Abstract:Objectives. This study aimed to assess human papilloma virus (HPV) 16 18/45 typing test results combined with cytology for cervical exfoliated cells from women who screened positive in an HPV E6/E7 mRNA assay (Aptima HPV, AHPV). Methods. In total, 3257 AHPV-positive women aged 25-65 years were underwent AHPV 16 18/45 Genotype assay (AHPV-GT) testing with cytology. Women were referred for colposcopy and further biopsy if indicated. Different triaging strategies were compared. Results. Overall, 624 women (19.2%) tested AHPV-GT positive. When identifying CIN2+, compared with cytology, AHPV-GT achieved a similar AUC (0.72 vs. 0.69, P = 0.158) but a higher specificity (85.1% vs. 79.3%, P < 0.001) and positive predictive value (PPV) (29.6% vs. 23.2%, P < 0.001). When identifying CIN2+, compared with cytology, the cotesting strategy (cytology combined with AHPV-GT) increased the AUC from 0.69 to 0.76 (P< 0.001), with a higher sensitivity (84.6% vs. 59.5%, P< 0.001), higher NPV (97.6% vs. 94.9%, P< 0.001) and similar PPV (21.6% vs. 23.2%, P = 0.054). When identifying CIN2+, the results of combination strategy (AHPV-GT genotyping plus reflex cytology in women positive for the 11 other hrHPV genotypes) were consistent with those of the cotesting strategy. Similar results were achieved when identifying CIN3 +. Conclusions. The AHPV-GT test may be a promising triage approach with high specificity in women receiving AHPV-positive primary screening tests. Although a combination strategy and cotesting strategy detected the same CIN2+ and CIN3+ cases, the former required significantly fewer screening tests. (C) 2018 Published by Elsevier Inc.

Abstract:Objectives: This article aims at exploring the clinical value of high-risk human papillomavirus (HPV) positive and paired boxed 1 (PAXI)/zinc finger protein 582 (ZNF582) gene methylation shunt as a new approach for accurate cervical cancer screening. Material and methods: Selecting 115 patients were treated in the Cervical Department of Xuzhou Matemal and Child Health Hospital from October 2018-October 2020. All patients underwent cervical exfoliated cell thinprep cytologic test (TCT) detection, HPV typing quantitative detection, and PAX1/ZNF582 gene methylation level Detection. Taking the biopsy pathological diagnosis under colposcopy as the gold standard, analyzing the test results statistically, and the sensitivity, specificity, and accuracy of the three screening methods alone and combined screening schemes were compared. Results: Comparison of the three methods of cervical exfoliated cell TCT, HPV typing and quantification, and PAX1/ZNF582 methylation gene detection showed that the gene detection method has the highest specificity, 97.30%; The HPV typing quantitative detection has the highest sensitivity, 89.71%, but its specificity is poor; and the PAX1/ZNF582 gene detection has the highest accuracy. Conclusion: For patients with high-grade cervical lesions and cervical cancer, PAX1/ZNF582 gene methylation level can be used as an important biomarker for the diagnosis and classification of cervical cancer. PAX1/ZNF582 methylation gene detection is effective in high-grade cervical lesions and cervical cancer. Screening has high clinical value and can become a new way of accurate cervical cancer screening.
Month of birth and risk of autism spectrum disorder: a retrospective cohort of male children born in Israel

Abstract:Background Increased incidence and prevalence of autism spectrum disorder (ASD) over the last two decades have prompted considerable efforts to investigate its aetiological factors. We examined an association between month of birth and ASD incidence. Methods In a retrospective cohort of male children born from January 1999 to December 2008 in a large health organisation in Israel (Maccabi Healthcare Services), ASD was followed from birth through December 2015. Results Of 108 548 boys, 975 cases of ASD were identified. The highest rates (10.3 and 10.2 per 1000 male live births) were recorded for children born in May and August, respectively, and the lowest rates for February (7.6 per 1000 male live births). Among lower socioeconomic status households, boys born in August were more likely (OR=1.71; 95% CI 1.06 to 2.74) of being diagnosed with ASD than children born in January. Significantly higher rates were not observed for other months. Conclusions In line with several previous studies, we found a modestly higher likelihood of autism occurrence among male children of lower socioeconomic levels born in August.

