Abstract:A variable cluster method combining scenario cognition and membership degree ranking was proposed based on fuzzy C-means (FCM) cluster algorithm aiming at the postoperative survival prediction of colorectal cancer patients. Dimension reduction on samples of colorectal cancer patients were conducted; six characteristic variables were selected. Next, a postoperative survival prediction model was constructed for colorectal cancer patients with BP neural network. To verify the validity of this model, principal component analysis (PCA) was used to reduce the dimensions of the sample to train a BP neural network, and the comparison of accuracy rates between models based on FCM and PCA was conducted. Results verifly that the BP neural network model based on FCM variable cluster has more accurate prediction rate. The proposed variable cluster method can effectively screen out variables that have high pertinence and good interpretability of survival time, thus improves the forecast accuracy of BP neural network model.
Application of Artificial Neural Network in the Study of Factors Affecting Hospitalization Expenses on Diabetes

Abstract:Objective To analyze the main influencing factors of hospitalization expenses on diabetes,to explore effective analytical methods and to provide a theoretical basis for reasonable control of hospitalization expenses on diabetes. Method Inpatient medical record summaries of patients with type 2 diabetes in 2011 from 13 top hospitals in Beijing were used as data source. Back-propagation(BP) Artificial Neural Network(ANN) model was employed to do analysis and it was compared with the Multiple Liner Regression(MLR) model to analyze the influencing factors of hospitalization expenses on diabetes. Result(1) The performance of ANN is superior than MLR in identifying main factors affecting hospitalization expenses on diabetes.(2) Top 2 major factors affecting hospitalization expenses are hospitalization days and discharge department. And the top 3 single costs are medicine cost, assay cost and test cost. Conclusion ANN performs better than MLR in identifying the main factors affecting hospitalization expenses on diabetes.
Medical expenditure estimation by Bayesian network for lung cancer patients at different severity stages

Abstract:Lung cancer is one of the leading causes of mortality, and its medical expenditure has increased dramatically. Estimating the expenditure for this disease has become an urgent concern of the supporting families, medial institutes, and government. In this study, a conditional Gaussian Bayesian network (CGBN) model was developed to incorporate the comprehensive risk factors to estimate the medical expenditure of a lung cancer patient at different stages. A total of 961 patients were surveyed by the four severity stages of lung cancer. The proposed CGBN model identified the correlation and association of 15 risk factors to the medical expenditure of different severity stages of lung cancer patients. The relationships among the demographic, diagnosed-based, and prior-utilization variables are constructed. The model predicted the lung cancer-related medical expenditure with high accuracy of 32.63%, 50.30%, 50.36%, and 66.58%, respectively for stages 1-4, as compared with the reported models. A greedy search was also applied to find the upper threshold of R<sup>2</sup>, while our model also shows that it approached the upper threshold.

Forecasting Single Disease Cost of Cataract Based on Multivariable Regression Analysis and Backpropagation Neural Network

Abstract:In medical services, charge according to the disease is an important way to promote the reform of pricing mechanism, control the unreasonable growth of medical expenses, as well as reduce the burden on patients. Single disease cost forecasting that both identify potential influencing or driving factors and enable better proactive estimation of costs can guide the management and control of medical costs. This study aimed to identify the factors that affect the medical costs of single disease cataract and compare 2 regression models for anticipating acceptable medical cost forecasts. For this purpose, 483 patients with cataract surgery completed in West China Hospital from May 1, 2015, to October 1, 2015, were selected from hospital information system. For cost forecasting, multivariable regression analysis (MRA) and backpropagation neural network (BPNN) were used. Analysis of data was performed with SPSS21.0 and MATLAB2014a software. Total medical costs of patients with cataract (n = 483) ranged from 2015.00 to 13 359.00 CNY, and the mean ± standard deviation is 6292.29 ± 2639.43 CNY. Factors influencing costs of cataract in the MRA include, in importance order, intraocular lens (IOL) implantation (| r|: 0.805, P < .01), doctor level (| r|: 0.644, P < .01), payment source (| r|: 0.554, P < .01), admission status (| r|: 0.326, P < .01), additional diagnosis (| r|: 0.260, P < .01), type of surgery (| r|: 0.127, P < .05), and type of anesthesia (| r|: 0.126, P < .05). In terms of forecasting performance, BPNN (average error: 2.81%) outperforms, yet is less interpretable than MRA (average error: 5.79%). Both MRA and BPNN are technically and economically feasible in generating medical costs of cataract. And some insights on using results of the forecasting model in controlling and reducing disease costs are obtained.

