Abstract:Objective To investigate whether initial intervention based on WBC measured within 2 hours postoperatively can reverse the uroseptic shock induced by UUTEL.Methods From May,2015 to July 2015,24 female New Zealand rabbits,weighing 2.0-2.5 kg,born 2-3 months,used as uroseptic shock model.Their ureters were ligated and followed by injection of Escherichia coli solution into the renal pelvis.Then,those animals were randomly assigned into control group(the first group)and experimental groups (the second group,the third group).Each group had 8 rabbits initially.The first group did not receive sensitive antibiotic or fluid resuscitation.The second group received imipenem and cilastatin sodium 15mg/kg and normal saline 5 ml/kg 2 hours postoperatively.The third group received sensitive antibiotic and fluid resuscitation the same dosage as the second group 6 hours postoperatively.Mean arterial pressure(MAP) was recorded for 10 hours and survival rate of all groups for 72h postoperatively was recorded..The clinical data of 46 patients whose WBC count less than 2.85 × 109/L within two hours after UUTEL were analyzed retrospectively.These patients were divided into two groups based on the time of intervention.Group A including 19 patients received routine antibiotic,fluid therapy,low-dose corticosteroids when there was symptom of shock.Group B including 27 patients immediately received resuscitation bundle protocol when there was a drastic decrease in WBC.The incidence of septic shock,the rate of intubation,length of stay in ICU,length of stay in hospital postoperatively,hospitalization cost and survival rate of these two groups were compared.Results All 8 rabbits of the first group died within 72 h,postoperatively and the median time of survival was 11 h.None of the second group rabbits developed shock and all rabbits survived 72 h after operation.6/8 rabbits of the third group survived 72 h after operation.15 patients in Group A and 3 patients in Group B experienced acute uroseptic shock (P ＜ 0.05).11 patients in Group A and one patients in Group B underwent tracheal intubation (P ＜ 0.05).18 patients in Group A and 6 patients in Group B were transferred to ICU(P ＜ 0.05),and their length of stay in ICU was (10.8 ± 5.4) d and (7.5 ± 2.8) d,respectively(P ＞ 0.05).The length of stay in hospital and hospitalization cost of Group A and B were (19 ± 9.8)d vs.(7 ±4.7)d(P ＜0.05),(94 583 ±51 623) RMB vs.(35 389 ± 16 342) RMB respectively (P ＜ 0.05).One patient in Group A died due to acute uroseptic shock and none of Group B died.Conclusions Our animal model and clinical cohort study showed that initial intervention based on WBC mneasured within 2 hours postoperatively can reverse the uroseptic shock induced by UUTEL and improve the prognosis.
Distribution of Uranium in Rata Inhaled with Depleted Uranium Aerosols

Abstract:Objective To investigate the distribution of uranium in rats after inhalation with depleted uranium aerosols. Methods The depleted uranium aerosols were inhaled by Wistar rats. At 30, 90, 180, 270, 360, and 540 d after inhalation, the rata were sacrificed and tissue samples were collected. The contents of uranium in lung, kidney, liver, heart, brain, thighbone, spleen and thymus were measured by laser time-dependent spectroscopy analysis. Resulits The uranium contents of lung increased in the high-dosc and low-dose groups [(499833.3 ± 14214.8) ng/g and (25 424.0 ± 6193.4)ng/g, respectively] after inhalation, and significantly differed from the control (28.8 ± 13.9)ng/g, (P < 0.05).At 30 d after inhalation, the contents of uranium in lung, kidney and thighbone were higher than those of control, and then decreased time-dependently. At 60 d, the contents of uranium in liver, heart, brain, spleen and thymus were higher than those of control. Curve of the eontenta were biphasie, whieh went up first, reached at peak value and then went down. The contents of uranium were high in lung, thighbone, brain and thymus. Conclusions After inhalation of depleted uranium aerosols, lung and thighbone are the primary reservoirs for uranium redistributed, and accumulations in brain and thymus suggest other two organs for unanticipated injury by depleted uranium.
Renal Dysfunction Induced by Long-Term Exposure to Depleted Uranium in Rats

