Abstract:Soft tissue augmentation with fillers has witnessed a surge in popularity for rejuvenating facial features, offering solutions for wrinkles, volume loss, and contour irregularities. Non-biodegradable fillers like silicone, polyacrylamide hydrogel, and polymethylmethacrylate (PMMA) have been introduced, promising enduring results. However, reports on silicone filler usage have highlighted a spectrum of adverse events, ranging from erythema and edema to more severe complications like infection, vascular occlusion, and inflammatory nodules. The adverse effects of non-resorbable facial fillers can manifest even years post-procedure, resulting in significant discomfort and dissatisfaction for patients. Consequently, the literature is focusing on potential treatments for these outcomes, including systemic antibiotics, corticosteroid injections, surgical drainage, and excision. Despite these options, consensus on optimal treatment modalities remains elusive. Surgical excision is the definitive method for removing permanent fillers, albeit with the risk of post-removal irregularities. Fat grafting has emerged as a viable solution, allowing for the correction of volume deficits and asymmetries. Utilizing the patient's own tissue, fat grafting reduces the risk of adverse reactions and rejection. It offers targeted volume enhancement, restoring facial symmetry and proportion, and promoting tissue healing and regeneration through the presence of stem cells. After permanent filler removal, further filler injections are not advisable. In such cases, fat grafting offers several advantages, including reduced risk and targeted enhancement. Facial fat grafting effectively restores facial volume and symmetry, with stem cells aiding in tissue regeneration for long-term skin health. In essence, while the demand for aesthetic procedures continues to rise, there's a shift toward absorbable fillers like hyaluronic acid-based ones, favored for their safer outcomes. Evidence-based practices and ongoing research are crucial for ensuring the safety and efficacy of aesthetic procedures, ultimately enhancing patient outcomes and confidence.

Effects of low-level laser therapy and orthodontic tooth movement on dental pulps in rats.

Abstract:OBJECTIVES To describe the microscopic pulpal reactions resulting from orthodontically induced tooth movement associated with low-level laser therapy (LLLT) in rats. MATERIALS AND METHODS Forty-five young male Wistar rats were randomly assigned to three groups. In group I (n = 20), the maxillary right first molars were submitted to orthodontic movement with placement of a coil spring. In group II (n = 20), the teeth were submitted to orthodontic movement plus LLLT at 4 seconds per point (buccal, palatal, and mesial) with a GaAlAs diode laser source (830 nm, 100 mW, 18 J/cm(2)). Group III (n = 5) served as a control (no orthodontic movement or LLLT). Groups I and II were divided into four subgroups according to the time elapsed between the start of tooth movement and sacrifice (12 hours, 24 hours, 3 days, and 7 days). RESULTS Up until the 3-day period, the specimens in group I presented a thicker odontoblastic layer, no cell-free zone of Weil, pulp core with differentiated mesenchymal and defense cells, and a high concentration of blood vessels. In group II, at the 12- and 24-hour time points, the odontoblastic layer was disorganized and the cell-free zone of Weil was absent, presenting undifferentiated cells, intensive vascularization with congested capillaries, and scarce defense cells in the cell-rich zone. In groups I and II, pulpal responses to the stimuli were more intense in the area underneath the region of application of the force or force/laser. CONCLUSIONS The orthodontic-induced tooth movement and LLLT association showed reversible hyperemia as a tissue response to the stimulus. LLLT leads to a faster repair of the pulpal tissue due to orthodontic movement.

