Abstract:Background: It is believed that artificial intelligence (AI) will be an integral part of health care services in the near future and will be incorporated into several aspects of clinical care such as prognosis, diagnostics, and care planning. Thus, many technology companies have invested in producing AI clinical applications. Patients are one of the most important beneficiaries who potentially interact with these technologies and applications; thus, patients' perceptions may affect the widespread use of clinical AI. Patients should be ensured that AI clinical applications will not harm them, and that they will instead benefit from using AI technology for health care purposes. Although human-AI interaction can enhance health care outcomes, possible dimensions of concerns and risks should be addressed before its integration with routine clinical care. Objective: The main objective of this study was to examine how potential users (patients) perceive the benefits, risks, and use of AI clinical applications for their health care purposes and how their perceptions may be different if faced with three health care service encounter scenarios. Methods: We designed a 2×3 experiment that crossed a type of health condition (ie, acute or chronic) with three different types of clinical encounters between patients and physicians (ie, AI clinical applications as substituting technology, AI clinical applications as augmenting technology, and no AI as a traditional in-person visit). We used an online survey to collect data from 634 individuals in the United States. Results: The interactions between the types of health care service encounters and health conditions significantly influenced individuals' perceptions of privacy concerns, trust issues, communication barriers, concerns about transparency in regulatory standards, liability risks, benefits, and intention to use across the six scenarios. We found no significant differences among scenarios regarding perceptions of performance risk and social biases. Conclusions: The results imply that incompatibility with instrumental, technical, ethical, or regulatory values can be a reason for rejecting AI applications in health care. Thus, there are still various risks associated with implementing AI applications in diagnostics and treatment recommendations for patients with both acute and chronic illnesses. The concerns are also evident if the AI applications are used as a recommendation system under physician experience, wisdom, and control. Prior to the widespread rollout of AI, more studies are needed to identify the challenges that may raise concerns for implementing and using AI applications. This study could provide researchers and managers with critical insights into the determinants of individuals' intention to use AI clinical applications. Regulatory agencies should establish normative standards and evaluation guidelines for implementing AI in health care in cooperation with health care institutions. Regular audits and ongoing monitoring and reporting systems can be used to continuously evaluate the safety, quality, transparency, and ethical factors of AI clinical applications.

Abstract:Objective: This survey aims to understand frontline healthcare professionals' perceptions of artificial intelligence (AI) in healthcare and assess how AI familiarity influences these perceptions. Materials and methods: We conducted a survey from February to March 2023 of physicians and physician assistants registered with the Kansas State Board of Healing Arts. Participants rated their perceptions toward AI-related domains and constructs on a 5-point Likert scale, with higher scores indicating stronger agreement. Two sub-groups were created for analysis to assess the impact of participants' familiarity and experience with AI on the survey results. Results: From 532 respondents, key concerns were Perceived Communication Barriers (median = 4.0, IQR = 2.8-4.8), Unregulated Standards (median = 4.0, IQR = 3.6-4.8), and Liability Issues (median = 4.0, IQR = 3.5-4.8). Lower levels of agreement were noted for Trust in AI Mechanisms (median = 3.0, IQR = 2.2-3.4), Perceived Risks of AI (median = 3.2, IQR = 2.6-4.0), and Privacy Concerns (median = 3.3, IQR = 2.3-4.0). Positive correlations existed between Intention to use AI and Perceived Benefits (r = 0.825) and Trust in AI Mechanisms (r = 0.777). Perceived risk negatively correlated with Intention to Use AI (r = -0.718). There was no difference in perceptions between AI experienced and AI naïve subgroups. Discussion: The findings suggest that perceptions of benefits, trust, risks, communication barriers, regulation, and liability issues influence healthcare professionals' intention to use AI, regardless of their AI familiarity. Conclusion: The study highlights key factors affecting AI adoption in healthcare from the frontline healthcare professionals' perspective. These insights can guide strategies for successful AI implementation in healthcare.
Use of AI-based tools for healthcare purposes: a survey study from consumers' perspectives

