Abstract:Objective. To evaluate biomechanics of the new-designed cage for lumbar interbody fusion by using FEMs analysis.Methods. Five validated finite element models of L4–L5 lumbar segments were established to simulate the intact lumbar segement, lumbar discectomy and lumbar discectomy instrumented posterior lumbar interbody fusion with single three kinds of cage (threaded cage, impacted cage and new expanded cage), and analyzed the biomechanics of these models.Results. The maximum ranges of motion and the maximum stress of facet joint were appeared in discectomy model, and the insertion of cages obviously decreased the range and the maximum stresses in all motions. However, only the ranges in expanded cage model were lower than those in intact model. Conclusions. The new-designed expanded cage resulted in highest stability and lowest stress of facet joint in fusion models.

Abstract:Background Adjacent segment disease (ASD) is a common complication after posterior lumbar interbody fusion (PLIF). Recently, a topping-off surgery (non-fusion with Coflex) has been developed to reduce the risk of ASD, yet whether and how the topping-off surgery can relieve ASD remains unclear. The purpose of this study was to explore the biomechanical effect of PLIF and Coflex on the adjacent segments via finite element (FE) analysis and discuss the efficacy of Coflex in preventing ASD. Methods A FE model of L3–L5 segments was generated based on the CT of a healthy volunteer via three commercially available software. Coflex and PLIF devices were modeled and implanted together with the segment model in the FE software. In the FE model, a pre-compressive load of 500 N, equal to two-thirds of the human body mass, was applied on the top surface of the L3. In addition, four types of moments (anteflexion, rear protraction, bending, and axial rotation) set as 10 Nm were successively applied to the FE model combined with this pre-compressive load. Then, the range of motion (ROM), the torsional rigidity, and the maximum von Mises equivalent stress on the L3–L4 intervertebral disc and the implant were analyzed. Results Both Coflex and PLIF reduced ROM. However, no significant difference was found in the maximum von Mises equivalent stress of adjacent segment disc between the two devices. Interestingly enough, both systems increased the torsional rigidity at the adjacent lumbar segment, and PLIF had a more significant increase. The Coflex implant had a larger maximum von Mises equivalent stress. Conclusions Both Coflex and PLIF reduced ROM at L3–L4, and thus improved the lumbar stability. Under the same load, both devices had almost the same maximum von Mises equivalent stress as the normal model on the adjacent intervertebral disc. But it is worthy to notice the torsional rigidity of PLIF was higher than that of Coflex, indicating that the lumbar treated with PLIF undertook a larger load to reach ROM of Coflex. Therefore, we presumed that ADS was related to a higher torsional rigidity.
Finite element analysis comparing a PEEK posterior fixation device versus pedicle screws for lumbar fusion

Abstract:Background: Pedicle screw loosening and breakage are common causes of revision surgery after lumbar fusion. Thus, there remains a continued need for supplemental fixation options that offer immediate stability without the associated failure modes. This finite element analysis compared the biomechanical properties of a novel cortico-pedicular posterior fixation (CPPF) device with those of a conventional pedicle screw system (PSS). Methods: The CPPF device is a polyetheretherketone strap providing circumferential cortical fixation for lumbar fusion procedures via an arcuate tunnel. Using a validated finite element model, we compared the stability and load transfer characteristics of CPPF to intact conditions under a 415 N follower load and PSS conditions under a 222 N preload. Depending on the instrumented levels, two different interbody devices were used: a lateral lumbar interbody device at L4-5 or an anterior lumbar interbody device at L5-S1. Primary outcomes included range of motion of the functional spinal units and anterior load transfer, defined as the total load through the disk and interbody device after functional motion and follower load application. Results: Across all combinations of interbody devices and lumbar levels evaluated, CPPF consistently demonstrated significant reductions in flexion (ranging from 90 to 98%), extension (ranging from 88 to 94%), lateral bending (ranging from 75 to 80%), and torsion (ranging from 77 to 86%) compared to the intact spine. Stability provided by the CPPF device was comparable to PSS in all simulations (range of motion within 0.5 degrees for flexion-extension, 0.6 degrees for lateral bending, and 0.5 degrees for torsion). The total anterior load transfer was higher with CPPF versus PSS, with differences across all tested conditions ranging from 128 to 258 N during flexion, 89-323 N during extension, 135-377 N during lateral bending, 95-258 N during torsion, and 82-250 N during standing. Conclusion: Under the modeled conditions, cortico-pedicular fixation for supplementing anterior or lateral interbody devices between L4 and S1 resulted in comparable stability based on range of motion measures and less anterior column stress shielding based on total anterior load transfer measures compared to PSS. Clinical studies are needed to confirm these finite element analysis findings.
Biomechanical Analysis of Adjacent Segments after Spinal Fusion Surgery Using a Geometrically Parametric Patient-Specific Finite Element Model

