Abstract:Aims: The study aims were to determine autoantibodies associated with type 1 diabetes (T1D), celiac disease (CD) and autoimmune thyroid disease (AITD) in individuals living with type 2 diabetes (T2D) compared to T1D and matched controls. Methods: Individuals with T1D and T2D were randomly identified in health-care registers. Blood was collected through home-capillary sampling and autoantibodies associated with either T1D against glutamic acid decarboxylase (GADA), insulin (IAA), insulinoma antigen-2 (IA-2A), and zinc transporter 8 (ZnT8A), CD against tissue transglutaminase (tTGA) or AITD against thyroid peroxidase (TPOA) were determined in an automated, multiplex Antibody Detection by Agglutination-PCR (ADAP) assay. Results: GADA were detected in 46 % (88/191) of T1D and increased to 6.2 % (23/372) in T2D compared to 2.6 % (7/259) of controls (p = 0.0367). tTGA was low (1.1-2.6 %) and not different in between the study cohorts, nonetheless, in T1D tTGA was associated to islet autoantibodies. TPOA was more frequent in T1D, 27.1 % (53/191), compared to either T2D, 14.8 % (55/372; p = 0.0002) or controls, 14.3 % (37/259) (p = 0.0004). Overall, TPOA was more frequent in GADA positive (34.8 %; 8/23) than negative (13.5 %; 47/349; p = 0.0053) T2D individuals. Conclusion: It's suggested that analyzing GADA and TPOA may refine the autoimmune landscape in individuals clinically classified as T2D.

Abstract:To study the changes of pancreas, thyroid, adrenal, parathyroid and gonadal organ-specific antibodies in patients with type 1 diabetic patients and to explore the risk of development to other endocrine gland autoimmune diseases.Fifty one patients with type 1 diabetes mellitus were selected. ELISA was used to detect islet, adrenal gland, Parathyroid, gonadal organ-specific antibody levels, the level of thyroid-related antibodies by lectrochemiluminescence.Compared with the healthy control group, the levels of the 17-α-OHAb, 21-OHAb, NALP5Ab, P450sccAb, and CaSRAb in the T1DM group were significantly higher. GADAb-positive patients were more likely to have TPOAb-positive patients than GADAb-negative patients, and the positive rate of 2 thyroid antibodies in GADAb-positive patients was significantly higher than that in GADAb-negative patients. The presence of these antibodies is related to the age of onset of type 1 diabetes or Patient age. In combination with 1 or 2 islet antibody-positive patients, the combined non-islet antibody positive rate was higher than that of islet antibody-negative patients.Patients with type 1 diabetes with other autoimmune diseases at risk significantly increased compared with normal, of which the most common thyroid autoimmune disease, thyroid antibodies and hormone levels should be routinely detected at the first visit and long-term follow-up.

Abstract:Background: An understanding of the current status of patients with type 1 diabetes mellitus (T1DM) can help to provide appropriate treatment. Methods: This was a retrospective analysis of the DIabetes Versorgungs-Evaluation (DIVE) and the Diabetes-Patienten-Verlaufsdokumentation (DPV) databases for Germany. Results: The analysis included 56,250 people with T1DM (54.2% male), a median age of 36.8 years, and a median diabetes duration of 12.4 years. 15.3% were obese (body mass index ≥ 30kg/m 2 ). Long-acting insulin analogs were used by 53.3%, short-acting analogs by 72.1%, and oral antidiabetic drugs by 4.7%. Patients had a median glycosylated hemoglobin (HbA1c) of 7.8%. There was a drop in HbA1c and an increase in the rate of hypertension, oral antidiabetic drug use, and in the rate of severe hypoglycemia (all p < 0.01) with age. Flash glucose monitoring (FGM) showed the best glucose values with fewer complications compared to other monitoring systems. HbA1c and FBG were lower in patients using a pump versus multiple daily injections (MDIs; 7.7 versus 7.9% and 7.8 versus 8.7 mmol/l; all adjusted p < 0.01). Patients had a lower risk of at least one severe hypoglycemic or DKA episode during the most recent treatment year with pump treatment compared to MDI (9.4% versus 10.5% and 4.7% versus 6.1%, both adjusted p < 0.01). Conclusion: The data demonstrated less-than-optimal glycemic control in the young, an increasing metabolic pattern in T1DM with increasing age, a benefit of FGM to improve HbA1c control and adverse effects, as well as benefits of pump treatment over MDIs.

