Abstract:This paper defines the newly emerging concept of knowledge management. The topics presented include: principles and practices of knowledge management, organization, distribution, dissemination, collaboration and refinement of information, and the effect on productivity and quality in business today. The technical applications and tools currently utilized within this discipline are also discussed. Case studies are included on the following firms: Teltech, Ernst & Young, Microsoft, and Hewlett Packard. These are analyzed to determine the effect knowledge management practices have on quality improvement and increased productivity. The authors have included a recommended strategy for implementation of knowledge management “best practices”. Finally, conclusions are drawn regarding the strategic direction of this new discipline and its effect on competition, productivity and quality for the business of tomorrow.
Cancer Mortality Among Techa Riverside Residents ( Southern Urals ) , Chronically Exposed to Radiation During the Prenatal Period and in Childhood

Abstract:Imperfect technology and lack of management and utilization facilities for reprocessing liquid waste released by the Mayak PA resulted in a protracted combined (external and internal) environmental radiation exposure of the population resident on the banks of the Techa River. The followup of 8,640 subjects exposed during the prenatal period and in childhood covered the period of 49 years (1.1.1950 – 31.12.1998), the total person-years under observation amounted to 222,686. From 1950 through 1998 1,231 death cases were registered in the catchment area (5 raions in Chelyabinsk Oblast through which the Techa flows). In 70 cases death was caused by solid cancers, and in 12 cases by leukemia. Analyses of solid cancer mortality yielded higher rates for men as compared to women (p<0.001). No significant differences in death rates were observed between different ethnic groups (Slavs vs Tartars and Bashkirs). A statistically significant increase in solid cancer mortality with attained age was shown (p<0.001). The age at first exposure was demonstrated to be a factor modifying the solid cancer mortality rate (p=0.049). The highest risk of death from solid cancers was manifested by persons whose exposure started in the prenatal period or at the age under 5 years. There were 4.6 excess cases in this group of 30 observed solid cancer cases, whereas in the group including subjects aged 5 years or older at first exposure only 1 excess case was registered among the 40 observed cancer cases. The obtained leukemia mortality ERR value (CLL excluded) was 7.76, p = 0.09. The excess leukemia death was found to be 7.6 out of 10 observed cases. The analysis performed should be regarded as a preliminary one, taking into account the need to further extend the followup of the cohort and a potential verification of dose estimates in the future.

Abstract:Background: Cognitive dysfunction is a commonly reported symptom in the primary brain tumor (PBT) population but is associated with substantial heterogeneity in both the course and severity. Brief, reliable, and sensitive measures of patient-perceived impact of cognitive symptoms are needed for clinical care and research. This study aims to describe the prevalence and predictors of cognitive dysfunction in PBT patients enrolled in a natural history study (NCT02851706: PI T. Armstrong). Methods: Clinical and demographic characteristics and patient-reported outcomes (PRO), including symptom burden (MD Anderson Symptom Inventory-Brain Tumor (MDASI-BT) using symptom burden, interference, and cognitive symptom factor scores); perceived cognition (Neuro-QoL Cognitive Function); and anxiety/depression (Patient-Reported Outcomes Measurement Information System Anxiety and Depression Short Forms) were collected and characterized using descriptive statistics. Neuro-QoL Cognitive Function T-scores < 40 indicate moderate-severe (MS) cognitive dysfunction. Univariate tests on cognitive dysfunction included chi-square tests and independent samples t-tests with p-value < 0.10. Variables significantly associated with MS outcome were then included in a subsequent multivariate logistic regression with backward selection. Clinical variables and PROs were evaluated separately, then combined for a final multivariate model with p-value < 0.05. Results: The cohort included 327 patients [median age: 47 years (range: 18-85), primarily white (78%) males (55%) with high-grade tumors (73%), most commonly with glioblastoma (36%), with 26% reporting cognitive dysfunction as a presenting symptom]. Sixty-five percent had a Karnofsky Performance Status (KPS) ≥ 90, 73% had received radiation, and 44% had experienced one or more tumor recurrences. MS cognitive dysfunction was reported in 29% of patients. At the univariate level, age, employment status, cognitive symptoms at presentation, current corticosteroid use, anti-convulsant use, KPS, and number of surgeries were significant clinical predictors of MS cognitive dysfunction; as were, separately, worsening symptom burden, interference, cognitive symptom factor, anxiety, and depression. Evaluated together, a final multivariate model found increasing scores on symptom interference and cognitive symptom factor increased the likelihood of MS cognitive dysfunction (interference OR=1.25, 95% CI [1.09, 1.44], p<0.001; cognitive symptom factor OR=1.81, 95% CI [1.50, 2.19], p<0.001) with 84% classification rate. Conclusion: Understanding the impact and prevalence of cognitive functioning is essential for PBT patient care and identifying supportive care interventions. Findings indicate self-report of cognitive symptoms and interference of symptoms with daily living contribute to moderate-to-severe cognitive dysfunction. Future research in cognitive function will compare self-report with objective assessment, explore its predictors in select populations (i.e., IDH-mutant tumors), and examine its association with biomarkers. Abstract:surance and private practice have again increased (1). 