Abstract:Objectives: To examine the relationship between loneliness and cognitive function and to explore the mediating role of physical health on the loneliness-cognition relationship in Chinese older adults (OAs).Method: Data came from a nationally representative sample of 14,199 Chinese OAs (aged 65+) from 2002, 2005, 2008, and 2011 waves of the Chinese Longitudinal Healthy Longevity Survey. A latent variable cross-lagged panel model combined with mediation analysis was used to determine the relationship between loneliness and cognitive function and the mediating effect of increase in the number of chronic conditions (Delta NCCs) on the ascertained loneliness-cognition relationship.Results: Severe loneliness at prior assessment points was significantly associated with poorer cognitive function at subsequent assessments, and vice versa. The Delta NCCs partially mediated this prospective reciprocal relationships, accounting for 2.58% of the total effect of loneliness on cognition and 4.44% of the total effect of cognition on loneliness, respectively.Discussion: Loneliness may predict subsequent cognitive decline, and vice versa. This loneliness-cognition relationship is partially explained by their impact on physical health. Multidisciplinary interventions aimed at reducing loneliness and cognitive decline per se and their associated risk factors as well as improving chronic illness management would be beneficial for emotional well-being and cognitive health in OAs.

Abstract:Background: Studies that explore the nutritional status, sociodemographic factors, mental health variables, and physical health variables that affect the health-related quality of life (HRQoL) of elderly patients are scarce in China. Objective: This study aimed to examine the association between health-related quality of life (HRQoL) and nutritional status, sociodemographic characteristics, and health-related variables among Chinese elderly patients. Materials and Methods: Participants were recruited from six tertiary-level hospitals in six provinces or municipalities/cities throughout China from October 2018 to February 2019: a total of 9,996 participants aged 65 years and older were enrolled. The nutritional status and HRQoL were measured using the Mini Nutritional Assessment-Short Form (MNA-SF) and the EuroQoL Five-Dimension Visual Analog Scale, respectively. BMI was taken using standard measurement protocols. Sociodemographic characteristics included age, sex, education, marital status, ethnicity, smoking, alcohol drinking, and current residence. Mental and physical health variables such as frailty and depression were assessed using validated tested instruments. Multiple linear regression analysis was used to analyze whether the nutritional status, sociodemographic characteristics, and health-related variables were associated with HRQoL. Results: According to the MNA-SF scores at the 30- and 90-day follow-up, 9.7% and 9.1% of participants were malnourished, respectively. Higher MNA-SF scores were related to higher HRQoL scores in older patients (regression coefficient; 95% confidence interval) both at the 30-day (0.660; 0.499-0.821) and 90-day (0.622; 0.434-0.809) follow-up. However, there were no significant associations between the body mass index values and HRQoL. Sociodemographic characteristics (such as age, smoking, and current residence), physical health variables (frailty, urinary function, defecation function, sleeping condition, and falling accidents in the past 12 months), and mental health variables (depression) were the main factors influencing HRQoL in this group. Conclusion: There are several factors associated with HRQoL among the population derived from this investigation of a representative sample of the Chinese hospitalized elderly population in tertiary hospitals. These findings could have major importance for the planning of "active aging" policies and programs. Trial Registration: Chinese Clinical Trial Registry, ChiCTR1800017682, registered August 9, 2018.
[Study on Social Determinants and Health-Related Quality of Life among the 'empty Nest' (lonely) Elderly Chinese People].

Abstract:OBJECTIVE:To study the social determinants and health-related quality of life among the Chinese 'empty nest' (lonely) elderly.METHODS:A total of 4995 elderly, aged 60 or over were recruited in rural and urban areas of Zhejiang province, under a cluster sampling method. Stepwise logistic regression analysis was performed to analyze the social determinants on the 'empty nest' elderly. Health-related quality of life was assessed by SF-36 and compared between the 'empty nest' and 'non-empty nest' elderly, using multiple linear regression analysis after adjusting for confounding factors.RESULTS:Comparing with the 'non-empty nest' elderly, the characteristics of the empty nest population were as follows: age at 70 to 79 (P = 0.0417), having higher education level (P < 0.0001), living in rural area (P < 0.0001), lower income level (P = 0.0178, P = 0.0049), with higher level of satisfaction of life (P = 0.0070, P = 0.0035) and lower level of depressive mood (P = 0.0371). 'Empty nest' elderly people had lower scores of SF-36 in BP (P = 0.0032)dimension of physical function, and RE (P = 0.0033) dimension of psychological function as well as MCS (P = 0.0342) after adjusting for age, gender, marriage status, education level, residential areas, personal income, scores for satisfaction of life and depressive mood.CONCLUSION:The 'empty nest' elderly people with the characteristics such as higher education level, lower income level and living in the rural areas should be under greater concern, especially on those people with lower health-related quality of life or poorer psychological function.

