Abstract:Rosacea is a chronic inflammatory skin disorder that causes visible blood vessels and redness on the nose, chin, cheeks, and forehead. However, visual assessment, the current standard method used to identify rosacea, is often subjective among clinicians and results in high variation. Recent advances in artificial intelligence have allowed for the effective detection of various skin diseases with high accuracy and consistency. In this study, we develop a new methodology, coined "five accurate CNNs-based evaluation system (FACES)", to identify and classify rosacea more efficiently. First, 19 CNN-based models that have been widely used for image classification were trained and tested via training and validation data sets. Next, the five best performing models were selected based on accuracy, which served as a weight value for FACES. At the same time, we also applied a majority rule to five selected models to detect rosacea. The results exhibited that the performance of FACES was superior to that of the five individual CNN-based models and the majority rule in terms of accuracy, sensitivity, specificity, and precision. In particular, the accuracy and sensitivity of FACES were the highest, and the specificity and precision were higher than most of the individual models. To improve the performance of our system, future studies must consider patient details, such as age, gender, and race, and perform comparison tests between our model system and clinicians.

Abstract:The 1H-n.m.r. spectra (360 MHz) of 12-(beta-(3-pyridyl)-L-Ala) ribonuclease S-peptide (1-14), a tetradecapeptide incorporating (beta-3-pyridyl-L-Ala) instead of His at position 12, have been assigned. The shift vs. temperature dependence has been analyzed at three different pD's in terms of a two-state helix (3-13) in equilibrium coil equilibrium, and the corresponding values for the thermodynamic quantities delta H degrees and delta S degrees determined. Helix populations at 0 degrees C have been measured as a function of pD, showing their dependence on two apparent pKa's at approximately 3.3 and 5.5, with a maximum at pD approximately 4.2. All the obtained results show that the new peptide has very similar folding properties to those shown by S-peptide and particularly to those of C-peptide. The 3-13 helix formed is stabilized by two interactions: a salt-bridge Glu 2-...Arg 10+ and a partial stacking between the aromatic rings of residues Phe 8 and His 12. Calculations involving ring current shifts and potential energies validate the possible existence of this latter interaction, which must present a local geometry defined by chi 81 180 degrees, chi 82 100 degrees, chi 121-60 and chi 122 80.

Abstract:Rosacea is a common cutaneous disorder characterized by facial redness, swelling, and flushing, and it is usually diagnosed by a dermatologist after a visual examination. Qualitative human assessment often results in relatively high intra- and interobserver variability, which can negatively affect patient outcomes. Computer-assisted image analysis may improve visual assessment by human observers because it enables quantitative, consistent, and accurate analysis. Here, we combine classical multidimensional scaling (MDS) with deep convolutional neural networks (CNNs) to create an efficient framework to identify rosacea lesions. MDS is utilized to determine an appropriate amount of training data, which are used to train Inception-ResNet-v2 to classify facial images into rosacea and non-rosacea regions. Using a leave-one-patient-out cross-validation scheme with 128 128 non-overlapping image patches, the method resulted in a class weighted average Dice coefficient (DC) of 82.1% 2.4% and accuracy of 85.0% 0.6%. While this average performance is almost identical to our previous results (81.7% 2.7% and 84.9% 0.6% for DC and accuracy, respectively), with the new scheme, we use approximately 90% less data to train the system. We also report the results of quantitative experiments with overlapping patches with a stride of 50 pixels. With the same experimental setup, speedups of 25.6 times (128 128), 23.4 times (192 192), and 23.2 times (256 256) have been observed when the network is trained with the entire training data as the baseline. The class weighted average DC for this experiment with the proposed method is 83.9% 2.1% as in the case of 192 192 pixels overlapping patches, while it is 84.4% 2.2% when the entire set is trained at each fold. We conclude that the proposed method can be an efficient way to train deep neural networks using only a small subset of the training data.