0*c>edz Growth has been in the face of a National Health Service (NHS) which already provides virtually all of the medical services which private insurance covers. The present National Health Service in the United Kingdom began in 1948. Although there have been several administrative reorganizations, complete coverage of medical, surgical, and hospital care for all UK residents has remained intact. From the beginning medical specialists, if they worked less than fulltime in the NHS, were assured the right to engage in private practice. It was estimated that in 1971-72, part-timers derived a third of their income from private practice (2). However, the levels of supplemental earnings made from private practice by these part-time NHS consultants varied and was a function of specialty (mainly surgery and anesthesiology) and location (London and a few other urban centers). General practitioners (GPs) were also allowed to do private practice without defined limits, but in 1971-72 it made up only 6% of GP income (2). The 1980 contract allowed private practice for full-time as well as part-time specialists (3). This change was in keeping with the goal of the present Conservative government to increase the role of the private sector in health (3). Further evidence of government support for the private health sector is the recent decision that all private insurance payments by employers are no longer taxable as employees' benefits. In this report the nature and growth of private practice and private insurance in the UK will be described, trends analyzed, future implications discussed, and comparisons made with the Canadian health care system.

Abstract:Purpose of ReviewWe review and summarize the key issues affecting general health and quality of life (QOL) of pediatric long-term survivors of brain tumors.Recent FindingsLong-term survivors of brain tumors are at risk of considerable late morbidity and mortality. Lengthening survival in brain tumors has highlighted the deep impact of tumor and its treatment on the physical, psychological, functional, and social health and QOL of these survivors. Evolution in tumor therapy including surgery, radiotherapy, and systemic therapies, etc., has the potential to mitigate this impact to some extent.SummarySensitization of health staff, policy makers, and the primary designers of clinical trials towards integration of QOL end points while measuring survival in brain tumor patients is the need of the hour. New developments in tumor therapeutics must not only provide quantitative gain but also improve the quality of survival in these long-term survivors. While majority of the issues presented pertain to survivorship in pediatric brain tumor population, similar challenges are likely to exist in young adults surviving brain tumors as well.

Abstract:Abstract Background Early efforts at risk-adapted therapy for neuroblastoma are predicted to result in differential late effects; the magnitude of these differences have not been well-described. Methods Late mortality, subsequent malignant neoplasms (SMN), and severe/life-threatening chronic health conditions (CHCs), graded according to CTCAE v4.03, were assessed among 5-year CCSS survivors of neuroblastoma diagnosed 1987-1999. Using age, stage at diagnosis, and treatment, survivors were classified into risk groups (low [n = 425]; intermediate [n = 252]; high [n = 245]). Standardized mortality ratios (SMR) and standardized incidence ratios (SIR) of SMNs were compared to matched population controls. Cox regression models estimated hazard ratios (HR) and 95% confidence intervals (CI) for CHC compared to 1,029 CCSS siblings. Results Among survivors (49.8% male; median age 21 years, range 7-42; median follow-up 19.3 years, range 5-29.9), 80% with low-risk disease were treated with surgery alone, while 79.1% with high-risk disease received surgery, radiation, chemotherapy ± autologous stem cell transplant (ASCT). All-cause mortality was elevated across risk groups (SMRhigh=27.7 [21.4-35.8]; SMRintermediate=3.3 [1.7-6.5]; SMRlow=2.8 [1.7-4.8]). SMN risk was increased among high- and intermediate-risk survivors (SIRhigh=28.0 [18.5-42.3]; SIRintermediate=3.7 [1.2-11.3]), but did not differ from the US population for survivors of low-risk disease. Compared to siblings, survivors had an increased risk of grade 3-5 CHCs, particularly among those with high-risk disease (HRhigh=16.1 [11.2-23.2]; HRintermediate=6.3 [3.8-10.5]; HRlow=1.8 [1.1-3.1]). Conclusion Survivors of high-risk disease treated in the early days of risk stratification carry a markedly elevated burden of late recurrence, SMN, and organ-related multi-morbidity, while survivors of low/intermediate-risk disease have a modest risk of late adverse outcomes.

