Abstract:Purpose: Novels and autopathographies that employ cancer as a central theme offer a wealth of opportunities for researching the way patients with cancer make sense of their illness and its treatment. Such literatures can also inform clinical care, because they can support patients in living with their illness. The use of novels and autopathographies for research and care in persons with cancer fits within the framework of 'Health Humanities', the interdisciplinary field where medicine and social science meet. This paper presents a concise overview of novels and autopathographies that explore cancer as their theme. Methods: Literature searches were conducted using PubMed, major scientific journals of medicine and clinical oncology, and databases in the Health Humanities. Searches focused on novels and autopathographies where cancer is the central theme, which are available in English, and which can be considered to represent 'high literature'. Results: Twenty-nine books were identified. The majority of the books were written originally in English, and breast cancer and lung cancer were the most frequently discussed types of cancer. The core themes identified were giving meaning to illness; coping with medical treatment; and the psychological and social consequences of illness. Conclusion: Novels and autopathographies about cancer represent an innovative base for research on living with cancer and offer rich data on how people make sense of cancer and its medical treatment. Clinical implications of this review pertain to interventions based on bibliotherapy and expressive writing. Novels and autopathographies are just part of the Health Humanities context: a wide range of art genres may prove helpful in improving the quality of life of persons with cancer.

Abstract:This article presents and interprets illness narratives, told by patients treated for cancer. The aim is to discuss the meaning of cancer metaphors. The interpretation focuses on the relationships of metaphors to bodily experience and to social and cultural levels. The experience of cancer and cancer therapy obviously involve the patient's body. To comprehend what is happening is a process full of uncertainty and obscurity. Metaphors, such as cancer "eating," are widespread and persistent while the meaning of that metaphor today mirrors new scientific and lay explanations of cancer and cancer treatment. Personal newly created metaphors express threatening experiences in which fear of losing oneself and fear of death are involved. Ambivalent hope in relation to treatment and caregivers is expressed. Creating new metaphors involves imaginatively using the available concepts and metaphors. Narrative communication gives access to patients' experiences of an unwanted and painful physiologic process leading to forced embodiment of sickness. Patients want consolation and must overcome solitude by articulating experience, being listened to, and, in this way, recreating and strengthening identity.

Abstract:Cancer is a relatively new subject for the discipline of anthropology, but scholarship on the topic has already yielded a distinct and important body of literature. In biomedical terms, cancer can be thought of as the wide range of conditions characterized by the uncontrolled (and ultimately pathological) proliferation of cells. It is a disease that is responsible for the deaths of millions of people worldwide each year. As such, it is the focus of a vast number of discourses and practices in multiple areas, ranging from scientific research and media discussion to health insurance and government regulation, to name just a few. Anthropologists concerned with cancer typically use the methodology that is a hallmark of the discipline, long-term ethnographic fieldwork, in order to investigate these discourses and practices. This involves conducting participant observation among doctors, patients, nurses, family members, scientists, politicians, policymakers, and pharmaceutical representatives. Cancer is examined as a lived experience, revealing the numerous ways that local, regional, national, and transnational histories and politics shape the embodied realities of disease. Anthropologists also investigate the regimes of risk and statistical analysis to which bodies are subjected and the technologies around cancer, such as methods of screening or vaccination that aim to prevent it and the different ways in which these and other interventions and technologies fit into—or push uneasily against—the local words in which they are implemented. Anthropologists aim to look beyond the problem as simply one of biology and medicine, instead investigating cancer as pervasive within multiple dimensions of social, cultural, political, and economic life. Anthropological studies displace the prominent biomedical notion that cancers are the same in diverse locations and reveal the incoherence and intractability of cancer as an object. In paying close attention to this object in varied settings, anthropologists offer a critical account of discourses and practices that destabilize and decenter some of the assumptions on which global oncology is based.

Abstract:Being diagnosed with cancer can be one of those critical incidents that negatively affect the self. Identity is threatened when physical, psychological, and social consequences of chronic illness begin to erode one's sense of self and challenge an individual's ability to continue to present the self he or she prefers to present to others. Based on the notion of illness trajectory and adopting a Ricoeurian narrative perspective, this theoretical paper shall explore the impact of cancer disease on identity and establish the crucial importance of metaphor in the narratives of life with cancer. Findings indicate that in cancer narratives the illness experience supplies the narrative structure with temporal and spatial meeting points that make the narrative comprehensible and meaningful. Cancer forces identity changes not only from having to endure the long-term physical and psychosocial effects of the disease, but also from inevitable existential questions about life's meaning. Improved medical knowledge today means improved ethnomedical practices. Metaphor can bridge the gap between the cancer experience and the world of technology and treatment, helping patients to symbolically control their illness.