Abstract:This study aimed to perform a mechanical analysis of adjacent segments after spinal fusion surgery using a geometrically parametric patient-specific finite element model to elucidate the mechanism of adjacent segment degeneration (ASD), thereby providing theoretical evidence for early disease prevention. Fourteen parameters based on patient-specific spinal geometry were extracted from a patient's preoperative computed tomography (CT) scan, and the relative positions of each spinal segment were determined using the image match method. A preoperative patient-specific model of the spine was established through the above method. The postoperative model after L4-L5 posterior lumbar interbody fusion (PLIF) surgery was constructed using the same method except that the lamina and intervertebral disc were removed, and a cage, 4 pedicle screws, and 2 connecting rods were inserted. Range of motion (ROM) and stress changes were determined by comparing the values of each anatomical structure between the preoperative and postoperative models. The overall ROM of the lumbar spine decreased after fusion, while the ROM, stress in the facet joints, and stress in the intervertebral disc of adjacent segments all increased. An analysis of the stress distribution in the annulus fibrosus, nucleus pulposus, and facet joints also showed that not only was the maximum stress in these tissues elevated, but the areas of moderate-to-high stress were also expanded. During torsion, the stress in the facet joints and annulus fibrosus of the proximal adjacent segment (L3-L4) increased to a larger extent than that in the distal adjacent segment (L5-S1). While fusion surgery causes an overall restriction of motion in the lumbar spine, it also causes more load sharing by the adjacent segments to compensate for the fused segment, thus increasing the risk of ASD. The proximal adjacent segment is more prone to degeneration than the distal adjacent segment after spinal fusion due to the significant increase in stress.
Biomechanical Analysis of Stand-alone Lateral Lumbar Interbody Fusion for Lumbar Adjacent Segment Disease

Abstract:Study design Biomechanical cadaveric study Objective To compare biomechanical properties of a single stand-alone interbody fusion and a single-level pedicle screw construct above a previous lumbar pedicle fusion. Summary of background data Adjacent segment disease (ASD) is spondylosis of adjacent vertebral segments after previous spinal fusion. Despite the consensus that ASD is clinically significant, the surgical treatment of ASD is controversial. Methods Lateral lumbar interbody fusion (LLIF) and posterior spinal fusion (PSF) with pedicle screws were analyzed within a validated cadaveric lumbar fusion model. L3-4 vertebral segment motion was analyzed within the following simulations: without implants (intact), L3-4 LLIF-only, L3-4 LLIF with previous L4-S1 PSF, L3-4 PSF with previous L4-S1 PSF, and L4-S1 PSF alone. L3-4 motion values were measured during flexion/extension with and without axial load, side bending, and axial rotation. Results L3-4 motion in the intact model was found to be 4.7 ± 1.2 degrees. L3-4 LLIF-only decreased motion to 1.9 ± 1.1 degrees. L3-4 LLIF with previous L4-S1 fusion demonstrated less motion in all planes with and without loading (p < 0.05) compared to an intact spine. However, L3-4 motion with flexion/extension and lateral bending was noted to be greater compared to the L3-S1 construct (p < 0.5). The L3-S1 PSF construct decreased motion to less than 1° in all planes of motion with or without loading (p < 0.05). The L3-4 PSF with previous L4-S1 PSF constructs decreased the flexion/extension motion by 92.4% compared to the intact spine, whereas the L3-4 LLIF with previous L4-S1 PSF constructs decreased motion by 61.2%. Conclusions Stand-alone LLIF above a previous posterolateral fusion significantly decreases motion at the adjacent segment, demonstrating its utility in treating ASD without necessitating revision. The stand-alone LLIF is a biomechanically sound option in the treatment of ASD and is advantageous in patient populations who may benefit from less invasive surgical options.

