Abstract:BACKGROUND: Remote monitoring technologies are positioned to mitigate the problem of a dwindling care workforce and disparities in access to care for the growing older immigrant population in the United States. To achieve these ends, designers and providers need to understand how these supports can be best provided in the context of various sociocultural environments that shape older adults' expectations and care relationships, yet few studies have examined how the same remote monitoring technologies may produce different effects and uses depending on what population is using them in a particular context.OBJECTIVE: This study aimed to examine the experiences and insights of low-income, immigrant senior residents, family contacts, and staff of housing that offered a sensor-based passive monitoring system designed to track changes in movement around the home and trigger alerts for caregivers. The senior housing organization had been offering the QuietCare sensor system to its residents for 6 years at the time of the study. We are interested in adoption and discontinuation decisions and use over time, rather than projected acceptance. Our research question is how do cultural differences influence use and experiences with this remote monitoring technology? The study does not draw generalizable conclusions about how cultural groups interact with a given technology, but rather, it examines how values are made visible in elder care technology interactions.METHODS: A total of 41 participants (residents, family, and staff) from 6 large senior housing independent living apartment buildings were interviewed. Interviews were conducted in English and Korean with these participants who collectively had immigrated to the United States from 10 countries.RESULTS: The reactions of immigrant older adults to the passive monitoring system reveal that this tool offered to them was often mismatched with their values, needs, and expectations. Asian elders accepted the intervention social workers offered largely to appease them, but unlike their US-born counterparts, they adopted reluctantly without hope that it would ameliorate their situation. Asian immigrants discontinued use at the highest rate of all residents, and intergenerational family cultural conflict contributed to this termination. Social workers reported that none of the large population of Russian-speaking residents agreed to use QuietCare. Bilingual and bicultural social workers played significant roles as cultural navigators in the promotion of QuietCare to residents.CONCLUSIONS: This research into the interactions of culturally diverse people with the same monitoring technology reveals the significant role that social values and context play in shaping how people and families interact with and experience elder care interventions. If technology-based care services are to reach their full potential, it will be important to identify the ways in which cultural values produce different uses and responses to technologies intended to help older adults live independently.

Abstract:Integration of smartphone technology with the patient call-bell system provides the opportunity to enhance patient safety by supporting nurses' ability to communicate and prioritize care delivery directly. However, challenges are associated with achieving a balance between alarm support and alarm fatigue, including distracting nurses from patient care or desensitizing the nurse to other alarms and calls [1]. Our hospitals have quantitative and anecdotal reports of seriously high volumes of wireless alerts on the nurses' smartphones. Nurses have complained that the phones are generating too much noise to consume or timely prioritize. Preliminary alarm inventory revealed the Bed Exit wireless alert as a leading contributor of signal volume across many units and hospitals. The lack of standard policies and workflow improvement processes has increased nuisance alarms, making these Health Information Technologies less useful and safe. Using system data, workflow observations, and nursing interviews, Singh and Sittig's HIT Safety framework [2] was applied to identify and prioritize sociotechnical factors and interventions that impact the end-to-end Bed Exit alarm workflow. This study reviews the application of sociotechnical models and frameworks to reduce wireless calls without introducing risk and impacting patient care.

Abstract:Computer scientist and science-fiction writer Vernor Vinge sketched out a seminal modern singularity hypothesis 15 years ago in his essay “The Coming Technological Singularity.” Since then lots of other people have taken up the cause, so now you can either anticipate or scoff at many different kinds of singularities. Perhaps the most inclusive variant is the one we label “technotopia,” the scenario in which computers reach and exceed human levels of intelligence, essentially resulting in the resolution of all human problems, including illness and limited life span. The “event horizon” idea corresponds most closely to Vinge’s original: a confluence of technological changes, most likely including machine intelligence, beyond which we cannot see into the future to predict anything. “Machine intelligence” represents the prediction that computers will achieve human-level intelligence and either consciousness or something indistinguishable from it. “Uploading,” which is also part of the technotopian vision, refers to the prediction that individual human brains (and most probably their state of consciousness) will be replicable in computerized form, leading to (among other things) virtual immortality. Kevin Kelly’s “phase change” hypothesis likens the singularity to the changes between the solid and liquid or liquid and gaseous states of matter: the world becomes fundamentally different but in ways that may not be immediately perceptible to individuals immersed in it at the time. The “intelligence explosion” hypothesis, initially developed by Turing associate I.J. Goode, postulates that as machines (initially under the direction of humans) design the next generation of machines, eventually superintelligent levels will be reached. The effect is similar to the accelerating rate of technological change anticipated by technotopians, but the path for getting there is different.
Internet-of-Things (IoT) in healthcare and social services - experiences of a sensor system for notifications of deviant behaviours in the home from the users' perspective