Abstract:Depleted uranium (DU) is a kind of radioactive heavy metal which can enter into the body via inhalation (aerosols), ingestion (drinking and eating) and wounds (embedded), and causes chemical and/or radiation-induced toxicities. In this study, male Sprague Dawley rats were surgically implanted in gastrocnemius muscle with DU fragments at three dose levels (low-dose, medium-dose and high-dose), with biologically inert tantalum (Ta) fragments served as controls. At 1 day, 7 days, and 3, 6, and 12 months after implantation, the rats were euthanized and tissue samples were collected, and uranium levels were measured in a variety of tissues by inductively coupled plasma-mass spectrometry (ICP-MS) to analyze the dynamic changes and distribution of uranium in rats. Thereafter, at 3, 6 and 12 months after implantation, the rats were euthanized after the collection of 24 h urine, blood and kidney samples were collected for analysis of DU-induced renal histopathologic changes and renal dysfunction. It was observed that DU concentrations in all the DU implanted groups were higher than that in Ta control group, and DU concentrations in the kidney increased with the implanted times, peaked at the 90 days after implantation, with a high correlation to the implanted DU doses, and then began to decrease gradually and slowly, and the DU concentrations in kidney still maintained at a relatively high level even at the 360 days after implantation. Otherwise, chronic DU contamination could induce the pathological changes of renal glomeruli, tubules and mesenchyme, such as interstitial fibrosis, enlarged interstice of renal tubular epithelial cells, tumefactions and necrosis of epithelial cells, shrinkage and disappearance of cavity of Bowman's capsule. By TEM, it was shown that the basement membrane of glomerulus was incrassated, mitochondrial of kidney proximal tubule had visible tumefaction and became bigger, and the mitochondrial cristae became shorter and disorderly in alignment. Compared to the control group, it was found that there was significant increase in the DU implantation group in terms of their blood urea nitrogen (BUN) and serum creatinine, urinary beta 2-microglobulin (beta 2-MG) and albumin, with a high correlation to the implanted DU dosage and periods. It was concluded that DU could accumulate in kidneys for a long period, and causes kidney injury by the toxic chemical/radioactive action such as renal dysfunction and structural damage.
Pulmonary immune response of dogs after exposure to 239PuO2

Abstract:This study evaluated the cell-mediated (CMI) and humoral immune responses in four Beagle dogs five to six years after single inhalation exposures to different monodisperse 239PuO2 aerosols (0.72-1.4 microns activity median aerodynamic diameter). These exposures resulted in initial lung burdens ranging from 19 to 35 kBq. Four nonexposed dogs were used as age-matched controls. Anesthetized dogs were immunized by instillation of sheep red blood cells (SRBC) into selected lung lobes. Cells and fluids were obtained serially from blood samples and by bronchoalveolar lavage of the saline- and SRBC-treated lung lobes at 5-20 days after immunization. The CMI response evaluated by the leukocyte procoagulant activity test was similar in the saline- and SRBC-treated lobes of both groups of dogs. The humoral immune response was measured by the enzyme-linked immunosorbent assay. No differences were shown between the amount of antibody measured in the sera or lung lavages from control or Pu-exposed dogs. Histopathology of the tracheobronchial lymph nodes from the Pu-exposed dogs showed them to be fibrotic with no lymphoid cells, suggesting that these tissues could not respond to the antigen deposited in the lungs. However, both mediastinal and sternal lymph nodes did contain lymphoid tissue, and were likely to be the lymphoid tissues that produced the immunity to the antigen deposited in the lungs of the exposed dogs. Although both exposed and control dogs produced immune responses to the antigen instilled into their lungs, differences were observed in the number of neutrophils in lung lavages from the control and exposed animals. There was a dramatic influx of neutrophils into both the saline- and SRBC-treated lung lobes of the Pu-exposed dogs that was not seen in the age-matched controls. This suggests that the inhaled 239PuO2 produced chronically-active inflammation in the lung which may contribute to recruitment of lymphocytes to the lung following intrapulmonary deposition of antigen. In conclusion, the immune responses induced by lung immunization of dogs that had inhaled 239PuO2 were not suppressed by large doses of chronic alpha irradiation of the lungs and tracheobronchial lymph nodes, indicating that local pulmonary immune responses are preserved despite severe radiation-induced alteration of these tissues.
Biokinetics of inhaled 239PuO2 in the beagle dog: effect of aerosol particle size

