Abstract:Objective:To evaluate the clinical value of Auto Cyte PREP Thin-layer Cytology Test for cervical carcinoma screening.Methods:This study was performed on 2 198 women with cervical diseases consulting in the Department of Obstetrics and Gynecology of the Union Hospital in Wuhan,Hubei province.Cervical samples were collected into a liquid buffer and utilized for Auto Cyte PREP cytological screening.Those whose samples were classified as ASCUS or above were instructed to do subsequent colposcopic biopsies.Results:The specimen adequacy of the Auto Cyte PREP slides was good.The sensitivity,specificity and accuracy of the overall detection were 81.6%,91.4% and 87.7% respectively.The sensitivity of SCC,HSIL and LSIL was 100%,85.9% and 77.9% respectively.The grades of cytology agreed to those of histology roughly.Conclusion:The Auto Cyte PREP cytology improves the quality of preparation greatly and demonstrates a high accuracy for the detection of abnormal squamous intraepithelial lesions,especially for high-grade squamous intraepithelial lesions.
Artificial Classification of Cervical Squamous Lesions in ThinPrep Cytologic Tests Using a Deep Convolutional Neural Network.

Abstract:The diagnosis of squamous cell carcinoma requires the accurate classification of cervical squamous lesions in the ThinPrep cytologic test (TCT). It primarily relies on a pathologist's interpretation under a microscope. Deep convolutional neural networks (DCNN) have played an increasingly important role in digital pathology. However, they have not been applied to diverse datasets and externally validated. In the present study, a DCNN model based on VGG16 and an ensemble training strategy (ETS) based on 5-fold cross-validation was employed to automatically classify normal and abnormal cervical squamous cells from a multi-center dataset. First, we collected a dataset comprising 82 TCT samples from four hospitals and fine-tuned our model twice on the dataset with and without the ETS. Then, we compared the classifications obtained from the models with those provided by two skilled pathologists to discriminate the performance of the models in terms of classification accuracy and efficiency. Finally, paired sample t-tests were used to validate the consistency between the classification provided by the proposed methods and that of the pathologists. The results showed that ETS slightly, though not significantly, improved the classification accuracy compared with that of the pathologists: P0=0.387>0.05 (DCNN without ETS vs. DCNN with ETS), P1=0.771>0.05 (DCNN with ETS vs. pathologist 1), P2=0.489>0.05 (DCNN with ETS vs. pathologist 2). The DCNN model was almost 6-fold faster than that of the pathologists. The accuracy of our automated scheme was similar to that of the pathologists, but a higher efficiency in the accurate identification of cervical squamous lesions was provided by the scheme. This result allows for wider and more efficient screening and may provide a replacement for pathologists in the future. Future research should address the viability of the practical implementation of such DCNN models in the laboratory setting.
Improving the Accuracy and Efficiency of Abnormal Cervical Squamous Cell Detection with Cytologist-in-the-Loop Artificial Intelligence

Abstract:Population-based cervical cytology screening techniques are demanding and laborious and have relatively poor diagnostic accuracy. In this study, we present a cytologist-in-the-loop artificial intelligence (CITL-AI) system to improve the accuracy and efficiency of abnormal cervical squamous cell detection in cervical cancer screening. The artificial intelligence (AI) system was developed using 8000 digitalized whole slide images, including 5713 negative and 2287 positive cases. External validation was performed using an independent, multicenter, real-world data set of 3514 women, who were screened for cervical cancer between 2021 and 2022. Each slide was assessed using the AI system, which generated risk scores. These scores were then used to optimize the triaging of true negative cases. The remaining slides were interpreted by cytologists who had varying degrees of experience and were categorized as either junior or senior specialists. Stand-alone AI had a sensitivity of 89.4% and a specificity of 66.4%. These data points were used to establish the lowest AI-based risk score (ie, 0.35) to optimize the triage configuration. A total of 1319 slides were triaged without missing any abnormal squamous cases. This also reduced the cytology workload by 37.5%. Reader analysis found CITL-AI had superior sensitivity and specificity compared with junior cytologists (81.6% vs 53.1% and 78.9% vs 66.2%, respectively; both with P < .001). For senior cytologists, CITL-AI specificity increased slightly from 89.9% to 91.5% (P = .029); however, sensitivity did not significantly increase (P = .450). Therefore, CITL-AI could reduce cytologists' workload by more than one-third while simultaneously improving diagnostic accuracy, especially compared with less experienced cytologists. This approach could improve the accuracy and efficiency of abnormal cervical squamous cell detection in cervical cancer screening programs worldwide.

