Abstract:We prepared a lotion using electrolytic-reduction ion water (ERI), and evaluated the healing effects of this lotion (ERI lotion) on burn wounds. Third degree burn wounds were induced in the mouse dorsal skin, and ERI lotion or physiological salt (PS) lotion was applied to the wounds from immediately after injury [ERI (+) group and ERI (-) group as a control group, respectively]. The burn wound area was measured, and its serial changes were evaluated. In addition, histological examination of the burn wound site (on day 3) was performed. Comparison of the ERI (+) and (-) groups showed a significant reduction in the burn wound area in the former. Histological examination confirmed many interstitial spaces, blood vessels, and lymphatic vessels in the subcutaneous tissue in the ERI (-) compared with the ERI (+) group. These results suggest the promotion of burn wound healing by ERI lotion.

Abstract:Burn injuries impose challenges such as infection risk, pain management, fluid loss, electrolyte imbalance and psychological and emotional impact, on healthcare professionals, requiring effective treatments to enhance wound healing. The present study evaluated the efficacy superoxidized electrolyzed solution (SES), with low (SES-low) or high (SES-high) concentrations of active species, alone or in combination with a formulation in gel (G), in comparison with commonly prescribed treatments for burn injury, including nitrofurazone (NF) and silver sulfadiazine (S); normal saline was used as placebo (PI). A scald burn model was established in BALB/c mice. Measurements of the burned area and histological parameters such as inflammatory infiltration state, epithelial regeneration and collagen fibers were evaluated on days 3, 6, 9, 18 and 32 to assess healing score and status. All treatments achieved wound closure at day 32; histopathological parameters indicated that SES-low and SES-low + G performed better than the Pl and S groups (P<0.05). All treatments showed a lower count of inflammatory cells compared with S (P<0.05); for collagen deposition and orientation, SES-low + G showed a more uniform horizontal orientation compared with Pl, SES-high + G, NF and S groups (P<0.05). SES-Low was the most effective substance to induce favorable and organized healing, while S was the worst, inducing disorganized closure of the wound due to a pro-inflammatory effect.

Abstract:Background: Children make up a large percentage of those affected by burns worldwide, with most of them suffering from severe injuries that necessitate skilled medical attention. Despite medical progress, there is still no ideal dressing for the treatment of burn wounds in children. The aim of the study was to assess the impact of epidermal substitutes in the treatment of burn wounds in children. Materials and Methods: This retrospective study evaluates the use of three dressings in the treatment of pediatric burns at a major Polish burn center. A patient database was used to identify children who received treatment with silver dressings, paraffin dressings or epidermal substitutes from 2009 to 2023. A demographic analysis was performed to collect the following information: causes of burns, procedural details and patient outcomes. Results: There were 439 patients aged between 1 month and 18 years. For severe burns, the number of interventions was lowest among children with epidermal substitute application (p = 0.039). Paraffin gauze resulted in the greatest number of skin grafts, whereas alloplastic replacement produced the least amount of transplantation (p < 0.005) regardless of the severity of the burn. Conclusions: Epidermal substitutes offer a good dressing option for burn wounds to improve their treatment and reduce the need for skin graft coverage. In the future, extended comparative or randomized trials are needed to confirm our results.

