Abstract:OBJECTIVE:To investigate the results of using shifting fixation eye to manage monocular or asymmetrical binocular dissociated vertical deviation (DVD).METHODS:Twenty-three DVD children whose corrected vision was better than 0.5 in the amblyopic eye were studied. The goal of the shifting fixation eye was gained by decreasing the vision of the primary fixation eye to 2 or 3 lines (Snellen Visual Acuity) lower than that of the fellow fixation eye by occlusion or optical methods. The vision, alignment and retinal correspondence were evaluated before and after the treatment.RESULTS:After the treatment, in 10 patients, the vision of the primary fixation eye did not decrease significantly and the vision of the fellow eye increased 2 lines. In 19 patients, a cosmetic result was achieved; the primary fixation eye was shifted in 4 patients that had similar hepertropia in two eyes (<or= 10(Delta)). Normal retinal correspondence was restored in 3 patients who had normal retinal correspondence at near while depressed at distance.CONCLUSIONS:Shifting fixation eye is an effective treatment for DVD hepertropia, especially in patients with monocular or asymmetrical binocular DVD with hepertropia in one eye and latent hepertropia in the fellow eye. This method is suitable to DVD children who do not accept the surgical treatment and have slight to moderate amblyopia and ametropia.
Occurrence and subsequent development of vertical deviations in patients treated surgically for infantile esotropia

Abstract:Purpose: The purpose of this study was to evaluate the onset of dissociated vertical deviation (DVD) and inferior oblique overaction (IOOA), their subsequent development, and their correlation with pre and postoperative parameters. Methods: Medical records of patients with infantile esotropia who underwent surgery between 2005 and 2017 were retrospectively reviewed. DVD and IOOA were measured before and after surgery. Patients were divided into two groups based on horizontal and vertical deviation at the time of presentation: those with infantile esotropia only (group A) and patients with infantile esotropia who developed vertical deviation (group B). Results: Out of a total of 102 patients, DVD occurrence was seen in 53 patients (51.9%) and IOOA was seen in 50 patients (48.04%). DVD was seen in 22 patients at the time of initial examination and in 31 patients postoperatively. IOOA at presentation was seen in 45 patients (44.1%) and 5 patients (8.8%) postoperatively. No statistical difference was found in the age of surgery, angle of deviation, mean follow-up, and mean refractive error within both groups. The postoperative motor outcome was statistically comparable between the two groups (P = 0.29). Sensory outcomes of fusion (P = 0.048) and stereopsis (P-value = 0.00063) were better in group A. Conclusion: No correlation was found between the age of occurrence and development of vertical deviation with refractive error, angle of deviation, age, or type of surgery. We found that motor outcomes are not affected but sensory outcomes are affected in patients with vertical deviations. This indicates that DVD and IOOA are developed due to inherent disruption of fusion and stereopsis.
Factors influencing the development and severity of dissociated vertical deviation in patients with infantile esotropia

Abstract:Purpose: To investigate the factors associated with the development and the severity of dissociated vertical deviation (DVD) in patients who underwent surgery for infantile esotropia. Methods: The medical records of consecutive patients who underwent surgery from March 1998 to December 2011 for infantile esotropia were reviewed retrospectively. The development, severity, and onset of DVD were assessed. Patients were divided into three groups: spontaneous, latent, and no DVD. The age at the time of the initial surgery, the angle of deviation of esotropia, anisometropia, amblyopia, stereopsis measured at the final visit, and history of prematurity were compared. Results: A total of 90 patients were included. DVD was detected in 52 (58%). Of these, 34 patients demonstrated spontaneous DVDs and 18 showed latent DVDs. In the spontaneous DVD group, 71% underwent surgery after 24 months and 50% had a large angle of esodeviation (>60(Δ)), compared to the latent DVD group, in which 22% underwent later surgery and 11% had large-angle esodeviations (P < 0.001 and P = 0.002, respectively), and the no DVD group, in which 16% underwent later surgery and 13% had large-angle esodeviations (P < 0.001 each). Multivariate logistic regression analysis between groups revealed that later surgery (OR = 8.23; P < 0.001) and large-angle esodeviation (OR = 6.32; P = 0.003) were associated that greater development of spontaneous DVD. Conclusions: Surgical correction for infantile esotropia prior to 24 months of age, especially in cases with a large amount of esodeviation, decreased the incidence of spontaneous DVD.
Evaluation of risk factors for re-operation cases with dissociated vertical deviation

