Abstract:Background: Obstructive sleep apnea (OSA) is a common sleep disorder. However, current diagnostic methods are labor-intensive and require professionally trained personnel. We aimed to develop a deep learning model using upper airway computed tomography (CT) to predict OSA and to warn the medical technician if a patient has OSA while the patient is undergoing any head and neck CT scan, even for other diseases.Methods: A total of 219 patients with OSA [apnea-hypopnea index (AHI) ≥10/h] and 81 controls (AHI <10/h) were enrolled. We reconstructed each patient's CT into 3 types (skeletal structures, external skin structures, and airway structures) and captured reconstructed models in 6 directions (front, back, top, bottom, left profile, and right profile). The 6 images from each patient were imported into the ResNet-18 network to extract features and output the probability of OSA using two fusion methods: Add and Concat. Five-fold cross-validation was used to reduce bias. Finally, sensitivity, specificity, and area under the receiver operating characteristic curve (AUC) were calculated.Results: All 18 views with Add as the feature fusion performed better than did the other reconstruction and fusion methods. This gave the best performance for this prediction method with an AUC of 0.882.Conclusions: We present a model for predicting OSA using upper airway CT and deep learning. The model has satisfactory performance and enables CT to accurately identify patients with moderate to severe OSA.

Abstract:This study aimed to develop a deep learning–based model to detect obstructive sleep apnea (OSA) using craniofacial photographs. Participants referred for polysomnography (PSG) were recruited consecutively and randomly divided into the training, validation, and test groups for model development and evaluation. Craniofacial photographs were taken from five different angles (front, right 90° profile, left 90° profile, right 45° profile, and left 45° profile) and inputted to the convolutional neural networks. The neural networks extracted features from photographs and outputted the probabilities of the presence of the disease. Sensitivity, specificity, and area under the receiver operating characteristic curve (AUC) were calculated using PSG diagnosis as the reference standard. Predicting Obstructive Sleep Apnea Based on Computed Tomography Scans Using Deep Learning Models

Abstract:Rationale: The incidence of clinically undiagnosed obstructive sleep apnea (OSA) is high among the general population because of limited access to polysomnography. Computed tomography (CT) of craniofacial regions obtained for other purposes can be beneficial in predicting OSA and its severity. Objectives: To predict OSA and its severity based on paranasal CT using a three-dimensional deep learning algorithm. Methods: One internal dataset (N = 798) and two external datasets (N = 135 and N = 85) were used in this study. In the internal dataset, 92 normal participants and 159 with mild, 201 with moderate, and 346 with severe OSA were enrolled to derive the deep learning model. A multimodal deep learning model was elicited from the connection between a three-dimensional convolutional neural network-based part treating unstructured data (CT images) and a multilayer perceptron-based part treating structured data (age, sex, and body mass index) to predict OSA and its severity. Measurements and Main Results: In a four-class classification for predicting the severity of OSA, the AirwayNet-MM-H model (multimodal model with airway-highlighting preprocessing algorithm) showed an average accuracy of 87.6% (95% confidence interval [CI], 86.8-88.6%) in the internal dataset and 84.0% (95% CI, 83.0-85.1%) and 86.3% (95% CI, 85.3-87.3%) in the two external datasets, respectively. In the two-class classification for predicting significant OSA (moderate to severe OSA), the area under the receiver operating characteristic curve, accuracy, sensitivity, specificity, and F1 score were 0.910 (95% CI, 0.899-0.922), 91.0% (95% CI, 90.1-91.9%), 89.9% (95% CI, 88.8-90.9%), 93.5% (95% CI, 92.7-94.3%), and 93.2% (95% CI, 92.5-93.9%), respectively, in the internal dataset. Furthermore, the diagnostic performance of the Airway Net-MM-H model outperformed that of the other six state-of-the-art deep learning models in terms of accuracy for both four- and two-class classifications and area under the receiver operating characteristic curve for two-class classification (P < 0.001). Conclusions: A novel deep learning model, including a multimodal deep learning model and an airway-highlighting preprocessing algorithm from CT images obtained for other purposes, can provide significantly precise outcomes for OSA diagnosis.
Predicting obstructive sleep apnea severity from craniofacial images using ensemble machine learning models

