Abstract:Five cases of traumatic subdural hematomas in the subacute stage (from 7 to 20 days after head injury) were treated in one male and four females, aged from 63 to 82 years, with evacuation via craniotomy in three and aspiration via burr hole surgery in two. All hematomas were evaluated by T1-, T2-, and diffusion-weighted magnetic resonance imaging, and measurement of the apparent diffusion coefficient (ADC). Diffusion-weighted imaging showed the hematoma as a crescent high intensity area with a low intensity rim close to the brain surface (two-layered structure) in four cases and as high intensity with low intensity components in one case. The high intensity areas under the dura mater on diffusion-weighted imaging appeared as homogeneous high intensity on T1and T2-weighted imaging in four cases, and inhomogeneous high intensity on T1and isointensity on T2-weighted imaging in one case. The mean ADC value of the high intensity areas was 0.58± 0.23 (mean± standard deviation)× 10-3 mm2/sec. The operative findings revealed the high intensity areas as solid clots. The low intensity areas on diffusion-weighted imaging appeared as homogeneous high intensity in four cases and inhomogeneous isointensity with high intensity components in one case on T1and T2-weighted imaging. The mean ADC value of the low intensity areas was 2.03± 0.27× 10-3 mm2/sec. The operative findings revealed the low intensity areas as mixtures of resolved clot and cerebrospinal fluid. Diffusion-weighted imaging showed the characteristic two-layered structure in traumatic subdural hematomas in the subacute stage, and analysis of the ADC values was useful for differentiating solid from liquid hematoma and for selection of the surgical procedure.
Psychosocial factors associated with symptoms of depression, anxiety and stress among single mothers with young children: A population-based study

Abstract:BACKGROUND: Abundant evidence highlights single parenthood as a common risk factor for depression, anxiety and stress but few studies have comprehensively examined psychosocial factors (adversities), particularly during early parenting. We investigated symptom prevalence and potential risk factors among mothers with very young children.METHODS: Data stem from the 2015 National Psychosocial Burdens Prevalence Study (KiD 0-3). Mothers with children up to 3 years of age (n = 6925) were recruited from random probability-sampled paediatric clinics (n = 271) across Germany and reported on depression or anxiety, general and parenting stress using the Patient Health Questionnaire (PHQ-4), Perceived Stress Scale (PSS-4) and Parenting Stress Index (PSI). Multivariable logistic regression models determined risk factors and quantified potential mediation of psychosocial factors for all 3 outcomes.RESULTS: Approximately 30% of single mothers (n = 517) reported depressive or anxiety symptoms and 37% general stress, twice as high compared to partnered mothers (n = 6408; p < 0.0001). Parenting stress was also elevated (p < 0.0001). Adjusted regression models confirm that single mothers are twice as likely to report symptoms of depression or anxiety (OR 1.9, CI<sub>95%</sub> 1.4-2.5). Risk factors for stress correspond to those for depression and anxiety. Inadequate social support and history of partner or childhood maltreatment were also consistent risk factors across all outcomes.LIMITATIONS: The study design and self-reported symptoms are limitations to consider.CONCLUSIONS: Single mothers with young children are more predisposed to mental health disorders than partnered mothers, especially when facing financial, social or distal adversities. Appropriate social support programs and screening measures are necessary to reduce further disparities.

Mediating effects of parental psychological distress and individual-level social capital on the association between child poverty and maltreatment in Japan

Abstract:Child poverty is well known as a major risk factor for child maltreatment. However, it is not known whether parental psychological distress and individual-level social capital mediate the association. We examined the mediation effect of these two factors on the association between child poverty and maltreatment. In the Adachi Child Health Impact of Living Difficulty (A-CHILD) Study, a questionnaire was administered to all caregivers of first-grade children in every public elementary school in Adachi City between July and November 2015, and valid responses were used for analysis (N = 3944). Logistic and Poisson regression analyses were employed to examine the association between child poverty and maltreatment. Child poverty was defined in this study as meeting one of these criteria: 1) household income less than 3 million yen; 2) deprivation of specific material items that children or the household requires, or 3) experience of being unable to pay for lifeline utilities. Child maltreatment (physical abuse, neglect, and psychological abuse) was answered by parents. We confirmed a robust association between child poverty and maltreatment. Mediation analysis indicated that parental psychological distress mediated more than 60% of the association between child poverty on physical abuse and psychological abuse, while individual-level social capital mediated only 10% of the association with any type of maltreatment. In addition, structural equation modeling analysis revealed that the association was mediated by both parental psychological distress and social capital simultaneously. The findings suggest that supporting parental psychological distress may be an effective intervention to remedy the negative impact of child poverty on maltreatment.