Abstract:IntroductionDeep burn wounds undergo a dynamic process known as wound progression that results in a deepening and extension of the initial burn area. The zone of stasis is more likely to develop more severe during wound progression in the presence of hypoperfusion. Hydrogen has been reported to alleviate injury triggered by ischaemia/reperfusion and burns in various organs by selectively quenching oxygen free radicals. The aim of this study was to investigate the possible protective effects of hydrogen against early burn-wound progression.MethodsDeep-burn models were established through contact with a boiled, rectangular, brass comb for 20 s. Fifty-six Sprague-Dawley rats were randomly divided into sham, burn plus saline, and burn plus hydrogen-rich saline (HS) groups with sacrifice and analysis at various time windows (6 h, 24 h, 48 h) post burn. Indexes of oxidative stress, apoptosis and autophagy were measured in each group. The zone of stasis was evaluated using immunofluorescence staining, ELISA, and Western blot to explore the underlying effects and mechanisms post burn.ResultsThe burn-induced increase in malondialdehyde was markedly reduced with HS, while the activities of endogenous antioxidant enzymes were significantly increased. Moreover, HS treatment attenuated increases in apoptosis and autophagy postburn in wounds, according to the TUNEL staining results and the expression analysis of Bax, Bcl-2, caspase-3, Beclin-1 and Atg-5 proteins. Additionally, HS lowered the level of myeloperoxidase and expression of TNF-alpha, IL-1 beta, and IL-6 in the zone of stasis while augmenting IL-10. The elevated levels of Akt phosphorylation and NF-kappa B p65 expression post burn were also downregulated by HS management.ConclusionHydrogen can attenuate early wound progression following deep burn injury. The beneficial effect of hydrogen was mediated by attenuating oxidative stress, which inhibited apoptosis and inflammation, and the Akt/NF-kappa B signalling pathway may be involved in regulating the release of inflammatory cytokines.
Changes in Body Zinc and Copper Levels in Severely Burned Patients and the Effects of Oral Administration of ZnSO4 by Double Blind Method. (Chinese)

Abstract:The trace elements and the effect of zinc sulphate on wound healing of 46 severely burned patients in recent 2 years were studied. By double blind method 22 patients accepted capsular ZnSO4 (660 mg) per day as treated group and 24 patients medical starch as control. ZnSO4 and placebo were taken from 3rd or 5th day post burn until healing. Serum and 24h urine Zn and Cu were measured on days 3, 5, 9, 13 post burn and later weekly until healing. The changes of Zn and Cu in serum and urine in all patients were similar to the results reported by others. But the serum Zn on day 9 and urine Zn on days 5, 9, 13 post burn in treated group were significantly higher than that in control group. The basic wound healing time was same in both groups, but the patients in treated group whose coefficient UBS* (Unit of Burned Surface) was more than 180 were cured 8 days earlier than the patients of same severity in control group. At the healing the serum Zn in treated group was significantly higher than that in normal control group, while the serum Zn in control group was as same as in normal control group. The rising to normal of value of Zn/Cu in treated group was significantly earlier than that in control group. But the linear regression equations of serum Zn and Cu, urine Zn and Cu in both groups have no significant difference in the whole course of disease. No side effects of ZnSO4 have been found during the course.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract:Background. Burn wound healing is recognized as a complex process involving synergetic interactions between different cells, cytokines and growth factors. The adverse interactions can underlie chronicization of the process. Accordingly, the paper presents a relevant study into mechanisms of natural wound dressings, capable of influencing the processes of inflammation, angiogenesis, and skin resurfacing. Objective. To carry out a comparative evaluation of efficiency of burn wound healing with derma-based hydrogel according to the dynamics of proand anti-inflammatory factors. Methods. Development of a hydrogel material involved dermis samples of Landrace breed of pig, subjected to partial alkaline hydrolysis. In order to carry out a comparative evaluation of burn wound healing efficiency, the authors simulated direct thermal injury in three groups of sphinx (hairless) rats: group 1 (control group) — rats without treatment (n = 20), group 2 (comparison group) — rats treated with Levomekol ointment (n = 20), and group 3 (experimental group) — rats treated with hydrogel material (n = 20). Before and after injuring on days 1, 3, 7, 14, the content of cytokines interleukin-1β, interleukin-4, interleukin-6, interleukin-8, interleukin-10, tumor necrosis factor-α by enzyme immunoassay. The wound samples were explanted for histological examination on days 3, 7 and 14 after the beginning of the experiment. Statistical processing of the obtained results on DNA content in hydrogel, cytokine content in serum and morphometric data was performed using GraphPadPrism 6.04, Microsoft Excel 2016 (Microsoft, USA). Results. When determining the content dynamics of nonspecific markers of inflammation, an increase in the concentrations of interleukin-1β and tumor necrosis factor-α on day 1 after the hydrogel application was recorded, as well as an increase in interleukin-6 on days 3 and 7, while the concentrations of interleukin-8 did not change significantly throughout the experiment. Thus, dermal components are indicated to participate in the inhibition of acute-phase immune reactions. With regard to anti-inflammatory factors, the study revealed a decrease in the concentration of interleukin-10 on days 1 and 7, an increase in interleukin-4 on day 3 as compared to the control group, thereby indicating a pronounced anti-inflammatory effect and prolonged action of the hydrogel. Conclusion. Comparative analysis of the pro-inflammatory cytokines levels (interleukin-1β, interleukin-8) showed pronounced anti-inflammatory effects of the derma-based hydrogel material. Introduction of exogenous biological components of the extracellular matrix (collagen and its hydrolysates) had a significant influence on the regulation of anti-inflammatory cytokines synthesis, presumably contributing to faster successful epithelization and wound healing.

