Abstract:Objective: to unveil the experience of the elderly with social isolation in the pandemic of COVID-19. Methods: qualitative study, with 14 elderlies in social isolation. The content was recorded and processed using the software Interface de R pour les Analyses Multidimensionnelles de Textes et de Questionnaires. Results: six classes were obtained, the first focused on spirituality and pre-pandemic pleasurable activities; the second was related to missing the extra-household routine and family life; the third, to the construction of a new routine; the fourth, to the strategies adopted for the prevention of COVID-19; the fifth, to the signs/symptoms experienced during the infection; and the sixth class, to the fear of dying. Conclusion: the experience of the elderly was permeated by the adaptation of routine, adoption of preventive measures and feelings of anguish in the face of uncertainties.

Abstract:The study focused on examining the relationship between well-being and various psychological factors such as loneliness, anxiety, depression, and stress, whilst also considering changes in lifestyle. A total of 108 elderly participants, with an average age of 70.38 years, were enrolled in this quantitative cross-sectional study. The research employed a battery of assessment tools including a Sociodemographic Data Questionnaire, Mini-Mental State Examination, Positive Mental Health Scale, Stress Perception Scale, Geriatric Anxiety Inventory, Geriatric Depression Scale (reduced version), Loneliness Scale, and International Physical Activity Questionnaire. Descriptive analysis was conducted in order to understand the distribution of scores across these variables, followed by the categorization of participants based on the reported alterations in eating and physical activity behaviors. Correlations between variables were assessed using Spearman correlation and an EBIC-LASSO network analysis. The findings indicated a potential detriment to the well-being of elderly individuals practicing social distancing, evidenced by heightened symptoms of loneliness, depression, anxiety, and stress, alongside the reported changes in dietary patterns and physical activity. The study underscores the importance of understanding the pandemic's impact on the well-being of older adults and advocates for longitudinal investigations to delineate the evolving effects of social distancing measures across different phases of the pandemic.

Abstract:Background: Social distancing measures related to COVID-19 pandemic have challenged older adults’ social connectedness and mental health. Loneliness and social isolation have been reported, but their impact on cognition is unclear. Objective: To explore changes on daily activity, mental health and its relationship with cognitive performance in older adults during the pandemic. Methods: 100 individuals ≥60 years, without cognitive impairment, enrolled in the Brazilian Memory Study(BRAMS), were applied the UCLA Loneliness Scale, Perceived Stress Scale, Geriatric Depression Scale, Mini-Mental State Exam(MMSE), assessment of daily routine, memory complaints and social connectedness during the pandemic. Results: Almost half of the participants reported the pandemic significantly affected their life, 38.9% lost a relative due to COVID-19, 60% had daily routine changes, due to decreases in physical activity(78%), relationships(40.5%), emotion(22%) and autonomy. Voice message to keep social connectedness increased from 24.2% to 42.1% and memory complaints reached 40%. More than 30% felt more stress, loneliness or depression comparing to prepandemic. Controlling for age, sex, and education, higher UCLA scores were associated with low MMSE (p=0.018). Conclusion: COVID-19 pandemic impacted routine, social connectedness, and mental health in older adults. Loneliness was associated with lower cognitive performance, highlighting the importance of incorporating systematic evaluations into clinical practice to identify vulnerable individuals to mental and cognitive disorders during the pandemic.

Abstract:Introduction: Older adults have the poorest coronavirus (COVID-19) prognosis with the highest risk of death due to complications, making their COVID-19 experiences particularly important. Guided by the stress-appraisal-coping theoretical model, we sought to understand COVID-related perceptions and behaviors of older adults residing in the United States. Materials and Methods: We used convenience sampling to recruit persons with the following inclusion criteria: Aged ≥ 65 years, English fluency, and U.S. residency. Semi structured in-depth interviews were conducted remotely and audio recorded between April 25, 2020 and May 7, 2020. Interviews were professionally transcribed with a final study sample of 43. A low-inference qualitative descriptive design was used to provide a situated understanding of participants' life experiences using their naturalistic expressions. Results: The mean age of participants was 72.4 ± 6.7. Slightly over half were female (55.8%), 90.6% were White, and 18.6% lived alone. The largest percentages of participants resided in a rural area (27.9%) or small city (25.6%). We identified four themes, including (1) risk perception, (2) financial impact, (3) coping, and (4) emotions. Most participants were aware of their greater risk for poor COVID-19 outcomes but many did not believe in their increased risk. Financial circumstances because of the pandemic varied with largely no financial impacts, while others reported negative impacts and a few reported positive impacts. Coping was problem- and emotion-focused. Problem-focused coping included precautionary efforts and emotion-focused coping included creating daily structure, pursuing new and/or creative activities, connecting with others in new ways, and minimizing news media exposure. Overall, emotional health was negatively affected by the pandemic although some participants reported positive emotional experiences. Conclusions: Perceiving themselves as high risk for COVID-19 complications, older adults used precautionary measures to protect themselves from contracting the virus. The precautionary measures included social isolation, which can negatively affect mental health. Older adults will need to be resourceful and draw on existing resources to cope, such as engaging in creative activities and new strategies to connect with others. Our findings underscore the importance of the preservation of mental health during extended periods of isolation by taking advantage of low-to-no-cost existing resources.

