Risk of Stroke in Healthy Postmenopausal Women During and after Hormone Therapy: a Meta-Analysis.

Abstract:OBJECTIVE:This meta-analysis aims to examine the risk of stroke in healthy postmenopausal women during and after hormone therapy (HT).METHODS:Medline, The Cochrane Library, EMBASE, and Google Scholar were searched for randomized controlled trials involving healthy postmenopausal women who received HT and were followed up for at least 3 years after starting treatment. The primary outcome measures were stroke hazard ratio (HR) for the intervention phase, stroke HR for the postintervention phase, and overall stroke HR.RESULTS:Four studies, involving 15,423 participants who received HT and 14,582 participants who received placebo, met the criteria for inclusion in the meta-analysis. The mean age of participants ranged from approximately 50 to 64 years. HT was given as conjugated equine estrogens in three studies and as 17β-estradiol in one study. The duration of HT ranged from 3.0 to 10.1 years. The length of follow-up after the start of HT ranged from 3.0 to 15.8 years. Meta-analysis revealed that the stroke HR during the intervention phase and the overall stroke HR were significantly increased among women who received HT (intervention phase: pooled HR, 1.32; 95% CI, 1.12-1.56; P = 0.001; overall: pooled HR, 1.15; 95% CI, 1.03-1.28; P = 0.017). The stroke HR during the postintervention phase was not increased among women who received HT (pooled HR, 1.00; 95% CI, 0.85-1.16; P = 0.958). Sensitivity analysis confirmed the reliability of the meta-analysis for both outcomes.CONCLUSIONS:These findings suggest that HT may increase the risk of stroke during, but not after, HT in healthy postmenopausal women.
Change in Metabolic Parameters and Reproductive Hormones from Baseline to 6-Month Hormone Therapy

Abstract:Adequate evidence showed hormone therapy (HT) reduces the risk of new-onset diabetes in midlife women by decreasing fasting glucose and insulin. However, the improvement of these diabetic biomarkers varied with each individual in clinical observations. The objective of our study was to investigate potential baseline factors associated with the change of fasting glucose and insulin during HT. A retrospective cohort study was performed among 263 midlife participants aged 40 to 60 years with menopausal symptoms who have received 6-month individualized HT. Demographic information and laboratory indicators including reproductive hormone, lipid profiles, diabetic indicators were collected and measured at baseline and were followed-up. A series of statistical analyses were performed to confirm the effectiveness of HT and compare the baseline factors between participants with different glycemic or insulinemic response. Multivariable linear regression model with stepwise variable selection was further used to identify the associated factor with the change of fasting glucose and insulin. Of all participants, fasting glucose (P = .001) and fasting insulin (P < .001) were significantly decreased after individualized HT. Significant differences in baseline reproductive hormones were observed in participants with different glycemic response to HT (P < .001 for both follicle stimulating hormone [FSH] and estradiol). Stepwise linear regression model showed that in addition to baseline fasting glucose levels, baseline FSH was also independently associated with the change of fasting glucose (beta = -0.145, P = .019 for baseline FSH) but not fasting insulin. Greater reduction in fasting glucose in women with higher FSH levels was observed even though they have already been in better metabolic conditions (P = .037). Midlife women with higher baseline FSH levels have greater reduction in fasting glucose but not fasting insulin. FSH could be an independent predictor of glycemic response to HT in peri- and postmenopausal women.
Effects of hormone replacement therapy on mood and sleep quality in menopausal women

Abstract:Background: Hormone replacement therapy is an effective treatment strategy for the management of symptoms in naturally menopausal women. However, some patients report experiencing adverse effects. Aim: To analyze the effects of hormone replacement therapy in menopausal female patients. Methods: A total of 152 menopausal female patients admitted to the Gynecology Department of the Ganzhou Maternal and Child Health Hospital between January 2021 and December 2023 were divided into the observation group (n = 76, conventional treatment + hormone replacement therapy) and the control group (n = 76, conventional treatment only) via random casting. The improvement observed in the following items were compared between the groups: Kupperman menopausal index (KMI), emotional state [The Positive and Negative Affect Scale (PANAS)], sleep quality [Self-Rating Scale of Sleep (SRSS)], treatment effectiveness, and treatment safety. Results: The modified KMI and SRSS scores of the observation group were lower than those of the control group after three rounds of treatment. The improvement in the PANAS score observed in the observation group was greater than that observed in the control group (P < 0.05). The total treatment effectivity rate in the observation group was higher than that in the control group (86.84% vs 96.05%, χ 2 = 4.121, P = 0.042). The incidence rate of adverse reactions in the two groups was comparable (6.58% vs 9.21%, χ 2 = 0.361, P = 0.547). Conclusion: Hormone replacement therapy effectively improved the clinical symptoms, actively channeled negative emotions, and improved the quality of sleep in menopausal patients, indicating its effectiveness and safety.
Effect of Hormone Replacement Therapy on Heart Rate Variability in Postmenopausal Women