Abstract:Objective To investigate the long-term outcome of different surgery design for dissociated vertical deviation (DVD) and discuss the reasons of re-operation.Methods This was a retrospective comparative study involving 315 patients with DVD.The correlation between gender,age,surgical design and re-operation rate were analyzed.Results In the 315 patients,280 (88.89％) cases underwent surgery only once.The median age was 6.5 years old (range,10 months to 37 years),236 patients (82.86％) were less than 12 years of age,in which,145 (51.79％) patients were preschoolers (＜7 years of age).Unilateral surgery was performed in 226 cases.35 of 315 cases (11.11％) underwent surgery twice.The median age was 6.5 years old similarly (range,3 to 18 years).The intervals between first and second surgery were from 1 to 67 months (mean,20.51 months).80％ of the re-operation cases were preschoolers.The re-operation rate of preschoolers was 19.3％ (28 of 145 cases),larger than that of school-age children (7 of 135 cases) and adults (0 of 12 cases).Eighty-nine patients underwent superior rectus muscle recession (the group of rectus muscle surgery),while 191 patients underwent inferior oblique muscle anterior displacement or recession (the group of oblique-muscle surgery).In group of rectus muscle surgery,the most common reasons of re-operation was secondary inferior oblique overaction (IOOA) in the contralateral eye; while in group of oblique-muscle surgery,the main reason was DVD in the contralateral eye.Conclusions This finding suggests that properly selected surgery design can effectively lower the rate of re-operation.

Abstract:Millimetre-wave (mmWave) 5G communications is an emerging technology to enhance the capacity of existing systems by thousand-fold improvement. Heterogeneous networks employing densely distributed small cells can optimise the available coverage and throughput of 5G systems. Efficiently utilising the spectrum bands by small cells is one of the approaches that will considerably increase the available data rate and capacity of the heterogeneous networks. This challenging task can be achieved by spectrum sensing capability of cognitive radios and new modulation techniques for data transmission. In this study, a wavelet-based filter bank is proposed for spectrum sensing and modulation in 5G heterogeneous networks. The proposed technique can mitigate the spectral leakage and interference by adapting the subcarriers according to cognitive information provided by wavelet packet based spectrum sensing (WPSS) and lowering sidelobes using wavelet-based filter bank multicarrier modulation. The performance improvement of WPSS compared with Fourier-based spectrum sensing is verified in terms of power spectral density comparison and probabilities of detection and false alarm. Meanwhile, the bit error rate performance demonstrates the superiority of the proposed wavelet-based system compared with its Fourier-based counterpart over the 60 GHz mmWave channel.

Abstract:Introduction: Intermittent exotropia (IXT) is commonly seen in areas with more sunlight, females and Asians. Surgical alignment is recommended to improve binocular function, but a high recurrence rate has been reported. This study aimed to assess the stability of ocular alignment and factors contributing to exodrift post-surgery for IXT over 6-month follow-up. Methods: We retrospectively reviewed the medical records of patients with IXT who underwent unilateral recess resect procedures and attended follow-up examinations for at least six months post-surgery. A significant exodrift was defined as exodrift of more than 10PD at distance and near. Ocular deviation, binocularity, and stereopsis at one and six-month post-operative visits were studied to assess the incidence of exodrift post-surgery and the probable factors causing such drift. Results: Of the 50 patients studied, 26 (52%) were males, 42 (84%) were <20 years. The median age at surgery was 10 years (range 6-14), and the median preoperative angle of deviation was 37 PD. Majority of patients (62%) had exodrift within six months of surgery, and about a quarter of the study population had significant exodrift >10 PD. No clinical factors were found to contribute significantly to exodrift in this study.16 (61.5%) patients attained near and distance BSV post-surgery (p-value: <0.001). Six (12%) and 10 (20%) patients with subnormal near and distance stereopsis, respectively, attained normal stereopsis post-surgery. Conclusion: Over half of the patients operated for IXT had some Exodrift, and one-fourth had significant exodrift within six months post-surgery. Despite improved BSV, many patients failed to attain normal stereopsis with just motor alignment.
Divergence Insufficiency Esotropia: Surgical Treatment