Abstract:The incidence of autoimmune disorders (AIDs) has been dramatically increasing in both children and adults over the past few years, and type 1 diabetes (T1D) is one of the diseases that has seen the highest growth. It is well-known that the dysimmune process may spread to other systems, leading to the onset of one or more AIDs in the same individual; however, the relationship between AIDs is not often recognized. The most frequently diagnosed AIDs in children and adolescents with T1D are thyroid diseases and celiac disease, but it is also important to consider the onset of the other conditions, such as juvenile idiopathic arthritis, multiple sclerosis, atrophic gastritis, inflammatory bowel diseases, and skin disorders such as vitiligo and psoriasis. This review aims to explore the overlap of T1D and other AIDs, focusing on the less common and lesser-known diseases. A better knowledge of these comorbidities may facilitate the identification of patients at risk to treat them in the preclinical period, before the onset of complications.

Abstract:Introduction We aimed to characterise and compare individuals diagnosed with type 1 diabetes (T1D), latent autoimmune diabetes in adults (LADA) and type 2 diabetes (T2D), in a real‐world setting. Methods Anthropometric and clinical data from 36 959 people with diabetes diagnosed at age 30–70 years enrolled in the prospective diabetes patients follow‐up (DPV) registry from 1995 to 2022 were analysed cross‐sectionally at diagnosis and follow‐up (≥6 months after diagnosis). LADA was defined as clinical diagnosis of T2D, positivity of ≥1 islet autoantibody and an insulin‐free interval of ≥6 months upon diabetes diagnosis. Results At diagnosis, age, body mass index, waist circumference, C‐peptide and HbA1c in people with LADA (n = 747) fell in between individuals with T1D (n = 940) and T2D (n = 35 272) (all p‐values

Abstract:Background and aims: Autoimmune thyroid dysfunction (AITD) is a significant autoimmune disorder affecting the population across age groups. The clustering of autoimmune diseases tends to occur within the same patients and families. Thus, this study aimed to determine the association of Type 1 diabetes and other autoimmune ailments in patients with autoimmune thyroid disorders. Methods: We performed a cross-sectional study, evaluating 500 subjects with a diagnosis of AITD (130 with Graves' disease; 370 with Hashimoto's thyroiditis) on presentation to our tertiary care centre to ascertain the prevalence of associated autoimmune disorders. Results: The frequency of Type 1 diabetes and other autoimmune disorders was 18.5% in Graves' disease and 27.8% in Hashimoto's thyroiditis patients. Coeliac disease (8.8%) (found in 6.9% of Graves' disease and 9.5% of Hashimoto's thyroiditis patients) and type 1 diabetes (7.8%) (found in 3.1% of Graves' disease and 9.5% of Hashimoto's thyroiditis patients) were the most common coexisting autoimmune disorders. Rheumatoid arthritis was the most common non-endocrine autoimmunity (2.8%). Female sex and duration of AITD more than five years were associated with increased odds of associated autoimmune disorders. Conclusion: A high prevalence of associated autoimmune disorders was observed in subjects with autoimmune thyroid dysfunction. We suggest the patients who remain symptomatic and those who develop other symptoms even with appropriate treatment undergo screening for associated autoimmune disorders, thus preventing a delay in diagnosis.