Abstract:Cervical cancer presents a significant health and economic threat to women. Efficient and accurate screening strategies are crucial for achieving "cervical cancer elimination". This study is a multi-center prospective study aimed at evaluating the application value of PAX1/JAM3 gene methylation (PAX1m/JAM3m) detection in cervical cancer screening. The research included a total of over 6000 women in gynecological outpatient clinics from May to October 2022. Cervical scraping cells were collected for PAX1m/JAM3m testing and compared with liquid-based thin-layer cytology testing (TCT) and high-risk human papillomavirus (hrHPV) DNA testing. Ethics registration number of Peking Union Medical College Hospital, Chinese Academy of Medical Sciences: KS2021211 (approved on April 25, 2021). The results showed that PAX1m/JAM3m detection has high AUC, sensitivity, and specificity in identifying CIN2+ (cervical intraepithelial neoplasia 2 or more severe) at 0.85, 74.1%, and 95.9%, and CIN3+ at 0.87, 87.6%, and 86.8%. The study also indicated that PAX1m/JAM3m detection has significant advantages over TCT or hrHPV testing in identifying CIN3+. Therefore, the study concluded that PAX1m/JAM3m detection has excellent accuracy in cervical cancer screening and is expected to replace cytology screening as a triage option for hrHPV-positive women in opportunistic cervical cancer screening in hospitals. Overall, the large-scale study highlights the high accuracy of PAX1m/JAM3m detection in opportunistic cervical cancer screening in hospitals, especially in the detection of CIN2+ and CIN3+. The results suggest that PAX1m/JAM3m detection has the potential to become a precise tool for cervical cancer screening in the future, which is of great significance for optimizing cervical cancer screening strategies. This method may become a promising alternative to traditional screening methods and provide a more accurate screening option for hrHPV-positive women in the future.
Diagnostic Value of Quantitative Detection of PAX1 Methylation for Cervical High Grade Precancerous Lesion

Abstract:Objective:To explore the clinical diagnostic value of quantitative detection of the paired‐boxed gene1 (PAX1 ) methylation for cervical high grade precancerous lesions .Methods:According to pathological diagnosis ,which was set as the gold standard ,122 patients infected with high risk subtype HPV were divided to the normal cervix group(group I ,n=39) ,the cervical low grade precancerous lesions group(group II ,n=40) ,and the cervical high grade precancerous lesions group(group III ,n=43) .The cervical tissue samples from each group were collected .The methylation level of PAX1 from each sample was measured by pyrosequencing and the statistical analysis was done .The diagnostic threshold of PAX1 methylation level for cer‐vical high‐grade precancerous lesions was verified by receiver operator characteristic(ROC) curve .Results:The PAX1 methyla‐tion level of group I ,group II ,group III was (4 .77 ± 2 .43)% ,(3 .75 ± 2 .36)% ,(13 .51 ± 5 .31)% ,respectively .And the differences among them were statistically significant (F=23 .13 ,P<0 .001) .The area under the ROC curve was 0 .73 .The diagnostic threshold of PAX1 methylation level for cervical high grade precancerous lesions was 4 .15% ,and its sensitivity and specificity was 89 .39% and 79 .15% ,respectively .Conclusions:Quantitative detection of PAX1 methylation level in cervical tissue by pyrosequencing could reach effective diagnosis for cervical precancerous lesions .

Abstract:Introduction: Methylation assays have demonstrated potential as dependable and high-precision approaches for identifying or triaging individuals with cervical cancer (CA) or cervical intraepithelial neoplasia (CIN). Our investigation aimed to assess the efficacy of diagnosis and triage of the PAX1/SOX1 methylation panel in detecting CIN or CA. Methods: A total of 461 patients with abnormal high-risk human papillomavirus (hrHPV) or cytology test results were recruited for this study. Each patient underwent an assortment of assessments comprising cytology test, hrHPV test, colposcopy examination, and PAX1 and SOX1 methylation test. Results: The extent of methylation of both genes demonstrates a positive correlation with the severity of CIN lesions and CA. To determine the correlation for patients with CIN2 or worse (CIN2+), the area under curve (AUC) was 0.821 (95%CI 0.782-0.853) for PAX1 and 0.800 (95%CI 0.766-0.838) for SOX1, while for CIN3 or worse (CIN3+),0.881 (95%CI 0.839-0.908) for PAX1 and 0.867 (95% 0.830-0.901) for SOX1. The PAX1/SOX1 methylation marker panel performed sensitivity and specificity of 77.16% and 91.67% for CIN2+, 84.76% and 90.50% for CIN3+, respectively. Regarding triaging hrHPV+ patients, the PAX1/SOX1 methylation test only referred 11.83% of the patients who are unnecessary for coloscopy examination, which is comparatively lower than cytology, thereby signifying a promising triage strategy for hr-HPV positive women. Furthermore, we observed that the positive PAX1/SOX1 methylation test result for untreated CIN1 or less patients would result in a higher likelihood of progression upon a 24-month follow-up visit. Conclusion: The present investigation demonstrates that the PAX1/SOX1 methylation marker panel exhibits a favorable diagnostic performance in CIN detection and holds the potential to be employed for individual CIN tests or hrHPV-positive triage.