Abstract:Purpose Primary central nervous system lymphoma (PCNSL) is a rare malignancy of the central nervous system with high invasiveness. There is little consensus on the treatment of PCNSL. This study retrospectively studied data from PCNSL patients in a single center to summarize treatment experience and explore prognostic factors. Methods Survival curves were drawn using the Kaplan–Meier method and prognostic factors were analyzed using Cox's hazards model. Results In multivariate analysis, cerebrospinal fluid lactic acid dehydrogenase (CSF LDH; p = 0.005 and p = 0.002), neutrophil to lymphocyte ratio (NLR; p = 0.014 and p = 0.038), and completion of four cycles of induction therapy ( p < 0.001and p < 0.001) were significant and independent predictors of overall survival (OS) and progression-free survival (PFS), respectively. Conclusion On the basis of this study, we propose that PCNSL patients should receive early induction therapy with sufficient cycles. Subsequent consolidation therapy can prevent relapses and improve survival. In patients with PCNSL, the independent prognostic factors for OS and PFS were CSF LDH level, NLR, and full cycles of induction therapy.

Abstract:Abstract There is increased recognition that social determinants of health may impact well-being and health in those with various cancers, but they have not been systematically evaluated in primary brain tumor (PBT) patients. Here, we examined the association between Area Deprivation Index (ADI), a measurement of sociodemographic neighborhood disadvantage, and patient-reported outcomes (PROs) in a cohort of adult PBT patients enrolled in a large National Institutes of Health observational trial (NHS, NCT02851706, PI: Armstrong). We utilized linear regression to assess the relationship between ADI (less advantaged [ADI≥40, n=153, 24%], more advantaged [ADI<40, n=490, 76%]) and patient symptom burden (MDASI-BT), anxiety, and depression (PROMIS Short Forms v1.0 8a). Logistic regression was used to evaluate the association between ADI and health-related quality of life using the 5 dimensions of the EQ-5D-3L. Models were adjusted for age, sex, race/ethnicity, tumor grade, Karnofsky Performance Score (KPS), and tumor recurrence. A sub-analysis stratifying by tumor grade (low, high) was completed. Among 643 PBT patients, the majority resided in more advantaged areas (n=490; 76%) and had high-grade tumors (n=448; 70%). Those who were less advantaged reported more symptom severity (β=1.10, SE=1.05, P=0.041), more activity-related interference (β=1.11, SE=1.05, P=0.03), and had 4.68 times the odds of having moderate-severe difficulty completing self-care tasks (95% confidence interval [1.46, 14.96], P=0.009) compared to those who were more advantaged. KPS was a significant predictor for all three outcomes, and age at diagnosis was significant for PBT symptom severity. Among patients with low-grade tumors, those who were less advantaged reported higher symptom interference (β=1.24, SE=1.10, P=0.020), affective symptoms (β=1.24, SE=1.08, P=0.007), treatment-related symptoms (β=1.21, SE=1.08, P=0.021), general disease symptoms (β=1.26, SE=1.10, P=0.016), activity-related interference (β=1.26, SE=1.09, P=0.006), mood-related interference (β=1.20, SE=1.09, P=0.039), and anxiety (β=1.03, SE=1.02, P=0.023), with KPS being a significant predictor for all outcomes except anxiety. Overall, less advantaged PBT patients reported worse symptom severity and activity-related interference, and those with low-grade tumors experienced more affective symptoms, including fatigue, general disease and treatment-associated symptoms, and symptoms of anxiety, but not neurologic or cognitive symptoms. Living in less advantaged areas may lead to barriers to care access, symptom management medications, or other neighborhood resources that could alleviate PBT patient symptom burden but may compound over time for those with poor performance status or low-grade tumors. These results highlight the need for targeted supportive interventions for PBT patients living in less advantaged areas. Citation Format: Zuena A. Abstract:Background and purpose: Since patients with primary brain tumor are expected to become long-term survivors, the prevention of long-term treatment-induced side effects is particularly important. This study aimed to explore whether symptom experience and symptom distress change over five years in adults with primary brain tumors treated with proton therapy. An additional aim was to explore whether symptom experience and symptom distress correlate. Materials and methods: The study had a longitudinal observational design. Adult (≥18 years) patients (n = 170) with primary brain tumors treated with proton therapy were followed over five years. Symptom experience and symptom