Abstract:Kidney transplantation is the best choice for patients with end-stage renal disease. To date, allograft biopsy remains the gold standard for revealing pathologic changes and predicting long-term outcomes. However, the invasive nature of transplant biopsy greatly limits its application. Ultrasound has been a first-line examination for evaluating kidney allografts for a long time. Advances in ultrasound in recent years, especially the growing number of studies in elastography and contrast-enhanced ultrasonography (CEUS), have shed new light on its application in kidney transplantation. Elastography, including strain elastography and shear wave elastography, is used mainly to assess allograft stiffness and, thus, predict renal fibrosis. CEUS has been used extensively in evaluating blood microperfusion, assessing acute kidney injury and detecting different complications after transplantation. Requiring the use of microbubbles also makes CEUS a novel method of gene transfer and drug delivery, enabling promising targeted diagnosis and therapy. In this review, we summarize the advances of elastography and CEUS in kidney transplantation and evaluate their potential efficiency in becoming a better complement to or even substitute for transplant biopsy in the future.

Abstract:Although surgical techniques, post-transplant care medicine, and immunosuppressants have been greatly improved, permanent acceptance of renal allograft remains a clinical challenge owing to the appearance of various influencing factors. To predict graft dysfunction, development of noninvasive biomarkers is becoming a highlighted research topic in the field of renal transplantation, which provides a possibility for physicians to give preemptive rescue treatment. From the viewpoint of diagnostic techniques, repetitive sampling is prerequisite to identify applicable biomarkers in the clinic. Early biomarkers can be used to dynamically monitor renal graft status and accurately predict transplant outcome independent of various confounders. This review highlights recent studies on the predictive value of biomarkers and methods to quantify biomarkers for monitoring kidney transplant. It is important to analyze and compare different biomarkers for living, and nonliving donors. Analysis of identified clinically relevant biomarkers will advance our understanding of distinct molecular and cellular mechanisms of transplantation and provide insight into developing novel potential approaches to induce transplant tolerance.
Quantitative Analysis of Contrast-Enhanced Ultrasound in Acute Rejection of Kidney Graft

Abstract:Objective To study the value of quantitative analysis of contrast-enhanced ultrasound in diagnosing acute rejection of kidney graft.Methods Sixty-seven patients with normal kidney grafts and thirty-five patients with acute rejection were recruited.In conventional ultrasound,the peak systolic velocity (PSV) and resistance index (RI) of segmental artery and interlobar artery were measured.In quantitative analysis of contrast-enhanced ultrasound,four regions of interest including renal cortex,medulla,segmental artery and interlobar artery were drew and three parameters including rising time(RT),time to peak(TTP) and mean transit time(mTT) were obtained.In addition,the difference in RT,TTP and mTT between the renal cortex and interlobar artery,as well as medulla and interlobar artery were calculated.Results The differences of PSV in interlobar artery between the two groups were statistically significant (P ＜0.05).The time-intensity curves of the whole kidney grafts,and the difference in RT and TTP between the renal cortex and interlobar artery were statistically different between two groups (P ＜0.05).Conclasions Quantitative analysis of contrast-enhanced ultrasound proved a quantitative method for diagnosing kidney allograft acute rejection.

[A new method for standardized diagnosis following renal transplantation. Ultrasound with contrast enhancement]

Abstract:Ultrasound (US) imaging is an important diagnostic tool following renal transplantation. Unfortunately, due to the heterogeneity of the recipients and their multimorbidity, imaging procedures in the early phase after kidney transplantation are difficult and of limited use. We performed a study to evaluate the use of a contrast enhancer for US examination as a standardized method in the follow-up of kidney transplant recipients.The study included 40 recipients: 32 were examined on the 5th to 7th day following transplantation and 8 patients at the time when clinically suspicious findings occurred (acute rejection, tumor, acute tubular necrosis). Following the intravenous application of the contrast medium, pictures were taken during the arterial and parenchymatous phase and compared with conventional B-mode and power Doppler pictures of the same visual plane. Three examiners assessed different parameters of the transplant organ (max. vascularized area, suspected hematomas, tumors, rejection, acute tubular necrosis). Findings were confirmed by histological results of a biopsy if rejection, tumor, or acute tubular necrosis were suspected. Application of ultrasound contrast medium significantly increased visualization of the vascularized kidney area. Also, US findings in the follow-up after acute rejection therapy corresponded with the clinical course; 19 hematomas could be detected with contrast medium compared to only 9 without. With contrast medium a perfusion deficit was detectable in three patients compared to one patient with power Doppler US. Also, US contrast medium helped to detect tumor vascularization in two patients in whom conventional sonography suspected no abnormality. Ultrasound contrast medium enhancement is a reproducible, reliable, and easy to apply method which is superior to conventional sonography in the follow-up after kidney transplantation. This method is also helpful to detect and control acute rejections and to better visualize hematomas, deficits of perfusion, and tumors.
A Novel Simple Noninvasive Index To Predict Renal Transplant Acute Rejection By Contrast-Enhanced Ultrasonography