Abstract:How do metaphors shape and reflect patients' experience of cancer? How can repeated immersion in challenging poetry affect our understanding of cancer and other diseases? This essay addresses these two questions through an exploration of the varied and sometimes contradictory metaphors crafted by poet and seven-time cancer patient Judy Rowe Michaels. Although cancer is often regarded as an enemy intrusion to be eradicated, it is an illness in which the threat to life comes from within the self. Michaels's metaphors complicate the relationship between an embodied self and the cancerous cells the body produces, presenting multifarious responses to living with cancer. Defying expectations of a linear trajectory from symptom to diagnosis to treatment to recovery, these metaphors provide a means to understand the changing emotions; the changing relationship to illness, to wellness, and to self; and the changing status as patient (symptomatic, diagnosed, in treatment, in remission, in recurrence) that characterize the lived experience of cancer-and of other recurring or chronic diseases. Examining these poems demonstrates the value for practitioners, patients, and families of honing a poet's mode of understanding, as a vital complement to the mode of thinking that currently drives health care.

Abstract:The use of metaphors to talk about cancer experiences has attracted much research and debate, especially in the case of military metaphors. However, questions remain about what metaphors are used by different populations for different aspects of the cancer experience. This scoping review aims to answer them.We searched PubMed, PsycINFO, CINAHL, Scopus and Web of Science databases. Eligible sources include peer-reviewed scientific research published in English between 2013 and 2023, investigating patterns of metaphor use from adult populations (age ≥18) for cancer-related topics, such as cancer itself, the general experience of being ill, treatment, and people and relationships.Out of 1929 articles identified, 30 met the criteria, spanning over different populations. While most papers focused on cancer in general, some focused on specific cancer types, such as breast cancer. Both spontaneous and elicited data were collected in ten languages: mostly English (N=12), Swedish (N=3) and Arabic (N=3). The identified metaphors were subsumed under various broad categories, including particularly violence and journey. Other categories include education and non-human animate entity for the cancer itself, confinement and deprivation and cleanliness for the general experience of being ill with cancer, Poison and gardening for cancer treatment, and distance for patients' social relationships.It was found that metaphors help to identify how patients describe experiences of vulnerability and empowerment. To provide patient-centred care, clinicians and researchers should avoid blanket conclusions about helpful or unhelpful metaphors, but consider the ways in which different metaphors are used by different populations in different contexts.
Liminality: A major category of the experience of cancer illness

Abstract:Narrative analysis is well established as a means of examining the subjective experience of those who suffer chronic illness and cancer. In a study of perceptions of the outcomes of treatment of cancer of the colon, we have been struck by the consistency with which patients record three particular observations of their subjective experience: (1) the immediate impact of the cancer diagnosis and a persisting identification as a cancer patient, regardless of the time since treatment and of the presence or absence of persistent or recurrent disease; (2) a state of variable alienation from social familiars, expressed as an inability to communicate the nature of the experience of the illness, its diagnosis and treatment; and (3) a persistent sense of boundedness, an awareness of limits to space, empowerment and available time. These subjectivities were experienced in varying degree by all patients in our study. Individual responses to these experiences were complex and variable. The experiences are best understood under the rubric of a category we call "liminality". We believe that all cancer patients enter and experience liminality as a process which begins with the first manifestations of their malignancy. An initial acute phase of liminality is marked by disorientation, a sense of loss and of loss of control, and a sense of uncertainty. An adaptive, enduring phase of suspended liminality supervenes, in which each patient constructs and reconstructs meaning for their experience by means of narrative. This phase persists, probably for the rest of the cancer patient's life. The experience of liminality is firmly grounded in the changing and experiencing body that houses both the disease and the self. Insights into the nature of the experience can be gained from the Existentialist philosophers and from the history of attitudes to death. Understanding liminality helps us to understand what it is that patients with cancer (and other serious illnesses) seek from the system to which they turn for help. Its explication should therefore be important for those who provide health care, those who educate health care workers and those concerned to study and use outcomes as administrative and policy making instruments.

Abstract:The unravelling of the human genome created new perceptions of the origin and evolution of diseases, and for cancer in particular, it established the notion that neoplasia has been a companion of life since its appearance on Earth. It is not surprising that neoplasms, in various forms, develop in numerous species of animals and even in plants. Unmistakable accounts of cancer with clinical features as are understood today begin in the 5th c. B.C. The principles and practice of the Hippocratic and Galenic tradition dominated cancer care virtually into the 20th century. Advanced sequencing technologies at the dawn of the 21st century generated new therapeutic opportunities with immunotherapy, oncolytic virotherapy, and gene transfer, with the latter especially being used in cases of hereditary cancer.

