Abstract:Background: Lung cancer is currently the most fatal form of cancer worldwide, ranking as the fourth most prevalent type in Saudi Arabia, particularly among males. This trend is expected to increase with growing population, lifestyle changes, and aging population. Understanding the awareness of the Saudi population regarding the risk factors and symptoms of lung cancer is necessary to attenuate the predicted increase in cases. Method: A cross-sectional, population-based survey was performed using a previously validated questionnaire (Lung CAM). Multiple linear regression analysis was used to assess variables associated with deficiency in knowledge and awareness of risk factors and symptoms of lung cancer. Results: Majority of the 15,099 respondents were male (65%), aged between 18 and 30 years (53%), 50% of which were educated up to a bachelor's degree level. Overall awareness of lung cancer signs and symptoms was 53%, with painful cough and coughing up blood being the best-known symptoms. Conversely, persistent shoulder pain (44%) and clubbing fingers (47%) were the least known lung cancer symptoms. Also, 60% of the respondents showed low confidence in identifying the signs and symptoms of lung cancer. The overall awareness of the risk factors for lung cancer development was 74%, with first-hand (74%) and second-hand (68%) smoking being the most known risk factors. However, only ≤ 62% know the other non-smoking risk factors. Awareness of the risk factors and symptoms of lung cancer depended on age, gender, education, marital and employment status (p < 0.001). Conclusion: Public awareness of the risk factors and symptoms of lung cancer in Saudi Arabia is inadequate and heavily dependent on education and socio-economic status. Awareness can be improved through campaigns to raise awareness about other lesser-known lung cancer risk factors and symptoms.

Abstract:Background/aims: This study examines temporal trends in incidence of lung cancer in 1972-2007, temporal trends in histological types of lung cancer in 1988-2007, and age-period-cohort effects on the incidence rates of lung cancer in Canada.Methods: Using incidence data for 1972-2007, we calculated the three-year period rates and annual percentage change (APC): from 1988 to 2007 we were able to do this by histological types. We used age-period-cohort modelling to estimate underlying effects on the observed trends in incidence of adenocarcinoma of the lung.Results: In Canada, age-adjusted incidence rates have increased by 263% in women and 4% in men from 1972 to 2007. Annual percent change in age-adjusted rates for women by histological type from 1988 to 2007 were 2.2% for adenocarcinoma, -0.9% for squamous cell carcinoma and -0.4% for small cell carcinoma. Age-adjusted rates decreased for men over the same 20 years: adenocarcinoma (APC: -0.6%), squamous cell carcinoma (APC: -4.2%) and small cell carcinoma (APC: -3.2%) in men. Age-specific incidence rates increased most rapidly for adenocarcinoma in those aged 75+ years (APC: women 4.3%; APC: men 1.1%). The age-period-cohort modelling suggested that the risk of being diagnosed with adenocarcinoma is decreasing in men and will be decreasing slowly in women.Conclusions: The adenocarcinoma incidence trends observed are consistent with smoking trends, however, the relative risk with smoking is lower for adenocarcinoma than for squamous cell carcinoma and small cell carcinoma. Histopathological subtypes of lung cancer presented at a tertiary care cancer hospital in Kerala: a cross sectional study

Abstract:Background: Lung cancer is one of the commonest cancers causes high rate of mortality worldwide. An increasing incidence of lung cancer and the pathological profile varies among gender and geographical regions. The present study was aimed to assess the pattern of histological subtypes of lung cancer and their distribution with age and gender.Methods: Histologically proven primary lung cancers were selected from the cancer registry. Distribution of subtypes of lung cancer in various age and gender was collected. The major clinical presentation among the non-small cell lung carcinoma (NSCLC) and small cell lung carcinoma (SCLC) were also analysed. The data were statistically analysed.Results: A total of 155 cases of lung cancers were analysed. Adenocarcinoma of lung was the most common subtype followed by squamous cell carcinoma and SCLC. Majority were males with age of presentation from 23 to 93 years. Age of presentation in the female group was 23-75 years. Significant difference was found between cancer numbers in male and female patients (p=0.0001). Statistically significant difference was found among the distribution of smokers and non-smokers in the NSCLC and SCLC patients (p=0.046). Most of the NSCLC and SCLC patients were presented with dyspnea and coughing.Conclusions: Lung cancers were commonly seen in males and smokers. The most common histological subtype in males and females was adenocarcinoma. The diagnosis of histological subtype at the onset of clinical presentation of suspected cases of lung cancer is required to start the therapeutic regimen at the earliest to increase the longevity of patients.

