Abstract:β-site amyloid precursor protein cleaving enzyme (BACE1) represents a key target for Alzheimer's disease (AD) therapy because it is essential for producing the toxic amyloid β (Aβ) peptide that plays a crucial role in the disease's development. BACE1 inhibitors are a promising approach to reducing Aβ levels in the brain and preventing AD progression. However, systemic delivery of such inhibitors to the brain demonstrates limited efficacy because of the presence of the blood-brain barrier (BBB). Nose-to-brain (NtB) delivery has the potential to overcome this obstacle. Liposomal drug delivery systems offer several advantages over traditional methods for delivering drugs and nucleic acids from the nose to the brain. The current study aims to prepare, characterize, and evaluate in vitro liposomal forms of donepezil, memantine, BACE-1 siRNA, and their combination for possible treatment of AD via NtB delivery. All the liposomal formulations were prepared using the rotary evaporation method. Their cellular internalization, cytotoxicity, and the suppression of beta-amyloid plaque and other pro-inflammatory cytokine expressions were studied. The Calu-3 Transwell model was used as an in vitro system for mimicking the anatomical and physiological conditions of the nasal epithelium and studying the suitability of the proposed formulations for possible NtB delivery. The investigation results show that liposomes provided the effective intracellular delivery of therapeutics, the potential to overcome tight junctions in BBB, reduced beta-amyloid plaque accumulation and pro-inflammatory cytokine expression, supporting the therapeutic potential of our approach.

Abstract:The authors are advocating the "Minimum Core" architecture toward New Generation Network (NWGN). In the architecture, network functionalities in the core network (e.g. mobility management, session control and application servers) are minimized by shifting them to access networks. This approach ensures high scalability essential for future networks. This paper proposes a session control method for the "Minimum Core", in which the call-setup time is guaranteed by cooperating core and overlay networks while minimizing processing and traffic load on the core network. In this method, latency on the overlay network is estimated prior to the session being established. If the estimated latency is desirable, the overlay network is utilized and otherwise the core system. This paper also proposes a blacklist scheme, which realizes on-the-fly decisions of the latency without increasing measurement traffic on the overlay network. Evaluation results on a developed test-bed system show the proposed methods drastically suppress the probability of call-setup time exceeding the desired time, and also mitigate the processing burden of the core compared to current centralized architecture.

Abstract:Current therapies for Alzheimer's disease used in the clinic predominantly focus on reducing symptoms with limited capability to control disease progression; thus, novel drugs are urgently needed. While nanoparticles (liposomes, high-density lipoprotein-based nanoparticles) constructed with synthetic biomembranes have shown great potential in AD therapy due to their excellent biocompatibility, multifunctionality and ability to penetrate the BBB, nanoparticles derived from natural biomembranes (extracellular vesicles, cell membrane-based nanoparticles) display inherent biocompatibility, stability, homing ability and ability to penetrate the BBB, which may present a safer and more effective treatment for AD. In this paper, we reviewed the synthetic and natural biomembrane-derived nanoparticles that are used in AD therapy. The challenges associated with the clinical translation of biomembrane-derived nanoparticles and future perspectives are also discussed.
Resveratrol and Grape Extract-loaded Solid Lipid Nanoparticles for the Treatment of Alzheimer’s Disease

Abstract:The aggregation of amyloid-β peptide (Aβ) has been linked to the formation of neuritic plaques, which are pathological hallmarks of Alzheimer's disease (AD). Various natural compounds have been suggested as therapeutics for AD. Among these compounds, resveratrol has aroused great interest due to its neuroprotective characteristics. Here, we provide evidence that grape skin and grape seed extracts increase the inhibition effect on Aβ aggregation. However, after intravenous injection, resveratrol is rapidly metabolized into both glucuronic acid and sulfate conjugations of the phenolic groups in the liver and intestinal epithelial cells (within less than 2 h), which are then eliminated. In the present study, we show that solid lipid nanoparticles (SLNs) functionalized with an antibody, the anti-transferrin receptor monoclonal antibody (OX26 mAb), can work as a possible carrier to transport the extract to target the brain. Experiments on human brain-like endothelial cells show that the cellular uptake of the OX26 SLNs is substantially more efficient than that of normal SLNs and SLNs functionalized with an unspecific antibody. As a consequence, the transcytosis ability of these different SLNs is higher when functionalized with OX-26.