Abstract:Type 1 diabetes mellitus (T1DM) is a chronic immunemediated disease that specifically targets insulin-producing pancreatic b cells. Atopic dermatitis (AD) or eczema is a polyfactorial chronic relapsing inflammatory/autoimmune disease with a major impact on quality of life that has been often associated with other comorbidities. Both diseases have a mainly childhood onset. However, although both diseases are considered to be immune-mediated, AD is driven by T-helper (Th) 2-dependent inflammation, whereas T1DM is mediated mainly by a Th1 response; these two pathways being theoretically mutually inhibitory. In this issue of the BJD, Lin et al. propose a populationbased, retrospective cohort study, the main aim of which was to analyse the association between T1DM and AD using claims data from the National Health Insurance program of Taiwan. They identified 3386 patients with T1DM newly diagnosed from 1998 to 2011 and 12 725 randomly selected controls without T1DM. The 12 725 controls were matched by age, sex and year of diagnosis. All patients were followed up until the end of 2011 and AD was notified if it developed. Both cohorts were compared using Cox proportional hazards regression models to analyse the risk of AD. The main result of this study is that the overall incidence rate of AD was significantly higher in the T1DM cohort than in the non-T1DM cohort even after adjustment for potential risk factors. However, the AD incidence densities were shown to be very low in both groups (fewer than three new patients with AD per 1000 patient-years). This finding may be due to the high mean age (11 years) of the children in both cohorts. Indeed, most patients with AD have usually been diagnosed before this age. In addition, the authors showed that among patients with T1DM, only those with more emergency room visits or hospitalizations had a higher risk of subsequent AD compared with the non-T1DM cohort. This latter finding may raise the possibility of a surveillance bias in the T1DM cohort, as children with more hospitalizations undergo more clinical followup. In the past, the association of AD and T1DM has drawn conflicting results and to date no other study has found a significant association between these two diseases. Although the validity of the results may be restricted to Chinese ethnic groups, the strength of the study by Lin et al. is that this is the first nationwide study that has been performed to examine this association. The apparent and oversimplified antagonism between these two diseases, i.e. AD is driven by Th2-dependent inflammation whereas T1DM is mediated mainly by a Th1 response, may reflect the complexity of deciphering the immune pathways in autoimmune/autoinflammatory diseases. Moreover, it is probable that different endophenotypes exist within the spectrum of AD and T1DM, which may have different immune pathways and add to the complexity of the pathophysiological picture underlying these diseases. Further prospective studies taking into account the different endophenotypes/stages of both diseases and more potential confounding factors are thus needed to confirm this association.
Associations Between Clinical Characteristics and Chronic Complications in Latent Autoimmune Diabetes in Adults and Type 2 Diabetes

Abstract:BackgroundThis study aimed to assess the associations between clinical characteristics and chronic complications in latent autoimmune diabetes in adults (LADA) and type 2 diabetes.MethodsThis is a retrospective study. Our diabetes registry included 6975 patients aged 30-75years old with phenotypic type 2 diabetes who underwent islet autoantibody screening between 2003 and 2012; 384 patients were identified to have LADA. Rates of chronic complications for LADA and type 2 diabetes were compared using a 1:2 matched design. Logistic models were fitted to identify the presence of chronic diabetic complications using clinical characteristics including gender, age, duration of diabetes, glycemic control and metabolic syndrome.ResultsWhen duration of diabetes is <5years, the prevalence of diabetic nephropathy (nephropathy; 12.2% versus 21.8%, p=0.018) and diabetic retinopathy (retinopathy; 8.1% versus 15.9%, p=0.011) were significantly lower in patients with LADA than in patients with type 2 diabetes; the prevalence of nephropathy and retinopathy were comparable between both groups when duration is 5years. There was no significant difference in the prevalence of macrovascular complications between groups. The areas under the receiver operating characteristic curves based on the nephropathy and retinopathy models were larger for LADA than for type 2 diabetes (0.72 versus 0.61, p=0.013; 0.76 versus 0.68, p=0.056).ConclusionsPatients with LADA had a lower prevalence of microvascular complications than patients with type 2 diabetes when the duration of diabetes was <5years. Regression equation fitted by clinical characteristics can better predict the risk of microvascular complications in LADA than in type 2 diabetes. Copyright (c) 2014 John Wiley & Sons, Ltd.