Abstract:BackgroundA multitude of options are traditionally used for the treatment of acne scars; however, newer treatment modalities are emerging to decrease the propensity for post-inflammatory hyperpigmentation and upregulate new collagen production. The aim of this study was to evaluate the efficacy of nanofat and platelet-rich plasma (PRP) infiltration alone and combined with fractional CO2 laser resurfacing to improve atrophic scars of the face.MethodsFrom March 2014 to June 2015, 30 patients with atrophic acne scars on the cheeks were selected for this study. Patients were evaluated pre- and postoperatively by physical examination, photographs and ultrasound with a 22-MHz probe to measure subcutaneous tissue thickness. All patients were treated with infiltration of nanofat plus PRP. The production of PRP was achieved using the RegenLab THT tube® method. In 15 randomly chosen patients, a fractional CO2 laser resurfacing at 15 W was also performed right after the infiltration. An Italian version of the FACE-Q postoperative module was administered to analyze each patient’s satisfaction and aesthetic perception of the result.ResultsThe average preoperative thickness of subcutaneous tissue of patients from group A was 0.532 cm, while the average preoperative thickness of subcutaneous tissue of patients from group B was 0.737 cm. The average postoperative thickness of subcutaneous tissue was 1.201 cm in group A and 1.367 cm in group B. The improvement of thickness of subcutaneous tissue was 0.668 cm in group A and 0.63 cm in group B. We applied a t test on unpaired data, comparing the difference in thickness obtained with the treatment in both group A and in group B, with a p value =0.7289 (not significant). All patients in both groups had a treatment benefit, confirmed with FACE-Q postoperative module, but without a significant difference between the two groups.ConclusionsSubcutaneous infiltration with nanofat and PRP seems to be effective to improve atrophic scars, either alone or combined with fractional CO2 laser resurfacing. The FACE-Q module confirmed the impact of treatment of facial acne scars in social life and relationships.Level of Evidence IIIThis journal requires that authors assign a level of evidence to each article. New Protocol: EndoliftX Laser and Hyaluronic Ac+ Calcium Hydroxiapaptite as an Effective Approach for Facial Rejuvenation- Case Series of 7 Patients

Abstract:Background: Excessive submental fat and tissue laxity can significantly impact an individual's aesthetic appearance, leading to cosmetic dissatisfaction and concerns about aging. Non-invasive and minimally invasive treatments have gained popularity as alternatives to surgical interventions. Laser lipolysis, such as EndoliftX®, has emerged as a promising approach for submental fat reduction, and hybrid fillers, like HArmonyCa, have shown potential for enhancing tissue laxity. Objective: This study aimed to evaluate the efficacy and long-term outcomes of a novel combined treatment protocol utilizing laser lipolysis and hybrid filler injection for submental fat reduction and tissue tightening. Methods and Materials: A single-center, prospective, quasi-experimental controlled study was conducted, enrolling seven patients aged between 49 and 67 years. The protocol EndoliftX®, involved a 1470nm diode laser (LASEmaR 1500® Eufoton® Trieste,Italy) treatment (Endolift) followed by hybrid filler (HArmonyCa) injection. Various assessments, including clinical examination, Glogau Scale scoring, and 3D imaging, were performed at baseline and follow-up visits. Results: The results demonstrated significant reductions in submental fat, improved cutaneous elasticity, and reduced wrinkle severity according to the Glogau Scale. Notably, these improvements were visible almost immediately post-procedure and continued to enhance over time, remaining prominent even after 12 months of observation. The treatment was associated with minimal discomfort and downtime. Conclusion: The study highlights the effectiveness and longevity of the combined treatment protocol, making it a valuable option for individuals seeking non-surgical solutions for submental fat reduction and tissue tightening. While further research with control groups is warranted, laser lipolysis with EndoliftX®, and hybrid filler injection with HArmonyCa offer a minimally invasive and well-tolerated approach to address submental concerns and achieve favorable aesthetic outcomes, particularly in patients looking to avoid more invasive techniques.
Investigating the Efficacy of Modified Lipoaspirate Grafting to Improve the Appearance of Atrophic Acne Scars: A Pilot Study