Abstract:Prior research suggests that raising young children is a stressful stage of childcare, but it remains unclear how the age composition of young children is related to parental well-being. This study examines variations in parental well-being between men and women in Japan, with a special focus on the age composition of young children. Using the 2018 National Family Research of Japan data, we estimated ordinary least-squares regression models to investigate differences in well-being among (1) parents only with infants, (2) parents with infants and preschool children, and (3) parents with one child going to school. The roles of support from spouse, own parents, and parents-in-law were also tested. We found that women report significantly lower levels of well-being when they raise preschool-age children compared to those with school-age children. The effects of raising young children remain strong, even after controlling for familial support. Men's well-being does not vary by the age composition of children. Our results show that the preschool age is the most challenging stage of childcare for Japanese women, but their well-being improves as the child gets older. This finding suggests the importance of providing institutional support to women with young children, such as flexibility concerning parental leave.

Abstract:Objectives The aims of this study were to investigate perceived childcare burdens among mothers with an inter-pregnancy interval of less than 12 months, and to identify the factors associated with those perceptions.Methods Anonymous, self-administered questionnaires on inter-pregnancy intervals, the Edinburgh postnatal depression scale (EPDS), and the childcare emotion scale were distributed to mothers who visited one of three health and welfare centers in City A for three-month well-baby checkups. Six hundred eighty-five subjects were classified into four groups according to their inter-pregnancy interval (<12 months apart; ≥12 months but <24 months apart; ≥24 months apart; and a group with no siblings), and they was used to analyze associations between the timing of pregnancy and the mothers' perceptions of their childcare burdens. Chi-squared tests, a one-way analysis of variance, the Kruskal-Wallis test, and linear regression analyses were used for analysis.Results The inter-pregnancy interval was <12 months in 35 of the 685 respondents, ≥12 months but <24 months in 114 respondents, and ≥24 months in 194 respondents. There were 342 respondents in the no-siblings group. Linear regression analyses disclosed some factors associated with the subordinate items of the childcare emotion scale. "Feeling restricted by childcare duties" is significantly correlated to inter-pregnancy interval (P=.032), family composition (P=.014), time of sleeping (P=.010), and night wake-up time (P=.001). "Feeling of burden because of the child's attitude and behavior" was significantly related to inter-pregnancy interval (P<.001), mother's age (P=.003), time of sleeping (P=.009), and night wake-up time (P=.002). "Anxiety about the child's growth" was significantly related to inter-pregnancy interval (P<.001), mother's age (P=.016), and gestational age (P<.001). In addition, the group with gestation intervals of less than 12 months had significantly higher numbers of women in single-parent households (P=.005), who were unmarried (P=.007), had only a junior high school education (P=.0027), and were 24 years old or younger (P<.001).Conclusion The pregnancy interval is significantly related to the "Feeling restricted by childcare duties," "Feeling of burden because of the child's attitude and behavior," and "Anxiety about the child's growth," even after adjusting the influence of other factors. In addition, the results of this study demonstrated that mothers in the <12 months group were more likely to be in a single-parent household, unmarried, with low educational attainment, and were 24 years old or younger. The results suggest that greater support is needed for these mothers.
Childcare support and child social development in Japan: investigating the mediating role of parental psychological condition and parenting style