distress were evaluated using the patient-reported Radiotherapy-Related Symptom Assessment Scale. Data from baseline, 1, 12, and 60 months were analyzed using non-parametric tests. Results: Of the 170 patients, the levels of symptoms and symptom distress were low. Fatigue increased at 1 (p=0.005) and 12 months (p=0.025) and was the most frequent symptom from baseline to 60 months' follow-up. Cognitive impairment increased at 12 (p=0.027) and 60 months (p<0.001) and was the most distressing symptom at 60 months' follow-up. There were significant, moderate to strong, correlations at all time points between symptom experience and symptom distress of fatigue, insomnia, pain, dyspnea, cognitive impairment, worry, anxiety, nausea, sadness, constipation, and skin reactions. Conclusion: Symptom experience and symptom distress changed in intensity over time with cognitive impairment as the most distressing symptom at 60 months. Future research should focus on identifying effective interventions aimed at alleviating these symptoms and reducing symptom distress for this vulnerable group of patients.

Abstract:Purpose: Primary brain tumors have a high propensity for causing physical and activity-related disabilities due to the location within the brain and use of targeted treatments resulting in significant symptom burden. In other cancers, physical function impacts mood, quality of life, and even mortality. This exploratory study aims to identify factors associated with activity-related functional interference in a cross-sectional cohort of 584 primary brain tumor patients enrolled in a natural history study (NCT03251989, PI: T.S. Armstrong). Methods: Sociodemographic (age, race, ethnicity, income, education, employment, body mass index [BMI]) and disease-related clinical data were collected at study entry. Activity-related interference (work, walking, and general activity) and health status were reported via the MD Anderson Symptom Inventory-Brain Tumor. Activity-related interference means ≥ 2 were categorized as moderate/severe. Logistic regression assessed univariate associations with moderate/severe interference, with sig. <0.05. Results: This sample had a median age of 49 years (range 18-85), and was mostly male (57%), with a high-grade tumor (grade 3/4) (74%), glioblastoma (39%), on active treatment (21%) and prior tumor recurrence (49%). Risk factors for moderate/severe activity-related interference included: ≥ 2 surgeries (OR = 1.55, 95% CI [1.11, 2.18], P = 0.011), ≥ 2 radiation treatments (OR = 2.87, 95% CI [1.59, 5.17], P < 0.001), currently on active treatment (OR = 1.47, 95% CI [0.97, 2.22], P = 0.066), disease progression (OR = 1.87, 95% CI [1.24, 2.84], P = 0.003), prior recurrence (OR = 1.57, 95% CI [1.13, 2.19], P = 0.007), and poor KPS (OR = 3.61, 95% CI [2.55, 5.12], P < 0.001), but not tumor grade (OR = 1.05, 95% CI [0.72, 1.54], P = 0.790). Sociodemographic risk factors for moderate/severe activity-related interference included: less than a high school education (OR = 2.03, 95% CI [1.18, 3.51], P = 0.011), on disability (OR = 3.20, 95% CI [1.79, 5.71], P < 0.001), or unemployed (OR = 3.81, 95% CI [2.34, 6.19], P < 0.001), an annual income less than $49,999 (OR = 2.62, 95% CI [1.28, 5.35], P = 0.008), or childhood in a rural area (OR = 1.87, 95% CI [1.24, 2.84], P = 0.003), but not BMI, smoking status, or past physical activity in the 10 years prior to report. Conclusion: As expected, activity-related interference is associated with disease and treatment associated clinical characteristics, with significant risk derived from sociodemographic factors such as education, childhood environment, employment, and household income. Future studies should further explore these associations longitudinally to identify individuals at the highest risk of activity-related interference due to their brain tumor as well as interventions designed for those who may be socioeconomically disadvantaged. Abstract:Pediatric brain tumor (PBT) survivors are at significantly increased risk of cognitive, psychosocial, and educational/vocational sequelae that impact health-related quality of life. These complications and health morbidities result in high burden on survivors and their families, particularly those already vulnerable to disparities in health care access and outcomes. Since neurological comorbidities are common in this population, neurologists are uniquely positioned to screen, treat identified symptoms, and connect families with services and resources. A tiered assessment approach can facilitate early identification of concerns and reduce barriers to care. We review common presenting conditions, highlight risk factors, and provide screening tools and recommendations to facilitate comprehensive survivorship care for PBT survivors.

