Abstract:Background: The presence of philtrum in the middle area of the upper lip is a pre-eminent feature of facial aesthetics. However, the philtrum is invariably damaged in cleft lips, and the damage persists even after primary surgery. We attempted to use de-epidermized scar tissue flaps, to raise the philtral ridge and dimple, in secondary unilateral cleft lips. Methods: A prospective study was planned to determine the effectiveness of this novel procedure in 11 patients with secondary unilateral cleft lip deformities, from 2011 to 2013. Pre-and post-operative photographs were taken and postoperative evaluation was performed between 6 and 23 months. Eminence of the philtral ridge was scored on a 5 point grading scale; the dimple was scored on a 3 point grading scale as well as the scars. Scores were recorded both preoperatively and postoperatively. Results: The results demonstrated a significant improvement in both, the philtral ridge and dimple, as indicated by the preoperative and postoperative scores (2.27 vs. 3.91, 1.36 vs. 2.03, respectively, all P<0.05). Five patients had a significant improvement in the appearance of scar, while appearance of the scar was acceptable to the remaining patients. Conclusions: The use of deepidermized scar tissue is an effective and simple method to construct the philtrum, in cases of secondary cleft lip.
Computed Tomography-Assisted Auricular Cartilage Graft for Depression of the Alar Base in Secondary Unilateral Cleft Lip Repair: A Preliminary Report

Abstract:Objectives: To evaluate the feasibility, effectiveness, and safety of computed tomography-assisted auricular cartilage grafting for treating alar base depression secondary to unilateral cleft lip. Design and Setting: For patients with obvious depression of the alar base, the difference in heights of the alar base and the piriform margin between the cleft side and the noncleft side were measured with computed tomography. If both were >3.0 mm, the cartilage was harvested postauricularly and subdivided into 2 to 4 pieces. A multiple layer graft was inserted under the depressed alar base. The procedure was performed from 2006 to 2013, and the follow-up period was 3 to 15 months. Participants: Chinese patients with alar base depression secondary to unilateral cleft lip were selected. Intervention: Suture and cartilage graft techniques. Main Outcomes Measures: Differences in bilateral alar base heights and piriform apertures. Results: There was no wound dehiscence, exposure of bone, or donor site morbidity. The difference in heights in the bilateral alar bases and piriform apertures decreased. There were no obvious scars in any of the cases. Conclusions: This technique has several advantages including ease of operation, minimal trauma, satisfactory outcomes, and useful references for operation provided by computed tomography. It is a superior alternative for reconstruction of secondary alar depression.
Correction of secondary nasal deformities in unilateral cleft lip:Report of 38 consecutive cases

Abstract:PURPOSE:This study was designed to find the effective and reliable methods to correct secondary nasal deformities in unilateral cleft lip by review of clinical data of 38 patients.METHODS:38 patients(21 males,17 females, ranged from 17 to 34 years in age with mean age of 20.8 years) with nasal deformities in unilateral cleft lip were available for this study.The methods used for correction included :suspension or filling of the alar cartilages to correct the collapse deformities,correction of the severe septal deviations to improve the narial figure and airway obstruction,augmentation of the alar base to obtain bilateral symmetry with biomaterial or soft tissue,anatomic reposition of nasal-labial muscles which keep the balance of bilateral tension of muscles,etc.After operations,all patients had plastic stopple for two or three months.RESULTS:The nasal deformities of 38 patients were improved more or less after operations.27 cases (71.05%) were followed up for 3 months to 1 year and the results showed deformities in 5 cases trended to relapse,11 cases undergoing biomaterial implants had satisfied results without complications.CONCLUSIONS:The cause of secondary unilateral cleft lip nasal deformities was complex.The study indicated a significant improvement by synthetically correction of deformities of bone,cartilage and soft tissues,and the restoration of the nasal-labial muscles was more important to avoid relapse.Supported by Shanghai Leading Academic Discipline Project(Grant No.Y0203).
Correction of tear trough deformity with a cohesive polydensified matrix hyaluronic acid: a case series

Abstract:The tear trough or infraorbital hollow is a challenging area to treat, and only a few fillers are suitable for this delicate area. We report on a European case series of six subjects with mild to severe tear troughs who received treatment with cohesive polydensified matrix (CPM(®)) technology hyaluronic acid gel (Belotero(®) Balance). The product was injected as small depots (up to ten small boli 0.2 mL maximum each per side) at the supraperiosteal level along or below the orbital rim. Follow-up visits took place at 1, 3, 6, and 9 months after injection for independent evaluation of the clinical effect using the Merz Aesthetics Scale™ for infraorbital hollows and the Global Aesthetic Improvement Scale. Adverse events were also recorded. Mean hollowness scores were considerably improved compared with baseline in all subjects. In all women, the improvements remained throughout the 9-month study, with none reverting to their baseline score. Subjects' satisfaction with treatment was very high throughout the study, and all women stated that they would repeat treatment with the same product. The CPM hyaluronic acid gel was well tolerated. CPM hyaluronic acid gel is a safe and effective treatment for the tear trough area.

