Abstract:Whole-body vibration (WBV), a training method based on the stimulation of muscle contraction by mechanical vibration generated in a vibrating platform, is claimed to be effective in diabetes management. This meta-analysis evaluated WBV effects against other exercises, placebo, or no intervention in type-2 diabetes. Medline, Scopus, and Web of Science databases were systematically searched through June 2023. Randomized controlled trials reported the effect of WBV on glucose (hemoglobin A1C and fasting blood glucose), and lipid profiles (total cholesterol, triglycerides, high, and low-density lipoprotein) were included. Two researchers independently extracted the characteristics of the studies, participants, WBV intervention and comparisons, and the outcomes from the included articles. The Physiotherapy Evidence Database (PEDro) scale assessed trial quality. In this review, all articles had no high risk of bias according to the PEDro scale, with studies achieving optimal, excellent, and good scores. Network meta-analysis revealed that WBV was effective for reducing hemoglobin A1C when compared with conventional (mean difference: − 1.58%, 95%CrI: − 2.51, − 0.47) and resistance exercise (mean difference: − 1.32%, 95%CrI: − 1.96, − 0.33). WBV had also a desirable but insignificant effect on hemoglobin A1C compared to stretching and balance exercises, placebo, and no intervention. The current pairwise meta-analysis did not show that WBV favors fasting blood glucose and lipids. WBV may have potential advantages for glycemic control in type-2 diabetes. However, uncertainties in the findings remain due to the limited number of studies and their heterogeneity.

Abstract:Background: Vibration platforms have demonstrated systemic effects generated by the use of mechanical vibrations, which are similar to those of any physical activity. The effect that whole body vibration (WBV) generates on the organism could be recommended in Diabetes Mellitus 2 (DM 2) patients. Objective: To systematically review and meta-analyze the available evidence on the effects of WBV on glycemic control in patients with DM 2. Material and methods: Exhaustive bibliographic searches were carried out until October 2023 in different biomedical portals and databases: Public Medline (PubMed), Scientific Electronic Library Online (SciELO), VHL Regional Portal, Cochrane Central and Latin American and Caribbean Literature in Health Sciences (LILACS). Randomized clinical trials based on the effects of Whole Body Vibration on glycosylated hemoglobin levels, with control group and participants were non-insulin dependent were the inclusion criteria. Two reviewers extracted data independently. A third reviewer was available for discrepancies. Results: Six articles with 223 participants met the criteria and were included in the systematic review; only four of them met the criteria to be part of the meta-analysis. This meta-analysis reveals a positive and significant effect size (μ ê=0.5731), indicating a substantial difference between the groups studied. Although there is some variability between studies (heterogeneity of 30.05%), the overall direction of the effects is consistent. These findings conclusively suggest the presence of a significant influence of the variable evaluated, underscoring the robustness and consistency of the relationship observed in the literature reviewed. Conclusion: There are no conclusive results due to the lack of data for some variables, which prevents comparison; but WBV may be an effective therapy to improve glycemic control in DM 2 patients. More studies with more patients and longer follow-up are needed.

Abstract:Vascular endothelium insults caused by high serum glucose levels affect the oxygen supply to tissues, via the microvascular endothelium, resulting in an increased perfusion heterogeneity. These insults may lead to the underuse of blood capillaries, while other vessels are overused and effectively overload their oxygen supply capacity, which eventually causes damages to distal parts of the peripheral nervous system. Therefore, the proprioceptive and exteroceptive feedback information will be gradually lost and contribute to a mobility reduction. This study aims to assess the efficacy of whole-body vibration (WBV) associated with strength training (ST) on lower-limb blood flow and mobility in older adults with type 2 diabetes (DM2). Methods and analyses: This is a protocol (1st version) for Pa single-blind, randomized, controlled clinical trial guided by the SPIRIT guidelines. Our sample will consist of 51 older adults with DM2 randomly allocated to three groups: low frequency WBV (16–26 Hz) associated to ST (G1), WBV sham (G2) and nonintervention control (G3). The study protocol is set for a 12-week (three times per week) schedule. Primary outcomes: skin temperature using infrared thermographic imaging (ITI); mean peripheral arterial blood flow velocity (MBF) by a handheld Doppler ultrasound (DU), and functional mobility by Timed Up and Go (TUG) test. Secondary outcomes: quasi-static posture using the DX100 BTS Smart optoelectronic system, and plantar pressure and body balance using the MPS stabilometric platform. Data will be collected and analyzed at baseline and post-intervention, considering p-value < 0.05 level of significance. The analyses will also be conducted with an intention-to-treat method and effect size. Dissemination: All results will be published in peer-reviewed journals as well as presented in conferences.