Abstract:Abstract In our ongoing phase III randomized trial (NCT03741673) evaluating risk of leptomeningeal disease (LMD) in patients with brain metastasis, patients are stratified by tumor size, location, and histology, and randomized to pre-operative SRS versus post-operative SRS. The primary endpoint is 1-year LMD-free rate, and exploratory endpoints include neurocognitive function (NCF). NCF is assessed prior to treatment and longitudinally (1.5, 6, and 12 months from the start of treatment) with the HVLT-R, TMT, and COWA yielding the Clinical Trial Battery Composite score (CTB COMP). Patients also complete the MDASI-BT, which includes a Cognitive Factor measuring patient reported cognitive concerns. Currently, 41 patients completed pre-treatment NCF baseline. Characteristics of patients with pre-treatment NCF baseline include mean (SD): age 60.0+/-11.7 years, 2.6+/-2.2 metastases, 15.2+/-10.1 cc brain metastases volume, median KPS 90 (range: 70-100), 83% white, 52% male, and 50% received AEDs at time of surgery. The most common primary cancers among this cohort were: 31% NSCLC, 19% RCC, 14% melanoma, 7% breast, 2% thyroid, and 26% other. Mean (SD) NCF z-scores ranged from z= -0.09 (1.08) to -3.09 (6.13), CTB COMP z=-1.13 (1.65), and the mean (SD) MDASI-BT Cognitive Factor score was 1.1 (1.3). Using multivariable linear regression including CTB COMP, MDASI-BT Cognitive and Affective Factor, KPS, number of and intracranial volume of metastases, only intracranial volume of metastases was statistically significantly associated with CTB COMP (B=-.058, p=.034) and only the MDASI-BT Affective Factor was statistically significantly associated with the MDASI-BT Cognitive Factor (B=.305, p=<.001). At baseline, disease factors such as tumor burden appear to be the primary driver of objectively measured neurocognitive dysfunction and mood factors appear to be the primary driver of subjective patient reported neurocognitive concerns. This has important implications for interpretation of these different clinical outcome assessments in clinical trials measuring objective neurocognitive function and subjective neurocognitive concerns.

Abstract:Introduction: Intramedullary spinal cord tumors (IMSCTs) are rare but clinically significant entities that may cause severe neurological decline with progressive pain and motor or sensory deterioration. Beyond the beneficial effects of surgical treatment and the long-term progression-free survival, neurological deficits may still persist after surgery, and information about the long-term patients' health-related quality of life (QoL) is still lacking. In this study, we investigate the patients' health perception 15 years after the surgery in an overall patients' wellbeing framework. Methods: Patients surgically treated for IMSCT over a period from 1996 to 2011 were selected. After a mean of 15 years from the surgery, patient's self-administered questionnaire on disability, pain, sleep quality, and QoL was collected and neurological postoperative evaluation at the chronic stage was reexamined. Results: Neurological deficits are reported in half of the patients in the postoperative chronic phase. After 15 years of surgery, half of the patients still report mild or severe disability grades associated with significantly higher pain and poor sleep and QoL. In accordance, the neurological condition measured at the chronic stage is significantly related not only to disease-specific symptoms (i.e., pain) but even to sleep quality complaints and poor QoL, measured at 15 years follow-up. Conclusions: Health-related QoL is an important secondary outcome in patients. Although the progression-free survival, worse postoperative neurological conditions could predict long-term sequelae reflecting patients' poor health perception. It suggests the importance of preserving patients' functional status and globally evaluating patients' wellbeing to handle disease-specific symptoms but even more general aspects of QoL.