Abstract:Since the Resolution 198/2019 of Brazilian Dental Council, which regulates orofacial harmonization as a dental specialty, and the advent of various uses of facial fillers, such as hyaluronic acid (HA), it is possible to perform both esthetic and functional corrections in individuals. Individuals with cleft lip and palate (CLP) present lip irregularities even after orofacial rehabilitation with an interdisciplinary team with several corrective surgeries, interfering with the esthetics, which can cause problems in self-esteem and social insertion. Thus, facial filling is an innovation that, together with dentistry, contributes to the individual's esthetics and well-being. Considering the patient safety and health, more research is progressively being conducted to make such procedures less invasive. This work conducted a literature review on the use of HA as a facial filler to correct lip scars in patients with CLP. By a literature and transverse search in Scientific Electronic Library Online and PubMed databases using specific descriptors, the studies that met the inclusion criteria were selected, from 1990 to 2020. It can be concluded that the use of HA as a facial filling material in the correction of lip scars from reparative surgeries related to CLP has been shown to be effective both for correction of facial asymmetry and to improve the quality of life of patients who used the procedure.
Simultaneous Correction of Complex Secondary Deformities of Cleft Lip Using Autogenous Costal Cartilage Combined with Rib Grafts.

Abstract:OBJECTIVE:Complex secondary deformities of cleft lip include nasal and labial deformities, alveolar cleft, oronasal fistula, and so on. In this report, the authors present a method to simultaneously repair the above complex deformities with autogenous costal cartilage and rib grafts.METHODS:The surgical procedure of simultaneous correction includes RESULTS:: Satisfactory outcomes were achieved in all 22 patients. Fifteen patients were followed up for 6 to 24 months and exhibited good nasal shape, repaired alveolar cleft, improved labial shape, and closed oronasal fistula. Three patients presented with red nasal tips. One patient had local infection, which was cured by antibiotics.CONCLUSION:This methodAs this method has led to positive outcomes, it has the potential to be widely used in clinical practice.
Clinical Efficacy of Autologous Fat Grafting in Secondary Cleft Lipoperations

Abstract:Objective To evaluate the therapeutic effect of autologous fat grafting in secondary cleft lip.Methods Grafting of autologous fat granules was applied to correct lip under-capacity.The technique was used in 40 patients with secondary cleft lip from June 2011 to December 2015.The ratios between the point of upper lip peak to vermilion height and to white lip height on the affected side were measured by Photoshop CS6.Results Satisfactory results were achieved with all 40 patients;the lip contour and morphology improved.The preoperative ratio was 0.417±0.190 and the postoperative ratio was 0.499±0.197, which increased average 26.02%, with statistically significant (P<0.05).Conclusions Autologous fat grafting to correct lip under-capacity with secondary cleft lip is a safe, effective and micro-invasive method which can be sustained over a lengthy period.

Abstract:BACKGROUND: Barraquer-Simons syndrome (BSS) or acquired partial lipodystrophy is a disorder of loss of subcutaneous fat from the upper part of the body. The resulting cosmetic disfigurement causes significant psychological distress in the affected individuals. Fillers, autologous fat transplant or cosmetic surgeries can be used to correct the volume deficit.METHODS: The authors report a case of a young woman, who presented with gradual loss of subcutaneous fat from the face, upper limbs, and trunk. On evaluation, she had low complement C3 levels, but there were no features to suggest any metabolic complication. She was diagnosed with BSS. Her volume deficit in the midface area was managed with hyaluronic acid (HA) filler.RESULTS: The treatment has resulted in the correction of her facial volume loss immediately following the procedure with a maintained effect at follow-up visits. She experienced no major adverse events.CONCLUSION: Hyaluronic acid filler is safe and effective for the management of volume loss in BSS. It gives a long lasting result and an added self-confidence to the patient.