Abstract:Abstract Background Adjacent segment disease (ASD) is a well-known complication after interbody fusion. Pedicle screw-rod revision possesses sufficient strength and rigidity. However, is a surgical segment with rigid fixation necessary for ASD reoperation? This study aimed to investigate the biomechanical effect of different instrumentation on lateral lumbar interbody fusion (LLIF) for ASD treatment. Methods A validated L2~5 finite element (FE) model was modified for simulation. ASD was considered the level cranial to the upper-instrumented segment (L3/4). Bone graft fusion in LLIF with bilateral pedicle screw (BPS) fixation occurred at L4/5. The ASD segment for each group underwent a) LLIF + posterior extension of BPS, b) PLIF + posterior extension of BPS, c) LLIF + lateral screw, and d) stand-alone LLIF. The L3/4 range of motion (ROM), interbody cage stress and strain, screw-bone interface stress, cage-endplate interface stress, and L2/3 nucleus pulposus of intradiscal pressure (NP-IDP) analysis were calculated for comparisons among the four models. Results All reconstructive models displayed decreased motion at L3/4. Under each loading condition, the difference was not significant between models a and b, which provided the maximum ROM reduction (73.8 to 97.7% and 68.3 to 98.4%, respectively). Model c also provided a significant ROM reduction (64.9 to 77.5%). Model d provided a minimal restriction of the ROM (18.3 to 90.1%), which exceeded that of model a by 13.1 times for flexion-extension, 10.3 times for lateral bending and 4.8 times for rotation. Model b generated greater cage stress than other models, particularly for flexion. The maximum displacement of the cage and the peak stress of the cage-endplate interface were found to be the highest in model d under all loading conditions. For the screw-bone interface, the stress was much greater with lateral instrumentation than with posterior instrumentation. Conclusions Stand-alone LLIF is likely to have limited stability, particularly for lateral bending and axial rotation. Posterior extension of BPS can provide reliable stability and excellent protective effects on instrumentation and endplates. However, LLIF with the use of an in situ screw may be an alternative for ASD reoperation.

Abstract:Objective: An alternative to conventional posterior lumbar interbody fusion (PLIF) is a PLIF with transpedicular oblique screw fixation system. An assessment of new fixation system's viability and efficacy is conducted through a comparison of its biomechanical properties with those of conventional PLIF. Method: A comprehensive finite element model (FEM) of the lumbar regions L1-L5 was developed and the surgical segment L3-L4 was chosen to comprise the surgical models of both traditional PLIF and new PLIF. In new PLIF model, an analysis was conducted on segmental range of motion (RoM), cage stress, inferior endplates stress, vertebral stress, and internal fixation stress. Three-dimensional printers are utilized to fabricate and assemble the fusion cage and vertebrae, and compression test machines are employed to execute physiological load and extreme load experiments on new PLIF, so as to verify the accuracy of the FEM analysis and the mode of fatigue exhibited by new PLIF. Results: In new PLIF, the maximum stress on the inferior endplates under physiological loads was reduced in comparison to conventional PLIF. While the maximum stress on the cage, vertebral body, and screw increased, it remained within an acceptable range. The experimental data indicates that new fixation system can endure a vertical load exceeding 2800 N and an ultimate bending moment of 77 Nm. Conclusion: The new PLIF exhibits a comparable RoM to its predecessor, simultaneously mitigating inferior endplate stress and accommodating physiological loads, which reduce the amount of surgical incision and fusion fixation instruments. Consequently, it emerges as a sanguine surgical approach to fuse the degenerative lumbar spine.
Effects of dynamic and rigid implantation on biomechanical characteristics of different sagittal alignment lumbar after single- or double-level spinal fixations: a finite-element modeling study