Abstract:Obstructive sleep apnea (OSA) is a prevalent disease affecting 10 to 15% of Americans and nearly one billion people worldwide. It leads to multiple symptoms including daytime sleepiness; snoring, choking, or gasping during sleep; fatigue; headaches; non-restorative sleep; and insomnia due to frequent arousals. Although polysomnography (PSG) is the gold standard for OSA diagnosis, it is expensive, not universally available, and time-consuming, so many patients go undiagnosed due to lack of access to the test. Given the incomplete access and high cost of PSG, many studies are seeking alternative diagnosis approaches based on different data modalities. Here, we propose a machine learning model to predict OSA severity from 2D frontal view craniofacial images. In a cross-validation study of 280 patients, our method achieves an average AUC of 0.780. In comparison, the craniofacial analysis model proposed by a recent study only achieves 0.638 AUC on our dataset. The proposed model also outperforms the widely used STOP-BANG OSA screening questionnaire, which achieves an AUC of 0.52 on our dataset. Our findings indicate that deep learning has the potential to significantly reduce the cost of OSA diagnosis.
Deep learning technique to detect craniofacial anatomical abnormalities concentrated on middle and anterior of face in patients with sleep apnea

Abstract:Objectives: The aim of this study is to propose a deep learning-based model using craniofacial photographs for automatic obstructive sleep apnea (OSA) detection and to perform design explainability tests to investigate important craniofacial regions as well as the reliability of the method. Methods: Five hundred and thirty participants with suspected OSA are subjected to polysomnography. Front and profile craniofacial photographs are captured and randomly segregated into training, validation, and test sets for model development and evaluation. Photographic occlusion tests and visual observations are performed to determine regions at risk of OSA. The number of positive regions in each participant is identified and their associations with OSA is assessed. Results: The model using craniofacial photographs alone yields an accuracy of 0.884 and an area under the receiver operating characteristic curve of 0.881 (95% confidence interval, 0.839-0.922). Using the cutoff point with the maximum sum of sensitivity and specificity, the model exhibits a sensitivity of 0.905 and a specificity of 0.941. The bilateral eyes, nose, mouth and chin, pre-auricular area, and ears contribute the most to disease detection. When photographs that increase the weights of these regions are used, the performance of the model improved. Additionally, different severities of OSA become more prevalent as the number of positive craniofacial regions increases. Conclusions: The results suggest that the deep learning-based model can extract meaningful features that are primarily concentrated in the middle and anterior regions of the face.

Abstract:Abstract Introduction Obstructive sleep apnea (OSA) is a complex and heterogeneous sleep-related breathing disorder, associated with systemic consequences such as hypertension, stroke, and cardiovascular diseases. Although initial screening tools such as STOP-BANG or the Berlin questionnaires have been developed, it is still challenging to capture the wide spectrum of the condition alone. Besides the questionnaires including clinical presentations, craniofacial abnormalities are also recognized as an important risk factor for detecting OSA. Herein we aim to develop an efficient approach to predict the risk of having moderate to severe OSA using machine learning techniques by incorporating anatomical information from 2D photographs. Methods This retrospective analysis included 348 patients, who completed answering the STOP-BANG questionnaires and took facial images at Dankook University Medical Center in South Korea between 2012 and 2022. A 1:1 Random under-sampling (RUS) method was applied to solve the imbalance problem between moderate to severe OSA cases (Apnea-Hypopnea Index ≥15 events/hour) and control. Balanced data were randomly divided into two data sets (training and validation: 80%; testing: 20%). We performed leave-one-out cross-validation (LOOCV) to seek the optimal parameters to improve the model performance and generalization using four machine learning models (logistic regression, random forest, support vector machine, XGBoost) in the training set. Thereafter, we adopted the model with the highest area under the receiver operating characteristic (AUROC) in the testing set. Finally, we evaluated the importance of each input feature in assessing OSA risk by calculating the Shapley additive explanations (SHAP) values. Results The logistic regression model achieved the best AUROC of 94.1% for predicting moderate to severe risk OSA. The value of craniofacial images built from deep learning algorithms was found to be a predominant contribution in the risk screening models of OSA with the aforementioned levels of severity. Conclusion The results of the study suggest that the proposed model improved the diagnosis of patients with moderate to severe OSA based on the combination of sleep-related questionnaires and 2D facial images by identifying the risk factors more efficiently. Abstract:Apnea and hypopnea are common sleep disorders characterized by the obstruction of the airways. Polysomnography (PSG) is a sleep study typically used to compute the Apnea-Hypopnea Index (AHI), the number of times a person has apnea or certain types of hypopnea per hour of sleep, and diagnose the severity of the sleep disorder. Early detection and treatment of apnea can significantly reduce morbidity and mortality. However, long-term PSG monitoring is unfeasible as it is costly and uncomfortable for patients. To address these issues, we propose a method, named DRIVEN, to estimate AHI at home from wearable devices and detect when apnea, hypopnea, and periods of wakefulness occur throughout the night. The method can therefore assist physicians in diagnosing the severity of apneas. Patients can wear a single sensor or a combination of sensors that can be easily measured at home: abdominal movement, thoracic movement, or pulse oximetry. For example, using only two sensors, DRIVEN correctly classifies 72.4% of all test patients into one of the four AHI classes, with 99.3% either correctly classified or placed one class away from the true one. This is a reasonable trade-off between the model's performance and the patient's comfort. We use publicly available data from three large sleep studies with a total of 14,370 recordings. DRIVEN consists of a combination of deep convolutional neural networks and a light-gradient-boost machine for classification. It can be implemented for automatic estimation of AHI in unsupervised long-term home monitoring systems, reducing costs to healthcare systems and improving patient care.