Abstract:In this paper, we present a new automatic diagnosis method for facial acne vulgaris which is based on convolutional neural networks (CNNs). To overcome the shortcomings of previous methods which were the inability to classify enough types of acne vulgaris. The core of our method is to extract features of images based on CNNs and achieve classification by classifier. A binary-classifier of skin-and-non-skin is used to detect skin area and a seven-classifier is used to achieve the classification task of facial acne vulgaris and healthy skin. In the experiments, we compare the effectiveness of our CNN and the VGG16 neural network which is pre-trained on the ImageNet data set. We use a ROC curve to evaluate the performance of binary-classifier and use a normalized confusion matrix to evaluate the performance of seven-classifier. The results of our experiments show that the pre-trained VGG16 neural network is effective in extracting features from facial acne vulgaris images. And the features are very useful for the follow-up classifiers. Finally, we try applying the classifiers both based on the pre-trained VGG16 neural network to assist doctors in facial acne vulgaris diagnosis.
A convolutional neural network trained with dermoscopic images of psoriasis performed on par with 230 dermatologists

Abstract:BackgroundPsoriasis is a common chronic inflammatory skin disease that causes physical and psychological burden to patients. A Convolutional Neural Network (CNN) focused on dermoscopic images would substantially aid the classification and increase the accuracy of diagnosis of psoriasis.ObjectivesThis study aimed to train an efficient deep-learning network to recognize dermoscopic images of psoriasis (and other papulosquamous diseases), improving the accuracy of the diagnosis of psoriasis.MethodsEfficientNet-B4 architecture was trained with 7033 dermoscopic images from 1166 patients collected from the Department of Dermatology, Peking Union Medical College Hospital (China). We performed a five-fold cross-validation on the training set to compare the classification performance of EfficientNet-B4 over different networks commonly used in previous studies. From the test set, 90 images were used to compare the performance between our four-class model and that of board-certified dermatologists, whose diagnoses and information (e.g., age, titles) were obtained through an online questionnaire.ResultsThe mean sensitivity and specificity of EfficientNet-B4 on the training set was 0.927± 0.028 and 0.827 ± 0.043 for the two-class task, and 0.889 ± 0.014 and 0.968 ± 0.004 four-class task. The diagnostic sensitivity and specificity of the 230 dermatologists were 0.688 and 0.903 for psoriasis, 0.677 and 0.838 for eczema, 0.669 and 0.953 for lichen planus, and 0.832 and 0.932 for the "others" group, respectively; the diagnostic sensitivity and specificity of our four-class CNN was 0.929 and 0.952 for psoriasis, 0.773 and 0.926 for eczema, 0.933 and 0.960 for lichen planus, and 0.840 and 0.985 for the "others" group, respectively. Both the 230 dermatologists and CNN achieved at least moderate consistency with the reference standard, and there was no significant difference between them (P > 0.05).ConclusionsThe two-classification and four-classification models of psoriasis established in our study could accurately classify papulosquamous skin diseases. They showed generally comparable performances to the average level of dermatologists and would provide a strong support for the diagnosis of psoriasis.

Increasing Rosacea Awareness Among Population Using Deep Learning and Statistical Approaches

