Abstract:The word hysteria originated in the Corpus Hippocraticum (c420 BCE) as a natural explanation for a variety of diseases in women linked in the Greco-Roman mind to an animate or inanimate womb, but which in the last five centuries has evolved to describe an elusive disorder of brain ± mind in men and women, currently referred to by neurologists as "functional neurological disorder". The Babylonians, Assyrians and Egyptians had no knowledge of brain or psychological function. Babylonian and Assyrian descriptions of disease and behaviour include only rare examples suggestive of modern hysteria. An earlier suggestion that the Greek concept of hysteria was transmitted from Egypt is not supported by recent evidence. The Greco-Roman civilisations had some knowledge of neuroanatomy, but little of nervous system function, conceived in terms of humors. The examples cited here suggestive of modern hysteria are relatively infrequent and fragmentary. The most plausible are attempts to separate the "sacred disease" from other causes of loss of consciousness. The great achievement of Greco-Roman medicine was in introducing natural causation, including causation linked to the womb, rather than gods or evil spirits. Nevertheless medicine, magic and religion have remained intertwined to varying degrees in all cultures up to the present time, despite the growth of modern scientific medicine. The study of hysteria in ancient civilisations adds interesting insight into the evolution of thinking about brain, psyche, mind and self. Babylonian and Egyptian medical and behavioural descriptions are based on observation. Greek and Roman accounts include some subjective aspects, probably linked to early attempts to understand identity, the psyche, intellectual and emotional functions. The great philosophical debate whether the latter resided in the head/brain (Plato) or the heart (Aristotle) has only been settled in the last few centuries, during which hysteria also became linked to brain ± mind. Our more recent increasing knowledge of brain function has also been accompanied by increasing interest in subjective feelings, thoughts, the inner life and subconscious mechanisms, suggesting we may have become more self-aware than in earlier civilisations, which in turn may perhaps influence the clinical presentation of hysteria. The study of hysteria may be one of the keys to a greater understanding of the relationship between brain and mind.
Charcot and the psychology of hysteria, with special reference to a never published final case history

Abstract:Jean-Martin Charcot is perhaps best remembered for his contributions to organic neurology. However, his pursuit of hysteria, the most prevalent diagnosis in his hospital clinic, yielded no anatomical lesion to account for hysteria's plethora of somatic disorders assumed due to a purely functional or dynamic lesion in the cerebral cortex. This led Charcot to turn his attention to the psychology of hysteria. Taking advantage of institutional reforms at the Salpêtrière-notably, the establishment of his professorship in nervous diseases-Charcot from the early 1880s focused his teaching increasingly on case histories of hysteria in male as well as female patients. Already renown for his earlier dramatic public lessons on female hysteria, his lessons of the 1880s, of which two volumes were published at the end of the decade, elaborated the issue of psychology in terms of altered states of patient's suggestibility. By the decade's end, Charcot's worldwide reputation rested on the prospects of this work as acknowledged by numerous students, notably medical psychologists Pierre Janet and Sigmund Freud. Yet Charcot's views remained sketchy. They were discussed at length in his unpublished notes for a lesson intended for May 1893, just a few months before his sudden death. His unpublished notes reveal a detailed case for dreams as illustrating a psychological mechanism underlying hysteria in a 17-year-old Paris artisan. I conclude by considering why this significant climactic case of Charcot's might have been overlooked by his entourage.
Hysterical conversion and brain function

Abstract:Hysterical conversion disorders represent "functional" or unexplained neurological deficits such as paralysis or somatosensory losses that are not explained by organic lesions in the nervous system, but arise in the context of "psychogenic" stress or emotional conflicts. After more than a century of both clinical and theoretical interest, the exact nature of such emotional disorders responsible for hysterical symptoms, and their functional consequences on neural systems in the brain, still remain largely unknown. However, several recent studies have used functional brain imaging techniques (such as EEG, fMRI, PET, or SPECT) in the attempt to identify specific neural correlates associated with hysterical conversion symptoms. This article presents a general overview of these findings and of previous neuropsychologically based accounts of hysteria. Functional neuroimaging has revealed selective decreases in the activity of frontal and subcortical circuits involved in motor control during hysterical paralysis, decreases in somatosensory cortices during hysterical anesthesia, or decreases in visual cortex during hysterical blindness. Such changes are usually not accompanied by any significant changes in elementary stages of sensory or motor processing as measured by evoked potentials, although some changes in later stages of integration (such as P300 responses) have been reported. On the other hand, several neuroimaging results have shown increased activation in limbic regions, such as cingulate or orbitofrontal cortex during conversion symptoms affecting different sensory or motor modalities. Taken together, these data generally do not support previous proposals that hysteria might involve an exclusion of sensorimotor representations from awareness through attentional processes. They rather seem to point to a modulation of such representations by primary affective or stress-related factors, perhaps involving primitive reflexive mechanisms of protection and alertness that are partly independent of conscious control, and mediated by dynamic modulatory interactions between limbic and sensorimotor networks. A better understanding of the neuropsychobiological bases of hysterical conversion disorder might therefore be obtained by future imaging studies that compare different conversion symptoms and employ functional connectivity analyses. This should not only lead to improve clinical management of these patients, but also provide new insights on the brain mechanisms of self-awareness.