Abstract:This study was designed to evaluate the effects of whole body vibration (WBV) exercise, mental training (MT), and the concurrent effect of WBV and MT on lower body balance, neuromuscular performance, and leg muscle strength in elderly men. In a randomized control trial study with parallel group design, 42 older men (mean±standard deviation age, 68±5.78 years) were randomly divided into four groups: WBV (n=11), MT (n=12), WBV+MT (n=10), and control (n=9) groups. The protocol of training consisted of three sessions per week for 8 weeks and about 30 min for each session. The WBV exercise was performed on a vibration machine. In MT group, participants were asked to mentally visualize to do the Timed Up and Go and relaxation technics. postural stability, the Timed Up and Go test, 5-repetition chair-rising test, 6-m tandem gait test, 10-m walking, and leg isometric strength were measured in baseline and after 8-week intervention. Repeated measures-analysis of variance followed by <i>post hoc</i> was used to analyses the data. The results of this study showed that there were significant improvements (<i>P</i><0.05) in postural stability, Timed Up and Go, 5-repetition chair-rising, 6-m tandem gait test, 10-m walking, and leg isometric strength in WBV, MT, and WBV+MT in comparison with baseline and in comparison with control group. It seems that older adults can take benefit from WBV and MT and WBV+MT exercise as a cost-effective and practical way without side effects to improve postural control, mobility, and functional performance.

Abstract:Objective To evaluate the effects of whole-body vibration training (WBVT) on lower limb muscle strength and physical performance in older adults. Data Sources Web of Science, PubMed, Cochrane Library, and MEDLINE databases were searched for papers published in English, from January 1, 2000, to May 30, 2022. Study Selection Randomized controlled trials of WBVT in older adults (mean age, 65 years or older) published in English. The PEDro Scale was used to assess the quality of the selected studies. Data Extraction Two investigators independently assessed articles according to the evaluation criteria. Differences between investigator assessments were resolved by consulting a third investigator before reassessment. Data Synthesis Systematic review of 18 randomized controlled studies found that WBVT produced significant improvements in lower limb muscle strength and physical performance among older adults. We used the Cochrane Collaboration method to assess risk of bias and RevMan version 5.4 to extract means and calculate SDs. WBVT significantly improved knee strength (standard mean difference [SMD]=0.72, 95% CI [0.38, 1.07], p<0.0001, I 2 =58%) and explosive power (SMD=0.47, 95% CI [0.10, 0.83], p=0.01, I 2 =0%) among older adults. Significant improvements in the each physical performance were observed in the sit-to-stand test (SMD=0.57, 95% CI [0.30, 0.84], p<0.0001, I 2 =35%), the subgroup of studies that evaluated balance with TUG test, SMD was 0.53 (95% CI [0.19, 0.88], p=0.002, I 2 =56%) and the Tinetti total score, SMD was 0.72 (95% CI [0.04, 1.41], p=0.04, I 2 =81%), walking speed (SMD=0.46, 95% CI [0.14, 0.77], p=0.005, I 2 =49%) and walking endurance (SMD=0.43, 95% CI [0.02, 0.85], p=0.04, I 2 =24%). Conclusions WBVT may be an effective intervention to improve lower limb muscle strength and physical performance in older adults. Tinetti total score remains controversial and warrants assessment in future high-quality randomized controlled trials.