Abstract:Approximately 16 million Americans suffer from rosacea according to the National Rosacea Society. To increase rosacea awareness, automatic rosacea detection methods using deep learning and explainable statistical approaches are presented in this paper. The deep learning method applies the ResNet-18 for rosacea detection, and the statistical approaches utilize the means of the two classes, namely, the rosacea class vs. the normal class, and the principal component analysis to extract features from the facial images for automatic rosacea detection. The contributions of the proposed methods are three-fold. First, the proposed methods are able to automatically distinguish patients who are suffering from rosacea from people who are clean of this disease. Second, the statistical approaches address the explainability issue that allows doctors and patients to understand and trust the results. And finally, the proposed methods will not only help increase rosacea awareness in the general population but also help remind the patients who suffer from this disease of possible early treatment since rosacea is more treatable at its early stages. We analyze the results of skin physiology and dermatoscopy using machine learning method and identify the characteristics of acne rosacea, which differentiate it from other common facial inflammatory skin diseases so as to improve the accuracy of clinical and differential diagnosis of rosacea. Methods: A total of 495 patients who were jointly diagnosed by two experienced doctors were included. Basic data, clinical symptoms, physiological skin detection, and dermatoscopy results were collected, and the clinical characteristics of rosacea and other common facial inflammatory diseases were summarized according to the descriptive analysis results. The model was established using a machine learning method and compared with the judgment results of young and inexperienced doctors to verify whether the model can improve the accuracy of clinical diagnosis and differential diagnosis of rosacea. Results: The proportion of yellow and red halos, vascular polygons, as well as follicular pustules, showed by dermatoscopy, and the melanin index in physiological skin detection revealed statistical significance in differentiating rosacea and other common facial inflammatory diseases (all P < 0.01). After adopting the machine learning, we found that GBM (Gradient Boosting Machine) algorithm was the best, and the error rate of this model in the validation set was 5.48%. In the final man-machine comparison, the accuracy of the GBM algorithm model for the classification of skin disease was significantly higher than that of young and inexperienced doctors. Conclusion: Dermatoscopy combined with machine learning can effectively improve the diagnosis and differential diagnosis accuracy of rosacea and other facial inflammatory skin diseases. It mainly involves facial nerves and vessels as well as the sebaceous gland unit of the hair follicle. The main clinical manifestations of rosacea are intermittent flushing, persistent erythema, papules, pustules, and telangiectasia. Hypertrophy and eye changes have also been reported in a few patients. However, the clinical presentation of acne is similar to that of rosacea, with acne commonly appearing on the face and shoulders, and skin changes including intermittent flushing, papules and pustules. 1–3 The diagnosis of rosacea is clinically challenging. A Deep Learning Based Framework for Diagnosing Multiple Skin Diseases in a Clinical Environment

Abstract:Background: Numerous studies have attempted to apply artificial intelligence (AI) in the dermatological field, mainly on the classification and segmentation of various dermatoses. However, researches under real clinical settings are scarce. Objectives: This study was aimed to construct a novel framework based on deep learning trained by a dataset that represented the real clinical environment in a tertiary class hospital in China, for better adaptation of the AI application in clinical practice among Asian patients. Methods: Our dataset was composed of 13,603 dermatologist-labeled dermoscopic images, containing 14 categories of diseases, namely lichen planus (LP), rosacea (Rosa), viral warts (VW), acne vulgaris (AV), keloid and hypertrophic scar (KAHS), eczema and dermatitis (EAD), dermatofibroma (DF), seborrheic dermatitis (SD), seborrheic keratosis (SK), melanocytic nevus (MN), hemangioma (Hem), psoriasis (Pso), port wine stain (PWS), and basal cell carcinoma (BCC). In this study, we applied Google's EfficientNet-b4 with pre-trained weights on ImageNet as the backbone of our CNN architecture. The final fully-connected classification layer was replaced with 14 output neurons. We added seven auxiliary classifiers to each of the intermediate layer groups. The modified model was retrained with our dataset and implemented using Pytorch. We constructed saliency maps to visualize our network's attention area of input images for its prediction. To explore the visual characteristics of different clinical classes, we also examined the internal image features learned by the proposed framework using t-SNE (t-distributed Stochastic Neighbor Embedding). Results: Test results showed that the proposed framework achieved a high level of classification performance with an overall accuracy of 0.948, a sensitivity of 0.934 and a specificity of 0.950. We also compared the performance of our algorithm with three most widely used CNN models which showed our model outperformed existing models with the highest area under curve (AUC) of 0.985. We further compared this model with 280 board-certificated dermatologists, and results showed a comparable performance level in an 8-class diagnostic task. Conclusions: The proposed framework retrained by the dataset that represented the real clinical environment in our department could accurately classify most common dermatoses that we encountered during outpatient practice including infectious and inflammatory dermatoses, benign and malignant cutaneous tumors.

