Abstract:In urinary surgery, flexible ureteroscope plays the role of the eye to provide surgical vision. In robotic assisted surgery systems (urological surgery), the robot needs to move forward with a flexible ureteroscope, so the path of the flexible ureteroscope is crucial. In this paper, the path planning of flexible ureteroscope is based on CT image. Firstly, CT image is preprocessed to improve image quality, including image filtering and image segmentation, then three-dimensional reconstruction of CT image is used to realize visualization. After CT image preprocess and 3D reconstruction, the boundary distance algorithm is used to extract the voxel points, and the voxel points are connected to obtain the centerline. The centerline is very tortuous due to the large number of voxel points and the sharp change of direction. It is proposed that the centerline is smoothed by sampling the voxel points and angle constraints. At the same time, aiming at the limited bending degree when the turning angle of flexible ureteroscope is too large, the curvature is used to constrain the centerline, so that the flexible ureteroscope can move according to the planned path. By testing on the CT images of model and real human body, the approach requirements of flexible ureteroscope can be completely met.
Path Planning for Autonomous Driving with Curvature-considered Quadratic Optimization.

Abstract:Path planning is a crucial module in motion planning for autonomous driving, aiming at generating kinematically feasible and collision-free paths. Furthermore, the smoothness of generated path is significant for passengers’ comfortable feelings. In this paper, we propose an improved quadratic programming approach that generates optimal paths in urban structure scenarios with the Frenét frame, taking the cost of the path curvature into consideration explicitly. The proposed second-order Taylor-expansion estimation of the path curvature with the lateral spatial parameters can reflect the actual change of path curvature. Various simulated scenarios verify the effectiveness of our proposed method and the improvement of path quality by adding the curvature objective in the optimization procedure. The source code is released as an open-source package for the community.
Autonomous Trajectory Planning Based on Bézier Curve with Curvature Constraints and Piecewise-Jerk Speed-Time Optimization

Abstract:In this paper, we propose an efficient trajectory planning algorithm with path smoothing based on the Bézier curve with curvature constraints and piecewise-jerk speed-time optimization. We use hybrid A* to generate a rough path and construct a safe corridor by inflating the path. After that, we formulate the smooth problem as a nonlinear programming(NLP) with piecewise Bézier curves. Since the curvature constraints for Bézier curves are difficult, we employ quartic Bézier Curves with special forms and compute the closed-form solution for the maximum curvature to simplify the representation of the maximum curvature. By using the special Bézier curves, we realize the gear shifts and easily guarantee the security, continuity, and feasibility of the path. Meanwhile, we add time variables based on PJSO, improving the quality of trajectory within an acceptable increase in time, making the allocation of time and speed better. Simulation and real-world experiments with a car-like robot in various environments confirm that our algorithm can generate a smooth, feasible, and high-quality trajectory for robots.
Robust Path Planning via Learning from Demonstrations for Robotic Catheters in Deformable Environments

Abstract:Objective: Navigation through tortuous and deformable vessels using catheters with limited steering capability underscores the need for reliable path planning. State-of-the-art path planners do not fully account for the deformable nature of the environment. Methods: This work proposes a robust path planner via a learning from demonstrations method, named Curriculum Generative Adversarial Imitation Learning (C-GAIL). This path planning framework takes into account the interaction between steerable catheters and vessel walls and the deformable property of vessels. Results: In-silico comparative experiments show that the proposed network achieves a 38% higher success rate in static environments and 17% higher in dynamic environments compared to a state-of-the-art approach based on GAIL. In-vitro validation experiments indicate that the path generated by the proposed C-GAIL path planner achieves a targeting error of 1.26$\pm$0.55mm and a tracking error of 5.18$\pm$3.48mm. These results represent improvements of 41% and 40% over the conventional centerline-following technique for targeting error and tracking error, respectively. Conclusion: The proposed C-GAIL path planner outperforms the state-of-the-art GAIL approach. The in-vitro validation experiments demonstrate that the path generated by the proposed C-GAIL path planner aligns better with the actual steering capability of the pneumatic artificial muscle-driven catheter utilized in this study. Therefore, the proposed approach can provide enhanced support to the user in navigating the catheter towards the target with greater accuracy, effectively meeting clinical accuracy requirements. Significance: The proposed path planning framework exhibits superior performance in managing uncertainty associated with vessel deformation, thereby resulting in lower tracking errors.