Abstract:Background Lung cancer (Lca) is the leading cause of cancer morbidity and mortality worldwide. This study examines the Lca incidence and trends in Lebanon and compares them to regional and global ones. It also discusses Lca risk factors in Lebanon. Methods Lung cancer data from the Lebanese National Cancer Registry for 2005 to 2016 was obtained. The age-standardized incidence rates (ASRw) and age-specific rates per 100 000 population were calculated. Results Lung cancer ranked second for cancer incidence in Lebanon from 2005-2016. Lung cancer ASRw ranged from 25.3 to 37.1 per 100 000 males and 9.8 to 16.7 per 100 000 females. Males 70-74 and females 75+ had the highest incidence. Lung cancer ASRw in males increased significantly at 3.94% per year from 2005 to 2014 ( P > .05), then decreased non-significantly from 2014 to 2016 ( P .05), then increased non-significantly from 2009 to 2016 ( P < .05). Males' Lca ASRw in Lebanon was lower than the global average in 2008 and became similar in 2012 (34.1 vs 34.2 per 100 000); However, females' Lca ASRw was almost comparable to the global average in 2008 and exceeded it in 2012 (16.5 vs 13.6, respectively, per 100 000). Males’ and Females’ Lca ASRw in Lebanon were among the highest in the Middle East and North Africa (MENA) region but lower than those estimated for North America, China and Japan, and several European countries. The proportion of Lca cases attributed to smoking among Lebanese males and females was estimated at 75.7% and 66.3% for all age groups, respectively. The proportion of Lca cases attributed to air pollution with PM 10 and PM 2.5 in Lebanon was estimated at 13.5% for all age groups. Conclusion Lung cancer incidence in Lebanon is among the highest in the MENA region. The leading known modifiable risk factors are tobacco smoking and air pollution.

Abstract:Abstract Background Lung cancer in very young adults in China aged <30 is on the rise and the clinicopathological characteristics of this unique population are incomplete and are yet to be elucidated. Our study aims to explore the characteristics, trends and distinctive features of lung cancer this age group to provide more information that can aid in developing optimal treatment regimens and improve prognosis for this exclusive age group. Method A retrospective review of 141 lung cancer patients admitted at our hospital from August 2011 to October 2021 was performed. The focused age group in our study was 18-30 years old. The data collected included demographics, tumor pathology, gene mutations, and treatment. Overall survival and prognosis were systematically analyzed. Results From a total of 141 of young lung cancer patients, 139 were included in final analysis. The gender distribution in our series favored female(65.5%), and only 7.2% of the study cohort had a history of smoking. Adenocarcinoma was the most common histological type in our study, presenting 93.5% of all patients. The most common stages at diagnosis was stage I(61.2%) and stage IV(30.2%). Genetic testing was performed in only 24.5% (34/139) of the patients. From the 23/34 patients in our study who tested positive for a genetic mutation, 19 (82.3%) were stage IV. The most common gene mutation was Anaplastic Lymphoma kinase(ALK) rearrangement observed in 13/23 (56.5%) gene mutation positive patients. Among stage IV NSCLC, OS was longer in patients who received targeted therapy, compared to the patients not receiving target therapy: 88 months vs 24 months (95% CI: 0.15-1.0, p=0.0282). Conclusion Lung cancer in the young adult population has particular characteristics (Common in female, non-smoker and adenocarcinoma patients). Higher stages of disease were observed in those with a gene mutation (notably ALK rearrangement). Although the sample size was small, the current data suggests a higher mutation rate in the younger population. These findings have fundamental implications for practice and we propose that all patients under 30 diagnosed with lung cancer should undergo genetic testing so they can receive personalized treatment and therefore achieve better outcomes.
Histopathological Patterns of Lung Cancer in Iran: A Single-Center Study