Abstract:Objective To investigate the result of the correction of secondary deformities in the unilateral cleft lip patients with autologous septal cartilage.Methods A total of 41 unilateral cleft lip patients who underwent rhinoplasty were included in this study,including 15 cleft lip and 26 cleft lip and palate patients.In the rhinoplasty,the autologous septal cartilage was harvested and trimmed.Then one long piece of septal cartilage graft was used as columellar strut,and others as onlay grafts on the tip,dorsal side of the nose,and the lower and/or upper lateral cartilage of the cleft side.The results were assessed by X-ray.Results The appearance of the nose apparantly improved after the rhinoplasty.The deviation of the cartilaginous septum was corrected in 31 patients.Ten cases with severe bony septum deviation had no improvement.The subjective evaluation scores improved an average of 2.80 after the rhinoplasty.Conclusions With the rhinoplasty presented,we can reconstruct the cartilaginous bracket of the unilateral cleft lip nose with autologous septal cartilage and improve the appearance.The cartilaginous deviation also can be corrected.
Microcannula Injection of Large Gel Particle Hyaluronic Acid for Cheek Augmentation and the Correction of Age-Related Midface Contour Deficiencies

Abstract:BACKGROUND: There is a growing trend toward the use of blunt-tip microcannulas for the treatment of midface volume loss with injectable hyaluronic acid.OBJECTIVE: To evaluate the safety and efficacy of large gel particle hyaluronic acid with lidocaine, injected with a blunt-tip microcannula, for cheek augmentation and the correction of age-related midface contour deficiency.MATERIALS AND METHODS: Sixty subjects with mild to substantial loss of midface fullness were treated to optimal correction. Adverse events (AEs) were obtained during each examination visit. Efficacy was evaluated using the Global Aesthetic Improvement Scale, the Medicis Midface Volume Scale (MMVS), and the FACE-Q subject satisfaction questionnaire through 16 weeks.RESULTS: Five AEs were reported, with only one being considered injection procedure-related (mild presyncope). No serious AEs were reported. Global aesthetic improvements were reported for the majority of subjects (≥98.3% for investigator-assessed and ≥91.5% for subject-assessed) for up to 16 weeks. At 8 weeks, the MMVS responder rate was 100% and subject satisfaction scores (5/6 questions) in the FACE-Q were ≥91.5%.CONCLUSION: Hyaluronic acid with lidocaine, injected with a blunt-tip microcannula, has a favorable safety profile and is effective up to 16 weeks for cheek augmentation and the correction of age-related midface contour deficiencies (NCT03160716).

Abstract:Bilateral cleft lip often has nasal deformities of various degrees. It is not enough to elongate the nasal columella for such deformities. Based on the thoughts that secondary deformities of the nose and lip should be treated as a whole, the authors introduced a new technique, in which five aspects of the deformities were considered and treated at the same time. Thirteen patients have undergone surgical correction with satisfactory results.

Abstract:BACKGROUND: Non-animal-stabilized hyaluronic acid (NASHA) gel may offer longer-lasting cosmetic correction and lower antigenic risk than other soft tissue augmentation agents.OBJECTIVE: To assess the efficacy and safety of the NASHA gel Restylane SubQ (Q-Med AB, Uppsala, Sweden) in aesthetic facial contouring.METHODS: Fifty-seven adult patients seeking cheek and/or chin augmentation received subcutaneous and/or supraperiostal injections of Restylane SubQ (20 mg/mL) at 114 treatment sites; 13 of these patients received "touch-up" injections at 20 sites. Efficacy was assessed subjectively using a 5-grade Global Aesthetic Improvement Scale at 1, 3, 6, 9, and 12 months after the initial treatment.RESULTS: At 3 months post-baseline, patients and investigators independently considered the treatment sites to be improved in 96.4% and 100% of cases, respectively. Patient- and investigator-assessed response rates (proportion of patients showing moderate or better improvement) were 84% and 95%, respectively. The majority of reported adverse events were treatment related (local injection-site reactions, implantation complications, skin tightness, and skin induration), but these were generally of mild intensity and short-lived.CONCLUSION: Restylane SubQ is well tolerated and maintains aesthetic correction of the cheeks and chin for at least 3 months after subcutaneous and/or supraperiostal treatment.