Abstract:This study was designed to measure the effect of aerosol particle size on the deposition, retention, excretion and translocation of 239Pu inhaled as the dioxide by Beagle dogs. To address these questions, young adult male and female Beagle dogs received single brief inhalation exposures to one of three monodisperse aerosols of 239PuO2 having sizes of 0.72, 1.4 or 2.8 microns activity median aerodynamic diameter (AMAD). Periodic collections of urine and faeces were made for each dog until sacrifice at times ranging from 4 hours to 2 years after exposure. The results indicate long term retention of a substantial percentage of the initial pulmonary burden (IPB), and that the retention was affected by particle size. The percentage of the initial pulmonary lung burden retained in the long term component and its effective half time (TE) were 90 per cent with TE = 680 days for the 0.72 micron AMAD aerosol, 68 per cent with TE = 1400 days for the 1.4 microns AMAD aerosol and 82 per cent with TE = 1800 days for the 2.8 microns AMAD aerosol. The major route of elimination of 239Pu from lung was via the faeces, but significant amounts were also translocated to thoracic lymph nodes (approximately 15 per cent IPB by 2 years). Small amounts were translocated to liver (0.2 per cent IPB) and skeleton (0.1 per cent IPB) by 2 years after exposure. The average alpha-radiation dose to the lung was projected to be twice as large for the 2.8 microns AMAD group as for the 0.72 micron AMAD group at 10 years after exposure.

Abstract:Comparative Toxicity of Standard Nickel and Ultrafine Nickel in Lung after Intratracheal Instillation: Qunwei ZHANG, et al.; Department of Environmental Health, School of Medicine, Fukui Medical University-A comparison was made of the bronchoalveolar lavage fluid (BALF) response to ultrafine nickel (Uf-Ni) and standard-sized nickel (Std-Ni). Rats were intratracheally instilled with 0, 0.1, 0.5, 1 and 5 mg Uf-Ni and Std-Ni, respectively. At 3 d after instillation, the body weight and wet lung weight were determined. At the same time, BALF was analyzed for lactate dehydrogenase (LDH), total protein (TP), tumor necrosis factor-alpha (TNF-alpha), and total cell and differential cell counts. The results showed that indicators of lung injury and inflammation in BALF were markedly raised with increased Uf-Ni and Std-Ni for each from 0 to 1 mg, and there were no differences in the indices between instillation of Uf-Ni at 1 mg and 5 mg. The results also showed that the effects of Uf-Ni on the indices were significantly higher than those of Std-Ni. Additional groups of rats were intratracheally instilled with 1 mg of Uf-Ni or Std-Ni, and wet lung weight and BALF profiles were analyzed at 1, 3, 7, 15 and 30 d later. The effect of Uf-Ni and Std-Ni on indices that can be presumed to reflect epithelial injury and permeability (LDH or TP), and release of proinflammatory cytokine (TNF-alpha) were increased throughout the 30 d post-exposure and the effects of Uf-Ni on these indices were significantly higher than those of Std-Ni from 1 to 30 d after instillation. Moreover, the number of neutrophils and LDH activity in BALF of rats after exposure to Uf-Ni were significantly greater than those of Std-Ni-exposed rats up to 30 d after instillation. Our findings suggest that Uf-Ni has a much more toxic effect on the lung than St-Ni, but the mechanism remains to be elucidated.

Abstract:Purpose Depleted uranium (DU) is a kind of radioactive heavy metal which can enter the body by inhaling (aerosols), drinking, eating and through the wounds and then cause long time chemical and/or radioactive toxicity on critical organs. In the present study, the DU retention regulation, biological characters and mechanism of DU damage to bone after implantation were analyzed in order to clarify the potential health risks associated with chronic exposure to DU. Methods Sprague Dawley rats were surgically implanted in gastrocnemius muscle with DU pellets at 3 radio active dose levels (low, medium and high), biologically inert tantalum(Ta) pellets served as a negative control. Various times after implantation, uranium levels were measured in a variety of tissues using inductive coupled plasma-mass spectrometry, (ICP-MS) to analyze the distribution and retention rules of DU. At 90d after DU implantation, the bone mineral density(BMD)and bone biomechanics were analyzed, and the pathological changes were observed by light and electron microscope and measured by bone histomorphometry to investigate the dose-response relationship between DU exposure and bone metabolic changes and structure impairment. Results DU concentrations in all the DU implanted groups were higher than that in Ta control group, and DU concentrations in bone increased with the implanted times, peaked at the 90th day after implantation, with a high correlation to the implanted DU doses, and then began to decrease gradually but slowly, and the DU concentrations in bone still maintained at a relatively high level even at the 360th day after implantation. At the 90th day after implantation, the BMD in DU implanted animals was significantly lower than that of the control group, and with a high correlation to the implanted DU doses. In addition, the lumbar compressive loads and femoral three-point bending test in DU implanted rats were significantly declined compared with the control group, and alterations of the trabecular bone structure were observed in DU implanted rats, such as the trabecular area, trabecular thickness and mineral apposition rate were decreased to some extent and the trabecular separation increased. Conclusions DU can accumulate in the bone for a long periods, and cause bone damage by toxic chemical/radiation actions such as disturbance of bone metabolism, destruction of bone microarchitecture, loss in bone mass and biomechanical properties. Therefore, more attention should be paid to the damaged effects of chronic DU contamination to bone.
[Study of Optimization of Whole Lung Lavage Applied to Pneumoconiosis].

