Abstract:Background: We created a United States-based real-world data resource to better understand the continued impact of the COVID-19 pandemic on immunocompromised patients, who are typically under-represented in prospective studies and clinical trials. Methods: The COVID-19 Real World Data infrastructure (CRWDi) was created by linking and harmonizing deidentified HealthVerity medical and pharmacy claims data from December 1, 2018 to December 31, 2023, with SARS-CoV-2 virologic and serologic laboratory data from major commercial laboratories and Northwell Health; COVID-19 vaccination data; and for patients with cancer, 2010 to 2021 National Cancer Institute Surveillance, Epidemiology, and End Results registry data. Results: The CRWDi dataset contains data on 5.2 million people. Four populations were included in the dataset: (1) patients with cancer (n=1,294,022); (2) patients with rheumatic conditions receiving pharmacotherapy (n=1,636,940); (3) non-cancer solid organ (n=249,797) and hematopoietic stem cell (n=30,172) transplant recipients; and (4) people from the general population including adults (>18 years of age; n=1,790,162) and pediatric patients (<18 years of age; n=198,907). Conclusions: We have created a complex real-world data system to address unanswered questions that have arisen during the COVID-19 pandemic. Further, by making the data broadly and freely available to academic researchers from the United States, the CRWDi real-world data system represents an important complement to existing consortia studies and clinical trials that have emerged during the healthcare crisis, and is readily reproducible for future purposing.

Abstract:In 2020, the coronavirus disease 2019 (COVID-19) pandemic has caused unprecedented disruptions to global health systems. The Korea has taken full-fledged actions against this novel infectious disease, swiftly implementing a testing-tracing-treatment strategy. New obligations have therefore been given to the Health Insurance Review and Assessment Service (HIRA) to devote the utmost effort towards tackling this global health crisis. Thanks to the universal national health insurance and state-of-the-art information communications technology (ICT) of the Korea, HIRA has conducted far-reaching countermeasures to detect and treat cases early, prevent the spread of COVID-19, respond quickly to surging demand for the healthcare services, and translate evidence into policy. Three main factors have enabled HIRA to undertake pandemic control preemptively and systematically: nationwide data aggregated from all healthcare providers and patients, pre-existing ICT network systems, and real-time data exchanges. HIRA has maximized the use of data and pre-existing network systems to conduct rapid and responsive measures in a centralized way, both of which have been the most critical tactics and strategies used by the Korean healthcare system. In the face of new obligations, our promise is to strive for a more responsive and resilient health system during this prolonged crisis.

Abstract:The SARS-CoV-2 outbreak highlighted the challenges associated with limited healthcare resources during a surge of infectious spread and lack of policies addressing the equal or equitable distribution of resources. [ 1–3 ] This directly impacted the health care system, including the critical care services, and the clinical outcomes of patients with severe COVID-19. [ 4–7 ] In early 2020, there was no effective therapy against the viral infection. Quarantine and supportive treatment were the mainstays for patients hospitalized with severe SARS-COV-2 infection. The negative conversion of SARS-COV-2 RNA by RT-PCR was the only biomarker for release from quarantine for a considerable wave. The shortage of intensive care unit (ICU) resources almost reached the limit of locoregional collapse of healthcare system during the early pandemic wave. [ 8 ] Healthcare policy makers in Korea declared the so-called 3T strategy (test, trace, and treatment) [ 9 ] and tried to quarantine all PCR-confirmed cases at home or in dedicated hospitals. [ 10 ] The strategy was effective in decreasing the surge of each variant's spread compared to that in other countries. [ 9 ] Despite the success of Korea in preventing an overwhelming number of case fatalities, its healthcare system was overloaded. This resulted in collateral damage to non-covid-critically ill patients, which is yet to be fully evaluated. [ 11 , 12 ] Therefore, the pattern of healthcare resource use and outcome of critically ill COVID-19 patients deserved to be assessed. While there are many studies that have analyzed broader national trends and outcomes for COVID-19, there is no national compilation of ICU data. The current study may help designing policies that will guide the optimal utilization of healthcare resources through our accumulated experience and knowledge. This study aimed to collect data of critically ill COVID-19 patients and analyze the treatment patterns and outcomes of those who received critical care services during the early pandemic wave.