Thus, many technology companies and governmental projects have invested in producing AI-based clinical tools and medical applications. Patients can be one of the most important beneficiaries and users of AI-based applications whose perceptions may affect the widespread use of AI-based tools. Patients should be ensured that they will not be harmed by AI-based devices, and instead, they will be benefited by using AI technology for healthcare purposes. Although AI can enhance healthcare outcomes, possible dimensions of concerns and risks should be addressed before its integration with routine clinical care. Methods: We develop a model mainly based on value perceptions due to the specificity of the healthcare field. This study aims at examining the perceived benefits and risks of AI medical devices with clinical decision support (CDS) features from consumers' perspectives. We use an online survey to collect data from 307 individuals in the United States. Results: The proposed model identifies the sources of motivation and pressure for patients in the development of AI-based devices. The results show that technological, ethical (trust factors), and regulatory concerns significantly contribute to the perceived risks of using AI applications in healthcare. Of the three categories, technological concerns (i.e., performance and communication feature) are found to be the most significant predictors of risk beliefs. Conclusions: This study sheds more light on factors affecting perceived risks and proposes some recommendations on how to practically reduce these concerns. The findings of this study provide implications for research and practice in the area of AI-based CDS. Regulatory agencies, in cooperation with healthcare institutions, should establish normative standard and evaluation guidelines for the implementation and use of AI in healthcare. Abstract:ObjectiveThe attitudes about the usage of artificial intelligence in healthcare are controversial. Unlike the perception of healthcare professionals, the attitudes of patients and their companions have been of less interest so far. In this study, we aimed to investigate the perception of artificial intelligence in healthcare among this highly relevant group along with the influence of digital affinity and sociodemographic factors.MethodsWe conducted a cross-sectional study using a paper-based questionnaire with patients and their companions at a German tertiary referral hospital from December 2019 to February 2020. The questionnaire consisted of three sections examining (a) the respondents' technical affinity, (b) their perception of different aspects of artificial intelligence in healthcare and (c) sociodemographic characteristics.ResultsFrom a total of 452 participants, more than 90% already read or heard about artificial intelligence, but only 24% reported good or expert knowledge. Asked on their general perception, 53.18% of the respondents rated the use of artificial intelligence in medicine as positive or very positive, but only 4.77% negative or very negative. The respondents denied concerns about artificial intelligence, but strongly agreed that artificial intelligence must be controlled by a physician. Older patients, women, persons with lower education and technical affinity were more cautious on the healthcare-related artificial intelligence usage.ConclusionsGerman patients and their companions are open towards the usage of artificial intelligence in healthcare. Although showing only a mediocre knowledge about artificial intelligence, a majority rated artificial intelligence in healthcare as positive. Particularly, patients insist that a physician supervises the artificial intelligence and keeps ultimate responsibility for diagnosis and therapy.

