Clinical Observations on Auricular Point Plaster Therapy for Insomnia

Abstract:Objective To investigate the clinical efficacy of auricular point plaster therapy for insomnia.Method Ninety patients with insomnia of heart-spleen deficiency type were randomly allocated to treatment and control groups,45 cases each.The treatment group received auricular point plaster therapy and the control group,conventional acupuncture.Five main clinical symptoms: difficulty in falling asleep,insufficient sleep time,dreaminess,easy waking up and lassitude were scored in the two groups.The clinical therapeutic effects were compared between the two groups.Result There was a statistically significant difference in the whole therapeutic effect between the two groups(P0.05).The results showed that the whole therapeutic effect on insomnia was better in the treatment group than in the control group.There were no significant differences in increasing sleep time and resolving difficulty in falling asleep between the treatment and control groups(P0.05),but dreaminess,easy waking up and lassitude improved more in the treatment group than in the control group(P0.05).Conclusion Auricular point plaster therapy is superior to conventional acupuncture in the whole therapeutic effect on insomnia.Auricular point plaster therapy has the advantages of being convenient,economical and painless compared with conventional acupuncture.Therefore,auricular point plaster therapy is clinically preferably recommended for the treatment of insomnia.

Abstract:Abstract. Tobacco abuse and alcoholism cause cancer, emphysema, and heart disease, which contribute to high death rates, globally. Society pays a significant cost for these habits whose first demonstration in many cases is in the oral cavity. Oral cavity disorders are highly curable if a screening procedure is available to diagnose them in the earliest stages. The aim of the study is to identify the severity of tobacco abuse, in oral cavity, as reflected by the emission from endogenous fluorophores and the chromophore hemoglobin. A group who had no tobacco habits and another with a history of tobacco abuse were included in this study. To compare the results with a pathological condition, a group of leukoplakia patients were also included. Emission from porphyrin and the spectral filtering modulation effect of hemoglobin were collected from different sites. Multivariate analysis strengthened the spectral features with a sensitivity of 60% to 100% and a specificity of 76% to 100% for the discrimination. Total hemoglobin and porphyrin levels of habitués and leukoplakia groups were comparable, indicating the alarming situation about the risk of tobacco abuse. Results prove that fluorescence spectroscopy along with multivariate analysis is an effective noninvasive tool for the early diagnosis of pathological changes due to tobacco abuse.
[Effect of Tongdu Tiaoshen acupuncture on serum GABA and CORT levels in patients with chronic insomnia]

Abstract:Objective: To observe the clinical effect of Tongdu Tiaoshen acupuncture on chronic insomnia, and explore its possible mechanism. Methods: A total of 60 patients with chronic insomnia were randomly divided into an observation group (30 cases) and a control group (30 cases, 1 case dropped off ). The observation group was treated with Tongdu Tiaoshen acupuncture at Baihui (GV 20), Yintang (GV 29), Shenting (GV 24), etc. once every other day, 3 times a week. The control group was treated with estazolam tablets (1 mg) orally before going to bed every night. The treatments of both groups were required 4 weeks. The Pittsburgh sleep quality index (PSQI) scores, serum gamma-aminobutyric acid (GABA) and cortisol (CORT) levels were observed before and after treatment in the two groups, and the clinical effect was compared. Results: After treatment, the sleep quality, day dysfunction scores of PSQI in the observation group and the falling asleep time, sleep time, sleep efficiency, sleep disturbance scores and total scores of PSQI in the two groups were lower than before treatment (P<0.01, P<0.05). After treatment, the daytime dysfunction score of PSQI in the observation group was lower than that in the control group (P<0.05). After treatment, the serum GABA levels in the two groups were higher than before treatment (P<0.05), and the serum CORT levels were lower than before treatment (P<0.01). The total effective rate was 90% (27/30) in the observation group and 93.1% (27/29) in the control group, the difference was not statistically significant (P>0.05). Conclusion: Tongdu Tiaoshen acupuncture has the same effect as western medication estazolam tablets in the treatment of patients with chronic insomnia, and has little effect on daytime dysfunction. The mechanism may be related to the increase of serum GABA level and the inhibition of the hyperactivation of hypothalamus-pituitary-adrenal (HPA) axis.