Abstract:Alzheimer's disease (AD) is a neurodegenerative disorder characterized by cognitive and behavioral impairment. Curcumin-loaded mesoporous silica nanoparticles (MSN-CCM) can overcome the drawbacks related to the free curcumin (CCM) clinical application, such as water insolubility and low bioavailability, besides acting over the main causes associated to AD. A thermo-responsive hydrogel is an interesting approach for facilitating the administration of the nanosystem via a nasal route, as well as for overcoming mucociliary clearance mechanisms. In light of this, MSN-CCM were dispersed in the hydrogel and evaluated through in vitro and in vivo assays. The MSNs and MSN-CCM were successfully characterized by physicochemical analysis and a high value of the CCM encapsulation efficiency (EE%, 87.70 ± 0.05) was achieved. The designed thermo-responsive hydrogel (HG) was characterized by rheology, texture profile analysis, and ex vivo mucoadhesion, showing excellent mechanical and mucoadhesive properties. Ex vivo permeation studies of MSN-CCM and HG@MSN-CCM showed high permeation values (12.46 ± 1.08 and 28.40 ± 1.88 μg cm−2 of CCM, respectively) in porcine nasal mucosa. In vivo studies performed in a streptozotocin-induced AD model confirmed that HG@MSN-CCM reverted the cognitive deficit in mice, acting as a potential formulation in the treatment of AD.

Abstract:A novel cell membrane coating is designed by hybridizing the membranes from platelets and chemokine receptor 2 overexpressing cells to cross the blood brain barrier (BBB) and target neuroinflammatory lesions. Rapamycin and 1‐trifluoromethoxyphenyl‐3‐(1‐propionylpiperidin‐4‐yl) urea (TPPU) are loaded into the liposomes to achieve a synergistic therapy. These drug‐loaded hybrid cell membrane‐coated liposomes precisely target specific lesions and ameliorate cognitive deficiency in Alzheimer's disease. The blood brain barrier (BBB) limits the application of most therapeutic drugs for neurological diseases (NDs). Hybrid cell membrane‐coated nanoparticles derived from different cell types can mimic the surface properties and functionalities of the source cells, further enhancing their targeting precision and therapeutic efficacy. Neuroinflammation has been increasingly recognized as a critical factor in the pathogenesis of various NDs, especially Alzheimer's disease (AD). In this study, a novel cell membrane coating is designed by hybridizing the membrane from platelets and chemokine (C–C motif) receptor 2 (CCR2) cells are overexpressed to cross the BBB and target neuroinflammatory lesions. Past unsuccessful endeavors in AD drug development underscore the challenge of achieving favorable outcomes when utilizing single‐mechanism drugs.Two drugs with different mechanisms of actions into liposomes are successfully loaded to realize multitargeting treatment. In a transgenic mouse model for familial AD (5xFAD), the administration of these drug‐loaded hybrid cell membrane liposomes results in a significant reduction in amyloid plaque deposition, neuroinflammation, and cognitive impairments. Collectively, the hybrid cell membrane‐coated nanomaterials offer new opportunities for precise drug delivery and disease‐specific targeting, which represent a versatile platform for targeted therapy in AD.

Abstract:The blood–brain barrier (BBB) is a protective barrier for brain safety, but it is also a major obstacle to the delivery of drugs to the cerebral parenchyma such as the hippocampus, hindering the treatment of central nervous system diseases such as Alzheimer's disease (AD). In this work, an anti-AD brain-targeted nanodrug delivery system by co-loading icariin (ICA) and tanshinone IIA (TSIIA) into Aniopep-2-modified long-circulating (Ang2-ICA/TSIIA) liposomes was developed. Low-density lipoprotein receptor-related protein-1 (LRP1) was a receptor overexpressed on the BBB. Angiopep-2, a specific ligand of LRP1, exhibited a high binding efficiency with LRP1. Additionally, ICA and TSIIA, drugs with neuroprotective effects are loaded into the liposomes, so that the liposomes not only have an effective BBB penetration effect, but also have a potential anti-AD effect. The prepared Ang2-ICA/TSIIA liposomes appeared narrow dispersity and good stability with a diameter of 110 nm, and a round morphology. Cell uptake observations, BBB models in vitro , and imaging analysis in vivo showed that Ang2-ICA/TSIIA liposomes not only penetrate the BBB through endocytosis, but also accumulate in N2a cells or brain tissue. The pharmacodynamic analysis in vivo demonstrated that Ang2-ICA/TSIIA liposomes could improve AD-like pathological features in APP/PS1 mice, including inhibiting neuroinflammation and oxidative stress, reducing apoptosis, protecting neurons, and improving cognitive function. Therefore, Ang2-ICA/TSIIA liposomes are considered a potentially effective therapeutic strategy for AD.

