Abstract:Objective. Lower limb amputees suffer from a variety of functional deficits related to the absence of sensory communication between the central nervous system and the lost extremity. Indeed, they experience high risk of falls, asymmetric walking and balance, and low prosthesis embodiment, that significantly decrease their quality of life. Presently, there are no commercially available devices able to provide sensory feedback to leg amputees but recently some invasive solutions (i.e. requiring surgery) have been proposed by different research groups. However, a non-invasive effective alternative exploitable in everyday life is still missing.Approach. To address this need we developed and tested a lightweight, non-invasive, wearable technology (NeuroLegs) providing sensory (i.e. knee angle joint and tactile) feedback to the users through electro-cutaneous stimulation. Standard mechanical and electrical tests were performed to assess the safety and reliability of the technology. The NeuroLegs system was verified in terms of accuracy in measuring relevant gait parameters in healthy participants. The effectiveness of the NeuroLegs system at improving walking of three transfemoral amputees was then verified in movement laboratory tests.Main results. No mechanical failures, stable communication among system's parts and a long-lasting battery were demonstrated. A high temporal reliability was found when detecting stride features (important for the real-time configuration) with a correct match to the walking cadence in all assessed walking conditions. Finally, transfemoral amputees showed increased temporal gait symmetry and augmented confidence when walking with the sensory feedback compared to no feedback condition. Stepping outside from the lab, NeuroLegs was successfully exploited by a transfemoral amputee in CYBATHLON Global Edition 2020 in several challenging situations related to daily-living activities.Significance. Our results demonstrate that the NeuroLegs system provides the user with useful sensory information that can be successfully exploited in different walking conditions of daily life.
Virus‐specific cytotoxic T‐lymphocytes in HIV‐2-infected cynomolgus macaques

Abstract:ObjectiveSpecific cytotoxic T-lymphocytes (CTL) are induced in humans or monkeys after infection with HIV-1 or SIVmac, respectively. Since, like HIV-1, HIV-2 causes AIDS, our objective was to determine the characteristics of the HIV-2-specific CTL response. DesignSince it is rarely possible to study cellular immunity in individuals, because of the small number of HIV-2-infected patients available in Europe and the necessity for co-operation in the performance of sequential CTL assays, cynomolgus macaques were infected with HIV-2. Autologous transformed B-lymphoblastoid cell lines infected with recombinant vaccinia viruses were used as target cells for cytotoxicity assays. MethodsRecombinant vaccinia viruses expressing HIV-2 genes were constructed to infect B-lymphoblastoid cell lines from macaques. These cells were used as target cells for cytotoxicity assays with peripheral blood mononuclear cells from HIV-2BEN-infected cynomolgus macaques. To characterize the effector cells, CD8+ cells were separated with immunomagnetic beads. Major histocompatibility complex (MHC) restriction of the cytotoxic cells was determined by incubation with matched or mismatched target cells. ResultsHIV-2BEN-infected cynomolgus macaques raised CTL against proteins of the three major viral structural genes, gag, pol and env. The cytotoxic cells were CD8+ and their activity was MHC class I-restricted. In contrast to SIVmac-infected macaques, env-specific lysis was mediated exclusively by CD8+ cells. CTL from individual animals recognized different viral proteins and the recognition pattern varied over time. ConclusionsLike HIV-1 and SIVmac, HIV-2 induces virus-specific CTL. The variation of antigen recognition between individual animals and over time indicates that sequential experiments are necessary to determine the complete spectrum of the CTL response of infected animals. HIV-2-infected macaques represent a suitable model for investigations into the cellular immune response against HIV.