Abstract:Objectives To investigate the feasibility and benefits of WBV training in enhancing muscle strength and physical performance in older inpatients with sarcopenia. Methods We included 40 patients with sarcopenia (mean age 85 years; age range 80~90 years) and randomly separated into the WBV group or control (CON) group. The WBV group underwent 8 weeks of conventional rehabilitation training combined with WBV, whereas the CON group received 8 weeks of conventional rehabilitation training alone. The primary outcome was maximal isometric muscle strength. The secondary outcomes were the time-up-and-go test (TUGT), the five-times sit-to-stand test (FTSST), the 6-m gait speed test, and the Morse Fall Scale (MFS). Results After an 8-week intervention, only the quadriceps and tibialis anterior muscles in the CON group showed substantial gains in strength. In the WBV group, significant increases were observed in iliopsoas (P<0.05), quadriceps (P<0.01), hamstring(P<0.01), and tibialis anterior muscle (P<0.01) respectively. For all physical performance assessments, improvements were observed in both CON and WBV groups (P<0.05), the improvement in the WBC group was greater than the CON group (P 0.05). Conclusion This result highlights a eight weeks of whole-body vibration training can increase the lower-limb muscular strength and physical performance of older inpatients with sarcopenia. Ziele Untersuchung der Durchführbarkeit und des Nutzens von WVV Training zur Verbesserung der Muskelkraft und körperlichen Leistungsfähigkeit bei älteren stationären Patienten mit Sarkopenie. Methoden Wir haben 40-Patienten mit Sarkopenie (mittleres Alter 85 Jahre; Altersbereich 80~90 Jahre) einbezogen und zufällig in die WVV-Gruppe oder Kontrollgruppe (CON) getrennt. Die WBV-Gruppe durchlief acht Wochen konventionelles Rehabilitationstraining kombiniert mit WBV, während die CON-Gruppe acht Wochen konventionelles Rehabilitationstraining allein erhielt. Das primäre Ergebnis war die maximale isometrische Muskelkraft. Die sekundären Ergebnisse waren der Time-up-and-go Test (TUGT), der fünfmalige Sitz-to-Stand Test (FTSST), der 6-m Ganggeschwindigkeitstest und die Morse Fall Scale (MFS). Ergebnisse Nach einer 8-wöchigen Intervention zeigten nur die vorderen Quadrizeps- und Tibialismuskeln der CON-Gruppe deutliche Kraftzuwächse. In der WBV-Gruppewurden signifikante Erhöhungen in Iliopsoas (P<0,05), Quadrizeps (P<0,01), Hamstring (P<0,01) und Tibialis anterior Muskel (P<0,01) beobachtet. Bei allen körperlichen Leistungsbeurteilungen wurden Verbesserungen sowohl in CON- als auch in WBV-Gruppen beobachtet (P<0,05), die Verbesserung in der WBC-Gruppe war größer als in der CON-Gruppe (P 0,05). Abstract:Purpose: To compare the effects of whole-body vibration training (WBVT) with different frequencies on the balance ability of older adults. Methods: Randomized controlled trials (RCTs) on the WBVT interventions on balance ability in older adults were searched through PubMed, Web of Science, The Cochrane Library, ProQuest, Embase, Opengrey, China National Knowledge Infrastructure (CNKI), Wanfang, and China Science and Technology Journal Database (CSTJ) databases from the establishment of the database to August 2022, and all literature that met the PICOS (Participants, Intervention, Comparison, Outcomes, Study design) criteria were enrolled. Two reviewers screened and assessed the methodological quality of the included literature according to the physiotherapy evidence database (PEDro) scale criteria. Statistical analysis was performed using Stata 14.0 software after data extraction. Results: Twenty-five RCTs with a total of 1267 subjects were finally included. The results of the pairwise comparison of the Network Meta-analysis showed that the Timed Up and Go Test (TUGT) values of Low-frequency whole-body vibration training (LF-WBVT) was lower than the placebo and traditional rehabilitation groups, and the difference was statistically significant [WMD = −1.37, 95% CI (−2.53, −0.20)] [WMD = −1.84, 95% CI(-3.17,-0.51)]. The Five-repetition Sit-to-Stand Test (5STS) values of LF-WBVT, Medium-frequency whole-body vibration training (MF-WBVT), and High-frequency