Abstract:Divergence insufficiency esotropia, or acquired comitant esotropia that is at least 10Δ larger at distance than at near, is most often seen in older adults, and may also be known as "age-related distance esotropia." Surgical treatment is often indicated for patients who do not tolerate prism therapy, or for those with large angles of esotropia. Surgical treatments have evolved with our understanding of the disease-state. Currently, accepted treatments include lateral rectus resection and medial rectus recession. These surgeries can be performed unilaterally or bilaterally. New surgical techniques such as lateral rectus equatorial myopexy are under investigation and may hold promise as future therapies.
Vertical rectus muscle transposition for bilateral Duane syndrome

Abstract:Background: Augmented transposition of the superior and inferior rectus muscles to the lateral rectus muscle is effective surgical treatment for esotropia in unilateral Duane syndrome. Medial rectus muscle recession in bilateral Duane syndrome may increase the risk of consecutive exotropia and cause limitation to adduction postoperatively. Vertical rectus muscle transposition may be useful in bilateral Duane syndrome with esotropia. Methods: We undertook a retrospective review of 11 patients with bilateral Duane syndrome and esotropia in primary position. All patients had vertical rectus muscle transpositions. Six patients had unilateral vertical rectus transpositions (2 eyes with and 4 without suture augmentation). Twelve eyes from 7 children (2 unilateral and 5 bilateral) had transpositions augmented with posterior fixation sutures. Posterior fixation suture were added to large deviations in patients without prior medial rectus recessions. Results: The preoperative esotropia at distance was 22.8 +/- 6.3 prism diopters (PD). It reduced to 2.0 +/- 6.7 PD postoperatively. (P < 0.001) Esotropia at near changed from 21.0 +/- 5.8 PD preoperatively to 1.2 +/- 8.1 PD postoperatively. (P < 0.001) One patient with a 10-degree face turn had complete resolution postoperatively. One patient had a small undercorrection and developed a vertical deviation requiring additional surgery. All patients had improvement in abduction. Nine of 11 patients did not develop any limitation to adduction. One patient developed a -1 adduction deficit 5 years later. Three patients achieved fusion with a mean stereovision of 67 seconds of arc (range, 80-40 seconds.). Follow-up averaged 22.2 months (range, 1-100 months). Conclusion: Vertical rectus muscle transposition in patients with bilateral Duane syndrome and esotropia is an effective procedure to improve ocular alignment and motility while preserving adduction.
Surgical outcomes and occurrence of associated vertical strabismus during a 10-year follow-up in patients with infantile esotropia

Abstract:Purpose: Authors analyzed long-term surgical outcomes of infantile esotropia and the occurrence of associated strabismus, inferior oblique overaction (IOOA), and dissociated vertical deviation (DVD). Clinical factors related to the occurrence of IOOA and DVD in patients with infantile esotropia were also evaluated. Methods: Medical records of patients with infantile esotropia, who underwent surgery between 1995 and 2008, were reviewed retrospectively. Included patients were followed for at least 10 years. The incidence and age at development of IOOA and DVD were analyzed. To evaluate predisposing factors for developing IOOA or DVD, patients were divided into two groups: those with infantile esotropia only (group A) and those who developed IOOA or DVD (group B). Results: A total of 122 patients were enrolled and mean follow-up period was 16.0 years (range: 10-32 years). The mean number of surgeries was 1.7 (range: 1-5), and 64 (52.5%) patients achieved optimal horizontal alignment (esotropia <10 prism diopters [PD] and orthotropia). Fifty (41.0%) patients developed IOOA at a median age of 3 years (range: 1-21 years); 54 (44.3%) developed DVD at a median age of 5 years (range: 1-25 years). Patients in group B underwent more horizontal surgeries than those in group A (P = 0.028), and favorable surgical outcomes between the two groups were not different at final visit. There were no other significant differences in clinical factors between the two groups. Conclusion: Approximately, 52.5% of patients achieved favorable surgical outcomes through 1.7 surgeries during the 10-year follow-up period. DVD tended to develop at a later age than IOOA, and in some cases, up to 20 years after diagnosis of infantile esotropia. To achieve favorable horizontal alignment at final visit, patients with associated vertical strabismus underwent more horizontal muscle surgeries than patients with infantile esotropia only. The presence of IOOA/DVD may affect horizontal alignment outcomes.
The Role of Vertical Displacement of the Horizontal Rectus Muscles in the Treatment of Strabismns

