. We also measured the susceptibility to the pyrethroid lambda-cyhalothrin and evaluated the presence of the V1016I mutation in the sodium channel gene. Results: We found high ZIKV infection rates in Ae. aegypti females from Colombian rural municipalities with deficient water supply, such as Honda with 63.6%. In the face of natural infection with an arbovirus such as Zika, the diversity between an infective and non-infective form was significantly different. Bacteria associated with a state of infection with ZIKV and lambda-cyhalothrin resistance were detected, such as the genus Bacteroides, which was related to functions of pathogenicity, antimicrobial resistance, and bioremediation of insecticides. We hypothesize that it is a vehicle for virus entry, as it is in human intestinal infections. On the other hand, Bello, the only mosquito population classified as susceptible to lambda-cyhalothrin, was associated with bacteria related to mucin degradation functions in the intestine, belonging to the Lachnospiraceae family, with the genus Dorea being increased in ZIKV-infected females. The Serratia genus presented significantly decreased functions related to phenazine production, potentially associated with infection control, and control mechanism functions for host defense and quorum sensing. Additionally, Pseudomonas was the genus principally associated with functions of the degradation of insecticides related to tryptophan metabolism, ABC transporters with a two-component system, efflux pumps, and alginate synthesis. Conclusions: Microbiota composition may be modulated by ZIKV infection and insecticide resistance in Ae. aegypti Colombian populations. The condition of resistance to lambda-cyhalothrin could be inducing a phenome of dysbiosis in field Ae. aegypti affecting the transmission of arboviruses.
A numbers game: Mosquito-based arbovirus surveillance in two distinct geographic regions of Latin America

Abstract:mosquitoes, as vectors of medically important arthropod-borne viruses (arboviruses), constitute a major public health threat that requires entomological and epidemiological surveillance to guide vector control programs to prevent and reduce disease transmission. In this study, we present the collaborative effort of one year of mosquito-based arbovirus surveillance in two geographically distinct regions of Latin America (Nicaragua and Ecuador). Adult female mosquitoes were collected using backpack aspirators in over 2,800 randomly selected households (Nicaragua, Ecuador) and 100 key sites (Nicaragua) from eight distinct communities (Nicaragua: 2, Ecuador: 6). A total of 1,358 mosquito female pools were processed for RNA extraction and viral RNA detection using real-time RT-PCR. Ten positive dengue virus (DENV) pools were detected (3 in Nicaragua and 7 in Ecuador), all of which were found during the rainy season and matched the serotypes found in humans (Nicaragua: DENV-1 and DENV-4; Ecuador: DENV-2). Infection rates ranged from 1.13 to 23.13, with the Nicaraguan communities having the lowest infection rates. Our results demonstrate the feasibility of detecting DENV-infected mosquitoes in low-resource settings and underscore the need for targeted mosquito arbovirus sampling and testing, providing valuable insights for future surveillance programs in the Latin American region.

Abstract:Abstract Background Arbovirus diseases such as dengue, Zika, and chikungunya are a public health threat in tropical and subtropical areas. In the absence of a vaccine or specific treatment, vector management (in this case the control of the primary vector Aedes aegypti) is the best practice to prevent the three diseases. A good understanding of vector behaviour, ecology, human mobility and water use can help design effective vector control programmes. This study collected baseline information on these factors for identifying the arbovirus transmission risk and assessed the requirements for a large intervention trial in Colombia. Methods Baseline surveys were conducted in 5,997 households, randomly selected from 24 clusters (neighbourhoods with on average 2000 houses and 250 households inspected) in the metropolitan area of Cucuta, Colombia. The study established population characteristics including water management and mobility as well as larval-pupal indices which were estimated and compared in all clusters. Additionally, the study estimated disease incidence from two sources: self-reported dengue cases in the household survey and cases notified by the national surveillance system. Results In all 24 study clusters similar social and demographic characteristics were found but the entomological indicators and estimated disease incidence rates varied. The entomological indicators showed a high vector infestation: House Index = 25.1%, Container Index = 12.3% and Breteau Index = 29.6. Pupae per person Index (PPI) as an indicator of the transmission risk showed a large range from 0.22 to 2.04 indicating a high transmission risk in most clusters. The concrete ground tanks for laundry –mostly outdoors and uncovered- were the containers with the highest production of Ae. aegypti as 86.3% of all 17,613 pupae were identified in these containers. Also, the annual incidence of dengue was high: 841.6 self-reported cases per 100,000 inhabitants and the dengue incidence notified by the National surveillance system was 1,013.4 cases per 100,000 in 2019. Only 2.2% of the households used container water for drinking. 40.3% of the study population travelled during the day (when Aedes mosquitoes bite) outside their clusters. Conclusions The production of Ae. aegypti mosquitoes occurred almost exclusively in concrete ground tanks for laundry (lavadero), the primary intervention target. The baseline study provides necessary evidence for the design and implementation of a cluster randomized intervention trial in Colombia.