Abstract:Objective To assess the characteristics of autonomic control of heart rate in postmenopausal women before and after hormone replacement therapy (HRT).Methods Time domain and spectral domain heart rate variability ( HRV) were performed in 58 postmenopausal women before and after HRT.Results Compared with examinations at baseline, values of all HRV measurements in subjects given HRT were increased at the fourth month (P < 0.05), while they remained unchanged in the control group (P > 0.05). Results showed an inverse correlation between all measures of HRV and mean R-R interval in the treatment group.Conclusion HRT reduces sympathetic drive in postmenopausal symptomatic women. HRV may be an index of imbalance of autonomic control in perimenopausal women.

Abstract:Objective To evaluate efficacy and safety of standardized management of hormone therapy in menopausal women.Methods The menopausal women visited the endocrine clinic of Beijing Obstetrics and Gynecology Hospital affiliated to Capital Medical University after 2003 were divided into two group,hormone therapy and control group.There were 55 cases in hormone therapy group,and 34 cases in control group.We analyzed the variation of Kupperman sore,serum lipoprotein,resting energy expenditure,body fat,liver and renal function and mammary glands in the hormone therapy,so that assess the effectiveness and safety of hormone replacement therapy.Results ① The Kupperman scores tended to decrease.② BMI did not change significantly in HT group and control group(P >0.05);BF% in the control group was significantly higher in the second year than the first year(P < 0.05),while,compared with the first three years,the hormone therapy group BF% in the fourth year of treatment decreased significantly(P < 0.05).Compared with the first year,REE / kg was reduced gradually in the second year in control group,however,REE / kg increased in HT group.③ TC in control group increased gradually;LDL increased significantly in the second year(P < 0.05).However,TC,TG and LDL-C were became lower gradually in HT group.④ Liver and renal function was not significantly different during HT.The glands group of mammary gland between the first year and 5th year was not significantly different.Conclusion Hormone replacement therapy can relieve menopause-related symptoms;hormone therapy may alleviate abnormal blood lipids change in menopausal women;at the same time,it increases REE,eases the trend of BF% increase in menopausal women,so alleviates the progress of abdominal obesity after menopause,and to certain extent,it can reduce the risk of cardiovascular disease.During the standardized and individualized hormone therapy,the breast cancer risk is small.

Abstract:Objective: The aim of the study was to investigate the impact of hormone therapy (HT) on the ocular surface and intraocular pressure in postmenopausal women. Methods: This systematic review and meta-analysis was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-analyses Statement. PubMed, EMBASE, Cochrane Library of Systematic Reviews, Cochrane Central Register of Controlled Trials, ClinicalTrials.gov, Chinese Biomedical Literature Database, and China National Knowledge Infrastructure were searched from inception to November 2019 without language restrictions. Only randomized controlled trials that evaluated the impact of HT on the ocular surface and intraocular pressure in postmenopausal women were eligible. The trials had to report at least one of the following outcomes: break-up time, Schirmer test, corneal staining, ocular surface symptom score, and intraocular pressure. Two investigators independently extracted the information, assessed the risk of bias, and evaluated the publication bias. All data were analyzed by Review Manager V.5.3. Sensitivity analysis and subgroup analysis were performed to find the source of heterogeneity and evaluate the different effects among subgroups. Results: Nine randomized controlled trials (N = 612) were included. The HT group showed significant improvements compared with the control group in break-up time (mean difference [MD] = 2.09, 95% confidence interval [CI] 1.00-3.19, P = 0.0002), Schirmer test without anesthesia (MD = 4.17, 95% CI 1.55-6.80, P = 0.002), Schirmer test with anesthesia (MD = 1.44, 95% CI 0.71-2.18, P = 0.0001), and corneal staining scores (standardized mean difference [SMD] = -0.85, 95% CI -1.39 to -0.30, P = 0.002). Moreover, significant beneficial effects were observed on all four symptoms, including dryness (SMD = -1.21, 95% CI -1.99 to -0.44, P = 0.002), foreign body sensation (SMD = -1.02, 95% CI -1.29 to -0.76, P < 0.00001), ocular fatigue (SMD = -1.74, 95% CI -2.12 to -1.36, P < 0.00001), and burning (SMD = -0.53, 95% CI -0.78 to -0.29, P < 0.0001) after HT. Subgroup analysis revealed that, in terms of break-up time, postmenopausal women younger than 55 years achieved more improvements (MD = 0.88, 95% CI 0.16-1.59, P = 0.02) than women older than 55 years old (MD = 2.60, 95% CI -1.34 to 6.55, P = 0.20), and the estrogen subgroup received more benefits (MD = 3.11, 95% CI 0.93-5.30, P = 0.005) than the estrogen plus progestogen subgroup (MD = 0.42, 95% CI -0.02 to 0.85, P = 0.06). Sensitivity analysis and subgroup analysis suggested that the heterogeneity might derive from the methodological quality, the age of participants, and the intervention of the control group. Intraocular pressure (MD = -1.54, 95% CI -3.39 to 0.32, P = 0.10) was not evidently decreased after HT. No more specific adverse events (relative risk = 1.66, 95% CI 0.41-6.77, P = 0.48) were found in the HT group. Conclusions: Our study revealed that HT could improve ocular surface function in postmenopausal women effectively and safely, especially for those who were younger than 55 years, and estrogen only showed more improvements than estrogen plus progestogen. The effectiveness of HT in treating dry eye in postmenopausal women is, however, still a controversial topic. In addition, we did not find HT led to a significant reduction of intraocular pressure.