Abstract:Objective: To observe the clinical effect of "Yinqi Guiyuan" needling in the treatment of primary insomnia. Methods: A total of 79 primary insomnia outpatients were randomly divided into treatment group (n=40) and control group (n=39). The patients in the control group were given oral Estazolam tablets once a day, for successive 4 weeks. For patients of the treatment group, Zhongwan (CV12), Xiawan (CV10), Qihai (CV6), Guanyuan (CV4), Baihui (GV20), etc., were punctured with filiform needles for 30 min. The treatment was conducted three times per week for 4 successive weeks. The sleep quality (sleeping quality, falling asleep time, sleep duration, sleep efficiency, sleep disorders, hypnotic and daytime dysfunction, 0 to 21 points) was evaluated by using Pittsburgh Sleep Quality Index (PSQI). The severity of insomnia (self-perception, sleep satisfaction, daytime function damage, sensibility change, and concern for sleep problems, 0 to 28 points) was assessed using insomnia severity index (ISI) score. The therapeutic effect was evaluated according to the PSQI score reduction rate = (pre-treatment PSQI score-post-treatment PSQI score)/pre-treatment PSQI score ×100%. Results: After treatment, the total score of PSQI, ISI and the score of each item were all significantly reduced in the two groups relevant to their own pre-treatment (P<0.05). The total score, and scores of hypnotic and daytime dysfunction were significantly lower in the treatment group than in the control group (P<0.05). Of the 40 and 39 cases in the treatment and control groups, 5 (12.50%) and 4 (10.25%) were cured, 20 (50.00%) and 18 (46.15%) experienced marked improvement, 12 (30.00%) and 13 (33.33%) were effective, and 3 (7.50%) and 4 (10.25%) ineffective, with the total effective rate being 92.50% and 89.74%, respectively. No significant difference was found between the two groups in the effective rate (P>0.05). Conclusion: "Yinqi Guiyuan" needling and Estazolam are comparable in treatment primary insomnia, and the former is superior to the latter in avoiding hypnotic drug use and in improving daytime function.

Abstract:Objective: To investigate the clinical effect of shallow acupuncture combined with ear-acupoint pellet-pressing in the treatment of primary insomnia in patients with qi-stagnation constitution. Methods: A total of 60 primary insomnia outpatients with qi-stagnation constitution were randomly divided into treatment group and control group, with 30 patients in each group. The patients in the control group were given oral Alprazolam tablets once a day, and those in the treatment group given shallow acupuncture of Yintang (EX-HN3), Shangen (inferior to EX-HN3), Anmian (EX-HN16) and bilateral Xingjian (LR2) and bilateral Taichong (LR3), once a day, and combined with pellet-pressing of ear acupoints "Shenmen""Sympathy""Subcortex""Heart" and "Liver" once every other day. Each course of treatment was 10 consecutive days, and both groups were treated for three courses. The sleep quality was assessed using Pittsburgh Sleep Quality Index (PSQI), the emotional status assessed using Self-rating Depression Scale (SDS), and the qi-stagnation state evaluated according to "the Criteria for Classification and Judgement of Constitution of Traditional Chinese Medicine (2009)". The therapeutic effect was evaluated according to "the Criteria for Diagnosis and Evaluation of Therapeutic Effect of Diseases of Traditional Chinese Medicine"．. Results: Of the two 30 cases in the control and treatment groups, 2 (6.7%) and 6 (20.0%) were cured, 6 (20.0%) and 14 (46.7%) experienced marked improvement in their symptoms, 21 (70.0%) and 6 (20.0%) were effective, and 1 (3.3%) and 4 (13.3%) ineffective, with the effective rate being 86.7% and 96.7%, respectively. No significant difference was found between the two groups in the short-term effect (P>0.05). One month's follow-up showed that, of the two 30 cases in the control and treatment groups, 2 (6.7%) and 5 (16.7%) were cured, 4(13.3%) and 14 (46.6%) experienced marked improvement, 10 (33.3%) and 6 (20.0%) were effective, and 14(46.7%) and 5(16.7%) ineffective, with the effective rate being 53.3% and 83.3%, respectively. The long-term therapeutic effect of the treatment group was significantly superior to that of the control group (P<0.05). After the treatment, both PSQI and SDS scores in the two groups, and qi-stagnation score in the treatment group showed a significant reduction in comparison with their own pretreatment (P<0.05), and one-month's follow-up (not the short-term outcome) displayed that the PSQI, SDS and qi-stagnation scores of the treatment group were significantly lower than those of the control group (P<0.05)．. Conclusion: Shallow acupuncture combined with ear-acupoint pellet-pressing can significantly improve sleep quality, depression symptoms, and pathological constitution in primary insomnia patients with qi-stagnation constitution, possessing a stable long-term clinical effect.
The Clinical Efficacy Observation on Acupoint Application to the Sleep Quality of Patients with Functional Dys-pepsia Associated with Anxiety Insomnia