Abstract:OBJECTIVE Artificial intelligence (AI) could automatedly detect abnormalities in digital cytological images, however, the effect in cervical cancer screening is inconclusive. We aim to evaluate the performance of AI-assisted cytology for the detection of histologically cervical intraepithelial lesions (CIN) or cancer. METHODS We trained a supervised deep learning algorithm based on 188,542 digital cytological images. Between Mar 13, 2017, and Oct 20, 2018, 2145 referral women from organized screening were enrolled in a multicenter, clinical-based, observational study. Cervical specimen was sampled to generate two liquid-based slides: one random slide was allocated to AI-assisted reading, and the other to manual reading conducted by skilled cytologists from senior hospital and cytology doctors from primary hospitals. HPV testing and colposcopy-directed biopsy was performed, and histological result was regarded as reference. We calculated the relative sensitivity and relative specificity of AI-assisted reading compared to manual reading for CIN2+. This trial was registered, number ChiCTR2000034131. RESULTS In the referral population, AI-assisted reading detected 92.6% of CIN 2 and 96.1% of CIN 3+, significantly higher than or similar to manual reading. AI-assisted reading had equivalent sensitivity (relative sensitivity 1.01, 95%CI, 0.97-1.05) and higher specificity (relative specificity 1.26, 1.20-1.32) compared to skilled cytologists; whereas higher sensitivity (1.12, 1.05-1.20) and specificity (1.36, 1.25-1.48) compared to cytology doctors. In HPV-positive women, AI-assisted reading improved specificity for CIN1 or less at no expense of reduction of sensitivity compared to manual reading. CONCLUSIONS AI-assisted cytology may contribute to the primary cytology screening or triage. Further studies are needed in general population.

Abstract:An ELISA was developed to detect circulating antigens of Toxoplasma gondii in the serum of cats. For the experiment, toxoplasmosis was induced in a group of cats by oral administration of bradyzoites. An ELISA that detects anti-Toxoplasma IgG, an ELISA to detect circulating antigens, and fecal examinations were performed on samples from each cat for 1 year after inoculation. When coupled with IgG-class antibody measurement, antigen detection can aid in the diagnosis of some cases of subclinical feline toxoplasmosis.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Adequate cytology is limited by insufficient cytologists in a large‐scale cervical cancer screening. We aimed to develop an artificial intelligence (AI)‐assisted cytology system in cervical cancer screening program.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>We conducted a perspective cohort study within a population‐based cervical cancer screening program for 0.7 million women, using a validated AI‐assisted cytology system. For comparison, cytologists examined all slides classified by AI as abnormal and a randomly selected 10% of normal slides. Each woman with slides classified as abnormal by either AI‐assisted or manual reading was diagnosed by colposcopy and biopsy. The outcomes were histologically confirmed cervical intraepithelial neoplasia grade 2 or worse (CIN2+).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Finally, we recruited 703 103 women, of whom 98 549 were independently screened by AI and manual reading. The overall agreement rate between AI and manual reading was 94.7% (95% confidential interval [CI], 94.5%‐94.8%), and kappa was 0.92 (0.91‐0.92). The detection rates of CIN2+ increased with the severity of cytology abnormality performed by both AI and manual reading (<i>P </i><sub>trend</sub> < 0.001). General estimated equations showed that detection of CIN2+ among women with ASC‐H or HSIL by AI were significantly higher than corresponding groups classified by cytologists (for ASC‐H: odds ratio [OR] = 1.22, 95%CI 1.11‐1.34, <i>P </i> < .001; for HSIL: OR = 1.41, 1.28‐1.55, <i>P </i> < .001). AI‐assisted cytology was 5.8% (3.0%‐8.6%) more sensitive for detection of CIN2+ than manual reading with a slight reduction in specificity. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>AI‐assisted cytology system could exclude most of normal cytology, and improve sensitivity with clinically equivalent specificity for detection of CIN2+ compared with manual cytology reading. Overall, the results support AI‐based cytology system for the primary cervical cancer screening in large‐scale population.</p></section>