Abstract:Older adults were found to be the most susceptible group to suffer the physical health consequences of a COVID-19 infection and were considered vulnerable to the negative effects of the lockdowns due to the COVID-19 pandemic, yet unlike many young adults did not generally experience an increased rate of mental health problems. Our study explored the strategies older adults in Portugal deployed during the mandatory lockdowns in 2020 and 2021. Qualitative interview data were collected with 22 older adults in relatively good health (aged between 66 and 92 years old; 36% women). Three main themes were identified via thematic analysis: (1) Finding things to do and activities that can protect me; (2) Identifying how my thoughts can protect me; and (3) Counting myself lucky – me and my home advantages. Some of the thematic strategies identified by older adults to manage pandemic and lockdown-related stresses related to meaning-centered coping could be further developed via cognitive behavioral therapies of the third wave.

Abstract:Abstract Background Older adults are one of the most vulnerable groups to severe illness associated with the SARS-COV-2 virus. Therefore, it would be expected that the elderly would suffer great impact of the COVID-19 pandemic on their mental health. However, it seems not to be the case, possibly due to contextual aspects and the strategies used by them to deal with the COVID-19 pandemic. Our study aimed at exploring the strategies older adults in Portugal deployed during the mandatory lockdowns in 2020 and 2021 to protect their mental health. Methods A total of 22 older adults were interviewed and included in this study (36% women; age range between 66 and 92 years old). Qualitative data was collected through semi-structured interviews between Jan-Sept 2020 and analysed according to codebook thematic analysis. Results Three main themes were identified. The first theme - ‘finding things to do and activities that can protect me’ - referred to behavioural coping strategies adopted to deal with the pandemic (e.g., preventing behaviours to avoid COVID-19 infection, social support). The second theme - ‘identifying how my thoughts can protect me’ - encompassed cognitive coping strategies, such as engaging in meaning-making processes to make sense out of the circumstances. Finally, the third theme - ‘counting myself lucky: me and my home advantages’ - included aspects perceived as beneficial to the wellbeing of participants during lockdowns (e.g., being healthy, living in a rural area). Conclusions The thematic strategies identified by older adults to manage the pandemic and lockdown-related stresses could be linked to meaning-centred coping and could be further developed via existential therapy. Key messages • As older adults with health problems and living in urban areas may be at a disadvantage when facing lockdowns, they should be identified by government health entities and forward to psychologists. • The efficacy of preventing behaviours to avoid COVID-19 dissemination, distraction, social support, and meaning-making processes during health hazards should be targeted by treatment programmes.

