Abstract:Introduction: Linear IgA bullous dermatosis (LABD) is a rare autoimmune disease. Although dapsone is the initial treatment, other immunomodulators are used in resistant cases or when dapsone is unavailable. Case report: A 12-year-old Mexican child, with no relevant medical history, developed in May 2023 a disseminated dermatosis affecting all body segments, including mucous membranes, characterized by erythematous patches and plaques evolving into the formation of serous and serosanguinous blisters and vesicles, distributed in a "string of pearls" pattern. LABD was suspected and confirmed by skin biopsy, which showed a subepidermal blister with neutrophilic infiltration and linear Immunoglobulin A deposits at the dermo-epidermal junction by direct immunofluorescence. Treatment with prednisone (2 mg/kg/day) and cyclosporine (5 mg/kg/day) resulted in improvement and lesion remission within 2 weeks. Both drugs needed to be discontinued for 3 months due to intermittent blistering. Cyclosporine was continued as maintenance therapy at a dose of 4 mg/kg/day for 8 months. Conclusions: The report highlights the use of cyclosporine as an alternative immunomodulator for DAAL, an immunosuppressive agent used in autoimmune disorders. Few cases, including this one, have described complete remission and control of the dermatosis with cyclosporine, accompanied by prednisone at the start of treatment.
Linear Atrophoderma of Moulin: A Case Report

Abstract:Linear Atrophoderma of Moulin is a rare skin condition that is characterized by the development of one or more atrophic patches or depressions in the skin. These patches are usually located on the trunk, but they can also occur on the arms, legs, and neck. We here present a case of 33-year Nepalese male with brown to black color lesions over the left upper back, abdomen and thigh for the last 7 years. Clinical and dermatopathological findings were similar to the Linear Atrophoderma of Moulin. To our knowledge, this is the first case of LAM from Nepal. This case emphasizes the necessity of diagnosing Atrophoderma of Moulin and separating it from linear scleroderma due to differences in therapy and prognosis.
A case of linear scleroderma "en coup de sabre" with strabismus fixus in a child

Abstract:Scleroderma, "en coup de sabre" is a rare disorder, characterized by linear depressed scarring at frontoparietal area of the face, seldom associated with ophthalmological findings. Extraocular muscle involvement and paralytic strabismus are rarely associated with linear scleroderma. Here we are reporting a case of unilateral linear scleroderma in an 8-year-old child, with features of strabismus fixus secondary to fibrosed medial rectus (MR) muscle, enophthalmos and en coup de saber (sword-like scar) on the eyebrow. MRI orbit was suggestive of the bulky MR and the inferior rectus muscle. MR recession along with partial loop myopexy resulted in cosmetically acceptable ocular alignment in this case. Histopathology of the portion of muscle and intermuscular septum showed diffuse fibrosis of the muscle. Linear scleroderma may be the causative etiology of strabismus fixus in our case, never reported before in literature.

Abstract:Nicolau syndrome (embolia cutis medicamentosa) is a condition where we see variable degrees of tissue necrosis including the skin and deeper tissues, due to an iatrogenic cause, intramuscular, subcutaneous, intra-articular injections, could be some of them. It occurs due to intravascular inoculation leading to crystal embolization causing thrombotic occlusion, vasospasm, and marked inflammation and surrounding necrosis. Here, we present a case of a 35-year-old gentleman who presented to us with bluish-purple discoloration of the skin followed by a necrotic patch and ulceration with multiple hyperpigmented spots over the back and lower limb post intramuscular diclofenac sodium injection. Routine investigation showed neutrophilia and thrombocytosis with increased creatinine. A diagnosis of Nicolau's syndrome (embolia cutis medicamentosa) was made clinically, which was confirmed by histopathology following biopsy. Adequate surgical debridement and a combination therapy of analgesics, intravenous antibiotics, intravenous anticoagulants, and vasoactive therapy were administered. Split thickness skin grafting was done once the wound was healthy with rich granulation tissue, after 4 weeks of index surgery in this case. Patient recovered uneventfully.

Linear Morphea Distributed along Radial Nerve with High Incidence of Neuromusculoskeletal Disorders: A Single-Center Case Series

Abstract:Linear morphea is the most disabling subtype of morphea, which may cause a series of excutaneous manifestations and sequelae. To futher explore the clinical characteristics of linear morphea, we conducted a retrospective study of 22 patients diagnosed with linear morphea in our department during the past 2 years. Their baseline clinical information, skin manifestations, complications and therapeutic effect were analyzed. Here, we report six cases of a special linear morphea, usually occurring on the unilateral upper limbs of young women, spreading along the distribution of the radial nerve and frequently progressing across the joint, which increases the incidence of neuromusculoskeletal disorders. Instead of traditional topical drugs, a combination of systemic prednisone and methotrexate improved their skin lesions and complications. Recognition of this special type of linear morphea enables earlier diagnosis and active treatment plan, which contributes to ameliorate the symptoms and avoid functional sequelae.

