Abstract:The aim of this study was to evaluate the morphology and function of the left atrial appendage (LAA) in different rhythms of paroxysmal atrial fibrillation (PaAF) patients using real-time 3D transesophageal echocardiography. This study included 46 patients with PaAF-24 with PaAF with sinus rhythm (PaAF-S) and 22 with PaAF with fibrillation (PaAF-F)-and 23 healthy volunteers. Real-time 3D transesophageal echocardiography images of the LAA were acquired in all patients. LAA end-diastolic volume increased in PaAF-S and PaAF-F, whereas LAA end-systolic volume increased only in PaAF-F. LAA emptying index, end-diastolic volume and end-systolic volume were strongly negatively correlated. LAA emptying index was positively correlated with percentage changes in LAA area and diameter, as well as flow velocity. The co-occurrence of spontaneous echo contrast and thrombus gradually increased in the PaAF-F group, compared with the PaAF-S group. The morphology and change in function of the LAA in patients with paroxysmal PaAF can be evaluated with superior feasibility and reproducibility using real-time 3D transesophageal echocardiography. Morphologic changes in the LAA appeared earlier than changes in LAA function during the course of PaAF. Spontaneous echo contrast and thrombus co-occurred more often in PaAF-F than in PaAF-S.
4D Flow Component Reveals Left Ventricular Function in Atrial Fibrillation

Abstract:Purpose: Applicating cardiovascular magnetic resonance (CMR) 4D flow to evaluate left ventricular systolic and diastolic function in atrial fibrillation (AF). Methods: In this study, from May 2021 to October 2021, 26 AF patients and 15 healthy participants were recruited and underwent multiparametric CMR and echocardiogram scans before discharge. The CMR protocol incorporated an assessment of 4D flow, cardiac function. Notably, the AF patients maintained irregular heart rhythm during the CMR scan. The 4D flow components were compared with echocardiogram results. Results: AF patients had a lower proportion of direct flow (33.5% (8.43; 47.5) in AF vs. 69.1% (63.4; 74.4) in healthy), higher delayed ejection (24.1±12% in AF vs. 14.7±6.8% in healthy), retained inflow (32.5% (24.6; 36.9) in AF vs. 14.5% (12.5; 18.2) in healthy) and residual volume (4.73% (1.47; 15.0) in AF vs. 0.62% (0.46; 1.62) in healthy). A high correlation was observed between flow EF and CMR EF (R=0.78, P<0.001) in AF patients. The mean bias among the methods was higher for flow EF than CMR EF (24.86%). Multivariable linear regression showed that the correlation between retained inflow and E/e’ (β=0.152, P=0.048) remained significant when adjusted for confounders. Direct flow (β= -0.170, P=0.038) and retained inflow (β=0.350, P=0.024) significantly correlated with the Minnesota Living with Heart Failure Questionnaire score. Conclusion: CMR 4D flow component revealed a significantly different flow pattern in AF patients compared with healthy participants and provided de novo flow biomarker indicating LV systolic function (direct flow and flow EF) and diastolic function (retained inflow).