whole-body vibration training (HF-WBVT) were lower than the placebo and traditional rehabilitation groups, but none of them were statistically significant. In addition, the TUGT and 5STS values of HF-WBVT had a tendency to be lower than those of LF-WBVT and MF-WBVT, but neither of them was statistically different. The cumulative probability ranking results of both TUGT and 5STS showed that HF-WBVT was the best protocol. Conclusion: Current evidence shows that HF-WBVT may be the best protocol for improving balance in older adults. Due to the study's limitations, the conclusion obtained in this study still needs to be further confirmed by more high-quality studies. Abstract:Abstract Objective Loss of functional independence is more likely in older adults who reside in an institution as a consequence of a decline in muscle mass and a loss of force production capacity. The aim of this review was to assess the effect of whole-body vibration (WBV) interventions on the strength, balance, and mobility of nursing home residents older than 80 years. Methods An electronic search in MEDLINE, Scopus, and CINAHL databases was conducted. Randomized controlled trials that involved nursing home residents older than 80 years, that investigated WBV interventions compared with nonintervention, usual care, or placebo, and that measured physical function outcomes including strength, balance, gait, and mobility were included. Risk of bias was assessed by 2 reviewers using the Physiotherapy Evidence Database scale. The standardized mean difference (SMD) between the experimental and control groups were calculated with a random-effects model for each outcome and subgroup analysis was conducted for different outcomes. Results In total, 2864 articles were identified; of these, 14 randomized controlled trials met the inclusion criteria. The meta-analysis revealed that WBV significantly increased the lower limb muscle strength (SMD = 0.59; 95% CI = 0.16 to 1.03), mobility (SMD = 0.45; 95% CI = 0.10 to 0.81), gait score (SMD = 0.26; 95% CI = 0.01 to 0.51), balance (SMD = 0.41; 95% CI = 0.01 to 0.81), and physical performance (SMD = 1.33; 95% CI = 0.33 to 2.33). Conclusion WBV may be an effective intervention to improve the strength, balance, mobility, walking ability, and physical performance of older nursing home residents. Impact WBV presents a safe, accessible alternative for improving health in this vulnerable population, warranting further research and integration into health care practices.

Abstract:Whole-body vibration (WBV) is a novel exercise training measure that promotes the muscle strength, flexibility, and balance abilities of elderly groups. The feasibility and applicability of 20-30 min (lowering a heat pack at 73 °C by wrapping it in multiple layers of towels to 40-43 °C before it touched the skin) thermotherapy are increasingly being demonstrated by applications and clinical trials. Studies show that it increases the flexibility of macules and ligament. However, no studies have examined the interactions between the pre-exercise and post-exercise application of heat therapy (duration a training course). Therefore, this study investigates the effects of WBV and heat therapy on the muscle strength, flexibility, and balance abilities of elderly groups. Eighty middle-age and elderly participants with no regular exercise habits were enrolled in this study. They were randomly assigned to a WBV group, a WBV plus heat therapy group, a heat therapy alone group, and a control group. The WBV groups underwent 5-min, fixed-amplitude (4 mm), thrice-weekly WBV training sessions for 3 consecutive months on a WBV training machine. Participants' balance was measured using the limits of stability (LOS) test on a balance system. The pretest and posttest knee extensor and flexor strength were tested using an isokinetic lower extremity dynamometer. Pretest and posttest flexibility changes were measured using the sit-and-reach test. Significantly larger pretest and posttest differences in flexibility and muscle strength were observed in the WBV and WBV plus heat therapy groups. The addition of heat therapy to WBV resulted in the largest flexibility improvements.