Abstract:Background:The aim of burn management is to prevent hypovolemic shock ,infection ,septic shock, to promote early epithelialization as well as healing of wound . The present study aimed to compare the effects of Nano crystalline silver with collagen base cream and 1% silver sulfadiazine cream on the healing of burn wounds. Materials & Methods: A clinical prospective observational studyPatients with burn wounds were divided into two groups , one group was dressed with 1% silver sulfadiazine cream and the other group of patients was dressed with nano crystalline silver with collagen base cream. The wound dressing was changed everyday after washing the wound with normal saline. Wound parameters were assessed regularly , using clinical methods and photographs. The clinical data were statistically analyzed and results produced. Results: In this department study ,using silver sulphadiazine cream &nano crystalline silver with collagen base cream , enhanced epithelisation& granulation tissue following decreased slough was observed and the results were better with nano crystalline silver with collagen base cream because of early and increased appearance of granulation tissue in the wounds & this was also proved by statistical analysis.The mean hospital stay was less in patients who were undergoing dressing with nanocrystalline silver with collagen base cream. Conclusions: The effect of nanocrystalline silver with collagen base cream on the reduction of the healing time and necrotic tissue amount and increasing the granulation, epithelialization, and decreasing the necrotic tissue/slough was greater when compared to application of 1% SSD cream. Considering the above findings nanocrystalline silver with collagen base cream is more recommended to be used for burn wounds.

Abstract:Burns can cause severe damage to the skin owing to bacterial infection and severe inflammation. Although conductive hydrogels as electroactive burn‐wound dressings have achieved remarkable effects on accelerating wound healing, issues such as imbalance between their high conductivity and mechanical properties, easy dehydration, and low transparency must be addressed. Herein, a double‐network conductive eutectogel is fabricated by integrating polymerizable deep eutectic solvents (PDESs) including acrylamide/choline chloride/glycerol (acrylamide‐polymerization crosslink) and thiolated hyaluronic acid (disulfide‐bonding crosslink). The introduction of PDESs provides the eutectogel with a conductivity (up to 0.25 S·m−1) and mechanical strength (tensile strain of 59% to 77%) simulating those of natural human skin, as well as satisfactory tissue adhesiveness, self‐healing ability, and antibacterial properties. When combined with exogenous electrical stimulation, the conductive eutectogel exhibits the ability to reduce inflammation, stimulate cell proliferation and migration, promote collagen deposition and angiogenesis, and facilitate skin tissue remodeling. Abstract:Objective To observe the infection prevention, wound healing effect and safety of nanometer silver in treatment of Ⅱ degree burn wound. Methods The patients with 6%-10% TBSA Ⅱ degree burn wound ( deep or superficial) were randomly divided into test group ( treated with nanome-ter silver, n =35) and control group (treated with 1% sulfadiazine silver, n =35). Each group was reg-ulated to change dressing and medication one time a day for seven days on 5% superficial Ⅱ degree burn wound. Then, 1% iodophors and gauze were used for change dressing. Before and after change dressing, bacterial culture was done in two groups to observe the time for wound healing. The blood collected before change dressing and at days 1,7 and 14 after change dressing and urine within 24 hours were employed for detecting serum silver level and mean silver level in urine by using inductively coupled plasma mass-spec-trum ( thermoelectricity of American Ⅹ Series Ⅱ ). In the meantime, the liver and renal function was e-valuated at days 7 and 14. Results Positive rate of bacterial culture in test group and control group was 3% and 3.1% respectively after treatment, which showed a decrease compared with the levels before treatment (12. 1% and 15.6% respectively), with no statistical difference. The wound healing time of test group was (9. 18 ± 1.9) days, which was shorter than ( 12.9 ± 1.3 ) days in control group, with sta-tistical difference (P<0.01). Before treatment, the silver level of blood was ( 1.55 ± 1.26) μL and ( 1.54 ± 1.28 ) μg/L respectively in test group and control group, and silver level of urine within 24 hours was ( 1.67 ± 1.05 ) μg and ( 1.87 ± 1.37) μg respectively test group and control group, with no statistical difference (P > 0. 05 ). Silver medication could elevate the serum silver level ( P < 0.01 ), with lower level in test group than control group (P <0. 01 ). Average silver level of urine within 24 hours showed similar change with that in the serum. The patients showed normal renal function, with no abnormal change of ALT. Conclusions For Ⅱ degree burn wound, nanometer silver can more significantly short-en wound healing time compared with sulfadiazine silver. Moreover, nanometer silver has higher degree of safety on silve metabolism and is an ideal medcation for burn wound.