Abstract:Objective:To observe the efficacy of acupoint application to the sleep quality of patients with functional dyspepsia associated with anxiety insomnia. Methods: 81 cases of patients were randomly divided into treatment group and control group, the control group treated by basic treat-ment, while treatment group by acupoint application on the basis of basic treatment, evaluated the efficacy and PSQI grading after a month. Results:The overall response rate of treatment group was significantly higher than that of control group (P<0.05);after treatment, the PSQI score of treatment group was significantly lower than that of control group (P<0.05). Conclusion:Acupoint application has improved the sleep quality of patients with functional dyspepsia associated with anxiety insomnia, which is suitable to clinical promotion.

Abstract:Objective: To compare the effect of Shugan Tiaoshen (soothing the liver and regulating mind) acupoint thread-embedding therapy and Shugan Tiaoshen acupuncture therapy for insomnia of liver depression and qi stagnation. Methods: A total of 68 patients with insomnia of liver depression and qi stagnation were randomly divided into an acupoint embedding group (34 cases) and an acupuncture group (34 cases). Two groups were respectively treated with acupoint thread-embedding and acupuncture at Baihui (GV 20), Shenmen (HT 7), Neiguan (PC 6), Taichong (LR 3), Ganshu (BL 18) and Xinshu (BL 15). The treatment in the acupoint embedding group was given once a week, and in the acupuncture group was given once every other day for 6 weeks. The Pittsburgh sleep quality index (PSQI), self-rating anxiety scale (SAS), self-rating depression scale (SDS), TCM syndrome scores before and after treatment and medical expenses were compared between the two groups, and the clinical efficacy was evaluated. Results: After treatment, the PSQI total scores, each sub-item score and SAS and SDS scores of the two groups were lower than those before treatment (P<0.05); the sleep quality, time during fall asleep, sleep time, sleep efficiency, daytime dysfunction scores and PSQI total score in the acupoint embedding group after treatment were lower than those in the acupuncture group (P<0.05). After treatment, except for constipation and yellowish urine, the sub-item scores and total scores of TCM syndromes in the two groups were lower than those before treatment (P<0.05); the sleep disturbance score of the acupoint embedding group was lower than that of the acupuncture group (P<0.05). The medical expenses in the acupoint embedding group was lower than that in the acupuncture group (P<0.05). The total effective rate of the acupoint embedding group was 85.3% (29/34), and the acupuncture group was 82.4% (28/34), there was no significant difference between the two groups (P>0.05). Conclusion: Shugan Tiaoshen acupoint thread-embedding therapy and Shugan Tiaoshen acupuncture therapy both have significant curative effect on insomnia of liver depression and qi stagnation, but acupoint thread-embedding therapy is better than acupuncture therapy in improving sleep quality and reducing medical expenses.