A Computer Vision Application for Assessing Facial Acne Severity from Selfie Images

Abstract:We worked with Nestle SHIELD (Skin Health, Innovation, Education, and Longevity Development, NSH) to develop a deep learning model that is able to assess acne severity from selfie images as accurate as dermatologists. The model was deployed as a mobile application, providing patients an easy way to assess and track the progress of their acne treatment. NSH acquired 4,700 selfie images for this study and recruited 11 internal dermatologists to label them in five categories: 1-Clear, 2- Almost Clear, 3-Mild, 4-Moderate, 5-Severe. Using OpenCV to detect facial landmarks we cut specific skin patches from the selfie images in order to minimize irrelevant background. We then applied a transfer learning approach by extracting features from the patches using a ResNet 152 pre-trained model, followed by a fully connected layer trained to approximate the desired severity rating. To address the problem of spatial sensitivity of CNN models, we introduce a new image rolling data augmentation approach, effectively causing acne lesions appeared in more locations in the training images. Our results demonstrate that this approach improved the generalization of the CNN model, outperforming more than half of the panel of human dermatologists on test images. To our knowledge, this is the first deep learning-based solution for acne assessment using selfie images.
High-Fidelity Synthetic Face Generation for Rosacea Skin Condition from Limited Data

Abstract:Similarly to the majority of deep learning applications, diagnosing skin diseases using computer vision and deep learning often requires a large volume of data. However, obtaining sufficient data for particular types of facial skin conditions can be difficult, due to privacy concerns. As a result, conditions like rosacea are often understudied in computer-aided diagnosis. The limited availability of data for facial skin conditions has led to the investigation of alternative methods of computer-aided diagnosis. In recent years, generative adversarial networks (GANs), mainly variants of StyleGANs, have demonstrated promising results in generating synthetic facial images. In this study, for the first time, a small dataset of rosacea with 300 full-face images was utilized to further investigate the possibility of generating synthetic data. Our experimentation demonstrated that the strength of R1 regularization is crucial for generating high-fidelity rosacea images using a few hundred images. This was complemented by various experimental settings to ensure model convergence. We successfully generated 300 high-quality synthetic images, significantly contributing to the limited pool of rosacea images for computer-aided diagnosis. Additionally, our qualitative evaluations by 3 expert dermatologists and 23 non-specialists highlighted the realistic portrayal of rosacea features in the synthetic images. We also provide a critical analysis of the quantitative evaluations and discuss the limitations of solely relying on validation metrics in the field of computer-aided clinical image diagnosis.

Abstract:Skin disease is one of the most common types of human diseases, which may happen to everyone regardless of age, gender or race. Due to the high visual diversity, human diagnosis highly relies on personal experience; and there is a serious shortage of experienced dermatologists in many countries. To alleviate this problem, computer-aided diagnosis with state-of-the-art (SOTA) machine learning techniques would be a promising solution. In this paper, we aim at understanding the performance of convolutional neural network (CNN) based approaches. We first build two versions of skin disease datasets from Internet images: (a) Skin -10, which contains 10 common classes of skin disease with a total of 10,218 images; (b) Skin -100, which is a larger dataset that consists of 19,807 images of 100 skin disease classes. Based on these datasets, we benchmark several SOTA CNN models and show that the accuracy of skin -100 is much lower than the accuracy of skin -10. We then implement an ensemble method based on several CNN models and achieve the best accuracy of 79.01% for Skin -10 and 53.54% for Skin -100. We also present an object detection based approach by introducing bounding boxes into the Skin -10 dataset. Our results show that object detection can help improve the accuracy of some skin disease classes.

Abstract:INTRODUCTION: Dermatological disorders, particularly human skin diseases, have become more common in recent decades. Environmental factors, socioeconomic problems, a lack of a balanced diet, and other variables have all contributed to an increase in skin diseases in recent years. Skin diseases can cause psychological suffering in addition to physical injury, especially in people with scarred or disfigured faces. OBJECTIVES: The use of artificial intelligence or computer-based technologies in the detection of face skin disorders has advanced dramatically over time. Even for highly experienced doctors and dermatologists, identifying skin disorders can be tricky since many skin diseases have a visual affinity with the surrounding skin and lesions. METHODS: Today, the majority of skincare specialists rely on time-consuming, traditional methods to identify disorders. Even though several research have demonstrated promising results on the picture classification job, few studies compare well-known deep learning models with various metrics for categorizing human skin disorders. RESULTS: This study examines and contrasts various skin illnesses in terms of cosmetics and common skin concerns. Our dataset includes over 25000 of the eight most common skin disorders. Convolutional neural networks have shown imaging performance that is comparable to or greater than that of humans. We used 11 different network algorithms to identify the illnesses in the sample and compared the results. CONCLUSION: To adjust the format of incoming photographs, we do certain image pre-processing and image scaling for each model. ResNet152 beat other deep learning methods in terms of recall, accuracy, and precision on a test dataset of 1930 images.
Convolutional neural network assistance significantly improves dermatologists’ diagnosis of cutaneous tumours using clinical images

