Abstract:Higher levels of C-reactive protein (CRP), an inflammatory marker, are associated with increased fracture risk, although previous studies on CRP and bone mineral density (BMD) have yielded conflicting results. We aimed to test the hypotheses that composite indices of femoral neck strength relative to load, which are inversely associated with fracture risk, would also be inversely associated with CRP, and would explain part of the association between CRP and fracture risk. We analyzed data from a multisite, multiethnic prospective cohort of 1872 community-dwelling women, premenopausal or early perimenopausal at baseline. Femoral neck composite strength indices in three failure modes were calculated using dual-energy X-ray absorptiometry (DXA)-derived femoral neck width (FNW), femoral neck axis length (FNAL), femoral neck BMD and body size at baseline, as BMD*FNW/weight for compression strength, BMD*(FNW)(2) /(FNAL*weight) for bending strength, and BMD*FNW*FNAL/(height*weight) for impact strength. Incident nondigital, noncraniofacial fractures were ascertained annually over a median follow-up of 9 years. In analyses adjusted for age, race/ethnicity, diabetes, menopause transition stage, body mass index, smoking, alcohol use, physical activity, medications, prior fracture, and study site, CRP was associated inversely with each composite strength index (0.035-0.041 SD decrement per doubling of CRP, all p < 0.001), but not associated with femoral neck or lumbar spine BMD. During the follow-up, 194 women (10.4%) had fractures. In Cox proportional hazards analyses, fracture hazard increased linearly with loge (CRP), only for CRP levels ≥ 3 mg/L. Addition of femoral neck or lumbar spine BMD to the model did not attenuate the CRP-fracture association. However, addition of any of the composite strength indices attenuated the CRP-fracture association and made it statistically nonsignificant. We conclude that fracture risk increases with increasing CRP, only above the threshold of 3 mg/L. Unlike BMD, composite strength indices are inversely related to CRP levels, and partially explain the increased fracture risk associated with inflammation.
Serum high sensitivity C-reactive protein poorly predicts bone mineral density: A NHANES 2017–2020 analysis

Abstract:A reliable, widely available method to detect osteoporosis prior to fracture is needed. Serum levels of C-reactive protein may independently predict low bone mineral density (BMD) and high fracture risk. Existing empirical data focus on sexually and/or racially homogenous populations. This study tests the hypotheses that: C-reactive protein (1) negatively correlates with BMD and (2) fracture history, and (3) independently predicts BMD and fracture history in a diverse population. NHANES 2017–2020 pre-pandemic cycle data were analyzed in R studio. Strength and direction of relationships (-1 to +1) between variables were determined using Kendall’s rank correlation coefficient (τ). Linear models were optimized to predict femoral neck or lumbar spine BMD. C-reactive protein positively correlated with femoral (τ = 0.09, p<0.0001) and spine BMD (τ = 0.10, p<0.0001). Individuals identifying as female demonstrated more robust, but still weak, correlations between C-reactive protein and femoral neck (τ = 0.15, p<0.0001; male, τ = 0.06, p = 0.051) and spine BMD (τ = 0.16, p<0.0001; male, τ = 0.06, p = 0.04). C-reactive protein positively correlated with fracture history (τ = 0.083, p = 0.0009). C-reactive protein significantly predicted femoral neck (R 2 = 0.022, p = 0.0001) and spine BMD (R 2 = 0.028, p<0.0001) and fracture history (R 2 = 0.015, p<0.0001). Exploratory analyses identified weight was the single best predictor for femoral neck (R 2 = 0.24, p<0.0001) and spine BMD (R 2 = 0.21, p<0.0001). In sum, C-reactive protein statistically correlates with and predicts femoral neck and spine BMD, but the magnitude is too low to be biologically meaningful. While weight is a more robust predictor, individuals who are overweight or obese account for nearly half of all osteoporotic fractures, limiting the predictive power of this variable at identifying individuals at risk for osteoporosis. Identification of a robust predictor of fracture risk in a diverse population and across of range of body weights and compositions is needed.