Abstract:The burden of arboviruses in the Americas is high and may result in long-term sequelae with infants disabled by Zika virus infection (ZIKV) and arthritis caused by infection with Chikungunya virus (CHIKV). We aimed to identify environmental drivers of arbovirus epidemics to predict where the next epidemics will occur and prioritize municipalities for vector control and eventual vaccination. We screened sera and urine samples (n = 10,459) from residents of 48 municipalities in the state of Rio de Janeiro for CHIKV, dengue virus (DENV), and ZIKV by molecular PCR diagnostics. Further, we assessed the spatial pattern of arbovirus incidence at the municipal and neighborhood scales and the timing of epidemics and major rainfall events. Lab-confirmed cases included 1,717 infections with ZIKV (43.8%) and 2,170 with CHIKV (55.4%) and only 29 (<1%) with DENV. ZIKV incidence was greater in neighborhoods with little access to municipal water infrastructure (r = -0.47, p = 1.2x10-8). CHIKV incidence was weakly correlated with urbanization (r = 0.2, p = 0.02). Rains began in October 2015 and were followed one month later by the largest wave of ZIKV epidemic. ZIKV cases markedly declined in February 2016, which coincided with the start of a CHIKV outbreak. Rainfall predicted ZIKV and CHIKV with a lead time of 3 weeks each time. The association between rainfall and epidemics reflects vector ecology as the larval stages of Aedes aegypti require pools of water to develop. The temporal dynamics of ZIKV and CHIKV may be explained by the shorter incubation period of the viruses in the mosquito vector; 2 days for CHIKV versus 10 days for ZIKV.

Abstract:The increase in incidence and geographical expansion of viruses transmitted by the Aedes mosquitoes, such as dengue (DENV) and zika (ZIKV) in the Americas, represents a burden for healthcare systems in tropical and subtropical regions. These and other under-detected arboviruses co-circulate in Costa Rica, adding additional complexity to their management due to their shared epidemiological behavior and similarity of symptoms in early stages. Since diagnostics of febrile illness is mostly based on clinical symptoms alone, we gathered acute-phase serum and urine from 399 samples of acute dengue-like cases from two healthcare facilities of Costa Rica, during an outbreak of arboviruses from July 2017 to May 2018, and tested them using molecular and serological methods. The analyses showed that of the clinically presumptive arbovirus cases that were reported, only 39.4% (n=153) of the samples were confirmed positive by RT-PCR to be DENV (DENV (10.3%), CHIKV (0.2%), ZIKV (27.3%), or mixed infections (1.5%). RT-PCR for other alphaviruses and flaviviruses, and PCR for Leptospira sp were negative. Furthermore, to assess flavivirus positivity in post-acute patients, the negative sera were tested against Dengue-IgM. 20% of sera were found positive, confounding even more the definitive number of cases, and emphasizing the need of several distinct diagnostic tools for accurate diagnostics. Molecular characterization of the prM and E genes from isolated viruses revealed that the American/Asian genotype of DENV-2 and the Asian lineage of ZIKV were circulating during this outbreak. Two different clades of DENV-2 American/Asian genotype were identified to co-circulate in the same region and a difference in the platelet and leukocyte count was noted between people infected with each clade, suggesting a putative distinct virulence. Our study sheds light on the necessity for healthcare strategies in managing arbovirus outbreaks, emphasizing the importance of comprehensive molecular and serological diagnostic approaches, as well as molecular characterization. This approach aids in enhancing our understanding of the clinical and epidemiological aspects of arboviral diseases during outbreaks. Our research highlights the need to strengthen training programs for health professionals and the need to increase research-based on laboratory evidence for diagnostic accuracy, guidance, development and implementation of public health interventions and epidemiological surveillance.