Abstract:The International Severe Acute Respiratory and Emerging Infection Consortium (ISARIC) COVID-19 dataset is one of the largest international databases of prospectively collected clinical data on people hospitalized with COVID-19. This dataset was compiled during the COVID-19 pandemic by a network of hospitals that collect data using the ISARIC-World Health Organization Clinical Characterization Protocol and data tools. The database includes data from more than 705,000 patients, collected in more than 60 countries and 1,500 centres worldwide. Patient data are available from acute hospital admissions with COVID-19 and outpatient follow-ups. The data include signs and symptoms, pre-existing comorbidities, vital signs, chronic and acute treatments, complications, dates of hospitalization and discharge, mortality, viral strains, vaccination status, and other data. Here, we present the dataset characteristics, explain its architecture and how to gain access, and provide tools to facilitate its use.

Abstract:As the COVID-19 outbreak evolves around the world, the World Health Organization (WHO) and its Member States have been heavily relying on staying at home and lock down measures to control the spread of the virus. In the last months, various signs showed that the COVID-19 curve was flattening, but even the partial lifting of some containment measures (e.g., school closures and telecommuting) appear to favor a second wave of the disease. The accurate evaluation of possible countermeasures and their well-timed revocation are therefore crucial to avoid future waves or reduce their duration. In this paper, we analyze patient and route data of infected patients from January 20, 2020, to May 31, 2020, collected by the Korean Center for Disease Control & Prevention (KCDC). This data analysis helps us to characterize patient mobility patterns and then use this characterization to parameterize simulations to evaluate different what-if scenarios. Although this is not a definitive model of how COVID-19 spreads in a population, its usefulness and flexibility are illustrated using real-world data for exploring virus spread under a variety of circumstances.

Abstract:Background: The COVID-19 pandemic sparked a surge of research publications spanning epidemiology, basic science, and clinical science. Thanks to the digital revolution, large data sets are now accessible, which also enables real-time epidemic tracking. However, despite this, academic faculty and their trainees have been struggling to access comprehensive clinical data. To tackle this issue, we have devised a clinical data repository that streamlines research processes and promotes interdisciplinary collaboration. Objective: This study aimed to present an easily accessible up-to-date database that promotes access to local COVID-19 clinical data, thereby increasing efficiency, streamlining, and democratizing the research enterprise. By providing a robust database, a broad range of researchers (faculty and trainees) and clinicians from different areas of medicine are encouraged to explore and collaborate on novel clinically relevant research questions. Methods: A research platform, called the Yale Department of Medicine COVID-19 Explorer and Repository (DOM-CovX), was constructed to house cleaned, highly granular, deidentified, and continually updated data from over 18,000 patients hospitalized with COVID-19 from January 2020 to January 2023, across the Yale New Haven Health System. Data across several key domains were extracted including demographics, past medical history, laboratory values during hospitalization, vital signs, medications, imaging, procedures, and outcomes. Given the time-varying nature of several data domains, summary statistics were constructed to limit the computational size of the database and provide a reasonable data file that the broader research community could use for basic statistical analyses. The initiative also included a front-end user interface, the DOM-CovX Explorer, for simple data visualization of aggregate data. The detailed clinical data sets were made available for researchers after a review board process. Results: As of January 2023, the DOM-CovX Explorer has received 38 requests from different groups of scientists at Yale and the repository has expanded research capability to a diverse group of stakeholders including clinical and research-based faculty and trainees within 15 different surgical and nonsurgical specialties. A dedicated DOM-CovX team guides access and use of the database, which has enhanced interdepartmental collaborations, resulting in the publication of 16 peer-reviewed papers, 2 projects available in preprint servers, and 8 presentations in scientific conferences. Currently, the DOM-CovX Explorer continues to expand and improve its interface. The repository includes up to 3997 variables across 7 different clinical domains, with continued growth in response to researchers' requests and data availability. Conclusions: The DOM-CovX Data Explorer and Repository is a user-friendly tool for analyzing data and accessing a consistently updated, standardized, and large-scale database. Its innovative approach fosters collaboration, diversity of scholarly pursuits, and expands medical education. In addition, it can be applied to other diseases beyond COVID-19.
Evaluating Trends in COVID-19 Research Activity in Early 2020: The Creation and Utilization of a Novel Open-Access Database