Changes in bone resorption across the menopause transition: effects of reproductive hormones, body size, and ethnicity

Abstract:Objective: Our objective was to characterize changes in bone resorption in relation to the final menstrual period (FMP), reproductive hormones, body mass index (BMI), and ethnicity. Methods: Urinary type I collagen N-telopeptide (NTX), estradiol, and FSH levels were measured annually for up to 8 years spanning the menopause transition in 918 African American, Chinese, Japanese, or Caucasian women. Results: Urinary NTX began to increase sharply about 2 years before the FMP, reaching its peak level about 1 to 1.5 years after the FMP. NTX levels declined modestly from 2 to 6 years after the FMP but remained about 20% higher than before the menopause transition. The sharp rise in FSH occurred in conjunction with a sharp decline in estradiol and shortly after FSH levels began increasing rapidly. The mean increase in urinary NTX across the menopause transition was greatest in women with BMI <25 kg/m² and smallest in women with BMI >30 kg/m². Increases in NTX were greatest in Japanese women and smallest in African Americans. These differences were attenuated, but not eliminated, when analyses were adjusted for covariates, particularly BMI. Summary: During the menopause transition, a decline in ovarian function beginning about 2 years before the FMP is followed by an increase in bone resorption and subsequently by bone loss. The magnitude of the increase in bone resorption is inversely associated with BMI. Ethnic differences in changes in bone resorption are attenuated, but not eliminated, by adjustment for BMI. Ethnic differences in BMI, and corresponding ethnic differences in bone resorption, appear to account for much of the ethnic variation in perimenopausal bone loss.
Systemic Inflammation Markers Associated with Bone Mineral Density in Perimenopausal and Postmenopausal Women

Abstract:Objective: The aim of this research was to determine whether systemic inflammatory indicators, including aggregate index of systemic inflammation (AISI), neutrophils lymphocyte to platelet ratio (NLPR), systemic immune-inflammation index (SII), and systemic inflammation response index (SIRI), are related to bone mineral density (BMD) in perimenopausal and postmenopausal women.Methods: One hundred and eighty-one perimenopausal and 390 postmenopausal women were enrolled in this cross-sectional study. Continuous variables by analysis of variance and Kruskal Wallis test for comparing the clinical characteristics. Linear regression analysis was conducted to investigate the associations between inflammatory indicators with BMD. The comparison between the subgroups was performed using the nonparametric test and the T-test.Results: AISI, NLPR, SII, and SIRI quartile values were inversely associated with BMD in menopausal women (P = 0.021; P = 0.047; P < 0.001; P < 0.001, respectively). After adjusting for confounding factors, four inflammatory indicators remained significantly associated with BMD (all P for trend <0.001). Analysis according to menopausal status demonstrated that AISI, SII, and SIRI were significantly correlated with mean femoral neck BMD in postmenopausal women (P for trend = 0.015, 0.004, and 0.001), but not significantly associated with BMD in perimenopausal women (P for trend = 0.248, 0.054, and 0.352) after adjustment for covariates. Conclusion: The quartile values of AISI, SII, and SIRI were inversely associated with BMD in postmenopausal women, following adjustment for individual variables, hormone profiles and glucolipid metabolism profiles. AISI, SII, and SIRI have potential to be important tools for screening and prevention of bone loss in menopausal women in future clinical practice.
Persistently Elevated C-Reactive Protein Levels and Low Body Mass Index Are Associated with a Lack of Improvement in Bone Mineral Density in Crohn's Disease