Abstract:Abstract Background Atrial fibrillation (AF) is associated with left atrial (LA) mechanical dysfunction, which predisposes to stasis of blood within the LA and thus increases the risk of stroke. However, limited is known about the mechanical functional alterations of left atrial appendage (LAA) in relation to stages of AF. Here, we sought to analyse LA and LAA mechanics during different stages of AF in detail. Methods A total of 92 patients with AF referring for radiofrequency ablation were consecutively enrolled. Patients were categorized according to rhythm and stages of AF:sustained AF(SAF) (n=31), paroxysmal AF with sinus rhythm(PAF-SR)(n=30) andAF rhythm(PAF-AF)(n=31)at the time of echocardiography. Twenty-seven patients without a history of AF were included as controls. Using speckle-tracking echocardiography, LA and LAA global peak positive longitudinal strain (GLS) and mechanical dispersion (MD) were measured in all patients. Mechanical dispersion was defined as the standard deviation of the time to peak of the regional strain corrected by R-R interval. Results Patients with PAF-AF and SAF displayed prominently impaired LA and LAA mechanics(all P < 0.01 vs. PAF-SR and no AF). In the presence of SR, only LAA mechanical function were lower in PAF patients than patients without AF(LAA GLS 20.5±5.7%(no AF) vs 15.9±5.4%(PAF-SR); LAA MD 6.9±3.8%(no AF) vs 9.4±3.9(PAF-SR),both P<0.01). AF stages were independent determinants of LA and LAA mechanics after adjusting for age and comobidities. Conclusions AF affected mechanical function of LA and LAA on their own, and more so if stage of AF has advanced to more persistent forms. Abstract:Abstract Background Left atrial (LA) and left atrial appendage (LAA) dysfunction has been demonstrated to contribute to atrial fibrillation (AF)-related stroke. However, usefulness of LA and LAA mechanics has not been fully compared. We sought to investigate the association of LA and LAA mechanics with stroke and to compare their diagnostic values in the risk stratification of stroke in patients with nonvalvular AF. Methods A total of 208 consecutive patients with AF (63.58 ± 10.37 years, 63.9% male,57.7% persistent AF) who underwent echocardiography before catheter ablation were prospectively enrolled. Speckle-tracking was used to measure LA and LAA global longitudinal strain (GLS). LA and LAA mechanical dispersions (MD) were defined as the standard deviation (SD) of time to peak positive strain corrected by the R-R interval. Results Patients with prior stroke/ transient ischemic attack (TIA) (n = 31) had significantly higher LA and LAA MD than those without (n = 177) (11.56 ± 4.38% vs. 8.43 ± 3.44%, 15.15 ± 5.46% vs. 10.94 ± 4.40%, both P < 0.01). In multivariable analysis, LA and LAA MD were independently associated with stroke/TIA (odds ratio, 1.18–1.29, 1.19–1.22, respectively, both P < 0.01), providing incremental values over clinical and standard echocardiographic parameters. In a subgroup analysis, LA MD was more useful than LAA MD in patients with normal LA volumes, while LAA MD was superior to LA MD in patients with LA enlargement. Conclusions Higher LA and LAA mechanical dispersion are independently associated with stroke/TIA in AF patients and had incremental values over clinical and conventional echocardiographic parameters. What’s more, priorities of dispersion assessment are different depending on patients’ LA size.

Abstract:Atrial Fibrillation (AF) is associated with a five-fold increase in the risk of cerebrovascular events. Recent studies suggest that a computational fluid-dynamics (CFD) approach could provide insights on AF mechanisms thus potentially allowing a quantitative assessment of cardioembolic risk. The goal of this study was to use a previously developed patient specific CFD model of the left atrium (LA) to enhance differences in blood flow in AF patients and normal subjects. In this study we computed LA blood flow and derived parameters in normal subjects (NL), patients affected by paroxysmal AF (PAR-AF) and patients affected by persistent AF (PER-AF). Results showed mean peak velocities continuously decreasing from NL to PER-AF groups. In agreement, a lower number of vortex structures was observed in PER-AF with respect to PAR-AF and NL, thus limiting an effective washout of the LA and the left atrial appendage (LAA). Velocities at the LAA ostium and inside the LAA were also strongly reduced showing a limited washout effect as confirmed by blood stasis in terms of number of particles still present after five cardiac cycles (NL: 5±2, PAR-AF: 18±3, PER-AF: 41±10). The developed approach quantifies differences in LA hemodynamic between AF patients and NL.