Abstract:Background /Objective: Automatic apnea/hypopnea events classification, crucial for clinical applications, often faces challenges, particularly in hypopnea detection. This study aimed to evaluate the efficiency of a combined approach using nasal respiration flow (RF), peripheral oxygen saturation (SpO2), and ECG signals during polysomnography (PSG) for improved sleep apnea/hypopnea detection and obstructive sleep apnea (OSA) severity screening. Methods An Xception network was trained using main features from RF, SpO2, and ECG signals obtained during PSG. In addition, we incorporated demographic data for enhanced performance. The detection of apnea/hypopnea events was based on RF and SpO2 feature sets, while the screening and severity categorization of OSA utilized predicted apnea/hypopnea events in conjunction with demographic data. Results Using RF and SpO2 feature sets, our model achieved an accuracy of 94 % in detecting apnea/hypopnea events. For OSA screening, an exceptional accuracy of 99 % and an AUC of 0.99 were achieved. OSA severity categorization yielded an accuracy of 93 % and an AUC of 0.91, with no misclassification between normal and mild OSA versus moderate and severe OSA. However, classification errors predominantly arose in cases with hypopnea-prevalent participants. Conclusions The proposed method offers a robust automatic detection system for apnea/hypopnea events, requiring fewer sensors than traditional PSG, and demonstrates exceptional performance. Additionally, the classification algorithms for OSA screening and severity categorization exhibit significant discriminatory capacity.

Abstract:Obstructive Sleep Apnea (OSA) is one of sleep-disordered breathing characterized by repetitive episodes of partial or complete obstruction of upper airway during sleep, leading to significant health risks. Polysomnography (PSG) is gold standard for diagnosing OSA, but the process is labor-intensive and time-consuming which is often inaccessible. This study proposes a novel deep learning-based framework for severity classification of OSA using physiological signals from PSG type III devices. The proposed method comprises two key models: ApneaDetectNet for detecting apnea/hypopnea events called respiratory events and SleepDetectNet for classifying sleep stages. ApneaDetectNet utilizes abdominal respiratory effort (Abdo), nasal pressure (AIRFLOW), and oxygen saturation (SpO2) signals, while SleepDetectNet uses only Abdo signal. Both models are built using convolutional neural networks (CNNs) and bidirectional long short-term memory (Bi-LSTM) networks. An AHI Estimator consequently calibrates the predicted Apnea-Hypopnea Index (AHI) to resemble clinical observations. Three public datasets were employed: Multi-Ethnic Study of Atherosclerosis (MESA), Wisconsin Sleep Cohort (WSC), and University College Dublin Sleep Apnea (UCDDB). These datasets were split into training, validation, and test sets. Results indicated that the proposed model achieved superior or on par performance compared with state-of-the-art models. The MESA model achieved an accuracy of 71.84%, a Kappa of 0.6057, a macro-averaged F1-score of 70.31%, and an R-square of 0.7739, while the WSC model reached an accuracy of 62.16%, a Kappa of 0.4843, a macro-averaged F1-score of 61.90%, and an R-square of 0.6092. The combined MESA & WSC models showed improved overall metrics. External validation with the UCDDB dataset demonstrated 84.00% accuracy with 0.7743 Kappa, a macro-averaged F1-score of 86.69%, and an R-square of 0.7755, showing the model's robustness. The accuracy, macro-averaged F1-score, and generalizability of the model suggest potential for real-world application.