Abstract:Brain metastases (BMs) are increasingly a cause of significant cancer-related morbidity, mortality, and economic burden. While BM-related healthcare costs have been previously described, predictive analytics based on contemporary cohorts, particularly for inpatient care, remain unavailable. This study sought to evaluate inpatient care costs (IPCC) and associated reimbursements for patients with BMs and perform predictive modeling through supervised machine learning (sML) using a contemporary, commercial US claims database. This retrospective study, reported following STROBE and TRIPOD+AI reporting guidelines, included adult patients (aged 18-65 years) in the United States (US) with BMs included in the MarketScan Commercial Claims and Encounters Database who received inpatient care during January 2016 to December 2021. Eligible patients were identified using ICD-10 codes. IPCC measures were adjusted using the Medical Care component of the 2023 Consumer Price Index. The primary study outcome was total IPCC, represented by adjusted gross total payments. After log-transformation performed due to data skew, sML was carried out using Random Forest algorithm with 1000 iterations with a 70:30 training-test data split, followed by residual analysis and evaluation of predictive performance using root-mean-squared error (RMSE), out-of-bag error (OOBE), and calibration curve. Multivariable linear regression was carried out for sensitivity analyses. A total of 25117 unique inpatient admissions across 13477 BM patients were included, with median (IQR) age of 57 years (51-61). Gross median (IQR) CPI-medical-care-adjusted values per admission were: total payments 24,678.74 (14601.66-47864.84); and payments to principal physician $884.80 (441.07-2434.15), being 3.6% (IQR 1.9-6.8) of total payment. Median inpatient length of stay was 4 days (IQR 2-8). ML model had an OOBE of 0.61 and, using test data, RMSE of 0.85, indicating robust performance. No major deviations were found on residual analysis. Contemporary IPCCs for BM patients in US are substantial but driven minimally by payments to principal physician. sML may be utilized to successfully predict IPCC for BM patients.

Abstract:Objective : The purpose of graded care for chronic kidney disease is to share expert experience, so that doctors can more accurately diagnose chronic kidney disease, so that patients with chronic kidney disease can understand their condition in time and collect case data. The collected case data is established into a data warehouse, the data quality is evaluated, and the BP neural network method is used for data mining to analyze the data. Methods : The paper studied BP neural network and probabilistic neural network (PNN), and used 75% of the samples to compare the models. The model errors were analyzed including maximum, minimum, expectation, variance and running time to get Adaboost. The accuracy and robustness of the -PNN model and the IGABP model are good. Results : BP neural network model and probabilistic neural network method can achieve higher application of graded care for chronic kidney disease. The method is capable of quickly predicting disease grading and providing a standardized treatment care regimen. The method realizes the main functions of querying, managing, and collecting data of medical records. Conclusion : The external expansion function of BP neural network and probabilistic neural network can achieve accurate data analysis, which can effectively improve the diagnosis time and grade prediction accuracy of chronic kidney disease, and provide opinions for graded nursing.

Abstract:Introduction Many researchers used machine learning (ML) to predict the prognosis of breast cancer (BC) patients and noticed that the ML model had good individualized prediction performance. Objective The cohort study was intended to establish a reliable data analysis model by comparing the performance of 10 common ML algorithms and the the traditional American Joint Committee on Cancer (AJCC) stage, and used this model in Web application development to provide a good individualized prediction for others. Methods This study included 63145 BC patients from the Surveillance, Epidemiology, and End Results database. Results Through the performance of the 10 ML algorithms and 7th AJCC stage in the optimal test set, we found that in terms of 5-year overall survival, multivariate adaptive regression splines (MARS) had the highest area under the curve (AUC) value (0.831) and F1-score (0.608), and both sensitivity (0.737) and specificity (0.772) were relatively high. Besides, MARS showed a highest AUC value (0.831, 95%confidence interval: 0.820–0.842) in comparison to the other ML algorithms and 7th AJCC stage (all P < 0.05). MARS, the best performing model, was selected for web application development (https://w12251393.shinyapps.io/app2/). Conclusions The comparative study of multiple forecasting models utilizing a large data noted that MARS based model achieved a much better performance compared to other ML algorithms and 7th AJCC stage in individualized estimation of survival of BC patients, which was very likely to be the next step towards precision medicine.