Abstract:Abstract Background To evaluate the endocrine hormone and metabolic indices in postmenopausal women with euthyroid and mild subclinical hypothyroidism after menopause hormone therapy (MHT). Methods A retrospective study of 587 postmenopausal women receiving MHT was conducted. Median (25–75th percentile) age was 52 (49–54) years. According to thyroid stimulating hormone (TSH) levels at initial diagnosis, the patients were divided into three groups: I (euthyroid with low normal TSH range, n = 460), II (euthyroid with upper normal TSH range, n = 106) and III (mild subclinical hypothyroidism, n = 21). After a continuous oral MHT regimen using the same estradiol potency for 6–18 month cycles, serum endocrine hormone and metabolic indices were reassessed. Results Compared with baseline, serum TSH levels in groups I and II significantly changed but all values were within the normal range. No significant difference was observed in serum TSH levels in group III. After treatment, all serum free tri-iodothyronine and free thyroxine levels were within the normal range. Serum total cholesterol, triglyceride, fasting plasma glucose, fasting insulin levels and homeostasis model assessment of insulin resistance index had significantly decreased in group I. There were no significant differences in all observed lipid and glucose parameters in group III, before and after treatment. Conclusion MHT did not affect thyroid function in postmenopausal women with euthyroid and mild subclinical hypothyroidism. MHT led to an improvement in lipid and glucose indicators in euthyroid women with low normal TSH range.
Impact of Hormone Replacement Therapy on the Quality of Life in Climacteric Women

Abstract:OBJECTIVE To evaluate the effects of hormone replacement therapy (HRT) on the quality of life in climacteric women.METHODS The 202 patients were collected and divided into three groups according to whether or not they were treated with HRT:group 1 (not HRT),group 2 (HRT for the first time),group 3 (HRT for more than one year).All patients received follow-up visiting once per three months for one year.They were evaluated by the professionals through Kupperman index(KI),menopause-specific quality of life questionnaire(MENQOL),self-rating anxiety scale(SAS),self-rating depression scale(SDS).All patients were guided to improve the way of life.RESULTS After a year,KI,MENQOL and SAS in the three groups were improved significantly (P =0.003,0.007,0.014).KI and MENQOL were improved earlier than SAS.SDS was improved but not significantly(P =0.109).KI,MENQOL,SAS and SDS of patients in group 1 were improved significantly,but improved more weakly than those in group 2 and group 3.There is negative correlation between HRT and the values of every scale,and there is positive correlation between culture degree and SAS,SDS score values.CONCLUSION HRT canim prove the quality of life in climacteric women.The improvement in physical symptoms is earlier than that in mental symptoms.Anxiety and depression are more likely to occur in menopausal patients with high degree of education.Traditional Chinese medicine,exercise therapy and physical rehabilitation can be used as an auxiliary treatment for patients who are unable or unwilling to accept the HRT.
The effects of compounded bioidentical transdermal hormone therapy on hemostatic, inflammatory, immune factors; cardiovascular biomarkers; quality-of-life measures; and health outcomes in perimenopausal and postmenopausal women

