Abstract:Background: Physical activities play an important role in alleviating the aging problem and improving the physical fitness of middle-aged and elderly people. Blood flow restriction (BFR) training, also known as pressure training, has been widely used to improve athletes' performance and rehabilitation, which is a relatively novel exercise method for improving the physical fitness of middle-aged and elderly people. The purpose of this study is to conduct a systematic review and meta-analysis of domestic and foreign randomized controlled trial studies on BFR training for middle-aged and elderly women, further explore the impact of BFR training on health status. Methods: Meta-analysis was performed according to PRISMA standards, and charts were drawn using Review Manager 5.4 and Stata 17 software. In this study, the keywords such as "pressure training", "blood restriction training", "elderly women", "KAATSU", "blood flow restriction training" were used on CNKI, China Science and Technology Journal Database, PubMed, Embase, Web of Science, Cochrane Library, EBSCO, Scopus, and randomized controlled trials were searched in all languages. The search was performed from the establishment of database to 2 January 2024. The results of the combined effect were represented by standard mean differences. Results: Among the 681 literature retrieved, six eligible English articles were included in this meta-analysis. The overall effect test of the combined effect was performed on 10 groups of data, and the results were SMD = -0.18 (95%CI: -0.91 to 0.56; p > 0.05), the maximum dynamic force of 1RM SMD = 0.97 (95%CI: 0.35 to 1.58; p < 0.05), leg compression force SMD = -0.10 (95%CI: -0.78 to 0.57; p > 0.05), heart rate SMD = 0.33 (95%CI: -2.50 to 3.17; p > 0.05), systolic blood pressure (SBP) SMD = -1.44 (95%CI: -2.17 to -0.70; p < 0.05), diastolic blood pressure (DBP) SMD = -0.69 (95%CI: 2.54 to 1.15; p > 0.05). Conclusion: BFR training had a significant effect on the increase of the maximum dynamic force of 1RM and decrease of blood pressure in middle-aged and elderly women, but there was no significant difference found in heart rate and leg compression force. Abstract:Purpose: The aim of this study was to investigate the effect of blood flow restriction (BFR) combined with low-intensity resistance training (RT) on cardiovascular risk factors in obese individuals. Methods: Twenty-six male obese college students were recruited and randomly assigned to a control group (CON, n = 8), a low-intensity RT group (RT, n = 9), and a combined BFR training and low-intensity RT group (BFRT, n = 9). Results: The subjects in BFRT group showed significant reductions in body fat percentage and waist-to-hip ratio and a significant increase in lean mass and muscle mass; the peak torque, peak power, and endurance ratio of knee extensors and elbow flexors were significantly upregulated; the root mean square (RMS) for the medial femoral muscle, lateral femoral muscle and biceps significantly increased; the diastolic blood pressure (DBP) showed a significant decrease. The BFRT group also showed significant up-regulations in RMS of the difference between the adjacent R-R intervals (RMSSD), high-frequency power (HF) of parasympathetic modulatory capacity, the standard deviation of R-R intervals (SDNN) of overall heart rate variability (HRV) changes and low-frequency power (LF) of predominantly sympathetic activity. In addition, glycated hemoglobin (HbA1C), insulin resistance index (HOMA-IR) and fasting blood glucose (FBG) were all significantly downregulated in BFRT group. In parallel, low-density lipoprotein (LDL-C) significantly reduced while high-density lipoprotein (HDL-C) significantly increased in BFRT group. Conclusion: BFR combined with low-intensity RT training effectively improved body composition index, increased muscle mass, improved neuromuscular activation, enhanced muscle strength and endurance, which in turn improved abnormal glucolipid metabolism and enhanced cardiac autonomic regulation.