Abstract:Abstract Atrial fibrillation (AF) is a progressive disease that starts with structural or functional changes in the left atrium and left ventricle, and evolves from paroxysmal toward sustained forms. Early detection of structural or functional changes in the left atrium and left ventricle in the paroxysmal stage could be useful for identifying a higher risk of progression to persistent AF and future cardio-cerebrovascular events. The aim of this study was to test the hypothesis that the feature tracking (FT) left atrial (LA) strain and left ventricular (LV) extracellular volume fraction (ECV) derived from cardiovascular magnetic resonance (CMR) could detect early changes in remodeling of the left atrium and ventricle in the paroxysmal AF (PAF) stage. The participants were comprised of 106 PAF patients (age, 66.1 ± 10.7 years; 66% male) who underwent clinical CMR before pulmonary vein isolation and 20 control subjects (age, 68.3 ± 8.6 years; 55% male). The CMR-FT LA strain/phasic function and LV-ECV were compared between the PAF and control groups. The total and passive LA empty fraction (LAEF) and LA strain (corresponding to LA reservoir and conduit function) were decreased in the PAF group as compared to the control group. However, active LAEF (corresponding to the LA booster pump function) did not differ significantly between the PAF group (33.9 ± 10.9%) and control group (37.9 ± 13.3%, p = 0.15), while the active LA strain (corresponding to the LA booster pump function) was significantly decreased in the PAF group (11.4 ± 4.3 vs. 15.2 ± 5.6%, p = 0.002). The LV-ECV was significantly greater in the PAF group (28.7 ± 2.8%) than control group (26.6 ± 2.0%, p = 0.002). In the PAF group, the LV-ECV correlated significantly with the E/e′ and LA volume index. Regarding the LA strain, correlations were seen between the LV-ECV and both the reservoir function and conduit function. CMR-FT LA strain in combination with the LV-ECV in a single clinical study offers a potential imaging marker that identifies LA/LV remodeling including subtle LA booster pump dysfunction undetectable by the conventional booster pump LAEF in the PAF stage.

Abstract:Introduction: Atrial fibrillation (AF) is associated with an increased risk of stroke, often caused by thrombi that form in the left atrium (LA), and especially in the left atrial appendage (LAA). The underlying mechanism is not fully understood but is thought to be related to stagnant blood flow, which might be present despite sinus rhythm. However, measuring blood flow and stasis in the LAA is challenging due to its small size and low velocities. We aimed to compare the blood flow and stasis in the left atrium of paroxysmal AF patients with controls using computational fluid dynamics (CFD) simulations. Methods: The CFD simulations were based on time-resolved computed tomography including the patient-specific cardiac motion. The pipeline allowed for analysis of 21 patients with paroxysmal AF and 8 controls. Stasis was estimated by computing the blood residence time. Results and Discussion: Residence time was elevated in the AF group ( p < 0.001). Linear regression analysis revealed that stasis was strongest associated with LA ejection ratio ( p < 0.001, R 2 = 0.68) and the ratio of LA volume and left ventricular stroke volume ( p < 0.001, R 2 = 0.81). Stroke risk due to LA thrombi could already be elevated in AF patients during sinus rhythm. In the future, patient specific CFD simulations may add to the assessment of this risk and support diagnosis and treatment.

Abstract:Introduction: The left atrial appendage (LAA) is the most common site of thrombus formation in atrial fibrillation (AF) patients who develop strokes, yet its dynamic volumetric parameters remain uncharacterized. We present a quantitative analysis of cardiac phase-specific LAA volumes and function using cardiac CT-derived three-dimensional (3D) models of 920 LAA segmentations. Methods: We measured LAA volumes at 10% increments of the cardiac cycle (R-R interval) in patients with AF and controls with sinus rhythm (SR). After identifying the LAA orifice using anatomical landmarks on multiplanar CT images, we performed computational LAA segmentation using specialized software. For precise mapping, we used iterative active region-growing and cutting tools to separate the LAA from surrounding structures. We used the resulting 3D model for volumetric assessment of the LAA during the cardiac cycle. Results: We analysed 580 LAA segmentations in patients with AF and 340 segmentations in patients with SR. Both groups displayed LAA volume increase from 0 to 40% of the cardiac cycle followed by a gradual decrease during diastole. LAA volumes were significantly higher in AF at 0%, 10%, 70%, 80%, and 90% of the cardiac cycle (p =0.01, 0.021, 0.047, 0.025 and 0.011, respectively). LAA volumetric variance within the AF group was significantly greater (p<0.001). LAA ejection fraction was significantly lower in AF compared to SR (40.8±16.1% vs 49.7±12.6%, p=0.003). Polynomial plots of incremental volume changes showed an accelerated rate of LAA filling in early systole in the SR, which was significantly dampened in AF. Conclusions: We present the first comparative analysis of LAA volumetric and functional assessment in patients with AF and SR. We conclude that impaired LAA filling and emptying in AF can contribute to blood stasis and thrombogenicity. Further investigations into the utility of LAA metrics as prognostic and risk-stratification tools in patients with AF are warranted.

