Abstract:Frequency-domain diffuse optical spectroscopy (FD-DOS) is an established technique capable of determining optical properties and chromophore concentrations in biological tissue. Most FD-DOS systems use either manually positioned, handheld probes or complex arrays of source and detector fibers to acquire data from many tissue locations, allowing for the generation of 2D or 3D maps of tissue. Here, we present a new method to rapidly acquire a wide range of source-detector (SD) separations by mechanically scanning a single SD pair. The source and detector fibers are mounted on a scan head that traces a hypotrochoidal pattern over the sample that, when coupled with a high-speed FD-DOS system, enables the rapid collection of dozens of SD separations for depth-resolved imaging. We demonstrate that this system has an average error of 4±2.6% in absorption and 2±1.8% in scattering across all SD separations. Additionally, by linearly translating the device, the size and location of an absorbing inhomogeneity can be determined through the generation of B-scan images in a manner conceptually analogous to ultrasound imaging. This work demonstrates the potential of single optode diffuse optical scanning for depth resolved visualization of heterogeneous biological tissues at near real-time rates.
Monocular 3D Probe Tracking for Generating Sub-Surface Optical Property Maps From Diffuse Optical Spectroscopic Imaging

Abstract:Objective: Diffuse optical spectroscopic imaging (DOSI) is a promising biophotonic technology for clinical tissue assessment, but is currently hampered by difficult wide area assessment. A co-integrative optical imaging system is proposed for dense sub-surface optical property spatial assessment. Methods: The proposed system fuses a co-aligned set of camera frames and diffuse optical spectroscopy measurements to generate spatial sub-surface optical property maps. A 3D rigid body motion estimation model was developed by fitting automatically detected target features to an a priori geometric model using a single overhead camera. Point-wise optical properties were measured across the tissue using frequency domain photon migration DOSI. The 3D probe trajectory and temporal optical property data were fused to generate 2D spatial optical property maps, which were projected onto the tissue image using pre-calibrated camera parameters. Results: The system demonstrated sub-millimeter positional accuracy (error 0.24 ± 0.35 mm) across different probe speeds (1.0-3.8 cm/s), and displacement accuracy in overhead ([Formula: see text] mm) and tilted (0.51 ± 0.51 mm) camera orientations. Unstructured scans on a tumor inclusion phantom showed strong contrast under different probe paths, and significant ( ) changes in optical properties in an in vivo leg cuff occlusion protocol with spatial anatomy localization. Conclusion: The proposed co-integrative optical imaging system generated dense sub-surface optical property distributions across wide tissue areas with sub-millimeter accuracy at different probe speeds and trajectories, and does not require pre-planned probe route for tissue assessment. Significance: This system provides a valuable tool for real-time non-invasive tissue health and cancer screening, and enables longitudinal disease progression assessment through unstructured probe-based optical tissue assessment.
Research on the Best Measurement Situation Between Optical Probe and Tissue Surfaces in Non-Invasive Detection.

Abstract:Near-infrared spectroscopy is often used for the non-invasive detection of composition in the human body, such as that of blood glucose and haemoglobin, due to its high penetration depth into tissues. Although it is feasible to position the optical probe precisely, contact situation between probe and human tissues is a difficult problem to determine because of physiological tremor and mechanical performance of bio-soft tissue. Here, we proposed a novel estimation method for the situation between the optical probe and tissue surfaces based on the dynamic auto-correlation matrix of two-dimensional correlation spectroscopy (2DCOS) and radar chart. The diffuse reflectance spectra from the left palm of 4 healthy volunteers were collected while the optical probe gradually approached and pressed bio-tissues with a custom-design controlling device. 2DCOS in the wavelength with lower absorption (1000-1400 nm) was calculated under the perturbation of relative-distance and contact pressure between the optical probe and tissue surface. The synchronous 2DCOS showed that the surface reflection and diffuse reflectance were greatly affected by the contact conditions in 1100 nm, 1220 nm, and 1300 nm. Then the dynamic auto-correlation matrix of 2DCOS was established for the adjacent spectra, and the significant difference wavelengths were used to build radar charts to determine the critical contact situation visually. Results showed that the maximum variations of dynamic auto-correlation matrix appeared at near 1300 nm, and the relative distance between the probe and tissue corresponding to the critical contact state can be easily observed with radar charts with 0.25 mm uncertainty, which was consistent with the self-feeling of each volunteer. So this method can be applied to exactly determine the optimal measurement status for the non-invasive body composition detection in vivo. It is important for the design of human-machine interface and the accuracy improvement of body composition measurements.