Abstract:Introduction The coronavirus disease 2019 (COVID-19) pandemic has been unprecedented in recent history. The rapid global spread has demonstrated how the emergence of a novel pathogen necessitates new information to advise both healthcare systems and policy-makers. The directives for the management of COVID-19 have been limited to infection control measures and treatment of patients, which has left physicians and researchers alone to navigate the massive amount of research being published while searching for evidence-based strategies to care for patients. To tackle this barrier, we launched CovidReview.ca, an open-access, continually updated, online platform that screens available COVID-19 research to determine higher quality publications. This paper uses data from this review process to explore the activity and trends of COVID-19 research worldwide over time, while specifically looking at the types of studies being published. Materials and Methods The literature search was conducted on PubMed. Search terms included "COVID-19", "severe acute respiratory syndrome coronavirus 2", "coronavirus 19", "SARS-COV-2", and "2019-nCoV". All articles captured by this strategy were reviewed by a minimum of two reviewers and categorized by type of research, relevant medical specialties, and type of publication. Criteria were developed to allow for inclusion or exclusion to the website. Due to the volume of research, only a level 1 (title and abstract) screen was performed. Results The time period for the analysis was January 17, 2020, to May 10, 2020. The total number of papers captured by the search criteria was 10,685, of which 2,742 were included on the website and 7,943 were excluded. The greatest increase in the types of studies over the 16 weeks was narrative review/expert opinion papers followed by case series/reports. Meta-analyses, systematic reviews, and randomized controlled trials remained the least published types of studies. Conclusions The surge of research that accompanied the COVID-19 pandemic is unparalleled in recent years. From our analysis, it is clear that case reports and narrative reviews were the most widely published, particularly in the earlier days of this pandemic. Continued research that falls higher on the evidence pyramid and is more applicable to clinical settings is warranted.

Abstract:AbstractObjectiveThe novel coronavirus disease (COVID-19), broke out in December 2019, is a global pandemic. Rapidly in the past few months, a large number of clinical studies have been initiated worldwide to find effective therapeutics, vaccines, and preventive strategies. In this study, we aim to understand the landscape of COVID-19 clinical research and identify the gaps and issues that may cause difficulty in recruitment and the lack of population representativeness.Materials and MethodsWe analyzed 2,034 COVID-19 studies registered in the largest public registry - ClinicalTrials.gov. Leveraging natural language processing, descriptive analysis, association analysis, and clustering analysis, we characterized COVID-19 clinical studies by phase and design features. Particularly, we analyzed their eligibility criteria to understand: (1) whether they considered the reported underlying health conditions that may lead to severe illnesses, and (2) if these studies excluded older adults, either explicitly or implicitly, which may reduce the generalizability of these studies in older adults.ResultsThe 5 most frequently tested drugs are Hydroxychloroquine (N=148), Azithromycin (N=46), Tocilizumab (N=29), Lopinavir (N=20), and Ritonavir (N=20). Most trials did not have an upper age limit and did not exclude patients with common chronic conditions such as hypertension and diabetes that are prevalent in older adults. However, known risk factors that may lead to severe illnesses have not been adequately considered by existing studies.ConclusionsA careful examination of the registered COVID-19 clinical studies can identify the research gaps and inform future COVID-19 trial design towards balanced internal validity and generalizability.
Long-Term Autoimmune Inflammatory Rheumatic Outcomes of COVID-19 : A Binational Cohort Study