A Multi-Institutional Retrospective Analysis of T-Cell Lymphomas with Central Nervous System Relapse

Abstract:Abstract Introduction Central nervous system (CNS) relapse (CNSr) in patients with aggressive non-Hodgkin lymphoma (NHL) occurs uncommonly (estimated incidence 5%) but carries a high morbidity and mortality. Studies have identified risk factors for CNSr such as high tumor burden and extranodal (EN) disease. However, most focus on B-cell NHL with minimal data in T-cell lymphomas (TCL), which are significantly less common and more heterogenous. The few small series of CNSr in TCL report a median overall survival (OS) less than 3 months (mo) with incidence ranging from 2.6-9%. To better define CNSr in TCL, we performed a multi-institutional retrospective analysis of TCL patients with CNSr and herein describe clinicopathologic characteristics and treatment of CNSr. Methods We performed a retrospective observational study using data from 9 US academic centers with IRB approval at individual sites. We included adult patients diagnosed with a mature T-cell neoplasm as per the 2016 WHO classification between 1/1/2009-1/1/2019, who were found to have CNSr at any time after initial diagnosis, and collected patient, disease, and treatment characteristics at time of initial diagnosis as well as at CNSr. Patients with a diagnosis of a precursor T-cell malignancy or with CNS disease identified at initial TCL diagnosis (TCLd) and/or prior to first-line systemic treatment were excluded. Results In this analysis, we report the outcomes of 75 patients (male n=45, female n=30). At TCLd, the median age was 59 years (range 20-81), and 61% of patients (n=46) had an IPI score of at least 3, 92% (n=69) had EN involvement with 37% (n=28) involving at least 2 EN sites, and 59% (n=44) had BM involvement. The most common pathologic diagnoses were peripheral T-cell lymphoma, not otherwise specified (PTCL, NOS; 24%, n=18), angioimmunoblastic T-cell lymphoma (AITL; 17%, n=13), adult T-cell leukemia/lymphoma (ATLL; 17%, n=13), and mycosis fungoides (MF; 12%, n=9) (Figure 1A). First-line systemic therapy for TCL included anthracyclines for 72% (n=54). Autologous and allogenic transplants were performed prior to CNSr in 12% (n=9) and 8% (n=6) of patients, respectively. Prior to CNSr, 48% (n=36) had non-CNS relapse. Some form of CNS prophylaxis was used during initial systemic lymphoma therapy in 24% of patients (n=18), predominantly intrathecal methotrexate (IT MTX; n=16). Median time from TCLd to CNSr was 8.5mo, though this was significantly longer in MF (46.8mo [range 17.5-187.7]) versus PTCL, NOS (7.6mo [range 1.1-58.4], P=0.0002), AITL (21.2mo [range 2.0-61.6, P=0.008), and ATLL (7.3mo [range 0.7-46.4], P=0.0005) (Figure 1B). CNSr developed within 6mo of TCLd in 31% of patients (n=23) and within 12mo in 57% (n=43). Symptoms related to CNSr occurred in 71% of patients (n=53). CNSr patterns were 61% leptomeningeal (n=46), 21% parenchymal (n=16), and 17% both (n=13) with no significant survival difference between leptomeningeal or parenchymal disease alone (HR 1.45, 95% CI 0.78-2.70, P=0.28). Concomitant systemic relapse was observed in 59% of patients (n=44). The most common CNS-directed therapy for CNSr was IT MTX (56%; n=42), though multiple different IT and/or systemic regimens were used. Patients received a median of 1 line of CNS-directed treatment (range 0-5). The overall response rate to initial CNS directed treatment was 32% (16% CR, 16% PR). Median follow up after CNSr was 40.7mo. At last follow up, 83% had died (n=62). Median OS after CNSr was 4.6mo (range 0.1-68.7) (Figure 1C). Those with ATLL had the shortest median OS after CNSr (2.7mo) versus 6.3mo in MF (HR 3.69, 95% CI 1.44-9.42, P=0.005), 6.5mo in AITL (HR 2.16, 95% CI 0.92-5.06, P=0.054), and 4.8mo in PTCL, NOS (HR 1.49, 95% CI 0.70-3.19, P=0.27) (Figure 1D). The most common cause of death was progressive lymphoma (77%; n=48). Conclusions This is to our knowledge the largest series of CNSr in TCL to date. Most CNSr occurred within 12mo, though CNSr occurred later in patients with MF. Although the prognosis after CNSr was generally poor, we found that median OS in CNSr was longer than previously reported, perhaps reflecting more effective treatments for CNS and systemic relapse, inclusion of MF, or lead time bias. Further analysis of the impact of different treatment strategies and outcomes in CNSr was limited by the small sample size and heterogeneity within