Abstract:OBJECTIVE:To observe and evaluate the performances of intermittent positive pressure ventilation, beta-2 adrenergic receptor agonist, and pressure lavage in promoting residual fluid absorption and improving blood oxygen saturation during massive whole lung lavage (WLL). METHODS:A total of 155 patients were randomly divided into pressure ventilation (PV) group (n = 28), adrenaline (Ad) group (n = 31), PV plus Ad group (n = 29), pressure infusion bag (PIB) group (n = 30), and control group (n = 32). The patients underwent staged MWLL of bilateral lungs. The blood oxygen saturation (SpO2) of arterial blood of finger, chest X-ray findings, clinical symptoms, and lung functions were observed before and after MWLL. RESULTS:There were no significant differences in change in clinical symptoms among the five groups after MWLL (P > 0.05). The Ad group showed 6.3% increase in forced vital capacity (FVC) and 10.9% increase in forced expiratory flow at 25% of vital capacity (FEF(25%)) after MWLL (P < 0.05). The control group showed 5.7% decrease in FVC, 10.9% increase in forced expiratory volume in one second (FEV(1.0)), and 12.0% increase in FEF(25%) after MWLL (P < 0.05). No significant difference was found in other groups (P > 0.05). During and after MWLL, the incidence rates of hypoxemia in PV group, PV plus Ad group, and control group were 0, 0, and 12.5% (8/64), respectively (P < 0.01). There were no significant differences in total amount of lavage fluid and amount of residual fluid in the lung among all groups (P > 0.05). The smallest difference between the optical densities of the two lung fields on chest x-ray at 3 h after WLL was 0.152 ± 0.053 in the PV plus Ad group, compared to 0.194 ± 0.074 in the PV group, 0.197 ± 0.054 in the PIB group, 0.214 ± 0.054 in the Ad group, and 0.241 ± 0.109 in the control group, with significant differences between the saline group and other groups except Ad group (P < 0.05). CONCLUSION:Pressure ventilation, adrenaline, and pressure lavage can promote the transportation and absorption of residual fluid in the lung and decrease the incidence of hypoxemia during WLL.
Retention of inhaled 238PuO2 in beagles: a mechanistic approach to description

Abstract:Groups of Beagle dogs received inhalation exposure to one of three sizes of monodisperse or a polydisperse aerosol of 238PuO2. Periodic sacrifice of pairs of dogs through 4 yr after inhalation exposure provided data on the retention, translocation and mode of excretion of 238Pu. Fragmentation of 238PuO2 particles deposited in lung led to enhanced dissolution, translocation and excretion of 238Pu compared to previous studies using 239PuO2. A mechanistic simulation model was formulated to account for particle dissolution and fragmentation. This model provided adequate prediction of the time course of lung retention, uptake and retention in other organs and excretion. Predictions of the model provided more accurate description of the data than the Task Group on Lung Dynamics model incorporated in the ICRP Publication 30. The mechanistic model provided estimates of cumulative radiation dose to liver and bone that are a factor of five greater and to lung that are a factor of two less than predicted by the Task Group Lung Model.
Biased thinking assessed by external observers in borderline personality disorder.