Abstract:Menopause impacts 25 million women world wide each year, and the World Health Organization estimates 1.2 billion women will be postmenopausal by 2030. Menopause has been associated with symptoms of hot flashes, night sweats, dysphoric mood, sleep disturbance, and conditions of cardiovascular disease, depression, osteoporosis, osteoarthritis, depression, dementia, and frailty. Conventional hormone replacement therapy results in increased thrombotic events, and an increased risk of breast cancer and dementia as evidenced in large prospective clinical trials including Heart and Estrogen/Progestin Replacement Study I and the Women's Health Initiative. A possible mechanism for these adverse events is the unfavorable net effects of conjugated equine estrogens and medroxyprogesterone acetate on the hemostatic balance and inflammatory and immune factors. Physiologic sex steroid therapy with transdermal delivery for peri/postmenopausal women may offer a different risk/benefit profile, yet long-term studies of this treatment model are lacking. The objective of this study was to examine the long-term effects of compounded bioidentical transdermal sex steroid therapy including estriol, estradiol, progesterone, DHEA, and testosterone on cardiovascular biomarkers, hemostatic, inflammatory, immune signaling factors; quality-of-life measures; and health outcomes in peri/postmenopausal women within the context of a hormone restoration model of care. A prospective, cohort, closed-label study received approval from the Human Subjects Committee. Recruitment from outpatient clinics at an academic medical center and the community at large resulted in three hundred women giving signed consent. Seventy-five women who met strict inclusion/exclusion criteria were enrolled. Baseline hormone evaluation was performed along with baseline experimental measures. Following this, women received compounded transdermal bioidentical hormone therapy of BiEst (80%Estriol/20%Estradiol), and/or Progesterone for eight weeks to meet established physiologic reference ranges for the luteal phase in premenopausal women. The luteal phase hormone ratios were selected based on animal and epidemiologic studies demonstrating favorable outcomes related to traumatic, ischemic, or neuronal injury. Follow-up testing was performed at eight weeks and adjustment to hormone regimens were made including addition of androgens of DHEA and Testosterone if indicated. Experimental subjects were monitored for 36 months. Baseline, 2-month, and annual values were obtained for: blood pressure, body mass index, fasting glucose, Homeostasis Metabolic Assessment of Insulin Resistance (HOMA-IR), fasting triglycerides, total Factor VII, Factor VIII, fibrinogen, Antithrombin III, Plasminogen Activator Inhibitor1(PAL-1), C-reactive protein (CRP), Interleukin-6 (IL-6), Matrix Metalloproteinase-9 (MMP-9), Tumor Necrosis Factor-alpha (TNF), Insulin-like Growth Factor (IGF-1), and sex steroid levels. Psychosocial measures included: Greene Climacteric Scale, Visual Analog Pain Scale, Hamilton Anxiety Scale, Hamilton Depression Scale, Holmes Rahe Stress Scale, Job Strain, and Home Strain. Health outcome measures included the number of prescribed medications used, number of co-morbidities, and endometrial thickness in postmenopausal women with intact uteri. Subjects receiving compounded transdermal bioidentical hormone therapy showed significant favorable changes in: Greene Climacteric Scale scores, Hamilton Anxiety Scale, Hamilton Depression Scale, Visual Analog Pain Scale, fasting glucose, fasting triglycerides, MMP-9, C-reactive Protein, fibrinogen, Factor VII, Factor VIII, Insulin-Like Growth Factor 1, and health outcomes of co-morbidities and a number of prescribed medications. Antithrombin III levels were significantly decreased at 36 months. All other measures did not exhibit significant effects. Administration of compounded transdermal bioidentical hormone therapy in doses targeted to physiologic reference ranges administered in a daily dose significantly relieved menopausal symptoms in peri/postmenopausal women. Cardiovascular biomarkers, inflammatory factors, immune signaling factors, and health outcomes were favorably impacted, despite very high life stress, and home and work strain in study subjects. The therapy did not adversely alter the net prothrombotic potential, and there were no associated adverse events. This model of care warrants consideration as an effective and safe clinical therapy for peri/postmenopausal women especially in populations with high perceived stress and a history of stressful life events prior to, or during the menopausal transition.
Update on hormone therapy for the management of postmenopausal women