Abstract:Abstract Cervical cancer is a significant global health issue, its prevalence and prognosis highlighting the importance of early screening for effective prevention. This research aimed to create and validate an artificial intelligence cervical cancer screening (AICCS) system for grading cervical cytology. The AICCS system was trained and validated using various datasets, including retrospective, prospective, and randomized observational trial data, involving a total of 16,056 participants. It utilized two artificial intelligence (AI) models: one for detecting cells at the patch-level and another for classifying whole-slide image (WSIs). The AICCS consistently showed high accuracy in predicting cytology grades across different datasets. In the prospective assessment, it achieved an area under curve (AUC) of 0.947, a sensitivity of 0.946, a specificity of 0.890, and an accuracy of 0.892. Remarkably, the randomized observational trial revealed that the AICCS-assisted cytopathologists had a significantly higher AUC, specificity, and accuracy than cytopathologists alone, with a notable 13.3% enhancement in sensitivity. Thus, AICCS holds promise as an additional tool for accurate and efficient cervical cancer screening.

Abstract:Background: Primary cervical cancer screening and treating precancerous lesions are effective ways to prevent cervical cancer. However, the coverage rates of human papillomavirus (HPV) vaccines and routine screening are low in most developing countries and even some developed countries. This study aimed to explore the benefit of an artificial intelligence-assisted cytology (AI) system in a screening program for a cervical cancer high-risk population in China. Methods: A total of 1231 liquid-based cytology (LBC) slides from women who underwent colposcopy at the Chinese PLA General Hospital from 2018 to 2020 were collected. All women had received a histological diagnosis based on the results of colposcopy and biopsy. The sensitivity (Se), specificity (Sp), positive predictive value (PPV), negative predictive value (NPV), false-positive rate (FPR), false-negative rate (FNR), overall accuracy (OA), positive likelihood ratio (PLR), negative likelihood ratio (NLR) and Youden index (YI) of the AI, LBC, HPV, LBC + HPV, AI + LBC, AI + HPV and HPV Seq LBC screening strategies at low-grade squamous intraepithelial lesion (LSIL) and high-grade squamous intraepithelial lesion (HSIL) thresholds were calculated to assess their effectiveness. Receiver operating characteristic (ROC) curve analysis was conducted to assess the diagnostic values of the different screening strategies. Results: The Se and Sp of the primary AI-alone strategy at the LSIL and HSIL thresholds were superior to those of the LBC + HPV cotesting strategy. Among the screening strategies, the YIs of the AI strategy at the LSIL + threshold and HSIL + threshold were the highest. At the HSIL + threshold, the AI strategy achieved the best result, with an AUC value of 0.621 (95% CI, 0.587-0.654), whereas HPV testing achieved the worst result, with an AUC value of 0.521 (95% CI, 0.484-0.559). Similarly, at the LSIL + threshold, the LBC-based strategy achieved the best result, with an AUC of 0.637 (95% CI, 0.606-0.668), whereas HPV testing achieved the worst result, with an AUC of 0.524 (95% CI, 0.491-0.557). Moreover, the AUCs of the AI and LBC strategies at this threshold were similar (0.631 and 0.637, respectively). Conclusions: These results confirmed that AI-only screening was the most authoritative method for diagnosing HSILs and LSILs, improving the accuracy of colposcopy diagnosis, and was more beneficial for patients than traditional LBC + HPV cotesting.
Relativity Analysis of Thin-Prep-liquid-based cytology test and pathologic histology for diagnosis of cervical lesion

Abstract:Objective To investigate the accuracy of Thin-Prep-liquid-based cytology test for cervical lesion.Methods 511 samples were detected by TCT according to TBS diagnosis system,and all patients were proceeded by cervicalbiopsy under guidance of colposcope. The coincidence of TCT results was statistically compared with the pathological diagnosis.Results The positive rate of lesion up to low-grade squamous intraepithelial lesion (LSIL) was 74.36%(380/511) in TCT,and 61.64%(315/511) in cervicalbiopsy. There was significant difference between them (P<0.05). The coincidence rate of screening cases with LSIL and inflammation was 65.14%(114/175). The coincidence rate of screening cases with high-grade squamous intraepithelial lesion (HSIL) and squamous cell carcinoma was 66.67%(146/219). There was no significant difference between them (P>0.05). Conclusion TCT is the capital method for cervical lesion.
Assessing artificial intelligence enabled liquid-based cytology for triaging HPV-positive women: a population-based cross-sectional study