Abstract:Background: Osteoporosis prevalence is increased in Crohn's disease (CD). Its pathogenesis in these patients is incompletely understood. Objectives: To identify factors associated with decreased bone mineral density (BMD) status in CD patients on a time-line course. Methods: A retrospective study was performed that followed CD patients who underwent at least two bone mineral density scans (DEXAs). Follow-up began one year prior to the first DEXA test and lasted at least one year after a second test. Possible correlations between baseline and follow-up variables and changes in BMD status were examined. Change in BMD was defined as a transition from one bone density category to another (normal vs. osteopenia vs. osteoporosis). Binary variables were assessed using the Cochrane–Armitage test. Categorical variables were assessed using the chi-squared test. A multivariate analysis was performed. Results: The study included 141 patients. At baseline, 33 patients (23.4%) had normal BMD, 75 (53.2%) had osteopenia, and 33 (23.4%) had osteoporosis. Patients with low BMD had a lower baseline BMI compared to those with normal BMD (p < 0.0001). After a median follow-up of 48 months (IQR 29–71), BMD status worsened in 19 (13.5%) patients, whereas in 95 (67.3%) and 27 (19.1%) patients, BMD remained unchanged or improved, respectively. On the multivariate analysis, elevated median CRP throughout follow-up (OR = 0.8, 95% CI: 0.68–0.93) and low baseline BMI (OR = 0.9, 95% CI: 0.83–0.98) were associated with a lack of BMD status improvement. Conclusions: Persistently elevated CRP and low BMI are associated with a lack of improvement in BMD. These findings underscore the importance of effective inflammation control and nutritional support to maintain and improve bone health.

Abstract:Low bone mineral density (BMD) is an important risk factor for osteoporotic fractures. The impact of gynecological history on BMD is of great concern, but the results are largely inconsistent. In this study, we investigated the association of gynecological history with BMD in 214 postmenopausal women (60.4 +/- 5.7 yr), as well as with peak bone density (PBD) in 428 premenopausal women (30.8 +/- 5.3 yr) from Shanghai City in China. BMD was measured at lumbar spine (L1-4) and total hip by dual energy X-ray absorptiometry. Raw BMD values were adjusted by age, age2, height, and weight. In the postmenopausal group, more parity had significantly detrimental effects on BMD at both the spine and hip (p < 0.01). The age of the first delivery, the duration of lactation, and the age at menarche did not show significant impacts on BMD (p > 0.05). More years since menopause only had marginally significant decreasing effects at the spine (p = 0.09), but not at the hip (p > 0.10). In the premenopausal group, none of the three reproductive factors had significant impact on PBD (p > 0.05); the age of menarche inversely affected PBD at both the spine (p < 0.01) and hip (p < 0.05). Our results suggest that some gynecological events might influence BMD variation in healthy Chinese women.
Urinary N-telopeptide and Rate of Bone Loss Over the Menopause Transition and Early Postmenopause

Abstract:The purpose of this study was to assess the ability of urinary N-telopeptide (U-NTX) to gauge rate of bone loss across and after the menopause transition (MT). U-NTX measurement was measured in early postmenopause in 604 participants from the Study of Women's Health Across the Nation (SWAN). We examined the association between U-NTX and annualized rates of decline in lumbar spine and femoral neck bone mineral density (BMD) across the MT (1 year before the final menstrual period [FMP] to time of U-NTX measurement), after the MT (from time of U-NTX measurement to 2 to 4 years later), and over the combined period (from 1 year before FMP to 2 to 4 years after U-NTX measurement). Adjusted for covariates in multivariable linear regression, every standard deviation (SD) increase in U-NTX was associated with 0.6% and 0.4% per year faster declines in lumbar spine and femoral neck BMD across the MT; and 0.3% (lumbar spine) and 0.2% (femoral neck) per year faster declines over the combined period (across and after the MT) (all p < 0.01). Each SD increase in U-NTX was also associated with 44% and 50% greater risk of fast bone loss in the lumbar spine (defined as BMD decline in the fastest 16% of the distribution) across the MT (p < 0.001, c-statistic = 0.80) and over the combined period (across and after the MT) (p = 0.001, c-statistic = 0.80), respectively. U-NTX measured in early postmenopause is most strongly associated with rates of bone loss across the MT, and may aid early identification of women who have experienced fast bone loss during this critical period. © 2016 American Society for Bone and Mineral Research.