Abstract:Objective To explore the main factors affecting medical expenses of patients with colorectal cancer(CRC)and reduce such expenses by optimizing the clinical pathway of CRC according to the influencing factors.Methods Data of 467 discharged and medical insured CRC patients at Shanghai Cancer Center,Fudan University,were collected from 2009 to 2010.A multiple linear regression model was applied to analyze the influencing factors of medical expenses.The clinical pathway of CRC was optimized,and medical expenses before and after optimization were compared.Results Our data found days of stay and complications had impact on the medical expenses.Also,medical expenses were decreased accordingly with optimization of clinical pathway of CRC.Conclusion This study provides evidence for decision-making on single disease payment and medical insurance compensation on CRC.It is also meaningful to reduce the economic burden of patients with CRC.

Abstract:Objective:To study the direct medical expenses for inpatients with Multiple Myeloma and explore its influencing factors. Methods:A total of 1386 hospitalization records were collected using the reimbursement claim database in Guangzhou City during the period from January 2009 to December 2011 , which covered the entire enroll-ees of Urban Employee Basic Medical Insurance ( UEBMI) and Urban Resident Basic Medical Insurance ( URBMI) . Multivariate logarithmic regression models were used to analyze the influencing factors of direct hospitalization costs. Results:During the period from 2009 to 2011 , the total costs of inpatients with Multiple Myeloma were 19319 . 16 , 17364. 76 and 21427. 44 Yuan, and the individual out-of-pocket cash payments were 7049. 37, 5216. 5 and 6475. 39 Yuan respectively. The major influencing factors on direct medical costs of inpatients with the aforesaid disease were identified to be age, health insurance type, hospital level and length of hospitalization. Besides, the proportion of drug expenses as per average occupancy rate among UEBMI and URBMI cases during three years were 61. 92% and 69. 22% respectively. Conclusion: The direct medical costs for inpatients with Multiple Myeloma are high and the medicine cost is the main component of total medical expenses. In addition, the economic burden is higher for patients with URBMI than for those with UEBMI for treating Multiple Myeloma.
The Application and Comparison of Machine Learning Models for the Prediction of Breast Cancer Prognosis: Retrospective Cohort Study

Abstract:Background Over the recent years, machine learning methods have been increasingly explored in cancer prognosis because of the appearance of improved machine learning algorithms. These algorithms can use censored data for modeling, such as support vector machines for survival analysis and random survival forest (RSF). However, it is still debated whether traditional (Cox proportional hazard regression) or machine learning-based prognostic models have better predictive performance. Objective This study aimed to compare the performance of breast cancer prognostic prediction models based on machine learning and Cox regression. Methods This retrospective cohort study included all patients diagnosed with breast cancer and subsequently hospitalized in Fudan University Shanghai Cancer Center between January 1, 2008, and December 31, 2016. After all exclusions, a total of 22,176 cases with 21 features were eligible for model development. The data set was randomly split into a training set (15,523 cases, 70%) and a test set (6653 cases, 30%) for developing 4 models and predicting the overall survival of patients diagnosed with breast cancer. The discriminative ability of models was evaluated by the concordance index (C-index), the time-dependent area under the curve, and D-index; the calibration ability of models was evaluated by the Brier score. Results The RSF model revealed the best discriminative performance among the 4 models with 3-year, 5-year, and 10-year time-dependent area under the curve of 0.857, 0.838, and 0.781, a D-index of 7.643 (95% CI 6.542, 8.930) and a C-index of 0.827 (95% CI 0.809, 0.845). The statistical difference of the C-index was tested, and the RSF model significantly outperformed the Cox-EN (elastic net) model (C-index 0.816, 95% CI 0.796, 0.836; P=.01), the Cox model (C-index 0.814, 95% CI 0.794, 0.835; P=.003), and the support vector machine model (C-index 0.812, 95% CI 0.793, 0.832; P<.001). The 4 models’ 3-year, 5-year, and 10-year Brier scores were very close, ranging from 0.027 to 0.094 and less than 0.1, which meant all models had good calibration. In the context of feature importance, elastic net and RSF both indicated that TNM staging, neoadjuvant therapy, number of lymph node metastases, age, and tumor diameter were the top 5 important features for predicting the prognosis of breast cancer. A final online tool was developed to predict the overall survival of patients with breast cancer. Conclusions The RSF model slightly outperformed the other models on discriminative ability, revealing the potential of the RSF method as an effective approach to building prognostic prediction models in the context of survival analysis.