Association of PECAM1/CD31 polymorphisms with cerebral malaria.

Abstract:Platelet/endothelial cell adhesion molecule-1 (PECAM1/CD31), a receptor recognized by P. falciparum-infected red blood cells (iRBCs), on the vascular endothelium has been implicated in mediating cytoadherence in patients with P. falciparum malaria. To examine associations of PECAM1 polymorphisms with cerebral malaria, 11 tag single nucleotide polymorphisms (SNPs) of PECAM1 were analysed for 312 Thai patients with P. falciparum malaria (109 with cerebral malaria and 203 with mild malaria). The rs1122800-C allele was significantly associated with protection from cerebral malaria (P = 0.017), and the rs9912957-A significantly increased the risk for cerebral malaria (P = 0.0065) in malaria patients. Fine-scale mapping using genotyped and imputed SNPs and linkage disequilibrium (LD) analysis revealed that rs1122800 and rs9912957 were located in two distinct LD blocks and were independently associated with cerebral malaria. The rs1122800-C allele was significantly associated with lower expression level of PECAM1 in EBV-transformed lymphoblastoid cell lines (P = 0.045). The present results suggest that PECAM1-mediated cytoadherence of iRBCs to brain endothelium plays a crucial role in the pathogenesis of cerebral malaria.

Abstract:Cervical cancer (CC) was considered to be the most common gynaecological cancer, with an estimated 342,000 deaths worldwide each year, as the majority of patients were diagnosed at an advanced stage of the disease. The purpose of this study was to evaluate the predictive value of multi-locus methylation assay for the early detection of CC. The cervical exfoliated cell samples from 492 HPV-positive females with cervical lesions were collected and subjected to methylation detection of gene FAM19A4, EPB41L3 and PAX1 after bisulfite conversion. The levels of gene methylation in patients with different severity of cervical lesions were evaluated and compared. The receiver-operating characteristic (ROC) curve was established and efficacy indexes such as sensitivity, specificity and area under the curve (AUC) were calculated to assess the diagnostic value of DNA methylation detection at multiple gene loci for CC. The methylation levels of FAM19A4, EPB41L3 and PAX1 were significantly increased with the grade of cervical squamous intraepithelial lesions. The sensitivities of FAM19A4, EPB41L3 and PAX1 alone for high-grade squamous intraepithelial lesion (HSIL) and CC diagnosis were 84.6%, 86.3% and 88.0%, respectively; when three markers were combined by a logistic regression model, the sensitivity was 88.0%, with a high specificity of 97.7% and AUC of 0.957 (95% CI 0.937–0.977). Methylation status of FAM19A4, EPB41L3 and PAX1 were highly specific and effective for monitoring the progression of cervical lesions and the tri-gene methylation assay could be used as a triage tool for CC early screening.

Abstract:Cytology triage has been generally recommended for human papillomavirus (HPV)-positive women, but is highly dependent on well-trained cytologists. The present study was designed to explore whether HPV E6/E7 mRNA detection in cervical exfoliated cells can be a potential triage for HPV-positive women from a clinic-based population. Both the primary HPV testing and Papanicolaou (Pap) test were performed on all eligible HPV-positive women. HPV E6/E7 mRNA was detected by QuantiVirus ® HPV E6/E7 mRNA assay in cervical exfoliated cells. All HPV-positive women underwent colposcopy and further biopsy if indicated. The data were assessed by Pearson’s Chi-squared test and the receiver operating characteristic curve. A total of 404 eligible HPV-positive women were enrolled. Positive rate of E6/E7 mRNA in high-grade squamous intraepithelial lesion (HSIL) cases was higher than that in low-grade squamous intraepithelial lesion (LSIL) or normal cases. There was no statistical difference found between mRNA and cytological testing with sensitivity (89.52% vs. 86.67%, P =0.671), specificity (48.96% vs. 48.96%, P =1.000), positive predictive value (39.00% vs. 38.24%, P =1.000), and negative predictive value (92.76% vs. 90.97%, P =0.678) for detecting ≥HSIL. HPV E6/E7 mRNA detection in cervical exfoliated cells shows the same performance as Pap triage for HSIL identification for HPV-positive women. Detection of HPV E6/E7 mRNA may be used as a new triage option for HPV-positive women.
Association and Effectiveness of PAX1 Methylation and HPV Viral Load for the Detection of Cervical High-Grade Squamous Intraepithelial Lesion

