Abstract:Purpose Vasovagal syncope is a common clinical condition that lacks effective medical therapies despite being associated with significant morbidity. Current guidelines suggest that serotonin-specific reuptake inhibitors might suppress vasovagal syncope but supporting studies have been small and heterogenous. The purpose of this study was to evaluate the efficacy of serotonin-specific reuptake inhibitors to prevent syncope in patients with recurrent vasovagal syncope by conducting a systematic review and meta-analysis of published studies. Methods Relevant randomized controlled trials were identified from the MEDLINE and Embase databases without language restriction from inception to August 2022, and ClinicalTrials.gov. All studies were conducted in clinical syncope populations and compared the benefit of serotonin versus placebo. Weighted relative risks were estimated using random effects meta-analysis techniques. Results Three studies ( n = 204) met inclusion criteria. Patients were 42 ± 13 years of age and 51% female. Serotonin-specific reuptake inhibitors were found to substantially reduce the likelihood of a patient having at least one recurrence of vasovagal syncope [relative risk (RR) 0.34 (0.20–0.60), p < 0.01] with minimal between-study heterogeneity ( I 2 = 0%, p = 0.67). Serotonin-specific reuptake inhibitors in two reports provided significant protection against clinical presyncope [RR 0.43 (0.24–0.77), p < 0.01], with minimal between-study heterogeneity ( I 2 = 0%, p = 0.80). Conclusions Serotonin-specific reuptake inhibitors may be effective in preventing syncope induced by head-up tilt testing and in syncope in the community in randomized, double-blinded clinical trials.

Abstract:Vasovagal syncope (VVS) is a clinically common neurally mediated syncope. The relationship between different hemodynamic types of VVS and clinical syncopal symptoms has not been reported. The purpose of this research is to explore relationship between hemodynamic types and syncopal symptoms in pediatric VVS. Two thousand five hundred thirteen patients diagnosed with VVS at the age of 3–18 years, average age was 11.76 ± 2.83 years, including 1124 males and 1389 females, due to unexplained syncope and pre-syncope from single-center of January 2001 to December 2021 were retrospectively analyzed. Subjects were divided into two groups according to the presence or absence of syncopal symptoms: syncope group (1262 cases) and pre-syncope group (1251 cases). (1) Baseline characteristics: age, height, weight, systolic blood pressure (SBP), and diastolic blood pressure (DBP) increased in the syncope group compared with the pre-syncope group; the composition ratio of females was more than that of males in the syncope group; and the composition ratio of VVS-cardioinhibited (VVS-CI) and VVS-mixed (VVS-M) was more in the syncope group than that of the pre-syncope group (all P < 0.05). (2) Univariate analysis: age, height, weight, SBP, DBP, female, VVS-CI, and VVS-M were potential risk factors for the presence of syncopal symptoms (all P < 0.05). (3) Multivariate analysis: VVS-CI and VVS-M were independent risk factors for the presence of syncopal symptoms, with an increased probability of 203% and 175%, respectively, compared to VVS-vasoinhibited (VVS-VI) (all P < 0.01).

Abstract:Vasovagal syncope (VVS), characterized by transient loss of consciousness, is among the most prevalent reasons for emergency visits worldwide. Although benign in nature, VVS can be accompanied by traumatic injury, leading to morbidity and decreased quality of life, especially in those with VVS recurrence. The management includes non-pharmacologic and pharmacologic therapies (if resistant), patient education and reassurance, salt and fluid intake increase, and physical counter-pressure maneuvers. Among medications, midodrine has shown promising results in reducing VVS recurrence and positive head-up tilt tests. Fluoxetine and atomoxetine also might be suitable candidates for VVS therapy. Permanent pacemakers, such as closed-loop stimulation, are under research and can be effective in cases unresponsive to medical treatment. In summary, while data are scarce regarding the definite treatment of VVS, there is a need for further research with novel, easy-to-use and cost-effective therapeutic methods to enhance quality of life and prevent traumatic injury.
Prospective analysis of syncope/pre-syncope in a tertiary paediatric orthopaedic fracture outpatient clinic