Abstract:Background: Although it is critical to understand the accelerated degeneration of adjacent segments after fusion, the biomechanical properties of the spine have not been thoroughly studied after various fusion techniques. This study investigates whether four Roussouly's sagittal alignment morphotypes have different biomechanical characteristics after different single- or double-level spinal fixations. Methods: The parametric finite element (FE) models of Roussouly's type (1-4) were developed based on the radiological data of 625 Chinese community population. The four Roussouly's type models were reassembled into four fusion models: single-level L4-5 Coflex fixation model, single-level L4-5 Fusion (pedicle screw fixation) model, double-level Coflex (L4-5) + Fusion (L5-S1) model, and double-level Fusion (L4-5) + Fusion (L4-5) model. A pure moment of 7.5 Nm was applied to simulate the physiological activities of flexion, extension, lateral bending and axial rotation. Results: Both single-level and double-level spinal fixation had the greatest effect on lumbar range of motion, disc pressure, and annulus fibrosis stress in flexion, followed by lateral bending, extension, and axial rotation. In all models, the upper adjacent segment was the most influenced by the implantation and bore the most compensation from the fixed segment. For Type 2 lumbar, the L4-L5 Coflex effectively reduced the disc pressure and annulus fibrosis stress in adjacent segments compared to the L4-L5 Fusion. Similarly, the L4-L5 Coflex offered considerable advantages in preserving the biomechanical properties of adjacent segments for Type 1 lumbar. For Type 4 lumbar, the L4-L5 Coflex did not have superiority over the L4-L5 Fusion, resulting in a greater increase in range of motion at adjacent segments in flexion and extension. The difference between the two fixations was not apparent in Type 3 lumbar. Compared to the single-level Fusion, the changes in motion and mechanics of the lumbar increased after both the double-level Coflex + Fusion and Fusion + Fusion fixations, while the differences between two double-level fixation methods on adjacent segments of the four lumbar models were similar to that of the single-level fixation. Conclusion: Type 3 and Type 4 lumbar have good compensatory ability and therefore allow for a wider range of surgical options, whereas surgical options for small lordotic Type 1 and Type 2 lumbar are more limited and severe.

Abstract:Background: Posterior long spinal fusion was the common procedure for adult spinal deformity (ASD). Although the application of sacropelvic fixation (SPF), the incidence of pseudoarthrosis and implant failure is still high in long spinal fusion extending to lumbosacral junction (LSJ). To address these mechanical complications, advanced SPF technique by multiple pelvic screws or multirod construct has been recommended. This was the first study to compare the biomechanical performance of combining multiple pelvic screws and multirod construct to other advanced SPF constructs for the augmentation of LSJ in long spinal fusion surgery through finite element (FE) analysis. Methods: An intact lumbopelvic FE model based on computed tomography images of a healthy adult male volunteer was constructed and validated. The intact model was modified to develop five instrumented models, all of which had bilateral pedicle screw (PS) fixation from L1 to S1 with posterior lumbar interbody fusion and different SPF constructs, including No-SPF, bilateral single S2-alar-iliac (S2AI) screw and single rod (SS-SR), bilateral multiple S2AI screws and single rod (MS-SR), bilateral single S2AI screw and multiple rods (SS-MR), and bilateral multiple S2AI screws and multiple rods (MS-MR). The range of motion (ROM) and stress on instrumentation, cages, sacrum, and S1 superior endplate (SEP) in flexion (FL), extension (EX), lateral bending (LB), and axial rotation (AR) were compared among models. Results: Compared with intact model and No-SPF, the ROM of global lumbopelvis, LSJ, and sacroiliac joint (SIJ) was decreased in SS-SR, MS-SR, SS-MR, and MS-MR in all directions. Compared with SS-SR, the ROM of global lumbopelvis and LSJ of MS-SR, SS-MR, and MS-MR further decreased, while the ROM of SIJ was only decreased in MS-SR and MS-MR. The stress on instrumentation, cages, S1-SEP, and sacrum decreased in SS-SR, compared with no-SPF. Compared with SS-SR, the stress in EX and AR further decreased in SS-MR and MS-SR. The most significantly decreased ROM and stress were observed in MS-MR. Conclusion: Both multiple pelvic screws and multirod construct could increase the mechanical stability of LSJ and reduce stress on instrumentation, cages, S1-SEP, and sacrum. The MS-MR construct was the most adequate to reduce the risk of lumbosacral pseudarthrosis, implant failure, and sacrum fracture. This study may provide surgeons with important evidence for the application of MS-MR construct in the clinical settings.