Abstract:Background: PAX1 methylation (PAX1m) and HPV viral load (VL) have been reported to detect cervical high-grade squamous intraepithelial lesions (HSIL), but the relationship between them is unclear. This study aimed to evaluate the correlation between HPV VL and PAX1m and its effectiveness in predicting cervical lesions. (2) Methods: A total of 476 women referred to colposcopy for abnormal cervical screening at the Peking University People’s Hospital between November 2020 and November 2021 were enrolled. PAX1m and HPV VL were determined by QMSP and BMRT-HPV reports type-specific VL/10,000 cells, respectively. (3) Results: PAX1m was significantly increased in HSIL, especially in cervical cancer, but there was no significant difference between cervical intraepithelial neoplasms 1(CIN1) and CIN2. However, HPV VL significantly differed between CIN1 and CIN2 but not between CIN3 and cervical cancer. In general, PAX1m positively correlated with all hrHPV VL, mainly in the HPV16/18 VL (p < 0.001), but had no relationship with the other 12 types of hrHPV VL. PAX1m had the highest specificity in diagnosing CIN2+, followed by HPV16/18 VL, which are higher than cytology ≥ASCUS. (4) Conclusions: Hypermethylation of PAX1 is associated with high HPV VL, especially HPV16/18, and both present advantageous specificity in detecting CIN2+.
Quantitative analysis of LMX1A and PAX1 gene methylation in cervical cancer and cervical intraepithelial neoplasia

Abstract:Background and purpose:DNA methylation is a common epigenetic alteration in cervical carcino-genesis. The aim of this study was to measure the levels of LMX1A and PAX1 gene methylation in cervical cancer and pre-cursors and to identify their potential in clinical application. Methods:Cervical specimens were collected from 121 female patients including 27 cases with invasive cervical cancers (ICC), 34 cases with high-grade cervical intraepithelial neoplasia (HG-CIN), 32 cases with low-grade cervical intraepithelial neoplasia (LG-CIN) and 28 cases with chronic cervicitis as normal controls (NLM). DNA methylations of the LMX1A and PAX1 gene were quantified using the techniques of nest PCR and pyrosequencing. The mean methylation values of the 6 gene loci on the LMX1A gene and the 9 gene loci on the PAX1 gene were respectively calculated for a given sample. Receiver operating characteristic (ROC) curve was used to evaluate the accuracy of gene methylation analysis to discriminate the cervical diseases. Results:The mean methylation value of the LMX1A gene in ICC was 14.36%, which was significantly higher than those in HG-CIN (4.70%), LG-CIN (5.05%) and NLM (4.53%) (P<0.01). The mean methylation value of the PAX1 gene in ICC was 41.97%, which was significantly higher than those in HG-CIN (10.21%), LG-CIN (5.55%) and NLM (4.92%) (P<0.01). The area under the ROC curve (AUC) was 0.603 for LMX1A gene methylation, and was 0.883 for PAX1 gene methylation, to discriminate ICC from HG-CIN, LG-CIN, and NLM (P=0.104 and<0.001, respectively). The optimal cut-off value for PAX1 gene methylation was set at 20.50%with the sensitivity of 81%and with the specificity of 93%. If the cut-off value was set at 9.58%, the sensitivity and the specificity of PAX1 gene methylation were 89%and 84%respectively. Conclusion:Quantitative analysis of the PAX1 gene methylation in cervical tissue might be helpful to differentiate invasive cancers from precursors, while the clinical applica-tion of the LMX1A gene methylation was limited.
DNA methylation marker for the triage of hrHPV positive women in cervical cancer screening: Real-world evidence in Taiwan