Abstract:Human during walking can sense the joint angle and the contact force between the leg and the ground and uses such information as the feedback to regulate walking performance. To complete this motor control for the amputee with the leg prosthesis, human-in-the-loop control was proposed. This control method involves the restoration of the sensory feedback and the unified gait generator. By restoring the sensory feedback of the prosthesis with noninvasive Functional Electrical Stimulation (nFES), the subject can sense touch-down moment, leave-off moment, and height of small obstacles on the ground. Then, the unified gait generator converts the subject's intent into the prosthesis' motor command according to the damping and Spring-Loaded Inverted Pendulum (D-SLIP) model. When walking is slow, the prosthesis is stiff. When walking is fast, the prosthesis is compliant. Three experiments were conducted to prove the improvement of walking performance, such as the precision to sense the height of small obstacles, and the amount of increment for walking speed and frequency on level ground, S curve, or stairs. These results showed that the proposed control method can improve the motion capability of subjects greatly.
Hypoglycemia in the ICU: Discrepancy between capillary and serum glucose measurements in patients with renal disease

Abstract:Recently, a focus on tight glycemic control in intensive care units (ICU) has resulted in implementation of strict insulin protocols requiring frequent glucose monitoring. The use of point-of-care (POC) capillary glucose testing is widespread, but its validity in the ICU has been questioned. Our objective is to better understand the use of POC glucose at the extremes of glycemic control through a case review at our institution. We describe the case of a 75-year-old non-diabetic female with end stage renal disease (ESRD) on hemodialysis who was admitted with apparent hypoglycemia. After extensive workup was done for a seemingly refractive hypoglycemia, a discrepancy between POC capillary glucose and central serum glucose levels was discovered, revealing actual euglycemia and false low POC glucose values. Cases of hypoglycemia can be challenging, especially in non-diabetic patients with ESRD. While glucometers assessing capillary glucose are used both in the outpatient and inpatient environment, their validity in the critically ill patient has known limitations. Cases such as this have led to the development of systemic checks and balances, as well as further investigations regarding the use of POC glucose meters in the ICU. This case serves as a reminder to evaluate for all causes for abnormal laboratory values, including technological limitations.

Abstract:Objective. Bakumondo-to (Maimendong tang) is a traditional oriental herbal medicine that has been used as an antitussive agent. We previously demonstrated that Bakumondo-to attenuates airway hyperresponsiveness induced by ozone. However, the mechanism(s) responsible for this effect remains unclear. In the present study, we examined the mechanism whereby Bakumondo-to inhibits ozone-induced airway hyperresponsiveness. First, we examined the effect of Bakumondo-to on prostanoids production, which are key mediators to airway hyperresponsiveness after ozone exposure. Second, we studied its effects on the vagal neuroeffector transmission, because vagal nerve is likely to play an important role in airway hyperresponsiveness after ozone. Methods. We measured the effects of Bakumondo-to on the concentrations of prostanoids in bronchoalveolar lavage fluid before and after ozone. We evaluated the effects of Bakumondo-to on the contraction of guinea pig tracheal smooth muscle evoked by electrical field stimulation (EFS) or the exogenous application of acetylcholine (ACh). Isometric tension of tracheal strips was measured in the presence of indomethacin (10−6 M) and of guanethidine (10−6 M). Results. Ozone caused significant increase in prostaglandin E2 (PGE2) and thromboxane B2 (TXB2); however, Bakumondo-to did not affect the increase in these prostanoids. Bakumondo-to (0.01 mg/mL–1 mg/mL) significantly suppressed the contraction evoked by EFS, but did not affect the ACh-evoked contraction, indicating that Bakumondo-to suppressed tracheal smooth muscle contraction pre-junctionally. Conclusion. These results suggest that the mechanism by which Bakumondo-to inhibits airway hyperresponsiveness depends on inhibiting the release of acetylcholine from vagal nerve terminals.
Continuous neural control of a bionic limb restores biomimetic gait after amputation

Abstract:For centuries scientists and technologists have sought artificial leg replacements that fully capture the versatility of their intact biological counterparts. However, biological gait requires coordinated volitional and reflexive motor control by complex afferent and efferent neural interplay, making its neuroprosthetic emulation challenging after limb amputation. Here we hypothesize that continuous neural control of a bionic limb can restore biomimetic gait after below-knee amputation when residual muscle afferents are augmented. To test this hypothesis, we present a neuroprosthetic interface consisting of surgically connected, agonist-antagonist muscles including muscle-sensing electrodes. In a cohort of seven leg amputees, the interface is shown to augment residual muscle afferents by 18% of biologically intact values. Compared with a matched amputee cohort without the afferent augmentation, the maximum neuroprosthetic walking speed is increased by 41%, enabling equivalent peak speeds to persons without leg amputation. Further, this level of afferent augmentation enables biomimetic adaptation to various walking speeds and real-world environments, including slopes, stairs and obstructed pathways. Our results suggest that even a small augmentation of residual muscle afferents restores biomimetic gait under continuous neuromodulation in individuals with leg amputation.