Abstract:Using a recently developed hepsulfam-induced pancytopenia model in rhesus macaques, we have studied the effects of recombinant human interleukin-6 (rhIL-6) and rhIL-3 on marrow regeneration. Control animals were given hepsulfam (1.5 g/m2 by a single 30-minute intravenous [i.v.] injection, n = 4), while study animals received hepsulfam followed by rhIL-6, rhIL-3, or a combination of rhIL-6 and rhIL-3 (n = 3 per study group). Each cytokine was administered by once-daily subcutaneous (SC) injection (15 micrograms/kg/d) for 3 weeks beginning the day after chemotherapy (days 2 through 22). Mean platelet counts in control animals were < 100,000/microL on days 15 through 24, with 50% of the counts < 50,000/microL and two of four animals requiring platelet transfusion. In the rhIL-6- and rhIL-6/rhIL-3-treated groups, the nadir mean platelet counts were 164,000 +/- 58,700/microL and 162,300 +/- 23,800/microL, respectively, and occurred on day 15. Platelet counts in the rhIL-3-treated group were similar to those in controls. Mean absolute neutrophil counts (ANCs) < 1,000/microL occurred on days 10 through 29 in control animals, days 8 through 15 in rhIL-6-treated animals, and days 6 through 8 and 13 in rhIL-6/rhIL-3-treated animals. The frequency of ANCs < 500/microL was significantly less in the rhIL-6- and rhIL-6/rhIL-3-treated groups versus control groups (2.7 +/- 0.6 and 2.0 +/- 1.0 vs 7.0 +/- 1.4 occurrences, respectively; P < .05). rhIL-3-treated animals had ANCs similar to those in controls; one animal died with septicemia on day 21. Monkeys receiving rhIL-6 were significantly more anemic during the cytokine administration period; however, the anemia resolved by day 24. Coadministration of rhIL-3 and rhIL-6 partially corrected the anemia. The data indicate that rhIL-6 prevents significant thrombocytopenia and shortens the neutropenic period in this chemotherapy model.

Abstract:Introduction: Older adults, who already have higher levels of social isolation, loneliness, and sedentary behavior, are particularly susceptible to negative impacts from social distancing mandates meant to control the spread of COVID-19. We sought to explore the physical, mental, and social health impacts of the pandemic on older adults and their coping techniques. Materials and Methods: We conducted 25 semi-structured interviews with a sub-sample of participants in an ongoing sedentary behavior reduction intervention. Interviews were recorded and transcribed, and iterative coding was used to extract key themes. Results: Most participants reported an increase in sedentary behavior due to limitations on leaving their home and increased free time to pursue seated hobbies (e.g., reading, knitting, tv). However, many participants also reported increased levels of intentional physical activity and exercise, particularly outdoors or online. Participants also reported high levels of stress and a large decrease in in-person social connection. Virtual connection with others through phone and video was commonly used to stay connected with friends and family, engage in community groups and activities, and cope with stress and social isolation. Maintenance of a positive attitude and perspective gained from past hardships was also an important coping strategy for many participants. Discussion: The COVID-19 pandemic and associated social distancing measures have impacted older adults' perceived levels of activity, stress, and social isolation, but many leveraged technology and prior life experiences to cope. These themes could inform future interventions for older adults dealing with chronic stress and isolation.

Moving away from benzodiazepine as a primary sedative in the intensive care unit; is clonidine a viable alternative?