Abstract:Frequency domain optical spectroscopy in the diffusive regime is currently being investigated for biomedical applications including tumor detection, therapy monitoring, exercise metabolism, and others. Analog homodyne or heterodyne detection of sinusoidally modulated signals have been the predominant method for measuring phase and amplitude of photon density waves that have traversed through tissue. Here we demonstrate the feasibility of utilizing direct digital sampling of modulated signals using a 3.6 Gigasample/second 12 bit Analog to Digital Converter. Digitally synthesized modulated signals between 50MHz and 400MHz were measured on tissue simulating phantoms at six near-infrared wavelengths. An amplitude and phase precision of 1% and 0.6 degrees were achieved during drift tests. Amplitude, phase, scattering and absorption values were compared with a well-characterized network analyzer based diffuse optical device. Measured optical properties measured with both systems were within 3.6% for absorption and 2.8% for scattering over a range of biologically relevant values. Direct digital sampling represents a viable method for frequency domain diffuse optical spectroscopy and has the potential to reduce system complexity, size, and cost.

Abstract:Diffuse optical spectroscopy is a relatively new, noninvasive and nonionizing technique for breast cancer diagnosis. In the present study, we have introduced a novel handheld diffuse optical breast scan (DOB-Scan) probe to measure optical properties of the breast in vivo and create functional and compositional images of the tissue. In addition, the probe gives more information about breast tissue's constituents, which helps distinguish a healthy and cancerous tissue. Two symmetrical light sources, each including four different wavelengths, are used to illuminate the breast tissue. A high-resolution linear array detector measures the intensity of the back-scattered photons at different radial destinations from the illumination sources on the surface of the breast tissue, and a unique image reconstruction algorithm is used to create four cross-sectional images for four different wavelengths. Different from fiber optic-based illumination techniques, the proposed method in this paper integrates multi-wavelength light-emitting diodes to act as pencil beam sources into a scattering medium like breast tissue. This unique design and its compact structure reduce the complexity, size and cost of a potential probe. Although the introduced technique miniaturizes the probe, this study points to the reliability of this technique in the phantom study and clinical breast imaging. We have received ethical approval to test the DOB-Scan probe on patients and we are currently testing the DOB-Scan probe on subjects who are diagnosed with breast cancer.

Abstract:We present a framework for characterizing the performance of an experimental imaging technology, diffuse optical spectroscopic imaging (DOSI), in a 2-year multicenter American College of Radiology Imaging Network (ACRIN) breast cancer study (ACRIN-6691). DOSI instruments combine broadband frequency-domain photon migration with time-independent near-infrared (650 to 1000 nm) spectroscopy to measure tissue absorption and reduced scattering spectra and tissue hemoglobin, water, and lipid composition. The goal of ACRIN-6691 was to test the effectiveness of optically derived imaging endpoints in predicting the final pathologic response of neoadjuvant chemotherapy (NAC). Sixty patients were enrolled over a 2-year period at participating sites and received multiple DOSI scans prior to and during 3- to 6-month NAC. The impact of three sources of error on accuracy and precision, including different operators, instruments, and calibration standards, was evaluated using a broadband reflectance standard and two different solid tissue-simulating optical phantoms. Instruments showed <0.0010 mm−1 (10.3%) and 0.06 mm−1 (4.7%) deviation in broadband absorption and reduced scattering, respectively, over the 2-year duration of ACRIN-6691. These variations establish a useful performance criterion for assessing instrument stability. The proposed procedures and tests are not limited to DOSI; rather, they are intended to provide methods to characterize performance of any instrument used in translational optical imaging.