Abstract:Background: Processed lipoaspirate grafting describes several techniques theorized to leverage the inflammatory and regenerative capacities of mechanically processed adipocytes to rejuvenate and correct skin pathology. Although lipoaspirate grafting is typically leveraged to fill visible defects such as depressed scars and dermal lines, additional fat processing allows grafts to stimulate mechanisms of wound healing, including the promotion of fibroblast activation, neovascularization, and neocollagenesis. Objectives: This study intends to assess the efficacy and tolerability of processed lipoaspirate grafting monotherapy to improve the clinical appearance of atrophic acne scars. Methods: Patients underwent a single autologous processed lipoaspirate grafting procedure at the site of atrophic acne scars. Objective and subjective scar analysis was performed at 3 and 6 months posttreatment. Scars were assessed with standard photography, topographic analysis, and noninvasive skin measurements. In addition, microbiopsies were obtained before and after treatment to assess histological or genetic changes. Clinical improvement was assessed with patient and clinician Global Aesthetic Improvement Scales (GAIS) and blinded photographic evaluation. Results: Ten patients between ages 18 and 60 completed the study. Clinical evaluation demonstrated that fat grafting improved the appearance of atrophic acne scars. Clinician GAIS and patient GAIS scores showed clinical improvement at both 3- and 6-month follow-up compared with baseline (P < .05). Blinded clinician GAIS scores also showed statistically significant improvement when clinicians compared clinical photographs taken at 6-month follow-up to baseline (P < .0001). The attenuation coefficient increased at 6-month follow-up, suggesting collagen remodeling and reorganization over the study period. Patients experienced anticipated posttreatment symptoms including transient erythema and edema; however, no unexpected adverse events were reported. Conclusions: Micronized lipoaspirate injection is a viable and effective option to improve the appearance of facial acne scarring. Favorable improvements in atrophic acne scarring were captured by objective analysis of skin ultrastructure as well as improvement in subjective assessments of scarring.
A 12-Month Analysis of the Dermatologic Effects and Durability of Midface Volume Correction With DEFCL Volumizing Filler in a Prospective, Single-Center Study

Abstract:Background: Certain manufacturing processes confer distinctive rheologic features to hyaluronic acid (HA), ensuring long-lasting effects. Skin quality improvement and the volumizing effects of the DEFCL volumizer filler, a sterile, biodegradable, viscoelastic, transparent, isotonic, and homogenized injectable cross-linked high G' filler for the treatment of moderate-to-severe age-related midface changes, were initially documented at 6 months after the first injection. Objective: The authors aimed to objectively evaluate the effects of the DEFCL volumizer filler in improving skin density, thickness, and biomechanical properties and on tissue volume augmentation in women with midface volume loss after 12 months. Materials and methods: Fifty women with midface volume loss were recruited in this prospective, noncomparative, single-center, postmarket study. The authors report changes in skin density, thickness, and quality and subjective and objective evaluation of facial and/or cheek volume augmentation using the Global Aesthetic Improvement Scale, as well as injection site reactions and adverse events. Results: Improvements in skin quality and thickness and volumization were maintained 12 months after the first injection. Injector and patient satisfaction were highly rated with only mild adverse reactions observed. Conclusion: Skin improvement and volumizing effects persisted in patients treated with the DEFCL volumizer after 12 months.

The study investigates the aesthetic outcomes, patient satisfaction, and adverse events associated with the injectable filler. Patients and Methods: Seventy patients were initially enrolled, with 60 meeting study parameters. The post-market study involved a single session treatment, employing NEAUVIA Intense on each side of the NLF. Assessments utilized the Modified Fitzpatrick Wrinkle Scale (MFWS), Global Aesthetic Improvement Scale (GAIS), and Visual Analogical Scale (VAS). Results: The study demonstrated a statistically significant improvement in tissue depression immediately post-injection (p < 0.001), with sustained effects up to 6 months. MFWS assessments revealed that responder patients were 96.6% immediately after treatment, 76.6% one month, 48.3% after 3 months, and 28.3% at 6 months (p < 0.001). Additionally, there was a significant change in the frequency distribution of MFWS scores post-treatment (p < 0.001), with the majority of patients experiencing improvement in tissue depression. Maximum improvement was observed at 30- and 90-days post-treatment based on GAIS assessments. Patient and physician satisfaction, measured by VAS, remained stable over time, with fluctuations at 4 and 24 weeks after treatment (p < 0.001, Anova; p < 0.05, Wilcoxon). Throughout the entire follow-up duration of the patients enrolled in the study, no adverse effects related to the use of the product were observed. Conclusion: NEAUVIA Intense proved to be an effective solution for correcting NLF, providing significant and lasting improvements in tissue depression and aesthetic outcomes. The study underscores the necessity for continuous assessment in aesthetic medicine to align outcomes with evolving patient expectations and optimize long-term results. A treatment algorithm for hyaluronic acid filler related complications of the face