Abstract:Topical steroids are known for their anti-inflammatory properties and are commonly prescribed to treat many adverse skin conditions such as eczema and psoriasis. While these treatments are known to be effective, adverse effects including skin atrophy are common. In this study, the progression of these effects is investigated in an in vivo mouse model using multimodal optical microscopy. Utilizing a system capable of performing two-photon excitation fluorescence microscopy (TPEF) of reduced nicotinamide adenine dinucleotide (NADH) to visualize the epidermal cell layers and second harmonic generation (SHG) microscopy to identify collagen in the dermis, these processes can be studied at the cellular level. Fluorescence lifetime imaging microscopy (FLIM) is also utilized to image intracellular NADH levels to obtain molecular information regarding metabolic activity following steroid treatment. In this study, fluticasone propionate (FP)-treated, mometasone furoate (MF)-treated and untreated animals were imaged longitudinally using a custom-built multimodal optical microscope. Prolonged steroid treatment over the course of 21 days is shown to result in a significant increase in mean fluorescence lifetime of NADH, suggesting a faster rate of maturation of epidermal keratinocytes. Alterations to collagen organization and the structural microenvironment are also observed. These results give insight into the structural and biochemical processes of skin atrophy associated with prolonged steroid treatment.

Laser Depigmentation: A Case Report

Abstract:The pigmented lesions of the oral cavity may be endogenous or exogenous. Among the options for depigmenting these areas, laser therapy stands out for being a minimally invasive procedure. This study aims to report a clinical case of the gingival depigmentation technique using a high-power diode laser in the anterior maxillary region, for the ablation of the pigmented tissue to improve gingival aesthetics. The patient had an aesthetic complaint of the darkened aspect of the gingiva in the anterior maxillary portions. After local anesthesia, we started depigmentation with a high-power diode laser and performed ablation from the attached gingiva toward the free marginal gingiva. The patient returned after 30 and 180 days presenting healthy gingiva and absence of melanin repigmentation. Thus, we concluded that the diode laser was a good alternative for melanin depigmentation because it is a procedure with lower morbidity and satisfactory results.
A case of linear immunoglobulin A bullous dermatosis in a patient exposed to sun and an analgesic

Abstract:Background: Medications are the most common triggers of linear immunoglobulin A bullous dermatosis (LABD). LABD induced by ultraviolet (UV) radiation has rarely been described. This article reports a case of LABD in a patient exposed simultaneously to an analgesic and UV radiation. Case summary: A 45-year-old woman developed LABD lesions on sun-exposed skin after 3 days of sunbathing and consumption of a medication for headache containing propyphenazone, butalbital, and caffeine. The lesions spread to unexposed skin and, by day 5, the patient had vesicles and bullae on the palms and soles, face, trunk, and extremities. LABD was diagnosed with direct and indirect immunofluorescence microscopy and Western blot analysis. Treatment was successful with prednisone, started at a dosage of 1 mg/kg/d, for 5 months. Lesions located on sun-exposed areas, the absence of relapse for 5 years despite continuing sun exposure against medical advice, and subsequent avoidance of the suspected medication suggest that the bullous flare may have been due to the concomitant action of 2 triggers. That the analgesic had a role in this cutaneous manifestation is possible according to the Naranjo algorithm for adverse drug reactions. Conclusions: A case of LABD possibly associated with sun exposure and an analgesic is described. Treatment with prednisone successfully resolved the lesions in this patient.