Abstract:Objective: Human papillomavirus (HPV) testing as the primary cervical cancer screening followed by reflex cytology if high-risk HPV is present (hrHPV+) is recently adopted in some countries. However, reflex cytology's sensitivity is variable, and a suitable triage approach for hrHPV+ remains controversial. Here, we compared the performance of three triage tools in hrHPV+ women. Methods: Three triage tools-cytology, HPV16/18 genotyping, and DNA methylation biomarker PAX1m-were analyzed for their clinical performance in hrHPV+ women. In addition, women without cervical cancer at enrollment were followed for histologically confirmed high-grade cervical intraepithelial neoplasia or worse (CIN3+) annually using Papanicolaou smear. Results: Of 4762 women aged ≥20 years enrolled, 502 (10.5%) were hrHPV+. PAX1m and cytology demonstrated similar accuracy (>90%), sensitivity (>78%), and specificity (>92%) as triage tools in 429 hrHPV+ women aged 30-64 years. PAX1m had better accuracy and specificity (91.6% and 92.5%, respectively) than HPV16/18 (76.9% and 76.8%, respectively). The incidence of CIN3+ among hrHPV+ women was 10.7 cases/1000 person-years. The incidence was significantly greater in PAX1m-positive women than in PAX1m-negative women. Conclusions: PAX1m has comparable clinical performance to cytology and better accuracy and specificity than HPV16/18 as the triage tool for detecting CIN3+ in hrHPV+ women. The PAX1m assay is thus a promising molecular-based triage tool for early detection of CIN and predicting disease progression in hrHPV+ women. It can be especially useful in countries where adequate cytology-based infrastructure is lacking, such as some Southeast Asian countries, for cervical cancer screening and prevention.
High - grade cervical lesions diagnosed by JAM3/PAX1 methylation in high - risk human papillomavirus - infected patients

Abstract:Objectives: Currently, traditional cervical cancer screening methods, such as high-risk human papillomavirus testing and liquid based cytology (LBC), still possess limitations. This study aims to identify new diagnostic biomarkers to achieve the goal of "precision screening" via exploring the clinical value of DNA methylation [ΔCtP: paired box gene 1 (PAX1)and ΔCtJ: junctional adhesion molecule 3 (JAM3)] detection in cervical exfoliated cells for the diagnosis of high-grade cervical lesions. Methods: A total of 136 patients who underwent gynecological examinations in the vaginal room of the Department of Gynecology at the Third Xiangya Hospital of Central South University from June 2021 to June 2022 were retrospectively studied. Among them, 122 patients had non-high-grade cervical lesions, and 14 patients had high-grade cervical lesions. The variables included general information (age, body mass index, and menopause status), LBC, high-risk human papillomavirus, cervical tissue pathology, vaginal examination results, and the ΔCt values of JAM3 and PAX1 gene methylation. Logistic regression analysis was used to identify the factors affecting the diagnosis of high-grade cervical lesions, followed by correlation analysis and construction of a conditional inference tree model. Results: Logistic regression analysis showed that the methylation ΔCt values of PAX1 and JAM3 genes and LBC detection results were statistically significant between the high-grade cervical lesions group and the non-high-grade cervical lesions group (all P<0.05). Correlation analysis revealed a negative correlation between cervical pathological changes and ΔCtP (r=-0.36, P<0.001), ΔCtJ (r=-0.448, P<0.001), LBC (r=-0.305, P<0.001), or bacterial diversity (r=-0.183, P=0.037). The conditional inference tree showed that when ΔCtJ>10.13, all of patients had non-high-grade cervical lesions, while ΔCtP>6.22, the number of non-high-grade lesions accounted for 97.5% (117/120), and high-grade lesions accounted for only 2.5% (3/120). When ΔCtJ>8.61 and LBC were atypical squamous cell of undetermined significance or negative for intraepithelial lesions or malignancy (NILM), 105 (99.1%) patients were non-high-grade cervical lesions, only 1 (0.9%) patient was high-grade lesion. When the results of LBC were high-grade lesions, only 9 patients' histopathological examination was the high-grade lesions and 3 non-high-grade lesions. When LBC indicated low-grade lesions, atypical squamous cell of undetermined significance, no intraepithelial lesions, and ΔCtP>6.22, 117 (97.5%) of patients' histopathological examination was the non-high-grade lesions. Conclusions: The JAM3/PAX1 gene methylation test can be used independently for the stratified diagnosis of high-grade/non-high-grade cervical lesions in women with high-risk human papillomavirus infection, independent of the cytological results of cervical excision. The JAM3/PAX1 gene methylation test can also be used in combination with LBC to make up for the shortcomings of low sensitivity of LBC. In addition, the application of methylation kit in large-scale cervical cancer screening in the future will be good to the detection of more patients with high-grade cervical lesions, and achieve early screening and early treatment for cervical lesions/cancer.
Diagnostic accuracy of extended HPV DNA genotyping and its application for risk-based cervical cancer screening strategy