Abstract:Purpose:This study aimed to propose a novel deep-learning method for automatic sleep apneic event detection and thus to estimate the apnea hypopnea index (AHI) and identify obstructive sleep apnea (OSA) in an event-by-event manner solely based on sleep sounds obtained by a noncontact audio recorder.Methods:We conducted a cross-sectional study of participants with habitual snoring or heavy breathing sounds during sleep to train and test a deep convolutional neural network named OSAnet for the detection of OSA based on sleep sounds. Polysomnography (PSG) was conducted, and sleep sounds were recorded simultaneously in a regular room without noise attenuation. The study was conducted in two phases. In phase one, eligible participants were enrolled and randomly allocated into training and validation groups for deep learning algorithm development. In phase two, eligible patients were enrolled in a test group for algorithm assessment. Sensitivity, specificity, accuracy, unweighted Cohen kappa coefficient (κ) and the area under the curve (AUC) were calculated using PSG as the reference standard.Results:A total of 135 participants were randomly divided into a training group (n, 116) and a validation group (n, 19). An independent test group of 59 participants was subsequently enrolled. Our algorithm achieved a precision of 0.81 and sensitivity of 0.78 in the test group for overall sleep event detection. The algorithm exhibited robust diagnostic performance to identify severe cases with a sensitivity of 95.6% and specificity of 91.6%.Conclusion:Our results showed that a deep learning algorithm based on sleep sounds recorded by a noncontact voice recorder served as a feasible tool for apneic event detection and OSA identification. This technique may hold promise for OSA assessment in the community in a relatively comfortable and low-cost manner. Further studies to develop a tool based on a home-based setting are warranted.
A novel deep learning model for obstructive sleep apnea diagnosis: hybrid CNN-Transformer approach for radar-based detection of apnea-hypopnea events

Abstract:Study objectives: The demand for cost-effective and accessible alternatives to polysomnography (PSG), the conventional diagnostic method for obstructive sleep apnea (OSA), has surged. In this study, we have developed and validated a deep learning model for detecting apnea-hypopnea events using radar data. Methods: We conducted a single-center prospective cohort study, dividing participants with suspected sleep-disordered breathing into development and temporally independent test sets. Utilizing a hybrid CNN-Transformer architecture, we performed 5-fold cross-validation on the development set to develop and subsequently validate the model. Evaluation metrics included sensitivity for event detection, mean absolute error (MAE), intraclass correlation coefficient (ICC), and Pearson correlation coefficient (r) for apnea-hypopnea index (AHI) estimation. Linearly weighted kappa statistics (κ) assessed OSA severity. Results: The development set comprised 54 participants (July 2021-May 2022), while the test set included 35 participants (June 2022-June 2023). In the test set, our model achieved an event detection sensitivity of 67.2% (95% CI: 65.8%, 68.5%) and demonstrated a MAE of 7.54 (95% CI: 5.36, 9.72), indicating good agreement (ICC = 0.889 [95% CI: 0.792, 0.942]) and a strong correlation (r = 0.892 [95% CI: 0.795, 0.945]) with the ground truth for AHI estimation. Furthermore, OSA severity estimation showed substantial agreement (κ = 0.780 [95% CI: 0.658, 0.903]). Conclusions: Our study highlights radar sensors and advanced AI models' potential to improve OSA diagnosis, paving the path for future radar-based diagnostic models in sleep medicine research.
Estimating the severity of obstructive sleep apnea using ECG, respiratory effort and neural networks