Abstract:Background Population aging is an inevitable trend and previous studies have showed the relationship between social health related factors and health-related quality of life (HR-QOL) in the elderly. The objective of this study is to investigate the association of social health status with HR-QOL among community-dwelling elderly in Zhejiang. Methods This cross-sectional study was based on community-dwelling elderly individuals from July 2018 to September 2018 in Zhejiang, China. HR-QOL was measured by the 12-item Short-Form Health Survey (SF-12). Social health status was estimated by the long-form of the Social Health Scale for the Elderly (SHSE-L) and classified into three categories (poor, moderate and good). Multivariable linear regression models were conducted to evaluate the association between social health status and HR-QOL (PCS, MCS and SF-12 total score). Results A total of 2952 elderly participants were included in this study. The mean age was 70.68 ± 7.75 years (mean ± SD); of the eligible participants, more than half (50.4%) were females; the mean scores were 48.10 ± 8.49, 47.70 ± 7.09 and 47.90 ± 5.86 for PCS, MCS and SF-12 total score, separately. Results from the multivariable models showed that social health status was positively related to HR-QOL after adjusting for covariates. Compared with individuals with a poor social health status, those who had a moderate or good social health status were more likely to report better HR-QOL (for moderate social health status: β = 1.90(95%CI: 1.09, 2.71) for PCS, β = 1.78(1.08, 2.48) for MCS, β = 1.84(1.29, 2.39) for SF-12 total score; for good social health status: β = 3.29(2.24, 4.34) for PCS, β = 3.10(2.12, 4.01) for MCS, β = 3.20(2.48, 3.91) for SF-12 total score). Conclusion In our study, we found that social health status was positively associated with HR-QOL among the elderly in Zhejiang. Our findings could provide valuable information for decision-makers to develop interventions to improve the HR-QOL of the elderly.

Abstract:<span><b>Objectives:</b> To assess whether social isolation, social support, and loneliness are independently associated with health-related quality of life (HRQoL).<b>Method:</b> Retrospective analysis including 10,517 women aged 70–75 years from the Australian Longitudinal Study on Women's Health (ALSWH). Social isolation, social support (Duke Social Support Index), and loneliness (single item) were investigated for their association with standardised HRQoL (physical [PCS] and mental [MCS] components of the SF-36® questionnaire). Analyses were adjusted for sociodemographic variables and number of medical conditions.<b>Results:</b> Only 3% reported being socially isolated, having low social support and being lonely, and 34% reported being not socially isolated, high social support and not being lonely. Each construct was independently associated with HRQoL, with loneliness having the strongest inverse association (PCS: isolation −0.98, low support −2.01, loneliness −2.03; MCS: isolation −1.97, low support −4.79, loneliness −10.20; p-value < 0.001 for each). Women who were not isolated or lonely and with high social support had the greatest HRQoL (compared to isolated, low social support and lonely; MCS: 17 to 18 points higher, PCS: 5 to 8 points higher). Other combinations of social isolation, social support and loneliness varied in their associations with HRQoL.<b>Conclusion:</b> Ageing populations face the challenge of supporting older people to maintain longer, healthy, meaningful and community-dwelling lives. Among older women, social isolation, low social support and loneliness are distinct, partially overlapping yet interconnected concepts that coexist and are each adversely associated with HRQoL. Findings should be replicated in other cohorts to ensure generalisability across other age groups and men.</span>

Quality of life and related factors: a questionnaire survey of older people living alone in Mainland China