Abstract:Studies on use of IT in residential care are limited; thus, there is a need for investigations to understand both older people's and nursing staff's perspectives on experiences of new technology. 'Smart homes' provide home automation solutions, making life easier for those residing there. The aim was to explore, from the users' perspective, experiences of a sensor system installed in the home. The sensors are meant to provide notifications of deviations in behaviours or routines by the resident, requiring healthcare staff or relatives to do a supervisory visit. The sensor notification system made the users feel secure by being monitored, having control over the situation, and allowing them to become more independent in their daily lives; furthermore, they emphasised the importance of having well-functioning systems. Further development of the technology and use, in co-creation with the users, is needed. Careful preparation in installing/starting the system and repeated information about its aim are needed.

Abstract:Background: Neuropsychiatric symptoms (NPS) are common in affected individuals and can be challenging for (in)formal caregivers. Therefore, they are also referred to as challenging behaviors (CBs). Sensor technology measuring context and behavior can be assistive to effectively manage CBs in an objective fashion. Sensors can help support healthcare professionals, such as nurses, by enabling remote monitoring and alarming on early-stage behavioral changes associated with CBs. This might/ will improve the quality of life (QoL) for both caregivers and clients living in a nursing homes (NH). In the project "MOnitoring Onbegrepen Gedrag bij Dementie met sensortechnologie" (MOOD-Sense), we aim to develop such a monitoring system. Our research focuses on two questions 1) How to develop and implement a monitoring system within the context of nursing homes with parameters on environment, physiology, and behavior, identify and process relevant precursors of challenging behavior with this monitoring system and 2) gain insight in which behaviors are challenging according to nurses and how they are described. This will be represented in an ontology such that sensor data can be translated into the same conceptual information. Methods: The first research question will be examined with a set of experiments in the field (in NH) with an iterative approach. Insights from previous experiments on usability and added value of sensors will be used to improve successive experiments. During each experiment, multiple participants (clients with dementia and CBs) are monitored with both ambient and wearable sensors. For the second research question a qualitative approach is employed, using focus groups (FG) and consensus methods. These FGs will be held amongst nursing staff who are involved in daily care tasks for people with dementia. Subsequently, consensus methods are used to align behavioral descriptors/labels. Results: early findings will be presented at the symposium Discussion: Within this project we expect to find precursors of challenging behavior in a personalized fashion based on nurse's expert knowledge and sensor data. In order to develop a monitoring system that can be embedded within NH's, real-time alarming, in-situ behavior recognition and trustworthiness are part of our technological requirements. Just-in-time interventions may then be deployed to prevent behavior escalation or the persistence of undesirable situations.

Smart Home Sensing and Monitoring in Households With Dementia: User-Centered Design Approach