Abstract:Introduction: Cytology-based triaging is commonly used to manage the care of women with positive human papillomavirus (HPV) results, but it suffers from subjectivity and a lack of sensitivity and reproducibility. The diagnostic performance of an artificial intelligence-enabled liquid-based cytology (AI-LBC) triage approach remains unclear. Here, we compared the clinical performance of AI-LBC, human cytologists and HPV16/18 genotyping at triaging HPV-positive women. Material and methods: HPV-positive women were triaged using AI-LBC, human cytologists and HPV16/18 genotyping. Histologically confirmed cervical intraepithelial neoplasia grade 2/3 or higher (CIN2+/CIN3+) were accepted as thresholds for clinical performance assessments. Results: Of the 3514 women included, 13.9% (n = 489) were HPV-positive. The sensitivity of AI-LBC was comparable to that of cytologists (86.49% vs 83.78%, P = 0.744) but substantially higher than HPV16/18 typing at detecting CIN2+ (86.49% vs 54.05%, P = 0.002). While the specificity of AI-LBC was significantly lower than HPV16/18 typing (51.33% vs 87.17%, P < 0.001), it was significantly higher than cytologists at detecting CIN2+ (51.33% vs 40.93%, P < 0.001). AI-LBC reduced referrals to colposcopy by approximately 10%, compared with cytologists (51.53% vs 60.94%, P = 0.003). Similar patterns were also observed for CIN3+. Conclusions: AI-LBC has equivalent sensitivity and higher specificity compared with cytologists, with more efficient colposcopy referrals for HPV-positive women. AI-LBC could be particularly useful in regions where experienced cytologists are few in number. Further investigations are needed to determine triaging performance through prospective designs.
Study on Cervix Carcinoma Screening with Liquid-based Cytology Test

Abstract:Objective To evaluate the efficacy of liquid-based cytology test for screening cervix carcinoma.Methods 2246 and 2635 patients were screened for cervical carcinoma with TCT and traditional cervical cells smears respectively.Cell diagnosis adopted TBS-group system,all the subjects of above-ASCUS pathological changes were examined with colposcope.Results The above-LISL pathological changes identified by TCT and traditional smears were 4.1% and 1.56 % respectively.There was significant difference between them(P0.01).The detection rates by TCT and traditional cervical cells smears were all 100% for SCC,90.91% and 55.56% for HSIL,and 81.82%,55.17% for LSIL.There was distinct difference between them(P 0.05).Conclusion TCT is more sensitive than traditional cervical cells smears.Liquid-based cytology can improve the detection rate of LSIL in cervical screen,especially in LISL and HSIL.
Artificial Intelligence Assisted Cytological Detection for Early Esophageal Squamous Epithelial Lesions by Using Low‐grade Squamous Intraepithelial Lesion As Diagnostic Threshold

Abstract:AbstractBackgroundManual cytological diagnosis for early esophageal squamous cell carcinoma (early ESCC) and high‐grade intraepithelial neoplasia (HGIN) is unsatisfactory. Herein, we have introduced an artificial intelligence (AI)‐assisted cytological diagnosis for such lesions.MethodsLow‐grade squamous intraepithelial lesion or worse was set as the diagnostic threshold for AI‐assisted diagnosis. The performance of AI‐assisted diagnosis was evaluated and compared to that of manual diagnosis. Feasibility in large‐scale screening was also assessed.ResultsAI‐assisted diagnosis for abnormal cells was superior to manual reading by presenting a higher efficiency for each slide (50.9 ± 0.8 s vs 236.8 ± 3.9 s, p = 1.52 × 10−76) and a better interobserver agreement (93.27% [95% CI, 92.76%–93.74%] vs 65.29% [95% CI, 64.35%–66.22%], p = 1.03 × 10−84). AI‐assisted detection showed a higher diagnostic accuracy (96.89% [92.38%–98.57%] vs 72.54% [65.85%–78.35%], p = 1.42 × 10−14), sensitivity (99.35% [95.92%–99.97%] vs 68.39% [60.36%–75.48%], p = 7.11 × 10−15), and negative predictive value (NPV) (97.06% [82.95%–99.85%] vs 40.96% [30.46%–52.31%], p = 1.42 × 10−14). Specificity and positive predictive value (PPV) were not significantly differed. AI‐assisted diagnosis demonstrated a smaller proportion of participants of interest (3.73%, [79/2117] vs.12.84% [272/2117], p = 1.59 × 10−58), a higher consistence between cytology and endoscopy (40.51% [32/79] vs. 12.13% [33/272], p = 1.54 × 10−8), specificity (97.74% [96.98%–98.32%] vs 88.52% [87.05%–89.84%], p = 3.19 × 10−58), and PPV (40.51% [29.79%–52.15%] vs 12.13% [8.61%–16.75%], p = 1.54 × 10−8) in community‐based screening. Sensitivity and NPV were not significantly differed. AI‐assisted diagnosis as primary screening significantly reduced average cost for detecting positive cases.ConclusionOur study provides a novel cytological method for detecting and screening early ESCC and HGIN.