Abstract:Purpose: There is a growing population of older people living alone within the context of dramatic population ageing and changing living arrangements. However, little is known about the quality of life (QoL) of older people living alone in Mainland China. This study aimed to investigate QoL and its related factors among Chinese older people who live alone. Methods: A stratified random cluster sample of 521 community-dwelling older people living alone in Shanghai completed a structured questionnaire through face-to-face interviews. QoL was measured using the Older People's Quality of Life Questionnaire. Other data collected included self-rated health, physical health, cognitive function, depression, functional ability, loneliness, social support, physical activity, health services satisfaction, satisfaction with overall dwelling conditions and socio-demographic variables. Results: Older people living alone in Mainland China rated social relationships and financial circumstances as sources of low satisfaction within their QoL. Multiway analysis of variance showed that satisfaction with overall dwelling conditions, self-rated health, functional ability, depression, economic level, social support, loneliness, previous occupation and health services satisfaction were independently related to QoL, accounting for 68.8% of the variance. Depression and previous occupation had an interaction effect upon QoL. Conclusions: This study identified nine factors influencing the QoL of older people living alone in Mainland China. Interventions to increase satisfaction with dwelling conditions, improve economic level, social support and functional ability, decrease loneliness and depression and improve health services satisfaction appear to be important for enhancing their QoL.
Relationship between quality of life and perceived social support and the mediating effect of resilience in old people

Abstract:Objective To explore the relationship between quality of life and resilience,and to analyze the mediating effect of perceived social support.Methods A total of 445 old people were surveyed with Perceived Social Support Scale (PSSS),Connor-Davidson Resilience Scale (CD-RISC 10) and WHO Quality of Life-BREF (WHOQOLBREF).Results There was significantly positive correlation between perceived social support,resilience and quality of life,respectively (P ＜ 0.01).Both perceived social support and resilience predicted the quality of life (β =0.333,β =0.415).Bootstrap test showed that perceived social support partly mediated between quality of life and resilience,and the mediating effect was 0.160 7 (95 ％ CI: 0.1120-0.2184).Conclusion Perceived social support plays a mediating role between resilience and quality of life.
Chinese older adults' resilience to the loneliness of living alone: a qualitative study

Abstract:Objective: The purpose of this study was to investigate resilience factors that help Chinese older adults living alone cope with senses of loneliness. Method: Qualitative in-depth interviews were conducted on 13 older adults living alone in the community who did not show severe loneliness. Trained research assistants conducted interviews guided by semi-structured interview guidelines. The analysis followed a five-step interpretive approach. Results: The primary theme of resilience was found in cognitive competence, self and personality, and social relations. The secondary theme of resilience showed a family-oriented, relationship-focused resilience coping, which is in line with Chinese culture. Conclusion: This study demonstrated that family-oriented and relationship-focused resilience at the cognitive, self and personality, and social relations levels played significant roles in helping the Chinese older adults overcome their high risk of loneliness. Developing and maintaining dynamic daily rhythms that can integrate family-oriented and relationship-focused coping strategies at the cognitive and social levels are recommended.

Abstract:Quality of life (QOL) of older adults is a complex issue that requires an understanding of the intersection between socioeconomic and health factors. A poor quality of life (QOL) is frequently reported as sub-optimal among older adults whereby concerted and collective actions are required through an evidence-based approach. Hence, this cross-sectional study aims to determine the social and health predictors of the QOL of a community-dwelling older adult Malaysian population through a quantitative household survey using multi-stage sampling. A total of 698 respondents aged 60 years old and older were recruited and the majority of them had a good quality of life. Risk of depression, disability, living with stroke, low household income, and lack of social network were identified as the predictors of a poor QOL among the community-dwelling older Malaysians. The identified predictors for QOL provided a list of priorities for the development of policies, strategies, programmes, and interventions to enhance the QOL of the community-dwelling older Malaysians. Multisectoral approaches, especially collective efforts from both social and health sectors, are required to address the complexities of the ageing issues.