Abstract:Background As life expectancy grows, so do the challenges of caring for an aging population. Older adults, including people with dementia, want to live independently and feel in control of their lives for as long as possible. Assistive technologies powered by artificial intelligence and internet of things devices are being proposed to provide living environments that support the users’ safety, psychological, and medical needs through remote monitoring and interventions. Objective This study investigates the functional, psychosocial, and environmental needs of people living with dementia, their caregivers, clinicians, and health and social care service providers toward the design and implementation of smart home systems. Methods We used an iterative user-centered design approach comprising 9 substudies. First, semistructured interviews (9 people with dementia, 9 caregivers, and 10 academic and clinical staff) and workshops (35 pairs of people with dementia and caregivers, and 12 health and social care clinicians) were conducted to define the needs of people with dementia, home caregivers, and professional stakeholders in both daily activities and technology-specific interactions. Then, the spectrum of needs identified was represented via patient–caregiver personas and discussed with stakeholders in a workshop (14 occupational therapists; 4 National Health Service pathway directors; and 6 researchers in occupational therapy, neuropsychiatry, and engineering) and 2 focus groups with managers of health care services (n=8), eliciting opportunities for innovative care technologies and public health strategies. Finally, these design opportunities were discussed in semistructured interviews with participants of a smart home trial involving environmental sensors, physiological measurement devices, smartwatches, and tablet-based chatbots and cognitive assessment puzzles (10 caregivers and 2 people with dementia). A thematic analysis revealed factors that motivate household members to use these technologies. Results Outcomes of these activities include a qualitative and quantitative analysis of patient, caregiver, and clinician needs and the identification of challenges and opportunities for the design and implementation of remote monitoring systems in public health pathways. Conclusions Participatory design methods supported the triangulation of stakeholder perspectives to aid the development of more patient-centered interventions and their translation to clinical practice and public health strategy. We discuss the implications and limitations of our findings, the value and the applicability of our methodology, and directions for future research.
Evaluation of 1-Year in-Home Monitoring Technology by Home-Dwelling Older Adults, Family Caregivers, and Nurses

Abstract:Introduction: Population aging is increasing the needs and costs of healthcare. Both frailty and the chronic diseases affecting older people reduce their ability to live independently. However, most older people prefer to age in their own homes. New development of in-home monitoring can play a role in staying independent, active, and healthy for older people. This 12-month observational study aimed to evaluate a new in-home monitoring system among home-dwelling older adults (OA), their family caregivers (FC), and nurses for the support of home care. Methods: The in-home monitoring system evaluated in this study continuously monitored OA's daily activities (e.g., mobility, sleep habits, fridge visits, door events) by ambient sensor system (DomoCare®) and health-related events by wearable sensors (Activity tracker, ECG). In the case of deviations in daily activities, alerts were transmitted to nurses via email. Using specific questionnaires, the opinions of 13 OA, 13 FC, and 20 nurses were collected at the end of 12-months follow-up focusing on user experience and the impact of in-home monitoring on home care services. Results: The majority of OA, FC, and nurses considered that in-home sensors can help with staying at home, improving home care and quality of life, preventing domestic accidents, and reducing family stress. The opinion tended to be more frequently favorable toward ambient sensors (76%; 95% CI: 61-87%) than toward wearable sensors (Activity tracker: 65%; 95% CI: 50-79%); ECG: 60%; 95% CI: 45-75%). On average, OA (74%; 95% CI: 46-95%) and FC (70%; 95% CI: 39-91%) tended to be more enthusiastic than nurses (60%; 95% CI: 36-81%). Some barriers reported by nurses were a fear of weakening of the relationship with OA and lack of time. Discussion/Conclusion: Overall, the opinions of OA, FC, and nurses were positively related to in-home sensors, with nurses being less enthusiastic about their use in clinical practice.

Integrated Telehealth and Telecare for Monitoring Frail Elderly with Chronic Disease