Abstract:Patients with rheumatoid arthritis (RA) die at a younger age than their peers in most hospital series [1], while an increase in standardized mortality rate (SMR) has not been a consistent feature in recent studies of RA patients from date of diagnosis (inception cohorts) [2]. The reasons for this apparent discrepancy are likely to be the presence of more severe disease in hospitalbased series and the advent of earlier and more effective treatment in recent years. However, an increased mortality from vascular disease in RA has been consistently reported in both established and inception studies [1, 3], and an increased SMR for respiratory disease and lymphoreticular cancers has also been recognized in most series. A detailed study in this month’s issue addresses the important question of whether patients with RA treated early and actively with disease-modifying anti-rheumatic drugs (DMARDs) still have increased mortality rates [4]. The study assesses the effect of modern out-patient intervention on all-cause mortality in a large inception cohort over the first 7 yrs of their disease, and examines whether this relates to disease severity or duration. Detailed assessment of the causes of death offers new insights into the relevance of both cardiovascular and respiratory contributions to mortality and attempts to identify risk factor profiles. Although increasing age and male gender have been recognized as predictors for an earlier death in RA, few other clues as to an individual’s risk of premature death have been identified. However, recent work has shown direct links between rheumatoid disease activity, increased comorbidity and early death [5, 6]. The Early Rheumatoid Arthritis Study (ERAS) group [4] reports on 1429 patients recruited within 2 yrs of diagnosis over the 11 yrs from 1986. Although 84% received therapy with DMARDs, the all-cause SMR was significantly raised. After increased age, the odds ratios for death were most elevated in those patients with extra-articular disease and in those with established comorbidity. Early death was most often due to cardiovascular disease or interstitial pulmonary fibrosis, although other vascular and respiratory causes were also over-represented. Cardiovascular disease accounted for 31% of all deaths while pulmonary problems (including respiratory infection and lung cancer) were responsible for almost 29%. Ischaemic heart disease (IHD) accounted for a quarter of all deaths and carried an SMR of 1.49. It was also the most common cause of death at a young age and had a worse prognosis than in patients without RA. There was a relationship between disease activity as assessed by erythrocyte sedimentation rate (ESR) and probability of death from IHD, a finding consistent with previous reports linking increased markers of inflammation with coronary artery disease [7]. This suggests that DMARD therapy should protect RA patients from developing IHD, and reinforces the need for such therapy to be commenced early in the disease and used aggressively where possible, aiming for remission within the first year of disease. The link between cardiac disease and other therapeutic agents is less clear. Oral steroids did not have any clear effect on cardiovascular mortality but high-dose or long-term therapy might be expected to adversely affect the traditional risk factor profile by increasing blood pressure and body mass index and worsening the lipid profile. The influence of non-steroidal anti-inflammatory drugs (NSAIDs) on cardiovascular health has been much debated lately. COX-2 drugs especially have been linked with an increase in vascular events (in the order of one additional event per 100 patient years), with the data leading to the ultimate withdrawal of rofecoxib. However, a recent major meta analysis of the data on all NSAIDs report that many nonselective agents such as ibuprofen and diclofenac are also associated with an increased risk of adverse cardiovascular events, although it was unclear as to whether this effect was linked to dose or duration of treatment [8]. Perhaps the most striking aspect of the ERAS report is the high mortality resulting from interstitial pulmonary fibrosis (IPF). This accounted for 6% of all deaths and exceeded the combined number of deaths from all other extra-articular features of RA. It especially affected those patients with a younger age at onset of RA. Given that features of IPF can be shown using highresolution computed tomography (HRCT) in up to 20% of RA patients [9], this is a significant finding. Although there is some evidence that the natural history of IPF in RA is better than that in cryptogenic fibrosis [10], there is a paucity of data in the literature on this condition. Recent concern has been expressed that the natural history is worse in patients on biological agents following reports linking death from accelerated IPF in patients receiving etanercept, infliximab or adalimumab. Indeed the BSRBR confirms