Abstract:Current pain, agitation, and delirium (PAD) guidelines endorse a benzodiazepine-sparing approach to aid in achieving optimal patient outcomes.[1] Several benzo-sparing strategies, including protocolized and frequent pain, sedation and delirium assessments, achieving the goal of awake and alert, utilization of bolus and symptom-triggered, patient-specific pharmacotherapy, interruption of sedatives, a pain-first approach or analagosedation, early mobilization, rotation of medications, and adjustment of ventilator settings are nonmedication specific principles shown to improve clinical outcomes in mechanically ventilated (MV), intensive care unit (ICU) patients. These techniques and strategies have been associated with some of the most relevant clinical outcomes including a decrease in discomfort, ventilator-associated pneumonia, length of MV, and ICU and hospital length of stay.[2,3] Interestingly, extremely few well-conducted sedative versus sedative trials show improvements in these hard clinical outcomes. Dexmedetomidine is an effective agent for PAD management and a viable alternative to benzodiazepine therapy. Dexmedetomidine maintains a light level of sedation, while minimizing the deep sedation commonly seen with benzodiazepines. In randomized, controlled trials, dexmedetomidine has been associated with shorter duration of delirium and MV compared to benzodiazepine infusion, specifically midazolam.[4] It should be noted, however, that these trials showing a reduction in MV duration are not without criticism. For example, the mean dose of midazolam in the SEDCOM trial was approximately 5 mg/h (based on the average patient weight of 88 kg); an extremely high dose by clinical standards when the goal is to keep a patient awake and alert.[5] Dexmedetomidine is relatively safe with fairly predictable cardiovascular (CV) effects (most frequently hypotension and bradycardia). These CV risks can often be mitigated by avoiding the use of bolus therapy and extending titration intervals to no more frequently than every 30 min.[6] The cost of dexmedetomidine is often a concern in many parts of the world. There are several strategies to address the cost of dexmedetomidine including use of alternative medication or by following restrictive institution specific guidelines. A recent publication on dexmedetomidine stewardship showed an institution's approach to managing the benefits of the drug versus the cost. The guideline followed in this analysis supports judicious use of dexmedetomidine in patients who fail traditional therapy.[7] Another important note in terms of cost is in certain parts of the world dexmedetomidine will lose its patent in the near future and is likely going to be price reduced. We commend the investigators for their fine analysis entitled “comparison of clonidine and dexmedetomidine for short term sedation of ICU patients” published in this month's journal.[8] This trial examined the efficacy and safety of clonidine as an alternative to dexmedetomidine in MV patients. The authors conclude both agents are effective, although clonidine is associated with a higher risk of hypotension. This paper adds to the literature suggesting clonidine may be a useful medication for PAD management although further investigation is needed to confirm these findings. We support the author's conclusion although we think there are some key critiques of this analysis. (1) The authors assessed and titrated to Ramsay Sedation Score (RSS) but pain and delirium assessment scores were not reported. Therefore, it is hard to tell if the pain-first approach was employed prior to the use of either sedative. (2) The goal RSS in this trial was 3-4 yet an RSS of 5, corresponding to awake and alert, may have been a more appropriate goal and these patients may have been more deeply sedated than would be desired in clinical practice. (3) Dexmedetomidine performed better statistically than clonidine, therefore lumping them both as effective may be slightly misleading. It is also important to note that the relatively high incidence of hypotension in the clonidine arm is concerning. A recent trial published on clonidine in noncardiac surgery patients highlights safety concerns with a troubling increase in clinically important hypotension and nonfatal cardiac arrest.[9] Although a different patient population than this study, when combined with the results of this analysis it raises concerns about the safety of this medication. The current analysis should be considered hypothesis generating and clonidine should be further investigated before it is systematically used for management of PAD.
The psychosocial health, experiences and needs of older adults and care partners during the first surge of the COVID-19 pandemic: a mixed-methods study

Abstract:Background: Minimal research has leveraged qualitative data methods to gain a better understanding of the experiences and needs of older adults (OAs) and care partners of OAs with and without Alzheimer's Disease (AD) and AD-related dementias (AD/ADRD) during the first surge of the COVID-19 pandemic. In this study, we: 1) quantitatively evaluated the psychosocial health of community-dwelling OAs; 2) quantitatively evaluated the perceived stress of care partners for OAs; 3) qualitatively characterized the experiences and needs of community-dwelling OAs and their care partners; and 4) explored differences in the experiences of care partners of OAs with and without AD/ADRD during the first surge of the COVID-19 pandemic in the New York metropolitan area. Methods: In this mixed-methods study, telephone interviews were conducted with 26 OAs and 29 care partners (16 of whom cared for OAs with AD/ADRD) from April to July 2020. Quantitative data included: demographics; clinical characteristics (Katz Index of independence in activities of daily living (Katz ADL) and the Lawton-Brody instrumental activities of daily living scale (Lawton-Brody)); and psychosocial health: stress was assessed via the Perceived Stress Scale (PSS), social isolation via the Lubben Social Network Scale (LSNS), loneliness via the DeJong Loneliness Scale (DeJong), and depression and anxiety via the Patient Health Questionnaire-Anxiety and Depression (PHQ). Qualitative questions focused on uncovering the experiences and needs of OAs and their care partners. Results: OAs (N = 26) were mostly female (57.7%), and White (76.9%), average age of 81.42 years. While OAs were independent (M = 5.60, Katz ADL) and highly functional (M = 6.92, Lawton-Brody), and expressed low levels of loneliness, stress, depression and anxiety (M = 1.95 on DeJong; M = 12.67 on PSS; M = 1.05 on PHQ depression; and M = 1.09 on PHQ anxiety), open-ended questions elicited themes of fear and worry. Care partners (N = 29) were mostly female (75.9%), White (72.4%), and married (72.4%), and reported moderate stress (M = 16.52 on the PSS), as well as a psychological impact of the pandemic. Conclusions: Early in the pandemic, OAs reported minimal stress and loneliness; this may have been related to their reports of frequent interaction with family, even if only virtually. By contrast, care partners were moderately stressed and worried, potentially more than usual due to the additional challenges they face when trying to meet their loved ones' needs during a pandemic.