Abstract:Objective: To observe the effects of acupuncture for treatment of intractable insomnia. Methods: Totally 90 patients were randomly divided into a treatment group (50 cases), treated by the body acupuncture plus auricular-plaster therapy, and a control group (40 cases), the Baihui (GV 20)-through-Sishencong (EX-HN1). puncture, auricular-plaster therapy and moxibustion on the back-shu points were not used in the control group. The clinical effects were observed. Results: The total effective rate was 98.0% in the treatment group and 77.5% in the control group. The therapeutic effect in the treatment group was much better than that in the control group (P < 0.01). Conclusion: The body acupuncture plus auricular-plaster therapy may show better effect for intractable insomnia.

Abstract:Objective To observe the influence of point massage at head and body points on sleep structure in chronic insomnia patients.Methods 45 patients were treated by point massage on the same main points including Anmian,Sishencong,Shenmen,Sanyinjiao temple for two to five minutes,once every day,abidingly for 20 to 30 days(averaged 24 ± 5 days).The change of patients' sleep structure before and after treatment was tested by poly-somnography analytical system.Self-control statistical methods were used to analyze the data.Results Before treatment,sleep structure of 45 cases were all incomplete; some patients missed S3 or S4.Being treated with point massage for 30 days,all patients got some improvement of falt and broken sleep structure.Sleep Monitoring showed that Point massage shortened latency period of sleep,lengthened the time of slow-wave sleep(S + S4 period),shorten the time of somnolence(S1 + S2 period),and relatively lengthened the time of NREMS and REMS (P＜ 0.05).Conclusion Point massage can repair the sleep structure,remold its continuity to a certain extent,and lengthen the time of slow-wave sleep,which is the main mechanism of improving sleep quality by acupuncture.

Abstract:OBJECTIVE To compare the clinical effect differences among different acupoint selection methods for primary insomnia. METHODS A total of 333 patients with primary insomnia were recruited from 3 study centers and randomly divided into a group A (111 cases, 7 cases dropped off), a group B (111 cases, 5 cases dropped off) and a group C (111 cases, 2 cases dropped off). The patients in the group A were treated with acupuncture at Shenmen (HT 7) and Baihui (GV 20), the patients in the group B were treated with acupuncture at Sanyinjiao (SP 6) and Baihui (GV 20), and the patients in the group C were treated with acupuncture at non-acupoint and Baihui (GV 20). All the treatment was given once a day, 30 min each time; 5 treatments were taken as a course and 5 courses of treatment were given. The Pittsburgh sleep quality index (PSQI) and Athens insomnia scale (AIS) scores were evaluated before and after treatment as well as 4 weeks after treatment. The encephalofluctuograph technology (ET) was observed before and after treatment. RESULTS Compared before treatment, the PSQI scores after treatment and at follow-up were significantly decreased in three groups (P<0.01), and the decrease in the group A and the group B was greater than that in the group C (P<0.01). Compared before treatment, the AIS scores after treatment and at follow-up was significantly decreased in three groups (P<0.01), and the decrease in the group A was greater than that in the group C (P<0.05). The interclass and between-groups ET before and after treatment had no significant difference (P>0.05). CONCLUSION The acupuncture at acupoints along the meridians could improve the sleep quality in patients with primary insomnia, and the therapeutic effect of acupoint along the meridians is better than that of non-acupoint.