Abstract:Objective: To explore the application of manual screening collaborated with the Artificial Intelligence TPS-Assisted Cytologic Screening System in urinary exfoliative cytology and its clinical values. Methods: A total of 3 033 urine exfoliated cytology samples were collected at the Henan People's Hospital, Capital Medical University, Beijing, China. Liquid-based thin-layer cytology was prepared. The slides were manually read under the microscope and digitally presented using a scanner. The intelligent identification and analysis were carried out using an artificial intelligence TPS assisted screening system. The Paris Report Classification System of Urinary Exfoliated Cytology 2022 was used as the evaluation standard. Atypical urothelial cells and even higher grade lesions were considered as positive when evaluating the recognition sensitivity, specificity, and diagnostic accuracy of artificial intelligence-assisted screening systems and human-machine collaborative cytologic screening methods in urine exfoliative cytology. Among the collected cases, there were also 1 100 pathological tissue controls. Results: The accuracy, sensitivity and specificity of the AI-assisted cytologic screening system were 77.18%, 90.79% and 69.49%; those of human-machine coordination method were 92.89%, 99.63% and 89.09%, respectively. Compared with the histopathological results, the accuracy, sensitivity and specificity of manual reading were 79.82%, 74.20% and 95.80%, respectively, while those of AI-assisted cytologic screening system were 93.45%, 93.73% and 92.66%, respectively. The accuracy, sensitivity and specificity of human-machine coordination method were 95.36%, 95.21% and 95.80%, respectively. Both cytological and histological controls showed that human-machine coordination review method had higher diagnostic accuracy and sensitivity, and lower false negative rates. Conclusions: The artificial intelligence TPS assisted cytologic screening system has achieved acceptable accuracy in urine exfoliation cytologic screening. The combination of manual screening and artificial intelligence TPS assisted screening system can effectively improve the sensitivity and accuracy of cytologic screening and reduce the risk of misdiagnosis.

Abstract:Objective To investigate the diagnose accordance rate of liquid-based cytology test(LCT) and tissue biopsy for cervix,and to investigate the clinical application value of LCT combined with tissue biopsy in early screening for cervical epithelial lesions.Methods The results of 6 498 cases undergoing cervical LCT inspection during January to December 2011 were retrospectively analyzed.Patients with atypical squamous cells of undetermined significance(ASC-US) or severe than ASC-US were defined as abnormal cytology according to Bethesda system for cytology classifications.Among 377 cases of abnormal cytology,148 cases received biopsy inspection.The cytology and histology results were compared,and the relationship of lesion and ages were analyzed.Results In 6 498 cases receiving cervical LCT,the abnormal cytology ratio was 5.80%.Among them,ASC-US were 215 cases(57.03%),atypical squamous cell cannot exclude high grade squamous intraepithelial lesion(ASC-H) were 36 cases(9.55%),low grade squamous intraepithelial lesion(LSIL) were 82 cases(21.75%),high grade squamous intraepithelial lesion(HSIL) were 39 cases(10.34%),atypical glandular cells of undetermined significance not otherwise specified(AGC-NOS) were 4 cases(1.06%),and squamous cell carcinoma(SCC) were 1 cases(0.27%).Among the 148 cases of biopsy,cases of inflammation reaction were 39 cases(26.35%),koilocytotic cells 16 cases(10.81%),cervical intraepithelial neoplasia(CINⅠ) 32 cases(21.62%),CINⅡ35 cases(23.65%),CINⅢ 17 cases(11.49%),squamous carcinoma 8 cases(5.41%),and adenocarcinoma 1 cases(0.68%),which revealing that the cytology results had a high conformity rate with biopsy results.And the onset age of cervical lesions ranged from 30 to 50.Conclusion LCT is an important approach of screening for cervical epithelial lesions with its high accuracy.Combination of LCT and tissue biopsy can greatly improve early diagnosis of CIN and carcinoma in cervix.
Artificial intelligence-assisted fast screening cervical high grade squamous intraepithelial lesion and squamous cell carcinoma diagnosis and treatment planning