Abstract:Purpose. This study aimed to evaluate the application of quantitative contrast-enhanced ultrasonography (CEUS) in the assessment of renal allograft dysfunction and to establish a new noninvasive index to predict acute rejection (AR). Methods. Fifty-seven renal transplant recipients were enrolled in this prospective study. Before renal allograft biopsy, CEUS examinations were performed. The biopsy results proved 23 cases of AR (AR group), 10 cases of acute tubular necrosis (ATN group), and 24 with normal evolution (stable group). Contrast-enhanced ultrasonography parameters including rising time (RT), time to peak (TTP), and the delta-time among regions of interest (Delta RT and Delta TTP) were analyzed. Results. In the AR group, RT and TTP of interlobar artery and medulla (RTi, RTm, TTPi, and TTPm) as well was Delta RT and Delta TTP between medulla and cortex (Delta RTm-c and Delta TTPm-c) were significantly higher compared with those in the stable group. RTm and TTPm as well as Delta RTm-c and Delta TTPm-c were remarkably higher compared with those in the ATN group. Delta RTm-c and estimated glomerular filtration rate (eGFR) were identified as independent predictors by multivariate analysis (P = 0.008 and P = 0.024). On the basis of the multivariate analysis results and area under the receiver operating characteristic curves of individual markers, we constructed a new simple index, P = -0.587 + 0.286 x Delta RTm-c - 0.028 x eGFR; new index = e(P)/(1 + e(P)), to discriminate AR, which had better area under the receiver operating characteristic curves than eGFR or individual CEUS parameters. Conclusion. Contrast-enhanced ultrasonography parameters are reliable markers for differentiating the perfusion status of transplanted kidneys. Furthermore, the new simple index can easily predict AR with a high degree of accuracy.

Abstract:Acute rejection is a predominant factor that has adverse influence on the long-term survival of renal allografts. With novel immunosuppressive agents coming into clinical use,manifestations of acute rejection are not typical and refractory rejection is increasing. In this article,we summarized the risk factors,diagnosis,and advances in treatment of acute rejection in 3 aspects:the preoperative screening and management of risk factors for acute rejection; the methods for early diagnosis; and the rational use of immunosuppressants and prophylaxis of infection. Acute rejection management is a key to improve the long-term survival of renal grafts.
Ultrasound Evaluation in Diagnosis of Renal Transplanted Complications

Abstract:Objective To investigate the value of ultrusound in monitoring renal allograft.Methods Ultrasound and colour Doppler flow imaging(CDFI) were performed in 55 renal transplanted patients within 3-30 days after operations.Ultrasound appearance and clinical data of 55 cases were analysed.Results The results of this study were divided into four groups:normal,renal allograft dysfunction,urinary tract obstruction and perirenal hydrops.The diagnoses of ultrasound was confirmed by clinical results.Main renal artery RI in groups of renal allograft dysfunction and perirenal hydrops were higher than that in the normal group(P<0.01,P<0.05) respectively.Conclusions Ultrasound can provide morphologic,internal structural information and CDFI information about renal allograft for clinical,so it is an effective modality for monitoring renal allografts.
Characterization of mobile phone localization errors with OpenCellID data