Abstract:BackgroundConvolutional neural networks (CNNs) have demonstrated expert-level performance in cutaneous tumour classification using clinical images, but most previous studies have focused on dermatologist-versus-CNN comparisons rather than their combination. The objective of our study was to evaluate the potential impact of CNN assistance on dermatologists for clinical image interpretation.MethodsA multi-class CNN was trained and validated using a dataset of 25,773 clinical images comprising 10 categories of cutaneous tumours. The CNN's performance was tested on an independent dataset of 2107 images. A total of 400 images (40 per category) were randomly selected from the test dataset. A fully crossed, self-control, multi-reader multi-case (MRMC) study was conducted to compare the performance of 18 board-certified dermatologists (experience: 13/18 ≤ 10 years; 5/18>10 years) in interpreting the 400 clinical images with or without CNN assistance.ResultsThe CNN achieved an overall accuracy of 78.45% and kappa of 0.73 in the classification of 10 types of cutaneous tumours on 2107 images. CNN-assisted dermatologists achieved a higher accuracy (76.60% vs. 62.78%, P < 0.001) and kappa (0.74 vs. 0.59, P < 0.001) than unassisted dermatologists in interpreting the 400 clinical images. Dermatologists with less experience benefited more from CNN assistance. At the binary classification level (malignant or benign), the sensitivity (89.56% vs. 83.21%, P < 0.001) and specificity (87.90% vs. 80.92%, P < 0.001) of dermatologists with CNN assistance were also significantly improved than those without.ConclusionsCNN assistance improved dermatologist accuracy in interpreting cutaneous tumours and could further boost the acceptance of this new technique.

Abstract:<h3>Importance</h3><p>Convolutional neural networks (CNNs) achieve expert-level accuracy in the diagnosis of pigmented melanocytic lesions. However, the most common types of skin cancer are nonpigmented and nonmelanocytic, and are more difficult to diagnose.</p><h3>Objective</h3><p>To compare the accuracy of a CNN-based classifier with that of physicians with different levels of experience.</p><h3>Design, Setting, and Participants</h3><p>A CNN-based classification model was trained on 7895 dermoscopic and 5829 close-up images of lesions excised at a primary skin cancer clinic between January 1, 2008, and July 13, 2017, for a combined evaluation of both imaging methods. The combined CNN (cCNN) was tested on a set of 2072 unknown cases and compared with results from 95 human raters who were medical personnel, including 62 board-certified dermatologists, with different experience in dermoscopy.</p><h3>Main Outcomes and Measures</h3><p>The proportions of correct specific diagnoses and the accuracy to differentiate between benign and malignant lesions measured as an area under the receiver operating characteristic curve served as main outcome measures.</p><h3>Results</h3><p>Among 95 human raters (51.6% female; mean age, 43.4 years; 95% CI, 41.0-45.7 years), the participants were divided into 3 groups (according to years of experience with dermoscopy): beginner raters (<3 years), intermediate raters (3-10 years), or expert raters (>10 years). The area under the receiver operating characteristic curve of the trained cCNN was higher than human ratings (0.742; 95% CI, 0.729-0.755 vs 0.695; 95% CI, 0.676-0.713;<i>P</i> < .001). The specificity was fixed at the mean level of human raters (51.3%), and therefore the sensitivity of the cCNN (80.5%; 95% CI, 79.0%-82.1%) was higher than that of human raters (77.6%; 95% CI, 74.7%-80.5%). The cCNN achieved a higher percentage of correct specific diagnoses compared with human raters (37.6%; 95% CI, 36.6%-38.4% vs 33.5%; 95% CI, 31.5%-35.6%;<i>P</i> = .001) but not compared with experts (37.3%; 95% CI, 35.7%-38.8% vs 40.0%; 95% CI, 37.0%-43.0%;<i>P</i> = .18).</p><h3>Conclusions and Relevance</h3><p>Neural networks are able to classify dermoscopic and close-up images of nonpigmented lesions as accurately as human experts in an experimental setting.</p>

