Abstract:Infants with possible cerebral palsy (CP) are commonly assumed to benefit from early diagnosis and early intervention, but substantial evidence for this is lacking. There is no consensus in the literature on a definition of 'early', but this review focuses on interventions initiated within the first 6 months after term age. We cover basic neuroscience, arguing for a beneficial effect of early intervention, and discuss why clinical research to support this convincingly is lacking. We argue that infants offered early intervention in future clinical studies must be identified carefully, and that the intervention should be focused on infants showing early signs of CP to determine an effect of treatment. Such signs may be efficiently detected by a combination of neuroimaging and the General Movements Assessment. We propose a research agenda directed at large-scale identification of infants showing early signs of CP and testing of high-intensity, early interventions.

Abstract:This paper reviews the opportunities and challenges for early diagnosis and early intervention in cerebral palsy (CP). CP describes a group of disorders of the development of movement and posture, causing activity limitation that is attributed to disturbances that occurred in the fetal or infant brain. Therefore, the paper starts with a summary of relevant information from developmental neuroscience. Most lesions underlying CP occur in the second half of gestation, when developmental activity in the brain reaches its summit. Variations in timing of the damage not only result in different lesions but also in different neuroplastic reactions and different associated neuropathologies. This turns CP into a heterogeneous entity. This may mean that the best early diagnostics and the best intervention methods may differ for various subgroups of children with CP. Next, the paper addresses possibilities for early diagnosis. It discusses the predictive value of neuromotor and neurological exams, neuroimaging techniques, and neurophysiological assessments. Prediction is best when complementary techniques are used in longitudinal series. Possibilities for early prediction of CP differ for infants admitted to neonatal intensive care and other infants. In the former group, best prediction is achieved with the combination of neuroimaging and the assessment of general movements, in the latter group, best prediction is based on carefully documented milestones and neurological assessment. The last part reviews early intervention in infants developing CP. Most knowledge on early intervention is based on studies in high-risk infants without CP. In these infants, early intervention programs promote cognitive development until preschool age; motor development profits less. The few studies on early intervention in infants developing CP suggest that programs that stimulate all aspects of infant development by means of family coaching are most promising. More research is urgently needed.

Abstract:AIM: To systematically review the evidence on the effectiveness of motor interventions for infants from birth to 2 years with a diagnosis of cerebral palsy or at high risk of it.METHOD: Relevant literature was identified by searching journal article databases (PubMed, Embase, CINAHL, Cochrane, Web of Knowledge, and PEDro). Selection criteria included infants between the ages of birth and 2 years diagnosed with, or at risk of, cerebral palsy who received early motor intervention.RESULTS: Thirty-four studies met the inclusion criteria, including 10 randomized controlled trials. Studies varied in quality, interventions, and participant inclusion criteria. Neurodevelopmental therapy was the most common intervention investigated either as the experimental or control assignment. The two interventions that had a moderate to large effect on motor outcomes (Cohen's effect size>0.7) had the common themes of child-initiated movement, environment modification/enrichment, and task-specific training.INTERPRETATION: The published evidence for early motor intervention is limited by the lack of high-quality trials. There is some promising evidence that early intervention incorporating child-initiated movement (based on motor-learning principles and task specificity), parental education, and environment modification have a positive effect on motor development. Further research is crucial.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Aim</h3><p>To perform an overview of systematic reviews and more recent randomized controlled trials (RCTs) on early motor interventions in infants aged 0 to 3 years with or at risk of cerebral palsy to inform current clinical and research efforts and provide a benchmark to assess future interventions ideally initiated within the first 6 months.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Method</h3><p>Standardized searches of the PubMed, Embase, Scopus, and Web of Science databases were conducted for systematic reviews (2009–2020) and RCTs (2015–2020).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>From 840 unique records, 31 full texts were reviewed, yielding three systematic reviews encompassing 46 studies, 16 with comparison groups, and six additional RCTs that met the criteria. Two enrichment‐ and activity‐based approaches had medium effect sizes on motor development, only one with low risk of bias; two others had large task‐specific effect sizes but some bias concerns; and three enriched environment studies with some bias concerns had medium effect sizes on cognitive development. Most had small or no effect sizes, bias concerns, and uncertain diagnostic determinations.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Interpretation</h3><p>Data synthesis revealed limited data quantity and quality, and suggest, although not yet confirmed, greater benefit from early versus later intervention. Research efforts with greater early diagnostic precision and earlier intervention are accelerating, which may transform future outcomes and practices.</p></section>