Abstract:Objective Secondary deformities are common in people born with unilateral cleft lip and palate.In recent years,more and more attempts and efforts have been directed toward the restoration of normal muscle anatomy.The authors present a new method of orbicularis oris repair in secondary cleft lip repair.Methods From July 2009 to December 2011,24 patients underwent this procedure in the authors' department.Muscle reconstruction was divided into three units that dealed with the nasal base,white lip and red lip.Common anatomic pathologies including the displacement of upper lip,flared ala,blunted ala-facial groove,depressed nasal floor,lateral bulge,lack of philtrum column,free border deficiency and unapparent vermilion tubercle were corrected in a single operation.Results The average follow-up period was 16.79±6.23 months.No major complications occurred.All the patients were satisfied with their nasolabial appearance.Conclusions "Three-unit" muscle repair has found to be effective and practical in secondary repair.Favorable results demonstrate that improved aesthetic and functional results can be achieved with this comprehensive procedure.
Short-term qualitative and quantitative analyses of preseptal injection of hyaluronic acid on the treatment of acquired lower eyelid cicatricial ectropion

Abstract:Purpose: Recently, hyaluronic acid (HA) was proposed as a promising option for the treatment of acquired lower eyelid cicatricial ectropion. However, this effect was not confirmed by quantitative assessments. This study aimed to assess the effect of hyaluronic acid on the treatment of acquired lower eyelid cicatricial ectropion. Methods: Eight patients with acquired lower eyelid cicatricial ectropion (13 eyelids) were treated with a single 1 mL injection of hyaluronic acid in the preseptal area of the lower eyelid. Evaluation of symptoms and biomicroscopic exam was performed before and 30 days after hyaluronic acid injection. Quantitative analysis of the lower eyelid position (with and without lid traction) was determined before and 30 days after hyaluronic acid injection through standard photographs analyzed using the ImageJ. Results: All patients experienced partial improvement of symptoms. The lower eyelid position was significantly lifted after hyaluronic acid injection with a significant reduction of medial and lateral angles, reduction of the margin reflex distance, and total and medial ocular fissure area. However, signs of lid margin inflammation and corneal punctate keratitis persisted. Conclusions: Hyaluronic acid injected in the pre-septal area of the lower eyelid improved acquired lower eyelid cicatricial ectropion symptoms and significantly lifted the position of the lower eyelid. Further studies, with a large number of participants and a long-term follow-up period, are needed to better determine the permanency of the effects of hyaluronic acid injections on the treatment of acquired lower eyelid cicatricial ectropion.
Ultrasonography in the management of lip complications caused by hyaluronic acid

Abstract:Ultrasonography is highly accurate for evaluating soft tissues. Given that minimally invasive aesthetic procedures are on the rise, complications have become more prevalent. Thus, ultrasonography holds promise for assisting in the diagnosis and management of complications arising from these interventions. This report highlights the importance of ultrasonography in the treatment of complications caused by hyaluronic acid injection. A patient visited a dental office 24 hours after hyaluronic acid application, presenting pain and bruising in the middle and inferior thirds of the face on the right side. To evaluate blood vessels, the surgeon used Doppler-mode ultrasonography, which enabled the precise application of hyaluronidase to reestablish blood perfusion and preserve adjacent structures. Therefore, to avoid severe outcomes, such as necrosis or even amaurosis, the use of ultrasonography is suggested, improving the precision and safety of these procedures.
Repair of Scars and Tissue Deficiency With Free Grafts of Full-thickness Scalp in Secondary Unilateral Cleft Lip Deformity

Abstract:Although surgical techniques have developed, the incidence of secondary cleft lip deformities after the primary repair is still high. Asymmetry of Cupid's bow and philtrum is a common presentation and a technical challenge to reconstruct the upper lip. The authors introduce a technique to use the free grafts of the full-thickness scalp for the correction of scars and tissue deficiency in secondary unilateral cleft lip deformity. Thirty-seven patients with a prominent scar, tight upper lip, deformed Cupid's bow and philtrum, and irregular vermillion border were included in this study. The specific points of interest were assessed before and after surgery by independent examiners with both subjective and objective methods. All surgeries were successfully completed, and the assessment results were satisfactory. The rating scores of philtrum improvement was 1.6 & PLUSMN;0.4, the POSAS result of patient scales was 13.48 & PLUSMN;3.21, and the observer scale result was 11.98 & PLUSMN;3.88. The asymmetry of Cupid's bow was corrected (P=0.004), the central tubercle of vermilion was more natural (P=0.001), and the irregular vermillion border was improved (P=0.015). The results presented significant differences before and after surgery. This method could be an optional treatment for repairing scars and tissue deficiency in secondary unilateral cleft lip deformity.
Autologous Fat Transplantation to Improve Lip Contour in Secondary Cleft Lip Deformity.