that deaths from IPF are increased nearly 3fold in patients on anti-tumor necrosis factor (anti-TNF) agents when compared with controls on methotrexate. Methotrexate itself has not been shown to cause deterioration in lung function in large series of patients, but nor does it appear to have any beneficial effect on IPF. Its ability to cause pneumonitis places patients with reduced respiratory reserve at increased risk, and is a cogent argument for the assessment of pulmonary function prior to its use, with patients demonstrating a restrictive defect requiring greater definition using HRCT to identify those patients with significant underlying lung disease [11]. Perhaps the choice of DMARD in patients with proven IPF should be driven by consideration of the lung disease as much as by the degree of articular involvement? Drug-related deaths were reported in the ERAS report, and these related largely to the use of NSAIDs which were responsible for eight fatalities resulting from upper gastrointestinal haemorrhage or perforation. Most of these patients had been on long-term therapy with NSAIDs but some had only recently commenced these agents. There is good evidence to implicate Helicobacter pylori infection in upper gastrointestinal ulceration, and recent work has shown that the presence of this bacterial agent together with NSAID exposure in RA patients greatly increases their risk of having an endoscopically visualized peptic ulcer [12]. Drugs may also have contributed to the greatly increased SMR (350) recorded for renal failure in the study, given the propensity of NSAIDs to precipitate renal failure in patients with reduced glomerular filtration rates. Taking these findings together with those linking NSAIDs with increased cardiovascular mortality, it is time for rheumatologists and primary care practitioners to reduce dependence on NSAIDs, particularly in patients with a high 10-yr risk of cardiovascular disease, a history of dyspepsia or evidence of reduced renal function. Abstract:Background: This study explored physician anesthesiologists' knowledge, exposure, and perceptions of artificial intelligence (AI) and their associations with attitudes and expectations regarding its use in clinical practice. The findings highlight the importance of understanding anesthesiologists' perspectives for the successful integration of AI into anesthesiology, as AI has the potential to revolutionize the field. Methods: A cross-sectional survey of 27,056 US physician anesthesiologists was conducted to assess their knowledge, perceptions, and expectations regarding the use of AI in clinical practice. The primary outcome measured was attitude toward the use of AI in clinical practice, with scores of 4 or 5 on a 5-point Likert scale indicating positive attitudes. The anticipated impact of AI on various aspects of professional work was measured using a 3-point Likert scale. Logistic regression was used to explore the relationship between participant responses and attitudes toward the use of AI in clinical practice. Results: A 2021 survey of 27,056 US physician anesthesiologists received 1086 responses (4% response rate). Most respondents were male (71%), active clinicians (93%) under 45 (34%). A majority of anesthesiologists (61%) had some knowledge of AI and 48% had a positive attitude toward using AI in clinical practice. While most respondents believed that AI can improve health care efficiency (79%), timeliness (75%), and effectiveness (69%), they are concerned that its integration in anesthesiology could lead to a decreased demand for anesthesiologists (45%) and decreased earnings (45%). Within a decade, respondents expected AI would outperform them in predicting adverse perioperative events (83%), formulating pain management plans (67%), and conducting airway exams (45%). The absence of algorithmic transparency (60%), an ambiguous environment regarding malpractice (47%), and the possibility of medical errors (47%) were cited as significant barriers to the use of AI in clinical practice. Respondents indicated that their motivation to use AI in clinical practice stemmed from its potential to enhance patient outcomes (81%), lower health care expenditures (54%), reduce bias (55%), and boost productivity (53%). Variables associated with positive attitudes toward AI use in clinical practice included male gender (odds ratio [OR], 1.7; P < .001), 20+ years of experience (OR, 1.8; P < .01), higher AI knowledge (OR, 2.3; P = .01), and greater AI openness (OR, 10.6; P < .01). Anxiety about future earnings was associated with negative attitudes toward AI use in clinical practice (OR, 0.54; P < .01). Conclusions: Understanding anesthesiologists' perspectives on AI is essential for the effective integration of AI into anesthesiology, as AI has the potential to revolutionize the field.
Perspectives of Patients With Chronic Diseases on Future Acceptance of AI-Based Home Care Systems: Cross-Sectional Web-Based Survey Study