Abstract:Burn injuries represent a significant problem in clinical practice due to the high risk of infection and the prolonged healing process. Recently, more attention has been given to natural remedies such as water extracts of various medicinal plants, which possess anti-inflammatory and wound healing properties. The aim of this study is to evaluate the efficacy and safety of Satureja montana L. and other water extracts in a burn wound model. The study involved male Californian rabbits (n = 52) divided into eight groups. Burn wounds were modeled on the animals and subsequently treated with gels based on Satureja montana L. and other water extracts. The reparative potential of the epidermis (assessed by Ki-67 expression), the state of local immunity (measured by the number of CD-45 cells), and the anti-inflammatory role of mast cells (measured by tryptase levels) were evaluated. Bacteriological and morphological studies were conducted. The most pronounced bactericidal, reparative, and immunostimulatory effects were observed after the treatment using a gel mixture of water extracts from Satureja montana L., Salvia sclarea, Coriandrum sativum L., and Lavandula angustifolia in equal proportions (1:1:1:1). The other gels also demonstrated high efficacy in treating burn wounds, especially when using a strain of Pseudomonas aeruginosa resistant to several antibiotics. Immunohistochemical studies showed a significant increase in the number of Ki-67-positive cells in the basal layer of the epidermis and a decrease in the number of CD-45-positive cells, indicating improved proliferative activity and reduced inflammation. This study confirms the hypothesis that the use of water extract mixtures significantly enhances the reparative potential, improves the immune response in the treatment of burns, and promotes wound healing. These findings pave the way for further research and the application of complex phytotherapeutic agents, specifically water extracts of medicinal plants containing phenols and antioxidants in burn wound therapy.

Abstract:The paper investigates the effect of silver nanoparticles preparations on the rate of burn healing and scar quality. Three preparations on burn were considered: one with silver sulfadiazine and two with silver nanoparticles woven into two types of dressing: one, of polyethylene and second, carboxymethyl cellulose. The experiment was performed on pigs, due to anatomical and pathophysiological similarities with human skin. All three silver preparations have antimicrobial properties with a beneficial effect on the healing of burns. Preparations with silver nanoparticles proved to be the most effective, since they encourage very fast epithelialization of burns, affect reduction of the level of matrix metalloproteinase-9 in the environment of the burn wound, lead to faster expression of Vascular Endothetial Growth Factor - VEGF, cause less thickening of the epidermis and contractility, and improve tension characteristics of the scar compared to the preparation with silver sulfadiazine. By comparing results of healing parameters and evaluation of the scar achieved with preparations with silver nanoparticles, it was determined that the best overall results of local treatment were achieved with silver nanoparticles in crystalline form. Due to quantum-mechanics, surface and chemical oxidation-reduction (reactive oxygen species) phenomenological characteristics Ag nanoparticles in crystalline form have unique ability to catalyze rate of healing.