Abstract:Objectives: To evaluate the consistency of 14 high-risk HPVs (hr-HPVs) detection between extended HPV DNA genotyping and a well-validated partial HPV genotyping kit, and to explore the diagnostic accuracy of risk stratification strategy based on extended HPV genotyping for cervical cancer (CC) screening.Methods: Baseline data from a clinical trial of recombinant HPV 9-valent vaccine in China was analyzed. All enrolled women aged 20-45 years received cervical cytology, HPV detection by extended and partial HPV genotyping kits. Those who met the indications would further receive colposcopy. The primary endpoints were cervical intraepithelial neoplasia 2/3 or worse (CIN2+/CIN3+).Results: A total of 8,000 women were enrolled between April 2020 and July 2020 and 83/33 cases were diagnosed as CIN2+/CIN3+. The overall agreement between the extended and partial HPV genotyping was 92.66 %. And the agreement further increased with the progression of lesions, which lead to similarly high sensitivity and negative predictive value of these kits. A stratified triage strategy of CC screening was constructed based on the immediate CIN2+/CIN3+ risk of specific HPV. Compared with the conventional HPV primary CC screening strategy, the risk-based strategy had higher specificity for CIN (CIN2+: 94.84 vs. 92.46 %, CIN3+: 96.05 vs. 91.92 %), and needed fewer colposcopies for detecting one cervical disease.Conclusions: Extended HPV genotyping had good agreement with a well-validated partial HPV genotyping CC primary screening kit in hr-HPV detection. Extended HPV genotyping could facilitate risk-based stratified management strategy and improve the diagnostic accuracy of primary CC screening.

Abstract:PurposeThe aim of this study was to investigate the clinical performance of high risk (HR) HPV E6/E7 mRNA assay in detecting cervical high-grade intraepithelial neoplasia and cancer among women with atypical squamous cells of undetermined significance (ASCUS) Papanicolaou (Pap) smears.MethodsA total of 160 patients with ASCUS who underwent HR-HPV DNA assay, HR-HPV E6/E7 mRNA assay and colposcopy biopsy at Third Affiliated Hospital of Zhengzhou University, China, from December 2015 to March 2017, were enrolled. Logistic regression analysis was used to evaluate the relationship between pathological results with clinical biologic factors.ResultsUnivariate analysis showed that the qualitative results of HR-HPV DNA, qualitative results of HR-HPV E6/E7 mRNA and expression levels of HR-HPV E6/E7 mRNA were risk factors of high-grade cervical intraepithelial neoplasia (CIN) and cervical cancer (all P < 0.05). Multivariable analysis found that only the expression levels of HR-HPV E6/E7 mRNA was associated with high-grade CIN and cervical cancer (OR = 8.971, 95% CI = 2.572–31.289, P = 0.001). An optimal cut-off value of ≥ 558.26 copies/ml was determined using receiver operating characteristic curve, and specificity of cut-off value were higher than E6/E7 mRNA qualitative assay and DNA qualitative assay.ConclusionHPV E6/E7 mRNA quantitative assay may be a valuable tool in triage of ASCUS pap smears. A high specificity of E6/E7 mRNA quantitative assay as a triage test in women with ASCUS can be translated into a low referral for colposcopy.

Abstract:Objective: The aim of this study was to clinically evaluate the application of liquid-based cytology P16, cytologic evaluation, and high-risk human papillomavirus (HR-HPV) testing in cervical cancer screening. Methods: This study screened 900 women, who attended the outpatient clinic, according to the exclusion criteria of study participants. The study participants' screening results of liquid-based cytology P16, cytologic evaluation, and HR-HPV testing were analyzed. According to the pathological results of the biopsy, the efficacy of different screening strategies for the identification of highgrade lesions was evaluated. Results: The positive rate of p16 expression increased with the severity of cervical lesions. P16 had the highest sensitivity and negative predictive value in identifying high-grade lesions (98.45% and 99.67%, respectively). Liquid-based Papanicolaou test (LBP), on the other hand, had the lowest sensitivity (85.27%) but the highest specificity (85.88%). HR-HPV's positive predictive value and accuracy rate were the lowest (32.77% and 70.03%, respectively). The difference was statistically significant (P < 0.05). Dual combinations of certain tests were set up for this study; P16+LBP, HPV+LBP, and P16+HPV had sensitivities of 98.45%, 96.90%, and 99.22%, and specificities of 80.29%, 63.42%, and 64.33%, respectively. The P16 screening rates of histological and liquid-based cytology approaches were 75.74%. Conclusion: Compared with traditional LBP+HPV, the application of a test that solely screen for P16 or the combined screening method that involves the screening of P16 is more effective in identifying high-grade lesions.