Abstract:The blood-brain barrier (BBB) is a barrier that maintains brain homeostasis, but it is also one of the major problems that must be overcome in the development of Alzheimer's disease (AD) drugs. To solve this problem, Salidroside (Sal) and Icariin (Ica), drugs with neuroprotective effects were loaded into liposomes, and the targeting molecule Angiopep-2 was modified on the surface of liposomes (Ang-Sal/Ica-Lip), so that the constructed nano-drug delivery system could effectively cross the BBB and exert anti-AD effects. The prepared liposomes exhibited ideal physicochemical properties. In vitro and in vivo targeting studies showed that Ang-Sal/Ica liposome could cross the BBB to increase drug accumulation in the brain, and increase the uptake of N2a cells and bEnd.3 cells. The pharmacodynamic analysis in vivo showed that Ang-Sal/Ica liposome could reverse neuronal and synaptic damage, inhibit neuroinflammation and oxidative stress and improve learning and cognitive function. Therefore, Ang-Sal/Ica liposome may be a promising therapeutic strategy for mitigating AD-related symptoms.
Intranasal Delivery of Pure Nanodrug Loaded Liposomes for Alzheimer's Disease Treatment by Efficiently Regulating Microglial Polarization

Abstract:The pure curcumin nanoparticles (CNPs) exhibit multivalent binding with amyloid‐β (Aβ) to inhibit Aβ aggregation for the removal of roadblock to microglial polarization. And CNPs‐loaded reactive oxygen species responsive cardiolipin liposomes inhibit TLR4/NF‐κB pathway for microglia polarization (M1→M2) to treat Alzheimer's disease. The activated M1‐like microglia induced neuroinflammation is the critical pathogenic event in Alzheimer's disease (AD). Microglial polarization from pro‐inflammatory M1 toward anti‐inflammatory M2 phenotype is a promising strategy. To efficiently accomplish this, amyloid‐β (Aβ) aggregates as the culprit of M1 microglia activation should be uprooted. Interestingly, this study finds out that the self‐reassembly of curcumin molecules into carrier‐free curcumin nanoparticles (CNPs) exhibits multivalent binding with Aβ to achieve higher inhibitory effect on Aβ aggregation, compared to free curcumin with monovalent effect. Based on this, the CNPs loaded cardiolipin liposomes are developed for efficient microglial polarization. After intranasal administration, the liposomes decompose to release CNPs and cardiolipin in response to AD oxidative microenvironment. The CNPs inhibit Aβ aggregation and promote Aβ phagocytosis/clearance in microglia, removing roadblock to microglial polarization. Subsequently, CNPs are endocytosed by microglia and inhibit TLR4/NF‐κB pathway for microglia polarization (M1→M2). Meanwhile, cardiolipin is identified as signaling molecule to normalize microglial dysfunction to prevent pro‐inflammatory factors release. In AD transgenic mice, neuroinflammation, Aβ burden, and memory deficits are relieved after treatment. Through combined attack by extracellularly eradicating roadblock of Aβ aggregation and intracellularly inhibiting inflammation‐related pathways, this nanotechnology assisted delivery system polarizes microglia efficiently, providing a reliable strategy in AD treatment.