Abstract:Obiective To study the role of vertical displacement and transposition of horizontal rectus muscles in the surgical treatment ofstrabismus.Metheds We examinated Sixty-two patients of the deviation in all diagnostic positions by prism and cover test,Binocular motility in preoperation and postoperation.the subjective cyclodeviations were tested by synoptophore and the objective cyclodeviations were showed by fundus photographs.We followed up in a mean period of 6 months (range 1 to 34 months).Results (1)In 26 cases of V-pattern deviation that was not associated with oblique muscle dysfunction, vertical displacement(in the same direction)of bilateral horizontal rectus muscles and vertical displacement (in opposite directions)of monocular horizontal rectus muscles were performed and corrected up 10-30△ and 8-25△ of vertical incomitant deviations respectively.No postoperative cyclodeviation was noted. (2)In 25 cases of A-pattern deviation associated with oblique muscle dysfunction,vertical displacement of the horizontal rectus muscles alone corrected up 8-30△ of vertical incomitant deviations and had residual deviations of 5-25△ between primary position and downgaze position.Combining bilateral weakening of the superior oblique muscles reduced the residual deviations to 2△-8△. (3)In 8 cases of horizontal comitant deviation combined with small angle of vertical deviation, downward transposition or upward transposition of the bilateral horizontal rectus muscles corrected up 2-8△ of hyperdeviation and the residual hyperdeviation was 2-5△. (4)In 3 cases of congenital monocular double elevator palsy,the horizontal rectus muscles were transposed to the two sides of superior rectus tendon to correct up 25-30△ of vertical deviation and the residual vertical deviation was 5△.All of the patients had significant improvement of binocular elevation movements.Comclusions vertical displacement of horizontal rectus muscles can efficiently correct the vertical incomitant deviations of V-pattern strabismus without oblique muscle dysfunction.Bilateral weakening of the superior oblique muscles, along with the vertical displacement of the horizontal rectus muscles, can produce considerable elimination of A-pattern deviations in downgaze;however,this procedure has certain limitations in treating horizontal comitant deviation.Vertical displacement and transposition of horizontal rectus muscles is an effective surgical approach for patients with congenital monocular double elevator palsy.
Evaluation and Management of Divergence Insufficiency-Type Esotropia

Abstract:Divergence insufficiency-type ET is a common cause of distance diplopia in elderly adults. A recent prospective multicenter data collection study has provided additional guidance on management. Either base-out prism glasses or strabismus surgery were found to have high success rates, based on patient report of diplopia, and health-related quality-of-life domain scores. It was concluded that either prism or surgery were reasonable initial treatment strategies. Although allocation bias precluded formal comparison of prism versus surgery (evident in baseline differences between groups), there were hints that surgery may yield superior outcomes. For surgery, the most common approach was bilateral medial rectus recession, which was highly successful when assessed 10 weeks and 12 months postoperatively. It was unclear whether adjustable sutures were helpful, but in the vast majority of adjustable cases, additional recession at the time of adjustment was performed, suggesting that larger than standard surgical doses are needed.