Abstract:Left atrial (LA) wall fibrosis plays an important role in the perpetuation of atrial fibrillation (AF) since the abnormal electrophysiological properties of the fibrotic areas sustains the arrhythmia by favoring both re-entry circuits as well as abnormal impulse generation. Despite its crucial contribution, the mechanisms by which LA fibrosis develops are not well understood. The LA wall is constantly exposed to the hydraulic forces exerted by the blood flow arriving from the pulmonary veins. The purpose of the present study was to examine the association between regional wall shear stress and areas with fibrosis in the LA of patients with AF. 15 patients (13 males, mean age 61±11 years) with AF, no significant mitral regurgitation and clinical indication for a primary catheter ablation were prospectively recruited for the study. All participants underwent a baseline three-dimensional electro-anatomical mapping of the LA during the ablation procedure and a pre-interventional cardiovascular magnetic resonance (CMR) imaging with phase contrast for mitral flow estimation and Gadolinium injection for LA fibrosis detection. Fibrotic areas were detected either by low bipolar voltage (BV≤0.5mV) and/or by areas with enhanced late Gadolinium uptake as assessed by the image intensity ratio (IIR≥1.2). For all subjects, a detailed 3D anatomical model of the LA was extracted from the invasive electro-anatomical maps and was used to calculate regional time-averaged wall shear stress (TAWSS) and blood age (BA), an index of blood stagnation, by performing patient-specific computational fluid dynamic simulations. Globally, areas around the pulmonary veins and the LA roof exhibited the highest values of TAWSS. In all cases, high TAWSS was strongly correlated with low voltage (n=15, r from -0.002 to -0.449, p<0.01) and enhanced late Gadolinium uptake (n=12, r from 0.071 to 0.475, p<0.001). Fibrotic areas as detected by both low BV and high IIR were more prevalent in areas exposed to high TAWSS (21.6% vs 8.1% and 26.2% vs 13.2% respectively, p<0.001). Inversely, in all but one case, areas with low TAWSS presented more intense blood stagnation as assessed by the highest BA (r from -0.268 to -0.688, p<0.001). In patients with AF, regional high TAWSS is associated with corresponding CMR biomarkers of left atrial wall fibrosis and electrical scaring. Inversely, areas with low TAWSS are associated with blood stagnation and could favor thrombus formation. This may provide insights of a novel pathophysiological mechanism explaining the characteristic atrial electrical remodeling and thrombus formation seen in patients with AF.

Abstract:Atrial fibrillation ( AF ) is a common arrhythmia mainly affecting the elderly population, which can lead to serious complications such as stroke, ischaemic attack and vascular dementia. These problems are caused by thrombi which mostly originate in the left atrial appendage ( LAA ), a small muscular sac protruding from left atrium. The abnormal heart rhythm associated with AF results in alterations in the heart muscle contractions and in some reshaping of the cardiac chambers. This study aims to verify if and how these physiological changes can establish hemodynamic conditions in the LAA promoting thrombus formation, by means of computational fluid dynamic (CFD) analyses. In particular, sinus and fibrillation contractility was replicated by applying wall velocity/motion to models based on healthy and dilated idealized shapes of the left atrium with a common LAA morphology. The models were analyzed and compared in terms of shear strain rate ( SSR ) and vorticity, which are hemodynamic parameters directly associated with thrombogenicity. The study clearly indicates that the alterations in contractility and morphology associated with AF pathologies play a primary role in establishing hemodynamic conditions which promote higher incidence of ischaemic events, consistently with the clinical evidence. In particular, in the analyzed models, the impairment in contractility determined a decrease in SSR of about 50%, whilst the chamber pathological dilatation contributed to a 30% reduction, indicating increased risk of clot formation. The equivalent rigid wall model was characterized by SSR values about one order of magnitude smaller than in the contractile models, and substantially different vortical behavior, suggesting that analyses based on rigid chambers, although common in the literature, are inadequate to provide realistic results on the LAA hemodynamics.

