Abstract:Currently, the precise causes of over 40 % of recurrent spontaneous abortion (RSA) cases cannot be identified, leading to the term “unexplained RSA” (URSA). Through an exploration of the gut microbiota, metabolites, and immune cell subsets in URSA, this study establishes a link between gut microbiota-derived metabolites and immune cells. The results indicate reduced diversity in the gut microbiota of URSA. Targeted metabolomic analyses reveal decreased levels of gut microbiota-derived deoxycholic acid (DCA), glycolithocholic acid (GLCA), acetate, propionate, and butyrate in URSA. Furthermore, elevated frequencies of Th1, Th17, and plasma B cells, along with decreased frequencies of Tregs and Bregs, are observed in the peripheral blood of URSA. The results demonstrate correlations between the levels of gut microbiota-derived bile acids and short-chain fatty acids and the frequencies of various immune cell subsets in circulation. Collectively, this study uncovers an association between gut microbiota-derived metabolites and circulating immune cell subsets in URSA.
Expression of TLR2 and TLR4 on Decidual and Villi in Unexplained Recurrent Spontaneous Abortion and Its Significance.

Abstract:Objective In this study,by detecting the TLR2,TLR4 in the expression of villi and decidua of unexplained recurrent spontaneous abortion(URSA) and early pregnancy,we can preliminarily study the role of TLR2 and TLR4 in the genesis and development of URSA.Meantime,by detecting serum level of TNF-α and IL-10,we can preliminarily learn the available ways of TLR2 and TLR4 in the occurrence of URSA.Methods Prospective study on outpatients of Obstetrics and Gynecology at the Medical College of Zhejiang University and Hangzhou First People′s Hospitalin 2008-2009 was carried out.Those women whose age were between 25-35 years old,and gestational age <12 weeks were put together as a study group.For those URSA patients who have been spontaneously aborted for ≥3 times,we regarded then as study group.While for those healthy patients who had normal reproductive history,and demanded induced abortion in early pregnancy termination of pregnancy,we distinguished them as a control group.We selected both URSA patients(study group) and healthy pregnant patients(control group) with 20 cases.By immunohistochemical staining,we detected the expression of TLR2 and TLR4 in villi and decidua to compare the different expression of TLR2 and TLR4 in villi and decidua of URSA and early pregnancy.At the same time,we detected serum concentration of TNF-α and IL-10 with ELISA to compare the concentration differences of serum TNF-α and IL-10 between URSA and early pregnancy.Results ①No significant difference in expression of TLR2 in the cytoplasm and the membrane of villous trophoblast cells and glandular epithelial cells was found between two groups(P>0.05).②Compared with contrast group,expression of TLR4 in the cytoplasm and the membrane of trophoblast cells and glandular epithelial cells in study group was significantly increased,and the differences were statistically significant(P<0.05).③Compared with the control group,concentration of TNF-α in the study group was significantly higher,and the difference was statistically significant(P<0.05);while the concentration of IL-10 had no significant difference(P>0.05).Conclusion TLR4 causes URSA through releasing of excessive levels of Th1 type cytokines,and thus leading to Th1/Th2 imbalance.
Causal Relationship Between Circulating Immune Cells and Recurrent Spontaneous Abortion: A Bidirectional Mendelian Randomization Study

Abstract:ABSTRACT Background Recurrent spontaneous abortion (RSA) is a serious and common complication of pregnancy caused by multiple factors. The etiology remains incompletely understood, but immunologic factors play important roles. Here, we aimed to evaluate whether circulating immune cells causally impacted RSA. Methods In this study, we conducted a comprehensive two‐sample Mendelian randomization (MR) study to determine the causal association between the 731 immunophenotypes of human peripheral blood lymphocytes and the number of spontaneous abortions as well as recurrent miscarriage. Sensitivity analyses were performed to assess and minimize heterogeneity and horizontal pleiotropy. Reverse MR analysis was used to assess reverse causality. Results After Bonferroni‐correction, eight immunophenotypes were significantly associated with the number of spontaneous abortions: FSC‐A on CD4 + T cell (beta = −0.051, 95% CI = [−0.085, −0.017], P ‐value = 0.004), CD8 on HLA DR + CD8 + T cell (beta = −0.040, 95% CI = [−0.067, −0.014], P ‐value = 0.003), HLA DR on CD33 dim HLA DR + CD11b − (beta = −0.021, 95% CI = [−0.036, −0.005], P ‐value = 0.010), HLA DR + T cell Absolute Count (beta = 0.022, 95% CI = [0.006, 0.037], P ‐value = 0.008), HLA DR + T cell % lymphocyte (beta = 0.026, 95% CI = [0.010, 0.041], P ‐value = 0.001), HLA DR + T cell % T cell (beta = 0.023, 95% CI = [0.007, 0.039], P ‐value = 0.004), HLA DR + CD4 + T cell % lymphocyte (beta = 0.034, 95% CI = [0.007, 0.060], P ‐value = 0.012), and HLA DR on B cell (beta = 0.012, 95% CI = [0.003, 0.021], P ‐value = 0.010). In addition, we identified two immunophenotypes associated with recurrent miscarriage: HLA DR on B cell (OR = 0.854, 95% CI = [0.757, 0.964], P ‐value = 0.011), and CD19 on naive‐mature B cell (OR = 4.595, 95% CI = [1.674, 12.617], P ‐value = 0.003). There was no evidence of heterogeneity, horizontal pleiotropy and reverse causality. Conclusions Our study demonstrated a tight link between adaptive immune cells and RSA through genetic means, thus providing potential therapeutic targets or novel diagnostic biomarkers.