Abstract:Abstract Background Osteopenia and osteoporosis are associated with Crohn’s disease (CD). Diagnosis is made by dual-energy X-ray absorptiometry scan (DEXA). Pathophysiology of osteoporosis may reflect an interplay between persistent inflammation, steroid usage, as well as risk factors similar to general population. We aimed to identify factors associated with low or deteriorating bone mineral density (BMD) among patients. Methods A retrospective case-control study, included patients with CD followed at our IBD clinic from January 2000 to February 2023, with ≥ 2 DEXA scans. The observation started one year before the initial DEXA scan and extended until one year after the second scan. Demographic, clinical and laboratory data as well as medication exposure were collected before and during F/U. Patients were classified into three groups based on change in DEXA results: improved, worsened and no change. Multivariable proportional odds regression model was built to assess independent predictors of change in BMD. Results 121 patients [60 (49.6%) females, median age 33 years (IQR 23-43), median disease duration 5 years (IQR 0-35), median baseline BMI 22.4 kg/m2 (IQR 12.9-39.3), baseline CRP 1.4 ULN (IQR 0.5-4.5), baseline vitamin D 54.2 nmol/L (IQR 40.9-67.5)], were enrolled and followed for a median of 48 (IQR 29-71) months. At baseline, patients with osteoporosis had a lower BMI (18.5 kg/m2) than patients with normal BMD (26 kg/m2) and osteopenia (22.4 kg/m2), p<0.001. During F/U 63%, 59% and 66% of patients were exposed to high dose CS, anti-TNF and thiopurines respectively. At the end of F/U,19 (15.7%) patients had deterioration in BMD status while 78 (64.5%) remained unchanged, and in 24 (19.8%) BMD improved. Baseline vitamin D levels (in nmol/L) were higher in patients with improvement in BMD status at the end of the F/U compared to those with maintained or deterioration in BMD (66, 54.2, 41.2; p=0.001, Figure 1). Active smoking was associated with decline in BMD status (p=0.03). No correlation was found between different types of medication exposure and changes in BMD status. In multivariable analysis, elevated CRP was the only independent variable associated with lower odds of improvement in BMD (OR=0.45 (CI 0.2-1.0), p=0.05,Table 1). Conclusion Low BMI, low baseline vitamin D levels, and active smoking were found to be risk factors for low BMD status. Elevated CRP was associated with lack of improvement in BMD during the F/U. No correlation was found between different medications and changes in BMD status.

Abstract:This study aimed to evaluate the associations of baseline high-sensitivity C-reactive protein (Hs-CRP) and its change with subsequent cognitive decline and cognitive impairment. Data for this study were obtained from the China Health and Retirement Longitudinal Study, a national community-based prospective cohort study. Hs-CRP level and cognitive function were measured repeatedly over a 7-year follow-up. Linear mixed models and cox proportional hazard models were used to evaluate the associations. The study comprised 7385 participants (50.67% women, mean age 59.08 ± 8.86 years) with baseline Hs-CRP ranging from 0.03 to 178.10 mg/L (median: 1.01 mg/L, IQR: 0.55–2.11 mg/L). During a median of 5.79 years follow-up, the highest quartile of the Hs-CRP group showed a faster rate of cognitive decline (−0.0053 SD/year, p = 0.006) and a higher risk of cognitive impairment (HR 1.0814, p = 0.044) than those in the lowest quartile. Individuals in the elevated group of Hs-CRP change had a significantly faster cognitive decline (−0.0070 SD/year, p = 0.016) compared with those in the stable group. In this study, significant longitudinal associations between baseline Hs-CRP, elevated Hs-CRP, and long-term cognitive deterioration were observed. Hs-CRP level could perhaps serve as a predictor for cognitive deterioration in middle-aged and older adults.