Abstract:Restoring locomotion functionality of transfemoral amputees is essential for early rehabilitation treatment and for preserving mobility and independence in daily life. Research in wearable robotics fostered the development of innovative active mechatronic lower-limb prostheses designed with the goal to reduce the cognitive and physical effort of lower-limb amputees in rehabilitation and daily life activities. To ensure benefits to the users, active mechatronic prostheses are expected to be aware of the user intention and properly interact in a closed human-in-the-loop paradigm. In the state of the art various cognitive interfaces have been proposed to online decode the user's intention. Electromyography in combination with mechanical sensing such as inertial or pressure sensors is a widely adopted solution for driving active mechatronic prostheses. In this framework, researchers also explored targeted muscles re-innervation for an objective-oriented surgical amputation promoting wider usability of active prostheses. However, information kept by the neural component of the cognitive interface deteriorates in a prolonged use scenario due to electrodes-related issues, thereby undermining the correct functionality of the active prosthesis. The objective of this work is to present a novel controller for an active transfemoral prosthesis based on whole body awareness relying on a wireless distributed non-invasive sensory apparatus acting as cognitive interface. A finite-state machine controller based on signals monitored from the wearable interface performs subject-independent intention detection of functional tasks such as ground level walking, stair ascent, and sit-to-stand maneuvres and their main sub-phases. Experimental activities carried out with four transfemoral amputees (among them one dysvascular) demonstrated high reliability of the controller capable of providing 100% accuracy rate in treadmill walking even for weak subjects and low walking speeds. The minimum success rate was of 94.8% in performing sit-to-stand tasks. All the participants showed high confidence in using the transfemoral active prosthesis even without training period thanks to intuitiveness of the whole body awareness controller.

Cognitive benefits of using non-invasive compared to implantable neural feedback

Abstract:A non-optimal prosthesis integration into an amputee's body schema suggests some important functional and health consequences after lower limb amputation. These include low perception of a prosthesis as a part of the body, experiencing it as heavier than the natural limb, and cognitively exhausting use for users. Invasive approaches, exploiting the surgical implantation of electrodes in residual nerves, improved prosthesis integration by restoring natural and somatotopic sensory feedback in transfemoral amputees. A non-invasive alternative that avoids surgery would reduce costs and shorten certification time, significantly increasing the adoption of such systems. To explore this possibility, we compared results from a non-invasive, electro-cutaneous stimulation system to outcomes observed with the use of implants in above the knee amputees. This non-invasive solution was tested in transfemoral amputees through evaluation of their ability to perceive and recognize touch intensity and locations, or movements of a prosthesis, and its cognitive integration (through dual task performance and perceived prosthesis weight). While this managed to evoke the perception of different locations on the artificial foot, and closures of the leg, it was less performant than invasive solutions. Non-invasive stimulation induced similar improvements in dual motor and cognitive tasks compared to neural feedback. On the other hand, results demonstrate that remapped, evoked sensations are less informative and intuitive than the neural evoked somatotopic sensations. The device therefore fails to improve prosthesis embodiment together with its associated weight perception. This preliminary evaluation meaningfully highlights the drawbacks of non-invasive systems, but also demonstrates benefits when performing multiple tasks at once. Importantly, the improved dual task performance is consistent with invasive devices, taking steps towards the expedited development of a certified device for widespread use.

Abstract:Commercial prosthetic devices currently do not provide natural sensory information on the interaction with objects or movements. The subsequent disadvantages include unphysiological walking with a prosthetic leg and difficulty in controlling the force exerted with a prosthetic hand, thus creating health issues. Restoring natural sensory feedback from the prosthesis to amputees is an unmet clinical need. An optimal device should be able to elicit natural sensations of touch or proprioception, by delivering the complex signals to the nervous system that would be produced by skin, muscles and joints receptors. This Review covers the various neurotechnological approaches that have been proposed for the development of the optimal sensory feedback restoration device for arm and leg amputees.