Abstract:Background: Some data suggest a higher incidence of diagnosis of autoimmune inflammatory rheumatic diseases (AIRDs) among patients with a history of COVID-19 compared with uninfected patients. However, these studies had methodological shortcomings. Objective: To investigate the effect of COVID-19 on long-term risk for incident AIRD over various follow-up periods. Design: Binational, longitudinal, propensity-matched cohort study. Setting: Nationwide claims-based databases in South Korea (K-COV-N cohort) and Japan (JMDC cohort). Participants: 10 027 506 Korean and 12 218 680 Japanese patients aged 20 years or older, including those with COVID-19 between 1 January 2020 and 31 December 2021, matched to patients with influenza infection and to uninfected control patients. Measurements: The primary outcome was onset of AIRD (per appropriate codes from the International Classification of Diseases, 10th Revision) 1, 6, and 12 months after COVID-19 or influenza infection or the respective matched index date of uninfected control patients. Results: Between 2020 and 2021, among the 10 027 506 Korean participants (mean age, 48.4 years [SD, 13.4]; 50.1% men), 394 274 (3.9%) and 98 596 (0.98%) had a history of COVID-19 or influenza, respectively. After propensity score matching, beyond the first 30 days after infection, patients with COVID-19 were at increased risk for incident AIRD compared with uninfected patients (adjusted hazard ratio, 1.25 [95% CI, 1.18 to 1.31]) and influenza-infected control patients (adjusted hazard ratio, 1.30 [CI, 1.02 to 1.59]). The risk for incident AIRD was higher with more severe acute COVID-19. Similar patterns were observed in the Japanese cohort. Limitations: Referral bias due to the pandemic; residual confounding. Conclusion: SARS-CoV-2 infection was associated with increased risk for incident AIRD compared with matched patients without SARS-CoV-2 infection or with influenza infection. The risk for incident AIRD was higher with greater severity of acute COVID-19. Primary funding source: National Research Foundation of Korea.

Abstract:In response to the ongoing coronavirus disease 2019 (COVID-19) pandemic, an online laboratory surveillance system was established to monitor severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) real-time reverse transcription-PCR (rRT-PCR) testing capacities and results. SARS-CoV-2 rRT-PCR testing data were collected from 97 clinical laboratories, including 84 medical institutions and 13 independent clinical laboratories in Korea. We assessed the testing capacities to utilize SARS-CoV-2 rRT-PCR based on surveillance data obtained from February 7th to June 4th, 2020 and evaluated positive result characteristics according to the reagents used and sample types. A total of 1,890,319 SARS-CoV-2 rRT-PCR testing were performed, 2.3% of which were positive. Strong correlations were observed between the envelope (E) gene and RNA-dependent RNA polymerase (RdRp)/nucleocapsid (N) genes threshold cycle (Ct) values for each reagent. No statistically significant differences in gene Ct values were observed between the paired upper and lower respiratory tract samples, except in the N gene for nasopharyngeal swab and sputum samples. Our study showed that clinical laboratories in Korea have rapidly expanded their testing capacities in response to the COVID-19 outbreak, with a peak daily capacity of 34,193 tests. Rapid expansion in testing capacity is a critical component of the national response to the ongoing pandemic.

Abstract:The COVID-19 pandemic has necessitated the development of robust tools for tracking and modeling the spread of the virus. We present 'K-Track-Covid,' an interactive web-based dashboard developed using the R Shiny framework, to offer users an intuitive dashboard for analyzing the geographical and temporal spread of COVID-19 in South Korea. Our dashboard employs dynamic user interface elements, employs validated epidemiological models, and integrates regional data to offer tailored visual displays. The dashboard allows users to customize their data views by selecting specific time frames, geographic regions, and demographic groups. This customization enables the generation of charts and statistical summaries pertinent to both daily fluctuations and cumulative counts of COVID-19 cases, as well as mortality statistics. Additionally, the dashboard offers a simulation model based on mathematical models, enabling users to make predictions under various parameter settings. The dashboard is designed to assist researchers, policymakers, and the public in understanding the spread and impact of COVID-19, thereby facilitating informed decision-making. All data and resources related to this study are publicly available to ensure transparency and facilitate further research.