Transposition procedures in Duane retraction syndrome

Abstract:Duane retraction syndrome, or Duane syndrome (DS), is one of several congenital cranial dysinnervation disorders. Patients present with limited horizontal eye movement(s) and globe retraction with eyelid fissure narrowing on attempted adduction due to co-contraction of the lateral and medial rectus muscles in one or both eyes. Various surgical approaches have been proposed to improve binocular alignment, reduce head turn, and minimize undesirable up- or downshoots in DS. Transposition procedures are one such approach, and a number of techniques have been described. These may involve one or both vertical rectus muscles and may or may not include full or partial disinsertion of the rectus muscle(s) from the insertion. Options involving both vertical rectus muscles include full vertical rectus transposition (VRT), partial VRT, rectus muscle union, and other modifications to be discussed. Options involving one vertical rectus muscle include superior rectus transposition (SRT) and inferior rectus transposition (IRT). The effectiveness of any transposition procedure may be enhanced with augmentation (posterior fixation) sutures, resection of the transposed muscle(s), and/or simultaneous weakening of the ipsilateral medial rectus muscle. This review discusses the indications, strengths, weaknesses, and other considerations of these approaches within the context of DS. Since the majority of DS cases are unilateral and most have the esotropic form, this will be the main focus of the review, although other forms will also be discussed.
Some clinical characteristics of V-pattern exotropia and surgical outcome after bilateral recession of the inferior oblique muscle: a retrospective study of 22 consecutive patients and a comparison with V-pattern esotropia

Abstract:Background and purpose: A V-pattern exotropia (XT) with bilateral overaction of the inferior obliques (IO) and/or underaction of the superior obliques (SO) is a common finding. The clinical characteristics of this condition in a large series are not available in the scientific literature. Also, data is lacking about the surgical outcome of graded bilateral IO recession. Lastly, a comparison of the pre- and postoperative findings of a V-pattern exotropia population with those of a V-pattern esotropia population is also lacking in the literature. Subjects and methods: Twenty-two consecutive patients without complicating factors were fully evaluated and submitted to bilateral graded recessions of the IO; 19 were also operated for a horizontal binocular alignment imbalance. Results: Preoperative findings: The distribution of V patterns showed 77.2% in the range of 15 to 25 prism diopters (PD) and 22.8% in the range 26 to 35 PD. An overaction of the IO was present bilaterally in 86.4% of the patients and unilaterally in 4.5%. Bilateral underaction of the SO was observed in 4.5% and unilateral in 13.6%. Bilateral overaction of the SO was seen in 18.2% and unilateral in 31.8%. Elevation in adduction was observed bilaterally in 13.6% and unilaterally in 31.8%. A vertical deviation was seen in 50.0% and a marked tendency of it to vanish or diminish in the up- and down positions, as well as at near, was observed. After surgery, 72.7% had less than 15 PD of V pattern or less than 10 PD of A pattern; 36.4% had the V pattern fully corrected. Surgery eliminated a vertical imbalance in 72.7% of the patients, reduced in 9.1%, left unchanged in 9.1% and increased in 9.1% Of the 11 patients devoid of vertical deviation, 72.7% remained so and in 27.3% a vertical deviation was created. Binocularity: There was an improvement of the fusional status with surgery, (ascertained with the Worth Four Dot Test and the major amblyoscope measurement). Conclusion: All cases were in the range of 15-35 PD preop'. Overaction of the IO was a prominent finding. Underaction of the SO was less prevalent as compared with a V-ET population. Overaction of the SO was observed bilaterally in 18.2% and unilaterally in 31.8%, a finding never detected in a V-ET population. A vertical imbalance was observed in one-half of the cases. A good outcome (collapse of the V pattern) was obtained with bilateral graded recession of the IO, This procedure is relatively safe concerning the vertical alignment.
Distance Esotropia. Differential Diagnosis and Methods of Management

Abstract:Primary divergence insufficiency is a generally benign condition affecting the elderly. Many patients have spontaneous resolution of the double vision within few months, so it is advisable to postpone strabismus surgery and follow the patient. It is also important to perform a differential diagnosis in order to rule out other causes of distance esotropia. For this purpose, a thorough history and careful ophthalmologic/strabologic examination should be performed. Clinical neurological evaluation is an effective tool to distinguish between patients with primary and secondary forms of divergence insufficiency. When making the diagnosis, special attention should be paid to the presence of nystagmus and the role of neuroimaging studies.