Abstract:To investigate the biomechanical changes in the proximal adjacent segment with different grades of degeneration after posterior lumbar interbody fusion (PLIF). We created three finite element models of the L3–5 with different grades of degeneration (healthy, mild, and moderate) at the L3–4 that were developed by changing the disc height and material properties of the nucleus pulposus. The L4–5 were operated by interbody fusion cages and pedicle screws. All models were loaded with a compressive pre-load of 400 N and a bending moment of 10 N a in three planes to recreate flexion, extension, lateral bending, and axial rotation. The range of motion (ROM), nucleus pressure, and annulus fibrosus pressure of the L3–4 were evaluated. The ROM, nucleus pressure, and annulus fibrosus pressure increased at the L3–4 after PLIF. As the degeneration progressed in the L3–4, the ROM of the L3–4 decreased while the nucleus pressure and annulus fibrosus pressure increased. Adjacent segment degeneration (ASD) may be related to the ROM and intradiscal pressure after PLIF. However, as the degeneration of the proximal adjacent segment increases, the ROM in the proximal adjacent segment gradually decreases, but the pressure on the nucleus pulposus and annulus fibrosus gradually increases. Degeneration of the proximal adjacent segment before operation is a risk factor for the ASD after PLIF.

Abstract:Lumbar spinal fusion may cause adjacent segment degeneration (ASD) in the long term. Recently, inserting an interspinous process device (IPD) proximal to the fusion has been proposed to prevent ASD. The aim of this study was to investigate the biomechanics of lumbar fusion with proximal IPD implantation (LFPI) under both static loads and whole body vibration (WBV). A previously validated finite element (FE) model of the L1-5 lumbar spine was modified to simulate L4-5 fusion. Three different IPDs (Coflex-F, Wallis and DIAM) were inserted at the L3-4 segment of the fusion model to construct the LFPI models. The intact and surgical FE models were analyzed under static loads and WBV, respectively. Under static loading conditions, LFPI decreased range of motion (ROM) and intradiscal pressure (IDP) at the transition segment L3-4 compared with the fusion case. At the segment (L2-3) adjacent to the transition level, LFPI induced higher motion and IDP than rigid fusion. Under WBV, vibration amplitudes of the L3-4 IDP and L4-5 facet joint force (FJF) decreased by more than 54.3% after surgery. The LFPI model with the DIAM system offered the most comparable biomechanics to the intact model under static loads, and decreased the dynamic responses of the L4-5 FJF under WBV. The LFPI model with the Wallis and Coflex-F systems could stabilize the transition segment, and decrease dynamic responses of the L3-4 IDP. The DIAM system may be more suitable in LFPI.
Biomechanical Comparison of Multilevel Lumbar Instrumented Fusions in Adult Spinal Deformity According to the Upper and Lower Fusion Levels: A Finite Element Analysis