Abstract:Objective: To investigate the potential of an integrated care system that acquires vital clinical signs and habits data to support independent living for elderly people with chronic disease. Materials and Methods: We developed an IEEE 11073 standards-based telemonitoring platform for monitoring vital signs and activity data of elderly living alone in their home. The platform has important features for monitoring the elderly: unobtrusive, simple, elderly-friendly, plug and play interoperable, and self-integration of sensors. Thirty-six (36) patients in a primary care practice in the United Kingdom (mean [standard deviation] age, 82 [10] years) with congestive heart failure (CHF) or chronic obstructive pulmonary disease (COPD) were provided with clinical sensors to measure the vital signs for their disease (blood pressure [BP] and weight for CHF, and oxygen saturation for COPD) and one passive infrared (PIR) motion sensor and/or a chair/bed sensor were installed in a patient's home to obtain their activity data. The patients were asked to take one measurement each day of their vital signs in the morning before breakfast. All data were automatically transmitted wirelessly to the remote server and displayed on a clinical portal for clinicians to monitor each patient. An alert algorithm detected outliers in the data and indicated alerts on the portal. Patient data have been analyzed retrospectively following hospital admission, emergency room visit or death, to determine whether the data could predict the event. Results: Data of patients who were monitored for a long period and had interventions were analyzed to identify useful parameters and develop algorithms to define alert rules. Twenty of the 36 participants had a clinical referral during the time of monitoring; 16 of them received some type of intervention. The most common reason for intervention was due to low oxygen levels for patients with COPD and high BP levels for CHF. Activity data were found to contain information on the well-being of patients, in particular for those with COPD. During exacerbation the activity level from PIR sensors increased slightly, and there was a decrease in bed occupancy. One subject with CHF who felt unwell spent most of the day in the bedroom. Conclusions: Our results suggest that integrated care monitoring technologies have a potential for providing improved care and can have positive impact on well-being of the elderly by enabling timely intervention. Long-term BP and pulse oximetry data could indicate exacerbation and lead to effective intervention; physical activity data provided important information on the well-being of patients. However, there remains a need for better understanding of long-term variations in vital signs and activity data to establish intervention protocols for improved disease management.

Abstract:Social determinants of health, such as poverty and minority background, severely disadvantage many people with mental disorders. A variety of innovative federal, state, and local programs have combined social services with mental health interventions. To explore the potential effects of such supports for addressing poverty and disadvantage on mental health outcomes, we simulated improvements in three social determinants-education, employment, and income. We used two large data sets: one from the National Institute of Mental Health that contained information about people with common mental disorders such as anxiety and depression, and another from the Social Security Administration that contained information about people who were disabled due to severe mental disorders such as schizophrenia and bipolar disorder. Our simulations showed that increasing employment was significantly correlated with improvements in mental health outcomes, while increasing education and income produced weak or nonsignificant correlations. In general, minority groups as well as the majority group of non-Latino whites improved in the desired outcomes. We recommend that health policy leaders, state and federal agencies, and insurers provide evidence-based employment services as a standard treatment for people with mental disorders.
Exuberant fibroadenomatoid proliferation in a pulmonary mesenchymoma (hamartoma): report of a lesion mimicking a sclerosing pneumocytoma.

Abstract:We report the cytologic and histopathologic features of an unusual peripheral primary lung tumor in a 61-year-old female. The central portion showed features of typical pulmonary chondroid hamartoma (mesenchymoma). Arising from the peripheral margin of this tumor, however, was an exuberant proliferation of papillary mesenchymal structures lined by type 2 pneumocates, mimicking papillary sclerosing hemangioma (pneumocytoma). This extreme example of fibroadenomatoid proliferation shares histologic features with other pulmonary neoplasms characterized by mesenchymal cores lined by type 2 pneumocytes. We propose that such lesions be unified under the term "alveolar mixed tumor".
Care4AD: A Technology-Driven Platform for Care Coordination and Management: Acceptability Study in Dementia