Abstract:OBJECTIVE - Autoimmune thyroid disease (AIT), celiac disease, and Addison's disease are characterized by the presence of autoantibodies: thyroid peroxidase antibody (TPOAb) and thyroglobulin antibody (TGAb) in AIT, tissue transglutaminase antibody (TTGAb) in celiac disease, and 21-hydroxylase antibody (21-OHAb) in Addison's disease. The objective OF this study was to define the prevalence of these autoantibodies and clinical disease in a population with type I diabetes.RESEARCH DESIGN AND METHODS - We screened 814 individuals with type I diabetes for TPOAb, TGAb, TTGAb, and 21-OHAb. Clinical disease was defined by chart review. Factors related to the presence of autoimmunity and clinical disease including age at onset of type 1 diabetes, duration of diabetes, age at screening, sex, and the presence of autoantibodies were reviewed.RESULTS - The most common autoantibodies expressed were TPOAb and/or TGAb (29%), followed by TTGAb (10.1%) and 21-OHAb (1.6%). Specific HLA DR/DQ genotypes were associated with the highest risk for expression of 21-OHAb (DRB1*0404-DQ8, DR3-DQ2) and TTGAb (DR3-DQ2- DR3-DQ2). The expression of thyroid autoantibodies was related to 21-OHAb but not to TTGAb. The presence of autoantibodies was associated with and predictive of disease.CONCLUSIONS - In this large cohort of individuals with type I diabetes, the expression of organ-specific autoantibodies was very high. The grouping OF autoantibody expression Suggests common factors contributing to the clustering.

Abstract:Growth factor signaling is deregulated in cancer and often leads to invasion, yet receptor tyrosine kinase signaling pathways driving invasion under different growth factor conditions are not well understood. To identify specific signaling molecules regulating invasion of A549 non-small cell lung carcinoma (NSCLC) cells downstream of the epidermal growth factor receptor (EGFR) and Met, quantitative site-specific mass spectrometric analysis of tyrosine phosphorylation was performed following epidermal growth factor (EGF) or hepatocyte growth factor (HGF) stimulation, at three different growth factor concentrations and at two time points. Through this analysis, the temporal and concentration dependent phosphorylation profiles were obtained for 131 and 139 sites downstream of EGF and HGF stimulation, respectively. To characterize the effect of these signaling network alterations, we quantified 3D cell migration/invasion through Matrigel. Partial least-squares regression (PLSR) analysis was performed to identify the tyrosine phosphorylation sites most strongly correlated with EGF and/or HGF mediated invasion. Potential common and specific signaling events required for driving invasion downstream of EGFR and Met were identified using either a combined or two independent PLSR models, based on the quantitative EGF or HGF data. Our data highlight the integration and compartmentalization of signaling required for invasion in cancer.