What Does Financial Literacy Training Teach Us?

Abstract:The authors use data from a finance-related theme park to explore how financial education changes investment, financing, and consumer behavior. Students were assigned fictitious life situations and asked to create household budgets. Some students received a 19-hour financial literacy curriculum before going to the park, and some did not. After controlling for demographic variables, the authors show that the treatment effects of the financial literacy program are strong. Students were more frugal, delayed gratification, paid off debt faster, and relied less on credit financing after training. Students who attended training showed greater uptake of decision support that was offered in the park, which indicates that decision support and financial literacy training are complements, not substitutes.

Abstract:Introduced to U.S. medical schools in 1972, the field of literature and medicine contributes methods and texts that help physicians develop skills in the human dimensions of medical practice. Five broad goals are met by including the study of literature in medical education: 1) Literary accounts of illness can teach physicians concrete and powerful lessons about the lives of sick people; 2) great works of fiction about medicine enable physicians to recognize the power and implications of what they do; 3) through the study of narrative, the physician can better understand patients' stories of sickness and his or her own personal stake in medical practice; 4) literary study contributes to physicians' expertise in narrative ethics; and 5) literary theory offers new perspectives on the work and the genres of medicine. Particular texts and methods have been found to be well suited to the fulfillment of each of these goals. Chosen from the traditional literary canon and from among the works of contemporary and culturally diverse writers, novels, short stories, poetry, and drama can convey both the concrete particularity and the metaphorical richness of the predicaments of sick people and the challenges and rewards offered to their physicians. In more than 20 years of teaching literature to medical students and physicians, practitioners of literature and medicine have clarified its conceptual frameworks and have identified the means by which its studies strengthen the human competencies of doctoring, which are a central feature of the art of medicine.

Abstract:The low‐latency extension was introduced in DVB‐SH (digital video broadcast‐satellite to handheld) to permit the transmission of latency‐sensitive data in S‐band Mobile Interactive Multimedia (S‐MIM) such as signaling and acknowledgements alongside the latency‐insensitive data such as broadcast streams. This enables existing receivers to still receive the latency‐insensitive data. This compatibility to existing receivers was achieved by exploiting a periodicity in the interleaved data. Abstract:OBJECTIVES Enterococcus faecalis is a human commensal that has the ability to become a pathogen. Because of its ruggedness, it can persist in the hospital setting and cause serious nosocomial infections. E. faecalis can acquire multiple drug resistance determinants but is also intrinsically tolerant to a number of antibiotics, such as penicillin or vancomycin, meaning that these usually bactericidal drugs only exhibit a bacteriostatic effect. Recently, evidence has been presented that exposure to bactericidal antibiotics induced the production of reactive oxygen species in bacteria. Here, we studied the role of enzymes involved in the oxidative stress response in the survival of E. faecalis after antibiotic treatment. METHODS Mutants defective in genes encoding oxidative stress defence activities were tested by time-kill curves for their contribution to antibiotic tolerance in comparison with the E. faecalis JH2-2 wild-type (WT). RESULTS In killing assays, WT cultures lost 0.2 +/- 0.1 and 1.3 +/- 0.2 log(10) cfu/mL after 24 h of vancomycin or penicillin exposure, respectively. A deletion mutant of the superoxide dismutase gene (DeltasodA) exhibited a lack of tolerance as cultures lost 4.1 +/- 0.5 and 4.8 +/- 0.7 log(10) cfu/mL after 24 h of exposure to the same drugs. Complementation of DeltasodA re-established the tolerant phenotype. Bacterial killing was an oxygen-dependent process and a model is presented implicating the superoxide anion as the mediator of this killing. As predicted from the model, a mutant defective in peroxidase activities excreted hydrogen peroxide at an elevated rate. CONCLUSIONS SodA is central to the intrinsic ability of E. faecalis to withstand drug-induced killing, and the superoxide anion seems to be the key effector of bacterial death.

Abstract:Qualitative Health Research, Ahead of Print. This study feeds into ongoing discussions on the metaphors used by cancer patients. Its aim is to explore how women living with a history of breast cancer use metaphors to express and interpret the experience of cancer remission. Data were collected in interviews designed to capture a rich and metaphorical description of participants' experiences with breast cancer and what these experiences mean to them. Ten participants were recruited. An interpretative phenomenological analysis of the participants' narratives highlighted a central metaphor: the cancer trace in one's life. The participants had to adapt to four specific traces of cancer: (1) the identity trace, (2) the existential trace, (3) the bodily trace, and (4) the narrative trace. We discuss how cancer challenges one's sense of biographical continuity and initiates a search for a new way of being. We also discuss how the metaphor of the trace differs from the metaphor of the cancer hero living without any trace of cancer.