Abstract:Prosthetic limb embodiment remains a significant challenge for many amputees due to traditional designs’ lack of sensory feedback. To address this challenge, the effectiveness of non-invasive neuromuscular electrical stimulation (NMES) controlled by a hybrid proportional-differential (PD)-Fuzzy logic system was evaluated for providing real-time proprioception and tactile feedback. The study used a human-in-the-closed-loop approach with ten participants: five upper limb amputees and five non-disabled individuals as the control group. An applied force, the joint angle of a prosthetic hand’s finger, and surface electromyography signals generated by the biceps muscle all regulate the intensity of sensory feedback. Additionally, the C6 and C7 myotomes were selected as elicitation sites. The average threshold for detecting action motion and force was around 21° and 1.524N, respectively. The participants successfully reproduced desired joint angles within the range of 0°-110° at five separate intervals. In the weight recognition experiment, the amputee participant’s minimum number of false predictions was four. The highest accuracy achieved was 80.66% in detecting object size and stiffness. Additionally, unpaired t-tests were performed for the means of the results of the experiments to determine statistically significant differences between groups. The results suggest that stimulation of myotomes by NMES is an effective non-invasive method for delivering rich multimodal sensation information to individuals with disabilities, including upper limb amputees, without needing visual or auditory cues. These findings contribute to the development of non-invasive sensory substitution in prostheses.

Abstract:Cutaneous sensation is vital to controlling our hands and upper limbs. It helps close the motor control loop by informing adjustments of grasping forces during object manipulations and provides much of the information the brain requires to perceive our limbs as a part of our bodies. This sensory information is absent to upper-limb prosthesis users. Although robotic prostheses are becoming increasingly sophisticated, the absence of feedback imposes a reliance on open-loop control and limits the functional potential as an integrated part of the body. Experimental systems to restore physiologically relevant sensory information to prosthesis users are beginning to emerge. However, the impact of their long-term use on functional abilities, body image, and neural adaptation processes remains unclear. Understanding these effects is essential to transition sensate prostheses from sophisticated assistive tools to integrated replacement limbs. We recruited three participants with high-level upper-limb amputation who previously received targeted reinnervation surgery. Each participant was fit with a neural-machine-interface prosthesis that allowed participants to operate their device by thinking about moving their missing limb. Additionally, we fit a sensory feedback system that allowed participants to experience touch to the prosthesis as touch on their missing limb. All three participants performed a long-term take-home trial. Two participants used their neural-machine-interface systems with touch feedback and one control participant used his prescribed, insensate prosthesis. A series of functional outcome metrics and psychophysical evaluations were performed using sensate neural-machine-interface prostheses before and after the take-home period to capture changes in functional abilities, limb embodiment, and neural adaptation. Our results demonstrated that the relationship between users and sensate neural-machine-interface prostheses is dynamic and changes with long-term use. The presence of touch sensation had a near-immediate impact on how the users operated their prostheses. In the multiple independent measures of users' functional abilities employed, we observed a spectrum of performance changes following long-term use. Furthermore, after the take-home period, participants more appropriately integrated their prostheses into their body images and psychophysical tests provided strong evidence that neural and cortical adaptation occurred.

Abstract:Objective.Lower-limb amputees suffer from a variety of health problems, including higher metabolic consumption and low mobility. These conditions are linked to the lack of a natural sensory feedback (SF) from their prosthetic device, which forces them to adopt compensatory walking strategies that increase fatigue. Recently, both invasive (i.e. requiring a surgery) and non-invasive approaches have been able to provide artificial sensations via neurostimulation, inducing multiple functional and cognitive benefits. Implants helped to improve patient mobility and significantly reduce their metabolic consumption. A wearable, non-invasive alterative that provides similar useful health benefits, would eliminate the surgery related risks and costs thereby increasing the accessibility and the spreading of such neurotechnologies.Approach.Here, we present a non-invasive SF system exploiting an optimally-calibrated (just noticeable difference-based) electro-cutaneous stimulation to encode intensity-modulated foot-ground and knee angle information personalized to the user's just noticeable perceptual threshold. This device was holistically evaluated in three transfemoral amputees by examination of metabolic consumption while walking outdoors, walking over different inclinations on a treadmill indoors, and balance maintenance in reaction to unexpected perturbation on a treadmill indoors. We then collected spatio-temporal parameters (i.e. gait dynamic and kinematics), and self-reported prosthesis confidence while the patients were walking with and without the SF.Main results.This non-invasive SF system, encoding different distinctly perceived levels of tactile and knee flexion information, successfully enabled subjects to decrease metabolic consumption while walking and increase prosthesis confidence. Remarkably, more physiological walking strategies and increased stability in response to external perturbations were observed while walking with the SF.Significance.The health benefits observed with the use of this non-invasive device, previously only observed exploiting invasive technologies, takes an important step towards the development of a practical, non-invasive alternative to restoring SF in leg amputees.
Broadband Prosthetic Interfaces: Combining Nerve Transfers and Implantable Multichannel EMG Technology to Decode Spinal Motor Neuron Activity