Abstract:The insufficient volume of lip is one of the pathological problems of cleft lip, despite that the cleft lip the muscles and skin are repositioned. Autologous fat is an ideal material for soft tissue repair, including correction of contour deformity, restoration of volume insufficient, and even improvement of tissue characteristics. In this study, the authors evaluated the result of fat grafting in improvement of lip contour in secondary cleft lip deformity. The authors performed a retrospective analysis of their series of patients who underwent lip fat grafting. A total of 65 subjects with lip fat grafting for cleft lip revision were recruited for the study. The photographs documented the original malformation and the posttreatment photographs. The authors recruited female and male raters, who were experts in the field or lay persons to form a panel of 12 raters altogether. Symmetry and aesthetics were improved after fat grafting based on vermillion border (P=0.02), symmetry of lip (P=0.007), and nasal profile including upper lip (P=0.04) using a 1- to 5-point scale. After treatment, both sides of the lip are more symmetrical, the thickness of the filling part is significantly increased, the filling area is soft, the appearance is satisfied. Fat grafting is an efficient secondary procedure to improve lip contour after cleft lip repairs.

Abstract:OBJECTIVE:To investigate the correction of secondary nasal deformity of cleft lip with autogenous costal cartilage framework.METHODS:237 cases with secondary nasal deformity of unilateral cleft lip were treated. The rib cartilage was harvested through a mini-invasive incision, and was fabricated as a C-shaped framework, as well as some cartilage fragments. Through transcolumella incision, the C-shaped framework was implanted to support the depressed alar and the cartilage fragments were used to augment the nasal base.RESULTS:Satisfactory cosmetic and functional results were achieved in all the patients with primary healing. 93 patients were followed up one year after operation with good cosmetic results.CONCLUSIONS:Autogenous costal cartilage framework can be used for the correction of secondary nasal deformity of cleft lip with satisfactory results.

Abstract:BACKGROUND Hyaluronic acid (HA) fillers are the preferred injectable products for aesthetic correction of skin depressions and restoration of facial volume. OBJECTIVE To investigate the subcutaneous distribution of 3, biophysically distinct, CE-marked and FDA-approved HA fillers. MATERIALS AND METHODS BELB, JUVV, and RESL were injected ex vivo in porcine and human skin. Immediately after injection, the skin samples were snap-frozen, cross-sectioned, and visualized using stereomicroscopy and full-field optical coherence tomography. Images were compared with histological sections after hematoxylin and eosin staining. RESULTS Hyaluronic acid fillers were distributed as homogeneous bolus in the ex vivo skin. The injection bulks were found to preserve the fibrous trabecular network, shift the fat lobules, and displace the adjacent adipocyte layers independently of the formulation injected. CONCLUSION For the first time, the subcutaneous injection of 3 HA fillers with markedly different biophysical properties was systematically investigated by complementary visualization techniques. Despite their different properties, no difference in distribution was found after subcutaneous injection. The global preservation of the hypodermis structure observed was consistent with the good tolerability seen in clinical practice after implantation of the HA fillers in the subcutaneous skin layer.

[Management of Secondary Nasolabial Deformities after Bilateral Cleft Lip Repair].

Abstract:OBJECTIVE:To evaluate the method to correct the seconary nasolabial deformities after the surgical treatment in the patients with bilateral cleft lip.METHODS:From January 2000 to June 2003, forty patients with secondary deformities following repair of bilateral cleft lip were treated with a combined treatment procedures. AU of the forty cases underwent the following preoperative treatments: alveolar bone graft in 28 cases, preoperative orthodontics in 22 cases, prosthodontics in 20 cases and orthognathic surgery or distraction osteogenesis treatment in 20 cases, respectively. In order to improve the enlongation of nasal column, reconstruction of Cupid's bow, philtrum and correction procedures, continuous incision was made from the vermilion in median of the upper lip, the scar edge, the bilateral sides of the nasal column to the inner side of the nose, even extending the bilateral incision to nasolabial groove and nostril fundus.RESULTS:Forty patients were got the follow-ups for 3 months to 3.5 years and the satisfactory rate reached 95%.CONCLUSION:It is natural to emphasize the setting up of odontomaxillary frame and then utilize the surgical procedure to correct the secondary nasolabial parenchyma deformities. The method could be feasible and reliable for the correction of the secondary nasolabial parenchyma deformities after bilateral cleft lip repair.