Abstract:Background: Artificial intelligence (AI)-based home care systems and devices are being gradually integrated into health care delivery to benefit patients with chronic diseases. However, existing research mainly focuses on the technical and clinical aspects of AI application, with an insufficient investigation of patients' motivation and intention to adopt such systems. Objective: This study aimed to examine the factors that affect the motivation of patients with chronic diseases to adopt AI-based home care systems and provide empirical evidence for the proposed research hypotheses. Methods: We conducted a cross-sectional web-based survey with 222 patients with chronic diseases based on a hypothetical scenario. Results: The results indicated that patients have an overall positive perception of AI-based home care systems. Their attitudes toward the technology, perceived usefulness, and comfortability were found to be significant factors encouraging adoption, with a clear understanding of accountability being a particularly influential factor in shaping patients' attitudes toward their motivation to use these systems. However, privacy concerns persist as an indirect factor, affecting the perceived usefulness and comfortability, hence influencing patients' attitudes. Conclusions: This study is one of the first to examine the motivation of patients with chronic diseases to adopt AI-based home care systems, offering practical insights for policy makers, care or technology providers, and patients. This understanding can facilitate effective policy formulation, product design, and informed patient decision-making, potentially improving the overall health status of patients with chronic diseases.

Abstract:Artificial intelligence (AI) is an emerging technological system that provides a platform to manage and analyze data by emulating human cognitive functions with greater accuracy, revolutionizing patient care and introducing a paradigm shift to the healthcare industry. The purpose of this study is to identify the underlying factors that affect the adoption of artificial intelligence in healthcare (AIH) by healthcare providers and to understand “What are the factors that influence healthcare providers’ behavioral intentions to adopt AIH in their routine practice?” An integrated survey was conducted among healthcare providers, including consultants, residents/students, and nurses. The survey included items related to performance expectancy, effort expectancy, initial trust, personal innovativeness, task complexity, and technology characteristics. The collected data were analyzed using structural equation modeling. A total of 392 healthcare professionals participated in the survey, with 72.4% being male and 50.7% being 30 years old or younger. The results showed that performance expectancy, effort expectancy, and initial trust have a positive influence on the behavioral intentions of healthcare providers to use AIH. Personal innovativeness was found to have a positive influence on effort expectancy, while task complexity and technology characteristics have a positive influence on effort expectancy for AIH. The study’s empirically validated model sheds light on healthcare providers’ intention to adopt AIH, while the study’s findings can be used to develop strategies to encourage this adoption. However, further investigation is necessary to understand the individual factors affecting the adoption of AIH by healthcare providers.

Abstract:To investigate opinions and attitudes of medical professionals towards adopting AI-enabled healthcare technologies in their daily business, we used a mixed-methods approach. Study 1 employed a qualitative computational grounded theory approach analyzing 181 Reddit threads in the several subreddits of r/medicine. By utilizing an unsupervised machine learning clustering method, we identified three key themes: (1) consequences of AI, (2) physician-AI relationship, and (3) a proposed way forward. In particular Reddit posts related to the first two themes indicated that the medical professionals' fear of being replaced by AI and skepticism toward AI played a major role in the argumentations. Moreover, the results suggest that this fear is driven by little or moderate knowledge about AI. Posts related to the third theme focused on factual discussions about how AI and medicine have to be designed to become broadly adopted in health care. Study 2 quantitatively examined the relationship between the fear of AI, knowledge about AI, and medical professionals' intention to use AI-enabled technologies in more detail. Results based on a sample of 223 medical professionals who participated in the online survey revealed that the intention to use AI technologies increases with increasing knowledge about AI and that this effect is moderated by the fear of being replaced by AI.
Trust in and Acceptance of Artificial Intelligence Applications in Medicine: Mixed Methods Study

Abstract:Background: Artificial intelligence (AI)-powered technologies are being increasingly used in almost all fields, including medicine. However, to successfully implement medical AI applications, ensuring trust and acceptance toward such technologies is crucial for their successful spread and timely adoption worldwide. Although AI applications in medicine provide advantages to the current health care system, there are also various associated challenges regarding, for instance, data privacy, accountability, and equity and fairness, which could hinder medical AI application implementation. Objective: The aim of this study was to identify factors related to trust in and acceptance of novel AI-powered medical technologies and to assess the relevance of those factors among relevant stakeholders. Methods: This study used a mixed methods design. First, a rapid review of the existing literature was conducted, aiming to identify various factors related to trust in and acceptance of novel AI applications in medicine. Next, an electronic survey including the rapid review-derived factors was disseminated among key stakeholder groups. Participants (N=22) were asked to assess on a 5-point Likert scale (1=irrelevant to 5=relevant) to what extent they thought the various factors (N=19) were relevant to trust in and acceptance of novel AI applications in medicine. Results: The rapid review (N=32 papers) yielded 110 factors related to trust and 77 factors related to acceptance toward AI technology in medicine. Closely related factors were assigned to 1 of the 19 overarching umbrella factors, which were further grouped into 4 categories: human-related (ie, the type of institution AI professionals originate from), technology-related (ie, the explainability and transparency of AI application processes and outcomes), ethical and legal (ie, data use transparency), and additional factors (ie, AI applications being environment friendly). The categorized 19 umbrella factors were presented as survey statements, which were evaluated by relevant stakeholders. Survey participants (N=22) represented researchers (n=18, 82%), technology providers (n=5, 23%), hospital staff (n=3, 14%), and policy makers (n=3, 14%). Of the 19 factors, 16 (84%) human-related, technology-related, ethical and legal, and additional factors were considered to be of high relevance to trust in and acceptance of novel AI applications in medicine. The patient's gender, age, and education level were found to be of low relevance (3/19, 16%). Conclusions: The results of this study could help the implementers of medical AI applications to understand what drives trust and acceptance toward AI-powered technologies among key stakeholders in medicine. Consequently, this would allow the implementers to identify strategies that facilitate trust in and acceptance of medical AI applications among key stakeholders and potential users.