Treatment Complications in Children With Lyme Meningitis

Abstract:Background: The rate and type of treatment complications in children treated for Lyme meningitis have not been described. Methods: We performed a retrospective cohort study of children with Lyme meningitis who presented to 1 of 3 emergency departments located in Lyme disease endemic areas between 1997 and 2010. We defined a case of Lyme meningitis as a child with cerebrospinal fluid pleocytosis and either positive Lyme serology or an erythema migrans rash. We identified prescribed treatment and reasons for all return visits. Our primary outcome was the presence of any treatment complication within 30 days of diagnosis. Results: We identified 157 patients with Lyme meningitis with a median age of 10 years (interquartile range: 7–13 years). Of the 149 children with Lyme meningitis and available follow-up records, 39 (26%) had 1 or more complications, and 21 (14%) required a change in prescribed antibiotic therapy. The median time for developing the first complication was 11 days (interquartile range: 9–14 days). Ten percent of the patients had an adverse drug reaction. Of the 144 children who had a peripherally inserted central catheter placed, 25 (17%) had at least 1 peripherally inserted central catheter-associated complication: 14 (10%) had a mechanical problem, 11 (8%) had an infectious complication and 1 (1%) had a venous thromboembolism. Conclusions: As current Lyme meningitis treatment regimens have substantial associated morbidity, future research should investigate the efficacy of alternate regimens.

Methods: Retrospective analysis of 66 patients with divergence excess IXT who underwent either BLR or RR from January 2013 to December 2020 was conducted. Data on demographics, pre- and postoperative deviations, fusion, stereopsis, control, and accommodative convergence/accommodation ratio were collected. Success was defined as esodeviation ≤5 PD (prism diopter) to exodeviation ≤10 PD with a follow-up time of at least 8.0 ± 2.0 weeks. Results: BLR (42 cases) and RR (24 cases) groups had the same success rate (83.3%, P = 0.688) and similar reduced postoperative deviations both in distance and at near (P > 0.05). Near-distance disparity decreased significantly in both groups (P = 0.000) with no intergroup difference (P = 0.193). Conclusion: BLR and RR were equally effective for divergence excess IXT with comparable outcomes of both distance and near deviations.
Surgical management of Helveston syndrome (Triad exotropia)

Abstract:Purpose: To evaluate and compare different surgical approaches for the treatment of Helveston syndrome and provide further information for preoperative planning. Methods: From February 2008 to December 2018, data of 52 patients with Helveston syndrome were retrospectively reviewed. Different surgical approaches were selected based on the extent of A-pattern exotropia, dissociated vertical deviation (DVD), and both superior oblique muscle overaction (SOOA) with fundus photograph intorsion. Eye position, A-pattern, DVD, superior oblique muscle function, and binocular vision function were evaluated pre- and postoperatively. The average follow-up duration was 20.5 months. Results: Nine cases underwent simultaneous horizontal deviation correction with bilateral superior rectus recession, 24 underwent simultaneous horizontal deviation correction with bilateral superior oblique muscle lengthening, and 19 underwent two stages of horizontal deviation correction with superior oblique muscle lengthening, and later bilateral superior rectus recession. A-pattern, DVD, SOOA, and fundus intorsion were all collapsed in all patients postoperatively. Forty-five patients had an orthophoric eye position with considerably aligned ocular movements postoperatively. The total success rate was 86.5%. Postoperatively, eight of the 10 patients with diplopia experienced a recovery of binocular single vision and three had a recovery of rudimentary stereopsis (Titmus 3000-400 s of arc). The compensatory head posture of patients improved significantly postoperatively. Conclusions: The surgical planning of Helveston syndrome should be designed based on the degree of the A-pattern, SOOA, DVD, and the intorsion in fundus photographs, and the appropriate approach should be selected to improve patient satisfaction.