Abstract:Abstract Nicolau syndrome (NS) is a rare cutaneous drug reaction that occurs at the site of an intramuscular drug or intra-articular injection. NS has been reported with the administration of various other drugs such as penicillins {Modzelewska I, Dawidowicz-Szczepanowska A. [Nicolau syndrome following administration of procaine penicillin]. Wiad Lek 1980; 33: 231–3 (in Polish)}, local anaesthetics, corticosteroids (Cherasse A, Kahn MF, Mistrih R et al. Nicolau’s syndrome after local glucocorticoid injection. Joint Bone Spine 2003; 70: 390–2) and nonsteroidal anti-inflammatory drugs. Patients experience extremely severe pain around the injection site of the drug immediately after injection, followed by rapid development of erythema, a livedoid reticular patch or a haemorrhagic patch. It may result in necrosis and ulceration of the skin. We report the case of 50-year-old woman who presented with areas of skin discoloration over her left axilla and left arm after an injection of Copaxone (glatiramer acetate) for her multiple sclerosis. It was painful and swollen. She has a past medical history of multiple sclerosis, epilepsy and an underactive thyroid. She has been on glatiramer acetate injections for multiple sclerosis for more than 3 years but has never had any reaction. Drug history included glatiramer acetate, levothyroxine, pregabalin, udenafil, propranolol, levetiracetam and docusate. There was no known drug allergy. She was seen by an on-call dermatologist. On examination there were bruise-like patches on her left lateral arm and left axilla, with some livedoid pattern. Bloods and medical photographs were taken. Incisional biopsy was done. On review after a few days, the swelling had decreased but the skin discoloration persisted and the skin was tender. Skin bacteriology swab was taken. Blood showed a slightly deranged clotting profile and raised creatinine kinase. Autoimmune screen was negative. Skin biopsy showed necrosis of the squamous epithelium, fibrinoid thrombi in superficial and mid-dermal blood vessels, and occasional eosinophils. This was suggestive of NS. Over time the rash evolved into indurated ulcerated plaques with a necrotic surface. Oral doxycycline 100 mg once daily was started, as a swab showed a heavy growth of Staphylococcus aureus. Glatiramer acetate injection was discontinued. She was treated with AQUACEL (silver-containing) dressing, clobetasol propionate cream for the surrounding skin, and surgical debridement. The skin healed completely within 20 weeks with scarring. NS is an important cause for morbidity and also of mortality in severe cases. There is no specific therapy. Conservative treatment with debridement, pain control and dressings are the mainstays of therapy. This case highlights the need for awareness about this rare condition.

Abstract:Linear atrophoderma of Moulin (LAM) is an uncommon dermatological disease characterised by linear, depressed plaques typically following Blaschko's lines. LAM generally occurs in adolescence or early adulthood and is more commonly observed in females. The aetiology of LAM is still unclear. We report a rare case of LAM in an 18-year-old female presenting with an atypical Blaschkoid distribution (multiple band-like pattern) on her right trunk. A clinical examination and histopathological analysis were performed to make the diagnosis. Partial improvement was obtained with calcipotriol and colchicine. LAM is a rare condition; we were only able to identify 23 case reports in the published literature. The findings of this report contribute to the limited literature on LAM, highlighting the clinical variability of LAM and suggesting potential novel variants beyond the classic presentation, emphasising the importance of recognising diverse manifestations for accurate diagnosis and management. Early recognition of LAM is crucial for appropriate treatment and improved patient outcomes. Further research is needed to elucidate LAM's aetiology and underlying mechanisms to facilitate the development of more targeted therapeutic strategies.
The injection of nodules of Dupuytren's disease with triamcinolone acetonide

Abstract:Over a 4-year period 63 patients (75 hands) with Dupuytren's nodules were treated with a series of injections with the steroid triamcinolone acetonide directly into the area of disease. The purpose of this study was to determine whether intralesional injections of triamcinolone acetonide could produce softening and flattening in nodules of Dupuytren's disease as seen in the intralesional injections of hypertrophic scars and keloids. After an average of 3.2 injections per nodule 97% of the hands showed regression of disease as exhibited by a softening or flattening of the nodule(s). Although some patients had complete resolution of the nodules, most experienced definite but incomplete resolution of the nodules in the range of 60% to 80%. Although a few patients did not experience recurrence or reactivation of the disease in the injected nodules or development of new nodules, 50% of patients did experience reactivation of disease in the nodules 1 to 3 years after the last injection, necessitating 1 or more injections. The findings of this study indicate that the intralesional injection of nodules of Dupuytren's disease with triamcinolone acetonide may modify the progression of the disease.
Inhibition of putative hyalurosome platform in keratinocytes as a mechanism for corticosteroid-induced epidermal atrophy