Abstract:Background: Changes in muscle mass, strength, vascular function, oxidative stress, and inflammatory biomarkers were compared in older adults after resistance training (RT) performed with low-intensity without blood flow restriction (RT-CON); low-intensity with BFR (RT-BFR); and high-intensity without BFR (RT-HI). Methods: Thirty-two untrained individuals (72±7 y) performed a 12-week RT after being randomized into three groups: RT-CON -30% of 1 repetition maximum (RM); RT-BFR -30% of 1RM and mild BFR (50% of arterial occlusion pressure); RT-HI -70% of 1 RM. Results: Improvements in handgrip strength were similar in RT-BFR (17%) and RT-HI (16%) vs. RT-CON (-0.1%), but increases in muscle mass (6% vs. 2% and -1%) and IGF-1 (2% vs. -0.1% and -1.5%) were greater (p < 0.05) in RT-BFR vs. RT-HI and RT-CON. Changes in vascular function, morphology, inflammation, and oxidative stress were similar between groups, except for time to reach maximum red blood cell velocity which showed a greater reduction (p < 0.05) in RT-BFR (-55%) vs. RT-HI (-11%) and RT-CON (-4%). Conclusion: RT with low intensity and mild BFR improved muscle strength and mass in older individuals while preserving vascular function. This modality should be considered an adjuvant strategy to improve muscle function in older individuals with poor tolerance to high loads.

Abstract:Abstract Background Resistance training with blood flow restriction (BFR) results in hypertrophy, and its magnitude depends on various training variables. This study aimed to compare the long-term effect of passive recovery (PR) and active recovery (AR) during low-intensity resistance training with BFR on hormonal levels and performance in young men. Methods In the randomized clinical trial, 20 men were randomly divided into PR and AR groups during resistance training with BFR. The intervention consisted of six upper and lower body movements with 30% of one maximum repetition (1RM), three sessions per week for six weeks. Both groups wore pneumatic cuffs on the proximal part of thighs and arms. The cuff pressure was 60% of the calculated arterial blood occlusion and increased 10% every two weeks. The AR group performed seven repetitions in 30 s break between sets by one second for concentric and eccentric phases and two seconds rest, and the other group had passive rest. The blood samples and a series of performance tests were gathered before and after the intervention. A repeated measure ANOVA was used to analyze data. Results AR and PR interventions significantly improved the C-reactive protein (CRP) (− 38% vs. − 40%), Lactate dehydrogenase (LDH) (− 11% vs. − 3%), Sargent jump (9% vs. 10%), peak power (20% vs.18%), and average power (14% vs. 14%), upper 1RM (8% vs. 8%) and no significant differences were observed between groups. The AR intervention significantly increased growth hormone (GH) (423% vs. 151%, p = 0.03), lower body 1RM (18% vs. 11%) and muscle endurance (34% vs. 22% for the upper body, p = 0.02 and 32% vs. 24% for the lower body, p = 0.04) than the PR group. The PR intervention further increased the minimum power than the AR group (19% vs. 10%). There were no significant changes in testosterone ( p = 0.79) and cortisol ( p = 0.34) following interventions. Conclusion The findings indicated that by increasing muscle activation and higher metabolic load, AR during resistance training with BFR might cause more remarkable improvements in serum GH, muscle strength, and endurance. Thus, to gain further benefits, AR during training with BFR is recommended. Trial registration : IRCT20191207045644N1. Registration date: 14/03/2020. Abstract:Background and ObjectiveIndividuals with Alzheimer disease and dementia experience cognitive decline and reduction in physical capabilities. Engaging in cognitive challenges and physical exercises is effective in reducing age-related cognitive and physical decline. It is believed that physical activity in the context of cognitive challenges might enhance the process of neurogenesis in the adult brain, but how effective are such interventions? Is there enough evidence to support that dual-task training is more effective than cognitive or physical training alone? To what extent can such training improve cognitive and physical functions in patients at various stages of cognitive decline?MethodologyThis systematic review with meta-analysis summarizes the emerging evidence of dual-task training for enhancing cognitive and physical functions in older individuals with cognitive impairment, dementia or Alzheimer’s disease. A systematic search was carried out in MEDLINE, PubMed, EMBASE, and Cochrane Library with the following search terms: randomized control trials, dual-task training, SCD, MCI, dementia, and Alzheimer’s disease.ResultsA total of 21 studies with 2,221 participants were identified. The results of dual-task tanning intervention are summarized as change in global cognitive function; SMD = 0.24, (P= 0.002), memory; SMD = 0.28, (P = 0.000), executive function; SMD = 0.35, (P = 0.000), attention; SMD = −0.19, (P = 0.1), gait speed; SMD = 0.26, (P = 0.007), dual-task cost; SMD 0.56, (P = 0.000), and balance; SMD 0.36, (P = 0.004).ConclusionPrimary analysis showed a small-to-medium positive effect of dual-task training interventions on cognitive functions and medium-to-large positive effect on gait functions and balance.