Abstract:This paper is an investigation into the prominence of hysteria in Lacan's work and the enduring significance of the diagnosis for contemporary practice. Beginning with Lacan's theory of neurosis, the importance of language, and symbolic functions, we will begin to understand why the hysterical symptom is the symptomatic structure par excellence. Lacan lauds hysteria as the neurosis in direct dialogue with a given historical moment, teaching the psychoanalyst where we are in the unfolding struggle between neurosis and civilization. He returned to the case of Dora throughout his twenty-eight years of teaching to refine his work. He even saw psychoanalysis as the progressive "hystericization" of the patient and depicted himself as an hysteric walking around on a stage, not knowing what he was saying, while attempting to teach the psychoanalysts. Lacan's reading of Dora seems to mark important shifts in his own life, from his beginning to write as a psychoanalyst, to being thrown out of the IPA, to struggling with his own school and the ensuing political eruptions in France in 1968. By tracking his elaboration of Dora we can witness the evolution of Lacan's work and how he uniquely positions the analyst in the transference.

Abstract:This paper deals with the history of the relationship between the mind-body dualism and the epistemology of madness. Earlier versions of such dualism posed little problem in regard to the manner of their communication. The Cartesian view that mind and body did, in fact, name different substances introduced a problem of incommunicability that is yet to be resolved. Earlier views that madness may be related to changes in the brain began gaining empirical support during the 17th century. Writers on madness chose to resolve the mind-body problem differently Some stated that such communication was not needed; others, that mind was a redundant concept, as madness could be fully explained by structural changes in the brain; and yet others described psychological spaces for madness to inhabit as a symbolic conflict. The epistemology of the neurosciences bypasses the conundrum, as it processes all together the variables representing the brain, subjectivity, and behavior and bridges the "philosophical" gap by means of correlational structures.

Abstract:Work at the intersection of philosophy and psychiatry has an extensive and influential history, and has received increased attention recently, with the emergence of professional associations and a growing literature. In this paper, we review key advances in work on philosophy and psychiatry, and their related clinical implications. First, in understanding and categorizing mental disorder, both naturalist and normativist considerations are now viewed as important – psychiatric constructs necessitate a consideration of both facts and values. At a conceptual level, this integrative view encourages moving away from strict scientism to soft naturalism, while in clinical practice this facilitates both evidence‐based and values‐based mental health care. Second, in considering the nature of psychiatric science, there is now increasing emphasis on a pluralist approach, including ontological, explanatory and value pluralism. Conceptually, a pluralist approach acknowledges the multi‐level causal interactions that give rise to psychopathology, while clinically it emphasizes the importance of a broad range of "difference‐makers", as well as a consideration of "lived experience" in both research and practice. Third, in considering a range of questions about the brain‐mind, and how both somatic and psychic factors contribute to the development and maintenance of mental disorders, conceptual and empirical work on embodied cognition provides an increasingly valuable approach. Viewing the brain‐mind as embodied, embedded and enactive offers a conceptual approach to the mind‐body problem that facilitates the clinical integration of advances in both cognitive‐affective neuroscience and phenomenological psychopathology.

The vicissitudes of the mind-body relationship have to be considered within the context of development of object-relationships and affect regulation.