Abstract:Hormone therapy (HT) has been used in postmenopausal women for decades in clinical practice. With further analysis and newer studies, the benefits and risks of HT have been repeatedly verified and discussed. HT is recommended for the treatment of vasomotor symptoms (VMS), genitourinary syndrome of menopause (GSM) and the prevention of osteoporosis. However, the precise association between HT and the risks of cardiovascular diseases, venous thromboembolism, neurodegenerative diseases, breast cancer, and endometrial cancer remains controversial. Therefore, determining how to take advantage of and control the risks of HT by adjusting the initiation time, regimen, and duration is crucial. Recent studies have indicated that HT is not related to the risk of all-cause, cardiovascular, or breast cancer mortality although it might increase the incidence of some chronic diseases. For symptomatic postmenopausal women under the age of 60 without contraindications, early initiation of HT is safe and probably has a mortality benefit over the long term. Initiating HT close to menopause at the lowest effective dose is more likely to have maximal benefits and the lowest risks. Transdermal and vaginal HT may have a lower risk, but recent evidence suggests additional clinical benefits of oral HT formulations in relieving VMS and preventing osteoporosis. The pooled cohort risk equation for atherosclerotic cardiovascular disease (ASCVD) and the free app named Menopro can be used to perform individual risk assessments. In addition, Chinese herbal medicines have benefits in alleviating hot flashes, depression, and menopausal symptoms, although further data are needed to strongly support their efficacy. Acupuncture and electroacupuncture have definite efficacy in the treatment of postmenopausal symptoms with few adverse effects, so they are a reasonable option as an alternative therapy for high-risk women. This review discusses the history of, guidelines on, and strategies for HT in order to make suggestions based on the most up-to-date evidence for the management of postmenopausal women.

Abstract:Background The influence of menopausal hormone therapy (MHT) on the probability of developing diabetes mellitus in individuals with prediabetes remains uncertain. Methods This retrospective cohort study, utilizing the TriNetX U.S. Collaborative Network, investigated cohorts, implemented propensity score matching, and analyzed outcomes associated with diabetes mellitus. The study focused on individuals aged 46–60 with prediabetes prior to menopause, categorizing them into MHT and non-MHT groups. Further stratified analyses, including variables such as age and race, were conducted to thoroughly examine potential variations in outcomes. Results The study involved 6566 individuals (MHT and non-MHT), with propensity score matching ensuring balanced cohorts. Over a 20-year follow-up, the MHT group demonstrated a lower incidence of diabetes mellitus compared to the non- MHT group, with a Hazard Ratio of 0.693 (95 % CI: 0.577, 0.832). Stratified analyses revealed age-specific nuances, with significant protective effects in individuals aged 46–50 and 55–60. Additionally, ethnicity played a role, with MHT demonstrating significant benefits in White individuals but not in the Black or Asian populations. BMI analysis indicated a significant risk reduction with MHT in individuals with BMI less than or equal to 24.9 and 25–29.9 kg/m 2 , but not in those with BMI greater than or equal to 30 kg/m 2 . Conclusion In our study, we demonstrate a sustained 20-year decrease in the risk of diabetes among premenopausal individuals with prediabetes who undergo menopausal hormone therapy.

Abstract:Objective:To explore the associations of menopause with cardiovascular disease (CVD) and related metabolic disorders (including hypertension,diabetes or higher blood sugar,obesity,dyslipi-demia)in Chinese females aged 45 -59 years .Methods:Data were acquired from a national cross-sectional survey conducted in 201 3,China,which was also the second follow-up survey of China Health and Retirement Longitudinal Study (CHARLS).In the study,4 702 Chinese perimenopausal women aged 45 -59 years were enrolled,including 1 769 premenopausal women and 2 933 postmenopausal women.Information was collected from questionnaires of health status and functioning and physical exa-mination.General liner models were employed to calculate age-adjusted or age-and-body-mass-index-adjusted or multiple-factor-adjusted means and their 95% confidence intervals (95% CIs)of cardiovas-cular risk factors (CRFs).The comparisons of CVD and its risk factors according to menopausal status, and calculation of adjusted odds ratios (ORs)and their 95%CIs for the associations of menopause with CVD and its risk factors were performed by multivariate Logistic regression models separately.Results:After adjustment for age and other confounders (including body mass index,marriage,education,current smoking,drink alcohol more than once per month),statistically significant associations of menopause with cardiovascular disease,which referred to having a history of heart disease or stroke in this study, were observed in the participants (OR =1 .34,95% CI:1 .04 -1 .74);prevalence of hypertension (OR =1 .42,95%CI:1 .1 0 -1 .84),prevalence of CRFs clustering number≥2 (OR =1 .31 ,95%CI:1 .02 -1 .68)and average waist circumference level (87.1 1 cm,95%CI:86.81 -87.42 cm in post-menopausal group vs.86.41 cm,95%CI:85.99 -86.84 cm in premenopausal group)were presented higher in postmenopausal group,compared with the premenopausal one.However,diabetes or higher blood sugar (OR =0.96,95%CI:0.60 -1 .52),dyslipidemia (OR =0.84,95%CI:0.59 -1 .20) and obesity (OR =1 .06,95%CI:0.86 -1 .32)were not shown significantly statistically related to me-nopause,after excluding effect of age and other confounders.Conclusion:Postmenopausal women in China had worse CRFs profile than the premenopausal ones,which implied menopause might aggravate the CRFs epidemic and increase the risk of cardiovascular disease beyond effects of aging,which would increase the CVD burden during and after their middle ages.
Impact of Chinese Herbal Medicine on Glucolipid Metabolic Outcomes in Women with Polycystic Ovary Syndrome: A Systematic Review and Meta-Analysis