COVID-19: An exploration of consecutive systemic barriers to pathogen-related data sharing during a pandemic

Abstract:In 2020, the COVID-19 pandemic resulted in a rapid response from governments and researchers worldwide. As of late 2023, over millions have died as a result of COVID-19, with many COVID-19 survivors going on to experience long-term effects weeks, months, or years after their illness. Despite this staggering toll, those who work with pandemic-relevant data often face significant systemic barriers to accessing, sharing or re-using this data. In this paper we report results of a study, where we interviewed data professionals working with COVID-19-relevant data types including social media, mobility, viral genome, testing, infection, hospital admission, and deaths. These data types are variously used for pandemic spread modelling, healthcare system strain awareness, and devising therapeutic treatments for COVID-19. Barriers to data access, sharing and re-use include the cost of access to data (primarily certain healthcare sources and mobility data from mobile phone carriers), human throughput bottlenecks, unclear pathways to request access to data, unnecessarily strict access controls and data re-use policies, unclear data provenance, inability to link separate data sources that could collectively create a more complete picture, poor adherence to metadata standards, and a lack of computer-suitable data formats.

Abstract:The coronavirus disease (COVID-19), caused by the SARS-CoV-2 virus, was declared a pandemic by the World Health Organization (WHO) in February 2020. Currently, there are no vaccines or treatments that have been approved after clinical trials. Social distancing measures, including travel bans, school closure, and quarantine applied to countries or regions are being used to limit the spread of the disease and the demand on the healthcare infrastructure. The seclusion of groups and individuals has led to limited access to accurate information. To update the public, especially in South Africa, announcements are made by the minister of health daily. These announcements narrate the confirmed COVID-19 cases and include the age, gender, and travel history of people who have tested positive for the disease. Additionally, the South African National Institute for Communicable Diseases updates a daily infographic summarising the number of tests performed, confirmed cases, mortality rate, and the regions affected. However, the age of the patient and other nuanced data regarding the transmission is only shared in the daily announcements and not on the updated infographic. To disseminate this information, the Data Science for Social Impact research group at the University of Pretoria, South Africa, has worked on curating and applying publicly available data in a way that is computer-readable so that information can be shared to the public - using both a data repository and a dashboard. Through collaborative practices, a variety of challenges related to publicly available data in South Africa came to the fore. These include shortcomings in the accessibility, integrity, and data management practices between governmental departments and the South African public. In this paper, solutions to these problems will be shared by using a publicly available data repository and dashboard as a case study.

Abstract:The COVID-19 Open Research Dataset (CORD-19) is a growing resource of scientific papers on COVID-19 and related historical coronavirus research. CORD-19 is designed to facilitate the development of text mining and information retrieval systems over its rich collection of metadata and structured full text papers. Since its release, CORD-19 has been downloaded over 200K times and has served as the basis of many COVID-19 text mining and discovery systems. In this article, we describe the mechanics of dataset construction, highlighting challenges and key design decisions, provide an overview of how CORD-19 has been used, and describe several shared tasks built around the dataset. We hope this resource will continue to bring together the computing community, biomedical experts, and policy makers in the search for effective treatments and management policies for COVID-19.

In response to the COVID-19 pandemic, the Allen Institute for AI has partnered with leading research groups to prepare and distribute the COVID-19 Open Research Dataset (CORD-19), a free resource of scholarly articles, including full text content, about COVID-19 and the coronavirus family of viruses for use by the global research community. This dataset is intended to mobilize researchers to apply recent advances in natural language processing to generate new insights in support of the fight against this infectious disease. The corpus will be updated weekly as new research is published in peer-reviewed publications and archival services like bioRxiv, medRxiv, and others. Abstract:Health Insurance and Review Assessment (HIRA) in South Korea, also called National Health Insurance (NHI) data, is a repository of claims data collected in the process of reimbursing healthcare providers. Under the universal coverage system, having fee-for-services covering all citizens in South Korea, HIRA contains comprehensive and rich information pertaining to healthcare services such as treatments, pharmaceuticals, procedures, and diagnoses for almost 50 million beneficiaries. This corpus of HIRA data, which constitutes a large repository of data in the healthcare sector, has enormous potential to create value in several ways: enhancing the efficiency of the healthcare delivery system without compromising quality of care; adding supporting evidence for a given intervention; and providing the information needed to prevent (or monitor) adverse events. In order to actualize this potential, HIRA data need to actively be utilized for research. Thus understanding this data would greatly enhance this potential. We introduce HIRA data as an important source for health research and provide guidelines for researchers who are currently utilizing HIRA, or interested in doing so, to answer their research questions. We present the characteristics and structure of HIRA data. We discuss strengths and limitations that should be considered in conducting research with HIRA data and suggest strategies for optimal utilization of HIRA data by reviewing published research using HIRA data.