Abstract:The spread of arboviruses can be influenced by factors such as climate, socioeconomic and spatial distribution. The identification of populations at highest risk of exposure to arboviruses is an important topic in the public health scenario. Therefore, the present study is a review of the literature, which aimed to identify the association between the spatial distribution of Dengue Virus, Zika Virus and Chikungunya Virus and the social, environmental and economic aspects of individuals affected by arboviruses, the based on the analysis of published literature on the topic. Articles published in Portuguese, Spanish and English between 1997 and 2019 were included; and studies that addressed the clinic, infection, molecular biology, genetics, entomology and microbiology of viruses and mathematical modeling studies and reviews were excluded. The search for articles was carried out in the IBECS databases; CINAHL; MEDLINE; PubMed; Web of Science. After data mapping, PRISMA was used to extract the data. Therefore, of the 32 articles included in the analysis, the results showed that climatic factors such as temperature and precipitation affect the origin, evolution and distribution of vectors; and, consequently, the transmission of related pathogens. Furthermore, when considering socioeconomic aspects, populations from regions with greater social vulnerability are at greater risk of becoming ill from an arbovirus. Therefore, these results suggest that climatic factors and socioeconomic aspects are associated with the spatial distribution of arboviruses, making it essential to develop efficient control and prevention measures for arboviruses, especially in the most vulnerable regions.

Abstract:CONTEXT The diagnosis of focal segmental glomerulosclerosis (FSGS) is a descriptive pathologic diagnosis that in certain clinical situations (ie, primary or idiopathic) becomes its own disease. The clinical diversity, varied histology, and nonspecific morphologic features of FSGS all contribute to the complexity and problematic nature in making a pathologic diagnosis of FSGS. The definitions of the disease and of the morphologic features have evolved during the last century. OBJECTIVE To review historic and morphologic features of FSGS in order to demonstrate a practical approach in achieving a pathologic diagnosis of FSGS on kidney tissue. DATA SOURCES In 2004 a working proposal on the pathologic (morphologic) classification of FSGS was published in an attempt to unify the complexity of diagnosing FSGS, and it has shown to be both reproducible and with unique clinical implications for each defined FSGS variant. CONCLUSIONS An accurate diagnosis of FSGS can be challenging. During the last few decades, numerous new scientific discoveries have enriched our knowledge of pathogenetic mechanisms of nephrotic syndrome. Thus, it is expected there will be a need for a further modification to a morphologic classification and that the pathologist's role in diagnosing FSGS will remain in evolution. This review recapitulates the history of the pathologic diagnosis of FSGS and a current morphologic classification, hopefully opening up a discussion for further modifications that reflect the status of knowledge evolving in the 21st century.
Coronavirus awareness, confinement stress, and mental health: Evidence from Honduras, Chile, Costa Rica, Mexico and Spain

Abstract:Rationale: The purpose of the current study was to analyze the influence of coronavirus awareness, psychological stress responses, and sociodemographic variables on mental health indicators (somatization, depression, and anxiety) in residents of Honduras, Chile, Costa Rica, Mexico, and Spain. Methods: The study used a quantitative, cross-sectional approach. Data was collected online using the Brief Symptom Inventory-18 (BSI-18); the Coronavirus Awareness Scale-6 (CAS-6) and a questionnaire that included psychological and sociodemographic questions. The total sample size consisted of 1559 respondents from Honduras (34%), Chile (29%), Costa Rica (17%), Mexico (11%), and Spain (9%). Results: The most common stress domains correspond to family (22.97%), financial (22.53%), academic (16.47%), leisure time constraints (14.23%), health (12.48%), peer group (7.63%), and religious concerns (3.69%). These domains are significantly associated with the respondent's country, sex, employment status, and being or not a health worker. Respondents who reported confinement stress also reported higher scores in anxiety, depression, and somatization. The Global Severity Index was significantly predicted by confinement stress, health, academic, and leisure time-related stress, sex, age, being a health worker, COVID-19 Personal Concern, and Perceived Seriousness. Non-significant predictors were employment status, the number of people at home, presence of older adults and children at home, financial, peer group, family, and religious concerns; the regression model had an R2 of 0.26. Similar analyses were conducted for somatization, depression, and anxiety subscales. Conclusions: The COVID-19 pandemic has adverse effects on the mental health of the general population, particularly regarding anxiety, depression, and somatization. Specific populations, such as women and healthcare workers, are at particular risk of suffering a deterioration in mental wellbeing. The implications of the study for public policy are discussed.

Abstract:In this paper, an efficient method to characterize electrical memory effects arising from the interaction between the static nonlinearity and the frequency response is proposed using a multi-tone measurement. A transmitter system with nonlinear power amplifier (PA) is modeled with two box Wiener structure that consists of a static Saleh model and a linear filter. A nonlinear least square method with a multi-tone measurement is used to extract frequency response and nonlinear characteristics. The proposed method is verified with a simulation and experiment with a 10W class AB gallium-nitride (GaN) HEMT PA.