our cohort, and future analyses in a larger cohort should focus on factors associated with outcomes. Abstract:Purpose: The goal of this analysis was to provide insight into differences in perceived symptom burden and general health status between young adults (YA; 18-39 years of age) and older adults (OA; ≥ 40 years of age) with primary central nervous system (PCNS) tumors. Determining differences in characteristics that impact symptom burden and general health status in YA will allow for tailored survivorship programs that meet the unique needs of this population. Methods: Data were retrospectively analyzed from the National Cancer Institute Neuro-Oncology Branch's Natural History Study (NCT02851706 PI: T.S. Armstrong), which is an observational prospective cohort study designed to follow and collect PROs and clinical information on patients diagnosed with PCNS tumors throughout their disease trajectory. Differences in patient-reported outcomes (general health status [EQ-5D-3L], symptom burden [MDASI-BT and MDASI-SP], anxiety/depression [PROMIS], and perceived cognition [Neuro-QOL]) and demographic and clinical data were assessed using chi-square tests, one-way ANOVAs, and Students' t-tests. Linear regression with backward elimination determined which characteristics impacted perceived symptom burden and general health status as evidenced by MDASI-BT and EQ-5D-3L, respectively. Results: The sample included 271 YA (82% with a primary brain tumor [PBT]; median age 31 [range, 18-39]) and 516 OA (88% with a PBT; median age 54 [range, 40-85]). YA were more likely to be single (P<0.001), employed (P<0.001), and make < $50,000 per year (P=0.014). More YA reported pain (P=0.008), nausea (P<0.001), drowsiness (P=0.043), and vomiting (P=0.001) than OA. More YA presented with vision problems (25% of YA, 16% of OA), whereas more OA presented with cognitive deficits (17% of YA, 25% of OA). Compared to YA, 7% more OA with PBTs and 19% more OA with primary spinal tumors (PST) were diagnosed in outpatient clinics, whereas the majority of YA with PBTs (47%) were diagnosed in an emergency room. Among demographic and clinical characteristics, only Karnofsky Performance Scale score was a predictor of general health status (P<0.001) in YA with PCNS tumors and symptom interference (P=0.014) in YA with PBTs. YA reported more affective, cognitive, neurologic, treatment-related, generalized disease, and gastrointestinal symptom burden than OA, with the affective factor being the most severe among the symptom factors reported. Among individuals with PSTs, 20% more YA reported moderate-severe anxiety (P=0.050) and 32% more YA reported moderate-severe perceived cognitive deficits (P=0.023) than OA. Conclusion: YA patients were more likely to be single, employed, have a lower income, and report more symptom burden than OA. These findings underscore the need for tailored survivorship programs providing additional psychosocial support and resources to address symptom presentation in YA. Abstract:Abstract Cognitive dysfunction is commonly reported but variable in course and severity in the primary brain tumor (PBT) population. Brief, reliable, and sensitive measures of patient-reported outcomes (PRO) of cognitive symptoms on function are needed for clinical care and research. This study aims to assess the reliability and validity of the Neuro-QoL Perceived Cognitive Function tool (NQC) and explore associations with symptoms and objective testing of cognitive dysfunction in a cohort of PBT patients enrolled in a natural history study (NCT02851706: PI T. Armstrong). Patient sample characteristics and PROs, including NQC (<40 = moderate/severe (MS) dysfunction), a symptom burden measure (MD Anderson Symptom Inventory-Brain Tumor (MDASI-BT), scores ≥5 = MS) and the Montreal Cognitive Assessment ((MoCA), scores ≤ 25 indicating cognitive impairment (range: 0-30)) were collected. Descriptive statistics and a psychometric analysis consisting of bivariate correlations, Cronbach’s alpha, and factor analyses using principal axis factoring with nonorthogonal rotation were performed. Cohen’s kappa and correlation were used to assess agreement between objective and subjective measures of cognitive function. The cohort included 327 patients who completed the NQC at study entry [median age: 47 years (range: 18-85), primarily white (78%) males (55%) with high-grade tumors (73%), 65% with a Karnofsky Performance Status (KPS) ≥ 90 (good), and 