Abstract:ObjectivesTo review the impact of social isolation during COVID-19 pandemic on mental and physical health of older people and the recommendations for patients, caregivers and health professionals.DesignNarrative review.SettingNon-institutionalized community-living people.Participants20.069 individuals from ten descriptive cross-sectional papers.MeasurementsArticles since 2019 to 2020 published on Pubmed, Scielo and Google Scholar databases with the following MeSh terms (‘COVID-19’, ‘coronavirus’, ‘aging’, ‘older people’, ‘elderly’, ‘social isolation’ and ‘quarantine’) in English, Spanish or Portuguese were included. The studies not including people over 60 were excluded. Guidelines, recommendations, and update documents from different international organizations related to mental and physical activity were also analysed.Results41 documents have been included in this narrative review, involving a total of 20.069 individuals (58% women), from Asia, Europe and America. 31 articles included recommendations and 10 addressed the impact of social distancing on mental or physical health. The main outcomes reported were anxiety, depression, poor sleep quality and physical inactivity during the isolation period. Cognitive strategies and increasing physical activity levels using apps, online videos, telehealth, are the main international recommendations.ConclusionMental and physical health in older people are negatively affected during the social distancing for COVID-19. Therefore, a multicomponent program with exercise and psychological strategies are highly recommended for this population during the confinement. Future investigations are necessary in this field.

Abstract:The COVID-19 pandemic imposed a psychological burden on elderly and particularly individuals with cognitive impairment and dementia. Few studies have investigated the quarantine experiences of older adults with mild cognitive impairment (MCI) during the COVID-19 pandemic.The objective of this study was to explore the experiences and feelings of older adults with MCI during the COVID-19 outbreak in Chile and to know what strategies they used to overcome social isolation.A qualitative design was used. Ten participants with a diagnosis of MCI took part in this study. All interviews were recorded and coded using thematic analysis.The thematic analysis identified three themes related to the quarantine experience of older adults with MCI diagnosis: (1) Effects of social isolation during the COVID-19 pandemic (2) Believes, feelings and behaviors about the SARS-CoV-2 virus (3) Coping with social isolation/response to difficulties during the pandemic. It was found that older adults with MCI have been mainly psychologically and socially affected by social distancing and isolation, particularly individuals who were alone during COVID-19 outbreak. The only physical dimension negatively affected was the level of activity. Social isolation led to a significant number of negative emotions such as anger, fear of contracting the virus or possibility of contagion for their families, worries and sadness as well as emotional loneliness. It is noteworthy that the majority of participants have used several coping strategies during this challenging time.Since social isolation and a sedentary life have been associated with poorer cognition and functionality in people with MCI, a rational plan to both prevent the progression of cognitive decline and to increase social contact, is essential. Special attention must be drawn to maintaining people physically active at home and keeping their daily routine (within the possibilities) and also to ensure social connectedness through technology. Implementation of these measures could potentially reduce negative emotions during the pandemic.

Abstract:Introduction: The literature draws a mitigated picture of the psychosocial effects of the lockdown in older adults. However, the studies conducted so far are mainly based on web surveys which may involve selection bias. The PACOVID survey relies on a population-based design and addresses the attitudes, psychological and social experiences of the oldest old regarding the pandemic and lockdown and their impact. Material and Methods: Cross-sectional phone survey involving 677 persons. Baseline report on attitudes, psychological, and social experiences of the oldest old, regarding the pandemic and lockdown measures. Results: The mean age was 87.53 (SD 5.19). About 46% were living alone during the lockdown. Concerning difficulties, “ none ” was the most frequent answer (35.6%). For questions addressing how often they had felt sad, depressed, or lonely (CESD-scale), the most frequent answers were “ never/very rarely ” (58.7, 76.6, 60.8%) and 27.1% had anxious symptomatology (STAI scale). Most (92.9%) felt socially supported. Engaging in leisure activities was the most frequent coping strategy, and for numerous participants the lockdown did not represent much of a change in terms of daily routine. A very good knowledge and awareness of COVID-19 and the safety measures was observed. Comparisons with measures collected before the pandemic showed low changes in subjective health and the CES-D questions. Discussion: With a methodological design limiting selection bias, our results claim for a weakened psychosocial impact even though the participants are concerned and aware of the pandemic issues. These results highlight the resources and resilience abilities of older persons including in advancing age.