Abstract:The purpose of this study was to identify the determinants of, and relationship between, subjective loneliness, general health and quality of life (QoL) among lonely community-dwelling older adults. A cross-sectional study was carried out among a sample of lonely (i.e. live alone without any assistance) older adults living in Gorgan city, Iran. The older adults were invited through a census sampling technique to complete six questionnaires, namely a 20-item UCLA loneliness scale, a six-item Lubben Social Network Scale, a 12-item Multidimensional Scale of Perceived Social Support, a 17-item Self-care Ability Scale for the Elderly (SASE), a General Health Questionnaire-12, and a Control, Autonomy, Self-Realization and Pleasure (CASP-19) questionnaire. Hierarchical multiple linear regressions were done with subjective loneliness and QoL as outcome variables. In total, 213 (91% females) lonely older adults (mean age: 68.2 ± 6.2) participated. Hierarchical multiple linear regressions yield socio-demographic characteristics ( p < 0.001), predictor group 1 ( p < 0.001), predictor group 2 ( p < 0.001) explained 12.5, 13.5 and 46.6% of the observed variance in loneliness, respectively. Moreover, socio-demographic characteristics ( p < 0.001), predictor group 1 ( p < 0.001), predictor group 2 ( p < 0.001) and feeling of loneliness ( p < 0.001) accounted for 16.9, 21.1, 48.6 and 55.3% of the observed variance in quality of life, respectively. Both subjective loneliness and QoL were moderate in our sample. We suggest the presence of a probable causal inference between subjective loneliness, general health, and QoL; implying the need of population strategies for correcting subjective loneliness for its own sake as well as to help have an improved general health and QoL among older adults. Beside several non-modifiable factors (such as age, ethnic groups), we suggest that corrective measures should be based on social support and networking, self-care behavior, leisure activities, adapted for age, literacy and racial specifications. Given its population health relevance, there should be increased research and intervention on loneliness among older adults.

Abstract:Objective:To know about the status quo of quality of life(QOL)and its influencing factors of people more than 80 years old living at home.Methods:A total of 390 community-dwelling senile aged more than 80 years old in Shanghai were investigated by the WHOQOL-BREF questionnaire.The influencing factors were analyzed by t-test and analysis of variance.Results:The scores were in turn 44.60±11.02、48.15±10.32、55.85±9.85、42.34±11.15 respectively in the physical domain,psychological domain,social relations and the environmental domain.The score of each area was significantly lower than the norm(P<0.01).The main influencing factors of QOL were gender,dwelling style,education level,economic level,the number of species suffering from chronic diseases and daily drug administration.There were significant differences for each factor in different areas of QOL of the oldest people.Conclusion:The overall QOL of the community-dwelling senile aged is poor,which should be paid sufficient attention to by the social and the community health services.

Abstract:Abstract Resilience is one of the components for successful aging and is related to wellbeing in late life. Studies have shown that older people living alone have low resilience. However, most of these studies were mainly conducted on unhealthy participants. The aim of this study is to examine the factors that contribute to resilience of healthy older adults living alone. Older people living alone who are not subject to public health care service provided to the economically or physically challenged or depressed people were recruited. Data collected from 295 participants were used to conduct hierarchical multiple regression analyses, controlling demographic characteristics, level of cognitive and physical functions, and emotional status. A self-reported questionnaire, UCLA Loneliness Scale, Lubben Social Network Scale(LSNS), and Multidimensional Individual and Interpersonal Resilience Measure(MIIRM) were used to measure study variables. A hierarchical model accounted for 48.8% of the variance in resilience. In model 1(demographics), the religion(β=.178, p<.001) and the perceived economic status(β=-.176, p<.001) variables were significantly related to resilience. The subjective health(β=-.109, p=.038) in model 2(level of function) and the loneliness(β=-.379, p<.001) in model 3(emotional status) had a significant effect on resilience. In model 4, the size(β=-.115, p=.029) and the frequency(β=.160, p=.003) of social networks significantly predicted resilience. The results showed that protecting older adults’ social networks could lead to promote their health and wellbeing. What can be inferred from this finding is that even though the members are small, the social network they often have contact with is important for the resilience of older adults living alone.