Abstract:In humans, support from partners and alloparents is crucial for successful child-rearing and optimal child development. However, the complex relationships among childcare support, children's outcomes and parental characteristics have not been fully examined. We investigate how three sources of partner and alloparental support—partner's childcare participation, support from children's grandparents and support from non-kin—can be associated with child social development. We hypothesize that the associations between childcare support from partners/alloparents and child social development are partly mediated by parental psychological condition and parenting style. To test this, we conducted path analyses on online survey data collected in 2016 from parents of 3- to 5-year-old children in Japan. We found no evidence that childcare support had direct positive effects on child social development. Rather, the benefit of childcare support was mediated by its effects on parental psychological condition and parenting style, which in turn improved children's outcomes. At the same time, we found some evidence that greater availability of childcare support was directly associated with more behavioural difficulties in children. Our findings reveal the complex pathways between childcare support, parental characteristics and children's outcomes in Japan, showing potential mechanisms behind parental and alloparental effects in industrialized populations. Abstract:Mitochondria are probably a target in antiepileptic drug—induced hepatotoxicity accompanied by oxidative stress. Most studies discuss valproic acid. The information regarding other antiepileptic drugs is scarce. Most studies used in vitro methods and animal models. In this study, the authors have investigated the effect of antiepileptic drugs, other than valproic acid, on the oxidative phosphorylation process in children, by measuring mitochondrial adenosine triphosphate (ATP) production and the enzymatic activities of respiratory chain complexes II-IV in peripheral white blood cells. The results demonstrate that several antiepileptic drugs can affect the mitochondrial oxidative phosphorylation. The authors have concluded that the effect of antiepileptic drugs on the mitochondria is not limited only to valproic acid, but can affect different mitochondrial pathways and can be performed in humans by relatively simple methods, using small samples of peripheral white blood cells.

Abstract:In the first comprehensive examination of its kind, we investigated the association between maternal depression and preschoolers' sleep problems, as well as factors that influence children's sleep. A cross-sectional questionnaire survey was administered to mothers of children in nine kindergartens and nursery schools across six Japanese prefectures. The sample included 232 mother–child pairs (children's average age = 61.72 months; 51.29% boys; mothers' average age = 38.38 years). Mothers' depression and parenting behaviors were assessed using the Patient Health Questionnaire-9 and Parenting Behavior Checklist to Promote Preschoolers' Sleep, respectively. Children's sleep problems and emotional and behavioral problems were assessed using the Japanese Sleep Questionnaire for Preschoolers and the Strengths and Difficulties Questionnaire, respectively. Mothers reported on their children's duration of daytime activity and screen time. Thirty-two percent of mothers had more than minimal depressive symptoms. Path analysis revealed that children's emotional and behavioral problems, and sleep problems were associated with maternal depression ( β = 0.24, β = 0.21). Furthermore, children's emotional and behavioral problems, duration of daytime activity, and parenting behaviors to promote children's sleep were associated with children's sleep problems ( β = 0.45, β = − 0.09, β = − 0.45). The values of GFI (0.99), AGFI (0.95), and RMSEA (0.05) showed a good fitness of the model. The results suggest the necessity of assessing children' sleep problems and emotional and behavioral problems when considering mothers' mental health problems in the future.

Did Inequalities in Mothers’ and Children’s Health and Well-Being in Japan Increase through the Pandemic? Evidence from Nationwide Surveys and Routinely Collected Data

Abstract:Marginalised families faced significant challenges during the COVID-19 pandemic. This study explores inequalities in Japanese mothers’ and children’s health and well-being and family economic stability before and during the pandemic. Data sources were as follows: nationwide surveys in 2019 and 2021 of families with children using medical institutions across Japan; infant mortality and adolescent suicide rates between 2018 and 2021 from publicly available sources. Outcomes by poor and non-poor families were compared for 2019 and 2021 using simple descriptive statistics. Poor mothers’ part-time working increased from 41% to 61% and regular employment was reduced by two thirds. The well-being of poor mothers worsened from 39% to 55%. Employment opportunities and well-being did not change for non-poor mothers. School subsidies among poor families increased from 23% to 55%. The infant mortality rate (IMR) among unemployed families increased significantly from 12.9/1000 to 18.2/1000 between 2018 and 2021 compared with a decreasing overall IMR from 1.9/1000 to 1.7/1000. Suicide rates in 10–19-year-olds increased over the same period although no socio-economic indicators were available. Inequalities in mothers’ and children’s health and well-being indicators and family economics increased between 2019 to 2021 in Japan. This study cannot attribute causes but suggests a possible role of the pandemic.