Abstract:The biomedical aspiration of psychiatry has fundamentally reoriented clinical practice since the DSM-III in 1980 and reverberated in the public sphere. Over time, lay public understanding of the causes of mental suffering has increasingly endorsed biological conceptions. In this paper, I explore the sources from which a neurobiological model for mental suffering reaches ordinary people, and investigate its rhetorical appeal, personal appropriation, and consequences. Drawing on interviews and other data, I show that these sources-physicians, popular media, and advertising-share common ontological and moral assumptions. These assumptions, in turn, influence how people take up neurobiological explanation to account for their suffering, and how, paradoxically, they join it to their projects of self-determination. I conclude by considering how, from a phenomenological perspective, a neurobiological account fails to enhance self-knowledge or determination but leads to a hermeneutic dead end.
Two interesting cases highlighting an oblivious specialty of psychoneuroendocrinology

Abstract:Psychoneuroendocrinology deals with the overlap disorders pertaining to three different specialties. Awareness about the somatic manifestations of psychiatric diseases and vice versa is a must for all the clinicians. The knowledge of this interlinked specialty is essential because of the obscure presentation of certain disorders. Our first case was treated as depressive disorder, whereas the diagnosis was hypogonadism with empty sella. Our second patient was managed as schizophrenia and the evaluation revealed bilateral basal ganglia calcification and a diagnosis of Fahr's disease. We report these cases for their unusual presentation and to highlight the importance of this emerging specialty.

Abstract:Background: Hysteria in its most severe expression may reach psychotic manifestations. Such symptomatology has been occasionally described by various authors starting from the 19th century and defined as “hysterical psychosis” (HP) by Hollender and Hirsch in 1964. Currently, diagnostic psychiatric manuals such as DSM and ICD do not include the diagnosis of HP, although this term is commonly used in clinical practice. This raises a well-known problem with case definition due to an inconsistent use of terminology. Summary: Here, we propose a review of the literature that aims to highlight the clinical features of HP endorsed by the majority of authors, such as histrionic premorbid personality, acute reactive onset, short duration, altered state of consciousness, unstable delusions, typical hallucinations, labile mood, lack of flat affect. In the discussion, we focus on the differential diagnosis between HP and other diagnoses such as brief psychosis and schizophrenia, trying to point out aspects of distinction and continuity. Key Messages: The debate about this nosographic entity still remains a huge dilemma and needs further contributions.

Abstract:The efficiency of transformation of point mutations depends upon base pair mismatches during the recombination process. For low-efficiency markers, the genetic information carried on the donor deoxyribonucleic acid is preferentially lost. To understand this elimination process, we investigated the effect of the size of donor deoxyribonucleic acid on the relative efficiency of low-efficiency point mutations. The deoxyribonucleic acid was shortened either by mechanical shearing or by restriction enzyme treatments. The results indicate that transformation by low-efficiency markers was not affected by shortening the distance between them and the end of the molecule any more than was transformation by the other markers. Moreover, no lethal event could be detected for either cell or chromosomal marker survival. These data do not exclude the double-strand-break hypothesis that was proposed to explain the loss of genetic information for low-efficiency markers, but they offer no support for it.

Abstract:As a consequence of the pioneering work of Scharrer and Scharrer (1940) and Harris (1960), it is now appreciated that the secretion of anterior pituitary hormones is prominently regulated by the central nervous system (CNS). Small polypeptide and catecholamine hormones are secreted into the portal vessels of the pituitary stalk from neurosecretory cells in the external layer of the median eminence of the basal hypothalamus and carried to the anterior pituitary to stimulate or inhibit the synthesis of pituitary hormones and their release into the systemic circulation (Guillemin, 1978; Schally, 1978). Various putative neurotransmitters such as dopamine, norepinephrine, serotonin, acetylcholine, GABA and histamine (Weiner and Ganong, 1978), as well as some of the recently described CNS peptides such as endorphins, bombesin, substance P and vasoactive intestinal peptide, appear to be involved in the regulation of hypothalamic neuropeptide release (Hökfelt et al., 1978). These neurotransmitters and/or neuromodulators may act singly to increase or decrease the secretion of a specific pituitary hormone, but more likely a number of different transmitters are involved in a complex web of interactions, with the final response of the pituitary being determined by their net influence on the secretion of hypothalamic releasing and inhibiting factors into the hypophysial portal blood.

Abstract:The diagnosis of hysteria, despite being fundamental in the birth of psychiatry, has currently been removed from nosography. This choice speaks of the renunciation by contemporary nosography of understanding psychopathological conditions as structural entities, with internal coherence and meaningfulness – which on the contrary should be reconsidered. Hysteria represents a mirror of social and cultural changes. The metamorphoses throughout history of hysterical symptoms reflect the changing interests of medicine (the greater legitimation of somatic symptoms over psychic ones) and, in general, mirror the increasing importance of images in the contemporary world. Despite its nosographical fragmentation, hysteria continues to be talked about. Phenomenologically, hysteria is described not as a diagnosis but as an existential position, freeing it from gender prejudices. Hysterical persons suffer from a hypo-sufficiency of the self, from a difficulty in feeling from within, which ends up hypertrophying the identity definitions coming from without: the gaze of others, socio-cultural stereotypes, gender models. Visibility therefore takes on a central role, and images become a vehicle to represent oneself – capable of attracting the attention of others and enchanting them. Different powers have been attributed to images throughout the history of thought: that of paralyzing, that of moving to action, that of underlining the contingency of experience. Hysterical persons embody images, generating with their symptoms a world of representations. However, what characterizes hysteria is not the symptom, but the use made of that symptom: it becomes a catalyst for the gaze of others, which allows one to assume an otherwise lacking identity-consistency. For this reason, hysterical persons are not only passively subject to their symptoms, but actively make use of them in interpersonal relations.