Abstract:Objective. This investigation was conducted to analyze and evaluate the impact of Chinese herbal medicine on glucolipid metabolism in women with polycystic ovary syndrome (PCOS). Methods. We used manual and computer-aided search methods, and the search scopes included Chinese databases (China National Knowledge Infrastructure, Wanfang, the China Science and Technology Journal Database, and the Chinese Biomedical Literature Database) and English databases (PubMed, Embase, Web of Science, and the Cochrane Library). We searched these eight databases for randomized controlled trials investigating the effects of Chinese herbal medicine on glucolipid metabolism in women with PCOS, with the retrieval deadline being June 2021. Two reviewers screened, selected, and extracted data and verified the results independently. The NoteExpress software was used to manage and screen the literature, the risk of bias assessment tool was used to evaluate the methodological quality of the included studies, and the RevMan 5.4 software was used for meta-analysis. Results. A total of 13 trials were included, including 825 patients with PCOS. Because the drugs used in the control group were different, we divided the results into two parts, with four trials using placebo and nine trials using metformin as the control. The results of the meta-analysis showed that fasting insulin (MD = −2.45, 95% CI = [−4.74, −0.17], P = 0.04), 2 h fasting plasma glucose (MD = −0.33, 95% CI = [−0.64, −0.02], P = 0.04), serum total cholesterol (MD = −0.38, 95% CI = [−0.58, −0.18], P = 0.0002), triglycerides (MD = −0.36, 95% CI = [−0.58, −0.14], P = 0.001), and low-density lipoprotein cholesterol (MD = −0.58, 95% CI = [−0.75, −0.41], P < 0.00001) were significantly improved in the Chinese herbal medicine group compared with the placebo group. In addition, compared with metformin, body mass index (MD = −1.04, 95% CI = [−1.55, −0.53], P < 0.0001), serum total cholesterol (MD = −0.27, 95% CI = [−0.46, −0.07] P = 0.007), and low-density lipoprotein cholesterol were significantly reduced (MD = −0.12, 95% CI = [−0.22, −0.02], P = 0.02) and high-density lipoprotein cholesterol (MD = 0.09, 95% CI = [0.02, 0.17], P = 0.01) was significantly improved after treatment with Chinese herbal medicine. Conclusion. Compared with the placebo group, Chinese herbal medicine had positive effects on glucolipid metabolism in women with PCOS. Chinese herbal medicine had a positive effect on lipid metabolism when the control group was metformin, but no effect on glucose metabolism. These findings need to be verified in high-quality, large-sample, randomized controlled trials in the future.