Abstract:Objective: To explore and analyze the clinical efficacy of Chinese medicine combined with acupuncture and moxibustion in the treatment of insomnia. Method: 80 patients with primary insomnia were selected as the study sample, with 20 cases randomly selected as the control group and the rest as the observation group. The control group was treated with ordinary western medicine, while the observation group patients were treated with traditional Chinese medicine combined with acupuncture and moxibustion based on this syndrome differentiation. The comparison items were treatment effect, sleep quality, mental health, quality of life, specific sleep indicators, cerebral blood flow and neurotransmitters. Result: The treatment effectiveness showed that the observation group had a higher treatment effectiveness rate than the control group (P<0.05); The results of sleep quality showed that after treatment, the scores of the observation group were lower than those of the control group in seven dimensions: sleep quality, sleep time, sleep onset time, sleep efficiency, sleep disorders, hypnotic drugs, and daytime dysfunction score (P<0.05); The quality of life results showed that after treatment, the observation group had higher scores than the control group in eight dimensions: physiological function, physiological function, physical pain, health status, energy, social function, emotional function, and mental health (P<0.05); The results of slow wave sleep time and fast eye movement sleep time showed that after treatment, the HAMA and HAMD scores of the physiological mechanism in the observation group were lower than those in the control group (P<0.05); The results of cerebral blood flow and neurotransmitter indicators show that after treatment, glutamate γ- the peak systolic blood flow velocity of aminobutyric acid, vertebral artery, and basilar artery in the observation group was higher than that in the control group (P<0.05). Conclusion: The clinical treatment of Chinese medicine combined with acupuncture and moxibustion in the treatment of insomnia patients can significantly improve the clinical treatment effect, and improve the cerebral hemorrhage and neurotransmitter status of patients, promote the improvement of their physical and mental status, can more significantly improve the quality of sleep, reduce sleep difficulties, reduce the dosage of sleeping pills, which has important clinical application value.

Abstract:OBJECTIVE To observe the effects of acupuncture at "umbilical four-acupoints" on chronic insomnia and its comorbid symptoms. METHODS A total of 120 patients with chronic insomnia were randomly divided into an observation group (60 cases, 8 cases dropped off) and a control group (60 cases, 5 cases dropped off). The patients in the observation group were treated with acupuncture at regular acupoints (Baihui [GV 20] and bilateral Shenmen [HT 7], Neiguan [PC 6], Anmian [Extra]) and "umbilical four-acupoints", while the patients in the control group were treated with acupuncture at regular acupoints. Acupuncture was given once a day, 6 times a week, for a total of 3 weeks in the two groups. The Pittsburgh sleep quality index (PSQI), insomnia severity index (ISI) scores were observed before treatment, after treatment and in follow-up of one month after treatment completion; the Beck anxiety inventory (BAI), Beck depression inventory (BDI), fatigue severity scale (FSS), and Epworth sleepiness scale (ESS) scores were observed before and after treatment; the sleep parameters of polysomnography (PSG), including sleep latency (SL), awake-up time (AT), sleep efficiency (SE) and total sleep time (TST), were observed before and after treatment using polysomnography monitor in the two groups. RESULTS Compared with those before treatment, the PSQI and ISI scores in both groups were reduced after treatment and in follow-up (P<0.05), and the PSQI and ISI scores in the observation group were lower than those in the control group after treatment and in follow-up (P<0.05). Compared with those before treatment, the BAI, BDI, FSS and ESS scores in both groups were reduced after treatment (P<0.05), and the BAI, BDI, FSS and ESS scores in the observation group were lower than those in the control group after treatment (P<0.05). Compared with those before treatment, the SL and AT in both groups were reduced after treatment (P<0.05), while SE and TST were increased after treatment (P<0.05); after treatment, the SL and AT in the observation group were lower than those in the control group (P<0.05), while SE and TST in the observation group were higher than those in the control group (P<0.05). CONCLUSION On the basis of regular acupoint selection, acupuncture at "umbilical four-acupoints" could improve sleep quality, alleviate the severity of insomnia, and improve the comorbid symptoms i.e. anxiety, depression, fatigue and lethargy in patients with chronic insomnia.