Abstract:We examine the publicly available OpenCellID database for the purpose of estimating the position of mobile devices along with the positioning accuracy in any given geographic region. We present a method coping with typical distortions and artifacts in the data to compute robust estimates of cell positions and localization error distributions. Applying the method to analyze different regions and mobile networks reveals that the localization errors follow a fat-tailed distribution with similar shapes and properties. We experimentally validate the approach using an independent set of GPS and cell ID data collected by 250 individuals over a period of several weeks. The localization error estimated from OpenCellID data agrees well with the experimental error distribution. While there are significant differences between urban and rural areas, we show that there is only a weak correlation between antenna density and empirical localization errors.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Rationale and Objectives</h3><p>Contrast-enhanced ultrasound (CEUS) has increasingly gained acceptance in the postoperative evaluation of kidney-transplantation recipients. Our meta-analysis aims to evaluate the diagnostic accuracy of CEUS in identifying post-transplantation complications.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Materials and Methods</h3><p>PubMed, Scopus, Ovid Medline, and Cochrane databases were searched from their inception until February 28, 2020, for diagnostic test accuracy studies comparing CEUS to a reference standard for monitoring complications after kidney transplantation. A meta-analysis was conducted to calculate the pooled sensitivity, specificity, accuracy, and diagnostic odds ratio using a bivariate random effects model. Sensitivity analysis was performed using R software by stratifying the studies based on study design, sample size, age, and origin of the study to evaluate the influence of these factors on the overall effect.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Two independent reviewers analyzed 285 publications, out of which 29 were determined directly relevant and 12 (with a total of 542 cases) contained all required data for the meta-analysis. The overall sensitivity of included studies was estimated to be 0.86 (95% confidential interval (CI); 0.78-–0.92). Similarly, the overall specificity was estimated to be 0.90 (95% CI; 0.82–0.94). Log diagnostic odds ratio was 4.25 (95% CI; 3.43–5.07), and the area under the curve of the pooled receiver operating characteristic was 0.94. Stratified sensitivity analyses showed study design, sample size, age group, and origin of the study had no significant impact on the overall diagnostic value of CEUS.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Evidence suggests that CEUS is a potentially effective and accurate method to evaluate a variety of complications such as rejection, vascular complications, and malignancies after kidney transplantation.</p>

Vascular rejection in renal transplant: Diagnostic value of contrast-enhanced ultrasound (CEUS) compared to biopsy

Abstract:Background: Despite of the more potent immunosuppressive medication, vascular rejection is still a major issue after renal transplantation. Renal biopsy is the gold standard diagnostic to evaluate acute and chronic allograft rejection. As it is an invasive diagnostic there is the risk of complications like haematoma, arteriovenous fistulas, active bleeding or infection. Contrast-enhanced ultrasound is a non-invasive imaging modality that allows visualising renal transplant perfusion. Objective: To analyse the sensitivity and specificity of contrast-enhanced ultrasound (CEUS) compared to biopsy as gold standard in diagnosing vascular rejection in renal transplant patients. Methods: A total of 57 renal transplant recipients with poor renal allograft function with initial diagnostic imaging between 2006 and 2017 were included in the study. Clinical data and imaging studies were analysed retrospectively. The diagnostic accuracy of CEUS in diagnosing vascular rejection of the renal transplant was compared to renal biopsy as gold standard. Out of 57 patients 7 patients showed signs of vascular rejection in biopsy. In 6 out of these 7 patients CEUS described irregularities in renal perfusion suspicious of vascular rejection. Results: CEUS showed a sensitivity of 85.7%, a specificity of 100%, a positive predictive value (PPV) of 100%, and a negative predictive value (NPV) of 98.0%. Conclusions: CEUS is a safe, non-nephrotoxic imaging modality for the initial imaging of renal transplant recipients with elevated kidney function parameters suspicious of vascular rejection. Compared to renal biopsy as gold standard CEUS shows a high specificity and PPV in detecting signs of vascular rejection. Since sub-types of vascular rejection with cellular and humoral components with greater risk for allograft loss have been described renal biopsy is inevitable in these cases.
Intensive Surveillance of Porcine-Rhesus Kidney Xenotransplant Using Different Ultrasound Techniques