Abstract:Abstract Every year cervical cancer affects more than 300,000 people, and on average one woman is diagnosed with cervical cancer every minute. Early diagnosis and classification of cervical lesions greatly boosts up the chance of successful treatments of patients, and automated diagnosis and classification of cervical lesions from Papanicolaou (Pap) smear images have become highly demanded. To the authors’ best knowledge, this is the first study of fully automated cervical lesions analysis on whole slide images (WSIs) of conventional Pap smear samples. The presented deep learning-based cervical lesions diagnosis system is demonstrated to be able to detect high grade squamous intraepithelial lesions (HSILs) or higher (squamous cell carcinoma; SQCC), which usually immediately indicate patients must be referred to colposcopy, but also to rapidly process WSIs in seconds for practical clinical usage. We evaluate this framework at scale on a dataset of 143 whole slide images, and the proposed method achieves a high precision 0.93, recall 0.90, F-measure 0.88, and Jaccard index 0.84, showing that the proposed system is capable of segmenting HSILs or higher (SQCC) with high precision and reaches sensitivity comparable to the referenced standard produced by pathologists. Based on Fisher’s Least Significant Difference (LSD) test (P < 0.0001), the proposed method performs significantly better than the two state-of-the-art benchmark methods (U-Net and SegNet) in precision, F-Measure, Jaccard index. For the run time analysis, the proposed method takes only 210 seconds to process a WSI and is 20 times faster than U-Net and 19 times faster than SegNet, respectively. In summary, the proposed method is demonstrated to be able to both detect HSILs or higher (SQCC), which indicate patients for further treatments, including colposcopy and surgery to remove the lesion, and rapidly processing WSIs in seconds for practical clinical usages.

Abstract:Objective:To explore the application of ThinPrep Cytology Test (TCT) and high-risk human papillomavirus(HR-HPV) DNA test for the screening cervical lesions.Methods: 1 794 women were monitored by Thinprep liquid based cytology and hybrid capture Ⅱ,taking biopsy pathology result as golden standard.Results:In 1 794 women, by Thinprep liquid based cytology 159,69 and 34 women were found to be ASCUS,LSIL and HSILrespectively.By biopsy under the celpascope 104,21 and 18 women were diagnosed as CINⅠ,CINⅡ and CINⅢ,resp ectively,while 4 women were diagnosed as squamous cervical cancer;The infection rate of HR-HPV was 100.0%(4/4) in SCC patients, 94.9% (37/39) in HS1L patients,89.4%(93/104) in LSIL patients.For Thinprep liquid based cytology,the sensitivity,specificity and negative predictive values were 89.1%,91.8% and 99.0% respectively,The sensitivity,specificity and negative predictive values of HR-HPV were 91.2%,85.5% and 99.1% respectively.Additionally,the sensitivity,specificity and negative predictive values were 99.3%,82.6% and 99.9% by HR- HPV combined Thinprep liquid based cytology.ConclusioniCombining Thinprep liquid based cytology with HR-HPV-DNA test may increase the sensitivity and negative predictive value for detecting cervical lesions.

Abstract:Abstract Cervical cancer is one of the most common cancers in daily life. Early detection and diagnosis can effectively help facilitate subsequent clinical treatment and management. With the growing advancement of artificial intelligence (AI) and deep learning (DL) techniques, an increasing number of computer-aided diagnosis (CAD) methods based on deep learning have been applied in cervical cytology screening. In this paper, we survey more than 80 publications since 2016 to provide a systematic and comprehensive review of DL-based cervical cytology screening. First, we provide a concise summary of the medical and biological knowledge pertaining to cervical cytology, since we hold a firm belief that a comprehensive biomedical understanding can significantly contribute to the development of CAD systems. Then, we collect a wide range of public cervical cytology datasets. Besides, image analysis approaches and applications including cervical cell identification, abnormal cell or area detection, cell region segmentation and cervical whole slide image diagnosis are summarized. Finally, we discuss the present obstacles and promising directions for future research in automated cervical cytology screening.