Abstract:CONTEXT Exposure to retail tobacco marketing is associated with youth smoking, but most studies have relied on self-reported measures of exposure, which are prone to recall bias. OBJECTIVE To examine whether exposure to retail cigarette advertising, promotions, and retailer compliance is associated with youth smoking-related outcomes using observational estimates of exposure. DESIGN Data on retail cigarette advertising and promotions were collected from a representative sample of licensed tobacco retailers in New York annually since 2004. County-level estimates of retail cigarette advertising and promotions and retailer compliance with youth access laws were calculated and linked to the New York Youth Tobacco Survey, administered to 54,671 middle and high school students in 2004, 2006, and 2008. Regression models examined whether cigarette advertising, promotions, and retailer compliance were associated with youth's awareness of retail cigarette advertising, attitudes about smoking, susceptibility to smoking, cigarette purchasing behaviors, and smoking behaviors. RESULTS Living in counties with more retail cigarette advertisements is associated with youth having positive attitudes about smoking (odds ratio [OR] = 1.10, 95% confidence interval [CI] = 1.03-1.19, P < .01). Living in counties with more retail cigarette promotions is associated with youth current smoking (OR = 1.57, 95% CI = 1.01-2.44, P < .05). Living in counties with higher retailer compliance with youth access laws is associated with higher odds of youth being refused cigarettes when attempting to buy in stores (OR = 1.12, 95% CI = 1.01-1.25, P < .05) and lower odds of retail stores being youth's usual source of cigarettes (OR = 0.88, 95% CI = 0.80-0.97, P < .01). CONCLUSIONS Strong retailer compliance programs and policies that eliminate cigarette advertising and promotions may help reduce youth smoking.
Domestic work stress and self-rated psychological health among women: a cross-sectional study in Japan

Abstract:Abstract Background Despite the huge burden of domestic work on women in Japan, its effects on their health have been poorly investigated. We aimed to assess the association between domestic work stress and self-rated psychological health among women. Methods We conducted a cross-sectional survey using an online social research panel in February 2018. Participants were 2,000 women with paid work (the “workers” group) and 1,000 women without paid work (the “homemakers” group), aged between 25 and 59 years old and living with a partner. Self-rated psychological health (Mental Health and Vitality scales of the Japanese SF-36), occupational and domestic work stress (the Brief Job Stress Questionnaire), the 10-item Work–Family Conflict Scale, and sociodemographic factors were assessed. Results The workers had lower domestic job control and higher support from a partner and their parents than the homemakers ( p < 0.001), whereas domestic job demand and psychological health were similar between the groups. After adjustment for the covariates using multiple linear regression models, better psychological health was significantly associated with lower domestic job demand, higher domestic job control, and having a young child in both groups. In addition, work–family conflicts and occupational job stress among the workers and caregiving among the homemakers showed negative associations with psychological health. Conclusion Self-rated psychological health in women was associated with domestic work stress regardless of employment status. To promote women’s health, we need to take into account the effects of domestic work, work–family conflicts, and social support from families, as well as occupational factors.

Molecular Phylogenetics and Evolution

Abstract:Nine families of bivalve molluscs have undergone successful radiations in freshwater habitats, including three heterodont taxa: the Sphaeriidae, Corbiculidae, and Dreissenidae. Although the phylogenetic relationships of these freshwater heterodont families are controversial, most workers place the first two in the superfamily Corbiculoidea and assume that they represent a monophyletic grouping. We have tested competing phylogenetic hypotheses for the Corbiculoidea by constructing a representative molecular phylogeny, based on domains D1-D3 of the nuclear large subunit 28S rDNA, for 18 heterodont bivalves and for two oyster outgroup taxa. Our results do not support the monophyly of the Corbiculoidea and are consistent with the hypothesis that all three families of freshwater heterodonts represent independent colonization events by marine ancestors. Similarities in developmental mode specializations exhibited by some sphaeriids and corbiculids, such as sequential direct-developing broods, represent convergent adaptations to the freshwater environment. The corbiculid taxa form a clade with venerid and mactrid outgroups but we were not able to identify a putative marine outgroup for the sphaeriids.
Mothers' parental divorce experience in childhood and their children's mental health: Mediating role of maternal education