Abstract:Frequency-domain near-infrared spectroscopy (FD-NIRS) provides quantitative noninvasive measurements of tissue optical absorption and scattering, as well as a safe and accurate method for characterizing tissue composition and metabolism. However, the poor scalability and high complexity of most FD-NIRS systems assembled to date have contributed to its limited clinical impact. To address these shortcomings, we present a scalable, digital-based FD-NIRS platform capable of measuring optical properties and tissue chromophore concentrations in real-time. The system provides single-channel FD-NIRS amplitude/phase, optical property, and chromophore data at a maximum display rate of 36.6 kHz, 17.9 kHz, and 10.2 kHz, respectively, and can be scaled to multiple channels as well as integrated into a handheld format. The entire system is enabled by several innovations including an ultra-high-speed k-nearest neighbor lookup table method (maximum of 250,000 inversions/s for a large 2500x700 table of absorption and reduced scattering coefficients), embedded FPGA and CPU high-speed co-processing, and high-speed data transfer (due to on-board processing). We show that our 6-wavelength, broad modulation bandwidth (1-400 MHz) system can be used to perform 2D high-density spatial mapping of optical properties and high speed quantification of hemodynamics.

Abstract:Diffuse optical spectroscopic imaging (DOSI) and diffuse correlation spectroscopy (DCS) are model-based near-infrared (NIR) methods that measure tissue optical properties (broadband absorption, ? a , and reduced scattering, ? s ? ) and blood flow (blood flow index, BFI), respectively. Abstract:The application of optical techniques as tools for biomedical research has generated substantial interest for the ability of such methodologies to simultaneously measure biochemical and morphological parameters of tissue. Ongoing optimization of optical techniques may introduce such tools as alternative or complementary to conventional methodologies. The common approach shared by current optical techniques lies in the independent acquisition of tissue’s optical properties (i.e., absorption and reduced scattering coefficients) from reflected or transmitted light. Such optical parameters, in turn, provide detailed information regarding both the concentrations of clinically relevant chromophores and macroscopic structural variations in tissue. We couple a noncontact optical setup with a simple analysis algorithm to obtain absorption and scattering coefficients of biological samples under test. Technically, a portable picoprojector projects serial sinusoidal patterns at low and high spatial frequencies, while a spectrometer and two independent CCD cameras simultaneously acquire the reflected diffuse light through a single spectrometer and two separate CCD cameras having different bandpass filters at nonisosbestic and isosbestic wavelengths in front of each. This configuration fills the gaps in each other’s capabilities for acquiring optical properties of tissue at high spectral and spatial resolution. Experiments were performed on both tissue-mimicking phantoms as well as hands of healthy human volunteers to quantify their optical properties as proof of concept for the present technique. In a separate experiment, we derived the optical properties of the hand skin from the measured diffuse reflectance, based on a recently developed camera model. Additionally, oxygen saturation levels of tissue measured by the system were found to agree well with reference values. Taken together, the present results demonstrate the potential of this integrated setup for diagnostic and research applications.

Abstract:Diffuse optical imaging (DOI) is a label-free, safe, inexpensive, and quantitative imaging modality that provides metabolic and molecular contrast in tissue using visible or near-infrared light. DOI modalities can image up to several centimeters deep in tissue, providing access to a wide range of human tissues and organ sites. DOI technologies have benefitted from several decades of academic research, which has provided a variety of platforms that prioritize imaging depth, resolution, field-of-view, spectral content, and other application-specific criteria. Until recently, however, acquisition and processing speeds have represented a stubborn barrier to further clinical exploration and implementation. Over the last several years, advances in high-speed data acquisition enabled by high-speed digital electronics, newly available sources and detectors, and innovative new scanning methods have led to major improvements in DOI rates. These advances are now being coupled with new data processing algorithms that utilize deep learning and other computationally efficient methods to provide rapid or real-time feedback in the clinic. Together, these improvements have the potential to help advance DOI technologies to the point where major impacts can be made in clinical care. Here, we review recent advances in acquisition and processing speed for several important DOI modalities.