Abstract:Background: Perimenopausal syndrome (PMS) significantly impacts the quality of life in women during the perimenopausal phase. Despite being the primary treatment, hormone replacement therapy has notable limitations. Consequently, dietary interventions, particularly low-fat diets, have garnered international attention as an alternative. Methods: The prospective Randomized Controlled Trial (RCT), approved by the Ethics Committee of the Affiliated Hospital of Guizhou Medical University, enrolled 62 perimenopausal women. Participants were divided into two groups: a low-fat diet group and a menopausal hormone therapy (MHT) group, with interventions lasting eight weeks. Both groups underwent assessments for perimenopausal symptoms, lipid profile, and oxidative stress markers, including superoxide dismutase (SOD), malondialdehyde (MDA), and reactive oxygen species (ROS), before and after the interventions. Additionally, DNA methylation levels of the PDE4C and NPAS2 genes were measured in 10 women from the low-fat diet group before and after the intervention. Results: Both the low-fat diet and hormone supplementation improved specific perimenopausal symptoms, reduced specific lipid levels, and had a positive effect on oxidative stress markers. The low-fat diet proved more effective than hormone replacement in improving the serum lipid profile and also resulted in reductions in methylation at specific sites of the PDE4C and NPAS2 genes. Conclusions: A low-fat diet may alleviate perimenopausal symptoms, lower lipid levels, and alter gene methylation sites. Abstract:Objectives: To observe the effectiveness and safety of menopause-related hormone therapy (MHT) to prevent bone loss in Chinese women during the menopausal transition and early menopause, as well as to evaluate the effects of 5-year MHT on overall health to add Level I evidence for the prevention of osteoporosis using MHT.Design: This clinical study was a prospective, double-blind, randomized, parallel placebo-controlled study. Chinese women in the menopausal transition and early menopause were randomly allocated to the MHT group or the placebo group. All subjects received a 5-year intervention. The effectiveness of MHT for bone mineral density (BMD) and bone metabolism and the safety of MHT in relation to glycolipid metabolism, breast cancer, and cardiovascular disease were studied.Results: In the MHT group, women in both transition and early menopause showed a significant increase in lumbar and femoral neck BMD after the 1st year of therapy; BMD tended to decrease in the 3rd year but ultimately was sustained at stable levels that were near the baseline levels. In the placebo group, BMD decreased at both sites. Metabolism indexes and breast ultrasound examination findings did not differ significantly between the MHT and placebo groups. Three cases of breast cancer and three cases of cardiovascular disease were diagnosed during follow-up. One breast cancer case and two cardiovascular disease cases occurred in the MHT group.Conclusions: Five-year sequential therapy with estrogen and progesterone can increase or maintain the BMD of women in their menopausal transition and early menopause. This regimen had no negative effect on glycolipid metabolism and did not increase the risk of breast cancer or cardiovascular events.
Effects of physical exercise on health-related quality of life and blood lipids in perimenopausal women: a randomized placebo-controlled trial.

Abstract:OBJECTIVE:This study aims to evaluate the treatment effects of physical exercise on menopausal symptoms in middle-aged female medical staff experiencing perimenopausal syndrome. METHODS:A total of 157 female medical staff aged 40 to 55 years and with a Kupperman index score of 15 points or higher were randomized 1:1 into an intervention group (n = 78) or a control group (n = 79). Women in the intervention group were asked to perform aerobic physical exercise (walking with strides) three times a week or more, whereas those in the control group continued as normal. Measurements were taken at baseline and on weeks 4, 8, and 12, with total Kupperman index score, scores on individual elements of the scale, weight, and waist circumference recorded. In addition, fasting blood glucose, triglycerides, total cholesterol, high-density lipoprotein cholesterol, and low-density lipoprotein cholesterol were recorded at baseline and on week 12. The effects of physical exercise therapy on perimenopausal syndrome were evaluated by comparing changes in these parameters between the control group and the intervention group. RESULTS:Fifty-four and 57 women completed all three follow-ups in the intervention and control groups, respectively. On week 12, the mean (SD) change in total Kupperman index score (-9.23 [6.23]) from baseline to week 12; the mean (SD) changes in individual scores for paresthesia (-1.08 [1.51]), insomnia (-1.00 [1.46]), irritability (-1.00 [1.34]), joint or muscle pain (-0.75 [0.74]), fatigue (-0.56 [0.75]), headache (-0.54 [0.75]), formication (-0.38 [0.66]), and sexual life (-0.62 [1.71]); and the mean (SD) changes in total cholesterol (-0.76 [0.63] mmol/L) and triglycerides (-0.20 [0.50] mmol/L) were significantly higher in the intervention group than in the control group (P < 0.05). In the intervention group, total Kupperman index score, weight, body mass index, waist circumference, triglycerides, and total cholesterol were significantly lower on week 12 compared with baseline (P < 0.05). CONCLUSIONS:Physical exercise can substantially reduce menopausal symptoms and improve blood lipid status and body weight.