Abstract:To study the medical expenditure of insured patients with cancer and its influence factors, to analyze the utilization of pharmaceuticals, treatment-seeking behavior and the relationship between the medical expenditure and payment patterns of medical insurance. This study involved 5351 cancer cases in 3 municipalities (Beijing, Shanghai, Chongqing) and 5 province-capital cities (Shenyang, Fuzhou, Jinan,Zhengzhou, Xining).The actual claim data of their medical expenditure and medical care utilization in 2008 were collected. Descriptive analysis and multivariate linear regression analysis were applied. 1) Outpatient's annual medical expenditure per head was US$802 and inpatient's annual medical expenditure per head was US$6200. Although the basic medical insurance (BMI) fund covered about 70% of the expenditure, the patients' economic burden is still heavy. 2) Western medicine cost accounted for 44% to 60% of the inpatients' medical expenditure in various cities. Cytotoxic drug usage accounted for over 80% of the anti-tumor drugs. 3) The proportion of the patients who sought outpatient and inpatient treatment in the third level hospitals was 81% and 74 %, respectively. 4) Multivariate linear regression analysis showed that the main influence factors of inpatients' medical expenditure include medical insurance payment pattern, hospital level, category of cancer, gender, length of stay in hospital, and medical insurance type. When all factors other than payment pattern were set to be control parameters, inpatient's annual medical expenditure per head under flat rate payment was US$348 lower than that under fee-for service. Medical expenditure causes heavy burden for cancer patients and the BMI fund. Flat rate payment has proved to be more effective in expenditure control compared with fee-for service. To keep the fund safe and running effectively, it is necessary to adjust present payment patterns and set rational payment standard to encourage medical care providers to control the expenditure actively.
Comparison of Breast Cancer Risk Predictive Models and Screening Strategies for Chinese Women

Abstract:Background: Previous studies have shown that organized mammographic screening implementation in China may not be cost-effective. Our aim was to develop a valid predictive mathematical model for selecting high-risk groups eligible for mammography examinations (MAMs) and cost-effective strategies for breast cancer screening among Chinese women. Methods: Between 2009 and 2012, 13,355 eligible women aged 30-65 years were enrolled from the community in Chengdu City. All subjects were administered a valid questionnaire and given MAMs. Using biopsies and 1-year follow up, we compared the accuracy indexes of three predictive models (back-propagation artificial neural network [BP-ANN], logistic regression [LR], and Gail) and four serial screening strategies (BP-ANN→MAM, LR→MAM, Gail→MAM, and MAM alone). We also evaluated the benefits of the four strategies by comparing their incidence-adjusted positive predictive value (PPV). All analyses were conducted with three age-based subgroups: 30-39, 40-49, and 50-65. Results: The BP-ANN1, in conjunction with additional continuous risk factor variables, was the best predictive model, with the highest sensitivity (SEN, 76.99%) and specificity (SPE, 54.20%). The BP-ANN1→MAM strategy was best for the 40-49 age group, with the highest adjusted PPV (9.80%) and reasonable SEN (81.82%). Conclusion: We found that the BP-ANN model performed the best and was the most accurate for predicting high risk for breast cancer among Chinese women, and the BP-ANN→MAM screening strategy was most effective among the 40-49 age group. However, mammography alone may be a sufficient screening strategy for women aged 50-65.
Medical expenditure and its influencing factors of patients with hypertension in Shanxi Province, China: a study based on ‘System of Health Accounts 2011’ and multiple-layer perceptron neural network