Abstract:Abstract Background The measures imposed during the COVID-19 pandemic, especially social distancing, had important effects on feelings of loneliness. The objective of this work is to assess the perception of loneliness in older adults living in nursing homes during the pandemic, how it has changed during the pandemic and its explanatory factors. Methods The data come from the cross-sectional project “Protective environments of the elderly in health crises”, carried out in the Community of Madrid (Spain) in 2021. The variables used were the frequency of feelings of loneliness and its change during the pandemic, as well as socio-demographic, health, emotional and contextual characteristics. Descriptive statistics and logistic regression analysis were calculated. Results The sample consisted of 447 people; mean age was 83.8; 63.1% were women; half of the sample were widowed; 40% had an educative level lower than primary. Almost 3 out of 10 residents stated that they often or always/almost always felt alone. In the regression model, loneliness was negatively associated with age, number of medications, emotional balance, coping ability, self-perception of aging, and engaging in rewarding activities; while having a low assessment of mobility in the residential environment was positively associated. In addition, 28% of the participants who declared loneliness, also felt lonelier than before the pandemic. The variables associated with change in feelings of loneliness were age, self-perception of aging and a low assessment of the residential environment. Conclusions The restrictions on mobility and social relationships during the COVID-19 epidemic have affected older people who were living in residences, with an increase in the feeling of loneliness related to demographic, emotional and contextual variables. Interventions on the design of residential spaces can mitigate the effects of isolation and loneliness related to health crises such as COVID-19 pandemic.

The impact of social participation on the quality of life among older adults in China: a chain mediation analysis of loneliness, depression, and anxiety

Abstract:This cross-sectional study investigates the impact of social participation on the quality of life (QOL) among older adults in China. Using convenience sampling, data were collected from 508 individuals aged 60 and above (M_age = 70.53 ± 7.90 years; 56.5% women). Statistical analyses were conducted using SPSSAU software, including Pearson correlation analysis to assess relationships between social participation, psychological health indicators (loneliness, depression, and anxiety), and QOL. Multiple regression analysis and chain mediation analysis were subsequently performed to explore the mediating effects of loneliness, depression, and anxiety on the relationship between social participation and QOL. The results indicated significant correlations between social participation and loneliness (r = -0.313, p < 0.001), depression (r = -0.487, p < 0.001), anxiety (r = -0.305, p < 0.001), and QOL (r = 0.476, p < 0.001). The mediation analysis revealed significant chain mediation effects of loneliness, depression, and anxiety on the relationship between social participation and QOL (β = 0.006, p < 0.001, 95% CI [0.001, 0.007]). Higher levels of social participation were associated with lower levels of loneliness, which in turn reduced depression and anxiety, thereby enhancing QOL. These findings highlight the importance of promoting social participation to improve psychological wellbeing and QOL among older adults in China. The study advocates for active social engagement and the provision of relevant services, as well as psychological support and emotional counseling for those facing mental health challenges due to insufficient social participation.

Abstract:Background Loneliness is a significant public health challenge in the United States, especially among older adults. The epidemiology of loneliness among older adults in primary care is lacking, and specific research is needed on how loneliness impacts older primary patients' physical, mental, and cognitive health. A large sample of older primary care patients were recruited for a trial during the COVID‐19 pandemic to measure the relationship between loneliness and physical and mental quality of life (QOL). Methods Baseline data come from the Caregiver Outcomes of Alzheimer's Disease Screening (COADS) study, an ongoing randomized controlled trial evaluating benefits and risks of Alzheimer's disease and related dementias screening among primary care patients ages 65 and older, collected April 2020 to September 2021. Loneliness was measured with the 5‐item, Loneliness Fixed Form Ages 18+ from The NIH Toolbox Emotion Battery, physical and mental health‐related QOL was measured with the SF‐36v2, and depression and anxiety severity were measured with the PHQ‐9 and GAD‐7, respectively. Quality of life in aging process: the role of work-related psychosocial conditions and social support

Abstract:Aging has been a growing source of increased opportunities raised by longevity, but also a growing source of concern,particularly with regard to the mental health and the well-being of elderly people. As such, it is highly relevant to have a better understanding of the relationship between workplace related psychosocial factors and quality of life, not only among working population, but also among retired individuals. The main objective of this study is to better understand and characterize the relation between quality of life (QoL), work-related psychosocial conditions, and social support for both working and retired adults. Participants were 1.330, 62.2% of which female, with ages ranging between 55 and 75 years old. 47% of the respondents were working and 46 % were retired. Three structural equation models were tested, using the total sample and by labor situation (working and retired). The results showed that the stress management (with a greater effect in retired participants) and the social support (with a greater effect in working participants) have a positive influence on the QoL factor. Better work-related psychosocial conditions and higher social support were predictors of higher/better QoL. Our study allowed us to conclude that QoL is influenced by working situation, social support and work-related psychosocial conditions. This is a powerful message to policy makers in view of the growth of older populations.