Abstract:The aims of this study were to describe physical therapy (PT) and occupational therapy (OT) services for a cohort of 399 children with cerebral palsy (CP), 2-6 years old, residing in the United States and Canada. Parents completed a services questionnaire by telephone interview. Therapists classified children's Gross Motor Function Classification System (GMFCS) level. Mean minutes per month of PT and OT were greater for children receiving services in both an educational and clinic setting. Mean minutes per month of PT and OT were greater for children in levels IV-V than children in level I and greater for children in the United States than children in Canada. Parents reported that interventions focused a moderate to great extent on primary impairments, secondary impairments, activity, and structured play activities, a moderate extent on environmental modifications and equipment; and a moderate to small extent on self-care routines. The results support the importance of coordination of PT and OT services.

Abstract:Background/Objectives: Despite evidence of the effectiveness of early intervention (EI) programs, eligible infants often experience delays in initiation of services or fail to receive services entirely. Disparities have been documented, including lower enrollment rates for infants with public insurance. The objective of this pilot study was to evaluate the feasibility of initiating home physical therapy (PT) services promptly after neonatal or cardiac intensive care unit (NICU/CICU) discharge for infants with public insurance and to assess early motor outcomes for children who received study therapy compared with a standard of care group. Methods: Infants were recruited if discharged from a study NICU/CICU, had public insurance, and were eligible for Illinois EI services. Infants living in Chicago (n = 46) received weekly home-based PT from a study therapist until 3–4 months corrected age (CA). Infants living outside Chicago received standard of care services and served as a control group (n = 14). At discharge, infants were referred to EI and underwent the Test of Infant Motor Performance (TIMP). Outcomes at 3–4 months CA included initiation rates for study PT and EI and follow-up TIMP testing. Results: By 3–4 months CA, 78% of the intervention group had received ≥1 PT session. In contrast, just 13% of the entire cohort had received any EI therapy. Infants who had 8–10 PT sessions in the first 3–4 months after discharge were more likely to have a change in the TIMP Z-score of >0.5 SD. Conclusions: Prompt transition to home therapy was feasible for infants with public insurance in an urban setting who may benefit most due to the potential for neuroplastic change. Addressing barriers identified in this study may assist in improving access to EI for young infants.

Abstract:This study aimed to evaluate assessment and referral practices for the early detection and diagnosis of children at risk for or with cerebral palsy (CP) by health care and education providers in Maryland and Delaware. A secondary aim was to identify barriers for using early detection tools and identify opportunities for change to support early diagnosis and improve care. Seventy-two participants answered ≥ 50% of the survey questions. Most were occupational or physical therapists (86%) working in early intervention (61%). Eighty-eight percent indicated awareness that CP can be diagnosed by 12 months. Though 86% stated they typically suspect a diagnosis of CP between 0 and 12 months, only 19% reported that their patients received a CP diagnosis < 12 months. The Developmental Assessment of Young Children (73%) and the Peabody Developmental Motor Scales-2 (59%) were used most. Many respondents indicated never using magnetic resonance imaging (70%), the General Movements Assessment (87%), or the Hammersmith Infant Neurological Exam (69%). Participants identified clinical signs and symptoms prompting a referral for the diagnostic assessment of CP, most commonly stiffness in legs (95%), excessive head lag (93%), and persistent fisting (92%). Policy and organizational change, clinician education, and training are needed to support the implementation of CP early detection guidelines.

Early intervention in high-risk infants with cerebral palsy

Abstract:Objective: Because of improvement in obstetrical and neonated care, the perinatal mortality is decreased, yet infants at high risk cerebral palsy (CP) are increased. The aim of this study was to investigate and evaluate the method of early intervention in high risk infants, in order to reduce the sequelae of brain damages. Method: Ninety infants (61 boys, 29 girls) ranging from 12 days to 12 months (average age: 3.83±2.56 months) underwent an early intervention program. Sixty-eight of them were suspected as high risk infants of CP, and the other 22 infants (average age: 4.74±2.77 months) were diagnosed CP already. The Gesell Scale of Infant Development and neurologic examination were used for evaluation. Result: After 2 years early rehabilitation and follow-up, 68.2% infants with CP were cured, 18.2% were at borderline and only 13.6% were still abnormal. Sixty-eight infants with suspected CP developed normal except 3 of them were lost to follow-up. The prevalence of CP among the infants with premature, low birth weight, apnea and hypoxic ischemic encephalopathies were reduced significantly. Conclusion: The sequelae of brain damages could be reduced by early intervention of high risk infants of CP, and the cured effect could be achieved by early rehabilitation in infants with CP.