Abstract:Background: The incidence of vasovagal syncope (VVS) precipitated by medical procedures such as blood donation is quoted as between 0.13% and 4.17%. Vasovagal events have been observed to occur following cast removal at our paediatric orthopaedic clinic; however, there is no available information in the existing literature regarding incidence or risk factors. This study aims to identify the incidence and demographic traits of patients experiencing syncopal events following cast removal. Methods: Over a 12-month period, paediatric patients experiencing a syncopal or pre-syncopal event during an outpatient appointment for cast removal were prospectively enrolled into the study. Basic demographic data were recorded, as well as injury and procedure details and a description of the event. Statistical analysis as well as calculation of incidence of vasovagal events were performed. Results: A total of 6078 patients presented for cast removal in the 12-month period. Twenty syncopal or pre-syncopal events were recorded. Incidence was calculated as 0.32%. Mean patient age was 10.8 years. Male : female ratio was 2.3:1. Mean body mass index (BMI) was 20.08, with a trend for higher prevalence of males under the 50th BMI percentile-for-age. The mean time post-injury was 31.4 days. Ninety-five percent of patients were being treated for an upper limb injury and 30% had injuries that had been treated surgically. There were no associated secondary complications or injuries. Conclusions: Incidence of VVS following cast removal is comparable to the values quoted in literature for other medical procedures. Demographic data of our cohort suggested that those who experienced VVS were predominantly young males of lower-than-average BMI.

Abstract:The pathophysiology of vasovagal syncope (VVS) is reviewed, focusing on hemodynamic aspects. Much more is known about orthostatic than about emotional VVS, probably because the former can be studied using a tilt table test (TTT). Recent advances made it possible to quantify the relative contributions of the three factors that control blood pressure: heart rate (HR), stroke volume (SV) and total peripheral resistance (TPR). Orthostatic VVS starts with venous pooling, reflected in a decrease of SV. This is followed by cardioinhibition (CI), which is a decrease of HR that accelerates the ongoing decrease of BP, making the start of CI a literal as well as fundamental turning point.The role of hormonal and other humoral factors, respiration and of psychological influences is reviewed in short, leading to the conclusion that a multidisciplinary approach to the study of the pathophysiology of VVS may yield new insights.
Association between vitamin D deficiency and vasovagal syncope: A systematic review and meta‐analysis

Abstract:Studies have investigated vitamin D levels in patients with vasovagal syncope (VVS) and compared them with healthy controls. Our systematic review showed that patients with VVS have significantly lower levels of vitamin D. Also, VVS occurrence was significantly higher in vitamin D‐deficient patients. Vasovagal syncope (VVS) is the most prevalent type of syncope and its management includes pharmacologic and non‐pharmacologic interventions. Recently, studies have investigated vitamin D levels in VVS patients. In this systematic review and meta‐analysis, we aim to review these studies to find possible associations between vitamin D deficiency and vitamin D levels with VVS. International databases including Scopus, Web of Science, PubMed, and Embase were searched with keywords related to "vasovagal syncope" and "vitamin D." Studies were screened and the data were extracted from them. Random‐effect meta‐analysis was conducted to calculate the standardized mean difference (SMD) and 95% confidence interval (CI) for vitamin D levels in comparison to VVS patients and controls. Also, VVS occurrence was measured and the odds ratio (OR) and 95% CI were calculated for comparison of vitamin D deficient cases and nondeficient individuals. Six studies were included with 954 cases investigated. Meta‐analysis showed that patients with VVS had significantly lower vitamin D serum levels in comparison to non‐VVS cases (SMD −1.05, 95% CI −1.54 to −0.57, p‐value