Construction of a molecular diagnostic system for neurogenic rosacea by combining transcriptome sequencing and machine learning

Abstract:Patients with neurogenic rosacea (NR) frequently demonstrate pronounced neurological manifestations, often unresponsive to conventional therapeutic approaches. A molecular-level understanding and diagnosis of this patient cohort could significantly guide clinical interventions. In this study, we amalgamated our sequencing data (n = 46) with a publicly accessible database (n = 38) to perform an unsupervised cluster analysis of the integrated dataset. The eighty-four rosacea patients were partitioned into two distinct clusters. Neurovascular biomarkers were found to be elevated in cluster 1 compared to cluster 2. Pathways in cluster 1 were predominantly involved in neurotransmitter synthesis, transmission, and functionality, whereas cluster 2 pathways were centered on inflammation-related processes. Differential gene expression analysis and WGCNA were employed to delineate the characteristic gene sets of the two clusters. Subsequently, a diagnostic model was constructed from the identified gene sets using linear regression methodologies. The model's C index, comprising genes PNPLA3, CUX2, PLIN2, and HMGCR, achieved a remarkable value of 0.9683, with an area under the curve (AUC) for the training cohort's nomogram of 0.9376. Clinical characteristics from our dataset (n = 46) were assessed by three seasoned dermatologists, forming the NR validation cohort (NR, n = 18; non-neurogenic rosacea, n = 28). Upon application of our model to NR diagnosis, the model's AUC value reached 0.9023. Finally, potential therapeutic candidates for both patient groups were predicted via the Connectivity Map. In summation, this study unveiled two clusters with unique molecular phenotypes within rosacea, leading to the development of a precise diagnostic model instrumental in NR diagnosis.
Outlier detection in dermatology: Performance of different convolutional neural networks for binary classification of inflammatory skin diseases

Abstract:Background: Artificial intelligence (AI) and convolutional neural networks (CNNs) represent rising trends in modern medicine. However, comprehensive data on the performance of AI practices in clinical dermatologic images are non-existent. Furthermore, the role of professional data selection for training remains unknown. Objectives: The aims of this study were to develop AI applications for outlier detection of dermatological pathologies, to evaluate CNN architectures' performance on dermatological images and to investigate the role of professional pre-processing of the training data, serving as one of the first anchor points regarding data selection criteria in dermatological AI-based binary classification tasks of non-melanoma pathologies. Methods: Six state-of-the-art CNN architectures were evaluated for their accuracy, sensitivity and specificity for five dermatological diseases and using five data subsets, including data selected by two dermatologists, one with 5 and the other with 11 years of clinical experience. Results: Overall, 150 CNNs were evaluated on up to 4051 clinical images. The best accuracy was reached for onychomycosis (accuracy = 1.000), followed by bullous pemphigoid (accuracy = 0.951) and lupus erythematosus (accuracy = 0.912). The CNNs InceptionV3, Xception and ResNet50 achieved the best accuracy in 9, 8 and 6 out of 25 data sets, respectively (36.0%, 32.0% and 24.0%). On average, the data set provided by the senior physician and the data set provided in accordance with both dermatologists performed the best (accuracy = 0.910). Conclusions: This AI approach for the detection of outliers in dermatological diagnoses represents one of the first studies to evaluate the performance of different CNNs for binary decisions in clinical non-dermatoscopic images of a variety of dermatological diseases other than melanoma. The selection of images by an experienced dermatologist during pre-processing had substantial benefits for the performance of the CNNs. These comparative results might guide future AI approaches to dermatology diagnostics, and the evaluated CNNs might be applicable for the future training of dermatology residents.
Differentiation of Active Corneal Infections from Healed Scars Using Deep Learning