Abstract:Type 1 diabetes (T1D) and autoimmune thyroid disease (AITD) are the most frequent chronic autoimmune diseases worldwide. Several autoimmune endocrine and non-endocrine disorders tend to occur together. T1D and AITD often cluster in individuals and families, seen in the formation of autoimmune polyendocrinopathy (AP). The close relationship between these two diseases is largely explained by sharing a common genetic background. The HLA antigens DQ2 ( DQA1*0501-DQB1*0201 ) and DQ8 ( DQA1*0301-DQB1*0302 ), tightly linked with DR3 and DR4, are the major common genetic predisposition. Moreover, functional single nucleotide polymorphisms (or rare variants) of various genes, such as the cytotoxic T-lymphocyte- associated antigen (CTLA4) , the protein tyrosine phosphatase non-receptor type 22 (PTPN22) , the interleukin-2 Receptor (IL2Ra) , the Vitamin D receptor (VDR) , and the tumor-necrosis-factor-α (TNF) that are involved in immune regulation have been identified to confer susceptibility to both T1D and AITD. Other genes including cluster of differentiation 40 (CD40) , the forkhead box P3 (FOXP3) , the MHC Class I Polypeptide-Related Sequence A (MICA) , insulin variable number of tandem repeats (INS-VNTR) , the C-Type Lectin Domain Containing 16A (CLEC16A) , the Erb-B2 Receptor Tyrosine Kinase 3 (ERBB3) gene, the interferon-induced helicase C domain-containing protein 1 (IFIH1) , and various cytokine genes are also under suspicion to increase susceptibility to T1D and AITD. Further, BTB domain and CNC homolog 2 ( BACH2) , C-C motif chemokine receptor 5 ( CCR5) , SH2B adaptor protein 3 ( SH2B3) , and Rac family small GTPase 2 ( RAC2) are found to be associated with T1D and AITD by various independent genome wide association studies and overlap in our list, indicating a strong common genetic link for T1D and AITD. As several susceptibility genes and environmental factors contribute to the disease aetiology of both T1D and AITD and/or AP subtype III variant (T1D+AITD) simultaneously, all patients with T1D should be screened for AITD, and vice versa.

Abstract:BACKGROUND: The interaction between type 1 diabetes mellitus (T1DM) and attention deficit hyperactivity disorder (ADHD) in children and adolescents has been studied rarely. We aimed to analyse metabolic control in children and adolescents with both T1DM and ADHD compared to T1DM patients without ADHD.PATIENTS AND METHODS: Auxological and treatment data from 56.722 paediatric patients (<20 years) with T1DM in the multicentre DPV (Diabetes Prospective Follow-up Initiative) registry were analysed. T1DM patients with comorbid ADHD were compared to T1DM patients without ADHD using multivariable mixed regression models adjusting for demographic confounders.RESULTS: We identified 1.608 (2.83%) patients with ADHD, 80.8% were male. Patients with comorbid ADHD suffered twice as often from diabetic ketoacidosis compared to patients without ADHD [10.2; 9.7-10.8 vs [5.4; 5.3-5.4] (P < .0001). We also found significant differences in HbA1c [8.6% (7.3-9.4); 66.7 mmol/mol (56.3-79.4) vs 7.8% (7.0-9.0); 62.1 mmol/mol (53.2-74.7)], insulin dose/kg [0.9 IU/kg (0.7-1.1) vs 0.8 IU/kg (0.7-1.0)], body mass index-standard deviation score (BMI-SDS) [0.2 (-0.5 to 0.8) vs 0.3 (-0.3 to 0.9)], body weight-SDS [0.1 (-0.5 to 0.8) vs 0.3 (0.3 - 0.9)]; (all P < 0.0001), and systolic blood pressure after adjustment [mean: 116.3 vs 117.1 mm Hg)]; (P < 0.005).CONCLUSION: Paediatric patients with ADHD and T1DM showed poor metabolic control compared to T1DM patients without ADHD. Closer cooperation between specialized paediatric diabetes teams and paediatric psychiatry/psychology seems to be necessary to improve diabetes care and metabolic control in this group of patients.

Abstract:Type 1 diabetes mellitus (T1DM) is an autoimmune disorder characterized by the destruction of beta islet cells in the pancreas, in which T cells play a key role. [ 1 ] In addition, patients with T1DM have a higher risk of other autoimmune disorders such as celiac disease, vitiligo, alopecia, Hashimoto thyroiditis, and Graves' disease. [ 2 , 3 ] The pathogenetic mechanism underlying this concomitant occurrence has not been clearly understood. [ 4 ] Studies have suggested genetic predisposition and environmental factors, which might be involved in the pathogenesis of these diseases. [ 3–5 ] Some conditions have consequences and can affect diabetes control if its concomitant with T1DM. [ 2 ]