Abstract:Vavovagal syncope (VVS) is not generally associated with cardiovascular, neurological or other diseases, and, therefore, represents an isolated manifestation. Isolated VVS cannot be regarded as a disease for several reasons: spontaneous syncope occurs in about half of individuals during their lives, and the unidentified neural pathways involved in the vasovagal response are probably present in all healthy humans, with individual differences in susceptibility; VVS is induced during tilt testing in several subjects with no history of syncope; during haemorrhagic shock, the vasovagal reaction can be observed in subjects with no history of syncope; about 20% of astronauts, who are selected on the basis of their great resistance to orthostatic stress, experience syncope or presyncope on landing after a short-duration space flight; to date, no genetic basis of VVS has been demonstrated; subjects with VVS are generally normotensive and, importantly, have normal blood pressure regulation apart from the episodes of syncope; hormonal disorders or a generalized state of autonomic involvement, although frequently investigated, have never been clearly demonstrated. Isolated VVS should be distinguished from those forms that start in old age and which are often associated with cardiovascular or neurological disorders, and other dysautonomic disturbances such as carotid sinus hypersensitivity, post-prandial hypotension, and symptoms of autonomic dysfunction. In these subjects, VVS appears as an expression of a pathological process, i.e. a disease, mainly related to a generalized involvement of the autonomic nervous system, which is not yet well-defined from a nosological point of view.
Vagal and Sympathetic Mechanisms in Patients With Orthostatic Vasovagal Syncope

Abstract:Background Autonomic and particularly sympathetic mechanisms play a central role in the pathophysiology of vasovagal syncope. We report direct measurements of muscle sympathetic nerve activity in patients with orthostatic vasovagal syncope. Methods and Results We studied 53 otherwise healthy patients with orthostatic syncope. We measured RR intervals and finger arterial pressures and in 15 patients, peroneal nerve muscle sympathetic activity before and during passive 60 degree head-up tilt, with low-dose intravenous isoproterenol if presyncope did not develop by 15 minutes. We measured baroreflex gain before tilt with regression of RR intervals or sympathetic bursts on systolic or diastolic pressures after sequential injections of nitroprusside and phenylephrine. Orthostatic vasovagal reactions occurred in 21 patients, including 7 microneurography patients. Presyncopal and nonsyncopal patients had similar baseline RR intervals, arterial pressure, and muscle sympathetic nerve activity. Vagal baroreflex responses were significantly impaired at arterial pressures below (but not above) baseline levels in presyncopal patients. Initial responses to tilt were comparable; however, during the final 200 seconds of tilt, presyncopal patients had lower RR intervals and diastolic pressures than nonsyncopal patients and gradual reduction of arterial pressure and sympathetic activity. Frank presyncope began abruptly with precipitous reduction of arterial pressure, disappearance of muscle sympathetic nerve activity, and RR interval lengthening. Conclusions Patients with orthostatic vasovagal reactions have impaired vagal baroreflex responses to arterial pressure changes below resting levels but normal initial responses to upright tilt. Subtle vasovagal physiology begins before overt presyncope. The final trigger of human orthostatic vasovagal reactions appears to be the abrupt disappearance of muscle sympathetic nerve activity.