Abstract:Introduction: The technology-driven solution can reduce the caregiving burden; however, the needs of dementia caregiving are unique, and attitudes towards adopting technology from the perspectives of all the stakeholders involved in dementia caregiving are unclear. This study aims to assess the acceptability and feasibility of a technology-driven platform to facilitate care coordination platform, Care4AD, from the end-user perspective. Methods: Care4AD includes three components: (1) Care4AD app: the app is used by caregivers to coordinate care, monitor physical activity, and schedule reminders; (2) Care4AD tablet: a smart tablet is used by the care recipient to display scheduled reminders; and (3) Care4AD tags: a series of wireless sensor tags attached to various objects of daily care to facilitate monitoring instrumental activities of daily living (IADL) and adherence to scheduled tasks. Stakeholders in caregiving, including 11 experts in dementia care (age: 53.3 ± 8, 73% female), 10 individuals with dementia (IWD) (age: 76.1 ± 7.3, 50% female), and 14 caregivers (age: 66.9 ± 10.6, 75% female) were interviewed to determine perceived ease of use, attitude towards use, and perceived usefulness, based on the technology acceptance model (TAM) questionnaire. Additionally, we assessed technology anxiety and concerns with data sharing by caregivers and IWD. The interviews were conducted through videoconferencing or in-person meetings. The interview was composed of open-ended questions, a demonstration of the proposed Care4AD platform, and a survey based on TAM. Results: Compared to the neutral response, stakeholders showed significantly higher acceptance (70-100% satisfied to highly satisfied, p < 0.05) for all components of the TAM. Among IWD, age (r = -0.68, p = 0.03) and for caregivers the perceived ease of use (r = 0.73, p < 0.01) were significant predictors of attitude towards using the technology. Interestingly, neither concerns about data sharing nor educational level were limiting factors in the acceptability of the system in our sample. Conclusion: Overall, the results support a high perception of usefulness, ease of use, and attitude towards using Care4AD. The key barriers to adopting such technology are the age of IWD and the caregiver's perception of ease of use. Future studies are warranted to explore the effectiveness of such a platform to reduce caregiver stress and improve the quality of life and independence of IWD.
Co-designing Strategies to Provide Telecare Through an Intelligent Assistant for Caregivers of Elderly Individuals

Abstract:AbstractWorldwide, the number of people who are over 60 is growing more than any other age group. In the scenario of this research, a mediumsized city in Brazil, elderlies with Chronic Noncommunicable Diseases can have home care services. However, the healthcare professionals who assist these patients regularly face challenges in the visits, such as persistent fear of violence in certain areas. This research proposes the use of Intelligent Conversational Agents as a tool for supporting Telecare practices and tame these challenges. Our literature review, however, has demonstrated that previous works did not follow a methodology to promote a co-design of these agents in a scenario such as ours. There is also a lack of works that balance technological and social issues with non-technological experts in the design process (conceptualization, dialogues elaboration, intelligent agent goals and evaluation). We, thus, have performed a study following a qualitative methodology with the participation of health professionals. The first step aimed to raise the context, dialogues, and objectives of the agent. The second step, using observational and interviewing in a controlled environment, aimed to determine and evaluate the design goals, usefulness, compatibility with the reality, and find out additional requirements to aid the development of an Intelligent Conversational Agent. Results have led us to propose the agent MarIANA (Maria Intelligent conversAtioNal Agent). Therefore, it will act to prevent the clinical complications of these older adults suffering from Chronic Noncommunicable Diseases pointed as a critical matter in a recent work conducted in the city of the scenario of this research.
A smart home application to eldercare: Current status and lessons learned

Abstract:To address an aging population, we have been investigating sensor networks for monitoring older adults in their homes. In this paper, we report ongoing work in which passive sensor networks have been installed in 17 apartments in an aging in place eldercare facility. The network under development includes simple motion sensors, video sensors, and a bed sensor that captures sleep restlessness and pulse and respiration levels. Data collection has been ongoing for over two years in some apartments. This longevity in sensor data collection is allowing us to study the data and develop algorithms for identifying alert conditions such as falls, as well as extracting typical daily activity patterns for an individual. The goal is to capture patterns representing physical and cognitive health conditions and then recognize when activity patterns begin to deviate from the norm. In doing so, we strive to provide early detection of potential problems which may lead to serious health events if left unattended. We describe the components of the network and show examples of logged sensor data with correlated references to health events. A summary is also included on the challenges encountered and the lessons learned as a result of our experiences in monitoring aging adults in their homes.