Abstract:Objectives To study the medical expenditure and influencing factors of patients with hypertension in Shanxi Province, China. Design A cross-sectional study. Setting 1088 medical institutions, including general hospitals, traditional Chinese medicine hospitals, special hospitals, basic medical institutions and public health institutions. Participants 180 441 hypertensive outpatients and 14 763 inpatients from 1 January to 31 December 2017. Primary and secondary outcome measures Curative care expenditure for hypertension (CCE ht ) was measured based on System of Health Accounts 2011. Influenced factors were analysed by univariate analysis and multiple layer perceptron neural network. Results In 2017, CCE ht was US$307.71 million, accounting for 3.63% of total CCE and 0.14% of gross domestic product (GDP) in Shanxi Province of China. CCE of hypertensive outpatients (CCE ht-out ) and inpatients (CCE ht-in ) accounted for 44.49% and 55.51% of CCE ht . Drug fee accounted for 81.55% of CCE ht-out and 22.50% of CCE ht-in , respectively. The top three influencing factors were drug fee, surgical fee and hospitalisation days for inpatients, and drug fee, examination fee and test fee for outpatients. Conclusions The medical expenditure of hypertension is still heavy for individuals and society. The diagnosis and treatment capacities of primary healthcare system has been enhanced. New rural cooperation medical insurance and urban employee basic medical insurance have a trend of overusing, and the burden of family healthcare expenditure is still heavy. To ease the economic burden of patients with hypertension and improve the efficiency of social resources utilisation, policymakers should pay more attention to key groups, further increase support for primary healthcare system, standardise the treatment and reimbursement of hypertension and incline the reimbursement policy to outpatient service.

Abstract:Objective: To assess the effectiveness of different machine learning models in estimating the pharmaceutical and non-pharmaceutical expenditures associated with Diabetes Mellitus type II diagnosis, based on the clinical risk index determined by the analysis of comorbidities. Materials and methods: In this cross-sectional study, we have used data from 11,028 anonymized records of patients admitted to a high-complexity hospital in Bogota, Colombia between 2017-2019 with a primary diagnosis of Diabetes. These cases were classified according to Charlson's comorbidity index in several risk categories. The main variables analyzed in this study are hospitalization costs (which include pharmaceutical and non-pharmaceutical expenditures), age, gender, length of stay, medicines and services consumed, and comorbidities assessed by the Charlson's index. The model's dependent variable is expenditure (composed of pharmaceutical and non-pharmaceutical expenditures). Based on these variables, different machine learning models (Multivariate linear regression, Lasso model, and Neural Networks) were used to estimate the pharmaceutical and non-pharmaceutical expenditures associated with the clinical risk classification. To evaluate the performance of these models, different metrics were used: Mean Absolute Percentage Error (MAPE), Mean Squared Error (MSE), Root Mean Squared Error (RMSE), Mean Absolute Error (MAE), and Coefficient of Determination (R2). Results: The results indicate that the Neural Networks model performed better in terms of accuracy in predicting pharmaceutical and non-pharmaceutical expenditures considering the clinical risk based on Charlson's comorbidity index. A deeper understanding and experimentation with Neural Networks can improve these preliminary results, therefore we can also conclude that the main variables used and those that were proposed can be used as predictors for the medical expenditures of patients with diabetes type-II. Conclusions: With the increase of technology elements and tools, it is possible to build models that allow decision-makers in hospitals to improve the resource planning process given the accuracy obtained with the different models tested.

Abstract:Background: To construct two prognostic models to predict survival in breast cancer patients; to compare the efficacy of the two models in the whole group and the advanced human epidermal growth factor receptor-2-positive (HER2+) subgroup of patients; to conclude whether the Hybrid Bayesian Network (HBN) model outperformed the logistics regression (LR) model. Methods: In this paper, breast cancer patient data were collected from the SEER database. Data processing and analysis were performed using Rstudio 4.2.0, including data preprocessing, model construction and validation. The L_DVBN algorithm in Julia0.4.7 and bnlearn package in R was used to build and evaluate the HBN model. Data with a diagnosis time of 2018(n = 23,384) were distributed randomly as training and testing sets in the ratio of 7:3 using the leave-out method for model construction and internal validation. External validation of the model was done using the dataset of 2019(n = 8128). Finally, the late HER2 + patients(n = 395) was selected for subgroup analysis. Accuracy, calibration and net benefit of clinical decision making were evaluated for both models. Results: The HBN model showed that seventeen variables were associated with survival outcome, including age, tumor size, site, histologic type, radiotherapy, surgery, chemotherapy, distant metastasis, subtype, clinical stage, ER receptor, PR receptor, clinical grade, race, marital status, tumor laterality, and lymph node. The AUCs for the internal validation of the LR and HBN models were 0.831 and 0.900; The AUCs for the external validation of the LR and HBN models on the whole population were 0.786 and 0.871; the AUCs for the external validation of the two models on the subgroup population were 0.601 and 0.813. Conclusion: The accuracy, net clinical benefit, and calibration of the HBN model were better than LR model. The predictive efficacy of both models decreased and the difference was greater in advanced HER2 + patients, which means the HBN model had higher robustness and more stable predictive performance in the subgroup.
Lifespan and medical expenditure prognosis for cancer metastasis - a simulation modeling using semi-Markov process