Abstract:Background: Serum C-reactive protein (CRP) is a marker of general systemic inflammation. It has been suggested that inflammation may have a role in osteoporosis and subsequent fracture risk. We examined the prospective relationship between CRP and fracture risk in a large cohort study. Methods: The study populations were 18,586 men and women aged 40-79 enrolled between 1993 and 1997 in the European Prospective Investigation into Cancer in Norfolk study (EPIC-Norfolk), who had high sensitivity serum CRP measured in frozen serum from baseline. After 261,563 person-years of follow up (Median (inter-quartile range) follow-up time was 14.8 (13.6-16.1) years), 792 incident fractures (291 hip fractures) were ascertained by hospital record linkage. All multivariate regression models were adjusted for age, sex, body mass index, smoking, alcohol intake, physical activity, past history of fractures, history of osteoporosis, use of NSAIDs and corticosteroid medication, medical history of arthritis, cancer, myocardial infarction, stroke and diabetes, and in women only menopausal status and postmenopausal Hormone Replacement Therapy. Results: We found a U-shaped association for CRP and fracture risk. The lowest risk of fracture was observed for men and women with baseline CRP levels in the range of 1.1-2 mg/L in fractional polynomial Cox-proportional hazards regression analysis. Hazard ratios of all-fractures for participants with CRP levels 0.1-0.5; 0.6-1; 1.1-2; 2.1-3; 3.1-10; and above 10 mg/L were 1.31 (1.03, 1.66); 1.21 (0.98, 1.51); 1.00 (referent category); 1.06 (0.82, 1.35); 1.22 (1.00, 1.51); and 1.29 (0.91, 1.81) respectively. Although a similar U-shaped association was observed for hip fractures, the corresponding hazard ratios were not statistically significant. Conclusion: A U-shaped association was observed between CRP and fracture risk. The increased risk of fracture observed at lower end of CRP compared to intermediate levels require further exploration, confirmation in other populations, and investigation into potential biological mechanisms.

Abstract:The objective of this study was to describe the time of onset and offset of bone mineral density (BMD) loss relative to the date of the final menstrual period (FMP); the rate and amount of BMD decline during the 5 years before and the 5 years after the FMP; and the independent associations between age at FMP, body mass index (BMI), and race/ethnicity with rates of BMD loss during this time interval. The sample included 242 African American, 384 white, 117 Chinese, and 119 Japanese women, pre- or early perimenopausal at baseline, who had experienced their FMP and for whom an FMP date could be determined. Loess-smoothed curves showed that BMD loss began 1 year before the FMP and decelerated (but did not cease) 2 years after the FMP, at both the lumbar spine (LS) and femoral neck (FN) sites. Piecewise, linear, mixed-effects regression models demonstrated that during the 10-year observation period, at each bone site, the rates and cumulative amounts of bone loss were greatest from 1 year before through 2 years after the FMP, termed the transmenopause. Postmenopausal loss rates, those occurring between 2 and 5 years after the FMP, were less than those observed during transmenopause. Cumulative, 10-year LS BMD loss was 10.6%; 7.38% was lost during the transmenopause. Cumulative FN loss was 9.1%; 5.8% was lost during the transmenopause. Greater BMI and African American heritage were related to slower loss rates, whereas the opposite was true of Japanese and Chinese ancestry.