Abstract:Modern robotic hands/upper limbs may replace multiple degrees of freedom of extremity function. However, their intuitive use requires a high number of control signals, which current man-machine interfaces do not provide. Here, we discuss a broadband control interface that combines targeted muscle reinnervation, implantable multichannel electromyographic sensors, and advanced decoding to address the increasing capabilities of modern robotic limbs. With targeted muscle reinnervation, nerves that have lost their targets due to an amputation are surgically transferred to residual stump muscles to increase the number of intuitive prosthetic control signals. This surgery re-establishes a nerve-muscle connection that is used for sensing nerve activity with myoelectric interfaces. Moreover, the nerve transfer determines neurophysiological effects, such as muscular hyper-reinnervation and cortical reafferentation that can be exploited by the myoelectric interface. Modern implantable multichannel EMG sensors provide signals from which it is possible to disentangle the behavior of single motor neurons. Recent studies have shown that the neural drive to muscles can be decoded from these signals and thereby the user's intention can be reliably estimated. By combining these concepts in chronic implants and embedded electronics, we believe that it is in principle possible to establish a broadband man-machine interface, with specific applications in prosthesis control. This perspective illustrates this concept, based on combining advanced surgical techniques with recording hardware and processing algorithms. Here we describe the scientific evidence for this concept, current state of investigations, challenges, and alternative approaches to improve current prosthetic interfaces.
A computational model to design neural interfaces for lower-limb sensory neuroprostheses

Abstract:Background: Leg amputees suffer the lack of sensory feedback from a prosthesis, which is connected to their low confidence during walking, falls and low mobility. Electrical peripheral nerve stimulation (ePNS) of upper-limb amputee's residual nerves has shown the ability to restore the sensations from the missing limb via intraneural (TIME) and epineural (FINE) neural interfaces. Physiologically plausible stimulation protocols targeting lower limb sciatic nerve hold promise to induce sensory feedback restoration that should facilitate close-to-natural sensorimotor integration and therefore walking corrections. The sciatic nerve, innervating the foot and lower leg, has very different dimensions in respect to upper-limb nerves. Therefore, there is a need to develop a computational model of its behavior in response to the ePNS. Methods: We employed a hybrid FEM-NEURON model framework for the development of anatomically correct sciatic nerve model. Based on histological images of two distinct sciatic nerve cross-sections, we reconstructed accurate FEM models for testing neural interfaces. Two different electrode types (based on TIME and FINE) with multiple active sites configurations were tested and evaluated for efficiency (selective recruitment of fascicles). We also investigated different policies of stimulation (monopolar and bipolar), as well as the optimal number of implants. Additionally, we optimized the existing simulation framework significantly reducing the computational load. Results: The main findings achieved through our modelling study include electrode manufacturing and surgical placement indications, together with beneficial stimulation policy of use. It results that TIME electrodes with 20 active sites are optimal for lower limb and the same number has been obtained for FINE electrodes. To interface the huge sciatic nerve, model indicates that 3 TIMEs is the optimal number of surgically implanted electrodes. Through the bipolar policy of stimulation, all studied configurations were gaining in the efficiency. Also, an indication for the optimized computation is given, which decreased the computation time by 80%. Conclusions: This computational model suggests the optimal interfaces to use in human subjects with lower limb amputation, their surgical placement and beneficial bipolar policy of stimulation. It will potentially enable the clinical translation of the sensory neuroprosthetics towards the lower limb applications.