Abstract:The interest in an association between cancer and connective tissue disease stems from the insights that such an association might provide into the pathogenesis of these diseases. Although this seems to be a case of the blind leading the blind, the situation still merits a careful analysis. The known syndromes can be placed into one of two categories, based on the apparent relationship between the onsets and subsequent courses of the component diseases. I will bypass the clinical conditions caused by direct tumor invasion of muscles or joints. Some of these are encountered relatively frequently and can cause diagnostic problems; however, this mechanism is not germane to the present discussion. The first category is that in which the rheumatic disease begins after the neoplasm has presumptively started, even though the tumor does not become evident until later. The time relationship suggests that the cancer somehow initiates the connective tissue disease. The disease associations that seem to fulfill these conditions are shown in TABLE 1. It is important to the rheumatologist that in each case the clinical features of the connective tissue disease are indistinguishable from those encountered in uncomplicated cases. Presumptively, the pathogenesis of the isolated and tumor-associated conditions is similar. A number of other associations between connective tissue disease and cancer have been reported.' It is important to bear in mind that certain of these diseases are relatively common, and it is probable that the frequency of coincidence does not exceed that dictated by chance. Dermatomyositis is the most dramatic of the first group of syndromes. Although it may occur in association with tumors at any age, it is particularly prone to develop in later life. The converse relationship is even more striking in that the peak incidence of uncomplicated dermatomyositis is prior to the fifth decade, and cases that begin after age 50 are associated almost always with malignancy.' This age-related incidence should tell us something. Perhaps the older individual is more prone to dermatomyositis, but some factor important in the pathogenesis of sporadic disease is missing in later life and is supplied by the tumor. This seems more probable than one major alternative: that is, the tumor that appears after age 50 is qualitatively different in some way that is responsible for the complication. Another attractive hypothesis is that the host response to the tumor is different in older age and that this response rather than the tumor itself is the cause of dermatomyositis. Because of the prompt improvement in the dermatomyositis that occurs when the tumor is successfully treated, the tumor must be continuously synthesizing or stimulating the production of the pathogenic factor. Several groups who have studied patients with the dermatomyositis syndrome have demonstrated cutaneous reactivity to an aqueous tumor extract and have passively transferred such reactivity with serum.2 This observation argues strongly in favor of an antibody-mediated immunological response to the tumor but tells us little about the pathogenesis of the dermatomyositis. Abstract:Background Serotonin is an important neurohormone that regulates vascular tone and autonomic reflexes, though its pathophysiological role in vasovagal syncope (VVS) remains uncertain. Objective This study sought to explore the involvement of serotonin and serotonergic-related metabolites in the pathogenesis of VVS. Methods Sixty-six patients (age 45.6±17.0 years; 33 females) with recurrent VVS underwent a head-up tilt test (HUTT). Blood samples were collected from all patients in a resting supine position, with an additional sample obtained from HUTT-positive patients during syncope. Plasma and platelet serotonin levels, and plasma concentrations of serotonergic-related metabolites—including serotonin's precursor 5-hydroxytryptophan (5-HTP), major metabolite 5-hydroxyindoleacetic acid (5-HIAA), and synthesis source tryptophan—were measured using the liquid chromatography tandem mass spectrometry (LC-MS/MS) method. Results HUTT was positive in 45 patients and negative in 21 patients. Significant differences were observed in plasma 5-HTP and 5-HIAA levels between HUTT+ and HUTT- patients ( P <0.001 and P =0.040, respectively), as well as before and after syncope (all P <0.001), whereas no significant changes were found in serotonin and tryptophan levels. Notably, plasma serotonin levels significantly increased during syncope in patients with drug-free VVS ( P =0.037), and a greater change in serotonin correlated with a shorter time to syncope ( R 2 =0.38, P =0.015). Furthermore, certain serotonergic-related metabolites exhibited significant correlations with hemodynamic changes during VVS episodes, with 5-HTP demonstrating the highest sensitivity. Conclusions Despite the unchanged plasma and platelet serotonin levels, certain serotonergic-related metabolites significantly changed and correlated with hemodynamic parameters during VVS episodes, suggesting the potential involvement of an altered serotonergic metabolic pathway in VVS.

Abstract:We study the existence and nonexistence of positive (super-) solutions to a singular semilinear elliptic equation −∇ · (|x| A ∇u) − B|x| A−2 u = C|x| A−σ u p in cone-like domains of R N (N ≥ 2), for the full range of parameters A, B, σ, p ∈ R and C> 0. We provide a characterization of the set of (p, σ) ∈ R 2 such that the equation has no positive (super-) solutions, depending on the values of A, B and the principal Dirichlet eigenvalue of the cross-section of the cone. The proofs are based on the explicit construction of appropriate bar- riers and involve the analysis of asymptotic behavior of super-harmonic functions associated to the Laplace operator with critical potentials, Phragmen-Lindelof type comparison arguments and an improved ver- sion of Hardy's inequality in cone-like domains.
Risk Factors and Prognostic Follow-Up of Vasovagal Syncope Children With Seizure-Like Activities During Head-Up Tilt Test Induced-Syncope