Abstract:SignificanceFrequency-domain diffuse optical tomography (FD-DOT) could enhance clinical breast tumor characterization. However, conventional diffuse optical tomography (DOT) image reconstruction algorithms require case-by-case expert tuning and are too computationally intensive to provide feedback during a scan. Deep learning (DL) algorithms front-load computational and tuning costs, enabling high-speed, high-fidelity FD-DOT.AimWe aim to demonstrate a simultaneous reconstruction of three-dimensional absorption and reduced scattering coefficients using DL-FD-DOT, with a view toward real-time imaging with a handheld probe.ApproachA DL model was trained to solve the DOT inverse problem using a realistically simulated FD-DOT dataset emulating a handheld probe for human breast imaging and tested using both synthetic and experimental data.ResultsOver a test set of 300 simulated tissue phantoms for absorption and scattering reconstructions, the DL-DOT model reduced the root mean square error by 12%±40% and 23%±40%, increased the spatial similarity by 17%±17% and 9%±15%, increased the anomaly contrast accuracy by 9%±9% (μa), and reduced the crosstalk by 5%±18% and 7%±11%, respectively, compared with model-based tomography. The average reconstruction time was reduced from 3.8 min to 0.02 s for a single reconstruction. The model was successfully verified using two tumor-emulating optical phantoms.ConclusionsThere is clinical potential for real-time functional imaging of human breast tissue using DL and FD-DOT.

Abstract:Diffuse optical imaging (DOI) is a noninvasive optical technique that employs near-infrared (NIR) light to quantitatively characterize the optical properties of thick tissues. Although NIR methods were first applied to breast transillumination (also called diaphanography) nearly 80 years ago, quantitative DOI methods employing time- or frequency-domain photon migration technologies have only recently been used for breast imaging (i.e., since the mid-1990s). In this review, the state of the art in DOI for breast cancer is outlined and a multi-institutional Network for Translational Research in Optical Imaging (NTROI) is described, which has been formed by the National Cancer Institute to advance diffuse optical spectroscopy and imaging (DOSI) for the purpose of improving breast cancer detection and clinical management. DOSI employs broadband technology both in near-infrared spectral and temporal signal domains in order to separate absorption from scattering and quantify uptake of multiple molecular probes based on absorption or fluorescence contrast. Additional dimensionality in the data is provided by integrating and co-registering the functional information of DOSI with x-ray mammography and magnetic resonance imaging (MRI), which provide structural information or vascular flow information, respectively. Factors affecting DOSI performance, such as intrinsic and extrinsic contrast mechanisms, quantitation of biochemical components, image formation/visualization, and multimodality co-registration are under investigation in the ongoing research NTROI sites. One of the goals is to develop standardized DOSI platforms that can be used as stand-alone devices or in conjunction with MRI, mammography, or ultrasound. This broad-based, multidisciplinary effort is expected to provide new insight regarding the origins of breast disease and practical approaches for addressing several key challenges in breast cancer, including: Detecting disease in mammographically dense tissue, distinguishing between malignant and benign lesions, and understanding the impact of neoadjuvant chemotherapies.

Abstract:Optical techniques for tissue diagnostics currently are experiencing tremendous growth in biomedical applications, mainly due to their noninvasive, inexpensive, and real-time functionality. Here, we demonstrate a hand-held fiber optic probe instrument based on fluorescence/reflectance spectroscopy for precise tumor delineation. It is mainly aimed for brain tumor resection guidance with clinical adaptation to minimize the disruption of the standard surgical workflow and is meant as a complement to the state-of-the-art fluorescence surgical microscopy technique. Multiple light sources with fast pulse modulation and detection enable precise quantification of protoporphyrin IX (PpIX), tissue optical properties, and ambient light suppression. Laboratory measurements show the system is insensitive to strong ambient light. Validation measurements of tissue phantoms using nonlinear least squares support vector machines (LS-SVM) regression analysis demonstrate an error of <5% for PpIX concentration ranging from 400 to 1000 nM, even in the presence of large variations in phantom optical properties. The mean error is 3% for reduced scattering coefficient and 5% for blood concentration. Diagnostic precision of 100% was obtained by LS-SVM classification for in vivo skin tumors with topically applied 5-aminolevulinic acid during photodynamic therapy. The probe could easily be generalized to other tissue types and fluorophores for therapy guidance and monitoring.
Ultra-miniature dual-wavelength spatial frequency domain imaging for micro-endoscopy