Abstract:Recurrent spontaneous abortion affects approximately 1–2% of women of childbearing, and describes a condition in which women suffer from three or more continuous spontaneous miscarriages. However, the origin of recurrent spontaneous abortion (RSA) remains unknown, preventing effective treatment and placing stress upon patients. It has been acknowledged that successful pregnancy necessitates balanced immune responses. Therefore, immunological aberrancy may be considered a root cause of poor pregnancy outcomes. Considerable published studies have investigated the relationship between various immune cells and RSA. Here, we review current knowledge on this area, and discuss the five main categories of immune cells involved in RSA; these include innate lymphocytes (ILC), macrophages, decidual dendritic cells (DCs), and T cells. Furthermore, we sought to summarize the impact of the multiple interactions of various immune cells on the emergence of RSA. A good understanding of pregnancy-induced immunological alterations could reveal new therapeutic strategies for favorable pregnancy outcomes.

Abstract:Introduction: Spontaneous abortion is defined as pregnancy loss before the twentieth week and Recurrent Spontaneous Abortion (RSA) is defined as at least three spontaneous and successive pregnancy losses in the same period. Among the different types of immunological causes, this study refers specifically to the alloimmune cause. Women with RSA of alloimmune cause share a greater number of Human Leukocyte Antigen (HLA) with their husbands, leading to the inhibition of the production of anti-paternal asymmetric blocking antibodies, which would protect embryonic cells. Objectives: To evaluate the e ffect of immunotherapy with paternal HLA-sensitizing mononuclear cells in cases of RSA through the positivity of the cross-match test and its efficacy in pregnancy success. Patients and Methods: Written consent was obtained, 12 couples with a history of RSA presenting negative cross-match were included in the study. Blood samples were collected from the couple for cross-matching and the separation of paternal mononuclear cells. Immunotherapy was performed with paternal mononuclear cells intradermally in the wives’ arms on day (D) 0, D15 and D30. After the third dose, a second evaluation of the cross- match tests was performed. Results and Discussion: The age of the wives ranged from 28 to 41 years, with a mean of 34.5 years. The twelve couples had a negative cross-match test (100%). Eleven couples (96.6%) tested positive in the cross- match test after immunotherapy. Of these, 10 (90.9%) had successful preg nancies. Immunotherapy with paternal mononuclear cells showed an excellent ability to sensitize the maternal immune system, with positive cross-match, resulting in a successful pregnancy.

Abstract:Recurrent pregnancy loss (RPL), defined as two or more consecutive miscarriages, is attributable to multiple causes. However, in 50% of cases no known cause is found. Although endometritis is a known cause of miscarriage, other inflammatory processes may play a role in idiopathic, recurrent loss. The fetoplacental unit evades rejection by the maternal immune system by poorly understood mechanisms. Despite this seemingly immune-privileged state for the fetus, human implantation requires inflammatory mediators for attachment and implantation. This review describes how the immune system must simultaneously permit and restrict trophoblastic invasion for healthy implantation and maintenance of pregnancy. Included in this review is a detailed description of the immune milieu in the decidua and abnormalities that are associated with RPL. Finally, autoimmune states associated with RPL and their treatment in an obstetrical setting are reviewed.
Clinical and pathologic aspects of recurrent placental villitis