Identification of the Risk Factors Associated with Low Bone Density in Peri- and Early Postmenopausal Women

Abstract:Evidence has shown that one of the most prevalent chronic conditions in postmenopausal women is osteoporosis. Despite the development of some medications, there are still safety and adherence concerns, and, thus, attention has been placed on understanding modifiable risk factors for bone loss. This study aimed to examine the differences in various sociodemographic and body composition factors, physical activity components, and nutrient and food group intake levels among peri- and early postmenopausal women with whole-body bone mineral density (BMD) Z-scores greater than and less than zero. This cross-sectional study utilized baseline data obtained from 45 peri- and early postmenopausal women aged 45–60 years old who participated in a 6-month three-arm, randomized, double-blind, placebo-controlled clinical trial that evaluated the effects of blackcurrant supplementation on bone metabolism. Anthropometric data, BMD values obtained via DXA scans, and self-reported demographic, health, dietary, and physical activity data were collected at baseline. Overall, participants with whole-body BMD Z-scores greater than zero had higher body mass indexes (BMIs), lean mass, fat mass, android fat percentages, ratios of trunk fat mass to limb fat mass, resting metabolic rates, relative skeletal muscle indexes, total and occupational physical activity, alcohol intakes, trans fatty acid intakes, and adequacy of potassium intake, but lower adequacy of vitamin E (p < 0.05). In addition, total calorie intake was positively correlated with added sugar, vitamin E, potassium, alcohol, trans fatty acids, calcium, and vitamin D intakes (p < 0.05); whole-body BMD was positively correlated with added sugars (p < 0.05); vitamin D intake was positively correlated with vitamin E, potassium, trans fatty acid, and calcium intakes (p < 0.05); and calcium intake was positively correlated with added sugar, vitamin E, potassium, and trans fatty acid intakes (p < 0.05). These findings suggest that numerous body composition factors, components of physical activity, and dietary factors are related to bone health in adult women in the menopause transition.
(090) The Relationship Between C-Reactive Protein and Total Testosterone in Aging Men: Findings from the Baltimore Longitudinal Study of Aging

Abstract:Introduction Elevated C-Reactive Protein (CRP) and testosterone deficiency are associated with advanced age and chronic inflammatory diseases, such as cancer, diabetes, and cardiovascular disease. Testosterone decreases inflammation through several mechanisms, and testosterone replacement therapy (TRT) has been shown to lower CRP levels in aging, hypogonadal men. While cross-sectional studies have shown an inverse relationship between CRP and total testosterone (TT) levels, mixed findings have been reported when individual components of metabolic syndrome are considered. We hypothesized that men with testosterone deficiency will have high levels of CRP. Objective We evaluated the relationship between CRP and TT levels in men from 2004-2018 using the Baltimore Longitudinal Study of Aging and specifically attempted to determine if low androgen status is associated with a high inflammatory profile independent of comorbidities. Methods Male participants with serum TT level measured during two separate visits >3 years apart were included in our cohort. CRP, High-Density Lipoprotein (HDL) and Triglyceride levels were collected via blood specimens. Comorbidity data were documented at each visit. Panel regression was used to analyze the relationship of a series of independent variables collected in pooled cross-sectional observations over time with a dependent variable for modeling. Results A total of 347 patients were included in this study (mean age = 69 ± 15, average follow up time = 6.7 ± 3.2 years). Participants had a median CRP level of 1.0 mg/dL and a median TT level of 446 ng/dL. On univariable analysis TT and HDL levels were associated with a decline in CRP, while BMI, congestive heart failure (CHF), Diabetes, and Triglycerides were associated with increased CRP. Age was not associated with CRP. On multivariable analysis, we found that increasing TT level was associated with a decline in CRP levels, independent of comorbidities (p = 0.018; Table 1). BMI was associated with a significant increase in CRP (p = 0.001, Table 1). Age, CHF, Diabetes, HDL, and Triglycerides were not significant predictors of CRP on multivariable analysis. Conclusions Lower levels of serum total testosterone are associated with an increased CRP in older men. Given the importance of CRP in pathogenesis of chronic disease, we highlight the potential benefits of restoring testosterone levels in older, hypogonadal men via TRT. Disclosure No.