The Personal Emergency Response System as a Technology Innovation in Primary Health Care Services: An Integrative Review

Abstract:BACKGROUND: Most western countries are experiencing greater pressure on community care services due to increased life expectancy and changes in policy toward prioritizing independent living. This has led to a demand for change and innovation in caring practices with an expected increased use of technology. Despite numerous attempts, it has proven surprisingly difficult to implement and adopt technological innovations. The main established technological innovation in home care services for older people is the personal emergency response system (PERS), which is widely adopted and used throughout most western countries aiming to support "aging safely in place."OBJECTIVE: This integrative review examines how research literature describes use of the PERS focusing on the users' perspective, thus exploring how different actors experience the technology in use and how it affects the complex interactions between multiple actors in caring practices.METHODS: The review presents an overview of the body of research on this well-established telecare solution, indicating what is important for different actors in regard to accepting and using this technology in community care services. An integrative review, recognized by a systematic search in major databases followed by a review process, was conducted.RESULTS: The search resulted in 33 included studies describing different actors' experiences with the PERS in use. The overall focus was on the end users' experiences and the consequences of having and using the alarm, and how the technology changes caring practices and interactions between the actors.CONCLUSIONS: The PERS contributes to safety and independent living for users of the alarm, but there are also unforeseen consequences and possible improvements in the device and the integrated service. This rather simple and well-established telecare technology in use interacts with the actors involved, creating changes in daily living and even affecting their identities. This review argues for an approach to telecare in which the complexity of practice is accounted for and shows how the plug-and-play expectations producers tend to generate is a simplification of the reality. This calls for a recognition that place and actors matter, as does a sensitivity to technology as an integrated part of complex caring practices.

The Ethical, Care, and Client-Caregiver Relationship Impacts Resulting From Introduction of Digital Communication and Surveillance Technologies in the Home Setting: Qualitative Inductive Study

Abstract:Background: Embedding communication and surveillance technology into the home health care setting has demonstrated the capacity for increased data efficiency, assumptions of convenience, and smart solutions to pressing problems such as caregiver shortages amid a rise in the aging population. The race to develop and implement these technologies within home care and public health nursing often leaves several ethical questions needing to be answered. Objective: The aim of this study was to understand the ethical and care implications of implementing digital communication and surveillance technologies in the home setting as perceived by health caregivers practicing in the region of Halland in Sweden with clients receiving home care services. Methods: A questionnaire was completed by 1260 home health caregivers and the written responses were evaluated by qualitative inductive content analysis. The researchers reviewed data independently and consensus was used to determine themes. Results: This study identified three main themes that illustrate ethical issues and unintended effects as perceived by caregivers of introducing digital communication and surveillance technologies in the home: (1) digital dependence vulnerability, (2) moral distress, and (3) interruptions to caregiving. This study highlights the consequences of technology developers and health systems leaders unintentionally ignoring the perspectives of caregivers who practice the intuitive artistry of providing care to other humans. Conclusions: Beyond the obtrusiveness of devices and impersonal data collection designed to emphasize health care system priorities, this study discovered a multifaceted shadow side of unintended consequences that arise from misalignment between system priorities and caregiver expertise, resulting in ethical issues. To develop communication and surveillance technologies that meet the needs of all stakeholders, it is important to involve caregivers who work with clients in the development process of new health care technology to improve both the quality of life of clients and the services offered by caregivers.
Capturing Home Care Information Management and Communication Processes Among Caregivers of Older Adults: Qualitative Study to Inform Technology Design

Abstract:Background: The demand for complex home care is increasing with the growing aging population and the ongoing COVID-19 pandemic. Family and hired caregivers play a critical role in providing care for individuals with complex home care needs. However, there are significant gaps in research informing the design of complex home care technologies that consider the experiences of family and hired caregivers collectively. Objective: The objective of this study was to explore the health documentation and communication experiences of family and hired caregivers to inform the design and adoption of new technologies for complex home care. Methods: The research involved semistructured interviews with 15 caregivers, including family and hired caregivers, each of whom was caring for an older adult with complex medical needs in their home in Ontario, Canada. Due to COVID-19-related protection measures, the interviews were conducted via Teams (Microsoft Corp). The interview guide was informed by the cognitive work analysis framework, and the interview was conducted using storytelling principles of narrative medicine to enhance knowledge. Inductive thematic analysis was used to code the data and develop themes. Results: Three main themes were developed. The first theme described how participants were continually updating the caregiver team, which captured how health information, including their communication motivations and intentions, was shared among family and hired caregiver participants. The subthemes included binder-based health documentation, digital health documentation, and communication practices beyond the binder. The second theme described how participants were learning to improve care and decision-making, which captured how they acted on information from various sources to provide care. The subthemes included developing expertise as a family caregiver and tailoring expertise as a hired caregiver. The third theme described how participants experienced conflicts within caregiver teams, which captured the different struggles arising from, and the causes of, breakdowns in communication and coordination between family and hired caregiver participants. The subthemes included 2-way communication and trusting the caregiver team. Conclusions: This study highlights the health information communication and coordination challenges and experiences that family and hired caregivers face in complex home care settings for older adults. Given the challenges of this work domain, there is an opportunity for appropriate digital technology design to improve complex home care. When designing complex home care technologies, it will be critical to include the overlapping and disparate perspectives of family and hired caregivers collectively providing home care for older adults with complex needs to support all caregivers in their vital roles.