Abstract:BACKGROUND Several studies have investigated the relationships of lipid levels with depressive and anxiety disorders, and their results revealed an association between low cholesterol and higher levels of depressive symptoms in young, middle-aged, and postpartum women. However, few studies have explored this relationship in menopausal women. Hence, in this study of menopausal Taiwanese women, we attempted to determine the correlation of depressive and anxiety disorders with serum lipid profiles. METHODS This was a cross-sectional study covering a 2-year period. Sixty-nine menopausal women who visited the gynecologic outpatient department of our hospital were enrolled. Psychiatric diagnoses were made using the semi-structured clinical interview for the Diagnostic and Statistical Manual (DSM-IV) criteria. Blood samples for serum lipid profiles were simultaneously collected. Data were analyzed using analysis of co-variance (ANCOVA) adjusted for age and body mass index (BMI). RESULTS Total cholesterol (TC) and low-density lipoprotein (LDL) were higher in postmenopausal women than in perimenopausal women, but this was not true for triglyceride (TG), high-density lipoprotein (HDL), very-low-density lipoprotein (VLDL), the TC/HDL ratio, or the LDL/HDL ratio. However, when peri- and postmenopausal women were categorized into normal controls and those having anxiety disorders and depressive disorders, no significant differences were found in lipid concentrations of TG, TC, HDL, VLDL, LDL, TC/HDL, or LDL/HDL among the 3 groups. CONCLUSIONS Although these results do not suggest that serum lipid profiles can be used as biological markers to distinguish depressive or anxiety disorders in menopausal women, larger samples are required to prove such results in the future.
Acceptance of clinical artificial intelligence among physicians and medical students: A systematic review with cross-sectional survey

Abstract:Background Artificial intelligence (AI) needs to be accepted and understood by physicians and medical students, but few have systematically assessed their attitudes. We investigated clinical AI acceptance among physicians and medical students around the world to provide implementation guidance. Materials and methods We conducted a two-stage study, involving a foundational systematic review of physician and medical student acceptance of clinical AI. This enabled us to design a suitable web-based questionnaire which was then distributed among practitioners and trainees around the world. Results Sixty studies were included in this systematic review, and 758 respondents from 39 countries completed the online questionnaire. Five (62.50%) of eight studies reported 65% or higher awareness regarding the application of clinical AI. Although, only 10–30% had actually used AI and 26 (74.28%) of 35 studies suggested there was a lack of AI knowledge. Our questionnaire uncovered 38% awareness rate and 20% utility rate of clinical AI, although 53% lacked basic knowledge of clinical AI. Forty-five studies mentioned attitudes toward clinical AI, and over 60% from 38 (84.44%) studies were positive about AI, although they were also concerned about the potential for unpredictable, incorrect results. Seventy-seven percent were optimistic about the prospect of clinical AI. The support rate for the statement that AI could replace physicians ranged from 6 to 78% across 40 studies which mentioned this topic. Five studies recommended that efforts should be made to increase collaboration. Our questionnaire showed 68% disagreed that AI would become a surrogate physician, but believed it should assist in clinical decision-making. Participants with different identities, experience and from different countries hold similar but subtly different attitudes. Conclusion Most physicians and medical students appear aware of the increasing application of clinical AI, but lack practical experience and related knowledge. Overall, participants have positive but reserved attitudes about AI. In spite of the mixed opinions around clinical AI becoming a surrogate physician, there was a consensus that collaborations between the two should be strengthened. Further education should be conducted to alleviate anxieties associated with change and adopting new technologies.