Prediction of Cervical Intraepithelial Neoplasia and Invasive Cancer Using Deep Learning System Applied to Microscopic Cytological Images: A Clinical, Multicenter, Diagnostic Study

Abstract:Background: Cytology-based cervical cancer screening is limited by its low sensitivity, especially in low-resource settings. Methods: We trained a deep learning system (DLS) using deep convolutional neural network models on a large dataset of 188,542 microscopic cytological images. We enrolled 2145 women aged 25-64 years to evaluate the algorithm by a clinical, multicenter, blinded case-control study. Cervical specimens were collected for liquid-based cytology by DLS analysis or by cytologists reading, and for Cobas HPV test, respectively. Colposcopy-directed biopsy were performed on all women. We compared the diagnostic performance of the DLS for detection of histopathologically confirmed CIN grade 2 or worse (CIN2+) with cytologists and with Cobas HPV test. Findings: The DLS identified 92·6% of CIN grade 2, 93·0% of CIN grade 3, and 94.6% of cervical cancer, with DLS scores significantly increasing with severity of cervical lesion (p 0·05 for both), respectively, whereas DLS specificity was higher than skilled cytologists (p<0·001) and similar to HPV test (p=0·30). In HPV positive women, the DLS showed similar sensitivity (p=0·79) but significantly higher specificity (p<0·001) compared to skilled cytologists in CIN2+ detection. Compared with cytologists at primary hospitals, the DLS increased sensitivity and specificity in CIN2+ detection by 49% and 84% (p<0·001), respectively. Interpretation: DLS could have similar diagnostic performance for CIN2+ compared with skilled cytologists and Cobas HPV test. Diagnostic Yield and Performance of a Large Population-Based Cervical Cancer Screening Program in High-Risk Rural China

Abstract:Primary cervical cancer screening by liquid-based cytology alone or with adjunctive HPV testing are available worldwide. However, little if any information is available about cervical cancer diagnostic yield of population-based cervical cancer screening in China. In response to it, we conducted a large prospective study on 40,000 women cervical cancer screening within six-month period in rural Shanxi Province, where has been reported as the highest cervical cancer rates in China. A standard cross-sectional survey by self-completed questionnaire was collected and followed by the liquid-based cytology screening. Follow-up biopsy with the diagnosis of cervical intraepithelial neoplasia 2 or higher lesion (CIN2+) were analyzed. Of initial 40,000 women participating in this study, 6.76% (2702/40,000) women had ASC-US or higher cytology screening results with ASC/SIL ratio at 6.14 (2381:388). Among them, 1.96% (782/40,000) women were found CIN lesions (95% CI, 1.68-2.23%) on confirmatory biopsies, including 0.55% (218/40,000) CIN2+ and 19 SCCa (47.5/100,000, 95% CI, 29-74/100,000). Women in Yangqu County had lower ASC/SIL ratio, but higher CIN2+ detection rate in comparison with that of Jiexiu County (6.69 vs. 8.84 and 56.7% vs. 43.9%), which reflects the cervical cancer distribution in different populations and regions. Analysis in age-stratified cytology results indicated women aged 60-65 years had the highest incidence of cytologic abnormality among all the age group; likewise, women aged >50 years were at higher risk in developing cervical high grade dysplasia or cancer than women aged <50 years (0.82% vs. 0.49%). This large-scale cervical cancer screening study provided important references as the instructive for establishing the nation-wide cervical cancer screening strategy.

Abstract:Nowadays, with the continuous improvement of people's health awareness and the increasing incidence of cancer year by year, the screening work of various clinical diseases, especially cancer, is also widely promoted. Cervical lesion screening is a common type. Cervical lesion screening is used to judge whether women have cervical lesions and increase attention to cervical lesions[1]. Proper prevention, diagnosis and treatment play an important role in ensuring women's health and life safety, and have been widely promoted in many places in our country. At the same time, in the screening of cervical lesions, how to adopt a more efficient and accurate diagnosis is very important to improve the screening effect of cervical lesions. With the rapid development of science and technology, AI and other advanced technologies have been gradually introduced in clinical disease diagnosis, especially in the liquid-based thin-layer cytology examination[2]. To further analyze the application effect of liquid based thin layer cytology AI diagnostic technology in the pathological diagnosis of cervical lesions. In this paper, we investigate the occurrence of HPV-16 integrated human host genome in cervical cancer and precancerous lesions and its application in cervical cancer screening. 50 cases of cervical cancer, 127 cases of LSIL, 83 cases of HSIL were collected, in addition, 22 cases of normal cervical epithelial tissues extracted by total hysterectomy were collected. The integration status of HPV-16 infection was detected by overlapping PCR.