Abstract:O contexto de pandemia estabelecido pela disseminação do vírus SARS-CoV-2 demandou um conjunto de ações preventivas em nível global. No Brasil, a elevada taxa de contaminação e mortalidade contextualiza a demanda por adoção de medidas preventivas mais rígidas. Este cenário pode afetar mais intensamente o público idoso, uma vez que, a desconexão social é diretamente proporcional à ocorrência de casos de ansiedade. Com base na literatura, são apresentadas três estratégias analítico-comportamentais que podem ser realizadas com idosos no contexto de pandemia, de modo a diminuir a possibilidade de contato com estimulação aversiva, possivelmente ansiogênica, além de tentar manter ou ampliar o acesso a possíveis reforçadores: (1) a aprendizagem de habilidades digitais, (2) a dosagem das informações consumidas e (3) a adaptação de atividades físicas para o ambiente doméstico. Conclui-se ressaltando a escassez de publicações sobre a temática de ansiedade em idosos na perspectiva da análise do comportamento e a importância de se inserir nesse campo de pesquisa e atuação.Palavras-chave: COVID-19; isolamento social; ansiedade; idosos, Análise do Comportamento.

Abstract:Objectives: This study examined the impacts of COVID-19 lockdown on health and lifestyle factors for older adults in Sydney, Australia. The study examined demographic differences, social engagement, loneliness, physical activity, emotion regulation, technology use, and grandparenting experiences and their contribution to emotional health and quality of life during lockdown.Methods: Participants were 201 community-dwelling older adults (60–87 years, M = 70.55, SD = 6.50; 67.8% female) who completed self-report scales measuring physical and emotional health outcomes, quality of life, health service utilization, changes in diet and physical activity, impacts on grandparenting roles, and uptake of new technology.Results: One-third of older adults experienced depression, and 1 in 5 experienced elevated anxiety and/or psychological distress during lockdown. Specific emotion regulation strategies, better social and family engagement, and new technology use were associated with better emotional health and quality of life; 63% of older adults used new technologies to connect with others.Conclusions: Older adults were adaptable and resilient during lockdown, demonstrating high uptake of new technologies to remain connected to others, while negative emotional health outcomes were linked to loneliness and unhelpful emotion regulation.Clinical Implications: Further diversifying use of video technologies may facilitate improved physical and emotional health outcomes.

Distancing Measures in COVID-19 Pandemic: Loneliness, More than Physical Isolation, Affects Health Status and Psycho-Cognitive Wellbeing in Elderly Patients with Chronic Obstructive Pulmonary Disease

Abstract:Since the outbreak of the SARS-CoV-2 pandemic in 2020, many governments have been imposing confinement and physical distancing measures. No data exist on the effects of lockdowns on the health status of patients affected by chronic pathologies, specifically those with Chronic Obstructive Pulmonary Disease (COPD). Our study aims to establish variations across the psychological and cognitive profile of patients during the isolation period in Italy, in a cohort of patients affected by COPD, between February and May 2020. Forty patients with established COPD were comprehensively evaluated by geriatric multidimensional assessment before the spread of the epidemic in Italy, and submitted to a second evaluation during the subsequent lockdown. We assessed functional ability, basic and instrumental Activities of Daily Living (ADL and IADL), cognition and mood status. We compared the scores obtained at baseline against those obtained during the pandemic, and used mean differences for correlation with major clinical and functional indexes. The score differences from MMSE, ADL and IADL were statistically significant. Such differences were correlated to the presence of a caregiver and to the total number of family members living together. Remarkably, the loneliness dimension, more than the restrictions themselves, seemed to represent the major determinant of altered health status and depressed psycho-cognitive profile in our population. Also remarkably, we detected no correlation between the score variation and the respiratory function indexes of disease severity. The isolation measures adopted during the SARS-CoV-2 pandemic have triggered the classic clinical string associated to geriatric isolation, which leads to a deterioration of cognitive functions, independence and frailty levels in a population affected by a chronic degenerative disease, such as COPD. If considered from a multidimensional geriatric point of view, the individual benefit of isolation measures could be small or non-existent.