Abstract:Background: Lung cancer (LC) is one of the leading causes of cancer-related deaths worldwide. In Iran, it is the second most common cause of cancer-related deaths for men and the third most common for women. This study aimed to examine the clinicopathological characteristics of Iranian patients with LC. Methods: Clinicopathological data of 1382 patients with primary LC diagnosed over 11 years (2012‒2023) at the "National Institute of Tuberculosis and Lung Disease" (NRITLD), Tehran, Iran, were retrospectively reviewed. Results: Adenocarcinoma was the most common type of cancer found in the patients (42.44%). The median age was 59.69 years (mean: 60.41 years) ranging 24-88 years. The mean male-to-female ratio was 3.65. Additionally, 65.84% of patients were smokers. The majority of patients (82.69 %) were diagnosed at an advanced stage (stage IV) of cancer. Conclusion: Although some of our findings are consistent with those of previous LC studies, there are some discrepancies, especially concerning the smoking status and median age of the Iranian patients. Therefore, additional clinical and epidemiological studies are needed to determine the impact of non-smoking factors, such as environmental exposure and genetic predisposition, on the development of LC.

Abstract:Lung cancer is the leading cause of cancer-related deaths worldwide. Non-small cell lung cancer (NSCLC) represents the most frequently diagnosed subtype of this morbid malignancy. NSCLC is causally linked to tobacco consumption with more than 500 million smokers worldwide at high risk for this fatal malignancy. We are currently lagging in our knowledge of the early molecular (e.g., genomic) effects of smoking in NSCLC pathogenesis that would constitute ideal markers for early detection. This limitation is further amplified when considering the variable etiologic factors in NSCLC pathogenesis among different regions around the globe. In this review, we present our current knowledge of genomic alterations arising during early stages of smoking-induced lung cancer initiation and progression, including discussing the premalignant airway field of injury induced by smoking. The review also underscores the wider spectra and higher age-adjusted rates of tobacco (e.g., water-pipe smoke) consumption, along with elevated environmental carcinogenic exposures and relatively poorer socioeconomic status, in low-middle income countries (LMICs), with Lebanon as an exemplar. This "cocktail" of carcinogenic exposures warrants the pressing need to understand the complex etiology of lung malignancies developing in LMICs such as Lebanon.

Clinical and Experimental Hypertension Part A Theory and Practice

Abstract:Stress is an important risk factor in cardiovascular diseases, including hypertension. Endogenous opioids are known to be elevated in stress and in various models of hypertension with differing etiologies. Blockade of endogenous opioids with naloxone has been demonstrated to attenuate or reverse the elevation in blood pressure in both renovascular and spontaneous hypertension. In the current study, increased blood pressure was induced using a model of psychosocial stress. During the first week of stress, systolic blood pressure rose rapidly to reach a level that was sustained throughout the remaining period of stress. Chronic infusion of the opiate antagonist, naloxone, both prevented and completely reversed the elevated blood pressure due to psychosocial stress. These data demonstrate that elevated endogenous opioids are important factors in cardiovascular regulation and are likely to influence both the development and maintenance of stress-induced hypertension.

Abstract:The objective was to evaluate the dose-related efficacy/tolerance profile of 240, 300, 360, and 420 mg diltiazem slow-release, once-daily (OD) doses, with emphasis on the 300 mg dose. The study was randomized and double-blinded with a 36-week parallel, two-branched, cross-over design after a single-blind, run-in period of 4 weeks on placebo and 6 weeks on 300 mg diltiazem OD. Each branch included six 6-week active treatment periods with 180, 240, and 300 mg tablets, one or two tablets OD. Participants were men and postmenopausal women, aged 40-70 years, with uncomplicated primary hypertension (WHO stages I and II) and a supine diastolic blood pressure of 95-115 mm Hg in the absence of antihypertensive medication. A total of 138 patients from various clinics participated in the study. All were included in the intention to treat analysis, and 117 patients were included in the per protocol analysis. Criteria for evaluation were blood pressure, heart rate, and response rate, as well as plasma diltiazem and metabolite M1 concentration before morning dose. Well-being of the patients and adverse events were recorded. Electrocardiogram and standard laboratory tests were obtained. Analysis of variance was used for statistical calculations. Supine blood pressures and response rates in the per protocol analysis were 161.2/97.0 for placebo (29.1%), 155.2/ 92.8 for 240 mg (54.7%), 153.8/91.6 for 300 mg (55.6%), 155.5/92.0 for 360 mg (59.0%), and 152.0/90.5 for 420 mg (63.2%) of diltiazem OD. The intention to treat analysis was very similar to the per protocol analysis. There was a small but statistically significant decrease in heart rate for all doses of diltiazem OD compared to placebo. A linear relationship existed between the dose and the plasma concentration of both diltiazem and metabolite M1, as well as a dose-response relationship. Diltiazem OD in the dose range 240-420 mg was generally well tolerated and not differently perceived from the placebo treatment except for ankle edema (2-6%). The study shows that OD diltiazem is significantly superior to placebo for mild to moderate hypertension and that the effect is large enough to be clinically valuable. It also shows that there is a linear dose-response relationship for diltiazem between 240 and 420 mg.
Arterial embolectomy. A long-term perspective.