Abstract:There is a need for a cost-effective, quantitative imaging tool that can be deployed endoscopically to better detect early stage gastrointestinal cancers. Spatial frequency domain imaging (SFDI) is a low-cost imaging technique that produces near-real time, quantitative maps of absorption and reduced scattering coefficients, but most implementations are bulky and suitable only for use outside the body. We present an ultra-miniature SFDI system comprised of an optical fiber array (diameter $0.125$ mm) and a micro camera ($1\times1$ mm package) displacing conventionally bulky components, in particular the projector. The prototype has outer diameter $3$ mm, but the individual components dimensions could permit future packaging to $<1.5$ mm diameter. We develop a phase-tracking algorithm to rapidly extract images with fringe projections at $3$ equispaced phase shifts in order to perform SFDI demodulation. To validate performance, we first demonstrate comparable recovery of quantitative optical properties between our ultra-miniature system and a conventional bench-top SFDI system with agreement of $15$\% and $6$\% for absorption and reduced scattering respectively. Next, we demonstrate imaging of absorption and reduced scattering of tissue-mimicking phantoms providing enhanced contrast between simulated tissue types (healthy and tumour), done simultaneously at wavelengths of $515$ nm and $660$ nm. This device shows promise as a cost-effective, quantitative imaging tool to detect variations in optical absorption and scattering as indicators of cancer.

Abstract:The authors describe the development of diffuse optical imaging (DOI) technologies, specifically the use of spatial and temporal modulation to control near infrared light propagation in thick tissues. We present theory and methods of DOI focusing on model-based techniques for quantitative, in vivo measurements of endogenous tissue absorption and scattering properties. We specifically emphasize the common conceptual framework of the scalar photon density wave for both temporal and spatial frequency-domain approaches. After presenting the history, theoretical foundation, and instrumentation related to these methods, we provide a brief review of clinical and preclinical applications from our research as well as our outlook on the future of DOI technology.

Abstract:We present the concept, design and first in vivo tests of a novel non-contact scanning imaging system for time-domain near-infrared spectroscopy of tissues. Employing a supercontinuum laser in combination with an acousto-optic tunable filter as light source, the tissue was scanned by a galvanometer scanner from a distance of more than 10 cm. The distance between the illumination spot (source) and the detection spot from which the diffusely remitted photons were collected was small (few mm) and kept fixed during the scan. A fast-gated single-photon avalanche diode was employed to eliminate the intense early part of the diffusely remitted signal and to detect late photons only. Polarization-selective detection was additionally applied to suppress specular reflections from the object. An array of gated time-of-flight distributions of photons was recorded by imaging TCSPC synchronized with the movement of the galvanometer scanner. A tissue area of several cm2 was scanned with 32 × 32 pixels within a frame rate of 1 s−1. The wavelength was switched line by line between two bands centred at 760 and 860 nm. Concentration changes of oxy- and deoxy-haemoglobin were derived from changes in photon counts in a selected time window of the gated distributions at the two wavelengths. First in vivo tests included the recording of haemodynamics during arm occlusion as well as brain activation tasks. These tests demonstrated the successful non-contact imaging of haemoglobin concentration changes in deeper tissues. Additional applications seem feasible by increasing the spectral information content of the non-contact scanning approach. To this end we implemented and tested the non-contact scanning in combination with eight-wavelength multiplexing.