Abstract:Background Adverse childhood experiences (ACEs) of mothers have been shown to be associated with children's mental health and behavior problems. It is important to identify effective intervention points to prevent negative consequences among children. This study aimed to investigate whether mothers' education is protective against children's depressive symptoms in families with mothers who experienced parental divorce in childhood. Methods Data from a cross-sectional study of 5th and 8th grade children and their caregivers in a prefecture in Japan were used. Final analytic sample consisted of 9666 child-caregiver pairs. Mediation analyses using inverse odds weighting were performed where the exposure was maternal experience of parental divorce in childhood, outcome was child depressive symptoms, and potential mediators were maternal education beyond high school, maternal mental health, and household income. Results Maternal experience of parental divorce was associated with an elevated risk of child depressive symptoms (risk ratio: 1.22, 95 % confidence interval (CI): 1.07–1.39). Mediation analyses indicated that the combination of maternal education, maternal depression and household income mediated about half of the total effect. In the model where maternal education was the sole mediator, maternal education mediated nearly half of the total effect (risk ratio: 1.10, 95 % CI: 1.01–1.20). Limitations Results should be cautiously interpreted given observational and cross-sectional nature of the data. Conclusions Maternal education beyond high school may be protective against negative mental health consequences among children due to mothers' experience of parental divorce. Further studies are needed regarding potential mechanisms and the roles of other potential mediators.

Abstract:Maternal prenatal and postnatal psychological distress, including depression and anxiety, may affect children's cognitive development. However, the findings have been inconsistent. We aimed to use the dataset from the Japan Environment and Children's Study, a nationwide prospective birth cohort study, to examine this association. We evaluated the relationship between the maternal six-item version of the Kessler Psychological Distress Scale (K6) scores and cognitive development among children aged 4 years. K6 was administered twice during pregnancy (M-T1; first half of pregnancy, M-T2; second half of pregnancy) and 1 year postpartum (C-1y). Cognitive development was assessed by trained testers, using the Kyoto Scale of Psychological Development 2001. Multiple regression analysis was performed with the group with a K6 score ≤ 4 for both M-T1 and M-T2 and C-1y as a reference. Records from 1,630 boys and 1,657 girls were analyzed. In the group with K6 scores ≥ 5 in both M-T1 and M-T2 and C-1Y groups, boys had significantly lower developmental quotients (DQ) in the language-social developmental (L-S) area (partial regression coefficient: −4.09, 95% confidence interval: −6.88 – −1.31), while girls did not differ significantly in DQ for the L-S area. Among boys and girls, those with K6 scores ≤ 4 at any one or two periods during M-T1, M-T2, or C-1y did not have significantly lower DQ for the L-S area. Persistent maternal psychological distress from the first half of pregnancy to 1 year postpartum had a disadvantageous association with verbal cognitive development in boys, but not in girls aged 4 years.

Abstract:Parent–child residential distance is a focus of housing policies in Japan, which is experiencing an aging society. This study aimed to clarify the relationship between mental health and parent–child residential distance for older parents. This study design was a cross-sectional study of older parents. The data utilized were anonymized data from the 2016 Comprehensive Survey of Living Conditions. A two-way ANOVA test set the outcome variable as the K6 scale and the three predictors as age, gender, and parent–child residential distance. A statistically significant difference was found in the interaction between parent–child residential distance and age group. In particular, the average mental score of older parents over 75 years old living together with their children was above the cutoff value indicating serious mental illness. Meanwhile, the mental scores of older parents whose children lived in the same city did not differ significantly between those aged 65 to 74 years old and those aged 75 years and older. This study concludes that mental health is poorer among late-old-stage parents living closer to their children. This conclusion implicated the promotion of parent and child households moving to the same city from the viewpoint of mental health.