Abstract:Background: Significant progress has been made in kidney xenotransplantation in the past few years, and this field is accelerating towards clinical translation. Therefore, surveillance of the xenograft with appropriate tools is of great importance. Ultrasonography has been widely used in kidney allotransplantation and served as an economical and non-invasive method to monitor the allograft. However, questions remain whether the ultrasonographic criteria established for human kidney allograft could also be applied in xenotransplantation. Methods: In the current study, we established a porcine-rhesus life sustaining kidney xenotransplantation model. The xenograft underwent intensive surveillance using gray-scale, colorful Doppler ultrasound as well as 2D shear wave elastography. The kidney growth, blood perfusion, and cortical stiffness were measured twice a day. These parameters were compared with the clinical data including urine output, chemistry, and pathological findings. Results: The observation continued for 16 days after transplantation. Decline of urine output and elevated serum creatinine were observed on POD9 and biopsy proven antibody-mediated rejection was seen on the same day. The xenograft underwent substantial growth, with the long axis length increased by 32% and the volume increased by threefold at the end of observation. The resistive index of the xenograft arteries elevated in response to rejection, together with impaired cortical perfusion, while the peak systolic velocity (PSV) was not compromised. The cortical stiffness also increased along with rejection. Conclusion: In summary, the ultrasound findings of kidney xenograft shared similarities with those in allograft but possessed some unique features. A modified criteria needs to be established for further application of ultrasound in kidney xenotransplantation.
Value of quantitative contrast-enhanced ultrasound based on S-G filter theory in the diagnosis of chronic rejection of transplant kidney

Abstract:Objective To explore the characteristics and differences of micro-perfusion between patients after transplant kidney with stable renal function,acute rejection and chronic rejection by contrast enhanced ultrasound (CEUS) quantifying.Methods Thirty-three patients with stable renal function,27 patients with acute rejection and 14 patients with chronic rejection were enrolled.The perfusion parameters of region of interest in renal cortex and medulla were obtained by CEUS offline data quantitative analysis software.The perfusion parameters in stable renal function group were compared with those in acute rejection and chronic rejection groups;furthermore,the value of color Doppler and CEUS in the diagnosis of chronic rejection was explored and compared.Results As for resistance index RI on color Doppler,it made no statistical difference among three groups (P＞0.05).The perfusion parameters derived from CEUS quantitative analysis and Color Doppler parameter resistance index (RI) were not statistically different among renal function stabilization,acute rejection and chronic rejection groups (P＞0.05);as compared with stable renal function group,there were marked reductions in peak intensity of cortex (PIc),peak intensity of medulla (PIm),ascending slope of cortex (a3c),ascending slope of medulla (a3m) and area under the curve of cortex (AUCc) in chronic rejection group.And mean transit time of cortex (MTTc) and mean transit time of medulla (MTTm) became significantly prolonged (P＜ 0.05).Except for a3m,PIc and PIm were not significantly different between stable renal function and acute rejection groups (P＜ 0.05).As compared with acute rejection group,MTTc and MTTm were significantly prolonged in chronic rejection group while AUCm and PIm declined markedly (P＜0.05).When PIm＜26.7dB,the diagnostic efficiency of chronic rejection was the highest and the sensitivity and specificity were 85.7％ and 72.7％ respectively.Conclusions As compared with color Doppler,CEUS quantitative parameters based on S-G filter theory can evaluate the micro-perfusion of transplant kidney more precisely and may provide diagnostic clue for chronic rejection.
Quantitative ultrasound for non-invasive evaluation of subclinical rejection in renal transplantation

Abstract:Objectives: This study aimed to investigate the predictive efficacy of shear-wave elastography, superb microvascular imaging (SMI), and CEUS for allograft rejection in kidney transplants without graft dysfunction. Methods: From January 2021 to November 2021, 72 consecutive patients who underwent both allograft biopsy and ultrasound were evaluated. Blood test results were obtained within a week of the ultrasound examinations, which were performed before the protocol biopsy. Resistive index (RI), tissue viscoelasticity, vascular index, and quantitative CEUS parameters were measured. Patients were divided based on biopsy results into the rejection and non-rejection groups. Results: Among the 72 patients, 21 patients had pathological characteristics of acute rejection. RI of allograft was significantly higher in the rejection group (p = 0.007), compared to the non-rejection group. There were no significant between-group differences in vascular indices of SMI, mean elasticity, and mean viscosity. Meanwhile, among the parameters obtained by the time-intensity curve on CEUS, the cortical and medullary ratios of average contrast signal intensity, peak enhancement, wash-in area AUC, wash-in perfusion index, wash-out AUC, and wash-in and wash-out AUC were significantly different between the two groups (p < 0.05). In the receiver operating characteristic curve analysis for predicting allograft rejection, the AUC was 0.853 for the combination of six CEUS parameters, RI, and blood urea nitrogen. Conclusions: Among non-invasive quantitative ultrasound measurements, CEUS parameters are the most useful for diagnosing subclinical allograft rejection. Furthermore, the combination of CEUS parameters, RI, and blood urea nitrogen may be helpful for the early detection of renal allograft rejection. Key points: • Among non-invasive quantitative ultrasound measurements, CEUS parameters are the most useful for the diagnosis of subclinical allograft rejection. • On CEUS, the C/M ratios of MeanLin, PE, WiAUC, WiPI, WoAUC, and WiWoAUC are significantly lower in the rejection group; the combination of these showed reliable predictive performance for rejection. • The combination of CEUS parameters, RI, and BUN has a high predictive capability for subclinical allograft rejection.
Evaluation of kidney allograft status using novel ultrasonic technologies