Abstract:Objective This study aimed to investigate the responses of periodontal environment to hormone replacement therapy (HRT) in postmenopausal women with or without periodontitis. Background HRT is a common and effective strategy for controlling menopausal symptoms, while the changes of periodontal environment under it, particularly in postmenopausal women with periodontitis, remain unclear. Methods As a prospective cohort study, a total of 97 postmenopausal women receiving HRT were screened, including 47 with and 50 without periodontitis. Correspondingly, 97 women did not receiving HRT were screened as controls during the same period. The full‐mouth sulcus bleeding index (SBI), bleeding on probing (BOP), probing pocket depth (PPD), and clinical attachment level (CAL) were measured using periodontal probes. The levels of interleukin‐6 (IL‐6) and tumor necrosis factor α (TNF‐α) in the gingival crevicular fluid were measured using enzyme‐linked immunosorbent assay. In addition, cone beam computed tomography was performed to measure the alveolar bone height (ABH) and bone mineral density (BMD). Results In postmenopausal women without periodontitis, no significantly changes on periodontal parameters were observed after HRT. In women with stage II periodontitis, SBI, BOP, IL‐6, and TNF‐α were significant decreased after one year and two years of HRT. Compared to the controls, women with stage II periodontitis who underwent HRT had significantly lower CAL and ABH and higher BMD in the second year. The incidence of at least one site with CAL increase ≥1 mm between baseline and 2 years was significantly lower in the HRT group than in the control group in women with stage II periodontitis. In addition, HRT was significantly associated with a decrease in SBI, BOP, IL‐6, and TNF‐α in the first year and with a decrease in CAL, SBI, BOP, IL‐6, and ABH and an increase in BMD in the second year. Conclusions In postmenopausal women with stage II periodontitis, HRT is associated with the alleviation of inflammation within two years and the remission of alveolar bone loss in the second year. HRT appears to decrease the incidence of CAL increase ≥1 mm within 2 years in women with periodontitis by inhibiting inflammation and alveolar bone loss.

Abstract:Objective To compare the endocrine and metabolic parameters of primary ovarian insufficiency (POI) patients with menopausal women and normal control group, and to analyze the differences of the endocrine indicators, glucose and lipid metabolism indicators and other biochemical indicators between POI group, menopausal women and normal control group,in order to increase the attention to the health status of patients with POI.Methods A total of 108 patients diagnosed with POI, 114 menopausal women and 63 normal women.Sex hormones, glucose and lipid metabolism, and other biochemical indicators were compared.The error of the relevant detection kit and the batch error is less than 3% to 5%.Results There was no significant difference in body mass index (BMI) among the three groups (P>0.05).There were significant differences between follicle stimulating hormone (FSH), luteinizing hormone (LH) and estradiol (E2) (P< 0.05).The levels of total cholesterol (TC), fasting insulin (FINS) and homeostasis model assessment of insulin resistance (HOMA-IR) in the POI group were significantly higher than those of the control group (P<0.05), high density lipoprotein-cholesterol (HDL-C) and testosterone (T) levels were significantly lower than those of the control group (P<0.05).Conclusion In this study, TC, FINS, HOMA-IR and HDL-C of POI patients had reached menopausal women (P<0.05).The clinicians should intervene and treat POI patients as soon as possible.

Abstract:Objective To investigate changes in levels of lipids and lipoproteins in Chinese women during perimenopause and postmenopause as primary study endpoints, for the first time including lipoprotein(a). Methods The retrospective study was performed in 1015 women without hormone therapy aged 34-76 years from 20 provinces of China who visited the Beijing Obstetrics & Gynecology hospital. Menopausal status was defined by the criteria of the 2011 Stages of Reproductive Aging Workshop. Results Levels of total cholesterol, triglycerides and low density lipoprotein (LDL) cholesterol increased and that of high density lipoprotein (HDL) cholesterol decreased in the postmenopausal compared to the perimenopausal group. In the women with body mass index (BMI) ≥ 25 kg/m(2), total cholesterol and LDL cholesterol increased, HDL cholesterol decreased and changes in triglyceride levels were not significant. In the women with BMI < 25 kg/m(2), the increase in triglyceride levels during the transition was significant. Changes in lipoprotein(a), apolipoprotein A1 and apolipoprotein B were not significant. Comparing the groups with BMI ≥ 25 kg/m(2) vs. BMI < 25 kg/m(2), only the differences for apolipoprotein A1 and for triglycerides were significant. Triglycerides correlated positively with follicle stimulating hormone and BMI, and total cholesterol correlated positively with follicle stimulating hormone and age (all p < 0.05). Conclusions Some changes in lipids can be related to menopausal status, some to increasing age, some to both; especially triglycerides and apolipoprotein A1 were found also to be related to BMI. Surprisingly lipoprotein(a) did not change either with increasing age or during the transition despite known possible interference with estrogenic status.