Abstract:Background and objective: A key reason of high mortality of cancers is attributed to the metastasized cancer, whereas, the medical expense for the treatment of cancer metastases generates heavily financial burden. The population size of metastases cases is small and comprehensive inferencing and prognosis is hard to conduct. Methods: Because metastases and finance state can develop dynamic transitions over time, this study proposes a semi-Markov model to perform risk and economic evaluation associated to major cancer metastasis (i.e., lung, brain, liver and lymphoma cancer) against rare cases. A nationwide medical database in Taiwan was employed to derive a baseline study population and costs data. The time until development of metastasis and survivability from metastasis, as well as the medical costs were estimated through a semi-Markov based Monte Carlo simulation. Results: In terms of the survivability and risk associated to metastatic cancer patients, 80% lung and liver cancer cases are tended to metastasize to other part of the body. The highest cost is generated by brain cancer-liver metastasis patients. The survivors group generated approximately 5 times more costs, in average, than the non-survivors group. Conclusions: The proposed model provides a healthcare decision-support tool to evaluate the survivability and expenditure of major cancer metastases.
Machine learning prediction of hospitalization costs for coronary artery bypass grafting operations

Abstract:Background With the steady rise in health care expenditures, the examination of factors that may influence the costs of care has garnered much attention. Although machine learning models have previously been applied in health economics, their application within cardiac surgery remains limited. We evaluated several machine learning algorithms to model hospitalization costs for coronary artery bypass grafting. Methods All adult hospitalizations for isolated coronary artery bypass grafting were identified in the 2016 to 2020 Nationwide Readmissions Database. Machine learning models were trained to predict expenditures and compared with traditional linear regression. Given the significance of postoperative length of stay, we additionally developed models excluding postoperative length of stay to uncover other drivers of costs. To facilitate comparison, machine learning classification models were also trained to predict patients in the highest decile of costs. Significant factors associated with high cost were identified using SHapley Additive exPlanations beeswarm plots. Results Among 444,740 hospitalizations included for analysis, the median cost of hospitalization in coronary artery bypass grafting patients was $43,103. eXtreme Gradient Boosting most accurately predicted hospitalization costs, with R 2 = 0.519 over the validation set. The top predictive features in the eXtreme Gradient Boosting model included elective procedure status, prolonged mechanical ventilation, new-onset respiratory failure or myocardial infarction, and postoperative length of stay. After removing postoperative length of stay, eXtreme Gradient Boosting remained the most accurate model ( R 2 = 0.38). Prolonged ventilation, respiratory failure, and elective status remained important predictive parameters. Conclusion Machine learning models appear to accurately model total hospitalization costs for coronary artery bypass grafting. Future work is warranted to uncover other drivers of costs and improve the value of care in cardiac surgery.

Prediction Models for Chronic Postsurgical Pain in Patients with Breast Cancer Based on Machine Learning Approaches