Abstract:Early diagnosis of kidney allograft injury contributes to proper decisions regarding treatment strategy and promotes the long-term survival of both the recipients and the allografts. Although biopsy remains the gold standard, non-invasive methods of kidney allograft evaluation are required for clinical practice. Recently, novel ultrasonic technologies have been applied in the evaluation and diagnosis of kidney allograft status, including tissue elasticity quantification using acoustic radiation force impulse (ARFI) and contrast-enhanced ultrasonography (CEUS). In this review, we discuss current opinions on the application of ARFI and CEUS for evaluating kidney allograft function and their possible influencing factors, advantages and limitations. We also compare these two technologies with other non-invasive diagnostic methods, including nuclear medicine and radiology. While the role of novel non-invasive ultrasonic technologies in the assessment of kidney allografts requires further investigation, the use of such technologies remains highly promising.
Preliminary Investigation of the Biomarkers of Acute Renal Transplant Rejection Using Integrated Proteomics Studies, Gene Expression Omnibus Datasets, and RNA Sequencing.

Abstract:A kidney transplant is often the best treatment for end-stage renal disease. Although immunosuppressive therapy sharply reduces the occurrence of acute allograft rejection (AR), it remains the main cause of allograft dysfunction. We aimed to identify effective biomarkers for AR instead of invasive kidney transplant biopsy. We integrated the results of several proteomics studies related to AR and utilized public data sources. Gene ontology (GO) and pathway analyses were used to identify important biological processes and pathways. The performance of the identified proteins was validated using several public gene expression omnibus (GEO) datasets. Samples that performed well were selected for further validation through RNA sequencing of peripheral blood mononuclear cells of patients with AR (n = 16) and non-rejection (n = 19) from our medical center. A total of 25 differentially expressed proteins (DEPs) overlapped in proteomic studies of urine and blood samples. GO analysis showed that the DEPs were mainly involved in the immune system and blood coagulation. Pathway analysis showed that the complement and coagulation cascade pathways were well enriched. We found that immunoglobulin heavy constant alpha 1 (IGHA1) and immunoglobulin κ constant (IGKC) showed good performance in distinguishing AR from non-rejection groups validated with several GEO datasets. Through RNA sequencing, the combination of IGHA1, IGKC, glomerular filtration rate, and donor age showed good performance in the diagnosis of AR with ROC AUC 91.4% (95% CI: 82–100%). Our findings may contribute to the discovery of potential biomarkers for AR monitoring.
Ultrasound super-resolution imaging for the assessment of renal allograft dysfunction: A pilot study

Abstract:Background: The purpose of this study was to examine the feasibility and practical application of ultrasound (US) super-resolution imaging (SRI) in evaluating microvasculature and measuring renal allograft function. Methods: Sixteen consecutive patients who received kidney transplants were prospectively enrolled. The patients were assigned as: normal allograft function (n = 6), and allograft malfunction (n = 10). Localizing each potential contrast signal resulted in super-resolution images (SRI). SRI was utilized to assess micro-vessel density (MVD) and microvascular flow rate, whereas contrast-enhanced (CE) US images were statistically processed to get the time to peak (TTP) and peak intensity. Logistic regression was utilized to evaluate their relationship. Results: US SRI may be utilized effectively on allografts to show microvasculature with significantly higher resolution than typical color Doppler flow and CEUS pictures. In the multivariate analysis, MVD and TTP were significant US markers of renal allograft failure (p = 0.031 and p = 0.045). The combination of MVD and TTP produced an AUC of 0.783 (p < 0.05) for allograft dysfunction. Conclusions: SRI can accurately portray the microvasculature of renal allografts, while MVD and TTP are appropriate US markers for assessing renal allograft failure.
Acute Renal Allograft Rejection is Associated with Increased Levels of Vascular Endothelial Growth Factor in the Urine