Abstract:A burn wound occurs as a result of exposure to high-temperature skin or chemicals and is a serious injury with systemic effects. The problem of treatment of thermal trauma is urgent for modern medicine. Despite the fact that the overall mortality rate has dropped significantly in recent years, research about the development of new treatments and technologies for patients with skin burns is extremely important. The aim of the study is to research the features of electron microscopic changes in the skin of rats during the month after burns of II-III degree against the background of the introduction of the first 7 days of 0.9% NaCl solution. Studies were performed on 180 laboratory white rats-males weighing 155-160 g. In the course of the experiment, the animals were divided into 2 groups: 1 – rats without thermal injury, which were infused with 0.9% NaCl solution at a dose of 10 ml/kg; group 2 – rats which infused 0.9% NaCl solution at a dose of 10 ml/kg for 7 days after skin burns. Burning skin damage was caused by applying to the lateral surfaces of the trunk of rats for 10 seconds four copper plates, heated in water at a constant temperature of 100 C. The total area of skin lesion in rats was 21-23%. Shaving of the lateral surfaces of rats’ trunk, catheterization of veins, staging of skin burns, and decapitation of animals (after 1, 3, 7, 14, 21 and 30 days) were performed under intravenous propofol anesthesia (calculated at 60 mg/kg body weight). Preparations for electron microscopic examination were prepared according to the standard procedure. The data obtained were studied using a PEM-125K electron microscope. Electron microscopic studies of the skin of animals after thermal trauma under the conditions of application of 0.9% NaCl solution have found that in the early stages of the experiment – the stage of shock and early toxemia (1, 3, 7 days) compensatory and adaptive changes of its structural components are combined with signs of destructive disorders. In the epitheliocytes of the epidermis of the affected area of the skin is the destruction of nuclei and cytoplasm. Vascular disorders in the dermis are combined with changes in fibroblasts, an intercellular substance of fibrous connective tissue. In the late stages of the experiment – the stages of late toxemia and septicotoxemia (14, 21 and 30 days), there is a further development and deepening of destructive-dystrophic changes of all structural components of the affected skin, they become irreversible. The processes of granulation tissue formation, its transformation into connective tissue, as well as marginal epithelialization are slow. This morphological condition of the burn wound indicates the need for the use of corrective drugs to reduce destructive changes and the activation of regenerative processes in the area of skin lesions.

The aim of the study was to evaluate the wound healing effect of matrices obtained on the basis of the recycling of collagen-containing waste. Materials and methods . Collagen matrices are obtained from substandard leather raw materials subjected to treatment with fermented milk composition (KM1) and lactic acid (KM2). The wound healing effect was studied on the model of thermal burns on Wistar rats in 5 groups: 1st – control (natural wound healing); 2nd – experimental 1 (processing KM1); 3rd – experimental 2 (processing KM2); 4th – experimental 3, comparison group (“Levomekol”) and 5th – intact animals (normal, without burns). Wound healing was assessed by the results of planimetry on days 1, 3, 5, 7, 9, and 13 and by histological analysis of the skin tissue on days 6, 13, and 20. On days 6 and 13, the summary antioxidant activity, the total content of leukocytes, erythrocytes, the activity of the catalase enzyme, and the content of malondialdehyde were determined in the blood of rats. Results . With thermal damage to the skin, an IIIA degree burn was formed, accompanied by the development of dry coagulation necrosis. The use of collagen matrices restored the total content of leukocytes, reduced the area of the burn wound, normalized the content of malondialdehyde, the total antioxidant activity and the activity of catalase in the blood. Histomorphometric studies have confirmed the dynamics of skin tissue recovery after a burn. The effectiveness of the use of matrices was comparable to the pharmacopoeial drug “Levomekol”. A higher wound healing effect was noted when using the KM1 matrix. Conclusion . On the model of thermal burn, the wound-healing effect of collagen matrices was established, as evidenced by the results of restoring the number of leukocytes, reducing the area of the burn wound and restoring the histostructure of the skin. One of the molecular and cellular mechanisms of wound healing is the inhibition of lipid peroxidation reactions and the restoration of the antioxidant potential of the body.
A Comparative Study of the Impacts of Aloe vera Gel and Silver sulfadiazine Cream 1% on Healing, Itching and Pain of Burn Wounds: A Randomized Clinical Trial