Abstract:Purpose: To develop and evaluate an automated, portable algorithm to differentiate active corneal ulcers from healed scars using only external photographs. Design: A convolutional neural network was trained and tested using photographs of corneal ulcers and scars. Participants: De-identified photographs of corneal ulcers were obtained from the Steroids for Corneal Ulcers Trial (SCUT), Mycotic Ulcer Treatment Trial (MUTT), and Byers Eye Institute at Stanford University. Methods: Photographs of corneal ulcers (n = 1313) and scars (n = 1132) from the SCUT and MUTT were used to train a convolutional neural network (CNN). The CNN was tested on 2 different patient populations from eye clinics in India (n = 200) and the Byers Eye Institute at Stanford University (n = 101). Accuracy was evaluated against gold standard clinical classifications. Feature importances for the trained model were visualized using gradient-weighted class activation mapping. Main outcome measures: Accuracy of the CNN was assessed via F1 score. The area under the receiver operating characteristic (ROC) curve (AUC) was used to measure the precision-recall trade-off. Results: The CNN correctly classified 115 of 123 active ulcers and 65 of 77 scars in patients with corneal ulcer from India (F1 score, 92.0% [95% confidence interval (CI), 88.2%-95.8%]; sensitivity, 93.5% [95% CI, 89.1%-97.9%]; specificity, 84.42% [95% CI, 79.42%-89.42%]; ROC: AUC, 0.9731). The CNN correctly classified 43 of 55 active ulcers and 42 of 46 scars in patients with corneal ulcers from Northern California (F1 score, 84.3% [95% CI, 77.2%-91.4%]; sensitivity, 78.2% [95% CI, 67.3%-89.1%]; specificity, 91.3% [95% CI, 85.8%-96.8%]; ROC: AUC, 0.9474). The CNN visualizations correlated with clinically relevant features such as corneal infiltrate, hypopyon, and conjunctival injection. Conclusions: The CNN classified corneal ulcers and scars with high accuracy and generalized to patient populations outside of its training data. The CNN focused on clinically relevant features when it made a diagnosis. The CNN demonstrated potential as an inexpensive diagnostic approach that may aid triage in communities with limited access to eye care.

Abstract:Skin disease has lower impact on mortality compared to others but instead it has greater effect on quality of life because it involves symptoms such as pain, stinging and itchiness. Psoriasis is one of the ordinary skin diseases which are relapsing, chronic and immune-mediated inflammatory disease. It is estimated about 125 million people worldwide being infected with various types of skin infection. Challenges arise when patients only predict the skin type disease they had without being accurately and precisely examined. This is because as human being, they only observe and look at the diseases on the surface of the skin with their naked eye, where there are some limits, for example, human vision lacks of accuracy, reproducibility and quantification in the collection of image information. As Plaque and Guttate are the most common Psoriasis skin disease happened among people, this paper presents an evaluation of Psoriasis skin disease classification using Convolutional Neural Network. A total of 187 images which consist of 82 images for Plaque Psoriasis and 105 images for Guttate Psoriasis has been used which are retrieved from Psoriasis Image Library, International Psoriasis Council (IPC) and DermNet NZ. Convolutional Neural Network (CNN) is applied in extracting features and analysing the classification of Psoriasis skin disease. This paper showed the promising used of CNN with the accuracy rate of 82.9% and 72.4% for Plaque and Guttate Psoriasis skin disease, respectively.

Abstract:A new species of the subgenus Rhodobaetis Jacob, 2003, namely Baetis (Rhodobaetis) vadimi sp. nov., is described based on immature larvae from several localities in Georgia and Turkey. It is the ninth species of the subgenus for Transcaucasia, following six species formally recorded from Turkey and two endemic species described from the Central Caucasus. Larval characters are investigated and illustrated by means of scanning electron microscopy, and discussed in details. The differential diagnosis of this species is provided with regard to other representatives of the subgenus considering the feature set common for Rhodobaetis. The new species can be distinguished from its relatives (species which lack stout setae on the gills' margins), mainly by some characters of mouthparts (particularly labium), the shape and arrangement of stout setae and scales on the body surface, as well as the lack of subapical tiny setae on tarsal claw, and presence of numerous marginal spines and semilunar scale bases on the paraproct plate.