Abstract:One hundred and thirteen elderly patients (median age 77 years, range 37-99) were followed up for 2-10.5 years after embolectomy in order to assess the late outcome. There were two groups of patients: those who had suffered an embolism (84) and those who in retrospect had thrombosis in situ (29). Thirty three of the embolus group (39%) died within 30 days of surgery, and factors associated with mortality (p less than 0.05) were age greater than 80 years, and failure to save the limb (17%). In the thrombosis group limb loss was more common (76%) and these patients also had a higher mortality (55%). Mean long term survival in the embolus group was 35 months (range 1-108 months) with a high mortality rate in the first 6 months after embolectomy and improved survival after this. There were no significant differences in mortality or cardiovascular events between the 27 patients on long term warfarin and the 19 patients not anticoagulated. Mortality after embolectomy is high but there remains a group of long term survivors. The value of oral anticoagulation for these patients remains unresolved.
Knowledge of cancer symptoms and risk factors: A cross-sectional study from a developing country

Abstract:The delayed presentation of cancer patients to healthcare facilities for diagnosis is a pressing issue, as late-stage cancer cases are often more challenging to treat effectively. In low-resource settings, such as those with limited access to healthcare facilities, the impact of inadequate awareness of cancer signs and symptoms can be particularly severe. Therefore, this study aimed to evaluate public knowledge of cancer signs and symptoms and risk factors in the context of Jordan. This cross-sectional study was conducted among participants from all settings. Data was obtained from an Arabic version of the cancer awareness measure (CAM), which was answered online. It described demographic data and knowledge of cancer prevalence, age-related risk, signs, symptoms, and risk factors in recall and recognition-type questions. Participants (n = 1998) completed the questionnaire with a Median age of 35 and an interquartile range of 24. About half (n = 1070) thought that cancer is unrelated to age. Most participants identified breast cancer as the most common cancer among women (81%). Awareness of cancer signs/symptoms significantly differed in the level of knowledge in favor of females. The symptom "unexplained weight loss" was most commonly recognized (66.3%) and "persistent difficulty swallowing" the least (42.6%). As for risk factors, "smoking" was the most identified (76.9%) and "eating less than 5 portions of fruits and vegetables a day" was the least (19%), and "doing <30 minutes of moderate physical activity 5 times a week" as a close second least (19.95%). Females identified "smoking," "passive smoking," "HPV infection," and "having a close relative with cancer" as risk factors significantly more than males. Those with good economic status were more aware that smoking is a cancer risk factor by 1.51 times compared to those with poor economic status. To enhance early diagnosis and presentation in Jordan, there is a need for increased public awareness of the signs, symptoms, and risk factors of cancer. One effective strategy to achieve this goal is to conduct targeted public campaigns that cater to different population groups, such as the youth, to improve their understanding and ensure that the message resonates.

Comprehensive Investigation of Mutational Features of Various Lung Adenocarcinoma Histological Subtypes among Chinese Patients.