Abstract:Multilevel lumbar fusion with posterior pedicle screw fixation is a widely performed surgical procedure for the management of adult spinal deformity. However, there has not been a comprehensive biomechanical study on the different types of fusion levels in terms of stability and possible complications. We aimed to investigate the biomechanical properties of multilevel lumbar fusion according to different types of upper and lower fusion levels. Six different types of fusions were performed using three-dimensional finite element models. Type A and B referred to the group of which upper fusion level was L1 and T10, respectively. Subtype 1, 2, and 3 referred to the group of which lower fusion level was L5, S1, and ilium, respectively (A1, L1-L5; A2, L1-S1; A3, L1-ilium; B1, T10-L5; B2, T10-S1; B3, T10-ilium). Flexion, extension, axial rotation, and lateral bending moments were applied, and the risk of screw loosening and failure and adjacent segment degeneration (ASD) was analyzed. Stress at the bone-screw interface of type B3 was lowest in overall motions. The risk of screw failure showed increasing pattern as the upper and lower levels extended in all motions. Proximal range of motion (ROM) increased as the lower fusion level changed from L5 to S1 and the ilium. For axial rotation, type B3 showed higher proximal ROM (16.2°) than type A3 (11.8°). In multilevel lumbar fusion surgery for adult spinal deformity, adding iliac screws and increasing the fusion level to T10-ilium may lower the risk of screw loosening. In terms of screw failure and proximal ASD, however, T10-ilium fusion has a higher potential risk compared with other fusion types. These results will contribute for surgeons to provide adequate patient education regarding screw failure and proximal ASD, when performing multilevel lumbar fusion.
Quantitative Biomechanical Evaluation for Optimal Spinal Instrumentation to Prevent Mechanical Complications in Spinal Fusion from the Lower Thoracic Spine to the Pelvis for Adult Spinal Deformity: A Finite Element Analysis

Abstract:Introduction: Mechanical complications, such as rod fracture (RF) and proximal junctional kyphosis (PJK), commonly occur after adult spinal deformity (ASD) surgery. A rigid construct is preferred to prevent RF, whereas it is a risk factor for PJK. This controversial issue urged us to conduct a biomechanical study for seeking the optimal construct to prevent mechanical complications.Methods: A three-dimensional nonlinear finite element model, which consisted of the lower thoracic and lumbar spine, pelvis, and femur, was created. The model was instrumented with pedicle screws (PSs), S2-alar-iliac screws, lumbar interbody fusion cages, and rods. Rod stress was measured when a forward-bending load was applied at the top of the construct to evaluate the risk of RF in constructs with or without accessory rods (ARs). In addition, fracture analysis around the uppermost instrumented vertebra (UIV) was performed to assess the risk of PJK.Results: Changing the rod material from titanium alloy (Ti) to cobalt chrome (CoCr) decreased shearing stress at L5-S1 by 11.5%, and adding ARs decreased it by up to 34.3% (for the shortest ARs). Although the trajectory (straightforward vs. anatomical) of PSs did not affect the fracture load for UIV+1, changing the anchor from PSs to hooks at the UIV reduced it by 14.8%. Changing the rod material from Ti to CoCr did not alter the load, whereas the load decreased by up to 25.1% as the AR became longer.Conclusions: The PSs at the UIV in the lower thoracic spine, CoCr rods as primary rods, and shorter ARs should be used in long fusion for ASD to prevent mechanical complications.

Abstract:BACKGROUND:Anterior lumbar interbody fusion combined with supplementary fixation has been widely used to treat lumbar diseases. However, few studies have investigated the influence of fixation options on facet joint force and cage subsidence. The aim of this study was to explore the biomechanical performance of anterior lumbar interbody fusion with various fixation options under both static and vertical vibration loading conditions.METHODS:A previously validated finite element model of the intact L1-5 lumbar spine was employed to compare five conditions: (1) Intact; (2) Fusion alone; (3) Fusion combined with anterior lumbar plate; (4) Fusion combined with Coflex-F fixation; (5) Fusion combined with bilateral pedicle screw fixation. The models were analyzed under static and vertical vibration loading conditions respectively.FINDINGS:Bilateral pedicle screws provided highest stability at surgical level. Applying supplementary fixation diminished the dynamic responses of lumbar spine. Compared with anterior lumbar plate and Coflex-F device, bilateral pedicle screws decreased the stress responses of the endplates and cage under both static and vibration conditions, while increased the facet joint force at adjacent levels. As for comparison between Coflex-F device and anterior lumbar plate, results showed a similarity in biomechanical performance under static loading, and a slightly higher dynamic response of the latter under vertical vibration.INTERPRETATION:The biomechanical performance of lumbar spine was significantly influenced by the variation of fixations under both static and vibration conditions. Bilateral pedicle screws showed advantages in stabilizing surgical segment and relieving cage subsidence, but may increase the facet joint force at adjacent levels.