Quantifying the Balance Between Total Bone Formation and Total Bone Resorption: An Index of Net Bone Formation

Abstract:Context: Bone gain vs loss across the skeleton loss depends on the balance between total bone formation and total bone resorption. Objective: The objective of the study was to determine whether resorption and formation markers can be combined to gauge net bone formation across the skeleton. Design: The study included a cohort followed up across menopause transition (Study of Women's Health Across the Nation). Setting and participants: Community-dwelling women, 42-52 years old, premenopausal or early perimenopausal at baseline, participated in the study. Outcome: The study included the following measures: 1) bone balance index (BBI) created by estimating the relationship between resorption (urinary N-telopeptide) and formation (osteocalcin) markers when the total formation equals the total resorption in 685 women with stable bone mineral density (BMD) (>5 y before the final menstrual period [FMP]) and applying this relationship to measured bone turnover markers in 216 women beginning to lose bone (≤2 y from FMP); and 2) annualized percentage declines over the following 3-4 years in the lumbar spine (LS) and femoral neck (FN) BMD. Results: Adjusted for covariates, the BBI was greater (more favorable) in women with a greater body mass index (P = .03) and lower (less favorable) in women closer to the FMP (P = .007). Each SD decrement in BBI was associated with 0.27%/y faster LS BMD decline (P 0.04) and a 38% higher odds of faster-than-average loss of LS bone mass (P = .008, c-statistic 0.76). BBI was not associated with decline in FN BMD. Urinary N-telopeptide alone was not associated with either LS or FN BMD decline. Conclusions: An index that quantifies net bone formation vs resorption can be created from bone turnover markers and may help identify individuals at high risk for LS bone loss.

Abstract:Bone mineral density (BMD) changes during the life span, increasing rapidly during adolescence, plateauing in the third decade of life, and subsequently entering a phase of age-related decline. In women, menopause leads to accelerated bone loss and an increase in fracture risk. Between peak bone mass attainment and menopause, BMD is generally stable and the risk of fracture is typically low. This time period is marked by life events such as pregnancy and lactation, which transiently decrease BMD, yet their long-term effects on fracture risk are less certain. BMD may also be altered by exposure to medications that affect bone metabolism (e.g., contraceptives, glucocorticoids, antidiabetic medications, antiepileptic drugs). Although oral contraceptives are often believed to be neutral with regard to bone health, depot medroxyprogesterone acetate (DMPA) and gonadotropin-releasing hormone (GnRH) agonists have been associated with decreases in BMD. Development of newer medical therapies, principally GnRH antagonists (e.g., ASP1707, elagolix, linzagolix, relugolix), for treatment of endometriosis-associated pelvic pain and heavy menstrual bleeding due to uterine fibroids has renewed interest in the short- and long-term impacts of changes in BMD experienced by premenopausal women. It is important to understand how these drugs influence BMD and put the findings into context with regard to measurement variability and naturally occurring factors that influence bone health. This review summarizes what is known about the effects on bone health pregnancy, lactation, and use of DMPA, GnRH agonists, and GnRH antagonists in premenopausal women and potential consequences later in life. Abstract:Five years ago we reported results from a cross-sectional study of the effect of nutritional factors on calcium-regulating hormones and bone loss in perimenopausal women. We found an inverse correlation between serum 25-hydroxyvitamin D (25OHD) and immunoreactive parathyroid hormone (PTH), and we postulated that over time, women with lower 25OHD would lose more bone because of increased bone remodeling induced by secondary hyperparathyroidism. We have followed 38 of these women for 5 years. Twenty-two have gone through menopause and we are reporting observations on these 22 subjects. Bone mineral analysis was performed twice a year at the distal and mid-radius using single-photon absorptiometry. The slope of the bone mineral content curve was calculated by least squares. Bone loss increased within 6 months of the rise in serum follicle stimulating hormone (FSH) to greater than 40 mIU/ml. We continued to see a negative correlation between 25OHD and PTH (r = -0.450, P = 0.03). Premenopause, PTH was negatively correlated with the proximal bone mineral content (PBMC) slope (-0.604, P = 0.002). The distal bone mineral content (DBMC) 5-year slope was correlated with dietary vitamin D (r = 0.509, P = 0.02), the higher the intake, the less negative the slope. The 5-year PBMC slope was negatively correlated with serum osteocalcin (OC) levels (r = -0.382, P = 0.08). Before menopause, the change in PBMC was positively correlated with OC (r = 0.450, P = 0.03). Age-related Changes of Serum Bone Alkaline Phosphatase and Cross-Linked C-telopeptides of Type I Collagen and the Relationship with Bone Mineral Density in Chinese Women