Abstract:Objective: To assess the effects of dietary intervention combined with comprehensive exercise instruction on somatic function and quality of life in elderly patients with diabetes mellitus (DM). Methods: This was application research. A total of 120 elderly patients with type-2 diabetes mellitus (T2DM) admitted to The No.2 Hospital of Baoding from March 10, 2022 to March 10, 2023 were included and randomly divided into the control group and the experimental group(n=60). Patients in the control group received conventional treatment and nursing regimen, while those in the experimental group were given dietary intervention combined with comprehensive exercise instruction based on the control group. The differences before and after treatment between the two groups were compared and analyzed. Results: After the intervention, the experimental group had remarkably lower levels of fasting blood glucose (FBG), 2h postprandial blood glucose(2hPG) and glycated hemoglobin (HbA1c) than the control group (p=0.00), while significantly improved somatic function, psychological function, social function and material life status compared to the control group (p=0.00). The levels of SAS and SDS in the experimental group were significantly decreased compared with those in the control group (p=0.00). The levels of SOD, MDA and CAT in the experimental group were obviously superior to those in the control group, with statistically significant differences (p=0.00). Conclusion: Dietary intervention combined with comprehensive exercise instruction is an effective treatment for elderly patients with DM, boasting a variety of benefits such as regulating patients' blood glucose levels, improving patient satisfaction, and ameliorating the level of oxidative stress in the body, which is worthy of clinical promotion. doi: https://doi.org/10.12669/pjms.40.6.7897 How to cite this: Sha X, Zhao X, Chen Y. Effects of whole-body vibration on bone properties in type 2 diabetes model rats

Abstract:Objectives: We examined the effects of whole-body vibration (WBV) on bone properties in type 2 diabetes (T2DM) model rats. Methods: Twenty male Hos:ZFDM-Lepr fa , fa/fa rats (divided into DM and WBV groups, N = 10 each) and 10 Hos:ZFDM-Lepr fa , fa/+ rats (as the control (CON) group) were used. The WBV group underwent WBV at 45 Hz frequency, with 0.5 g acceleration (15 min/day, 5 days/week) for 8 weeks. Trabecular and cortical bone mass, trabecular bone microstructure (TBMS), and cortical bone geometry (CBG) were analyzed via micro-computed tomography. Bone mechanical strength (maximum load, break point, and stiffness) was also measured. Additionally, bone metabolic and DM-related markers were determined. Results: The bone mechanical strength of the femur improved in the WBV group, although muscle atrophy and bone deterioration were observed in the DM and WBV groups. The serum levels of bone-specific alkaline phosphatase and tartrate-resistant acid phosphatase-5b were significantly higher in the DM and WBV groups than in the CON group. Serum glucose and blood urea nitrogen levels were significantly lower in the WBV group than in the DM group. Conclusions: This study suggests that WBV potentially improves the decrease in the bone mechanical strength of the femur, although it does not prevent the deterioration of bone mineral content, TBMS, and CBG parameters. Further studies are needed to investigate the effective timing and duration of WBV and the conditions that prevent T2DM and deterioration of bone properties and clarify the mechanism underlying WBV effects on bone properties in DM animals.

Abstract:Abstract Background Whole-body vibration training (WBV) performed on a vibration platform can significantly improve physical performance in patients with chronic obstructive pulmonary disease. It has been suggested that an important mechanism of this improvement is based on an improvement in balance. Therefore, the aim of this study was to investigate the effects of WBV compared to conventional balance training. Methods 48 patients with severe COPD (FEV 1 : 37 ± 7%predicted) and low exercise performance (6 min walk distance (6MWD): 55 ± 10%predicted) were included in this randomized controlled trial during a 3 week inpatient pulmonary rehabilitation. All patients completed a standardized endurance and strength training program. Additionally, patients performed 4 different balance exercises 3x/week for 2 sets of 1 min each, either on a vibration platform (Galileo) at varying frequencies (5–26 Hz) (WBV) or on a conventional balance board (BAL). The primary outcome parameter was the change in balance performance during a semi tandem stance with closed eyes assessed on a force measurement platform. Muscular power during a countermovement jump, the 6MWD, and 4 m gait speed test (4MGST) were secondary outcomes. Non-parametric tests were used for statistical analyses. Results Static balance performance improved significantly more ( p = 0.032) in favor of WBV (path length during semi-tandem stand: − 168 ± 231 mm vs. + 1 ± 234 mm). Muscular power also increased significantly more ( p = 0.001) in the WBV group (+ 2.3 ± 2.5 W/kg vs. − 0.1 ± 2.0 W/kg). 