Factors associated with acute postoperative pain in patients with primary total knee arthroplasty under enhanced recovery after surgery

Abstract:Background:Optimal pain management was the cornerstone of total knee arthroplasty (TKA) in enhanced re-covery after surgery (ERAS), but the related factors of acute pain after TKA remains unknown. Objective:To explore the re-lated factors of acute pain after ERAS TKA. Methods:From December 2015 to February 2016, a total of 82 patients treated by TKA were consecutively enrolled. The information about gender, age, body mass index (BMI), KSS score, ASA score, VAS score, operative time, whether or not to use a tourniquet, and hospital stay was recorded and multivariate logistic was used to test the related risk factors. Results:The VAS scores at rest were 2.7±0.6, 2.5±0.8 and 2.2±0.4 in the first three days after surgery, respectively. The VAS scores in the moving state were 3.9±1.4, 4.0±1.1 and 3.4±0.7, respectively. The average hospitalization time was (3.8±0.3) d, and the satisfaction ratio of patient was 96.3%±4.7%. The result of univariate analysis showed that the related factors were preoperative VAS score, the use of opioid drugs, the use of a tourniquet, and operation time. Furthermore, the multivariate logistic regression analysis showed that preoperative VAS score (OR=1.712, P=0.048), the use of opioid drugs (OR=2.015, P=0.011), the use of a tourniquet (OR=2.549, P=0.001) and operation time (OR=2.052, P=0.008) were the related risk factors of postoperative pain. Conclusions:Preoperative VAS scores, the use of opioid drugs, the use of tourniquet and operation time may be the related risk factors of acute pain after TKA under the ERAS model.