Abstract:PurposeThe present study aimed to investigate the effects of low-intensity resistance training with blood flow restriction (BFR resistance training) on vascular endothelial function and peripheral blood circulation.MethodsForty healthy elderly volunteers aged 71 ± 4 years were divided into two training groups. Twenty subjects performed BFR resistance training (BFR group), and the remaining 20 performed ordinary resistance training without BFR. Resistance training was performed at 20 % of each estimated one-repetition maximum for 4 weeks. We measured lactate (Lac), norepinephrine (NE), vascular endothelial growth factor (VEGF) and growth hormone (GH) before and after the initial resistance training. The reactive hyperemia index (RHI), von Willebrand factor (vWF) and transcutaneous oxygen pressure in the foot (Foot-tcPO2) were assessed before and after the 4-week resistance training period.ResultsLac, NE, VEGF and GH increased significantly from 8.2 ± 3.6 mg/dL, 619.5 ± 243.7 pg/mL, 43.3 ± 15.9 pg/mL and 0.9 ± 0.7 ng/mL to 49.2 ± 16.1 mg/dL, 960.2 ± 373.7 pg/mL, 61.6 ± 19.5 pg/mL and 3.1 ± 1.3 ng/mL, respectively, in the BFR group (each P < 0.01). RHI and Foot-tcPO2 increased significantly from 1.8 ± 0.2 and 62.4 ± 5.3 mmHg to 2.1 ± 0.3 and 68.9 ± 5.8 mmHg, respectively, in the BFR group (each P < 0.01). VWF decreased significantly from 175.7 ± 20.3 to 156.3 ± 38.1 % in the BFR group (P < 0.05).ConclusionsBFR resistance training improved vascular endothelial function and peripheral blood circulation in healthy elderly people.

Abstract:OBJECTIVES The purpose of this study is to investigate the efficacy of weight training with controlled blood flow occlusion compared to conventional resistance training, in the ageing population.METHODS Twenty-three healthy female subjects (aged 40-55) were randomly assigned to one of three groups; low intensity blood flow restriction training (LI-BFRT) (n = 9), conventional resistance training (RT) (n = 7) and control (CON) (n = 7). The RT group trained between 65-70% one repetition maximum (1RM) and the LI-BFRT group trained at 30% 1RM while wearing pressure cuffs inflated to 100-120% of brachial systolic blood pressure (bSBP). Relative appendicular skeletal muscle mass (ASM/weight), isokinetic strength and power were tested pre and post 8 weeks of training.RESULTS Upper limb ASM/weight increased significantly in the LI-BFRT and RT groups (both p CON). Only RT increased extension muscle strength (p < 0.05). Lower limb flexion and extension power improved following LI-BFRT (p < 0.05 and p < 0.01, respectively), significantly greater than RT in both flexion (p < 0.001) and extension (p < 0.01).CONCLUSIONS LI-BFRT may be as, if not more effective than RT for increasing muscle volume, strength and power in middle-aged women.
The Effect of Different Dual-Task Balance Training Methods on Balance and Cognitive Function of Older Adults

Abstract:Background: This research aims to investigate the effectiveness of different dual-task training methods on balance and cognitive function in the elderly. Methods: The participants in this study were 36 elderly individuals aged 65 years and older who met the research entry criteria. The selected individuals were divided into three groups, 12 people each, based on the pre-test Berg balance scores. The research groups included two dual-task balance training groups, one with fixed and one with variable priority, and a control group. The Berg balance test was used to assess functional balance in the elderly, and the serial reaction time test was used to assess cognitive function. Data analysis was conducted using covariance analysis and the Bonferroni post hoc test. Results: The data analysis showed no significant difference between the control group and the dual task groups with fixed and variable priority in the balance test. The serial reaction time test results indicated a significant difference between the control group and the dual-task practice groups with fixed and variable priority. Moreover, there was a significant difference between the two dual-task practice groups post-intervention. Conclusion: This research demonstrated that dual-task training, particularly with variable priority, resulted in faster information processing in the elderly. Therefore, it appears that, in addition to the theory of limited attention capacity, other mechanisms, such as the allocated attentional ability, can effectively impact cognitive performance.