Abstract:OBJECTIVE: wearable sensor technology has progressed significantly in the last decade, but its clinical usability for the assessment of obstructive sleep apnea (OSA) is limited by the lack of large and representative datasets simultaneously acquired with polysomnography (PSG). The objective of this study was to explore the use of cardiorespiratory signals commonly available in standard PSGs which can be easily measured with wearable sensors, to estimate the severity of OSA.METHODS: an artificial neural network was developed for detecting sleep disordered breathing events using electrocardiogram (ECG) and respiratory effort. The network was combined with a previously developed cardiorespiratory sleep staging algorithm and evaluated in terms of sleep staging classification performance, apnea-hypopnea index (AHI) estimation, and OSA severity estimation against PSG on a large cohort of 653 participants with a wide range of OSA severity.RESULTS: four-class sleep staging achieved a κ of 0.69 with PSG, distinguishing wake, combined N1-N2, N3 and REM. AHI estimation achieved an intraclass correlation coefficient of 0.91, and high diagnostic performance for different OSA severity thresholds.CONCLUSIONS: this study highlights the potential of using cardiorespiratory signals to estimate OSA severity, even without the need for airflow or oxygen saturation (SpO2), traditionally used for assessing OSA.SIGNIFICANCE: while further research is required to translate these findings to practical and unobtrusive sensors, this study demonstrates how existing, large datasets can serve as a foundation for wearable systems for OSA monitoring. Ultimately, this approach could enable long-term assessment of sleep disordered breathing, facilitating new avenues for clinical research in this field.

Deep learning approaches for assessing pediatric sleep apnea severity through SpO2 signals

Abstract:Pediatric Sleep Apnea–Hypopnea (SAH) presents a significant health challenge, particularly in diagnostic contexts, where conventional Polysomnography (PSG) testing, although effective, can be distressing for children. Addressing this, our research proposes a less invasive method to assess pediatric SAH severity by analyzing blood oxygen saturation (SpO2) signals. We adopted two advanced deep learning architectures, namely ResNet-based and attention-augmented hybrid CNN-BiGRU models, to process SpO2 signals in a one-dimensional (1D) format for Apnea–Hypopnea Index (AHI) estimation in pediatric subjects. Employing the CHAT dataset, which includes 844 SpO2 signals, the data was partitioned into training (60%), testing (30%), and validation (10%) sets. A predefined validation subset was randomly selected to ensure the models' robustness via a threefold cross-validation approach. Comparative analysis revealed that while the ResNet model attained an average accuracy of 72.9% across four SAH severity categories with a kappa score of 0.57, the CNN-BiGRU-Attention model demonstrated superior performance, achieving an average accuracy of 75.95% and a kappa score of 0.63. This distinction underscores our method's efficacy in both estimating AHI and categorizing SAH severity levels with notable precision. Further, to evaluate diagnostic capabilities, the models were benchmarked against common AHI thresholds (1, 5, and 10 events/hour) in each test fold, affirming their effectiveness in identifying pediatric SAH. This study marks a significant advance in the field, offering a non-invasive, child-friendly alternative for pediatric SAH diagnosis. Although challenges persist in accurately estimating AHI, particularly in severe cases, our findings represent a critical stride towards improving diagnostic processes in pediatric SAH.

Abstract:Obstructive Sleep Apnea (OSA) is a common sleep disorder which causes poor sleep quality, daytime drowsiness, memory loss, and lack of attention, and can contribute to accidents. Long-term OSA is associated with cardiovascular disease, high blood pressure, arrhythmia and diabetes. Early discovery and treatment of OSA can significantly reduce the difficulty and cost of treatment. OSA detection uses a scale method which is prone to subjective errors. Polysomnography (PSG) requires long detection times, entails equipment which can be difficult to obtain and high cost, and does not provide immediate diagnosis or the establishment of preventative treatment. Human biosignals have been found to provide a faster and more objective means of sleep apnea diagnosis, allowing for the early treatment of OSA. By identifying key physiological characteristics and observing their change over time, one can assess the severity of the patient’s condition, and appropriate treatment can reduce the likelihood of disease onset or slow its progression. This study applies machine learning techniques to electroencephalography (EEG), electrocardiography (ECG), oxygen saturation (SpO2) and other complex physiological characteristics to establish a model for OSA severity and evaluate the effectiveness of various machine learning methods to detect sleep apnea. In addition, the fuzzy C-mean (FCM) algorithm is used to construct a predictive model for OSA to assist physicians in the early clinical detection of symptoms. The study found that patients with severe sleep apnea have a self-rescue condition which causes wakening due to respiratory arrest. Only relying on SpO2 and the Apnea Hypopnea Index (AHI) will cause severe OSA cases to be misidentified as mild, and this study found that the A5 band brain wave can help detect sleep apnea, reducing the possibility of miscategorization and assisting physicians in accurate clinical diagnosis.