Abstract:PurposeThis study aimed to develop prediction models for chronic postsurgical pain (CPSP) after breast cancer surgery using machine learning approaches and evaluate their performance.MethodsThe study was a secondary analysis based on a high-quality dataset from a randomized controlled trial (NCT00418457), including patients with primary breast cancer undergoing mastectomy. The primary outcome was CPSP at 12 months after surgery, defined as modified Brief Pain Inventory > 0. The dataset was randomly split into a training dataset (90%) and a testing dataset (10%). Variables were selected using recursive feature elimination combined with clinical experience, and potential predictors were then incorporated into three machine learning models, including random forest, gradient boosting decision tree and extreme gradient boosting models for outcome prediction, as well as logistic regression. The performances of these four models were tested and compared.Results1152 patients were finally included, of which 22.1% developed CPSP at 12 months after breast cancer surgery. The 6 leading predictors were higher numerical rating scale within 2 days after surgery, post-menopausal status, urban medical insurance, history of at least one operation, under fentanyl with sevoflurane general anesthesia, and received axillary lymph node dissection. Compared with the multivariable logistic regression model, machine learning models showed better specificity, positive likelihood ratio and positive predictive value, helping to identify high-risk patients more accurately and create opportunities for early clinical intervention.ConclusionsOur study developed prediction models for CPSP after breast cancer surgery based on machine learning approaches, which may help to identify high-risk patients and improve patients’ management after breast cancer.
MRI-Based Back Propagation Neural Network Model as a Powerful Tool for Predicting the Response to Induction Chemotherapy in Locoregionally Advanced Nasopharyngeal Carcinoma

Abstract:Background: Pretreatment individualized assessment of tumor response to induction chemotherapy (ICT) is a need in locoregionally advanced nasopharyngeal carcinoma (LANPC). Imaging method plays vital role in tumor response assessment. However, powerful imaging method for ICT response prediction in LANPC is insufficient. Purpose: To establish a robust model for predicting response to ICT in LANPC by comparing the performance of back propagation neural network (BPNN) model with logistic regression model. Study type: Retrospective. Population: A total of 286 LANPC patients were assigned to training (N = 200, 43.8 ± 10.9 years, 152 male) and testing (N = 86, 43.5 ± 11.3 years, 57 male) cohorts. Field strength/sequence: T2 -weighted imaging, contrast enhanced-T1 -weighted imaging using fast spin echo sequences at 1.5 T scanner. Assessment: Predictive clinical factors were selected by univariate and multivariate logistic models. Radiomic features were screened by interclass correlation coefficient, single-factor analysis, and the least absolute shrinkage selection operator (LASSO). Four models based on clinical factors (Modelclinic ), radiomics features (Modelradiomics ), and clinical factors + radiomics signatures using logistic (Modelcombined ), and BPNN (ModelBPNN ) methods were established, and model performances were compared. Statistical tests: Student's t-test, Mann-Whitney U-test, and Chi-square test or Fisher's exact test were used for comparison analysis. The performance of models was assessed by area under the receiver operating characteristic (ROC) curve (AUC) and Delong test. P < 0.05 was considered statistical significance. Results: Three significant clinical factors: Epstein-Barr virus-DNA (odds ratio [OR] = 1.748; 95% confidence interval [CI], 0.969-3.171), sex (OR = 2.883; 95% CI, 1.364-6.745), and T stage (OR = 1.853; 95% CI, 1.201-3.052) were identified via univariate and multivariate logistic models. Twenty-four radiomics features were associated with treatment response. ModelBPNN demonstrated the highest performance among Modelcombined , Modelradiomics , and Modelclinic (AUC of training cohort: 0.917 vs. 0.808 vs. 0.795 vs. 0.707; testing cohort: 0.897 vs. 0.755 vs. 0.698 vs. 0.695). Conclusion: A machine-learning approach using BPNN showed better ability than logistic regression model to predict tumor response to ICT in LANPC. Evidence level: 3 TECHNICAL EFFICACY: Stage 2.

Abstract:As the cost of diabetes treatment continues to grow, it is critical to accurately predict the medical costs of diabetes. Most medical cost studies based on convolutional neural networks (CNNs) ignore the importance of multi-granularity information of medical concepts and time interval characteristics of patients' multiple visit sequences, which reflect the frequency of patient visits and the severity of the disease. Therefore, this paper proposes a new end-to-end deep neural network structure, MST-CNN, for medical cost prediction. The MST-CNN model improves the representation quality of medical concepts by constructing a multi-granularity embedding model of medical concepts and incorporates a time interval vector to accurately measure the frequency of patient visits and form an accurate representation of medical events. Moreover, the MST-CNN model integrates a channel attention mechanism to adaptively adjust the channel weights to focus on significant medical features. The MST-CNN model systematically addresses the problem of deep learning models for temporal data representation. A case study and three comparative experiments are conducted using data collected from Pingjiang County. Through experiments, the methods used in the proposed model are analyzed, and the super contribution of the model performance is demonstrated.