Abstract:OBJECTIVES Biased thinking (to some extent overlapping with the concepts of cognitive distortions and cognitive errors) is a key concept in cognitive therapy of borderline personality disorder (BPD). Specific contents and cognitive processes related to BPD functioning are known. However, most studies are based on self-report measures which present a number of important limitations, in particular the difficulty in assessing non-conscious processes infused by affect. So far, no studies were conducted using valid observer-rated methodology addressing the question of biased thinking in BPD as it unfolds spontaneously in session. DESIGN This is a controlled interview study comparing two matched groups, BPD patients and healthy controls. METHODS A total of N= 25 clinical dynamic interviews with patients presenting with BPD were transcribed and rated using the Cognitive Errors Rating Scale (Drapeau, Perry, & Dunkley, 2008); their cognitive profiles were compared to those of N= 25 healthy controls who underwent the same procedure. RESULTS Overall, results indicated that no between-group difference in the frequency of specific biases was found. However, heightened levels of negative cognitive biases, in particular over-generalizing and fortune-telling, were associated with BPD. Furthermore, negative over-generalizing was associated with the number of BPD symptoms. CONCLUSIONS These results have high levels of ecological validity and are promising for the refinement of cognitive theory of BPD. Clinical implications for assessment and intervention are discussed.
Risk factors for spontaneous abortion: a case-control study in France.

Abstract:A case-control study was conducted in seven maternity hospitals in the Paris area in 1988 to evaluate the role of several risk factors in spontaneous abortion. A total of 279 cases and 279 controls were compared for socio-demographic characteristics, reproductive history and for conditions of conception. Prior fetal losses [odds ratio (OR) = 2.30 for n greater than or equal to 2; 95% confidence interval (CI) = 1.17-4.61] and maternal age at pregnancy (greater than 30 years) appeared to be major and independent risk factors of spontaneous abortion. Other factors associated with an increased risk of fetal loss were: geographical or ethnic origin (OR = 2.85 for North African women; 95% CI = 1.58-5.10); psychological problems at the time of conception, either related to the outcome of the pregnancy (OR = 3.08; 95% CI = 0.92-10.25) or unrelated to this outcome (OR = 3.35; 95% CI = 1.41-8.00). The following factors were not associated with spontaneous abortion: gravidity, parity, prior induced abortion, prior sexually transmitted diseases and Chlamydia trachomatis serology, menstrual cycle abnormalities, induced conception cycle and in-vitro fertilization, cigarette smoking, current or past use of combined oestrogen/progestagen pill or intrauterine device. These findings confirm the importance of two risk factors for fetal loss: maternal age and number of prior spontaneous abortions. Two risk factors, ethnic origin and psychological problems at the time of conception are also identified, which require further study.

Abstract:In China, whole lung lavage slowed the rate of lung function decline at one-, two- and four-year follow up in men with pneumoconiosis when compared to their co-workers. Despite recognized weaknesses in the presentation of the information, we conclude that this procedure may show promise in altering the natural history of pneumoconiosis. Objective This study aimed to evaluate the role of whole lung lavage (WLL) in the treatment of pneumoconiosis and compare changes in lung function over time in treated and untreated miners. Methods We systematically reviewed and identified eight controlled studies with treated and comparison groups with lung function tests before WLL and a year or more later. Two hundred ninety-two patients were included in our meta-analysis. Results Studies consistently showed a slowing of the rate of lung function decline with WLL at 1, 2, and 4 years. In some reports, details of the population under study, reliability of lung function tests, the adequacy of matching, technical aspects of the procedure, and adverse effects associated with WLL were not available. Conclusions Despite recognized weaknesses in the presentation of information, this procedure may show promise in altering the natural history of pneumoconiosis.

Abstract:Obesity is associated with chronic low-grade inflammation. Thus, at metabolically relevant sites, including adipose tissue and muscle, there is abnormal production of proinflammatory cytokines such as TNF-alpha. Here we demonstrate that eNOS expression was reduced, with a concomitant reduction of mitochondrial biogenesis and function, in white and brown adipose tissue and in the soleus muscle of 3 different animal models of obesity. The genetic deletion of TNF receptor 1 in obese mice restored eNOS expression and mitochondrial biogenesis in fat and muscle; this was associated with less body weight gain than in obese wild-type controls. Furthermore, TNF-alpha downregulated eNOS expression and mitochondrial biogenesis in cultured white and brown adipocytes and muscle satellite cells of mice. The NO donors DETA-NO and SNAP prevented the reduction of mitochondrial biogenesis observed with TNF-alpha. Our findings demonstrate that TNF-alpha impairs mitochondrial biogenesis and function in different tissues of obese rodents by downregulating eNOS expression and suggest a novel pathophysiological process that sustains obesity.