Abstract:In a retrospective survey, recurrent villitis was identified in ten of 59 patients in whom placental villitis had been diagnosed. The ten patients had a total of 41 pregnancies, with a reproductive loss of 60 per cent. In addition to enhanced fetal losses in all trimesters of gestation and postnatally, the incidences of fetal growth retardation and premature delivery were increased. There was no evidence of recent TORCH (toxoplasma, rubella, cytomegalovirus, herpes) infection, but all patients tested had rubella immunity. In six patients genital cultures were positive for gonorrhea and assorted microorganisms. Uterine abnormalities, including two septate uteri, one incompetent cervix, one submucosal leiomyoma, and one retroflexion, were common, and vaginal bleeding had occurred in five patients. Other factors included obesity (five patients) and clinical and laboratory evidence of autoimmunity (four of the five patients tested). In a control group of 20 patients with nonrecurrent villitis, the perinatal loss rate (37 per cent) was lower, and the incidences of positive cultures, uterine structural anomalies, obesity, and autoimmunity were also lower. Placental histologic findings included decidual plasma cell and intervillous fibrin and histiocytic infiltration, in addition to villous inflammation. These lesions, although consistent for a given patient, defined two clinically relevant groups of patients. The results of this study suggest that recurrent villitis is more frequent than previously reported, that it is associated with high perinatal mortality, and that immunologic and structural abnormalities in the host may play a role in its pathogenesis.
Prevalence and association of placental lesions with obstetrical features and outcome: data from French prospective study

Abstract:Purpose: Since the Consensus Statement diffused by the Amsterdam Placental Workshop Group, knowledge of the meaning of placental vascular malperfusion has become essential in the unavoidable analysis of obstetrical history in a patient followed for autoimmune disease or any other maternal comorbidity. We aimed to analyse the prevalence of various placental lesions from a 6-months prospective observational study and to correlate the various placental profiles to obstetrical outcome, maternal diseases and pregnancy treatments. The frequency of foetal vascular malperfusion lesion could be estimated at 8.7%, in our population and to understand its neonatal associations. Methods: The study groups consisted of 208 consecutive women which ended the pregnancy and have placental analysis during the period of the study. Results: From December 2015 to October 2017, from overall 4398 delivered pregnancies in university obstetrical department, 208 (4.7%) placental analysis have been done and included in the study. The placental analysis have been done for vascular obstetrical complications during the pregnancy (n = 106; 51%), unexplained abnormal foetal heart rate tracings (n = 59; 28,3%), suspicion of intra-amniotic infection (n = 12; 5,7%%), term new-borns Apgar score <7 or arterial cord blood pH ≤ 7 (n = 7; 3,5%), spontaneous preterm delivery (n = 19; 9,1%), intrahepatic cholestasis of pregnancy (n = 5; 2,4%). An adverse obstetrical event was noted in 87 cases (42%): preeclampsia or HELLP syndrome (n = 15; 7%), FGR (n = 59; 28%), gestational diabetes (n = 33; 16%) and gestational hypertension (n = 19; 9%). Placental histological analysis showed abnormal vascular features in 159 cases (76%), inflammatory features in 16 placentas (8%), vascular and inflammatory features in 10 cases (4%), chorioamnionitis in 38 cases (18%) and absence of any abnormality in 43 cases (21%). A cluster analysis of histological features allowed distinguishing three placental patterns: a normal pattern characterised by the absence of any placental lesions, an inflammatory pattern characterised by the presence of villitis and/or chronic intervillositis; a vascular pattern with the presence of thrombosis, maternal floor infarct with massive perivillous fibrin deposition, infarction and chronic villositis hypoxia. Women with inflammatory placental profile have significantly increased frequencies of tobacco use (50% vs. 9%; P = 0.03), pathological vascular Doppler (50% vs. 5%; P = 0.001), FGR (100% vs. 14%; P = 0.0001) and oligohydramnios (67% vs. 5%; P = 0.0001) than those with normal placentas. A higher rate of vascular or inflammatory lesion were observed in women with Hypertensive disorder of pregnancy, where as those with inflammatory pattern have significantly more frequent FGR (100% vs 34%; P = 0.02) and oligohydramnios (67% vs 5%; P = 0.0002). Conclusion: The placenta analysis is important to understand the origin of adverse obstetrical outcome and the risk for subsequent pregnancy.