Abstract:Abstract Functional power-based exercise training can improve physical performance in older adults and cognitive training can improve measures of cognition, but their combined effects on cognition and related risk factors (neurological and inflammatory markers) remains uncertain. This 6-month cluster randomised controlled trial evaluated the effectiveness of dual-task functional power training (DT-FPT) on cognition and circulating neurological and inflammatory markers in older adults at increased falls risk, and whether intervention responses varied by apolipoprotein-E (ApoE) and brain derived neurotrophic factor (BDNF) polymorphisms. Three hundred residents aged ≥ 65 years at increased falls risk residing in 22 independent-living retirement communities, were randomised by village, to DT-FPT (n = 156, 11 villages) involving a multi-component power-based training program performed simultaneously with cognitive and/or motor tasks (45–60 min, 2/week), or a usual care control (CON) group (n = 144, 11 villages). Cognition (computerized CogState battery), inflammatory cytokines, BDNF, insulin-like growth factor-1, vascular endothelial growth factor, amyloid β (1–40) and (1–42) were assessed at baseline and 6-months. Overall, 233 (78%) participants completed the intervention and adherence averaged 50.1%. DT-FPT led to a net 0.18–0.20 SD benefit versus CON in psychomotor ability/attention and reaction time/attention (both P < 0.05). There were no significant intervention effects on circulating markers, except for a net 10.5% benefit in amyloid β (1–40) in DT-FPT versus CON (P < 0.05). Responses were not influenced by APOE or BDNF genotype. In conclusion, DT-FPT in older adults at increased falls risk can provide some cognitive benefits, but these were not related to corresponding changes in inflammatory or neurological markers or influenced by genotype. Abstract:Background: The elevated blood pressure (BP) and lower cardiac autonomic modulation (CAM) are associated with higher morbidity mortality risk among older adults. Although exercise is an important intervention for cardiovascular promotion, it is unclear whether combat sports training could benefit cardiovascular outcomes as much as autonomic in this population. This study compared the effects of 12 weeks of Muay Thai (MT) training against functional training (FT) on CAM and hemodynamic parameters in older adults. Methods: The sample consisted of 50 older adults (41 women; 66.0 ± 5.3 years old), who were equaly randomized into FT (n = 25) and MT (n = 25) intervention groups. CAM was measured by 30-min rest heart rate variability. Systolic blood pressure (SBP), diastolic blood pressure (DBP) and resting heart rate (RHR) were measured using an automatic oscillometric device. Pulse pressure (PP) and the double product (DP) were also calculated. The interventions were carried out three times a week, with 60-min length per session, during 12 consecutive weeks. The intensity of the interventions was measured using the subjective perception of exertion scale and by accelerometer. Two-factor repeated measures analysis of covariance was used for groups comparison, considering intervention group and body mass as factors. The 95% confidence interval of the difference (95%CIdif) was also calculated and the effect size was measured using partial eta squared (η2p). Results: CAM indices did not show significant changes across moments and intervention groups. In hemodynamic parameters, only in DBP was there an effect of the moment (F1,39 = 8.206; P = 0.007; η2p = 0.174, large) and interaction effect between group*moment (F1,39 = 7.950; P = 0.008; η2p = 0.169, large). Specifically, the MT group at the post-training moment showed lower DBP (P = 0.010; 95%CIdif = -13.3; -1.89) in relation to the FT group. Furthermore, the MT group showed a decrease in DBP during training (P = 0.002; 95%CIdif = -10.3; -2.6). Also, an increase in training intensity was also found over the 12 weeks in FT, with no difference between the groups. Conclusion: After 12 weeks of MT practice there was a reduction in DBP compared to FT in older adults. Trial registration: NCT03919968 Registration date: 01/02/2019.