Abstract:The integration of Artificial Intelligence (AI) into healthcare has the potential to revolutionize medical diagnostics, particularly in specialized fields such as Ear, Nose, and Throat (ENT) medicine. However, the successful adoption of AI-assisted diagnostic tools in ENT practice depends on the understanding of various factors; these include influences on their effectiveness and acceptance among healthcare professionals. This cross-sectional study aimed to assess the usability and integration of AI tools in ENT practice, determine the clinical impact and accuracy of AI-assisted diagnostics in ENT, measure the trust and confidence of ENT professionals in AI tools, gauge the overall satisfaction and outlook on the future of AI in ENT diagnostics, and identify challenges, limitations, and areas for improvement in AI-assisted ENT diagnostics. A structured online questionnaire was distributed to 600 certified ENT professionals with at least one year of experience in the field. The questionnaire assessed participants' familiarity with AI tools, usability, clinical impact, trust, satisfaction, and identified challenges. A total of 458 respondents completed the questionnaire, resulting in a response rate of 91.7%. The majority of respondents reported familiarity with AI tools (60.7%) and perceived them as generally usable and clinically impactful. However, challenges such as integration with existing systems, user-friendliness, accuracy, and cost were identified. Trust and satisfaction levels varied among participants, with concerns regarding data privacy and support. Geographic and practice setting differences influenced perceptions and experiences. The study highlights the diverse perceptions and experiences of ENT professionals regarding AI-assisted diagnostics. While there is general enthusiasm for these tools, challenges related to integration, usability, trust, and cost need to be addressed for their widespread adoption. These findings provide valuable insights for developers, policymakers, and healthcare providers aiming to enhance the role of AI in ENT practice.

Perceptions and Needs of Artificial Intelligence in Health Care to Increase Adoption: Scoping Review

Abstract:Background Artificial intelligence (AI) has the potential to improve the efficiency and effectiveness of health care service delivery. However, the perceptions and needs of such systems remain elusive, hindering efforts to promote AI adoption in health care. Objective This study aims to provide an overview of the perceptions and needs of AI to increase its adoption in health care. Methods A systematic scoping review was conducted according to the 5-stage framework by Arksey and O’Malley. Articles that described the perceptions and needs of AI in health care were searched across nine databases: ACM Library, CINAHL, Cochrane Central, Embase, IEEE Xplore, PsycINFO, PubMed, Scopus, and Web of Science for studies that were published from inception until June 21, 2021. Articles that were not specific to AI, not research studies, and not written in English were omitted. Results Of the 3666 articles retrieved, 26 (0.71%) were eligible and included in this review. The mean age of the participants ranged from 30 to 72.6 years, the proportion of men ranged from 0% to 73.4%, and the sample sizes for primary studies ranged from 11 to 2780. The perceptions and needs of various populations in the use of AI were identified for general, primary, and community health care; chronic diseases self-management and self-diagnosis; mental health; and diagnostic procedures. The use of AI was perceived to be positive because of its availability, ease of use, and potential to improve efficiency and reduce the cost of health care service delivery. However, concerns were raised regarding the lack of trust in data privacy, patient safety, technological maturity, and the possibility of full automation. Suggestions for improving the adoption of AI in health care were highlighted: enhancing personalization and customizability; enhancing empathy and personification of AI-enabled chatbots and avatars; enhancing user experience, design, and interconnectedness with other devices; and educating the public on AI capabilities. Several corresponding mitigation strategies were also identified in this study. Conclusions The perceptions and needs of AI in its use in health care are crucial in improving its adoption by various stakeholders. Future studies and implementations should consider the points highlighted in this study to enhance the acceptability and adoption of AI in health care. This would facilitate an increase in the effectiveness and efficiency of health care service delivery to improve patient outcomes and satisfaction.