Abstract:Recurrent Spontaneous Abortion (RSA) is a common pregnancy complication, that has multifactorial causes, and currently, 40%–50% of cases remain unexplained, referred to as Unexplained RSA (URSA). Due to the elusive etiology and mechanisms, clinical management is exceedingly challenging. In recent years, with the progress in reproductive immunology, a growing body of evidence suggests a relationship between URSA and maternal‐fetal immunology, offering hope for the development of tailored treatment strategies. This article provides an immunological perspective on the pathogenesis, diagnosis, and treatment of RSA. On one hand, it comprehensively reviews the immunological mechanisms underlying RSA, including abnormalities in maternal‐fetal interface immune tolerance, maternal‐fetal interface immune cell function, gut microbiota‐mediated immune dysregulation, and vaginal microbiota‐mediated immune anomalies. On the other hand, it presents the diagnosis and existing treatment modalities for RSA. This article offers a clear knowledge framework for understanding RSA from an immunological standpoint. In conclusion, while the "layers of the veil" regarding immunological factors in RSA are gradually being unveiled, our current research may only scratch the surface. In terms of immunological etiology, effective diagnostic tools for RSA are currently lacking, and the efficacy and safety of immunotherapies, primarily based on lymphocyte immunotherapy and intravenous immunoglobulin, remain contentious.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Problem</h3><p>We aimed to investigate the main causes of recurrent miscarriage (RM) in patients with losses after spontaneous gestation (SG) and after in vitro fertilization (IVF).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Method of Study</h3><p>A prospective case‐control study was conducted. The eligible patients were women who had experienced two or more consecutive abortions after less than 12 weeks gestation, two consecutive losses after SG or two consecutive losses after IVF. All patients were subjected to the following evaluations: karyotyping of the aborted material, alloimmune and autoimmune marker testing, and acquired and hereditary thrombophilia marker testing.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>In total, 58 patients were eligible: 32 patients with RM after SG and 26 patients with RM after IVF. The factors associated with RM were genetic (29%), immune (14%), thrombophilic (21%), and thrombophilic and immune (24%), and only 12% of the cases were idiopathic. Comparing the two study groups (SG and IVF), all studied factors were similar, except for a higher ANA positivity observed in the SG group (SG 30.4% versus IVF 5.3%, OR 8.6 (CI 1.1 ‐ 21.1, P 0.048).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Our study identified the possibly factors associated with recurrent miscarriage in 86% of the cases, and these factors appear to be similar in patients with recurrent miscarriage after spontaneous gestation and IVF. This study demonstrates that IVF with PGT‐A with euploid embryo transfer could reduce abortions by up to 29%, but other factors needs to be investigated even in patients undergoing in vitro fertilization.</p></section>

Abstract:PROBLEM: To determine the presence of absence of subclinical autoimmunity in healthy women with first trimester recurrent spontaneous abortion (RSA), the sera of 72 healthy women with a history of three or more consecutive abortions were analyzed for the presence of antibodies to phospholipid and nuclear antigens.METHOD: Lupus anticoagulant was determined, as were antibodies to cardiolipin. An indirect immunofluorescent antinuclear test was performed and specific antinuclear antibodies to double-stranded DNA, ribonucleoprotein, Smith antigen, and Sjogren's syndrome A and B antigens were tested.RESULTS: Nine women (13.2%) had low levels of antinuclear antibodies, none of which were specific. Four women (5.6%) had moderate to high levels of aCL antibodies.CONCLUSION: We conclude that the prevalence of aCLs and antinuclear antibodies in healthy women with first trimester RSA is low. Subclinical autoimmunity is not a common cause of first trimester RSA.

Abstract:Successful pregnancy in placental mammals substantially depends on the establishment of maternal immune tolerance to the semi-allogenic fetus. Disorders in this process are tightly associated with adverse pregnancy outcomes including recurrent miscarriage (RM). However, an in-depth understanding of the systematic and decidual immune environment in RM remains largely lacking. In this study, we utilized single-cell RNA-sequencing (scRNA-seq) to comparably analyze the cellular and molecular signatures of decidual and peripheral leukocytes in normal and unexplained RM pregnancies at the early stage of gestation. Integrative analysis identifies 22 distinct cell clusters in total, and a dramatic difference in leukocyte subsets and molecular properties in RM cases is revealed. Specifically, the cytotoxic properties of CD8+ effector T cells, nature killer (NK), and mucosal-associated invariant T (MAIT) cells in peripheral blood indicates apparently enhanced pro-inflammatory status, and the population proportions and ligand–receptor interactions of the decidual leukocyte subsets demonstrate preferential immune activation in RM patients. The molecular features, spatial distribution, and the developmental trajectories of five decidual NK (dNK) subsets have been elaborately illustrated. In RM patients, a dNK subset that supports embryonic growth is diminished in proportion, while the ratio of another dNK subset with cytotoxic and immune-active signature is significantly increased. Notably, a unique pro-inflammatory CD56+CD16+ dNK subset substantially accumulates in RM decidua. These findings reveal a comprehensive cellular and molecular atlas of decidual and peripheral leukocytes in human early pregnancy and provide an in-depth insight into the immune pathogenesis for early pregnancy loss.