Abstract:Background: Atrial fibrillation (AF) is associated with cardiac structural and functional remodeling. We investigated the left atrial (LA) and left ventricular (LV) changes in AF subtypes by using two-dimensional echocardiography strain techniques. Methods: The study population consisted of 102 subjects with sinus rhythm (control group) and 463 patients with AF, among which 284 patients had paroxysmal AF (PAF) and 179 patients had persistent AF (PerAF). A speckle tracking automatic functional imaging software was used to perform the strain analysis. Results: Patients with AF had dilated LA maximum and minimum volume, decreased LA reservoir strain, lower LV ejection fraction (LVEF), and impaired global longitudinal strain (GLS) compared to those of the sinus rhythm control group. In patients with PerAF, the LA maximum and minimum volumes were larger, and the LA reservoir strain [PAF vs. PerAF, 28% (21,33) vs. 19% (14, 28), P < 0.05], LVEF, and absolute GLS value (PAF vs. PerAF, −16.9 ± 3.3 vs. −14.1 ± 3.5%) were lower than those in patients with PAF. Patients with AF regardless of LA enlargement had decreased LA reservoir strain and lower LVEF and absolute GLS value than those in the sinus rhythm control group. Conclusion: Compared with those with normal sinus rhythm, patients with AF had dilated LA volume and impaired LA function, which were further worsened in patients with PerAF than those in patients with PAF. LA functional impairment occurred before LA enlargement. Left atrioventricular remodeling happened across different stages of AF development. Graphical .

Abstract:BACKGROUND: Abnormalities in the left atrial (LA) structure and function may develop in patients with paroxysmal atrial fibrillation (AF). We sought to determine the contribution of LA mechanical function, including LA stiffness, to AF by comparing patients with paroxysmal AF with normal control subjects, and to evaluate whether LA mechanical function and stiffness are related with the structural changes of LA.METHODS: Sixty-four paroxysmal AF patients (57 ± 13 years, 59% male) were studied, using a speckle tracking echocardiography, and were compared with 34 age-, gender-, and left ventricular (LV) mass-matched controls (53 ± 14 years, 61% male). LA volume indices, expansion index for reservoir function, active emptying fraction for contractile function, mitral annular velocities, and global longitudinal LA strain were measured. The ratio of E/e' to LA strain was used as an index of LA stiffness.RESULTS: Patients with paroxysmal AF had similar LV volume indices, ejection fraction, and diastolic function, when compared with that of the normal controls. However, paroxysmal AF patients showed increased LA volume indices and decreased LA reservoir function, but similar contractile function. LA stiffness was increased in patients with paroxysmal AF than in the control subjects (0.40 ± 0.25 vs. 0.29 ± 0.10, p = 0.002), and was related with LA volume indices and reservoir function.CONCLUSION: Patients with paroxysmal AF have decreased LA reservoir function and increased stiffness, in comparison with that of the control subjects. LA stiffness was significantly related with LA volume indices and reservoir function. LA stiffness can be used for the assessment of LA function in patients with paroxysmal AF.

Strain rate imaging for noninvasive functional quantification of the left atrium in hypertensive patients with paroxysmal atrial fibrillation

Abstract:Aim: Strain rate (SR) imaging has been applied to the detection of regional left ventricular (LV) dysfunction but not as much to the assessment of left atrial (LA) function. We aimed to assess atrial myocardial properties during atrial fibrillation (AF) by myocardial velocity, SR and strain, focusing on the effects of hypertension and atrial arrhythmias, especially paroxysmal AF. Methods: We compared 3 groups of a total of 110 consecutive patients with hypertension presenting to our institution: 20 with brief atrial tachycardia, 20 with paroxysmal AF and 70 with hypertension alone. These patients and 32 controls underwent transthoracic echocardiography, tissue velocity imaging (TVI), strain examination and SR imaging. Atrial tissue velocity, strain and SR values of hypertensive patients were compared with those of age-matched controls. Results: Compared with controls, hypertensive patients with paroxysmal AF showed significantly increased atrial myocardial features as assessed by TVI (p < 0.05-0.001). Time to peak late diastolic SR corrected for heart rate (TASRc) and the difference of early diastolic SR (deltaESR) were significantly increased (both p < 0.05), whereas the difference of late diastolic SR (deltaASR) (p < 0.05) and the difference of TASRc (deltaTASRc) (p < 0.001) were significantly decreased; moreover, systolic SR (SSR), deltaASR and deltaTASRc were significantly decreased (p < 0.05, p < 0.01 and p < 0.001, respectively), whereas deltaESR was significantly increased (p < 0.01), as compared with hypertensive patients without arrhythmia. No other parameters differed among the 4 groups. Conclusions: SR imaging combined with TVI enables noninvasive quantification of LA dysfunction due to hypertension and paroxysmal AF. In hypertensive patients, paroxysmal AF decreases the efficiency, not the ability, of LA myocardia to reserve potential energy, which suggests that LA myocardial reservoir function decreases. The impaired conductivity of the left atrium leads to decreased total active atrial contraction and prolonged interatrial conduction. Thus, the temporal asynchrony of the atria is enhanced, but contraction asynchrony is reduced.
Characteristics of left atrial remodeling in patients with atrial fibrillation and hypertrophic cardiomyopathy in comparison to patients without hypertrophy