Abstract:We examined the effect of postexposure hyperpnea on clearance of particles deposited in the lungs of adult male beagle dogs. Sedated dogs inhaled an insoluble 67Gg2O3 (T 1/2 = 78 h) aerosol (0.12 micron AMD) for one half hour on three separate occasions. Following aerosol exposures 1 and 2, dogs were assigned to either an eupneic (EUP) or hyperpneic (HYP) group and clearance was followed noninvasively for 9 days by whole body counting and gamma camera analysis. After exposure 2, EUP and HYP assignments were interchanged so that each dog was studied under both conditions. Hyperpnea was induced by housing dogs in an atmosphere of 7% CO2 in air, 6 h per day. Carbon dioxide inhalation increased VE a factor of 3.7 +/- 0.9 (SD). We found that pulmonary clearance was retarded by CO2-stimulated hyperpnea in 7 of 8 dogs. Following aerosol exposure 3, dogs were divided between EUP and HYP and subsequently were killed in pairs at 3 h, 1 day, 3 days, and 7 days. Distribution of activity in body organs was examined, and translocation to the hilar lymph nodes was followed. Both retention distribution and removal of activity by saline lavage were measured in postmortem lungs. The percentage of lavaged activity associated with pulmonary macrophages increased from 44% at 3 h after exposure to 91% at 4 and 7 days after exposure. Examination of dried lung slices by autoradiography showed clearance of particles from airways and formation of a more punctate distribution in the retained activity at increasing times after exposure. Abstract:OBJECTIVE:To observe the effects of three dosages of nebulized unfractionated heparin (UFH) on alveolar coagulation, inflammation and lung histology in endotoxin-induced acute lung injury rat model, and investigate the appropriated dose of local UFH in managing intrapulmonary coagulopathy.METHODS:Twenty-nine male Wistar rats were divided into control (n=5) and UFH group (n=24) in table of random number, which were duplicated to be endotoxin-induced ALI rat model with lipopolysaccharide (LPS) injecting by intravenous route. The UFH group was divided into three subgroups, which were administered once with 6, 12 and 18 U/g aerosolized UFH in 10 ml at 2 hours after challenge, respectively, while the control group was simply nebulized with normal saline. All rats were sacrificed at 6 hours after intravenous administration of LPS, bronchoalveolar lavage was performed, and the fluid was collected. Enzyme-linked immune sorbent assay (ELISA) was used to measure the level of thrombin-antithrombin complex (TATc), tumor necrosis factor-α (TNF-α) in bronchoalveolar lavage fluid (BALF), and lung wet/dry (W/D) weight ratio, histology score were recorded.RESULTS:At 6 hours after LPS-induced lung injury, the levels of TATc and TNF-α, lung W/D weight ratio and histology score in 6 U/g and 12 U/g group were all lower than those of control group significantly (TATc: 0.959±0.681 μg/L, 1.165±0.854 μg/L vs. 2.141±0.791 μg/L, TNF-α: 4.449±5.054 ng/L, 9.096±4.099 ng/L vs. 18.184±3.869 ng/L, W/D weight ratio: 7.018±1.137, 7.367±0.349 vs. 8.472±0.614, histology score: 16.0±1.0, 16.5±1.5 vs. 19.6±0.4, P<0.05 or P<0.01). There was no significant difference in the comparisons between the subgroups of UFH in TATc level in BALF and lung histology score. For the TNF-αlevel in BALF, 18 U/g group evidently exceeded that of 6 U/g group (15.503±8.753 ng/L vs. 4.449±5.054 ng/L, P<0.01), and lung W/D weight ratio in 18 U/g group was also significantly higher comparing to 6 U/g (8.850±1.157 vs. 7.018±1.137, P<0.05) and 12 U/g group (8.850±1.157 vs. 7.367±0.349, P<0.05).CONCLUSION:It was appropriate for the dose of nebulized UFH to be administered no more than 12 U/g in ALI treatment, which was enough to inhibit alveolar coagulant cascade, decrease early inflammatory response and alleviate lung tissue injury.