Automated Detection of Sleep Apnea Using Machine Learning: A Novel Approach Using Smartphone and Microphone for Breathing Sound Analysis

Abstract:In this study, we evaluate the accuracy of a novel setup in the detection of apneas and hypopneas and estimating the apnea-hypopnea index (AHI). The study device setup consists of a microphone placed underneath the nose and a smartphone to collect the data. We recruited patients who were referred to the St. Josephs Hamilton Sleep Clinic for a sleep study. Data from our study device is collected simultaneously with polysomnography (PSG) in the sleep lab. A total of 26 patients were recruited, of which 2 dropped out during the data collection. Data from the microphone was too noisy for interpretation in 3 patients. Across the remaining 21 patients, the AHI based on their PSG ranged from 2 to 125 events/h, with an average AHI of 34 events/h. We used regression models trained on microphone audio data to identify noise and we developed an algorithm based on root-mean-square of the audio data for automatic detection of apneas and hypopneas. With reference to the PSG, our study device had a sensitivity of 94% and specificity of 87% in detecting apneas/hypopneas across a cumulative 120.9 hours of sleep data and more than 3700 such events. Our study device was able to accurately predict the AHI to within 4.3 events/h (+/-3.1). As such, we can conclude that our study device is accurate in identifying apneas/hypopneas, estimating the AHI and at ruling out cases of severe sleep apnea and can potentially be used as a screening test for this purpose. Our study device has the practical advantages of being very low cost and potentially more accessible as a screening tool, though further validation studies are needed to study accuracy across a larger population and for use at-home.

Abstract:Obstructive Sleep Apnea (OSA) is a common disorder affecting both adults and children. It is characterized by repeated episodes of apnea (stopped breathing) and hypopnea (reduced breathing), which result in intermittent hypoxia. We recognize pediatric and adult OSA, and this paper focuses on pediatric OSA. While adults often suffer from daytime sleepiness, children are more likely to develop behavioral abnormalities. Early diagnosis and treatment are important to prevent negative effects on children's development. Without the treatment, children may be at increased risk of developing high blood pressure or other heart problems. The gold standard for OSA diagnosis is the polysomnography (sleep study) PSG performed at a sleep center. Not only is it an expensive procedure, but it can also be very stressful, especially for children. Patients have to stay at the sleep center during the night. Therefore, screening tools are very important. Multiple studies have shown that OSA screening tools can be based on facial anatomical landmarks. Anatomical landmarks are landmarks located at specific anatomical locations. For the purpose of the screening tool, a specific list of anatomical locations needs to be identified. We are presenting a survey study of the automatic identification of these landmarks on 3D scans of the patient's head. We are considering and comparing both knowledge-based and AI-based identification techniques, with a focus on the development of the automatic OSA screening tool.