Cardiovascular Research Using the Korean National Health Information Database

Abstract:The Korean National Health Information Database (NHID) contains nationwide claims data, including sociodemographic data, health care utilization, health screening data, and healthcare provider information. To compensate for the limitations of randomized clinical trials, real-world observational studies using claims data have emerged as a novel research tool. We summarized the structure of the Korean NHID and the recent researches conducted in the field of cardiovascular science. Epidemiological studies, prescription patterns, temporal trends, comparison of effectiveness and safety of treatments, variability index using laboratory data, and rare intractable disease constitute interesting topics of research in cardiovascular science using the NHID. The operational definition of covariates and clinical outcomes is important for researchers interested in using the NHID data as new tools to prove their hypothesis. A step-by-step approach adopted by a team of data scientists, epidemiologists, statisticians, and clinical researchers may be most effective while designing research studies. The ultimate direction of research using the NHID should aim to improve the welfare of the public by promoting public health, reducing medical costs, and guiding healthcare policies.

Abstract:Nandrolone, an anabolic androgenic steroid, reduced delta opioid receptor (DOR) mRNA and the number of DOR binding sites in two neuronal hybrid cell lines: NG 108-15 and the GT1-1 cells. Both cell lines express DOR but only GT1-1 cells express androgen receptors (AR). DOR mRNA levels were maximally decreased by ∼45% in NG 108-15 cells and by ∼38% in GT1-1 cells exposed for 24 h to 10−6 M nandrolone. This action was partly due to a decrease in the rate of transcription of DOR mRNA and was not blocked by the androgen antagonist flutamide. Flutamide antagonized the repression of AR mRNA induced by nandrolone. The synthetic glucocorticoid dexamethasone (10−6 M) did not modify DOR steady-state transcripts in either cell line. These results suggest that nandrolone presumably regulate DOR mRNA levels through mechanisms independent of the androgen and glucocorticoid receptors.

Abstract:Background: In December 2019, COVID-19 was first confirmed in Wuhan, China, and as the respiratory disease spread around the globe, there was a spike in interest worldwide in combating such contagious diseases. When such disasters occur, the central government of South Korea and its affiliated local governments-together with nongovernmental organizations-play a crucial role in crisis management systems. Objective: The purpose of this paper is to corroborate the characteristics government ministries and domestic and foreign institutions exhibit through their interconnection when the parties are undergoing a disease-related catastrophe such as the COVID-19 pandemic. Methods: Using the social network analysis technique, the span of the COVID-19 pandemic was segmented into 3 time frames, and the relational characteristics of the COVID-19 contagious disease response department and related agencies at home and abroad were analyzed based on 3 centralities. Results: Evidence from the second and third time frames indicates that the agents reacting to contagious diseases do not necessarily hold the central position in the network. From this, it can be inferred that it is not only the primary host that plays a pivotal role but the key to a successful response to various disasters also lies in cooperation with the relevant parties. Conclusions: The incongruency between the findings of this paper and the existing disaster response system gives rise to the corollary that both the essential parties and the adjoining ones need to collaborate for a coordinated crisis response in disaster situations. Furthermore, much significance lies in the fact that this paper explores the various aspects that could surface among the host and relevant parties in a real-life pandemic.