Abstract:The main limitation of using topical corticosteroids in dermatology is their atrophic effects on the skin. We have previously proposed a molecular platform composed of CD44, EGFR, and hyaluronate synthase (HAS) that is functionally defective in dermatoporosis, a chronic cutaneous insufficiency/fragility syndrome. In this study, we explored the molecular mechanisms of the skin atrophy induced by corticosteroids. We observed an important skin atrophy and a significant decrease of hyaluronic acid (HA), its main cell surface receptor CD44, and F-actin in mouse skin treated with topical clobetasol propionate (CP). Human keratinocytes exposed to CP showed an impaired HA secretion and diminished expression of CD44 and HAS3. CP also abolished filopodia of keratinocytes exposed to CP together with a redistribution of CD44 and F-actin depolymerization. We also show that HA fragments of intermediary size (HAFi) induced keratinocyte filopodia and protected them against CP. Topical HAFi induced hyperplasia in mouse epidermis and prevented CP-induced atrophy. Our results suggest that a CD44/EGFR/HAS platform associated with F-actin and filopodia of keratinocytes is the target of corticosteroids for their atrophogenic effects. These observations may lead to the development of new treatment and prevention strategies for corticosteroid-induced skin atrophy.
A girl with circular erythema on the forearm

Abstract:An 11 year old girl was referred to the dermatology clinic with circular urticarial lesions mainly on the forearm of the non-dominant arm (fig1, left panel). The lesions appeared every morning and lasted for 2-3 hours accompanied by pain. During hospital admission two weeks previously, the patient had been told the lesions were due to vasculitis. She received a course of systemic glucocorticoid treatment (40 mg/day methylprednisolone intravenous injection for 12 days), but the lesions did not improve. Routine tests for complete blood count, blood glucose, liver and renal function tests, antineutrophil cytoplasmic antibody, and serum complement were within normal range. Histological examination of a skin biopsy sample showed a non-specific pattern. Abstract:Linear IgA bullous dermatosis (LABD) is a rare, idiopathic, or drug-induced vesiculobullous disease caused by IgA autoantibodies in the basement membrane zone. An 84-year-old man was started on spironolactone two weeks before presentation for the management of hypertension and heart failure with preserved ejection fraction. He presented to our hospital for evaluation of worsening lower extremity swelling and a painful pruritic rash that started on the day preceding his presentation. On examination, he had 3+ lower extremity edema and an erythematous, painful, pruritic, bullous rash on all his extremities. He had a significantly elevated IgA level (1033 mg/dL). A lesional skin biopsy demonstrated epidermal ulceration with degenerated collagen fibers. Direct immunofluorescence of the perilesional skin showed linear IgA at the dermal-epidermal junction. The rash resolved following steroid therapy and discontinuation of spironolactone. There have been previous reports of bullous pemphigoid induced by spironolactone. To our knowledge, LABD associated with spironolactone has not previously been reported.

Abstract:Isolated cutaneous swelling can have varied etiologies. The clinical diagnosis is usually difficult, and a correct diagnosis always requires a pathological examination. Hereby, we report a case of linear keloidal morphea on the neck of an 18‐year‐old male who presented with an asymptomatic, firm lesion for 6 months. Histopathological examination was consistent with morphea. This case highlights the uncommon form of morphea in an unusual location, which can be misdiagnosed for numerous neoplastic conditions and for which simple histopathological evaluation can clinch the diagnosis.

Treatment of Iron-Induced Cutaneous Hyperpigmentation With Energy-Based Devices

Abstract:Objectives: Iatrogenic cutaneous siderosis is a well-recognized dermatologic complication after parenteral iron infusion. The condition manifests as discrete, hyperpigmented patches near the site of injection. Most cases do not resolve spontaneously, leading to significant aesthetic and psychological distress to patients. A recent case of iatrogenic cutaneous siderosis at our institution prompted a systematic review of the efficacy of energy-based devices previously reported in the treatment of this condition. Methods: PubMed and Cochrane databases were searched for all peer-reviewed articles published using the following search terms: "iron OR heme OR hemosiderosis OR siderosis" and "hyperpigmentation OR staining OR tattoo." Articles reporting on energy-based devices in the treatment of iron-induced hyperpigmentation were included. Results: A total of seven articles and 54 total patients were included in this review. All patients, including the patient treated at our institution, were female, with an average age of 44 years. Hyperpigmentation was most commonly associated with intravenous iron infusion (48/54, 89%), on the arm or forearm (44/54, 81%), and used for the treatment of underlying iron deficiency anemia (54/54, 100%). The application of six different nanosecond or picosecond quality-switched laser systems was reported in the treatment of cutaneous siderosis, with wavelengths ranging from 532 to 1064 nm. Spot sizes varied between 2 and 7 mm, with energy fluences spanning 0.5-40 J/cm2 depending on both the device and spot size. Outcomes were measured after an average of 5.4 laser treatments and 10.4 months, with over half of all reported patients experiencing complete clearance (27/50, 54%). Our patient received treatment in three test areas with picosecond alexandrite 785 nm, nanosecond Nd:YAG 532 nm, and picosecond Nd:YAG 532 nm devices. The nanosecond Nd:YAG 532 nm treated area demonstrated the greatest improvement, and the entire arm was subsequently treated with this device. Conclusions: Despite the often intractable nature of iatrogenic cutaneous siderosis, laser surgery is a reasonable and safe treatment modality for patients seeking cosmetic improvement of this dyschromia. Dermatologists should be aware of this entity and the efficacy of the energy-based devices currently in our armamentarium. A combination approach may need to be utilized with different wavelengths and pulsed widths to target iron pigment in both dermal and subcutaneous layers.
Nail Discoloration Following the Chronic Use of Skin-Depigmenting Creams