6MWD improved to a similar extent in both groups (WBV: 48 ± 46 m, p < 0.001 vs. BAL: 38 ± 32 m; p < 0.001) whereas the 4MGST increased significantly only in the WBV-group (0.08 ± 0.14 m/s 2 , p = 0.018 vs. 0.01 ± 0.11 m/s 2 , p = 0.71). Conclusions WBV can improve balance performance and muscular power significantly more compared to conventional balance training. Abstract:Effectiveness of an exercise program designed for improving postural control and mobility in older adults with type 2 diabetes was investigated. Ninety-three adults 65 or older diagnosed with type 2 diabetes and able to walk unaided were recruited. The intervention group received exercise training focused on ankle strengthening and mobility twice per week for 10 weeks. The control group did not participate in any exercise program. After 10 weeks, the intervention group showed significantly greater improvement in the mean Sensory Organization Test composite score (4.4 vs. 0.3; p = 0.01) as well as visual ratio (0.1 vs. 0.002; p = 0.01) and vestibular ratio (0.1 vs. 0.003; p < 0.001) than the control group after adjusting for covariates. A greater trend of improvement in the Timed Up and Go and Single-Leg Stance Test was also found in the intervention group. Exercise training focusing on the ankle is effective in enhancing the postural stability of older adults with type 2 diabetes and can potentially be effective in improving single-leg standing balance and mobility. Effects of Whole-Body Vibration Training on Low Extremity Muscle Strength and Mobility in the Elderly:A Me-ta-Analysis

Abstract:Objective To evaluate the effects of whole-body vibration (WBV) training on low extremity muscle strength and mobility in older adults by a meta-analysis. Methods Articles were searched from Cochrane Central Register of Controlled Trials, PubMed, EMbase, Elsevier, CBMdisc, Wanfang database and CNKI database, and the reference list of each eligible article were also included. Randomized controlled trials which investigated the effect of WBV on mobility and low extremity muscle strength were included in this article. The meth-odological quality of each selected article was rated by the Jadad Scale. The data on patients' characteristics, type of WBV intervention and outcomes were extracted. A meta-analysis was then performed. Results 15 randomized controlled trials involving 803 subjects were includ-ed finally. 8 were considered to have good or excellent methodological quality and the rest was rated as fair. Meta-analyses revealed that WBV had a significant treatment effect on leg extension isometric strength (MD=10.11, 95%CI 0.46-19.76), knee extension dynamic strength (MD=7.98, 95%CI 2.60-13.36), and functional measures of mobility such as performance in Timed-Up-and-Go test (MD=1.39, 95%CI 0.73-2.04), and performance in five times sit to stand test (MD=1.62, 95%CI 0.25-2.98) (P<0.05), whereas no significant treatment effect on walk speed (MD=0.02, 95%CI-0.06-0.10) and Tinetti Performance Oriented Mobility Assessment (MD=1.19, 95%CI-0.62-3.00) (P>0.05), among older adults compared with the control group. Conclusion WBV may be effective on improving the low extremity muscle strength and mobility among older adults.

Abstract:Objective: To evaluate the effects of comprehensive exercise training on frailty, negative emotions and physical functions of elderly patients with diabetes. Methods: This is a retrospective study. A total of 140 elderly patients with T2DM in The No.2 Hospital of Baoding were selected from December, 2021 to June, 2023 and randomly divided into two groups, with 70 patients in each group. The control group was given routine nursing and routine exercise education, and the study group was additionally given comprehensive exercise training. Tilburg frailty indicator (TFI), emotional status, physical functions, grip strength, fasting blood glucose and patient satisfaction were compared and analyzed between the two groups. Results: Before the intervention, TFI showed no significant differences between the two groups (p>0.05). After the intervention, physical, psychological and social frailty in the study group were significantly lower than those in the control group, with statistically significant differences (p= 0.00). SAS and SDS scores reduced significantly in the study group compared with those in the control group after the intervention, with statistically significant differences (p=0.00). After the intervention, the grip strength was significantly larger while the fasting blood glucose was significantly lower in the study group compared