Abstract:Ambient Assisted Living (AAL) and Ambient Intelligence technologies are providing support to older people in living an independent and confident life by developing innovative ICT-based products, services, and systems. Despite significant advancement in AAL technologies and smart systems, they have still not found the way into the nursing home of the older people. The reasons are manifold. On one hand, the development of such systems lack in addressing the requirements of the older people and caregivers of the organization and the other is the unwillingness of the older people to make use of assistive systems. A qualitative study was performed at a nursing home to understand the needs and requirements of the residents and caregivers and their perspectives about the existing AAL technologies.

Abstract:Old age is the leading cause of impaired bodily function, which gradually increases healthcare service needs. To offer the best possible care in the home and to be able to detect health-related functional impairment at an early stage, it is necessary to carry out systematic and structured observations. The assessment tool Subacute and Acute Dysfunction in the Elderly (SAFE) has been developed explicitly for these structured observations. This study aims to explore the experiences and challenges of home-based care work team coordinators (WTCs) regarding the introduction and use of SAFE.METHOD: The present qualitative study was performed following Consolidated Criteria for Reporting Qualitative Research (COREQ) guidelines. The data were collected through individual interviews (n = 3) and focus group (FG) interviews (n = 7). The interview transcripts were analysed using the Gioia method.RESULTS: Five aggregated dimensions were identified: Varying acceptance of SAFE, Structuring and quality-assurance of home-based nursing practice, Obstacles for the integration of SAFE in daily practice, Acceptance and use of SAFE require continuous supervision and SAFE contributes to increased quality of nursing care.CONCLUSION: The introduction of SAFE contributes to a structured follow-up of functional status in patients receiving home care. In order to incorporate the tool into home care practice, it is essential to set aside time to introduce the tool and to support nurses' use of it by offering continuous supervision.

Abstract:Background: Despite the ever-increasing offering of SMART technologies (ie, computer-controlled devices acting intelligently and capable of monitoring, analyzing or reporting), a wide gap exists between the development of new technological innovations and their adoption in everyday care for older adults. Objective: This study aims to explore the barriers and concerns related to the adoption of SMART technologies among different groups of stakeholders. Methods: Data from 4 sources were used: semistructured in-person or internet-based interviews with professional caregivers (n=12), structured email interviews with experts in the area of aging (n=9), a web-based survey of older adults (>55 years) attending the Virtual University of the Third Age (n=369), and a case study on the adoption of new technology by an older adult care facility. Results: Although all stakeholders noted the potential of SMART technologies to improve older adult care, multiple barriers to their adoption were identified. Caregivers perceived older adults as disinterested or incompetent in using technology, reported preferring known strategies over new technologies, and noted own fears of using technology. Experts viewed technologies as essential but expressed concerns about cost, low digital competency of older adults, and lack of support or willingness to implement technologies in older adult care. Older adults reported few concerns overall, but among the mentioned concerns were lack of ability or interest, misuse of data, and limited usefulness (in specific subgroups or situations). In addition, older adults' ratings of the usefulness of different technologies correlated with their self-rating of digital competency ( r =0.258; P <.001). Conclusions: Older adults appeared to have more positive views of various technologies than professional caregivers; however, their concerns varied by the type of technology. Lack of competence and lack of support were among the common themes, suggesting that educationally oriented programs for both older adults and their caregivers should be pursued.