Abstract:BACKGROUND: Throughout the last decade, a notable increase in HA-filler-related complications have been observed, owing to the increase in demand for filler injections and availability of multiple products.OBJECTIVES: The aim is to provide practical advice on the best way to prevent and treat HA-filler-related complications.METHODS: Thirty patients who experienced visible and/or symptomatic complications localized within the facial area were treated according to our algorithm. Patients with inflammatory lesions underwent antibiotic and anti-inflammatory therapy, followed by hyaluronidase injections. Patients with abscesses were treated with antibiotics, incision, and drainage. Each patient completed the dermatology-specific quality of life questionnaire (DLQI) at the first and last examinations.RESULTS: Among the 29 patients who received antibiotic therapy, 3 healed without further treatment. However, 18 received hyaluronidase injections, 9 underwent incision and drainage, and 5 presented with fistulas and developed retracted scars. Moreover, 80% of the patients were completely healed, 13% significantly improved, and 3% did not show any improvement. The DLQI scores analysis showed a notable impact of patients' diseases on their quality of life, mainly in the terms of personal relationships and symptoms, with minor impacts on intimate relationships, ability to work, and study. We demonstrated that our algorithm resulted in a significant improvement in the overall quality of life at the last follow-up (p < 0.001).CONCLUSIONS: The use of filler injections requires caution and specific training because they can lead to serious complications. If these complications are recognized promptly, healing can be optimized. Our treatment algorithm demonstrated high rate of healing and significant improvement in the patients' quality of life.

Abstract:BACKGROUND: Treatment with filler injections using a microneedle fractional radiofrequency (MFRF) device is a promising modality with proven efficacy for acne scar treatment.OBJECTIVE: To investigate the efficacy and histologic differences of intradermal injection of a filler (poly-d, l-lactic acid, PDLA) using an MFRF device for the treatment of acne scars.METHODS: Patients with acne scars on both cheeks were included. Poly-d, l-lactic acid was injected via the MFRF device every 4 weeks for a total of 4 sessions. Patients were evaluated using the grading system for acne scars before each session, as well as personal satisfaction. For histologic evaluation, 2 patients (who consented) underwent a skin biopsy from the upper arm before and after the same single session.RESULTS: After the final session, the acne scar grading (échelle d'évaluation clinique des cicatrices d'acné) scale and visual analog scale for evaluation of satisfaction showed improvement compared with initial assessment (36.99% and 79.65% respectively [p < .001, respectively]). For histologic evaluation, biodegradation of PDLA materials and increase in collagen and elastic fibers were observed after 5 months of treatment.CONCLUSION: Intradermal injection of PDLA using the MFRF device could be used as an effective treatment with fewer side effects in acne scar patients with Fitzpatrick skin type III-IV.

Abstract:BACKGROUND: An increasing number of soft tissue fillers have been introduced to the beauty market and these filler substances are widely used as non-toxic, non-immunogenic and relatively harmless injectable alternatives to surgical rejuvenation. Generally, facial fillers are injectable - or surgically insertable - products that are used to fill up the volume loss in the aging face. Depending on bioavailability, chemical composition and degradation, fillers can be classified as temporary or permanent, organic or inorganic and autologous or heterologous.OBJECTIVE: A plethora of new products has swamped the beauty market since face rejuvenation has become socially acceptable as well as affordable to a wider population, but adverse reactions cannot be excluded. We present 4 patients with complications after injection of facial fillers [including Artecoll(polymethylmethacrylate microspheres), Restylane (hyaluronic acid), DermaLive (hyaluronic acid plus acrylic hydrogel particles) and Newfill (polylactic acid)] and surgical correction.RESULTS: Surgical intervention led to good aesthetic and functional results after multiple unsuccessful conservative therapies.CONCLUSION: We recommend that only physicians familiar with the injection techniques and the biological and chemical characteristics of the various injectable products should perform such interventions. Especially permanent fillers should be used with utmost reticence in cosmetic surgery and we would recommend their application only in reconstructive procedures. Additionally, documentation and reporting of all adverse effects must be mandatory.