with those in the control group, with statistically significant differences (p=0.00). Patient satisfaction in the study group was higher than in the control group, with a statistically significant difference(p=0.03). Conclusion: Comprehensive exercise training for elderly patients with diabetes is beneficial to improving their frail state, negative emotions, blood glucose levels and physical functions. Abstract:Background and objective: Sedentary behavior is of increasing concern in older patients with type 2 diabetes mellitus (T2DM) due to its potential adverse effects on cardiovascular health, cognitive function, and motor function. While regular exercise has been shown to improve the health of individuals with T2DM, the most effective exercise program for elderly sedentary patients with T2DM remains unclear. Therefore, the objective of this study was to assess the impact of high-intensity interval training (HIIT), moderate-intensity continuous training (MICT), and guideline-based physical activity programs on the cardiovascular health, cognitive function, and motor function of this specific population. Methods: This study will be a randomized, assessor-blind, three-arm controlled trial. A total of 330 (1:1:1) elderly sedentary patients diagnosed with T2DM will be randomly assigned the HIIT group (10 × 1-min at 85–95% peak HR, intersperse with 1-min active recovery at 60–70% peak HR), MICT (35 min at 65–75% peak HR), and guideline-based group (guideline group) for 12 weeks training. Participants in the guideline group will receive 1-time advice and weekly remote supervision through smartphones. The primary outcomes will be the change in glycosylated hemoglobin (HbA1c) and brain-derived neurotrophic factor (BDNF) after 12-weeks. Secondary outcomes will includes physical activity levels, anthropometric parameters (weight, waist circumference, hip circumference, and body mass index), physical measurements (fat percentage, muscle percentage, and fitness rate), cardiorespiratory fitness indicators (blood pressure, heart rate, vital capacity, and maximum oxygen), biochemical markers (high-density lipoprotein, low-density lipoprotein, triglycerides, total cholesterol, and HbA1c), inflammation level (C-reactive protein), cognitive function (reaction time and dual-task gait test performance), and motor function (static balance, dynamic balance, single-task gait test performance, and grip strength) after 12 weeks. Discussion: The objective of this study is to evaluate the effect of 12 weeks of HIIT, MICT, and a guideline-based physical activity program on elderly sedentary patients diagnosed with T2DM. Our hypothesis is that both HIIT and MICT will yield improvements in glucose control, cognitive function, cardiopulmonary function, metabolite levels, motor function, and physical fitness compared to the guideline group. Additionally, we anticipate that HIIT will lead to greater benefits in these areas. The findings from this study will provide valuable insights into the selection of appropriate exercise regimens for elderly sedentary individuals with T2DM. Ethics and dissemination: This study has been approved by the Ethics Review Committee of the Reproductive Hospital Affiliated with China Medical University (approval number: 202203). Informed consent will be obtained from all participants or their guardians. Upon completion, the authors will submit their findings to a peer-reviewed journal or academic conference for publication. Abstract:Regular physical exercise has anti-inflammatory effects in elderly subjects. Yet, the inflammatory responses after whole body vibration (WBV) training, a popular exercise paradigm for the elderly, remain to be elucidated. This study assessed the effects of WBV training on the inflammatory response associated with toll-like receptors (TLRs) signaling pathways. Twenty-eight subjects were randomized to a training group (TG) or a control group (CG). TG followed an 8-week WBV training program. Blood samples were obtained before and after the training period in both groups. Peripheral blood mononuclear cells were isolated, and mRNA and protein levels of makers involved in the TLR2/TLR4 myeloid differentiation primary response gen 88 (MyD88) and TIR domain-containing adaptor inducing interferon (TRIF)-dependent pathways were analyzed. Plasma TNFα and C-reactive protein levels were also assessed. The WBV program reduced protein expression of TLR2, TLR4, MyD88, p65, TRIF and heat shock protein (HSP) 60, while HSP70 content increased. IL-10 mRNA level and protein concentration were upregulated, and TNFα protein content decreased, after WBV training. Plasma concentration of C-reactive protein and TNFα decreased in the TG. The current data suggest WBV may improve the anti-inflammatory status of elderly subjects through an attenuation of MyD88- and TRIF-dependent TLRs signaling pathways.