Although it is an organ-specific autoimmune disorder, T1DM is often associated with multiple other autoimmune disorders. The most prevalent concomitant autoimmune disorder occurring in T1DM is autoimmune thyroid disease (AITD), which mainly exhibits two extremes of phenotypes: hyperthyroidism [Graves' disease (GD)] and hypo-thyroidism [Hashimoto's thyroiditis, (HT)]. However, the presence of comorbid AITD may negatively affect metabolic management in T1DM patients and thereby may increase the risk for potential diabetes-related complications. Thus, routine screening of thyroid function has been recommended when T1DM is diagnosed. Here, first, we summarize current knowledge regarding the etiology and pathogenesis mechanisms of both diseases. Subsequently, an updated review of the association between T1DM and AITD is offered. Finally, we provide a relatively detailed review focusing on the application of thyroid ultrasonography in diagnosing and managing HT and GD, suggesting its critical role in the timely and accurate diagnosis of AITD in T1DM.

Islet autoantibody frequency in relatives of children with type 1 diabetes who have a type 2 diabetes diagnosis

Abstract:Aim: This study aimed to evaluate characteristics of autoimmunity in individuals who have a type 2 diagnosis and are relatives of children with type 1 diabetes. Methods: Pre-diagnosis samples (median 17 months before onset) from relatives who were later diagnosed with type 2 diabetes were measured for autoantibodies to glutamate decarboxylase 65 (GADA), islet antigen-2 (IA-2A), zinc transporter 8 (ZnT8A) and insulin (IAA) as well as the type 1 diabetes genetic risk score (GRS2). Associations between islet autoantibodies, insulin treatment and GRS2 were analysed using Fisher's exact and t-tests. Results: Among 226 relatives (64% men; mean age at sampling 41 years; mean age 54 years at diagnosis), 32 (14%) were islet autoantibody-positive for at least one autoantibody more than a decade before diagnosis. Approximately half of these (n = 15) were treated with insulin. GADA-positivity was higher in insulin-treated relatives than in non-insulin-treated relatives (12/18 [67%] vs. 6/18 [33%], p < 0.001). IAA-positivity was observed in 13/32 (41%) of relatives with autoantibodies. GRS2 scores were increased in autoantibody-positive relatives (p = 0.032), but there was no clear evidence for a difference according to treatment (p = 0.072). Conclusion: This study highlights the importance of measuring islet autoantibodies, including IAA, in relatives of people with type 1 diabetes to avoid misdiagnosis.
Association of Thyroid Autoantibodies with Diabetic Nephropathy in Type 2 Diabetes Mellitus Patients

Abstract:Background: Diabetic nephropathy (DN) is one of the most common and severe complications in patients with type 2 diabetes mellitus (T2DM). Thyroid dysfunction has been associated with diabetes and its complications but the relationship between thyroid autoantibodies and T2DM-DN remains unclear. Objective: The study aims to investigate the association between thyroid autoantibodies and diabetic nephropathy in patients with T2DM and analyze the expression of serum thyroid hormone levels in T2DM-DN patients and its prognostic value. Methods: 117 patients with T2DM who visited our hospital from December 2020 to December 2022 were recruited and assigned to group A (65 patients with T2DM-DN) and group B (52 patients with T2DM without DN). Serum TH levels of patients with DN and normal diabetic patients were analyzed, and the prognosis of patients was evaluated. Results: The results demonstrated that compared to group B, group A had higher serum cystatin C (cysC), serum creatinine (SCr), thyroglobulin antibody (TgAb), and urinary microalbumin/creatinine (UACR) levels, while the levels of free triiodothyronine (FT3), albumin (ALB), and estimated glomerular filtration rate (eGFR) were lower (P < .05). FT3 in group A2 was inferior to that in group A0 and group A1 (P < .05). After correction, the results demonstrated that the level of thyroid peroxidase antibody (TPOAb) in group A1 was superior to that in group A0 (P < .05). The positive rates of TPOAb (20%) and TgAb (23.08%) in patients with T2DM-DN were drastically superior to those in patients with T2DM without DN. Among the independent risk factors for DN, the OR of positive TPOAb was 8.125. Conclusion: The level and positive rate of thyroid autoantibodies in patients with T2DM-DN were high and TPOAb positivity might be a risk factor for the occurrence of T2DM-DN.
Association of HLA Haplotypes with Autoimmune Pathogenesis in Newly Diagnosed Type 1 Romanian Diabetic Children: A Pilot, Single-Center Cross-Sectional Study