Smaller household size and higher prevalence of serious psychological distress in younger people and never-married people: a nationwide cross-sectional survey in Japan

Abstract:Background: Small-member households are increasing worldwide. However, most previous studies have focused on older people and living alone. Using the latest national survey data, we investigated a dose–response relationship between household size and serious psychological distress (SPD). Methods: We analyzed data from the 2019 Comprehensive Survey of Living Conditions in Japan. The study participants were 405,560 community-dwelling adults aged 20 or older. Household size was classified into 5 or more, 3 or 4, two, and one (i.e., living alone). SPD was defined as ≥13 points based on the Kessler 6-item Psychological Distress Scale. We used multivariable logistic regressions and included age, education, equivalent household expenditures, housing tenure, employment contract, smoking, and illness under treatment as covariates. Results: After stratified analyses by age and gender, a dose–response relationship between smaller household size and more common SPD was significant for younger, but not for older people ( p -trend was <0.001 in men aged 20–59 and women aged 20–39). After stratified analyses by gender and marital status, a dose–response relationship was significant only for the never-married group in both genders ( p -trend was <0.001 in never-married men and women). Conclusion: Smaller households were associated with higher prevalence of SPD in younger adults and in never-married individuals, regardless of gender. Our findings suggest a need to focus on younger people and never-married people to reduce the mental health risks due to small household sizes.

Abstract:The participation of women with young children in the Japanese labor force has increased dramatically in recent years, bringing growing potential for conflict between work and family roles amid inadequate social systems, such as childcare support. Thus, work–life balance (WLB) of mothers may influence their children’s mental health and lifestyle. This study aims to clarify the relationship between parents’ WLB and children’s mental health, as well as the underlying factors of parental stress and nurturing attitude. The study is based on a questionnaire survey administered in 2019 to fifth-grade elementary school students and their caregivers in Aichi Prefecture, Japan. The regression results indicated that the higher the work–family negative spillover, the higher the child’s externalizing and internalizing problems, and the higher the positive spillover, the lower the problems and the higher the prosocial behaviors. Path analysis indicates that maternal WLB is negatively and positively related to children’s behavior through maternal stress and parenting practices. The study suggests that maternal WLB is related to children’s emotional and behavioral problems. WLB may impact children’s emotional and behavioral problems through parents’ mental health and involvement with their children, particularly because of work arrangements changing with the COVID-19 pandemic.

Abstract:OBJECTIVE Respiratory syncytial virus (RSV) is the most common cause of severe lower respiratory tract infection in young children and is increasingly recognized as a cause of influenza-like illness in those older than 65 years of age. A surveillance system to provide timely local information about RSV activity in Nova Scotia (NS) is described. METHODS A case report form was developed for weekly reporting of all laboratory isolates of RSV at diagnostic laboratories around the province. Laboratories were asked to send the forms by fax each Friday to the Nova Scotia Department of Health Promotion and Protection. Data were entered in Excel (Microsoft, USA) and aggregate results summarized by age, sex, health district and date of laboratory confirmation for 2005 to 2008. RESULTS During three winter seasons (2005-2006, 2006-2007 and 2007-2008), there were 207, 350 and 186 isolates of RSV reported in NS, respectively. The average incidences of RSV in NS varied greatly by age, with the highest rates in infants younger than 24 months of age, and approaching 4000 cases per 100,000 population in infants up to five months of age. The duration of the RSV outbreak was approximately five to six months each year, but the month of onset varied. CONCLUSIONS A RSV surveillance system was successfully established in NS that provides weekly data to the public health system, clinicians and infection control practitioners. The time of onset and severity of the RSV season varied over time. These data can be used to plan anti-RSV passive prophylaxis programs and infection control education, and distinguish RSV outbreaks from other viruses in acute care and long-term care settings.