Abstract:BACKGROUND:Studies have reported that myeloid-derived suppressor cells (MDSCs) contribute to maintain pregnancy. The aim of this case-control study was to test whether there is a dysregulation of peripheral MDSCs in recurrent implantation failure (RIF).METHODS:26 RIF patients and 30 controls were recruited. Flow cytometry was applied to characterize polymorphonuclear (PMN)-MDSCs, monocytic-MDSCs (M-MDSCs), effector T cells (Teffs) and regulatory T cells (Tregs) in blood. ELISA was used to define MDSCs correlative cytokines and chemokines in serum from all patients.RESULTS:Compared with controls, RIF patients showed significant reductions of blood PMN-MDSCs, M-MDSCs, Tregs and NO production by PMN-MDSCs, whereas the expression of ζ chain on CD4+T cell receptor (TCR) and CD8+TCR displayed a remarkable upregulation in RIF patients. Moreover, RIF patients presented a lower concentration of serum chemokine (C-C motif) ligand (CCL) 5 and transforming growth factor (TGF)-β than those from controls. Furthermore, the level of TCR ζ chain on CD4+ and CD8+ Teffs was negatively correlated not only with the percentage of PMN-MDSCs, but also with the amount of NO produced by PMN-MDSCs. The frequency of PMN-MDSCs had positive correlations with the concentration of CCL5 and TGF-β.CONCLUSIONS:This study indicated that the dysregulation of MDSCs might impair maternal-fetal immune balance thus resulting in RIF.
Decrease in Regulatory T-cells and Increase in T Helper 17 Cells in Women With Recurrent Spontaneous Abortion

Abstract:Abstract Background Appearance of improper immune responses against the fetus and/or inadequate immunoregulatory mechanisms during pregnancy may lead to recurrent spontaneous abortion (RSA). T H 17 cells play a significant role in inducing inflammation, autoimmune disease, and acute transplant rejection, while regulatory T (Treg) cells moderate the function of immune system in order to retain homeostasis. Methods This case-control study was designed to evaluate T H 17 as well as Treg cells in 25 women with RSA and 25 age-matched healthy non-pregnant women. Flow cytometric assay was performed using monoclonal antibodies to detect CD4 + CD25 + Treg cells (CD25 dim and CD25 bright ). FoxP3 and RORγt expressions were compared using real-time PCR, and pro-inflammatory and anti-inflammatory cytokines were measured by ELISA kits. Independent-samples T test was employed for statistical analysis. Results The ratio of CD4 + CD25 bright T cells was remarkably lower in women with RSA ( P <0.05), and CD4 + CD25 dim T cells did not show any significant difference among the groups ( P >0.05). RORγt was up-regulated, and FoxP3 was down-regulated significantly in case group ( P <0.05). The significant increase of IL-6 and IL-17 as well as the decrease of TGF-β was indicated in RSA group ( P <0.05). Also, IL-10 did not vary among the groups ( P >0.05). Conclusion These remarks prove that the decrease in regulatory factors such as CD4 + CD25 bright T-cells, TGF-β and FoxP3 expression may disrupt immune tolerance and homeostasis during pregnancy. Also, the environment rich in RORγt, IL-6, and IL-17 suggests the detrimental role of T H 17 cells, which may lead to fetal rejection.

Abstract:Previously, a continuous atmospheric pressure interfaced miniature mass spectrometer was developed in our lab. The continuous atmospheric pressure interface improves system robustness, stability, and scan speed, but it also results in limited ion transfer efficiency and reduced mass resolution. To solve these problems, a miniature ion funnel was designed and integrated into the miniature mass spectrometer for the first time. Besides ion transfer efficiency, dimension and power consumption of the ion funnel also need to be considered throughout the design process. After a systematic optimization, the designed miniature ion funnel could increase ion transfer efficiency by more than 10 times, while lowering the background pressure of ion trap by ∼2 times. As a result, sensitivity and mass resolution of the second generation miniature mass spectrometer were improved by 20 times and ∼2 times, respectively, while maintaining its high scan speed and stability. A sensitive and robust mini-MS, capable of coupling with ambient ionization sources would meet the needs of many on-site chemical analysis applications, such as in food, drug, and agricultural administrations, forensic science, homeland security, and etc.