Abstract:With an ageing world population, preservation of older adults’ health and quality of life (QoL) is paramount. Due to lower levels of physical functionality, older adults are particularly susceptible to local environment influences, especially those living alone and lacking family support. Using generalised additive mixed models, we examined associations and confounder-adjusted associations between objectively-measured neighbourhood attributes and QoL domains in 909 Hong Kong Chinese elderly community dwellers. Most examined neighbourhood attributes were not associated with QoL in the whole sample. Neighbourhood residential and entertainment density was curvilinearly and/or linearly related to specific QoL domains. Number of parks was negatively associated with social QoL and having well-treed parks with higher levels of social QoL. Older adults living alone in neighbourhoods with poor access to destinations and few activities in parks showed lower environmental and/or social QoL than their counterparts. Neighbourhood built environment characteristics do not seem to impact Hong Kong older adults’ physical and psychological QoL. Medium-to-high density, well-ordered neighbourhoods with optimal mixes of well-treed public open spaces and services meeting their daily needs may significantly contribute to social and environmental QoL in this population and appear particularly important to those living alone.

Abstract:BackgroundLitter is known about the well-being of nursing home (NH) residents in Singapore. This study aimed to identify predictors of self-reported quality of life (QOL) of NH residents in Singapore.MethodsIn face-to-face interviews, trained medical students assessed each consenting resident recruited from 6 local NHs using a modified Minnesota QOL questionnaire, and rating scales and questions assessing independence, cognitive function, depression, and communication. Predictors of residents’ QOL in five aspects (comfort, dignity, food enjoyment, autonomy, and security) were identified using the censored least absolute deviations (CLAD) models.ResultsA total of 375 residents completed the interviews. A higher score on comfort was negatively associated with major depression while a higher score on dignity was positively associated with no difficulty in communication with staff. Higher scores in food enjoyment were negatively associated with major depression and poorer cognitive function. Higher scores in autonomy were negatively associated with major depression, greater dependence, and difficulty in communication with staff. A higher score on security were negatively associated with major depression.ConclusionIt appears that depression and difficulty in communication with staff are the two main modifiable risk factors of poor quality of life of local NH residents.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Older adults with multiple complex care needs tend to receive fragmented care that may jeopardize their quality of life (QoL) and health outcomes. This study evaluated the determinants of improved QoL among integrated outpatient service recipients with multimorbidity.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>We conducted a retrospective cohort study of integrated geriatric outpatient services (IGOS) at a tertiary medical center in Taiwan. Data from 2018 to 2019 were retrieved. All patients underwent comprehensive geriatric assessment, which included demographic information, serial functional assessments, and assessment for QoL. QoL was reassessed through a telephone survey 6 months after the patients' first visit to IGOS. Factors associated with the interval changes in QoL were identified using multivariate logistic regression.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Data from 995 patients receiving IGOS (mean age: 82.21 ± 7.96 years, 54.5% males) were analyzed. An overall mean improvement in QoL was noted (EQ-5D index: +0.055±0.26, p <0.001) while 747 recipients reported maintained or improved QoL. The results of the multivariate logistic regression showed that poorer nutritional status (OR = 1.56, 95% CI: 1.07 - 2.28), depressive symptoms (OR = 1.99, 95% CI: 1.38 - 2.86), and frailty (OR = 1.66, 95% CI: 1.10 - 2.52) were independent risk factors for poorer QoL after adjustment for baseline QoL.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Integrated outpatient services improved the quality of life of older adults with multimorbidity. Those with poorer nutritional status, depressive symptoms and frailty were less likely to show improvement in their QoL.</p>