Abstract:Flexible-needle percutaneous puncture is a widely used medical technique in clinical practice and is known for its advantages of minimal patient, fast recovery, and ease of operation. However, ensuring accuracy and avoiding obstacles during the puncture process are challenging because of the nonholonomic motion of the needle tip and the presence of obstacles and sensitive organs. To address this issue, this study proposed a novel path-planning algorithm for robot-assisted flexible-needle punctures. The algorithm is based on a bidirectional rapidly exploring random tree and incorporates target-oriented and rapid-search strategies. This provides a feasible approach for the clinical application of flexible-needle puncture, outperforming commonly used algorithms in terms of computational speed, path representation, and search capabilities. Additionally, this study applies a duty-cycling control strategy for a flexible needle, enabling proportional control of the curvature of the needle trajectory during soft tissue insertion. The effectiveness of the proposed path planning algorithm was validated through experiments using a duty-cycling strategy, and an experimental error within 4 mm was deemed adequate for most clinical needle-insertion applications.

Abstract:In robot-assisted needle-based medical procedures, path planning for a flexible needle is challenging with regard to time consumption and searching robustness for the solution due to the nonholonomic motion of the needle tip and the presence of anatomic obstacles and sensitive organs in the intended needle path. We propose a novel and fast path planning algorithm for a robot-assisted active flexible needle. The algorithm is based on Rapidly-Exploring Random Trees combined with reachability-guided strategy and greedy heuristic strategy. Linear segments are taken into consideration to the paths, and insertion orientations are relaxed by the introduction of the linear segments. The proposed algorithm yields superior results as compared to the commonly used algorithm in terms of computational speed, form of path and robustness of searching ability, which potentially can make it suitable for the real-time intraoperative planning for clinical procedures.

Abstract:In the field of minimally invasive surgery, flexible needles can avoid blood vessels and organs more flexibly compared to rigid needles. One of the main challenges when using flexible needles to reach lesions is planning a suitable path. Due to the non-holonomic characteristic of the flexible needle dynamics and the tissue deformation caused by the needle tip during the insertion, the accessibility and safety of the needle’s states need to be considered in the path planning stage. In this article, we propose an adaptable algorithm by improving the canonical rapidly exploring random trees* (RRT*) algorithm to compute a path for the flexible needle to reach targets in a layered tissue environment. The improved RRT* algorithm that addresses the motion constraints of the flexible needle renders the computed path comparatively smoother and optimal in some approximation sense. In the proposed algorithm, a strategy of adapting some of its parameters for different tissues during the insertion is developed, which improves the safety of surgeries. Moreover, the path cost used in the algorithm takes the potential fields of surrounding obstacles into account, which is used to deal with the influence of the local movement of tissues during the needle puncture process. Simulations are conducted to verify the effectiveness of the proposed algorithm. The results show that the improved RRT* algorithm generates a smooth and safe path which satisfies the motion constraints of the flexible needle in layered tissue environment.

Abstract:In clinic it is of very practical significance to optimize the entry point and pose in path planning. We proposed a reverse path planning algorithm adopting multiform combined paths based on the improved kinematic model of flexible needle, and the objective function is established. Utilizing the reversibility of the path, we started from the target to optimize the whole path including the entry point and pose. Then we optimally calculated and simulated in the environment with obstacles. Results show that this algorithm effectively makes the needle steer clear of obstacles to reach the target precisely, and gains the entry point and pose at the same time, guaranteeing the optimal path.