Abstract:BACKGROUND Hyaluronic acid-based filler injections with parenteral anesthetics have become the standard in treating midface volume deficits. There are currently limited data on the effects of these types of fillers on skin density, thickness, and firmness. OBJECTIVE This study aimed to assess the efficacy of XTR CL filler in improving skin quality and tissue volume in women with midface volume loss. MATERIALS AND METHODS In this prospective, noncomparative, single-center study, 50 women aged between 40 and 60 years with midface volume loss were recruited. The primary endpoint was the improvement in investigator-assessed Global Aesthetic Improvement Scores (GAIS) 1 month after treatment. Secondary endpoints include objective measurements of skin density, thickness, and quality measurements, facial and/or cheek volume augmentation, subjective GAIS, and device evaluation from after the first injection until 6 months, and the documentation of injection site reactions and adverse events. RESULTS XTR CL use led to significant improvement in midface volume deficits, and skin quality and skin thickness. Injector and subject satisfaction with the treatment were documented and only mild-to-moderate adverse reactions were reported. CONCLUSION XTR CL was shown to be effective in improving volume loss and skin quality at 6 months.

A Systematic Review of Laser Therapy and Injectable Filler Approaches

Abstract:Localized scleroderma (LS), commonly known as morphea, presents a significant clinical challenge due to its chronic, inflammatory nature affecting the skin and potentially underlying tissues. This systematic review explores the innovative approach of combining laser therapy and injectable fillers, specifically hyaluronic acid, for the treatment of LS. We conducted a comprehensive literature review following PRISMA guidelines, examining articles from MEDLINE/PubMed to assess the combined efficacy of these treatments in improving both esthetic and functional outcomes for LS patients. The search yielded 64 articles, with six selected for in-depth analysis for a total of nine patients, covering a range of patient demographics and treatment types. Our review highlights cases where fractional CO2 laser therapy promoted long-term tissue remodeling and instances where hyaluronic acid fillers effectively addressed skin atrophy and volume loss, enhancing both immediate and long-lasting esthetic improvements. The synergy between these treatments suggests a promising dual approach, aiming to maximize esthetic outcomes and to improve the quality of life for LS patients. This review underscores the necessity of further research to establish a comprehensive, evidence-based clinical pathway integrating both treatments for managing LS, thereby enhancing patient satisfaction and addressing the multifaceted nature of this challenging dermatological condition.

Abstract:All fillers are associated with the risk of both early and late complications. Early side effects such as swelling, redness, and bruising occur after intradermal or subdermal injections. The patient has to be aware of and accept these risks. Adverse events that last longer than 2 weeks can be attributable to technical shortcomings (e.g., too superficial an implantation of a long-lasting filler substance). Such adverse events can be treated with intradermal 5-fluorouracil, steroid injections, vascular lasers, or intense pulsed light, and later with dermabrasion or shaving. Late adverse events also include immunologic phenomena such as late-onset allergy and nonallergic foreign body granuloma. Both react well to intralesional steroid injections, which often have to be repeated to establish the right dose. Surgical excisions shall remain the last option and are indicated for hard lumps in the lips and visible hard nodules or hard granuloma in the subcutaneous fat.
Soft-Tissue Augmentation and the Role of Poly-L-Lactic Acid

Abstract:BACKGROUND: Facial rejuvenation has traditionally centered on the correction of ptosis, lines, and wrinkles, typically achieved by surgical tightening of the skin or by injecting soft-tissue fillers. In recent years, volumetric augmentation and recontouring have been recognized as fundamentally important aspects of facial rejuvenation.METHODS: An overview of volume restoration and recontouring is presented. A literature search was also conducted to identify the injectable products representing potential treatment options for facial recontouring. The author also presents the results of his extensive experience with poly-L-lactic acid in the treatment of thousands of cosmetic patients in Europe, with special emphasis on the efficacy and tolerability of this product.RESULTS: Several injectable treatment options can be used to recontour and restore volume, to the face and beyond. All of these products, except calcium hydroxylapatite and poly-L-lactic acid, are nonresorbable, offering permanent volume augmentation, but with the increased risk of severe foreign-body reactions. Poly-L-lactic acid provides semipermanent results and has been documented to offer volume augmentation with a favorable safety profile. Achieving good efficacy and safety is dependent on the appropriate reconstitution of the product and adhering to the correct injection technique.CONCLUSIONS: Injectable products offer a flexible means of recontouring the face and localized regions beyond the face. The potential of some of these products is supported by a wealth of published results and clinical experience, helping the physician to identify the most appropriate treatment for their patient.