Early postprandial Insulin secretion: Its relation to Insulin sensitivity

Abstract:Background: Lack of first phase insulin secretion during oral glucose tolerance test [OGTT] in Type 2 Diabetes Mellitus (DM) is attributed to glucose toxicity. Alternatively, the role of insulin resistance in impaired insulin release secondary to lack of glucose entry into β cells may be responsible, but is not examined. Aim: The role of insulin sensitivity in 1st phase insulin secretion was assessed. Material and Methods: Plasma glucose (G) and insulin (I) concentrations were determined after an overnight fast (F) and upto 60 minutes during OGTT with glucose 75g in 12 normal (N), 14 with impaired glucose tolerance (IGT) and 41 subjects with Type 2 DM. First phase insulin secretion (Δ Insulin) was determined as a percentage rise from baseline 100x (Peak-Basal)/Basal. Insulin sensitivity was determined as FI x FG (mUxmM/L). Results: FG were normal (7.0 mM/L in Type 2 DM. FI x FG and ? insulin were 35 ± 4 and 389 ± 89% in N; 77 ± 5 and 254 ± 65% in IGT; and 235 ± 19 and 95 ± 15% in Type 2 DM. Significant negative correlations were noted between ? insulin one hand and FI x FG on the other amongst all subjects [p < 0.0001 for all correlations]. Conclusion: Decline of 1st phase insulin secretion in IGT and Type 2 DM may be attributed to inhibited release of depleted insulin stores in the β Cells induced by impaired glucose entry due to insulin resistance, and is unlikely to be caused by glucose toxicity in IGT in presence of fasting euglycemia.

Abstract:Artificial Intelligence is the focus term of the 21st century. It has been widely accepted and adopted in several key areas like education, marketing, retail, and engineering among numerous others however the healthcare sector seems to lag in its welcome. AI has the potential to unlock a new transformation in patient care, diagnosis, and mentoring and support services as seen from a review of relevant literature. It is therefore essential to understand how people view Artificial Intelligence and feel its need or demands in improving their health-related needs. This study was designed, keeping this thought in mind. A survey study was carried out in Delhi-NCR with fifty participants belonging to the age group of 18-50 years, who were health-conscious, and proficient in the use of technology like smartphones and other smart devices. The survey findings reveal that respondents desired the integration of Artificial Intelligence-based solutions to medical services ranging from appointment booking through intelligent chatbots to insightful diagnosis using risk profiles, intricate surgeries guided by intelligent robots to mentoring services which described health goals and discussed sustainable solutions towards achieving desired goals through lifestyle changes. Secondary data showed the application of such technologies in hospitals in form of unique personalized programs aimed at improving patient care and promote good health. The study further recommends the use of Artificial Intelligence in healthcare services in both rural and urban areas to reduce the burden on medical professionals, increase personalized and efficient healthcare service provision especially in times of global pandemic and increase people’s consciousness towards the need for good health through the adoption of positive lifestyle changes.

Abstract:Artificial intelligence technology and machine learning techniques are rapidly invoking their presence in healthcare. In clinical medicine, AI is already utilized in clinical diagnosis, risk stratification, treatment selection and patient monitoring. Additionally, biomedical research now exploits AI technology and leverages its unique advantages. Authors in this issue explored current physician anesthesiologists' attitudes towards the progress and use of AI in anesthetic practice. A cross-sectional survey of anesthesiologists was conducted and a summary of their findings is illustrated in this infographic. Several factors such as male gender, work experience greater than 20 years and familiarity with AI were associated with positive attitudes towards this technology. Understanding clinicians' perspectives on AI is crucial to its thoughtful adoption in clinical practice. The reader is strongly encouraged to review the cited article for a more thorough understanding of the illustrated concepts.