Abstract:Atrial fibrillation (AF) leads to remodeling characterized by changes in both size and shape of the left atrium (LA). Here we aimed to study the effect of hypertrophic cardiomyopathy (HCM) on the pattern of LA remodeling in AF-patients. HCM-patients (n = 23) undergoing AF ablation (2009-2012) were matched and compared with 125 Non-HCM patients from our prospective registry. Pre-procedural CT data were analyzed (EnSite Verismo, SJM, MN) to determine the maximal sagittal (anterior-posterior, AP), coronal (superior-inferior, SI and transversal, TV) dimensions and the sphericity index (LAS). Volume (LAV) was rendered after appendage (LAA) and pulmonary vein (PV) exclusion. A cutting plane, between PV ostia/LAA and parallel to the posterior wall, divided LAV into anterior- (LA-A) and posterior-LA (LA-P) parts. The ratio LA-A/LAV was defined as asymmetry index (ASI). HCM patients had a wider inter-ventricular septum and a smaller LV than Non-HCM patients. LA volume (LAV 166 ± 72 vs. 130 ± 36 ml, p = 0.03) and LA diameters were significantly larger in HCM patients. Anterior volume (LA-A: 112 ± 48 vs. 83 ± 26 ml, p < 0.001) differed significantly between groups, whereas the posterior volume LA-P (55 ± 28 vs. 47 ± 13 ml, p = 0.23) and LAS (75% vs. 78%, p = 0.089) was similar in both groups. As a result, ASI was significantly higher (67 ± 6 vs. 63 ± 6%, p = 0.01) in HCM than in Non-HCM patients. In conclusion, LA remodeling in patients with AF and HCM is characterized by asymmetric dilatation, driven by an anterior rather than a posterior dilatation. This can be characterized by three-dimensional imaging and could be used as surrogate of advanced atrial remodeling.
Asymmetric remodeling between the left and right atria in patients with advanced interatrial block and atrial fibrillation

Abstract:Background: Advanced interatrial block (A-IAB) on electrocardiography (ECG) represents the conduction delay between the left and right atria. We investigated the association of A-IAB with left and right atrial (LA/RA) remodeling in patients with atrial fibrillation (AF). Methods: We enrolled 74 patients who underwent ECG, cardiac computed tomography (CCT), and echocardiography during sinus rhythm before catheter ablation of AF. A-IAB was defined as P-wave duration ≥120 ms with a biphasic morphology in leads III and aVF or notched morphology in lead II. We compared the maximum and minimum LA/RA volume indices (max and min LAV/RAVI), LA/RA expansion index (LAEI/RAEI), and total, passive, and active LA/RA emptying fraction (LAEF/RAEF) between patients with and without A-IAB. Results: Of the 74 patients (mean age, 64.3 ± 9.6 years), 35 (47%) showed A-IAB. Patients with A-IAB had a significantly higher likelihood of hypertension and left ventricular diastolic dysfunction than those without. Patients with A-IAB had significantly larger max (69.2 [60.7-79.7]mL/m2 vs. 60.9 [50.4-68.3]mL/m2, P < 0.01) and min (44.0 [37.2-52.1]mL/m2 vs. 34.1 [29.2-43.5]mL/m2, P < 0.01) LAVI than those without. The max and min RAVI were not significantly different between groups. LAEI (55.1 [48.2-78.5]% vs. 72.1 [57.8-84.8]%, P < 0.05), total LAEF (35.5 [32.5-44.0]% vs. 41.9 [36.6-45.9]%, P < 0.05), and passive LAEF (12.2 [10.0-14.4]% vs. 15.5 [11.2-19.6]%, P < 0.05) were significantly lower in patients with A-IAB than without. Conclusions: A-IAB was associated with LA, but not RA enlargement, in patients with AF. A-IAB may indicate LA functional remodeling in the reservoir and conduit phases.
Correct Closure of the Left Atrial Appendage Reduces Stagnant Blood Flow and the Risk of Thrombus Formation: A Proof-of-Concept Experimental Study Using 4D Flow Magnetic Resonance Imaging