Abstract:Objectives: To analyze the risk factors associated with seizure-like activities during head up tilt test (HUTT)-induced syncope in children with vasovagal syncope (VVS) and assess the prognosis of these patients. Methods: This is a retrospective study. VVS children with or without seizure-like activities during HUTT-induced syncope were included in convulsive or non-convulsive group. The clinical characteristics, hemodynamic parameters during HUTT-induced syncope and follow-up data were reviewed from the HUTT case report form and analyzed. Results: 68 cases (25 males, mean age 11.86 ± 3.35 years) were enrolled in convulsive group and 65 cases in non-convulsive group (24 males, mean age 11.64 ± 2.11 years). There were statistical differences in history duration, response type, and asystole between the two groups (all P < 0.05). Fully adjusted logistic regression showed that the risk of seizure-like activities was increased by 37.18 folds for patients with asystole compared with those without asystole ( P = 0.005), by 308.25 and 6.08 folds for patients with cardioinhibitory type or mixed type compared with vasoinhibitory type ( P < 0.01). No significant difference was exhibited in negative HUTT conversion rate and the proportion of re-syncope patients between the two groups at follow-up (both P > 0.05). None of these convulsive patients underwent pacemaker implantation during follow-up. Conclusions: Asystole and response type were independent risk factors associated with seizure-like activities. Patients with asystole and mixed or cardioinhibitory responses to HUTT should be closely concerned. However, VVS children with seizure-like activities did not have a poor prognosis at follow-up.

Analysis of the clinical features of 83 vasovagal syncope patients

Abstract:Objective To analyze the clinical features of vasovagal syncope, enhance its clinial awareness and to promote the importance of history taking in the differential diagnosis of syncope. Methods A total of 83 patients with the diagnosis of vasovagal syncope were included in the study and their data were, undergone retrospective statistical analysis. Results A 63.9% of patients had recurrent syncope. There was 73.5% of them had motivation and 80.7% of them had presyncope symptoms. Complete loss of consciousness was found in 77.1% of patients. There was 39.1% of them had concomitant symptoms and 39.1% had postsyncopal symptoms. In 54.2% of the patients the duration of syncope was less than 2 minutes. Conclusion It is important to pay attention to history taking and to anylaze the clinical features in the diagnosis of vasovagal syncope.

Abstract:In the pathophysiological study of vasovagal syncope, the nature of the interaction between baroreceptor sensitivity (BS), sympathetic withdrawal, and parasympathetic activity has yet to be ascertained. Altered BS may predispose toward abnormal sympathetic and parasympathetic responses to orthostasis, causing hypotension that may progress to syncope if there is sympathetic withdrawal. To examine this hypothesis, we monitored blood pressure (BP), heart rate (HR), BS, forearm blood flow, and muscle nerve sympathetic activity (MNSA) continuously in 18 vasovagal patients during 60° head-up tilt, syncope, and recovery. Results were compared with those of 17 patients who were able to tolerate tilt for 45 min. During early tilt, BP was maintained in both groups by an increase in HR and MNSA from baseline ( P < 0.01), but BS decreased more in the syncopal group ( P < 0.05). At the start of presyncope (mean 2.7 ± 0.2 min before syncope and 15.2 ± 12 min after tilt), when BP fell, HR and sympathetic activity remained increased from baseline ( P< 0.01). Thereafter, BP and HR correlated directly with sympathetic activity and regressed in linear fashion until syncope ( P < 0.001), whereas BS increased to baseline. At syncope, BP, HR, and sympathetic activity fell below baseline ( P < 0.01, P < 0.05, and P < 0.01, respectively), but BS did not increase. During recovery, sympathetic activity increased to baseline and BS increased ( P < 0.05), whereas HR and BP remained low ( P < 0.01 and P < 0.05, respectively). The mechanism for the initiation of hypotension during presyncope remains unknown, but BS may contribute. Vasodilatation and bradycardia during presyncope appear to be more closely related to withdrawal of sympathetic activity than to increased parasympathetic cardiac activity.