Abstract:BACKGROUND:Lateral lumbar interbody fusion using cage supplemented with fixation has been used widely in the treatment of lumbar disease. A combined fixation (CF) of lateral plate and spinous process plate may provide multiplanar stability similar to that of bilateral pedicle screws (BPS) and may reduce morbidity. The biomechanical influence of the CF on cage subsidence and facet joint stress has not been well described. The aim of this study was to compare biomechanics of various fixation options and to verify biomechanical effects of the CF. METHODS:The surgical finite element models with various fixation options were constructed based on computed tomography images. The lateral plate and posterior spinous process plate were applied (CF). The 6 motion modes were simulated. Range of motion (ROM), cage stress, endplate stress, and facet joint stress were compared. RESULTS:For the CF model, ROM, cage stress, and endplate stress were the minimum in almost all motion modes. Compared with BPS, the CF reduced ROM, cage stress, and endplate stress in all motion modes. The ROM was reduced by more than 10% in all motion modes except for flexion; cage stress and endplate stress were reduced more than 10% in all motion modes except for rotation-left. After interbody fusion, facet joint stress was reduced substantially compared with the intact conditions in all motion modes except for flexion. CONCLUSIONS:The combined plate fixation may offer an alternative to BPS fixation in lateral lumbar interbody fusion.
Comparison of the Effect of Posterior Lumbar Interbody Fusion with Pedicle Screw Fixation and Interspinous Fixation on the Stiffness of Adjacent Segments

Abstract:BACKGROUND:Adjacent segment degeneration could seriously affect the long-term prognosis of lumbar fusion. Dynamic fixation such as the interspinous fixation, which is characterized by retaining the motion function of the spinal segment, has obtained satisfactory short-term effects in the clinical setting. But there are few reports about the biomechanical experiments on whether dynamic fixation could prevent adjacent segment degeneration. METHODS:The surgical segments of all 23 patients were L4/5. Thirteen patients with disc herniation of L4/5 underwent Wallis implantation surgery, and 10 patients with spinal stenosis of L4/5 underwent posterior lumbar interbody fusion (PLIF). L3-S1 segmental stiffness and displacement were measured by a spine stiffness gauge (SSG) device during surgery when the vertebral plate was exposed or during spinal decompression or internal fixation. Five fresh, frozen cadavers were used in the self control experiment, which was carried out in four steps: exposure of the vertebral plate, decompression of the spinal canal, implantation of a Wallis fixing device, and PLIF of L4/5 after removing the Wallis fixing device. Then, L3-S1 segment stiffness was measured by an SSG device. RESULTS:The experiments showed that the average stiffness of the L4/5 segment was (37.1 ± 8.9) N/mm after exposure of the vertebral plate, while after spinal decompression, the average stiffness fell to (26.2 ± 7.1) N/mm, decreasing by 25.8% (P < 0.05). For the adjacent segments L3/4 and L5/S1, their stiffness showed no significant difference between the L4/5 segment decompression and the exposure of the vertebral plate (P > 0.05). After Wallis implantation of L4/5, the stiffness of the cephalic adjacent segment L3/4 was (45.8 ± 10.7) N/mm, which was 20.5% more than that after the exposure of the vertebral plate (P < 0.05); after L4/5 PLIF surgery, the stiffness of L3/4 was (35.3 ± 10.7) N/mm and was decreased by 12.4% more than that after the exposure of the vertebral plate (P < 0.05). The stiffness of the cephalic adjacent segment L3/4 after fixation in the Wallis group was significantly higher than that of the PLIF group (P < 0.05). Cadaver experiments showed that the stiffness of the cephalic adjacent segment in the Wallis group was significantly higher than that of the PLIF group after L4/5 segment fixation (P < 0.05); the stiffness of the L5/S1 segment showed no significant difference between PLIF surgery and Wallis implantation (P > 0.05). CONCLUSIONS:After interspinous (Wallis) fixation, the stiffness of the cephalic adjacent segment increased. After PLIF with pedicle screw fixation, the stiffness of the cephalic adjacent segment decreased. An interspinous fixation system (Wallis) has a protective effect for cephalic adjacent segments for the immediate post-operative state.
Preserving Posterior Complex Can Prevent Adjacent Segment Disease Following Posterior Lumbar Interbody Fusion Surgeries: A Finite Element Analysis