THE EFFECT OF DIETS SUPPLEMENTED WITH SACCHAROMYCES CEREVISIAE BOULARDII PROBIOTIC YEAST ON THE REPRODUCTIVE PERFORMANCE OF PREGNANT AND LACTATING SOWS

Abstract:The aim of this study was to assess the efficacy of Levucell SB 10, a probiotic containing Saccharomyces cerevisiae boulardii strain, on the health status and productivity of sows and their litters. A total of 243 gilts and sows (PIC) were allocated into two experimental groups, as follows: untreated controls - 125 (C) and Levucell SB 10-118 (1.0x10 9 cfu/kg of feed of the gestation and lactation diets). Homogeneity of the groups was satisfied with regard to the parity (2.74 and 2.78). The treatment started from d 1 of gestation to d 28 of lactation. Considering the results it can be concluded that the sows of poor health who received feeds with the probiotic supplement are characterised by better fertility and mating effectiveness compared to the control group sows. The addition of the probiotic supplement to the sow diet contributed to the downward trend for the sows' reproductive problems (abscess, abortion). However, it did not have any influence on the sows' condition and the length of parturiency. There were no gestation and lactation diet treatment differences for sow back fat thickness (breeding, farrowing or weaning). There were no differences (P≤0.05) in total number of pigs born, born alive, stillborn or born mummified. The sows on this diet also showed a trend for a larger size of litter but as the number of weak piglets was higher (P≤0.05) the average size of litter on the 21st and 28th days of life did not differ between the test groups.

Abstract:In the absence of placebo‐controlled trials, determining the non‐inferiority (NI) margin for comparing an experimental treatment with an active comparator is based on carefully selected well‐controlled historical clinical trials. With this approach, information on the effect of the active comparator from other sources including observational studies and early phase trials is usually ignored because of the need to maintain active comparator effect across trials. This may lead to conservative estimates of the margin that translate into larger sample‐size requirements for the design and subsequent frequentist analysis, longer trial durations, and higher drug development costs. In this article, we provide methodological approaches to determine NI margins that can utilize all relevant historical data through a novel power adjusted Bayesian meta‐analysis, with Dirichlet process priors, that puts ordered weights on the amount of information a set of data contributes. We also provide a Bayesian decision rule for the non‐inferiority analysis that is based on a broader use of available prior information and a sample‐size determination that is based on this Bayesian decision rule. Abstract:Objective: To compare the difference between acupuncture and estazolam on arousal state in patients of primary insomnia, and to explore its nerve electrophysiology mechanism. Methods: Sixty-four patients of primary insomnia were randomized into an acupuncture group (32 cases) and a medication group (32 cases). After 3 patients were excluded, 31 cases in the acupuncture group and 30 cases in the medication group were included. Patients in the acupuncture group were treated with acupuncture at Sishencong (EX-HN 1), Anmian (Extra), Shenmen (HT 7), Sanyinjiao (SP 6), Zhaohai (KI 6), Shenmai (BL 62) as main acupoints, combined with supporting acupoints, once a day, five times per week, continuously for 4 weeks. Patients in the medication group were treated with oral administration of estazolam, once a day, continuously for 4 weeks. The Pittsburgh sleep quality index (PSQI) and mean sleep latency (MSL) of multiple sleep latency test (MSLT) were compared before and after treatment in the two groups; the polysomnography (PSG) was applied to monitor the indices regarding sleep structure. Results: Compared before treatment, PSQI score was reduced after treatment in the two groups (both P<0.01), which was more significant in the acupuncture group (P<0.05). Compared before treatment, sleep onset latency (SOL), number of awakenings (NWAK) and wake after sleep onset (WASO) were reduced, while total sleep time (TST) and sleep efficiency (SE) were significantly increased in the two groups after treatment (all P<0.01). Compared before treatment, the percentage of non-rapid eye movement period 1/2/3 (N1, N2, N3) and the percentage of rapid eye movement period (REM) to TST were not significantly changed after treatment in the medication group (all P>0.05). Compared before treatment, the percentage of N1, N2 to TST was reduced, while the percentage of N3 and REM to TST was increased after treatment in the acupuncture group (P<0.01). The SOL, NWAK, WASO, TST, SE were not statistically changed after treatment in each group (all P>0.05). Compared with the medication group, the percentage of N1 and N2 was reduced while that of N3 and REM was increased after treatment in the acupuncture group (all P<0.01). After treatment, MSL of MSLT were obviously decreased in the two groups (both P<0.01), which were more significant in the acupuncture group (P<0.05). Conclusions: Acupuncture can more effectively improve sleep quality of primary insomnia than estazolam, and is more beneficial for regulation of hyperarousal state.
Acupoint Catgut Embedding for Insomnia: A Meta-Analysis of Randomized Controlled Trials