Abstract:There is an essential need to develop wearable multimodality technologies that can continuously measure both blood flow and oxygenation in deep tissues to investigate and manage various vascular/cellular diseases. To develop a wearable dual-wavelength diffuse speckle contrast flow oximetry (DSCFO) for simultaneous measurements of blood flow and oxygenation variations in deep tissues. A wearable fiber-free DSCFO probe was fabricated using 3D printing to confine two small near-infrared laser diodes and a tiny CMOS camera in positions for DSCFO measurements. The spatial diffuse speckle contrast and light intensity measurements at the two different wavelengths enable quantification of tissue blood flow and oxygenation, respectively. The DSCFO was first calibrated using tissue phantoms and then tested in adult forearms during artery cuff occlusion. Phantom tests determined the largest effective source-detector distance (15 mm) and optimal camera exposure time (10 ms) and verified the accuracy of DSCFO in measuring absorption coefficient variations. The DSCFO detected substantial changes in forearm blood flow and oxygenation resulting from the artery occlusion, which meet physiological expectations and are consistent with previous study results. The wearable DSCFO may be used for continuous and simultaneous monitoring of blood flow and oxygenation variations in freely behaving subjects.

Quantifying tissue properties and absolute hemodynamics using coherent spatial imaging

Abstract:Significance: Measuring hemodynamic function is crucial for health assessment. Optical signals provide relative hemoglobin concentration changes, but absolute measurements require costly, bulky technology. Speckleplethysmography (SPG) uses coherent light to detect speckle fluctuations. Combining SPG with multispectral measurements may provide important physiological information on blood flow and absolute hemoglobin concentration. Aim: To develop an affordable optical technology to measure tissue absorption, scattering, hemoglobin concentrations, tissue oxygen saturation (StO2), and blood flow. Approach: We integrated reflectance spectroscopy and laser speckle imaging to create coherent spatial imaging (CSI). CSI was validated against spatial frequency domain imaging (SFDI) using phantom-based measurements. In vivo arterial and venous occlusion experiments compared CSI with diffuse optical spectroscopy/diffuse correlation spectroscopy (DOS/DCS) measurements. Results: CSI and SFDI agreed on tissue absorption and scattering in phantom tests. CSI and DOS/DCS showed similar trends and agreement in arterial occlusion results. During venous occlusion, both uncorrected and corrected blood flow decreased with increasing pressure, with an ∼200% difference in overall blood flow decrease between the methods. CSI and DOS/DCS data showed expected hemoglobin concentrations, StO2, and blood flow trends. Conclusions: CSI provides affordable and comprehensive hemodynamic information. It can potentially detect dysfunction and improve measurements, such as blood pressure, SpO2, and metabolism.
Portable System for Time-Domain Diffuse Correlation Spectroscopy

Abstract:We introduce a portable system for clinical studies based on time-domain diffuse correlation spectroscopy (DCS). After evaluating different lasers and detectors, the final system is based on a pulsed laser with about 550 ps pulsewidth, a coherence length of 38 mm, and two types of single-photon avalanche diodes (SPAD). The higher efficiency of the red-enhanced SPAD maximizes detection of the collected light, increasing the signal-to-noise ratio, while the better timing response of the CMOS SPAD optimizes the selection of late photons and increases spatial resolution. We discuss component selection and performance, and we present a full characterization of the system, measurement stability, a phantom-based validation study, and preliminary in vivo results collected from the forearms and the foreheads of four healthy subjects. With this system, we are able to resolve blood flow changes 1 cm below the skin surface with improved depth sensitivity and spatial resolution with respect to continuous wave DCS.

Abstract:In diffuse optical tomography (DOT) and spectroscopy (DOS) using handheld probes, tissue curvature can cause bad fiber-to-tissue contact. Understanding and minimizing image artifacts caused by these coupling errors would significantly improve DOT and DOS image quality. In this work, we utilized Monte Carlo simulations and experiments with gelatin-Intralipid phantoms to systematically study the influence of source or detector (optode) coupling errors. Optode coupling errors can increase the amplitude and decrease the phase of the measured diffuse reflectance, creating artifacts in the reconstructed absorption maps, such as hot spots on the edges. We propose an outlier removal algorithm that can correct these image artifacts, and we demonstrate its performance using simulations, phantom experiments, and breast patient data acquired with bad probe contact due to a dense or small breast. Further, we designed and implemented a new resistance-type thin-film force sensor array that provides real-time optode coupling feedback and guides the outlier removal to minimize optode coupling errors. Our approaches and study results have significant implications for reducing image artifacts arising from handheld probes, which are commonly used with mobile and wearable DOT and DOS devices.