The direct, precise infusion of drugs in the proximity of a tumor has been shown to enhance its effectiveness and diffusion in the surrounding tissue (Vogelbaum and Aghi in Neuro-Oncology 17(suppl 2):ii3-ii8, 2015. https://doi.org/10.1093/neuonc/nou354 ). However, planning for an appropriate insertion trajectory for needles such as the one proposed by EDEN2020 is challenging due to factors like kinematic constraints, the presence of complex anatomical structures such as brain vessels, and constraints on the required start and target poses. Methods: We propose a new parallelizable three-dimensional (3D) path planning approach called Adaptive Hermite Fractal Tree (AHFT), which is able to generate 3D obstacle-free trajectories that satisfy curvature constraints given a specified start and target pose. The AHFT combines the Adaptive Fractal Tree algorithm's efficiency (Liu et al. in IEEE Robot Autom Lett 1(2):601-608, 2016. https://doi.org/10.1109/LRA.2016.2528292 ) with optimized geometric Hermite (Yong and Cheng in Comput Aided Geom Des 21(3):281-301, 2004. https://doi.org/10.1016/j.cagd.2003.08.003 ) curves, which are able to handle heading constraints. Results: Simulated results demonstrate the robustness of the AHFT to perturbations of the target position and target heading. Additionally, a simulated preoperative environment, where the surgeon is able to select a desired entry pose on the patient's skull, confirms the ability of the method to generate multiple feasible trajectories for a patient-specific case. Conclusions: The AHFT method can be adopted in any field of application where a 3D path planner with kinematic and heading constraints on both start and end poses is required.

Abstract:Path planning algorithms for steerable needles in medical applications must guarantee the anatomical obstacle avoidance, reduce the insertion length, and ensure the compliance with the needle kinematics. The majority of the solutions from the literature focus either on fast computation or on path optimality, the former at the expense of suboptimal paths, the latter by making unbearable the computation in case of a high-dimensional workspace. In this article, we implement a three-dimensional path planner for neurosurgical applications, which keeps the computational cost consistent with standard preoperative planning algorithms and fine-tunes the estimated pathways in accordance to multiple optimization objectives. From a user-defined entry point, our method confines a sample-based path search within a subsection of the original workspace considering the degree of curvature admitted by the needle. An evolutionary optimization procedure is used to maximize the obstacle avoidance and reduce the insertion length. The pool of optimized solutions is examined through a cost function to determine the best path. Simulations on one dataset showed the ability of the planner to save time and overcome the state of the art in terms of obstacle avoidance, insertion length, and probability of failure, proving this algorithm as a valid planning method for complex environments.

A Novel Model-Based Path Planning Method for Robot-Assisted Flexible Needle Insertion

Abstract:In needle insertion procedures, path planning is crucial to the success of the operation. In this paper, a preoperative path planning algorithm is proposed that considers the needle-tissue interactions for flexible needle insertion operations. Vector Form Intrinsic Finite Element (VFIFE) and Finite Element Method (FEM) are used to calculate the deformation of the needle and tissue, respectively. The non-linearity of the needle and the change of boundary conditions during the insertion process can be integrated easily. The Potential Field-guided Rapidly-Exploring Random Trees (PF-RRT) is applied to generate the initial path set, in which the candidate path will be selected. The needle control sequence that is to generate the optimal path is obtained from the selected candidate path by combining Iteration Learning Control (ILC) method with the needle-tissue interaction model. The simulation results show that the proposed method is effective to generate candidate needle insertion paths that consider needle-tissue interactions.

Abstract:ABSTRACT Background Saturated biopsy is widely used in a histopathological examination of prostate biopsy surgery by expanding the target regions and the increasing the number of insertions. Methods The paper proposed a path planning algorithm for multiple targets with a single entry for a cannula flexible needle with variable curvatures. The algorithm is based on modified rapidly‐exploring random trees (RRTs), and the variable‐curvature motions of the flexible needle are used to achieve optimal path. Results The simulation results show that the algorithm has a better form of the paths. The experimental results of five trials showed that the error was 1.42 ± 1.57 mm (mean ± RMSE), and the maximum error was 1.8 mm, which proves high accuracy and effectiveness of the proposed path planning algorithm for multiple targets with a single entry. Conclusions This research provides a new solution for prostate saturated biopsy surgery and has high value in future clinical application.