Abstract:Objective: The study was conducted to investigate the effect of correct occlusion of the left atrial appendage (LAA) on intracardiac blood flow and thrombus formation in patients with atrial fibrillation (AF) using four-dimensional (4D) flow magnetic resonance imaging (MRI) and three-dimensional (3D)-printed phantoms. Materials and methods: Three life-sized 3D-printed left atrium (LA) phantoms, including a pre-occlusion (i.e., before the occlusion procedure) model and correctly and incorrectly occluded post-procedural models, were constructed based on cardiac computed tomography images from an 86-year-old male with long-standing persistent AF. A custom-made closed-loop flow circuit was set up, and pulsatile simulated pulmonary venous flow was delivered by a pump. 4D flow MRI was performed using a 3T scanner, and the images were analyzed using MATLAB-based software (R2020b; Mathworks). Flow metrics associated with blood stasis and thrombogenicity, such as the volume of stasis defined by the velocity threshold (|V̅| < 3 cm/s), surface-and-time-averaged wall shear stress (WSS), and endothelial cell activation potential (ECAP), were analyzed and compared among the three LA phantom models. Results: Different spatial distributions, orientations, and magnitudes of LA flow were directly visualized within the three LA phantoms using 4D flow MRI. The time-averaged volume and its ratio to the corresponding entire volume of LA flow stasis were consistently reduced in the correctly occluded model (70.82 mL and 39.0%, respectively), followed by the incorrectly occluded (73.17 mL and 39.0%, respectively) and pre-occlusion (79.11 mL and 39.7%, respectively) models. The surface-and-time-averaged WSS and ECAP were also lowest in the correctly occluded model (0.048 Pa and 4.004 Pa-1 , respectively), followed by the incorrectly occluded (0.059 Pa and 4.792 Pa-1 , respectively) and pre-occlusion (0.072 Pa and 5.861 Pa-1 , respectively) models. Conclusion: These findings suggest that a correctly occluded LAA leads to the greatest reduction in LA flow stasis and thrombogenicity, presenting a tentative procedural goal to maximize clinical benefits in patients with AF.
Structural, Functional and Electrical Remodelling of the Atria with Reduced Cardiorespiratory Fitness: Implications for AF

Abstract:Background Reduced cardiorespiratory fitness (CRF) is an independent risk factor for the progression of atrial fibrillation (AF). We hypothesize that reduced CRF is associated with structural, functional and electrical remodelling of the left atrium (LA). Objective To correlate objectively assessed CRF with functional and electrical LA parameters using invasive and non-invasive assessment. Methods Consecutive patients with symptomatic AF undergoing catheter ablation were recruited. CRF was objectively quantified pre-ablation using cardiopulmonary exercise testing (CPET). Using VO 2PEAK participants were classified as Preserved CRF (>20ml/kg/min) or Reduced CRF (<20ml/kg/min). LA stiffness was assessed invasively with hemodynamic monitoring and imaging during high-volume LA saline infusion. LA stiffness was calculated as ΔLA Diameter/ ΔLA Pressure over the course of the infusion. LA function was assessed with echocardiographic measures of LA emptying fraction (LAEF) and LA strain. Electrical remodelling was assessed using high-density elctroanatomical maps for LA voltage and conduction. Results In total 100 participants were recruited; 43 had Reduced CRF and 57 had Preserved CRF. Patients with Reduced CRF demonstrated elevated LA stiffness (p=0.004), reduced LA emptying fraction (p=0.006) and reservoir strain (p<0.001). Reduced CRF was also associated with reduced LA voltage (p=0.039) with greater heterogeneity (p=0.027) and conduction slowing (p=0.04) with greater conduction heterogeneity (p=0.02). On multivariable analysis, VO 2PEAK was independently associated with LA stiffness (p=0.003) and LA conduction velocities (p=0.04). Conclusion Reduced CRF in patients with AF is independently associated with worse LA disease involving functional and electrical changes. Improving CRF may be a target for restoring LA function in AF.