Abstract:e21090 Background: Lung adenocarcinoma (LUAD) is further classified into several histological subtypes according to the World Health Organization classification; however, data on the molecular profile of these histological subtypes are limited. In this study, we aimed to investigate the mutational features of various LUAD histological subtypes among Chinese patients. Methods: We retrospectively analyzed clinical, histopathologic, and sequencing data from 3,425 consecutive patients diagnosed with stage IA-IVB LUAD between January 2017 to December 2019 at the Department of Pathology. Results: Based on histologic subtype, the cohort comprised of 0.2% atypical adenomatous hyperplasia (AAH, n = 6), 7.2% adenocarcinoma in situ (AIS, n = 246), 14.0% minimally invasive adenocarcinoma (MIA, n = 479), and 78.6% invasive adenocarcinoma (INV, n = 2,694), including 42.0% acinar (n = 1,438), 12.7% papillary (n = 435), 8.8% lepidic (n = 302), 8.5% solid (n = 290), 3.2% micropapillary (n = 109), 2.8% invasive mucinous (n = 95), 0.6% enteric (n = 21), 0.1% fetal (n = 3), and 0.1% colloid (n = 1). Analysis of mutation profiles revealed significantly higher mutation detection rates of ERBB2 ( P< 0.01), BRAF ( P< 0.01), and MAP2K1 ( P= < 0.01) among patients with AIS, AAH and MIA as compared to patients with INV. Meanwhile, TP53 ( P< 0.01) and EGFR ( P< 0.01) mutations were significantly higher among patients with INV as compared to patients with AIS and MIA. Among the invasive subtypes, patients with invasive mucinous adenocarcinoma had significantly higher KRAS mutation rate (75%, P< 0.01). Lepidic (81%), acinar (80%), papillary (77%), and micropapillary (66%) were the subtypes with the highest EGFR mutation rates. Conclusions: By analyzing the genomic data of a large cohort of patients, our study provides a more comprehensive outlook of gene mutations according to LUAD histological subtypes. Enrichement of actionable mutation in particular histological subtype contributes to more treatment option and better clinical outcomes as compared to other histological subtype, indicating the clinical relevance of performing comprehensive mutation profiling in addition to histopathologic analysis in all LUAD patients.
Secular trends in incidence of lung cancer by histological type in Beijing, China, 2000 - 2016.

Abstract:Objective: The objective of this study was to characterize secular trends in the sex-specific, age-standardized incidence of lung cancer by histological type in Beijing, China, from 2000 to 2016 based on data from a population-based cancer registry. Methods: Data on the incidence of cancer from 2000 to 2016 were obtained from the Beijing Cancer Registry. We examined trends in the sex-specific, age-standardized incidence of lung cancer by histological type using a Joinpoint regression model. Results: A total of 117,409 cases of lung cancer were diagnosed from 2000 to 2016. Overall, 73,062 (62.23%) patients were males. The most common histological type among both sexes was adenocarcinoma; however, the proportion of adenocarcinoma differed significantly between males and females (45.36% vs. 77.14%, respectively, P<0.0001). The age-standardized incidence of total lung cancer increased from 2000 to 2010 with an annual percent change (APC) of 2.2% [95% confidence interval (95% CI), 1.5% to 2.9%] and stabilized thereafter. Among males, the incidence of total lung cancer peaked in 2008 and then decreased slightly, with an APC of -1.1% (95% CI, -2.1% to -0.1%). Among females, the incidence increased continuously during the study period, with an APC of 1.4% (95% CI, 0.9% to 1.9%). The incidence of squamous cell carcinoma decreased significantly in recent years among both sexes, with APCs of -2.6% (95% CI, -4.5% to -0.6%) from 2007 to 2016 for males and -5.4% (95% CI,-7.2% to-3.6%) from 2004 to 2016 for females. In contrast, the incidence of adenocarcinoma increased continuously throughout the study period, by APCs of 4.0% (95% CI, 2.6% to 5.4%) for males and 6.2% (95% CI, 4.8% to 7.6%) for females. The incidence of small cell carcinoma peaked in 2007 and stabilized thereafter among males, whereas it peaked in 2012 and then decreased with an APC of-14.7% (95% CI, -25.3% to -2.6%) among females. The incidence of large cell carcinoma and other specified malignant neoplasm did not change much, whereas the incidence of unspecified type decreased among both sexes during the study period. Conclusions: Although the incidence of squamous cell carcinoma decreased significantly among both sexes in recent years in Beijing, China, adenocarcinoma increased continuously throughout the study period among both sexes. Knowledge of differences in trends is useful for surveillance and control of lung cancer. However, the reason for the increase in adenocarcinoma remains unclear and warrants investigation.