Abstract:Abstract Introduction: Although several studies have highlighted the beneficial effects of Aloe vera on burn wounds, limited clinical evidence exists in this regard. This study aimed to evaluate the impact of the Aloe vera gel on healing, itching and pain of burn patients. Methods: This clinical trial was conducted at Sina Hospital in Tabriz, Iran. The patients with second and first degree burn wounds on symmetrical organs, were randomly assigned to control (n=34) and experimental (n=34) groups. The Aloe vera gel and Silver sulfadiazine cream were used in the experimental and control groups, respectively. To assess the healing effects, the Bates-Jensen Wound Assessment Tool was employed. Regarding itching and pain, Visual Analogue Scale was used for precise evaluation and comparison on days 1, 3, 5, 7, 9 and 14. The data were analyzed using SPSS version 13. Results: Although the wounds in both groups healed up completely within two weeks, the healing process among the patients in the experimental group was faster. The peak of wound itching was on day 7 in both groups. The wound itching significantly reduced half an hour after being dressed with Aloe vera gel. The wound pain in the experimental group was less than control group during the study period. Moreover, there was no pain in either experimental or control group on day 14. Conclusion: Aloe vera is an effective agent in reducing itching and pain, and it can substantially increase the rate of healing. Accordingly, this agent can be considered in the treatment of burn wounds.
Comparison between the effect of silymarin and silver sulfadiazine on burned wound healing in rats

Abstract:A burn is an injury caused by transfer of thermal energy into tissue resulting in disruption of its normal anatomical structure. An experiment was carried out to compare the effect of silymarin and silver sulfadiazine on burned wound healing in rat. In this study, 36 Wistar rats were divided into 6 groups. A burn was generated on the backs of all rats by brass probe. The burned areas in the first group (control) did not receive any medication, the second group received oral silymarin. Wound areas in third, fourth and fifth group were covered daily with 5%, 10% and 20% silymarin ointment. Silver sulfadiazine was applied as daily topical treatment of burned wound in the sixth group. At post injury days 0, 5, 7, 14 and 21, rats from each group were selected, sacrificed and burned skin tissue samples were collected for histopathological examination. Ulceration, acute inflammation, chronic inflammation, crust formation and granulation tissue formation were assessed. The result of this study showed that the use of 10% silymarin significantly enhanced the burn wound healing process as compared to silver sulfadiazine and other silymarin concentrations.

Abstract:Burns and their treatment are a significant medical problem. The loss of the physical barrier function of the skin opens the door to microbial invasion and can lead to infection. The repair process of the damage caused by the burn is impaired due to the enhanced loss of fluids and minerals through the burn wound, the onset of hypermetabolism with the concomitant disruption of nutrient supply, and derangements in the endocrine system. In addition, the initiated inflammatory and free radical processes drive the progression of oxidative stress, the inhibition of which largely depends on an adequate supply of antioxidants and minerals. Clinical experience and research provide more and more data to make the treatment of patients with thermal injury increasingly effective. The publication discusses disorders occurring in patients after thermal injury and the methods used at various stages of treatment.

Abstract:Background and aims: One of the major causes of death and disability in the world is burning, that from past to present the proper treatment of burns has been considered by physicians. Properties of medicine plants in burn wound healing have been proven in various studies. Arnebia euchroma is rich sources of antioxidants. Therefore, this work was carried out to investigate the effect of Arnebia euchroma on second-degree burn wound healing in Balb c. Methods: In an experimental study burn wounds were made by placing a hot plate with a surface area of 1 cm2 for 10s on the back of 30 balb c mice animals. Five groups of mice Vaseline, silver, without intervention, Arnebia euchroma 1% and 10% were treated topically for 21 days. Wound size at day 0, 7, 14 and 21 was measured. Data were analyzed by using Kruskal-Wallis statistical tests and mean. Results: Results showed that the most recovery was observed in Silver groups, without intervention, Vaseline, Arnebia euchroma 1% and Arnebia euchroma 10%respectively.Silver group showed the best healing effects between other study groups. Conclusion: Evidence show that Arnebia euchroma root did not have any effect on healing of second degree burn wounds in balb c. More studies were recommended for evaluating the effects of other dose and extracts of the plant on burn healing.