Abstract:BACKGROUND:In robot-assisted flexible ureteroscopy, planning a safety-enhanced path facilitates ureteroscope reaching the target safely and quickly. However, current methods rarely consider the safety impact caused by body motion of the anterior part, such as impingement on the lumen wall and sweeping motion risk, or the method can only be used in collision-free situations. METHODS:The kinematic model of the anterior part under C-shaped and S-shaped collision bending is first analysed. Considering the newly defined impingement cost and sweeping area, the differential evolution algorithm is adopted to optimise the path in the configuration space. Experiments were performed to verify the effectiveness of the method. RESULTS:Compared with the competing algorithm, the proposed algorithm reduced impingement cost and sweeping area by an average of 31.0% and 8.64%. Force measurement experiments verified the rationality of the impingement cost expression. CONCLUSION:The experimental results proved the feasibility of the proposed path planning algorithm.
Path Planning for a Prostate Intervention Robot Based on an Improved Bi-RRT Algorithm

Abstract:In the medical field, the application of robots must primarily ensure safety. An improved path-planning method based on bidirectional rapidly exploring random tree (Bi-RRT) is proposed to enable prostate intervention robots to avoid obstacles in unstructured natural environments. To address the limitations of the existing Bi-RRT algorithm, in our method, we introduce a greedy expansion strategy with dynamic nodes as the target and add a start point repulsion function to the random sampling process as well as sampling space limitations, resulting in a significant improvement in the sampling speed and efficiency. The RRT* parent node reselection and rewiring strategy is introduced to replace the original method to reduce the path cost. To remove redundant nodes and generate a smooth, curved, and continuous robot-executable path, a path optimization strategy based on a fusion of pruning functions, isometric interpolation, and cubic B-spline interpolation is proposed. Finally, a comparative simulation study of the different algorithms using MATLAB and realistic robot obstacle avoidance experiments in an unstructured natural environment is carried out to verify the effectiveness of the proposed method. The method can boost the speed of path planning, decrease the path cost, and create smooth paths that avoid obstacles.
Planning Nonlinear Access Paths for Temporal Bone Surgery

Abstract:Purpose: Interventions at the otobasis operate in the narrow region of the temporal bone where several highly sensitive organs define obstacles with minimal clearance for surgical instruments. Nonlinear trajectories for potential minimally-invasive interventions can provide larger distances to risk structures and optimized orientations of surgical instruments, thus improving clinical outcomes when compared to existing linear approaches. In this paper, we present fast and accurate planning methods for such nonlinear access paths. Methods: We define a specific motion planning problem in SE(3) = R3 x SO(3) with notable constraints in computation time and goal pose that reflect the requirements of temporal bone <a class="link-external link-http" href="http://surgery.We" rel="external noopener nofollow">this http URL</a> then present k-RRT-Connect: two suitable motion planners based on bidirectional Rapidly-exploring Random Trees (RRT) to solve this problem efficiently. Results: The benefits of k-RRT-Connect are demonstrated on real CT data of patients. Their general performance is shown on a large set of realistic synthetic anatomies. We also show that these new algorithms outperform state of the art methods based on circular arcs or Bezier-Splines when applied to this specific problem. Conclusion: With this work we demonstrate that pre- and intra-operative planning of nonlinear access paths is possible for minimally-invasive surgeries at the otobasis.