Abstract:Background Lung cancer is traditionally more prevalent in elderly people, men, and smokers, and is usually diagnosed at an advanced stage. However, as low-dose CT (LDCT) lung cancer screening is increasingly popular in some regions of China, and since it might lead to the detection of more small-sized lung cancers, we hypothesised the spectrum of lung cancer might change. Methods This observational study included three datasets. The first dataset included LDCT lung cancer screening results for employees from five hospitals in different regions of China in 2012–17. The second dataset included lung cancer incidence and mortality from the Shanghai Cancer Registry in 2005–14. The third dataset included clinicopathologic data for patients with primary lung cancer undergoing surgical resection in the Fudan University Shanghai Cancer Center (FUSCC), Shanghai, in 2008–17. To characterise trends in lung cancer incidence over time, we calculated the average annual percent change (APC) using Joinpoint (version 4.1.1.3) by the National Cancer Institute. We used the Cochrane Armitage trend test to determine if the proportion of the young, female, non-smoking, or stage 0/IA patients increased over the years. This study was approved by the ethics committee of each participating institution. Findings LDCT screening detected lung cancer in 164 hospital employees with pathological confirmation. Of the hospital employees with lung cancer, a high proportion were female (81·7%), non-smokers (91·5%), younger than 55 years (58·5%), and had stage 0/IA cancer (93·3%). The proportion of hospital employees with lung cancer was 1·2% in women and 0·7% in men. In Shanghai, lung cancer incidence in women increased significantly (APC 5·50%, p=0·0008) from 17·6 cases per 100 000 in 2005 to 25·8 cases per 100 000 in 2014, while the incidence in men did not change significantly (p=0·1255). Incidence in women increased significantly in all the age subgroups between 30 and 64 years. Among 8355 patients with lung cancer undergoing surgery in FUSCC in 2008–17, the proportion of women (from 32·8% to 55·7%), non-smokers (from 43·9% to 68·5%), patients aged 40 years or younger (from 2·2% to 8·6%), and patients with stage 0/IA cancer (from 32·2% to 73·0%) increased significantly from 2008 to 2017. Interpretation We observed an increasing trend of young, female, non-smoking, and early-stage lung cancer in China, which is in contradictory to classical concepts and brings new challenges and opportunities to change clinical practice. Abstract:Abstract Background: Lung cancer is the leading cause of cancer death worldwide. We explored the relationship between socioeconomic status and lung cancer incidence across world regions, using educational level as a proxy for socioeconomic status. Methods: We analyzed the harmonized database of the Lung Cancer Cohort Consortium (LC3). The current study included data from 18 prospective cohorts from 15 countries in the US, Europe, Asia, and Australia. Separately for participants who never or currently/formerly smoked, we estimated the association between educational level and incident lung cancer using Cox proportional hazards models. Information on education was harmonized using the International Standard Classification of Education and then modeled as an ordinal variable in 4 categories. Models were adjusted for age, sex, and for participants who currently/formerly smoked, smoking duration, cigarettes per day, and time since cessation. Results: Among 2.6 million participants from 15 countries, 62,645 developed lung cancer during follow-up (median follow-up = 12.6 years). Among current/former smokers, increased educational level was associated with decreased lung cancer incidence in most cohorts after adjustment for age, sex, and detailed smoking information, with HRs ranging from 0.77 (95%CI: 0.42-1.41) per 1-unit increase in educational level in the Iranian Golestan Cohort Study to 1.02 (95%CI: 0.95-1.09) in the Australian Melbourne Collaborative Cohort Study. When grouping by world region, the association between education and lung cancer incidence among currently/formerly smoking participants was similar for the US (HRpooled=0.88, 95%CI: 0.87-0.89), Europe (HRpooled=0.89, 95%CI: 0.88-0.91), and Asia (HRpooled=0.91, 95%CI: 0.86-0.96), but attenuated in the Australian cohort (HR=1.02, 95%CI: 0.95-1.09). Among never smokers, after adjustment for age and sex, there was no statistically significant association between educational level and lung cancer incidence (p-trend>0.05 in all cohorts), with the exception of the US Southern Community Cohort Study, which comprises primarily African-Americans and showed a HR of 0.75 (95%CI: 0.62-0.90). Conclusion: Among currently and formerly smoking individuals, higher educational level showed a strikingly consistent decreased risk of incident lung cancer across cohorts from 4 continents, after detailed adjustment for smoking. In contrast, among people who never smoked, there was no association between education and lung cancer incidence in any cohort, with the exception of the Southern Community Cohort Study. Further research is needed to clarify the mechanisms, either related or unrelated to smoking, that contribute to the association between education and lung cancer risk. Citation Format: Justina U. Abstract:Background Several screening models have recently been applied to study awareness and help people make informed decisions regarding cancer screening. Aims This study aimed to explore the knowledge, attitudes, and lung cancer screening health beliefs (LCSHBs) among Jordanians. Moreover, we intended to translate the LCSHBs scale into the Arabic language and test its validity and reliability. Methods An online survey (Google Forms) was completed by 916 participants in Jordan via snowball sampling technique from February 10, 2024, to March 2, 2024, assessing lung cancer screening knowledge, lung cancer preventive attitudes, screening‐related health beliefs, and stages of screening adoption. Linear regression analysis, correlations, and non‐parametric statistics were utilized for statistical inference. Results Participants had high mean percentage scores of 86.56% and 85.75% for knowledge and preventive attitudes towards lung cancer screening, respectively. The Arabic version of the LCSHBs model was found to be a valid and reliable tool, as indicated by principal component analysis extraction and Cronbach's alpha values. Lung cancer screening knowledge was positively associated with perceived benefits and negatively associated with perceived barriers to screening. Abstract:Background: Lung cancer ranks second for cancer incidence and first for cancer mortality. Investigation into its risk factors and epidemiologic trends could help describe geographical distribution and identify high-risk population groups. Research question: What is the global incidence, mortality, associated risk factors, and temporal trends of lung cancer by sex, age, and country? Study design and methods: Data on incidence and mortality were retrieved from the Global Cancer Observatory (GLOBOCAN), Cancer Incidence in Five Continents series I-X, World Health Organization (WHO) mortality database, the Nordic Cancer Registries (NORDCAN), and the Surveillance, Epidemiology, and End Results Program (SEER). We searched the WHO Global Health Observatory data repository for age-adjusted prevalence of current smoking. The Average Annual Percentage Change (AAPC) of the trends were obtained by Joinpoint Regression. Results: The age-standardized rate of incidence and mortality were 22.4 and 18.0 per 100,000 globally. The lung cancer incidence and mortality were associated with Human Development Index (HDI), Gross Domestic Products (GDP), and prevalence of smoking. For incidence, more countries had increasing trends in females but decreasing trends in males (AAPC, 1.06 to 6.43 for female; -3.53 to -0.64 for male). A similar pattern was found in those 50 years or older, whereas those aged younger than 50 years had declining incidence trends in both sexes in most countries. For mortality, similar to incidence, 17 of 48 countries showed decreasing trends in males and increasing trends in females (AAPC, -3.28 to -1.32 for male, 0.63 to 3.96 for female). Interpretation: Most countries had increasing trends in females but decreasing trends in males and in lung cancer incidence and mortality. Tobacco related measures and early cancer detection should be implemented to control the increasing trends of lung cancer in females, and in regions identified as having these trends. Future studies may explore the reasons behind these epidemiological transitions.