Abstract:Alzheimer's disease (AD) stands as a formidable challenge in modern medicine, characterized by progressive neurodegeneration, cognitive decline, and memory impairment. Despite extensive research, effective therapeutic strategies remain elusive. The antioxidant, anti-inflammatory, and neuroprotective properties of curcumin, found in turmeric, have demonstrated promise. The poor bioavailability and rapid systemic clearance of this drug limit its clinical application. This comprehensive review explores the potential of curcumin-loaded polymeric nanomaterials as an innovative therapeutic avenue for AD. It delves into the preparation and characteristics of diverse polymeric nanomaterial platforms, including liposomes, micelles, dendrimers, and polymeric nanoparticles. Emphasis is placed on how these platforms enhance curcumin's bioavailability and enable targeted delivery to the brain, addressing critical challenges in AD treatment. Mechanistic insights reveal how these nanomaterials modulate key AD pathological processes, including amyloid-beta aggregation, tau phosphorylation, oxidative stress, and neuroinflammation. The review also highlighted the preclinical studies demonstrate reduced amyloid-beta plaques and neuroinflammation, alongside improved cognitive function, while clinical trials show promise in enhancing curcumin's bioavailability and efficacy in AD. Additionally, it addresses the challenges of clinical translation, such as regulatory issues, large-scale production, and long-term stability. By synthesizing recent advancements, this review underscores the potential of curcumin-loaded polymeric nanomaterials to offer a novel and effective therapeutic approach for AD, aiming to guide future research and development in this field.

Abstract:Alzheimer's disease (AD) is a brain disorder that causes dementia and affects millions of people worldwide. Currently, the most effective pharmacological treatments for AD focus on alleviating and temporarily reducing symptoms rather than addressing the underlying mechanisms or achieving a complete cure. Advances in investigating the responsible genes and non-coding sequences at the molecular level of AD pathophysiology have led to numerous studies exploring small interfering RNA (siRNA)-based treatment. However, many challenges hinder the translation of siRNA into clinical trials, such as their in vivo poor stability and low cellular availability due to the presence of the blood-brain barrier (BBB). One strategy to bypass these limitations is the entrapment of siRNA within a liposomal system. Liposomes (LPs) are the most often utilized lipid-based nanovehicles due to their safe profile and ability to overcome the BBB through several mechanisms such as endocytosis, fusion, lipid exchange, and paracellular transport. This work aims to highlight the use of siRNA-loaded liposomal systems in targeting AD. It explores AD's pathophysiology and causes, the benefits of siRNA in treating this neurological disorder, and how LPs can advance this strategy and address current challenges. Extracellular Vesicle Delivery of Neferine for the Attenuation of Neurodegenerative Disease Proteins and Motor Deficit in an Alzheimer’s Disease Mouse Model

Abstract:Exosomes are nano-extracellular vesicles with diameters ranging from 30 to 150 nm, which are secreted by the cell. With their role in drug cargo loading, exosomes have been applied to carry compounds across the blood–brain barrier in order to target the central nervous system (CNS). In this study, high-purity exosomes isolated by the ultra-high-speed separation method were applied as the natural compound carrier, with the loading efficiency confirmed by UHPLC-MS analysis. Through the optimization of various cargo loading methods using exosomes, this study compared the efficiency of different ways for the separation of exosomes and the exosome encapsulation of natural compounds with increasing molecular weights via extensive in vitro and in vivo efficacy studies. In a pharmacokinetic study, our data suggested that the efficiency of compound’s loading into exosomes is positively correlated to its molecular weight. However, with a molecular weight of greater than 1109 Da, the exosome-encapsulated natural compounds were not able to pass through the blood–brain barrier (BBB). In vitro cellular models confirmed that three of the selected exosome-encapsulated natural compounds—baicalin, hederagenin and neferine—could reduce the level of neurodegenerative disease mutant proteins—including huntingtin 74 (HTT74), P301L tau and A53T α-synuclein (A53T α-syn)—more effectively than the compounds alone. With the traditional pharmacological role of the herbal plant Nelumbo nucifera in mitigating anxiety, exosome-encapsulated-neferine was, for the first time, reported to improve the motor deficits of APP/PS1 (amyloid precursor protein/ presenilin1) double transgenic mice, and to reduce the level of β-amyloid (Aβ) in the brain when compared with the same concentration of neferine alone. With the current trend in advocating medicine–food homology and green healthcare, this study has provided a rationale from in vitro to in vivo for the encapsulation of natural compounds using exosomes for the targeting of BBB permeability and neurodegenerative diseases in the future.