Neurologic manifestations of hypothalamic disease

Abstract:The hypothalamus, in addition to regulating the anterior and posterior pituitary, controls water balance through thirst, regulates food ingestion and body temperature, influences consciousness, sleep, emotion and other behaviors. Much has been learned of these effects in human disease through the clinical manifestations that occur with hypothalamic lesions. This study reviews the clinical pathologic correlations that have been made in recent years showing that regions of the hypothalamus exert functions in humans that are similar to those identified in experimental animals. Clinical pathologic correlations have not always provided precise analysis of hypothalamic function. The hypothalamus is small and often lesions that come to clinical attention achieve considerable size before their recognition, making local anatomic dissections of the effects of the lesions difficult. Nevertheless, the use of modern non-invasive techniques including CT scans and magnetic resonance imaging (MRI) have provided new information not previously available. This paper reviews several cases of hypothalamic disorder recognized recently. (1) A 33-year-old black man with hypothalamic sarcoidosis. Manifestations of hypothalamic dysfunction included panhypopituitarism, aggressive hyperphagia, polydipsia (partially due to hyperglycemia secondary to diabetes mellitus), drowsiness, depression, and irritability. (2) A 37-year-old woman with a large intrahypothalamic tumor (biopsy showed pituitary adenoma), with drowsiness, poikilothermia, lack of satiety, confusion, and memory loss. She becomes depressed when she is transiently more alert (as after hypertonic contrast-dye infusion). (3) A 60-year-old man with hypothalamic compression by a pituitary tumor, associated with syndrome of inappropriate ADH (SIADH), severe anorexia, memory loss, but preserved thirst. After surgical decompression of the tumor his appetite acutely recovered, but he developed severe hypo(poikilo)thermia. (4) A 45-year-old woman with a suprasellar craniopharyngioma presented with severe drowsiness, hyperphagia, depression, and memory loss post-operatively, which responded to antidepressants (except for the memory loss). She had extremely labile blood pressures and serum Na for about 1 week post-operatively.

Abstract:There is a conceptual crisis in the biomedical sciences that is particularly salient in psychopathology research. Underlying the crisis is a controversy that pertains to the current medical model of disease that largely draws from causal-mechanistic explanations. The bedrock of this model is the analysis of biological part-dysfunctions that aims at unequivocally defining a pathological condition and demarcating it from its neighboring entities. This endeavor has led to a quest for physiological, biochemical, and genetic signatures. Yet, so far there is little evidence for reliable biomarkers for any mental disorder. The contemporary biomedical paradigm largely ignores historical, dynamic, and system-level aspects—a view that has contributed much to the conceptual disjunction of the patient as a person from his/her disease. Notwithstanding the impressive progress in the biomedical sciences, increasingly more critics question whether the constituting framework is sufficient to convey a comprehensive understanding of illnesses, especially mental illnesses. Thus, the medical model urgently requires an update. But rather than revamping it by methodological advancements, it will be necessary to critically review its philosophical roots. The most problematic issues that require reworking are: the preponderance of the biostatistical theory; the undue decoupling of physiological from evolutionary explanations of function; the clinging to the Modern Synthesis (if etiological aspects are ever considered); and the neglect of dynamic and system-level properties. The proposed overhaul requires the heeding of historical explanations that draw from the Extended Evolutionary Synthesis as well as systems biology approaches for tackling multilevel and dynamic phenomena of complex systems.
The seductive allure of the brain: Dualism and lay perceptions of neuroscience

Abstract:Laypeople prefer brain explanations of behavior (Weisberg, Keil, Goodstein, Rawson, & Gray, 2008). We suggest that this preference arises from 'intuitive Dualism'. For the Dualist, mentalistic causation elicits a mind-body dissonance, as it suggests that the immaterial mind affects the body. Brain causation attributes behavior to the body, so it alleviates the dissonance, hence, preferred. We thus predict stronger brain preference for epistemic traits – those perceived as least material, even when no explanation is required. To test this prediction, participants diagnosed clinical conditions using matched brain- and behavioral tests. Experiments 1-2 showed that epistemic traits elicited stronger preference for brain tests. Experiment 3 confirmed that epistemic traits are perceived as immaterial. Experiment 4 showed that, the less material the trait seems, the stronger the surprise (possibly, dissonance) and brain preference. Results offer new insights into public perception of science, the role of intuitive Dualism, and the seductive allure of neuroscience.