Abstract:Background: Several studies have shown that whole-body vibration training (WBVT) improves exercise capacity in patients with severe COPD. The aim of this study was to investigate the determinants of improved exercise capacity following WBVT. Methods: Seventy-four COPD patients (FEV1: 34 ± 9%predicted) were recruited during a 3-week inpatient pulmonary rehabilitation (PR) program. Conventional endurance and strength exercises were supplemented with self-paced dynamic squat training sessions (4bouts*2min, 3times/wk). Patients were randomly allocated to either a WBVT-group performing squat training on a side-alternating vibration platform (Galileo) at a high intensity (24-26 Hz) or a control group performing squat training without WBVT. Results: Patients in the WBVT group significantly improved postural balance in several domains compared to the control-group (i.e. tandem stance: WBVT +20% (95%CI 14 to 26) vs. control -10% (95%CI 6 to 15), p < 0.001; one-leg stance: WBVT +11% (95%CI 4 to 19) vs. control -8% (95%CI -19 to 3), p = 0.009). Six-minute walk distance and muscle power but not muscle strength were also significantly improved compared to control group. Conclusions: Implementation of WBVT improves postural balance performance and muscle power output. The neuromuscular adaptation related to improved balance performance may be an important mechanism of the improvement in exercise capacity after WBVT especially in COPD patients with impaired balance performance and low exercise capacity.

Abstract:Purpose: The highly morbid condition known as sarcopenia affects middle-aged and elderly people and is characterized by a progressive loss of whole-body muscle mass accelerated by aging. It is also accompanied by a decline in muscle strength and physiological function. These weakening functions manifest as impaired physical movement, increased risk of falls, disability, lower quality of life, and even death. Sarcopenia is progressively emerging as a new public health issue in China with a quickly aging population, making it vital to investigate effective exercise therapies for sarcopenic patients. Methods: Sixty sarcopenic patients (aged 60-75) from a local hospital in northeast China participated in the study. They were randomly divided into a test group and a control group, with 30 patients in each. The test group underwent a 40-minute whole-body vibration training session for 12 weeks, five times a week, while the control group kept regular routines. Data were collected at the end of the experiment, including muscle strength, muscle mass, muscle function, gait speed, berg balance scale (BBS), five sit-to-stand test (FTSST), activities of daily living (ADL), fall risk assessment scale (FRA), and Sarcopenia Quality of Life Scale (SarQoL) scores. Results: The 12-week whole-body vibration training intervention resulted in a significantly higher value of muscular strength, muscle mass, and muscle function among the test group compared to the control group. The former achieved substantially higher SPPB, ADL, FTSST, and BBS scores than the latter (p < 0.01). With much lower FRA scores and significantly better SarQoL ratings compared to baseline, the test group also significantly outperformed the control group in terms of fall risk and quality of life (p < 0.01). Conclusion: With whole-body vibration training, people with sarcopenia can increase their skeletal muscle strength, considerably enhance their motor function, lower their risk of falls and accidents, and generally improve their quality of life. Whole-body vibration training is worthy of therapeutic use since it is both feasible and practical.

Abstract:Background: Whole-body vibration (WBV) training is a novel training method that stimulates the human neuromuscular system by the use of vibration, the frequency and amplitude of which are controlled, thereby inducing adaptive changes in the body. WBV training is widely used as a clinical prevention and rehabilitation tool in physical medicine and neuro-rehabilitation as a clinical prevention and rehabilitation tool. Objectives: The aim of the present study was to review the effects of WBV on cognitive function, provide an evidence-based foundation for future research on WBV training, and promote additional popularization and use of the methodology in clinical practice. Methods: A systematic review of articles extracted from the following six databases was conducted: PubMed, Web of Science, China National Knowledge Infrastructure, Embase, Cochrane, and Scopus. A literature search was performed on articles in which the effects of WBV on cognitive function were evaluated. Results: Initially, a total of 340 studies were initially identified, among which 18 articles that satisfied the inclusion criteria were selected for inclusion in the systematic review. Participants were allocated into two groups: patients with cognitive impairment and healthy individuals. The results demonstrated that WBV was both positive and ineffective in its influence on cognitive function. Conclusion: The majority of studies suggested that WBV may be a useful strategy for the management of cognitive impairment and should be considered for inclusion in rehabilitation programs. However, the impact of WBV on cognition requires additional, larger, and adequately powered studies.