Abstract:Abstract Objective: Hyperkalaemia is a potentially lethal electrolyte disorder. The objective of this study was to determine if the causes of hyperkalaemia-related visits to the emergency department (ED) have changed since 25 years. Methods: All patients presenting to the ED with hyperkalaemia between January 2009 and August 2011 were included in this retrospective, single-centre study. Patients were divided into one of these three categories: mild (5·2⩽K+<5·8 mEq/l), moderate (5·8⩽K+<7·0 mEq/l) or severe hyperkalaemia (K+⩾7·0 mEq/l). The causes of hyperkalaemia were divided into three groups: renal failure (RF), potassium-increasing drugs (PIDs) or others. Results: Overall, 139 patients with hyperkalaemia were included in the study (mean K+ of 6·2 mEq/l): 35% with mild, 49% with moderate and 16% with severe hyperkalaemia. Eighty-three per cent of patients (n = 115) had RF with creatinine levels ⩾1·25 mg/dl or estimated glomerular filtration rate (eGFR) levels ⩽60 ml/min/1·73 m2. Serum potassium levels were significantly related with creatinine and eGFR values (P<0·001). The severity of hyperkalaemia was significantly related with creatinine levels ⩾1·25 mg/dl (P = 0·002) and eGFR values ⩽60 ml/min/1·73 m2 (P = 0·005). Seventy-five per cent of patients (n = 105) were taking PIDs. Potassium levels were significantly related with PIDs (P<0·001), in particularly spironolactone (P = 0·001) and angiotensin-converting enzyme inhibitors (P = 0·008). The category ‘others’ included 7% of patients (n = 10). Conclusions: RF (83%) and PIDs (75%) remain common causes of hyperkalaemia. Hyperkalaemia is significantly related with four variables: creatinine levels, spironolactone, ACEIs and beta-blocker intake. The causes of hyperkalaemia have not changed in recent years.

Abstract:Objective: To investigate the roles of Th1 (interleukin-2) and Th2 (interleukin-4) cytokine balance at the fetal-maternal interface in women with unexplained recurrent spontaneous abortion.Methods: Thirty patients with unexplained recurrent spontaneous abortion (URSA) and twenty-four normal pregnancy women were included in this study. The expression of IL-2, IL-4 on chorionic villi and decidua at early pregnant was detected by immunohistochemistry, and the serum concentrations of IL-2, IL-4 were measured by enzyme-linked immunosorbant assay(ELISA).Results: Both antigens of IL-2 and IL-4 were located in the syncytiotrophoblast,cytotrophoblast cells and decidual glandular epithelium at early pregnant. IL-2 protein was mostly seen in aborted chorionic villi and decidua from URSA patients while it presented at a much lower level in those two tissues from normal pregnancy women(P0.01,P0.05, respectively). On the contrast, IL-4 expressed with higher amount in chorionic villi and deciduas from normal pregnancy women compared to URSA patients(P0.01,P(0.01,) respectively). IL-2 was not detectable in all blood samples except two of URSA patients. The blood level of IL-4 was also very low, but it was a little higher in sera from URSA patients compared to normal women(P0.01). Conclusion: A shift from Th2 to Th1 response at the fetal-maternal interface may contribute to unexplained recurrent spontaneous abortion.
Natural Killer Cell and Maternal-fetal Interface Immune Dysfunction in Recurrent Spontaneous Abortion

Abstract:The natural killer (NK) cells in peripheral blood and endometrial layers have been associated with the maintenance of pregnancy immunological intolerance. And the regulation roles of NK cells are reviewed in relation to maternal-fetal interface immune dysfunction in recurrent spontaneous abortions (RSA). The placenta has a complex anatomical structure and different subsets of NK cells with various function can directly interact with trophoblasts. The alterations in NK cell numbers and/or activity in women with RSA before and after conception are a critical clinical concern. We have reviewed articles about NK cells subpopulations (including peripheral blood NK cells and uterine NK cells) and their functions,putative mechanism of NK cells in RSA,and immuno-therapy treated with abnormal NK cell activity status. Further studies are needed to explore the underlying mechanism of NK cells in RSA and possible immunological treatment strategy.