Abstract:<b>Purpose:</b> Evidence for efficacy of early intervention in infants at high risk of cerebral palsy (CP) is limited. We compared outcome of infants at very high risk of CP after receiving the family centered program COPing with and CAring for infants with special needs (COPCA) or typical infant physiotherapy.<b>Materials and methods:</b> Forty-three infants were randomly assigned before the corrected age of 9 months to 1 year of COPCA (<i>n</i> = 23) or typical infant physiotherapy (<i>n</i> = 20). Neuromotor development, cognition, and behavior was assessed until 21 months corrected age. Video-recorded physiotherapy sessions were quantitatively analyzed for further process analyses. Outcome was evaluated with nonparametric tests and linear mixed effect models.<b>Results:</b> During and after the interventions, infant outcome in both intervention groups was similar [primary outcome Infant Motor Profile: COPCA 82 (69-94), typical infant physiotherapy 81 (69-89); Hodges Lehman estimate of the difference 0 (confidence interval -5;4)]. Outcome was not associated with contents of intervention.<b>Conclusions:</b> One year of COPCA and 1 year of typical infant physiotherapy in infants at high risk of CP resulted in similar neurodevelopmental outcomes. It is conceivable that combinations of active ingredients from different approaches are needed for effective early intervention.IMPLICATIONS FOR REHABILITATIONFor infants at very high risk of cerebral palsy, 1 year of intervention with the family-centred programme Coping with and Caring for infants with special needs resulted in similar infant outcome as 1 year of typical infant physiotherapy.Infant's neuromotor, cognitive, and behavioural outcome was not associated with specific interventional elements, implying that the various elements may have a similar effect on developmental outcome.We suggest that a specific mix of ingredients of different approaches may work best, resulting in comprehensive care including both infant and family needs.

Abstract:OBJECTIVE To test efficacy of a parent-delivered multidomain early intervention (Learning through Everyday Activities with Parents [LEAP-CP]) for infants with cerebral palsy (CP) compared with equal-dose of health advice (HA), on (1) infant development; and (2) caregiver mental health. It was hypothesized that infants receiving LEAP-CP would have better motor function, and caregivers better mental health. METHODS This was a multisite single-blind randomized control trial of infants aged 12 to 40 weeks corrected age (CA) at risk for CP (General Movements or Hammersmith Infant Neurologic Examination). Both LEAP-CP and HA groups received 15 fortnightly home-visits by a peer trainer. LEAP-CP is a multidomain active goal-directed intervention. HA is based on Key Family Practices, World Health Organization. Primary outcomes: (1) infants at 18 months CA: Pediatric Evaluation of Disability Inventory-Computer Adaptive Test (PEDI-CAT mobility); and (2) caregiver: Depression Anxiety and Stress Scale. RESULTS Of eligible infants, 153 of 165 (92.7%) were recruited (86 males, mean age 7.1±2.7 months CA, Gross Motor Function Classification System at 18 m CA: I = 12, II = 25, III = 9, IV = 18, V = 32). Final data were available for 118 (77.1%). Primary (PEDI-CAT mobility mean difference = 0.8 (95% CI −1.9 to 3.6) P = .54) and secondary outcomes were similar between-groups. Modified-Intention-To-Treat analysis on n = 96 infants with confirmed CP showed Gross Motor Function Classification System I and IIs allocated to LEAP-CP had significantly better scores on PEDI-CAT mobility domain (mean difference 4.0 (95% CI = 1.4 to 6.5), P = .003) compared with HA. CONCLUSIONS Although there was no overall effect of LEAP-CP compared with dose-matched HA, LEAP-CP lead to superior improvements in motor skills in ambulant children with CP, consistent with what is known about targeted goal-directed training.

Abstract:Physiotherapy plays a key role in the management of cerebral palsy (CP) and comprises of various therapeutic interventions in enhancing the various physiological and functional outcomes. Though physiotherapy is used widely and recommended by all members of the health-care team, the effectiveness of physiotherapy is inconsistent. The objective of this review was to summarize and evaluate the effectiveness of physiotherapy interventions in children with C P. PubMed and Cochrane database were searched from January 2006 to April 2017 using the Medical Subject Heading and general keywords. Only systematic reviews and meta-analysis on PT interventions in children diagnosed with CP were included. Two reviewers independently assessed the methodological quality and retrieved the results. Thirty-four systematic reviews were identified that distinguished 15 different interventions. Moderate evidence of effectiveness was found for constraint-induced movement therapy for upper limb recovery, goal-directed/functional training, and gait training to improve gait speed. Conflicting evidence was found for the role of exercises on strength training and cardiorespiratory training. Intervention such as neurodevelopmental therapy (NDT) was found ineffective. This review suffer from limitations such as including reviews that had small sample size and that had considered heterogeneity of treatment interventions. Hence, the effectiveness of most PT interventions is found to be limited. On the basis of the present evidence, functional goal-oriented approaches are found to be effective and future research is required to determine the best ways to improve functional outcomes in children with CP.