Abstract:Objective:To assess the clinical sensitivity and specificity of the APTIMA HPV E6/E7 mRNA assay ( A-HPV ) and MALDI-TOF-MS HPV genotyping assay ( M-HPV ) for the detection of high-grade cervical lesions. Methods:The study populations were enrolled from Shenzhen,2095 women ages 25~59,non-pregnant,no screening in <3 years,no hysterec-tomy or pelvic irradiation. Cervical sample obtained for SurePath cytology. A second sample col-lected in PreservCyt liquid( PC) was used for Hybrid CaptureⅡassay( HC-Ⅱ) ,APTIMA E6/E7 HPV mRNA assay ( A-HPV ) and MALDI-TOF MS HPV genotyping assay ( M-HPV) . Par-ticipants positive on any test were recalled for colposcopy and biopsy. All participants colpo-scoped had directed and random biopsies plus ECC( min. 5 bx/patient) . Histology confirmed >CINⅡ+ served as the gold standard. Results:1970 cases among of 2095 women had all test re-sults. Mean age (SD)=35. 89(7. 6),The≥ASCUS prevalence was 6. 4% (127/1970),Biop-sy≥CINⅡ1. 3% (26/1970) and ≥CINⅢ0. 76% (15/1970). Overall HPV positivity was 19. 4%,12. 1% and 14. 8% for HC-Ⅱ,A-HPV and M-HPV respectively (P<0. 05). Clinical sensitivity for detection of ≥CINⅡ were 88. 5% (68. 7 ~97. 0),100% (84. 0 ~100) and 92. 3% (73. 4~98. 7) for HC-Ⅱ,A-HPV and M-HPV,respectively. A-HPV was slightly high-er than HC-Ⅱ. There was not statistically significant between HC-Ⅱand M-HPV. The specifici-ty for ≥CINⅡwas 81. 5%(79. 7~83. 2),89. 1%(87. 6~90. 4) and 86. 3%(84. 6~87. 8) for HC-Ⅱ, A-HPV and M-HPV respectively ( P>0 . 05 , not statistically significant ) . Conclu-sions:The A-HPV and M-HPV has good sensitivity,accuracy,and has lower rate of biopsy com-pared to the HC-Ⅱ for detection of≥CINⅡ lesions ( P<0 . 05 ) . Which may improve patient management and reduce the cost of care. High risk HPV genotyping combined HPV E6/E7 m RNA can identify women at highest risk for high-grade cervical disease.
[Evaluation of P16(INK4α) Expression in Thinprep Cervical Specimens for the Predication of High-Grade Cervical Intraepithelial Neoplasia].

Abstract:OBJECTIVE:to explore the value of P16 in the predication of high-grade cervical intraepithelial (HGCIN) by P16 expression in cervical specimens. METHODS:one hundred ninety-two residual ThinPrep samples were collected and detected by HPV DNA test and P16 detected by immunocytochemistry. All women underwent colposcopy and histological examination of biopsy specimen if needed. P16 test, cytology and HR-HPV DNA (HC2) test were compared based on histological examination of colposcopic biopsies. RESULTS:(1) the expression of P16 showed 16.3% in normal or inflammatory cases, 46.7% in CIN 1, 93.8% in CIN 2, 91.1% in CIN 3 and 100.0% in carcinoma. A positive relation between P16 and the grade of cervical lesions was observed by Spearman analysis (P < 0.05, r = 0.900). (2) By P16 test, the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy for ≥ CIN 2 were 94.1%, 78.5%, 77.7%, 94.4% and 85.4% respectively. By HR-HPV DNA test, the sensitivity, specificity, PPV, NPV and accuracy for ≥ CIN 2 were 95.3%, 56.1%, 63.3%, 93.8% and 73.4% respectively. By cytological test, the sensitivity, specificity, PPV, NPV and accuracy for ≥ CIN 2 were 82.4%, 92.5%, 89.7%, 86.8% and 88.0% respectively. There were significant difference of specificity, PPV and accuracy between P16 and HR-HPV DNA (P < 0.05). And no significant difference of accuracy was found between P16 and cytology (P > 0.05). CONCLUSION:the specificity, PPV and accuracy of P16 are significantly higher than those of HR-HPV DNA. Thus P16 test is valuable to diagnose HGCIN in ThinPrep specimens.