Abstract:Objectives: A Meta-analysis was carried out to evaluate the efficacy and safety of acupoint catgut embedding (ACE), a procedure of embedding sutures made of absorbable materials into the skin tissue of acupoints, on insomnia. Methods: Relevant clinical randomized controlled trials (RCTs) were comprehensively searched from eleven electronic databases (up to 1 March 2020). Two authors independently screened literature, extracted data, and assessed the risk of bias of included studies. Stata 12 and RevMan 5.3.0 software were used for meta-analysis. PyCharm 2019 and Gephi software (version 0.9.2) were used for complex network analysis. Results: Thirty-four RCTs involving 2,655 patients were included. The meta-analysis suggested that ACE induced a better clinical efficacy compared with that in the estazolam tablets (EZ) group (RR = 1.22, 95% CI: 1.13, 1.31) or in the acupuncture (ACU) group (RR = 1.21, 95% CI: 1.14, 1.28) and could significantly reduce the score of Pittsburgh Sleep Quality Index (P < 0.05). ACE resulted in better long-term efficacy compared to that in the EZ group (RR = 1.87, 95% CI: 1.58, 2.22) and ACU group (RR = 1.30, 95% CI: 1.14, 1.48). ACE could significantly reduce the incidence of adverse events (RR = 0.30, 95% CI: 0.15, 0.60) compared with that in the EZ group. Complex network analysis indicated that acupoints of BL23, SP6, PC6, BL15, BL20, BL18, and HT7 were the core acupoints selected in ACE for insomnia. Conclusion: The clinical efficacy of ACE for insomnia is better than that of other interventions (EZ and ACU) in both short-term and long-term observations. Considering the efficacy and reduced visits to the clinic by ACE, the present study provides a practical and convenient complementary and alternative therapy for insomnia. Abstract:Background: Chronic insomnia (CI) is the most common subtype of insomnia. Auricular acupoint therapy (auricular acupoint bloodletting plus auricular acupressure) is a common treatment for CI, but its efficacy is uncertain due to the poor quality of previous studies. The purpose of this study is to evaluate the efficacy and safety of auricular acupoints in the treatment of CI by comparing with sham auricular acupoint therapy. Methods: This is a multicenter, randomized, two-arm interventional clinical trial. Participants will participate in the study for a total of 18 weeks, consisting of three phases: 2 weeks of screening, 4 weeks of treatment, and 12 weeks of follow-up. Based on the response rate of the primary outcome in our previous study, 156 randomly allocated participants are planned. Eligible participants will be randomly assigned to the auricular acupoint therapy group and sham auricular acupoint therapy group according to a ratio of 1:1. A total of 8 sessions in 4-week treatment period will be carried out. The primary outcome is a response rate at week 4, and the responder is defined as the percentage of those having at least 3 points reduction in insomnia symptoms from baseline as measured via the Pittsburgh Sleep Quality Index (PSQI). Furthermore, response rates at other weeks, PSQI, Insomnia Severity Index (ISI), Self-Rating Depression Scale (SDS), Self-Rating Anxiety Scale (SAS), World Health Organization Quality of Life Scale-Abbreviated Form (WHOQOL-BREF), as well as sleep efficiency (SE), sleep arousal frequency (SA) and total sleep time (TST) recorded by actigraphy are chosen as secondary outcomes.