Abstract:There have been several advancements in the field of myoelectric prostheses to improve dexterity and restore hand grasp patterns for persons with upper limb loss, including robust control strategies, novel sensory feedback, and multifunction prosthetic terminal devices. Although these advancements have shown to improve prosthesis performance, a key element that may further improve acceptance is often overlooked. Embodiment, which encompasses the feeling of owning, controlling and locating the device without the need to constantly look at it, has been shown to be affected by sensory feedback. However, the specific aspects of embodiment that are influenced are not clearly understood, particularly when a prosthesis is actively controlled. In this work, we used a sensorized simulated prosthesis in able-bodied participants to investigate the contribution of sensory feedback, active motor control, and the combination of both to the components of embodiment; using a common methodology in the literature, namely the rubber hand illusion (RHI). Our results indicate that (1) the sensorized simulated prosthesis may be embodied by able-bodied users in a similar fashion as prosthetic devices embodied by persons with upper limb amputation, and (2) mechanotactile sensory feedback might not only be useful for improving certain aspects of embodiment, i.e., ownership and location, but also may have a modulating effect on other aspects, namely sense of agency, when provided asynchronously during active motor control tasks. This work may allow us to further investigate and manipulate factors contributing to the complex phenomenon of embodiment in relation to active motor control of a device, enabling future study of more precise quantitative measures of embodiment that do not rely as much on subjective perception.

Abstract:The lower extremity exoskeleton, which can sense the neural motion state of the human body and then provide motion assistance, is gradually replacing the traditional wheelchairs and assistive devices, making many patients with disabilities or movement disorders able to regain the walking function. This survey provides a comprehensive review on recent technological advances in lower extremity neurorehabilitation exoskeleton from the perspectives of sensing, gait dynamics, and human–robot collaboration. For each technology category, a detailed comparison among state-of-the-art solutions is provided. The results show that the exoskeleton has been greatly improved in mechanical and learning ability. However, some issues, such as adaptability, safety, and efficiency still restrict the development of exoskeleton technology. To address these problems, the remaining open challenges and future directions to improve intelligence, sensing, gait analysis, trust, efficiency, generalization, and power consumption of exoskeleton are also presented and discussed.

Evaluation of experimental spinal cord injury using cortical evoked potentials.

Abstract:✓ Experiments were performed to determine if changes in cortical evoked responses could be used to predict the extent of the neurological deficits following spinal cord injury by sudden inflation of a Fogarty balloon in the epidural space cephalad to a laminectomy. The cortical responses to stimulation of the posterior tibial nerve were recorded over the sigmoid gyrus at various times following the lesion and compared with the control response. Severe, irreversible neurological deficits occurred in cats in which the cortical response either could not be evoked immediately after injury or disappeared rapidly during this period. At the end of at least 6 weeks following injury, all of these animals were paraplegic and showed severe cystic degeneration of the spinal cord. In animals in which the post-injury cortical response did not completely disappear, only mild changes were observed in a spinal cord 6 weeks following injury. Abstract:Introduction: Approximately 89% of all service members with amputations do not return to duty. Restoring intuitive neural control with somatosensory sensation is a key to improving the safety and efficacy of prosthetic locomotion. However, natural somatosensory feedback from lower-limb prostheses has not yet been incorporated into any commercial prostheses. Materials and methods: We developed a neuroprosthesis with intuitive bidirectional control and somatosensation and evoking phase-dependent locomotor reflexes, we aspire to significantly improve the prosthetic rehabilitation and long-term functional outcomes of U.S. amputees. We implanted the skin and bone integrated pylon with peripheral neural interface pylon into the cat distal tibia, electromyographic electrodes into the residual gastrocnemius muscle, and nerve cuff electrodes on the distal tibial and sciatic nerves. Results. The bidirectional neural interface that was developed was integrated into the existing passive Free-Flow Foot and Ankle prosthesis, WillowWood, Mount Sterling, OH. The Free-Flow Foot was chosen because it had the highest Index of Anthropomorphicity among lower-limb prostheses and was the first anthropomorphic prosthesis brought to market. Conclusion. The cats walked on a treadmill with no cutaneous feedback from the foot in the control condition and with their residual distal tibial nerve stimulated during the stance phase of walking.