Abstract:Atrial fibrillation (AF) alters left atrial (LA) hemodynamics, which can lead to thrombosis in the left atrial appendage (LAA), systemic embolism and stroke. A personalized risk-stratification of AF patients for stroke would permit improved balancing of preventive anticoagulation therapies against bleeding risk. We investigated how LA anatomy and function impact LA and LAA hemodynamics, and explored whether patient-specific analysis by computational fluid dynamics (CFD) can predict the risk of LAA thrombosis. We analyzed 4D-CT acquisitions of LA wall motion with an in-house immersed-boundary CFD solver. We considered six patients with diverse atrial function, three with either a LAA thrombus (removed digitally before running the simulations) or a history of transient ischemic attacks (LAAT/TIA-pos), and three without a LAA thrombus or TIA (LAAT/TIA-neg). We found that blood inside the left atrial appendage of LAAT/TIA-pos patients had marked alterations in residence time and kinetic energy when compared with LAAT/TIA-neg patients. In addition, we showed how the LA conduit, reservoir and booster functions distinctly affect LA and LAA hemodynamics. Finally, fixed-wall and moving-wall simulations produced different LA hemodynamics and residence time predictions for each patient. Consequently, fixed-wall simulations risk-stratified our small cohort for LAA thrombosis worse than moving-wall simulations, particularly patients with intermediate LAA residence time. Overall, these results suggest that both wall kinetics and LAA morphology contribute to LAA blood stasis and thrombosis.

Abstract:Abstract Background Left atrial (LA) strain is an echocardiographic method to detect atrial dysfunction as a process of structural and functional remodeling of the atria. In patients with persistent atrial fibrillation (AF), functional changes and an enlargement of the LA due to structural remodeling has been described. The aim of our study was 1) the echocardiographic assessment of LA function using LA strain and 2) to investigate the correlation between LA strain and left atrial size index (LAVI) using 3D reconstruction of the LA from multi-detector computed tomography (CT). Methods This study includes patients with persistent AF who underwent LA catheter ablation without prior treatment. All subjects underwent measurement of reservoir (LASr), conduit (LAScd) and contraction (LASct) LA strain using imaging using two-dimensional speckle tracking echocardiography (2D-STE) and cardiac CT. A 3D reconstruction of the LA from the CT was done using EnSite Verismo (Abbott, USA). The LA volume obtained at the end-systole included LA appendage reconstruction and excluded the pulmonary veins. LAVI was calculated by dividing LA volume by body surface area. Results The study included 36 patients (mean age 69 yrs [49 to 80 yrs], 27 male, mean LV-EF 56% [40 to 78%). We found the following LA strain measurement: mean LASr 21% [6 to 52%], mean LAScd -12% [-32 to -0,25%] and mean LASct -9% [-32 to -1%]. LAVI: mean 82 ml/m2 [38 to 127 ml/m2]. There is a statistically significant negative correlation between LASr (p = 0.002, r = -0.503) and a statistically positive correlation between LASct (p = 0.003, r = 0.482) compared to the LA volume, whereas there is no statistically significant correlation between LAScd and the LA volume (p = 0.088, r = 0.288). Discussion LA strain in patients with persistent AF as an expression of functional remodeling is reduced compared to normal values in literature. Correlation of LA strain and LAVI are statistically significant for LASr as well as LASct. The correlation between LAScd and LAVI did not reach statistical significance but shows a trend towards a positive correlation. Whether early treatment of AF could prevent further fibrotic remodeling with reducing LA volume and improving LA strain needs further research.