Abstract:This is the first comprehensive analysis of lung tumors in Sri Lanka and one of Asia's largest, featuring detailed histopathological analysis and demographic correlations. Adenocarcinomas were prevalent among primary malignant tumors, with colorectal adenocarcinomas leading in metastatic deposits. Crucially, our findings revealed correlations between patient demographics and tumor types, emphasizing the pivotal role of age and gender in diagnostic assessments. The prevalence of lung cancer is steadily increasing globally, and it is projected to become the second most prevalent cancer in men by 2030. Lung cancer is the leading cause of cancer‐related deaths worldwide, accounting for approximately 3.61% of total fatalities. Despite its significant impact, many Asian countries, including Sri Lanka, lack precise data on the epidemiological patterns of lung tumors. This study pioneers a comprehensive exploration in Sri Lanka, delving into the demographic and clinicopathological characteristics of lung cancer patients. The study included 733 consecutive patients with lung tumors from 2017 to 2021, with a median age of 59 years. The most common site of tumors was the right lower lobe and left upper lobes. Adenocarcinoma was the most prevalent histopathological type of primary malignant lung tumors, while colorectal adenocarcinomas were the most common cause of metastatic deposits in the lungs. The most common benign tumor was hamartoma. Significantly, our findings unveiled associations between patient demographics and tumor types, underscoring the importance of factoring in age and gender in diagnostic assessments. Notably, the absence of a dedicated lung cancer screening program in Sri Lanka underscores the critical reliance on clinical suspicion and accurate diagnostic methods.