Abstract:Abstract: Cognitive stimulation activities for older adults are generally carried out in face-to-face workshops. However, during the COVID-19 pandemic, these activities and consultations became remote due to social isolation, enabling care to continue safely. This study aims to analyze the remote cognitive stimulation and/or telerehabilitation activities for older people that were carried out as an intervention during the COVID-19 pandemic. This is a systematic review study with five selected articles, conducted according to the PRISMA statement methodology. Among the main results, the feasibility and acceptance of remote cognitive stimulation activities using technologies during the pandemic stand out, reflecting on future and expanded use for different realities and cultures. the studies reviewed also indicate the stabilization and improvement of the cognitive state and of depressive and anxious feelings, as well as the maintenance of independence of these participants, with an increase in scores on scales applied before and after the interventions. In conclusion, the activities carried out in cognitive stimulation and/or telerehabilitation therapies for older adults as an intervention during the COVID-19 pandemic had an average of 47 participants; the technologies used for the activities were tablet and personal computer; pre-installed programs were the most used strategy; and the interventions lasted from 1 to 3 months, with activities 2 to 3 times per week. The reinvention of techniques aimed at stimulating and rehabilitating the cognitive health of the older adults, via technologies as a strategy to replace or complement face-to-face activities, promotes the cognitive and mental health and independence of the older population.

Pandemic Diaries: Lived Experiences of Loneliness, Loss, and Hope Among Older Adults During COVID-19

Abstract:Background and objectives: While research on loneliness in later life has increased during the coronavirus disease 2019 (COVID-19) pandemic, we still lack diverse qualitative approaches capturing lived experiences of lonely older adults. Approaches to research with older adults instead of on them are also scant in gerontological literature on loneliness. Through solicited diaries, this study aimed to explore how older Australians who were already lonely before the pandemic managed lockdowns (stay-at-home orders) in Victoria, Australia, which lived through one of the longest lockdowns in the world. Research design and methods: This article is based on qualitative diaries completed by 32 older adults (aged 65 and older). Diaries provide a 'live' document where participants become active research partners, recording and sharing their perceptions and experiences. This method is useful to capture sensitive issues, but to the best of our knowledge has not been employed in loneliness studies. Data were thematically analyzed through the identification of themes within and across diaries. Results: Loneliness was defined by participants as a detrimental absence of companionship and meaningful social interactions. For most, lockdowns exacerbated loneliness, presented new triggers, and upended coping strategies. The disruption of the 'emotion work' involved with managing loneliness led to a reconfiguration of response strategies, including through digital technology, which brought both challenges (e.g., digital ageism) and opportunities (e.g., novel communication forms). Discussion and implications: Understanding how older people living alone define and respond to loneliness in diverse contexts-for example, before and during a pandemic that restricts social interaction-provides critical insights to inform interventions to tackle loneliness.
Lived experiences of older adults living in the community during the COVID-19 lockdown - The case of mauritius

Abstract:Adoption of strict social distancing and sanitary measures were inevitable in the attempt to thwart the spread of the Covid-19 virus. These measures, however, came at a cost for older adults who faced major mental health issues because of social isolation. The impact of social isolation remains well documented in the wake of the pandemic. This paper explored the lived experiences of older adults living in the community during the Covid-19 sanitary lockdown in the small-island state of Mauritius. As a small-island state with family structures that still skew towards extended or modified extended families, it was interesting to explore whether older adults would be reporting feelings of social isolation as was being reported elsewhere. This study utilized a qualitative approach using a lifeworld hermeneutical approach with 15 older adult participants, recruited through convenience and snowball sampling, in the community. Data were collected using a narrative approach supplemented by an interview guide. Thematic analysis was used to identify themes from transcribed and translated data. Six themes were discovered: Fears of the virus and fear of deprivation; Reliving and recreating bonds; Active contribution to family life; Being and feeling valued within the family; Rediscovering family time and family moments; and Fear of going back to 'normal'. The study revealed positive experiences of the lockdown period characterised by an increase bonding in the family, deepening of familial bond, and increased value despite an initial apprehension of the lockdown. A renewed sense of purpose as well as deepening of bonds was reported by older adults. Some limitations in relation to data trustworthiness and respondents bias, however, could not be avoided since the study was carried out during sanitary lockdown period. The feelings of social isolation and depression reported by older adults who lived alone were not seen in this study, suggesting that the family structure may have a role to play in mitigating these effects. There would also be a need to explore similar small-island states or similar cultures where the effects of sanitary lockdowns may be different from current literature.