Abstract:Aim: The purpose of this study was to assess whether measurement of urinary vascular endothelial growth factor (VEGF) could be adopted as a new non-invasive diagnostic tool for acute rejection following renal transplantation. Methods: Urinary concentration of VEGF was determined by an enzyme-linked immunosorbent assay technique in 215 renal allograft recipients and 80 healthy controls. Results: Subjects with acute rejection (n=67) excreted urinary VEGF at a significantly higher level (28.57 +/- 6.21, 95% CI: 16.18-40.97 pg/mu mol creatinine) than those without acute rejection. This included subjects with stable renal function and no abnormal histological findings (n = 119), acute tubular necrosis (n = 15), chronic allograft nephropathy (n = 14) and healthy controls (n = 80). Using a urinary VEGF/creatinine ratio of 3.64 pg/mu mol as the cut-off point, the sensitivity and specificity for diagnosing acute rejection were 85.1 and 74.8%, respectively (P < 0.001). Patients with steroid-resistant acute rejection had significantly greater urinary VEGF concentration than patients with steroid-sensitive acute rejection (42.09 +/- 10.00 vs 9.74 +/- 2.63 pg/mu mol creatinine, P < 0.001). Patients with graft loss after acute rejection had significantly greater urinary VEGF concentration than patients with reversible acute rejection (106.66 +/- 38.60 vs 19.46 +/- 4.13 pg/mu mol creatinine, P = 0.001). Using a urinary VEGF/creatinine ratio of 22.48 pg/mu mol as the cut-off point, the sensitivity and specificity of the prediction to graft loss after acute rejection were 85.7% and 78.3%, respectively (P = 0.001).Conclusion: This study demonstrates that the monitoring of urinary VEGF may be a useful non-invasive approach for the detection of acute rejection. Additionally, urinary VEGF levels were shown to predict the response to anti-rejection therapy and to predict a poor outcome after acute rejection.

Abstract:Post-transplant lymphoproliferative disorder (PTLD) is a rare but severe complication of solid organ transplantation, which is frequently related to the infection of Epstein-Barr virus (EBV) and cytomegalovirus (CMV). [ 1 ] Only 1% of kidney transplant recipients will develop PTLD, which is substantially lower than other organ recipients. [ 2 , 3 ] Early detection and diagnosis of PTLD in transplanted kidneys are crucial for prognosis, enabling patients to receive treatment as soon as possible. [ 4 ] As the first-line imaging method for routine follow-up after kidney transplantation, ultrasound (US) has the advantages of convenience, non-invasiveness, and non-radiation, which has become an essential tool for discovering and differentiating substantive causes in transplanted kidneys. [ 5 ] With the development of US technology, the application of contrast-enhanced ultrasound (CEUS) in kidney transplantation is increasingly mature and extensive, which can display real-time microcirculation blood perfusion in transplanted kidney tissue with unique advantages such as good repeatability and no nephrotoxicity. CEUS provides a good prospect for early detection and diagnosis of postoperative complications and improvement of patient survival rate after kidney transplantation. [ 6 , 7 ]

Abstract:Renal transplant is the first-line treatment of end-stage renal disease. The increasing number of transplants performed every year has led to a larger population of transplant patients. Complications may arise during the perioperative and postoperative periods, and imaging plays a key role in this scenario. Contrast-enhanced US (CEUS) is a safe tool that adds additional value to US. Contrast agents are usually administered intravenously, but urinary tract anatomy and complications such as stenosis or leak can be studied using intracavitary administration of contrast agents. Assessment of the graft and iliac vessels with CEUS is particularly helpful in identifying vascular and parenchymal complications, such as arterial or venous thrombosis and stenosis, acute tubular injury, or cortical necrosis, which can lead to graft loss. Furthermore, infectious and malignant graft involvement can be accurately studied with CEUS, which can help in detection of renal abscesses and in the differentiation between benign and malignant disease. CEUS is also useful in interventional procedures, helping to guide percutaneous aspiration of collections with better delimitation of the graft boundaries and to guide renal graft biopsies by avoiding avascular areas. Potential postprocedural vascular complications, such as pseudoaneurysm, arteriovenous fistula, or active bleeding, are identified with CEUS. In addition, newer quantification tools such as CEUS perfusion are promising, but further studies are needed to approve its use for clinical purposes. ©RSNA, 2024 Supplemental material is available for this article.