Abstract:To investigate the biomechanical effects of the lumbar posterior complex on the adjacent segments after posterior lumbar interbody fusion (PLIF) surgeries.A finite element model of the L1-S1 segment was modified to simulate PLIF with total laminectomy (PLIF-LAM) and PLIF with hemilaminectomy (PLIF-HEMI) procedures. The models were subjected to a 400N follower load with a 7.5-N.m moment of flexion, extension, torsion, and lateral bending. The range of motion (ROM), intradiscal pressure (IDP), and ligament force were compared.In Flexion, the ROM, IDP and ligament force of posterior longitudinal ligament, intertransverse ligament, and capsular ligament remarkably increased at the proximal adjacent segment in the PLIF-LAM model, and slightly increased in the PLIF-HEMI model. There was almost no difference for the ROM, IDP and ligament force at L5-S1 level between the two PLIF models although the ligament forces of ligamenta flava remarkably increased compared with the intact lumbar spine (INT) model. For the other loading conditions, these two models almost showed no difference in ROM, IDP and ligament force on the adjacent discs.Preserved posterior complex acts as the posterior tension band during PLIF surgery and results in less ROM, IDP and ligament forces on the proximal adjacent segment in flexion. Preserving the posterior complex during decompression can be effective on preventing adjacent segment degeneration (ASD) following PLIF surgeries.
Biomechanical Comparison of Unilateral and Bilateral Pedicle Screw Fixation after Multilevel Lumbar Lateral Interbody Fusion

Abstract:Study design: Human Cadaveric Biomechanical Study. Objectives: Lumbar Lateral Interbody Fusion (LLIF) utilizing a wide cage has been reported as having favorable biomechanical characteristics. We examine the biomechanical stability of unilateral pedicle screw and rod fixation after multilevel LLIF utilizing 26 mm wide cages compared to bilateral fixation. Methods: Eight human cadaveric specimens of L1-L5 were included. Specimens were attached to a universal testing machine (MTS 30/G). Three-dimensional specimen range of motion (ROM) was recorded using an optical motion-tracking device. Specimens were tested in 3 conditions: 1) intact, 2) L1-L5 LLIF (4 levels) with unilateral rod, 3) L1-L5 LLIF with bilateral rods. Results: From the intact condition, LLIF with unilateral rod decreased flexion-extension by 77%, lateral bending by 53%, and axial rotation by 26%. In LLIF with bilateral rods, flexion-extension decreased by 83%, lateral bending by 64%, and axial rotation by 34%. Comparing unilateral and bilateral fixation, LLIF with bilateral rods reduced ROM by a further 23% in flexion-extension, 25% in lateral bending, and 11% in axial rotation. The difference was statistically significant in flexion-extension and lateral bending (P < .005). Conclusions: Considerable decreases in ROM were observed after multilevel (4-level) LLIF utilizing 26 mm cages supplemented with both unilateral and bilateral pedicle screws and rods. The addition of bilateral fixation provides a 10-25% additional decrease in ROM. These results can inform surgeons of the incremental biomechanical benefit when considering unilateral or bilateral posterior fixation after multilevel LLIF.

Abstract:Candida arthritis is an uncommon cause of infectious arthritis that may occur in seriously ill or immunosuppressed patients. This report describes two patients, one who developed Candida tropicalis arthritis and another patient who developed C. parapsilosis arthritis. One patient developed nephrogenic diabetes insipidus secondary to amphotericin B therapy and was successfully treated with intravenous miconazole. The other was unsuccessfully treated with both intraarticular and intravenous amphotericin B.