Abstract:Objective The study tried to analyze the therapeutic effect of early intervention treatment on high-risk spastic type cerebral palsy.Methods Totally 67infants with high-risk cerebral palsy were divided into the super early intervention group(1~3months old,n=36cases)and the early intervention group(4~6months old,n=31cases),according to the age for treatment.Each group was treated with the exercise therapy,which is Bobath rehabilitation combined with Vojta rehabilitation.Finally,momosialotetrahexosylganglioside sodium infusion was carried out with 5%glucose.The treatment period lasted for 3months.Then,the treatment evaluation was performed based on Gesell assessment law,muscle tension inspection,Vojta posture reflection,and image alterations of brain CT before and after treatment.The Gesell developmental scales were used to test the development quotient(DQ)and development age(DA)of two groups after 3months treatment.Results After 3months training,the scores of DAis lower than CAof two groups,the super early intervention group has higher scores than the early intervention group.The scores of DQ,muscle tension,Vojta posture reflection and CT images were improved well in two treatment groups.The infants,symptoms were improved well after treatment in either super early intervention group or early intervention group with significant difference(P<0.05).Comparing between the two groups,the efficacy of super early intervention group was much better than early intervention group after treatment,suggesting early therapy is necessary for the disease.Conclusion Early intervention treatment on high-risk cerebral palsy has significant effects on symptoms,and early therapy is necessary for the high-risk infants.

Abstract:OTJR: Occupational Therapy Journal of Research, Ahead of Print. Hemiplegic Cerebral Palsy (CP) is the most common pediatric motor disability, characterized by unilateral motor weakness. Pediatric Constraint-Induced Movement Therapy (pCIMT) improves affected extremity function but faces variable clinical integration. This study assessed U.S. providers' awareness and use of pCIMT, educational practices, and barriers to broader implementation for more eligible children. Overall, 148 providers specializing in pediatric stroke or hemiplegic CP completed surveys on pCIMT familiarity, implementation challenges, and support for evidence-based practices (EBP). Participants indicated high pCIMT competency. Although 75% reported regional pCIMT availability, only 14% indicated that pCIMT is accessible to all children who could benefit. Reported barriers included therapist and family availability, cost, and institutional limitations. Despite valuing EBP, participants reported minimal workplace support for its use. The study revealed significant barriers to pCIMT accessibility and implementation. Further research is needed to address these challenges and improve clinical adoption of EBP, such as pCIMT.Plain Language SummaryUnderstanding How to Get a Targeted Movement Intervention to All Children With One-Sided Weakness in the United StatesHemiplegic Cerebral Palsy (CP) affects movement on one side of the body and is the most common pediatric movement disorder. Pediatric Constraint-Induced Movement Therapy (pCIMT) improves the use of the affected limb, but it is not widely used. We wanted to understand why and figure out where and how it is being offered. We surveyed 148 professionals in health care and academia about their awareness of pCIMT, what makes it hard to use, whether pCIMT is offered near them, and about workplace support for evidence-based treatments. The survey also asked how pCIMT is typically provided, including how long treatments last, type of constraint, and constraint wearing schedule. Most providers reported awareness of and local availability of pCIMT. However, they reported that not all children who might benefit from it had access. The main obstacles that were identified were finding enough time for therapists and families, the cost of treatment, and limits set by health care facilities. In addition, pCIMT programs were reported to vary widely in how they were structured and delivered, which could affect how well they work. Workplaces generally supported evidence-based practices but did not always make them a top priority. Better understanding provider's perspectives about pCIMT, how it is currently offered, and obstacles to availability can aid in the identification of implementation strategies to address the challenges that make it difficult for providers to offer widely. Future research can investigate the impact of educating therapists about its benefits and how it can be covered by insurance/ Medicaid; the use of flexible guidelines for pCIMT programs that can be adapted to different settings and patient needs; and specialized training.