[The current mind-brain theories in analytical philosophy of mind and their epistemic significance for psychiatry]

Abstract:This article begins with an orientational survey of the historical evolution of analytical philosophy of mind (APM) which was formulated in the last 40 years as "philosophy of mind" in the Anglo-Saxon scientific-cultural world and which, in the meantime, dominates to a great extent contemporary German philosophy. Then there follows a discussion of the currently most popular mind-brain theories in philosophy. In comparison to the more marginal dualist variants (interactionism, epiphenomenalism, parallelism), it is mainly the monistic positions of non-reductive, reductive and eliminative materialism and the materialist functionalism underlying it, which determines analytical philosophy of mind and its influence on psychopathology and psychiatry. Under the additional influence of modern brain research methods, particularly neuroimaging, it is progressively developing into a subdiscipline of neuroscience, a complex and increasingly more firmly established scientific discipline which comprises the totality of all sciences dealing with neuronal functions, including the close epistemic associations of APM and neuroimaging. This is the effective epistemic central idea determining the theory of the neuronal network which, in the form of a connectionist psychopathology, is intended to make possible a fundamentally new access to the comprehension of psychiatric forms of illness. In this respect it is evident, however, that the perception of the naturality of the mind as the fundamental thesis of APM and thus of connectionism cannot be followed through, since, up to now, neither from the phenomenality of the mind (especially the quality of senses, "Qualia") nor from intentionality of the mind (i. e. the ability to act intentionally, free from the constraint of the causality of nature and thus in self-responsible fashion) has proved it possible to reconstruct a generally accepted naturalist theory. Furthermore, it has not been possible to reformulate it in an exclusively physical, i. e. non-phenomenological concept and terminology which is, above all, free from the intentionality idiom. The consequence of this, however, is that a connectionist psychopathology can only represent a subpersonal, i. e., subhuman area and that in order to establish a personal psychopathology, naturalistic unreduced theories of experience-qualities and intentional acts of completeness are absolutely essential. The neuroscientific-connectionist paradigm of psychopathology must therefore - at least for the present - be supplement by the paradigm of a non-natural (e. g. phenomenological-hermeneutic) psychopathology. This result can only encourage the relinquishing of epistemically one-sided materialist and other monistic mind-brain theories of APM in favour of an epistemically open pragmatic interactionist dualism as the scientific position which best represents the current state of knowledge.
The Body in Question in the Existence of Hysteric Persons: A Phenomenological Perspective

Psychopathology

Abstract:The concept of hysteria, although apparently surpassed by contemporary nosographic classifications, continues to be talked about. Following Charbonneau's attempt to de-feminize and de-sexualize hysteria, clinical phenomenology can offer a perspective which, freed from stigma and prejudices through the suspension of judgement, allows us to understand hysteria not as a diagnostic category but as an existential position. In this sense, hysteria would be based on a hypo-sufficiency of the embodied self, which is not perceived as solid and continuous and needs external confirmations of its adequacy. According to the optical-coenaesthetic disproportion hypothesis, the hypo-sufficiency of the embodied self originates from the difficulty of experiencing one's body from the first-person perspective and from the consequent use of the gaze of others as a prosthesis to achieve a sense of selfhood and identity. Hysteric persons develop a mode of access to their corporeality mediated by visual representations - hence the theatricalization, centrality, and seductiveness of hysteric persons' behaviour. We suggest to call "figural body" the visual apprehension of one's body which tries to compensate for the weakness of coenaesthetic apprehension of the lived body. Over time, the figural body ends up superimposing itself on the immediate experience of the lived body. Placing itself on a representative register, this image conveys not only individual ghosts but also cultural aspects, social prejudices, gender stereotypes. Thus, the attempt to experience one's own body with the mediation of the other's gaze becomes an involuntary and unaware throwing of oneself into the meshes of representation that are necessarily alienating for the person. Hysterical persons remain stuck in their inability to access an experience of their body that is not figurative, alienating themselves in representations which always come from outside.