Abstract:BackgroundWe examined the effects of detraining after blood flow-restricted (BFR) low-load elastic band training on muscle size and arterial stiffness in older women.FindingsFourteen women were divided into BFR training (BFR-T) or non-BFR training (CON-T). Each group participated in 12 weeks of arm curl and press down training using an elastic band either with (BFR-T) or without BFR (CON-T). Muscle cross-sectional area (CSA) and maximum voluntary isometric contraction (MVIC) for upper arms and cardio-ankle vascular index (CAVI) were evaluated before and after the 12-week training period and also after 12 weeks of detraining. CSA and MVIC were higher at post and detraining (CSA: 16.3% (p < 0.01) and 6.9% (p < 0.01) for elbow flexion and 17.1% (p < 0.01) and 8.7% (p > 0.05) for elbow extension; MVIC: 7.3 and 3.9% (both p > 0.05) for elbow flexion and 17.6 and 15.1% (both p < 0.01) for elbow extension) than at pre for the BFR-T, but not for the CON-T. There was no change in CAVI for the two groups.ConclusionsIncreased muscle strength/size following 12 weeks of elastic band BFR-T was well maintained with a low risk of arterial stiffness after 12 weeks of detraining in older women.

Abstract:Background: Resuming a physical exercise program after a period of cessation is common in older women. Monitoring the responses during this detraining (DT) and retraining (RT) may allow us to analyze how the body reacts to an increase and a reduction in physical inactivity. Therefore, we conducted a follow-up training, DT, and RT in prehypertensive older women to analyze the response to these periods. Methods: Twenty-three prehypertensive older women (EG; 68.3 ± 2.8 years; 1.61 ± 0.44 m) performed 36 weeks of the multicomponent training program (MTP) followed by twelve weeks of DT plus eight weeks of RT. Fifteen prehypertensive older women (CG; 66.3 ± 3.2 years; 1.59 ± 0.37 m) maintained their normal routine. Functional capacity (FC), lipid, and hemodynamic profile were assessed before, during 24 and 36 weeks of the MTP, after 4 and 12 weeks of DT, and after 8 weeks of RT. Results: After 24 weeks of the MTP, only SBP did not improve. Four weeks of DT did not affect lower body strength (30-CS), TC, or GL. Eight weeks of RT improved BP (SBP: −2.52%; ES: 0.36; p < 0.00; DBP: −1.45%; ES: 0.44; p < 0.02), handgrip strength (3.77%; ES: 0.51; p < 0.00), and 30-CS (3.17%; ES: 0.38; p < 0.04) compared with 36 weeks of the MTP. Conclusions: Eight weeks of RT allowed patients to recover the benefits lost with detraining, which after only four weeks affected them negatively, and the systematic practice of exercise contributed to greater regulation of BP since 24 weeks of the MTP proved not to be enough to promote positive effects of SBP.

Abstract:A digit memory task and a delayed visual aiming task were used in a test of resource capacity theories. These tasks were run either singly, with short- or long-retention intervals, or in pairs. In the dual-task conditions, the short version of one was inserted into the longer version of the other. Delay by itself had no effect on retention, but the dual-task combinations showed asymmetrical interference: Inserting verbal memory into the visual-motor delay had little effect on either task, but inserting the motor task into the verbal retention interval disrupted both. These results thus support neither single nor multiple resource models, but can be explained with reference to component processes in working memory.