Abstract:Background: The increasing incidence of autoimmune diseases in type 1 diabetes mellitus (T1DM) patients highlights the influence of human leukocyte antigen (HLA) haplotypes on their development. This study aims to determine genetic predisposition to autoimmune diseases in T1DM patients, including thyroid disease and celiac diseases, and explore its correlation with vitamin D deficiency. Methods: A cross-sectional study involving thirty-six T1DM children was conducted. Typing was performed for the HLA A, B, C, DP, DR, and DQ loci. Regression analysis linked DR-DQ haplotypes to T1DM and the associated conditions. Results: The most frequent predisposing alleles and haplotypes were HLA-DR3 (70.27%), DQ2 (70.27%), DR3-DQ2 (70.27%), DQB1*02:01 (70.27%), A02 (54.05%), whereas the most prevalent protecting allele was DPB1*04:01 (52.63%). Positive correlations were observed between positive anti-thyroid peroxidase antibodies and the absence of protective alleles (DPB1*04:02, p = 0.036; DPB1*04:01, p = 0.002). Associations were found between the absence of DPB1*04:01 and anti-thyroglobulin antibodies (p = 0.03). HLA allele DPB1*03:01 was linked with vitamin D deficiency (p = 0.021). Positive anti-transglutaminase antibodies correlated with C03:03 (p = 0.026) and DRB1*04:01-DQA1*03-DQB1*03:01 (p < 0.0001) and the lack of DQA1*01:03-DQB1*06:03-DRB1*13:01 (p < 0.0001). Conclusions: The predisposing T1DM haplotypes were associated with the presence of anti-transglutaminase and anti-thyroid antibodies, indicating a genetic predisposition to autoimmune diseases.

Abstract:Type III autoimmune polyendocrine syndromes (APS) is characterized by the coexistence of autoimmune thyroid disease (AITD) and T1DM. There is a rare series of studies on APS in Chinese population. This paper retrospectively analyzed the clinical and serological features of Chinese patients with type III APS who admitted to our center from Sept 2018 to Oct 2022, and compared the similarities and differences with isolated T1DM, LADY/LADA patients at the same time. The study included 69 patients, 18 patients with type III APS, 20 patients with T1DM, and 31 patients with LADY/LADA. Type III APS was predominantly female, while T1DM and LADY/LADA groups were the opposite (72.22% vs 35.00% vs 38.71%, P = 0.037). The median age of onset of diabetes in the three groups was 30.50, 23.00 and 41.00 years, respectively (P < 0.001). Hashimoto's thyroiditis (HT) (n=13, 72.22%) was the main AITD in type III APS group, followed by Graves’ disease (Table 1). The median of GADAb in the three groups were 1501.43, 115.01 and 301.02 U/mL, respectively (P = 0.022). And the levels of TPOAb and TGAb in type III APS group were significantly higher than those in the other two groups (P <0.05). Type III APS is more common in women with diabetes onset later than those with isolated T1DM and earlier than those with LADY/LADA. HT is the main AITD in type III APS patients, and the levels of GADAb and thyroid autoantibodies in patients with type III APS are often higher. Disclosure Y. Qiu: None. J. Guo: None. P. Wei: None.