Abstract:Feed costs in dairy production systems may be decreased by extending the grazing season to periods such as autumn when grazing low-mass pastures is highly probable. The aim of this autumn study was to determine the effect of corn silage supplementation [0 vs. 8 kg of dry matter (DM) of a mixture 7:1 of corn silage and soybean meal] on pasture intake (PI), milk production, and grazing behavior of dairy cows grazing low-mass ryegrass pastures at 2 daily pasture allowances (PA; low PA=18 vs. high PA=30 kg of DM/cow above 2.5 cm). Twelve multiparous Holstein cows were used in a 4 × 4 Latin square design with 14-d periods. Pre-grazing pasture mass and pre-grazing plate meter pasture height averaged 1.8 t of DM/ha (above 2.5 cm) and 6.3 cm, respectively. The quality of the offered pasture (above 2.5 cm) was low because of dry conditions before and during the experiment (crude protein=11.5% of DM; net energy for lactation=5.15 MJ/kg of DM; organic matter digestibility=61.9%). The interaction between PA and supplementation level was significant for PI but not for milk production. Supplementation decreased PI from 11.6 to 7.6 kg of DM/d at low PA and from 13.1 to 7.3 kg of DM/d at high PA. The substitution rate was, therefore, lower at low than at high PA (0.51 vs. 0.75). Pasture intake increased with increasing PA in unsupplemented treatments, and was not affected by PA in supplemented treatments. Milk production averaged 13.5 kg/d and was greater at high than at low PA (+1.4 kg/d) and in supplemented than unsupplemented treatments (+5.2 kg/d). Milk fat concentration averaged 4.39% and was similar between treatments. Milk protein concentration increased from 3.37 to 3.51% from unsupplemented to supplemented treatments, and did not vary according to PA. Grazing behavior parameters were only affected by supplementation. On average, daily grazing time decreased (539 vs. 436 min) and daily ruminating time increased (388 vs. 486 min) from 0 to 8 kg of supplement DM. The PI rate was 6g of DM/min lower in supplemented than in unsupplemented treatments (17 vs. 23 g of DM/min). The high milk yield response to supplementation may be related to a cumulative effect of the low-mass pasture (low PI) and the low quality of the pasture, which strongly limited energy supply in unsupplemented cows.

Abstract:Obstructive Sleep Apnea Syndrome (OSAS) is the most common sleep-related breathing disorder. It is caused by an increased upper airway resistance during sleep, which determines episodes of partial or complete interruption of airflow. The detection and treatment of OSAS is particularly important in stroke patients, because the presence of severe OSAS is associated with higher mortality, worse neurological deficits, worse functional outcome after rehabilitation, and a higher likelihood of uncontrolled hypertension. The gold standard test for diagnosing OSAS is polysomnography (PSG). Unfortunately, performing a PSG in an electrically hostile environment, like a stroke unit, on neurologically impaired patients is a difficult task; also, the number of strokes per day outnumbers the availability of polysomnographs and dedicated healthcare professionals. Thus, a simple and automated recognition system to identify OSAS among acute stroke patients, relying on routinely recorded vital signs, is desirable. The majority of the work done so far focuses on data recorded in ideal conditions and highly selected patients, and thus it is hardly exploitable in real-life settings, where it would be of actual use. In this paper, we propose a convolutional deep learning architecture able to reduce the temporal resolution of raw waveform data, like physiological signals, extracting key features that can be used for further processing. We exploit models based on such an architecture to detect OSAS events in stroke unit recordings obtained from the monitoring of unselected patients. Unlike existing approaches, annotations are performed at one-second granularity, allowing physicians to better interpret the model outcome. Results are considered to be satisfactory by the domain experts. Moreover, based on a widely-used benchmark, we show that the proposed approach outperforms current state-of-the-art solutions.

Abstract:Purpose: As a global health concern, the diagnosis of obstructive sleep apnea hypopnea syndrome (OSAHS), characterized by partial reductions and complete pauses in ventilation, has garnered significant scientific and public attention. With the advancement of digital technology, the utilization of three-dimensional (3D) optical devices demonstrates unparalleled potential in diagnosing OSAHS. This study aimed to review the current literature to assess the accuracy of 3D optical devices in identifying the prevalence and severity of OSAHS. Methods: A systematic literature search was conducted in the Web of Science, Scopus, PubMed/MEDLINE, and Cochrane Library databases for English studies published up to April 2024. Peer-reviewed researches assessing the diagnostic utility of 3D optical devices for OSAHS were included. The Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2) guideline was employed to appraise the risk of bias. Results: The search yielded 3216 results, with 10 articles meeting the inclusion criteria for this study. Selected studies utilized structured light scanners, stereophotogrammetry, and red, green, blue-depth (RGB-D) cameras. Stereophotogrammetry-based 3D optical devices exhibited promising potential in OSAHS prediction. Conclusions: The utilization of 3D optical devices holds considerable promise for OSAHS diagnosis, offering potential improvements in accuracy, cost reduction, and time efficiency. However, further clinical data are essential to assist clinicians in the early detection of OSAHS using 3D optical devices.