Abstract:Skin-depigmenting creams are on the rise, driven by societal pressures that equate fair skin with beauty, success, and happiness. This trend has led to many unregulated products being released on the market, causing various adverse effects on users. This case report examines five patients with nail hyperpigmentation following the use of skin-depigmenting creams containing hydroquinone (HQ). The findings from this case report showcase overlooked side effects of these widely used creams. Healthcare providers may be unfamiliar with the rare side effects of depigmenting creams, leading to misdiagnoses and unnecessary evaluations. Moreover, there is a pressing need for healthcare authorities to enhance regulation and for healthcare providers to be more vigilant in diagnosing presentations of nail hyperpigmentation. This case report provides valuable insights into the side effects of HQ-based creams as observed in the Asiatic population.
Successful treatment of earlobe keloids in the pediatric population

Abstract:Background: Keloid scars present a difficult treatment challenge. Recently, intralesional steroid injection has become a common treatment modality [Akoz et al. Aesthetic Plast Surg. 2002;6:184-188; Studdiford et al. JABFM. 2008;21:149-152]. Although this has become a proven treatment technique, there is no standard injection protocol to which treating physicians commonly adhere. We hypothesize that timing of steroid injection may improve outcomes using this treatment technique in combination with lesion excision. Methods: Fifteen patients with 16 earlobe keloids were treated using a standard steroid injection protocol with Kenalog (Bristol-Myers Squibb, New York, NY), in combination with lesion excision. Strict follow-up was enforced, with repeat injections as needed at any sign of abnormal scar formation postoperatively. Results: Of 16 lesions, 15 (94%) were treated successfully with no sign of lesion recurrence at 6 months of follow-up. A single lesion was lost to follow-up and presented 18 months postoperatively with recurrence. This lesion was subsequently retreated successfully. Conclusions: Kenalog injection in combination with excision is a well-tolerated and effective treatment of earlobe keloids in the pediatric population. We feel that timing of injection and adherence to a strict follow-up regimen is crucial to success.
Local and Systemic Side Effects of Corticosteroid Injections for Musculoskeletal Indications

Abstract:Corticosteroid injections can be associated with a range of potential side effects, which may be classified as local or systemic and further stratified as immediate or delayed in onset. Radiologists performing image-guided musculoskeletal injections should recognize the potential side effects of corticosteroid medication when counseling patients before injection and consider such side effects in planning individual injections. This Review summarizes the available evidence regarding the local and systemic side effects of corticosteroid injections performed for musculoskeletal indications. Local side effects include postinjection flare, skin hypopigmentation and atrophy, infection, tendon rupture, accelerated progression of osteoarthritis, and osseous injury. Systemic side effects include adrenal suppression or insufficiency, facial flushing, hypertension, hyperglycemia, and osteoporosis. Additional targeted counseling is warranted regarding side effects that are specific to certain patient populations (i.e., premenopausal women, patients with diabetes, athletes, and pediatric patients). Corticosteroid injections are contraindicated in the presence of superficial or deep infection, fracture, or a prosthetic joint. Guidelines on the frequency, duration, and maximal lifetime use of corticosteroid injections are currently lacking. Further research is needed regarding the long-term complications of continuous corticosteroid use, particularly with regard to osseous effects.
Linear Psoriasis in a Pediatric Patient Successfully Treated With Ixekizumab

Abstract:Linear psoriasis is a rare clinical presentation of psoriasis characterized by erythematous scaly plaques distributed along Blaschko's lines. We describe an 8‐year‐old male with a linear Blaschkoid plaque in which biopsy findings and clinical history were consistent with linear psoriasis. The lesion showed only mild improvement with topical steroid therapy but exhibited significant clearance with the biologic agent ixekizumab, an IL‐17A inhibitor. With limited supporting literature regarding management, this case report illustrates the clinical manifestation, diagnosis, and an effective management strategy for linear psoriasis in a pediatric patient.