Abstract:Journals aim to improve their ‘Impact Factor’ by publishing articles of outstanding merit that as a result are widely cited. Could it be, however, that some articles achieve citations based on controversy rather than good science, and do they as a result achieve publication prominence based on the Impact Factor generating potential? The juxtaposition of a statement on conflict of interest with a very dubious article on testosterone replacement places the conflict responsibility on authors without a mention of the vested interest (real, potential or imagined) of the journals (1,2). In this study, frail elderly men of mean age 74 years who had limited mobility were randomised to receive either testosterone gel or placebo. The objective was to see if testosterone could improve mobility, which it did, but the study was stopped prematurely because major cardiovascular events were noted to be more frequent in the testosterone compared with the placebo group. The results appear alarming until the data is scrutinised – the testosterone group had more pre-existing cardiovascular risk factors such as hypertension or hyperlipidaemia (including statin therapy), and cardiovascular disease was not monitored directly. Added to this, the target testosterone was 17.4 nmol ⁄ l (in frail old men!) that required dosage regimes initiated outside the recommended starting regime and in 15% above the approved dose regime. The findings are at variance with other studies that show no cardiovascular signal – and this combined with no consistent pattern of cardiovascular events – strongly suggests that the differences were attributable to chance alone (3,4). So, was it responsible to publish even with a balanced linked editorial? (5) Citations yes, but value? Responsible publishing is further called into question by a meta-analysis of angiotensin-receptor blockers (ARBs) and cancer (6). This is an article of very little, if any, scientific merit that has had important implications for patient adherence to therapy as a result of media interpretation being, as usual, sensational. What is most concerning is the failure of the authors to follow meta-analysis guidelines and to point out that cancer screening at entry was not part of the trial protocols, yet they suggest a ‘modest but significant’ link between ARBs and cancer. Lancet Oncology, but not The Lancet, accepted the article and its Impact Factor will have certainly been enhanced, but was this responsible editing? It certainly could be considered irresponsible to have the insubstantial, uncritical linked editorial written by someone very closely linked academically to the authors (both from Cleveland Ohio) (6). Just considering the pathophysiology, how can a drug exposure with ARBs for 2–4 years (the trial’s duration) be in any way linked to cancer, unless cancer was undetected and not screened for at study entry. We have been this way before with calcium antagonists that we were virtually alone in defending (7). Calcium supplements in people with osteoporosis are now questioned as a cause of myocardial infarction (MI), but not stroke, death or the composite endpoint of MI, stroke or sudden death (8). The absence of a link between MI and mortality coupled with none of the studies in the meta-analysis having cardiovascular outcomes as primary end points with no systematic gathering of cardiovascular data does make you wonder what is the point of it all. Armchair science when you have nothing better to do – painting the shed may be more constructive. Well, I suppose I have fallen into the spin-bowler’s trap and cited the articles as the journals wanted. There is, however, a bigger picture here – these studies have important patient impact as well as journal impact, and we do a disservice to our patients if we prioritise highly doubtful analyses. Journals need to be just as responsible as authors.

Abstract:Cancer causes one of the most deaths due to a disease. World Health Organization (WHO) accounting more than 100 countries estimated that the first or second leading cause of death before the age of 70 years is due to suffering from cancer. Worldwide survey counted 19.3 million new cancer cases and almost 10.0 million cancer deaths has been estimated recently. It is a second leading cause of deaths in the United States having more than 1.7 million people diagnosed with cancer and almost 600000 people have died due to the disease. Finding a cure for cancer is one of the greatest challenges over the decades of researches globally until now. Through the ages of time, with the help of enormous research studies, great knowledge on cancer disease has been accumulated. With the advancement of modern technology, understanding on cancer has been partially unwound into a great detail of invisible molecular particles. However, the knowledge of cancer disease and the treatments developed in the ancient times without any of these advanced modern technologies, will keep us amazed always. Are those ancient human beings more intelligent than the modern mankind? Reading these articles cited in this review article makes us wonder.

Abstract:This paper explores the cultural representations of cancer in popular Hollywood films released between 1930 and 1970. These cinematic treatments were not representative of the types of cancer that increasingly afflicted Americans, nor were filmmakers and studios concerned with realistic representations of the disease, its treatment, and its outcomes. As in the "epidemic entertainments" of the early twentieth century that portrayed diseases as cultural commodities, popular filmmakers selectively projected some cancers rather than others, favoring those that were less offensive and more photogenic. Although the characters became weak and died, they did so without gross transformations of their bodies. This paper argues that such representations nonetheless informed American attitudes about cancer and the role of medical research in overcoming the disease.