Abstract:In tropical areas, the simultaneous transmission of multiple vector-borne diseases is common due to ecological factors shared by arthropod vectors. Malaria and dengue virus, transmitted by Anopheles and Aedes mosquitoes, respectively, are among the top vector-borne diseases that cause significant morbidity and mortality in endemic areas. Notably, tropical areas often have suitable conditions for the co-existence of these mosquito species, highlighting the importance of identifying markers that accurately indicate the risk of acquiring each specific disease entity. Aedes are daytime-biting mosquitoes, while Anopheles preferentially bite during the night. These biting patterns raise the possibility of concurrent exposure to bites from both species. This is important because mosquito saliva, deposited in the skin during blood feeding, induces immune responses that modulate pathogen establishment and infection. Previous studies have focused on characterizing such effects on the vector–pathogen interface for an individual pathogen and its mosquito vector. In this study, we evaluated associations between immune responses to salivary proteins from non-dengue and non-malaria vector mosquito species with clinical characteristics of malaria and dengue, respectively. Surprisingly, antibody responses against Anopheles antigens in dengue patients correlated with red blood cell count and hematocrit, while antibody responses against Aedes proteins were associated with platelet count in malaria patients. Our data indicate that concurrent exposure to multiple disease-carrying mosquito vectors and their salivary proteins with differing immunomodulatory properties could influence the transmission, pathogenesis, and clinical presentation of malaria, dengue fever, and other vector-borne illnesses.

Abstract:Background: This study focuses on urban arboviruses, specifically dengue (DENV), chikungunya (CHIKV), and Zika (ZIKV), which pose a significant public health challenge in Rio de Janeiro state, Southeast Brazil. In our research, we highlight critical findings on the transmission dynamics of these arboviruses in Rio de Janeiro, identifying distinct patterns of disease spread. Methods: By combining genomic data with case reports from the Brazilian Ministry of Health, we have analysed the phylogenetics, prevalence and spatial distribution of these endemic viruses within the state. Findings: Our results revealed sustained DENV transmission primarily in the northern part of the state, a significant ZIKV epidemic in 2016 affecting all mesoregions, and two major CHIKV outbreaks in 2018 and 2019, predominantly impacting the northern and southern areas. Our analysis suggests an inverse relationship between arboviral case incidence and urban density, with less populous regions experiencing higher transmission rates, potentially attributed to a complex interplay of factors such as the efficacy of vector control measures, environmental conditions, local immunity levels, and human mobility. Furthermore, our investigation unveiled distinct age and gender trends among affected individuals. Notably, dengue cases were predominantly observed in young adults aged 32, while chikungunya cases were more prevalent among individuals over 41. In contrast, cases of ZIKV were concentrated around the 33-year age group. Intriguingly, females accounted for nearly 60% of the cases, suggesting a potential gender-based difference in infection rates. Interpretation: Our findings underscore the complexity of arbovirus transmission and the need for interventions tailored to different geographical mesoregions. Enhanced surveillance and genomic sequencing will be essential for a deeper, more nuanced understanding of regional arbovirus dynamics. Identifying potential blind spots within the state will be pivotal for developing and implementing more effective public health strategies, specifically designed to address the unique challenges posed by these viruses throughout the state. Abstract:BackgroundDengue is an increasingly important public health problem in most Latin American countries and more cost-effective ways of reducing dengue vector densities to prevent transmission are in demand by vector control programs. This multi-centre study attempted to identify key factors associated with vector breeding and development as a basis for improving targeted intervention strategies.MethodsIn each of 5 participant cities in Mexico, Colombia, Ecuador, Brazil and Uruguay, 20 clusters were randomly selected by grid sampling to incorporate 100 contiguous households, non-residential private buildings (businesses) and public spaces. Standardized household surveys, cluster background surveys and entomological surveys specifically targeted to obtain pupal indices for Aedes aegypti, were conducted in the dry and wet seasons.ResultsThe study clusters included mainly urban low-middle class populations with satisfactory infrastructure and –except for Uruguay- favourable climatic conditions for dengue vector development. Household knowledge about dengue and “dengue mosquitoes” was widespread, mainly through mass media, but there was less awareness around interventions to reduce vector densities. Vector production (measured through pupal indices) was favoured when water containers were outdoor, uncovered, unused (even in Colombia and Ecuador where the large tanks used for household water storage and washing were predominantly productive) and –particularly during the dry season- rainwater filled. Larval infestation did not reflect productive container types. All productive container types, including those important in the dry season, were identified by pupal surveys executed during the rainy season.ConclusionsA number of findings are relevant for improving vector control: 1) there is a need for complementing larval surveys with occasional pupal surveys (to be conducted during the wet season) for identifying and subsequently targeting productive container types; 2) the need to raise public awareness about useful and effective interventions in productive container types specific to their area; and 3) the motivation for control services that-according to this and similar studies in Asia- dedicated, targeted vector management can make a difference in terms of reducing vector abundance.