Abstract:Abstract Background Pneumoconiosis is a diffuse interstitial fibronodular lung disease, which is caused by the inhalation of crystalline silica. Whole lung lavage (WLL) is a therapeutic procedure used to treat pneumoconiosis. This study is to compare the effects of different negative pressure suction on lung injury in patients with pneumoconiosis undergoing WLL. Materials and methods A prospective study was conducted with 24 consecutively pneumoconiosis patients who underwent WLL from March 2020 to July 2020 at Emergency General Hospital, China. The patients were divided into two groups: high negative suction pressure group (group H, n = 13, negative suction pressure of 300–400 mmHg) and low negative suction pressure group (group L, n = 11, negative suction pressure of 40–50 mmHg). The arterial blood gas, lung function, lavage data, oxidative stress, and inflammatory responses to access lung injury were monitored. Results Compared with those of group H, the right and left lung residual were significantly increased in the group L ( P = 0.04, P = 0.01). Potential of hydrogen (pH), arterial partial pressure of oxygen (PaO 2 ), arterial partial pressure of carbon dioxide (PaCO 2 ), lactic acid (LAC) and glucose (GLU) varied from point to point in time ( P < 0.01, respectively ). There was statistical difference in the trend of superoxide dismutase (SOD) and interleukin-10 (IL-10) over time between the two groups ( P < 0.01, P = 0.02). In comparison with the group H, the levels of IL-10 ( P = 0.01) and SOD ( P < 0.01) in WLL fluid were significantly increased in the group L. There was no statistical difference in the trend of maximal volumtary ventilation (MVV), forced vital capacity (FVC), forced expiratory volume in one second (FEV1%), residual volume (RV), residual volume/total lung capacity (RV/TLC), carbon monoxide dispersion factor (DLCO%), forced expiratory volume in one second/ forced vital capacity (FEV1/FVC%) over time between the two groups ( P > 0.05, respectively ). Conclusion Low negative suction pressure has the potential benefit to reduce lung injury in patients with pneumoconiosis undergoing WLL, although it can lead to increased residual lavage fluid. Despite differing suction strategies, pulmonary function parameters including FEV1%, RV and DLCO% became worse than before WLL. Abstract:Objective To evaluate the significance of inhalation of pulmicort respules combined with vantorin in the process of pneumoconiosis massive whole-lung lavage.Methods 90 patients with pneumoconiosis treated with unilateral stage high-capacity whole lung lavage were selected and evenly divided into the whole-lung lavage(WLL) group and the treatment group randomly.The treatment group was also administrated by inhalation of pulmicort combined vantorin from three days before WLL to three days after WLL.Then the therapeutic effect of the two groups was evaluated by WLL complications,the remanent fluids,lung function test and the improvement of clinical symptoms.Results The occurrence of intraoperative airway spasm,hypoxemia,airway pressure increased were lower in the treatment group than that in the WLL group(P 0.01).Lung function and clinical symptoms were better improved in the two groups after the treatment,and the improvement was more pronounced in the treatment group than in the WLL group(P 0.05).Conclusion The inhalation of pulmicort respules combined with vantorin can reduce complications and remanent fluids,improve lavage effect and accelerate their postoperative recovery in the treatment of patients with pneumoconioses treated with massive whole lung lavage.
Effects of bronchoalveolar lavage on refractory Mycoplasma pneumoniae pneumonia.

Abstract:INTRODUCTION: This study prospectively evaluated the effect of early bronchoalveolar lavage (BAL) on refractory Mycoplasma pneumoniae pneumonia with radiologically proven large pulmonary lesions in children. METHODS: A total of 35 children diagnosed as having refractory M. pneumoniae pneumonia with radiologically proven large pulmonary lesions completed the study. According to the time point of BAL, they were divided into 2 groups. In the early BAL group, BAL was performed within 3 d after admission (n = 22); in the late BAL group, BAL was performed 3 d later after admission (n = 13). Clinical effects were compared between these 2 groups. RESULTS: After BAL therapy, improvement in clinical symptoms and laboratory findings as well as resolution of pulmonary lesions on radiography were obtained in all subjects. The median fever duration after admission was 4 (2-7) d and 5 (2-10) d (P < .05), and the median hospitalization duration was 7 (5-10) d and 10 (5-14) d (P < .05), respectively, in the early BAL group and the late BAL group. Approximately 7 d after admission, 67% of subjects in the early BAL group showed resolution of atelectasis on x-ray image versus a corresponding rate of 14% in the late BAL group (P < .05). Furthermore, laboratory indices recovered quicker in the early BAL group than in the late BAL group (P < .01). CONCLUSIONS: BAL appeared to be efficacious and well-tolerated treatment for refractory M. pneumoniae pneumonia with radiologically proven large pulmonary lesions, and early application of BAL may be more beneficial.