Abstract:In robot-assisted needle-based medical procedures, insertion motion planning is a crucial aspect. 3D dynamic motion planning for a cannula flexible needle is challenging with regard to the nonholonomic motion of the needle tip, the presence of anatomic obstacles or sensitive organs in the needle path, as well as uncertainties due to the dynamic environment caused by the movements and deformations of the organs. The kinematics of the cannula flexible needle is calculated in this paper. Based on a rapid and robust static motion planning algorithm, referred to as greedy heuristic and reachability-guided rapidly-exploring random trees, a 3D dynamic motion planner is developed by using replanning. Aiming at the large detour problem, the convergence problem and the accuracy problem that replanning encounters, three novel strategies are proposed and integrated into the conventional replanning algorithm. Comparisons are made between algorithms with and without the strategies to verify their validity. Simulations showed that the proposed algorithm can overcome the above-noted problems to realize real-time replanning in a 3D dynamic environment, which is appropriate for intraoperative planning.
FDSPC: Fast and Direct Smooth Path Planning via Continuous Curvature Integration

Abstract:In recent decades, global path planning of robot has seen significant advancements. Both heuristic search-based methods and probability sampling-based methods have shown capabilities to find feasible solutions in complex scenarios. However, mainstream global path planning algorithms often produce paths with bends, requiring additional smoothing post-processing. In this work, we propose a fast and direct path planning method based on continuous curvature integration. This method ensures path feasibility while directly generating global smooth paths with constant velocity, thus eliminating the need for post-path-smoothing. Furthermore, we compare the proposed method with existing approaches in terms of solution time, path length, memory usage, and smoothness under multiple scenarios. The proposed method is vastly superior to the average performance of state-of-the-art (SOTA) methods, especially in terms of the self-defined $\mathcal{S}_2 $ smoothness (mean angle of steering). These results demonstrate the effectiveness and superiority of our approach in several representative environments.

Abstract:Needle insertion is widely used in modern clinical practice, and flexible needles enable surgeons to adjust the needle insertion path to avoid obstacles and compensate for target drift. To increase the accuracy and safety of the needle insertion procedure, a path planning and tracking control method for flexible needle steering in dynamic environment is proposed in this paper. The path planning method is based on the artificial potential field algorithm and curve fitting algorithm, after several feasible paths with variant curvatures is obtained, they are furtherly optimized based on path length, obstacle clearance and control effort to select the best one. Simulations show that the proposed path planning method performs better than the current constant curvature method and the rapidly-exploring random tree (RRT) based method. By finely tuning the initial planned path, an intraoperative path replanning method is proposed, which are used to compensate for the drift of target and obstacles. Then to ensure the needle is inserted along the planned path, a tracking control method is adopted based on fuzzy logic algorithm. With the proposed intraoperative path planning and tracking control method, the robustness and accuracy of needle insertion in congested environment can be improved, and experiments also show that our proposed method achieves satisfactory insertion safety and targeting accuracy in dynamic environment. The obtained accuracy of 1.57 mm is sufficient for most clinical needle insertion applications.

Dynamic path planning for percutaneous procedures in the abdomen during free breathing

Abstract:Purpose: Percutaneous procedures are increasingly used for the treatment of tumors in abdominal structures. Most of the time, these procedures are planned based on static preoperative images and do not take into account any motions, while breathing control is not always applicable. In this paper, we present a method to automatically adjust the planned path in real time according to the breathing. Methods: First, an estimation of the organs motions during breathing is performed during an observation phase. Then we propose an approach named Real Time Intelligent Trajectory (RTIT) that consists in finding the most appropriate moments to push the needle along the initially planned path, based on the motions and the distance to surrounding organs. We also propose a second approach called Real Time Straight Trajectory (RTST) that examines sixteen scenarios of needle insertion at constant speed, starting at eight different moments of the breathing cycle with two different speeds. Results: We evaluated our methods on six 3D models of abdominal structures built using image datasets and a real-time simulation of breathing movements. We measured the deviation from the initial path, the target positioning error, and the distance of the actual path to risky structures. The path proposed by RTIT approach is compared to the best path proposed by RTST. Conclusions: We show that the RTIT approach is relevant and adapted to breathing movements. The modification of the path remains minimal while collisions with obstacles are avoided. This study on simulations constitutes a first step towards intelligent robotic insertion under real-time image guidance.