Abstract:BackgroundIt is necessary to differentiate high-risk human papillomavirus (HR-HPV) infection and cervical lesions during pregnancy using an effective measure, so as to guide personalized diagnosis and treatment and to reduce unnecessary invasive examinations during pregnancy.ObjectiveTo assess the value of HR-HPV E6/E7 mRNA detection in differentiating HR-HPV infection and cervical lesions during pregnancy by comparing efficacy between it and HR-HPV DNA testing.MethodsParticipants were healthy women with singleton pregnancy (20-45-years old) selected from Department of Obstetrics and Gynecology of Capital Medical University Xuanwu Hospital during January 2016 to January 2019. All had file creation and underwent regular prenatal examination, and those with abnormal liquid-based cytology and HR-HPV DNA testing results further underwent colposcopy with biopsy taken for pathological examination (serving as a gold standard) , and performed HR-HPV E6/E7 mRNA detection in cervical exfoliated cells collected as samples. Pathologically detected CIN Ⅱand Ⅲ were defined as high-grade cervical lesions.ResultsOf the 1 058 participants, 118 had cytological abnormalities and/or HPV 16 and 18 infections, and 84 of them consented to perform colposcopy with biopsy pathological results successfully obtained. The prevalence of HR-HPV E6/E7 mRNA positivity was lower than that of HR-HPV DNA positivity in women with CINⅠ, normal cervical epithelium or cervicitis detected by pathological examination (P<0.05) . In contrast, the prevalence of HR-HPV E6/E7 mRNA positivity was similar to that of HR-HPV DNA positivity in those with CINⅡ and Ⅲ detected by pathological examination without statistical difference (P>0.05) . In predicting CINⅡ and Ⅲ, the HR-HPV DNA testing had a sensitivity of 89.7% (26/29) , a specificity of 21.8% (12/55) , a positive predictive value of 37.1% (26/69) , and a negative predictive value of 75.0% (12/29) , and the HR-HPV E6/E7 mRNA detection had a sensitivity of 65.5% (19/29) , a specificity of 54.5% (25/55) , a positive predictive value of 43.0% (19/44) , and a negative predictive value 75.0% (25/40) . McNemar's test revealed that HR-HPV E6/E7 mRNA detection had a lower sensitivity but a higher specificity than HR-HPV DNA testing in diagnosing CINⅡ and Ⅲ (P<0.05) .ConclusionHR-HPV E6/E7 mRNA detection may have an increased specificity in diagnosing CINⅡandⅢ than HR-HPV DNA testing, so it may be used in HR-HPV positive cases for differentiating HR-HPV infections and cervical lesions to avoid unnecessary invasive examinations during pregnancy.

Abstract:BACKGROUND: Detection of E6 and E7 mRNA load of high-risk human papillomavirus (HR-HPV) infection during pregnancy was compared with conventional cytopathology and DNA detection by pathological examination as colposcopy to evaluate the application of E6 and E7 mRNA detection in the diagnosis and management in HR-HPV infection for high -grade cervical lesions during pregnancy.METHODS: From January 2014 to June 2019, 1,058 pregnant women of childbearing age who were filed for regular obstetrics in the Department of Obstetrics and Gynecology, Xuanwu Hospital of Capital Medical University, were separately assessed using cervical liquid-based cytology and HPV DNA detection. If the results were abnormal, colposcopy was performed as a follow-up. The presence of HR-HPV E6/E7 mRNA fragments was detected through the HR-HPV E6/E7 mRNA test, and monitored at the same time as colposcopy. The diagnostic efficacy of the HR-HPV DNA test versus the HR-HPV E6/E7 mRNA test for high-grade cervical lesions during pregnancy was compared.RESULTS: The positive rate of HR-HPV E6/E7 mRNA detection in the overall cervical intraepithelial neoplasia (CIN) and above during pregnancy was lower than that with HR-HPV DNA detection, and there was a significant statistical difference between the two methods. In CIN I and normal or inflammatory results, the positive rate of HR-HPV E6/E7 mRNA detection was lower than that of HR-HPV DNA detection, while in the results of CIN II and CIN III, the positive rate of the two was not significantly different. HR-HPV E6/E7 mRNA detection is the same as HR-HPV DNA detection, both of which increased with the severity of cervical lesions, and the positive rate increased. In cases of maintenance or progression of cervical lesions, the positive rate of HR-HPV E6/E7 mRNA detection during pregnancy can reach 81.8%. High-grade cervical lesions during pregnancy had a higher rate of reversal to a lower level after delivery.CONCLUSIONS: The results suggested that the use of HR-HPV E6/E7 mRNA detection in cases of positive HR-HPV DNA detection can significantly improve the diagnostic specificity of CIN II and above high-grade cervical lesions.