Abstract:BACKGROUND: Midface volume loss and contour deficiency are often treated using soft-tissue fillers capable of elevating and supporting facial features.OBJECTIVE: To evaluate the effectiveness and safety of large gel particle hyaluronic acid with 0.3% lidocaine (LGP-HA-L) for the treatment of midface volume loss or contour deficiency.METHODS AND MATERIALS: This 24-week, open-label study recruited adults aged 18 to 65 with bilateral moderate to substantial midface volume loss or contour deficiency (Medicis Midface Volume Scale (MMVS) score 3-4). The primary effectiveness measure was MMVS response (≥1-grade improvement) 8 weeks after treatment according to evaluators blinded to injection volume.RESULTS: All enrolled patients (n = 40; 85.0% female; mean ± SD age 53.1 ± 7.0 years) completed the study. Mean ± SD injected volume for both sides of the midface was 5.3 ± 2.5 mL. Week 8 MMVS response rates were 97.5% according to blinded evaluators and treating investigators and 90.0% according to patients; week 24 rates were 90.0%, 92.5%, and 82.5%, respectively. Global Aesthetic Improvement Scale response rates were 95.0% to 100.0% throughout the study. Adverse events were reported in 60.0% of patients and were mild or moderate; all resolved by study end and most within 1 week.CONCLUSION: Midface volume loss or contour deficiency may be safely and effectively corrected using LGP-HA-L.

Abstract:Background: Microneedling and soft-tissue filler injections have been used independently to improve acne scarring. The effectiveness of a combined approach using microneedling followed by polymethylmethacrylate (PMMA)-collagen gel has not been carefully studied. Objective: The goal of this study was to assess the effectiveness and safety of microneedling alone versus microneedling followed by injection of PMMA-collagen gel filler for correction of atrophic facial acne scars. Methods: We conducted a multicenter, open-label, randomized, prospective study on subjects with distensible atrophic acne scars in the face to determine whether microneedling with PMMA-collagen gel is a superior acne scar treatment over microneedling alone. Forty-four subjects received 3 microneedling treatments over a 12-week period followed by randomization to treatments with PMMA-collagen gel (treatment group) or no further treatment (control group). Results: At 24 weeks, the treatment group achieved a statistically significant improvement in acne scores over microneedling alone. The improvement continued at 36 weeks. At 24 weeks, the treatment group showed a strong trend in improvement on the Physician Global Aesthetic Improvement Scale compared with microneedling alone.

Abstract:BACKGROUND A laser-induced needle-free microjet injector was developed for rapid, high-speed drug delivery of microliters into the skin. OBJECTIVE This study evaluated the clinical rejuvenation effect of repeated dermal injections of the collagen simulator poly- dl -lactic acid (PDLA) using a laser-induced needle-free microjet injector. METHODS Five PDLA injection sessions using a laser-induced needle-free microjet injector were conducted in patients concerned about aging skin. Facial uplifting, darkness, redness, roughness, pore size, subjective satisfaction, and side effects were evaluated before each session and 4 weeks after treatment completion. Histological evaluation was also performed with immunohistochemical staining of collagen and elastic fibers. RESULTS The clinical results of 27 female patients were evaluated. The treatment resulted in a noticeable skin surface uplifting (0.711 ± 0.42 mm) and significant improvements in darkness ( p = .013), redness ( p = .009), and roughness ( p = .036), with no significant difference in the pore size ( p = .770). Patients were reported being satisfied with the overall therapeutic effects, despite mild and tolerable adverse effects. Histological findings revealed growth and thickening of collagen and elastic fibers, with marked increase in collagen I and III levels. CONCLUSION Repeated dermal injections of PDLA using a laser-induced microjet injector offer excellent drug delivery, achieving high efficacy in skin rejuvenation, patient satisfaction, and safety.