Role play – an eff ective tool to teach clinical medicine

Abstract:Objective: This study was undertaken as an experimental effort to introduce role-play as a learning tool in teaching of clinical medicine, access its efficacy in understanding clinical scenarios and honing linguistic and examination skills through expression in medical students. The basic hypothesis was that, role plays can guide the medical students in a better way by gaining knowledge and attitude to deal with real life situations through simulation acts so that they can apply this knowledge and skill gained through these simulations in practical examinations as well as self application in clinical practice. Methods: This experimental study with a single group pre and post test design was carried out in DMIMS university (NAAC-A) and JN Medical college, Wardha. Twenty four final year MBBS students were selected for the study. All were subjected to a pre sensitization test in short case format covering affective, cognitive and psychomotor domains from cardiovascular, respiratory, and gastrointestinal system and marks were recorded. Then the students were sensitized with role plays of common clinical scenarios and all the role-plays were enacted. Finally post sensitization test in the same short case format was conducted.Statistical method: students paired ‘t’ test. Abstract:Alzheimer's disease (AD) is a global health crisis, one of the most complex neurodegenerative diseases that significantly affects populations worldwide without any age specificity. The development of AD is associated with the generation of intracellular amyloid β (Aβ) plaques and aggregation of tau proteins, leading to hyperphosphorylated microtubules, which cluster to form insoluble neurofibrillary tangles. Lipid nanoparticles (LNPs) have emerged as a promising tool in drug delivery, particularly for AD treatment. LNPs, as ideal nanocarriers, can encapsulate various therapeutic agents, such as peptides, drugs, and small molecules, all potential candidates for AD treatment. LNPs offer stability to encapsulated drugs, shielding them from degradation and premature release. Most importantly, LNPs can breach the blood-brain barrier (BBB), enabling targeted drug delivery to the central nervous system. In this review, we provide a comprehensive overview of various categories of drug-loaded LNPs that have been studied for AD treatment, including liposomes, niosomes, transferosomes, ethosomes, solid lipid nanoparticles, and nanostructured lipid carriers. We also emphasize the need for developing LNPs to facilitate the delivery of AD therapeutics across the BBB.

Abstract:Alzheimer's disease (AD) is a neurodegenerative disease that mainly occurs in old age and involves progressive cognitive impairment. AD has become a major global issue for public health, with approximately 24 million people currently affected by the disease. Estimates indicted that this number will quadruple by 2050. Because of the high incidence of AD, there is an urgent need to develop new strategies to diagnose and treat AD. Many recent studies have indicated the multiple, yet somewhat controversial, roles of exosomes in AD. Although the underlying mechanisms by which exosomes play a role in AD are still unknown, current evidence suggests that exosomes can carry and spread toxic amyloid-beta, and hyperphosphorylated tau, between cells, and then induce apoptosis, thus contributing to the loss of neurons. In addition, exosomes appear to possess the ability to reduce brain amyloid-beta, and tau hyperphosphorylation, and transfer neuroprotective substances between neural cells. The accumulating data brings hope that the application of exosomes may be helpful for early diagnostics and the identification of new therapeutic targets for AD. Here, we summarized the various roles of exosomes, and how they might relate to the pathogenesis of AD. We also highlight the potential application of exosomes as a therapeutic option in AD therapy.

Abstract:The management of Alzheimer's disease (AD) remains challenging owing to its complex pathophysiology and blood-brain barrier (BBB) restrictions. A rationally designed combination drug multi-target strategy has emerged as a promising approach for AD management. Considering the lipophilic nature of BBB and the need to embed dual drugs- Rivastigmine hydrogen tartrate and Resveratrol, nanostructured lipid carriers (NLCs) were used as drug delivery vehicle. NLCs were formulated by water/oil/water (w/o/w) double emulsion method and optimized using central composite design. HPLC method for simultaneous determination of drugs was developed and validated. To target the overexpressed Transferrin (TF) receptors, NLCs were surface functionalized with TF by covalent coupling employing carbodiimide chemistry. The success of conjugation was ascertained by Zeta potential, FTIR and TEM and the Bradford assay which estimated the conjugation efficiency of 47.52 ± 3.2%. A particle size of <100 nm was obtained which is favourable for brain delivery. Sustained release of drugs from nanoparticles was obtained, where TF-NLCs demonstrated a significantly slow release compared to NLCs. The functional stability and biocompatibility of the nanosystems was ensured by DPPH assay and hemolysis test, respectively. The confocal laser scanning microscopy (CLSM) revealed significantly higher (∼1.7 folds) brain uptake of TF-NLCs compared to NLCs in vivo following intraperitoneal administration. Dual drugs loaded TF functionalized NLCs demonstrated favourable in vitro and in vivo attributes for efficient brain targeting and hence AD management.