Methodological quality including risk of bias was scrutinized.RESULTS: Thirteen papers met the inclusion criteria. Seven studies with moderate to high methodological quality were analysed in detail; they evaluated neurodevelopmental treatment only (n=2), multisensory stimulation (n=1), developmental stimulation (n=2), and multifaceted interventions consisting of a mix of developmental stimulation, support of parent-infant interaction, and neurodevelopmental treatment (n=2). The heterogeneity precluded conclusions. Yet, two suggestions emerged: (1) dosing may be critical for effectiveness; (2) multifaceted intervention may offer best opportunities for child and family.INTERPRETATION: The literature on early intervention in very high-risk infants with sufficient methodological quality is limited, heterogeneous, and provides weak evidence on the effect. More studies are urgently needed. Suggestions for future research are provided.

Abstract:One of the oldest and probably well-known examples of cerebral palsy is the mummy of the Pharaoh Siptah about 1196–1190 B.C., and a letter from Hippocrates (460–390 B.C.). Cerebral palsy (CP) is one of the most common congenital or acquired neurological impairments in paediatric patients, and refers to a group of children with motor disability and related functional defects. The visible core of CP is characterized by abnormal coordination of movements and/or muscle tone which manifest very early in the development. Resulting from pre- or perinatal brain damage CP is not a progressive condition per se. However, without systematic medical and physiotherapeutic support the dystonia leads to muscle contractions and to deterioration of the handicap. Here we review the three general spastic manifestations of CP hemiplegia, diplegia and tetraplegia, describe the diagnostic procedures and delineate a time schedule for an early intervention.
The effect of intense physical therapy for children with cerebral palsy

Abstract:PURPOSE: To determine the effect of an intense physical therapy intervention on gross motor function, community walking and participation in children with cerebral palsy (CP).METHODS: A single group design was used with two pre-test and two post-test measures. Subjects were 17 ambulatory children with CP who participated in an intense intervention (i.e., four hours per day, five days per week, three weeks), a modified version of the TheraSuit protocol. Gross motor function measure (GMFM-66), Step watch activity monitor (SAM), Canadian occupational performance measure (COPM) and pediatric outcomes data collection instrument (PODCI) were tested twice at baseline, immediately following the intervention, and three months later.RESULTS: Immediately following the intervention, GMFM-66, COPM and PODCI scores improved significantly (p < 0.001). At three months, improvements remained for GMFM-66 and COPM (p < 0.01). Walking amount or intensity (SAM) did not improve.CONCLUSIONS: Participants improved gross motor skills and participation but not community ambulation following this intense physical therapy intervention.
Effects of Cerebral Palsy Early Intervention Model to Development and Prognosis of Premature Infants

Abstract:Objective: To explore the effects of cerebral palsy early intervention model on development and prognosis of premature infants.Methods: Four hundred and thirty-four survived premature infants,excluding those with congenital deformity and hereditary metabolic diseases,were randomly divided into two groups:intervention group and routine care group.The infants of intervention group received massage-exercise and active motion training on basis of early education at home after discharged from hospital.All infants with abnormal motion manifestations were given appropriate rehabilitation training.In the routine care group,infants received routine care only.Results: At the age of 1,the average scores of mental development index(MDI) and psychomotor development index(PDI) were significantly higher in early intervention group than those in routine care group(P0.01),and the incidence of hypoevolutism in motion development was lower in the intervention group than in the routine care group(P0.001).Conclusion: Early intervention can promote intellectual development,motion development and prognosis of the premature infants.

Abstract:Cerebral palsy (CP) is defined as motor impairment that limits activity, and is attributed to non-progressive disturbances during brain development in fetuses or infants. The motor disorders of CP are frequently accompanied by impaired cognition, communication, and sensory perception, behavioural abnormalities, seizure disorders, or a combination of these features. CP is thought to affect three to four individuals per 1000 of the general population. The incidence, prevalence, and most common causes of CP have varied over time because of changes in prenatal and paediatric care. Medical management of children and adults involves care from primary-care physicians with input from specialists in neurology, orthopaedics, and rehabilitation medicine. Physicians should also work in conjunction with rehabilitation therapists, educators, nurses, social care providers, and schoolteachers. The focus of rehabilitation treatment has recently shifted to neurological rehabilitation in response to increasing evidence for neuroplasticity. This approach aims to improve development and function by capitalising on the innate capacity of the brain to change and adapt throughout the patient's life. As the life expectancy of individuals with CP approaches that of the general population, therapies must be developed that address the needs of adults ageing with disability.