Abstract:Objective: Physical and cognitive impairments are common with aging and often coexist. Changes in the level of physical and mental activity are prognostic for adverse health events and falls. Dual-task (DT) training programs that can improve mobility and cognition simultaneously can bring significant improvements in rehabilitation. The objective of this mixed methods exploratory RCT was to provide evidence for the feasibility and therapeutic value of a novel game-assisted DT exercise program in older adults. Methods: Twenty-two community dwelling participants, between the ages of 70-85 were randomized to either dual-task treadmill walking (DT-TR) or dual-task recumbent bicycle (DT-RC). Both groups viewed a standard LED computer monitor and performed a range of cognitive game tasks while walking or cycling; made possible with the use of a "hands-free", miniature, inertial-based computer mouse. Participants performed their respective 1-h DT exercise program twice a week, for 12 weeks at a community fitness centre. Semi-structured interviews and qualitative analysis was conducted to evaluate the participant's experiences with the exercise program. Quantitative analysis included measures of standing balance, gait function (spatiotemporal gait variable), visuomotor and executive cognitive function, tested under single and DT walking conditions. Results: Compliance was 100% for all 22 participants. Four themes captured the range of participant's experiences and opinions: 1) reasons for participation, 2) difficulties with using the technologies, 3) engagement with the computer games, and 4) positive effects of the program. Both groups showed significant improvements in standing balance performance, visuomotor and visuospatial executive function. However, significant improvement in dual task gait function was observed only in the DT-TR group. Medium to large effect sizes were observed for most balance, spatiotemporal gait variables, and cognitive performance measure. Conclusion: With only minor difficulties with the technology being reported, the findings demonstrate feasible trial procedures and acceptable DT oriented training with a high compliance rate and positive outcomes. These findings support further research and development, and will direct the next phase of a full-scale RCT.
Changes in Some Fitness Factors, Testosterone, and Cortisol Following Combined Exercise with and Without Blood Flow Restriction in Older Men

Abstract:Background: Considering the importance of maintaining and promoting the health of the elderly, it is necessary to examine their hormonal responses during physical fitness exercises. This study aimed to investigate the effect of two combined training methods with and without blood flow restriction on some physical fitness (muscle strength and muscle endurance), anabolic (testosterone), and catabolic (cortisol) factors. Materials and Methods: The present research was quasi-experimental with a pretest-posttest design. A total of 24 elderly men with no history of training participated in this study voluntarily. They were divided randomly into two experimental groups and one control group. Measurement included the 30-second sit-up test, the sit-up test with 50% of one-repetition maximum (to the point of fatigue), testosterone level, and cortisol level. In the group with blood flow restriction, both upper thighs were closed with cuffs and the exercise was performed with the intensity of 20-30% of 1RM, to the point of fatigue. In the training group without restriction, the same movements were performed with an intensity of 65- 80% of 1RM, to the point of fatigue. The covariance test and Bonferroni post hoc test were used to analyze the data. Results: The results showed that training with blood flow restriction had a significant effect on increasing muscle strength and muscle endurance similar to that caused by high-intensity training without blood flow restriction. Moreover, the results of the study on the effect of exercise with and without vascular occlusion on testosterone and cortisol levels showed that both training methods had similar effects on increasing testosterone levels and decreasing serum cortisol levels in older men. Conclusion: Since heavy weight training is difficult and risky for the elderly, they can effectively practice low-intensity resistance exercises with blood flow restriction. Eventually, the study of other hormonal and neuromuscular mechanisms involved in the effectiveness of these exercises will expand knowledge in this field of study.
Effect of blood flow-restrictive resistance training on metabolic disorder and body composition in older adults with type 2 diabetes: a randomized controlled study

Abstract:Introduction: To explore whether blood flow-restrictive resistance exercise (BFRE) can be used as an alternative strategy to moderate-intensity resistance training (RT) to improve metabolic disorder and body composition in older adults with type 2 diabetes (T2DM). Methods: This is a single-blind, randomized, controlled trial. Ninety-eight older adults with T2DM were randomly divided into three groups: BFRE group (n = 34), RT group (n = 31) and control group (n = 33). Two exercise groups received supervised collective training for a period of six months, each lasting 50 min, three times a week. The primary outcomes included fasting plasma glucose (FPG), Glycosylated hemoglobin (HbA1c), blood lipids, blood pressure, and body composition. The secondary outcome was muscle performance. Results: After six months of intervention, the FPG, HbA1c, blood lipids, diastolic blood pressure, body composition, and muscle performance of the two exercise groups were significantly improved relative to the control group and baseline measurements (P 0.05). There was no significant decrease in systolic blood pressure between the two exercise groups compared to the control group (p > 0.05), but it was significantly lower than their baseline (P 0.05). Discussion: BFRE can safely and effectively improve the metabolic disorder and body composition of older adults with T2DM. For elderly exercise beginners, BFRE can be used as an alternative strategy to moderate-intensity resistance training.