29% reporting MS cognitive dysfunction]. Factor analysis of the NQC identified two underlying factor groupings: concentration/focus and executive function with Cronbach’s alphas of 0.92 and 0.91 that are interpretable and provided adequate fit for the data. While the NQC was moderately correlated with several MDASI-BT symptom factors, it was highly associated with the cognitive factor (r = -.646). To assess agreement, a subset of patients also completed the MoCA (n=172). These were primarily male (58%), white (79%), had high-grade tumors (67%), and a good KPS score (67%). In this subset, 24% had MS cognitive dysfunction as measured by the NQC while 44% were cognitively impaired using the MoCA resulting in a kappa agreement of 0.07. The correlation between the MoCA score and NQC T-score was -0.292. Understanding the impact and prevalence of cognitive symptoms, objective cognitive testing, and perceived impact is essential for PBT patient care and identifying supportive care interventions. Select NQC psychometric properties demonstrate reliability and validity in this population, with strong agreement with self-report of cognitive symptoms but weaker correlation with objective testing, highlighting the variability in perceived impact. Future research in cognitive function will explore the use of objective testing and perceived impact, its predictors in select populations (i.e., IDH-mutant tumors), and examine its association with biomarkers. Abstract:This article uses qualitative methods to describe the domains of health-related quality of life (HRQoL) that adolescent and young adult (AYA) survivors of central nervous system (CNS) tumors identify as important. Survivors clearly attributed aspects of their current HRQoL to their disease or its treatment. We identified 7 key domains of AYA CNS tumor survivorship: physical health, social well-being, mental health, cognitive functioning, health behaviors, sexual and reproductive health, and support systems. Although most aspects of HRQoL that survivors discussed represented new challenges, there were several areas where survivors pointed out positive outcomes. There is a need for a HRQoL tool designed for this population of survivors, given their unique treatment and survivorship experience. Aspects of HRQoL related to cognition, sexual and reproductive health, health behaviors, and support systems are not typically included in generic HRQoL tools but should be assessed for this population. Developing HRQoL measurement instruments that capture the most significant aspects of HRQoL will improve the ability to track HRQoL in AYA CNS tumor survivors and in the long-term management of common sequelae from CNS tumors and their treatments.

Neuropsychological rehabilitation for survivors of childhood and adolescent brain tumors: a view of the past and a vision for a promising future

Abstract:The neuropsychological sequelae associated with the diagnosis and treatment of a brain tumor in childhood or adolescence are well described. Providers in the oncology field have become more aware of these problems in recent years, and some treatment regimens are now aimed at limiting neurocognitive toxicity. Nevertheless, the likelihood of cure for many brain tumors is still relatively low and aggressive treatment courses often cannot be avoided. The neuropsychological side effects of surgery, radiation, chemotherapy, and even the tumor itself will remain difficult to avoid until tumor-directed therapy is improved in dramatic ways. After three decades of research and a multitude of published studies, the nature of these neurocognitive deficits and the associated risk factors are well-understood, but there is a paucity of research aimed at therapeutic interventions. This is the essential step that must be taken to translate our current understanding into improved quality of life for children who survive the underlying disease. Unfortunately, the current standard of care for treatment of these complex neurobehavioral deficits relies on a strained public school system. Recent advances and current approaches in treatment interventions for neuropsychological sequelae are reviewed in this article, including pharmacologic treatments, cognitive remediation derived from traumatic brain injury rehabilitation, and comprehensive school re-integration efforts. There is an acute need for further advances in this field, and a bright future of individualized school re-integration is within reach.