Abstract:Background: Previous studies have shown that bone turnover rate changes with age. At the same time, there is no definitive research regarding age-related changes of bone turnover level and its association with bone mineral density (BMD) in Chinese mainland women.Methods: In a cohort of 663 Chinese mainland women aged 20-70 years, serum bone alkaline phosphatase (BAP) and serum cross-linked C-telopeptides of type I collagen (sCTX) were measured to evaluate the state of bone fort-nation and resorption, respectively. BMD was measured in the posteroanterior spine, supine lateral spine, hip and forearm using a dual-energy X-ray absorptiometry.Results: The cubic polynomial regression model best fit age-related changes in serum BAP (R-2 = 0.398 p < 0.001) and sCTX concentrations (R-2 = 0. 148,p < 0.001) with largest R-2 from comparison 8 different regression models. Their values reached a minimal level in the 30-39 years age group, and increased dramatically in the 40-59 years groups. There was a decreasing trend of BAP in women > 60 years. The levels of BAP and sCTX were inversely correlated to BMD in various skeletal regions over the entire Population (r = -0.096 to -0.357, p < 0.05). sCTX was a significant predictor of a T-score <= -2.5 of BMD in postmenopausal women with sCTX levels above mean + 2 SD of women aged 30-39 years compared with other postmenopausal women, which indicated by odds ratios 1.9-3.7 (p < 0.05) for various skeletal regions, especially for the lateral lumbar spine (2.2, p < 0.01), Ward's triangle (3.7, p < 0.01), and ultradistal end of radius+ulna (2.8, p < 0.001).Conclusions: Age-dependent serum BAP and sCTX were inversely correlated to BMD, and sCTX was a useful parameter for the prediction of a low T-score of BMD at skeletal sites with abundant cancellous bone in postmenopausal women. Abstract:Objective: The aim of this study was to determine the prevalence of hypertension in midlife women, characterize the association between high-sensitivity C-reactive protein (hs-CRP) and hypertension in women, and describe differences in hypertension prevalence by menopausal stage. Methods: We included 1,625 women, aged 43 to 55 years, with measurements of hs-CRP and detailed reproductive histories in the Coronary Artery Risk Development in Young Adults study at follow-up year 25. Prevalent hypertension was defined as a systolic blood pressure of 140 mm Hg, or diastolic blood pressure of 90 mm Hg or greater, or use of antihypertensive medications. Logistic regression was used for analysis. Results: The prevalence of hypertension was 25.8%, 37.8%, and 39.0% in premenopausal, perimenopausal, and postmenopausal women, respectively. The median (25th and 75th percentiles) of hs-CRP was 3.08 (1.12, 7.98) μg/mL and 1.18 (0.48, 3.15) μg/mL in women with and without hypertension, respectively. After adjusting for confounders, metabolic factors and body mass index, a doubling (100% increment) in hs-CRP levels was significantly associated with hypertension in premenopausal (1.27 [1.01-1.59]), but not in perimenopausal (1.12 [0.99-1.27]) or postmenopausal (1.09 [0.95-1.26]) women. Conclusions: Hypertension was common in midlife women. The association of hs-CRP and hypertension was consistent across menopausal stages. The association of hs-CRP with hypertension was independent of body mass index in premenopausal but not in perimenopausal or postmenopausal women.
